Recent years have brought increasing interest in the use of the electrocardiogram as a means of understanding the damage to the heart muscle wrought by disease. Since Einthoven's discovery of the string galvanometer, this instrument has been used in the study of the direction, amplitude and time relations of the electrical currents produced by the contraction of the heart muscle. For over sixty years this was done only as a scientific procedure, with little clinical application. In recent years, however, more and more interest has been focused on the clinical value of such a study, until its merit has become definitely established. Three standard leads have been used, and these have yielded a great deal of information. However, it was thought that still more information could be gained if the technic was modified. Wolferth and Wood1 in 1932 reintroduced and made popular a fourth lead, or so-called chest lead.

Early attempts by us to use sulfanilamide as a prophylactic agent in rheumatic fever were discontinued because of the fear of provoking toxic manifestations of this then new drug. The encouraging results obtained by Coburn and Moore1 and Thomas and France,2 however, prompted us to resume this study. Our special purpose was to follow the course of the disease in a group of patients made up entirely of children wherein the control and the sulfanilamide-treated subjects were as nearly as possible comparable and yet under more or less uncontrolled conditions which closely approximated those encountered in the private practice of medicine. Although a relatively large number of patients were observed by Coburn and Moore,3 comparatively few children were in the control series; Thomas, France and Reichsman4 used older subjects almost entirely. Of the various streptococcus antiserums that have been employed to date, scarlet fever antitoxin and pooled convalescent scarlet fever serum have found the greatest favor clinically. Since the rise in popularity of these serums, precise classification of the streptococci has been achieved by serologic methods. Hemolytic streptococci of human origin have been classified by Griffith into thirty known types, of which twenty-six are included in Lancefield's group A while four fall into groups C and G. It has been shown by Green,1 Bailey,2 White,3 Keogh,4 Allison5 and de Waal6 that certain serologic types usually predominate in an epidemic of scarlet fever. Many of the types associated with clinical scarlet fever are proficient in producing erythrogenic toxin and as a result commercial scarlatina antitoxin, which has a high neutralizing titer, has become more and more popular in the treatment of this disease.
