11. Results of Abdominal Sacrocolpopexy and Le Fort Colpocleisis: Our Experiences at Urology Clinic.

Introduction: Pelvic organ prolapse (POP) can be the source of discomfort for women, leading to issues such as urinary incontinence, discomfort from herniated organs, and sexual dysfunction. In this study, our aim is to sh are our experiences with abdominal sacrocolpopexy and colpocleisis. Materials and Methods: In the study, 28 patients who underwent POP surgery between January 1, 2011 and November 1, 2022 were included. All patients had POP-Q stage 3 and above. Age, comorbidities, previous gynecological operations, history of prolapse and urinary incontinence surgery, and quality of life were recorded. In the postoperative period, the patients were followed up i n terms of complications, incontinence and recurrence. Results: A total of 28 patients underwent surgical treatment for POP, with 22 undergoing abdominal sacrocolpopexy and 6 undergoing Le Fort colpocleisis. Since 17 of the patients undergoing sacrocolpopexy had stress urinary incontinence, additional Burch colpo suspension was applied. Two patients who underwent sacrocolpopexy developed de novo urge incontinence. These patients benefited from med ical treatment. No cases of incontinence developed in the colpocleisis group. There were no recurrences observed in any of th e cases. Conclusion: Abdominal sacrocolpopexy and colpocleisis are methods with high success and satisfaction rates. Colpocleisis surgery stands out with shorter hospital stay. These patients should be well informed in terms of sexual life, and if there is concomitant stress urinary incontinence, it is recommended to perform the necessary surgery before colpocleisis. For sacrocolpopexy, the patient should be well informed about mesh and related complications beforehand. [ABSTRACT FROM AUTHOR]

Purpose: Varicocele is a common problem among infertile men. Varicocele repair (VR) is frequently performed to improve semen parameters and the chances of pregnancy. However, there is a lack of consensus about the diagnosis, indications for VR and its outcomes. The aim of this study was to explore global practice patterns on the management of varicocele in the context of male infertility. Materials and Methods: Sixty practicing urologists/andrologists from 23 countries contributed 382 multiple-choice-questions pertaining to varicocele management. These were condensed into an online questionnaire that was forwarded to clinicians involved in male infertility management through direct invitation. The results were analyzed for disagreement and agreement in practice patterns and, compared with the latest guidelines of international professional societies (American Urological As- sociation [AUA], American Society for Reproductive Medicine [ASRM], and European Association of Urology [EAU]), and with evidence emerging from recent systematic reviews and meta-analyses. Additionally, an expert opinion on each topic was provided based on the consensus of 16 experts in the field. Results: The questionnaire was answered by 574 clinicians from 59 countries. The majority of respondents were urologists/ uro-andrologists. A wide diversity of opinion was seen in every aspect of varicocele diagnosis, indications for repair, choice of technique, management of sub-clinical varicocele and the role of VR in azoospermia. A significant proportion of the re- sponses were at odds with the recommendations of AUA, ASRM, and EAU. A large number of clinical situations were identi- fied where no guidelines are available. Conclusions: This study is the largest global survey performed to date on the clinical management of varicocele for male in- fertility. It demonstrates: 1) a wide disagreement in the approach to varicocele management, 2) large gaps in the clinical prac- tice guidelines from professional societies, and 3) the need for further studies on several aspects of varicocele management in infertile men.

Amaç: Böbrek taşı varlığı ile COVID-19 hastalığı arasındaki ilişkinin değerlendirilmesiGereç ve yöntem: COVID-19 tanısı ile 15 Kasım-31 Aralık 2020 tarihleri arasında 2. ve 3. basamak 2 farklı merkezde ayaktan, servis ve/veya yoğun bakımda tedavi uygulanan hastalar retrospektif olarak tarandı. Öncelikle hastalar böbrek taşı varlığına göre alt gruplara ayrıldı ve daha sonra böbrek taşı olan olgular, ayaktan tedavi gören hastalar Grup 1, serviste yatarak tedavi gören hastalar Grup 2 ve yoğun bakımda tedavi ihtiyacı doğan olgular ise Grup 3 olarak kategorize edildi.Bulgular: Çalışmaya toplam 1335 COVID-19 hastası dahil edildi. Ayaktan tedavi edilen 450 hastanın 31'inde (%6,9), serviste yatan 460 hastanın 41'inde (%8,9) ve yoğun bakımda yatan 425 hastanın 60'ında (%14,1) böbrek taşı mevcuttu. Grup 1'de, böbrek taşı olan hastalarda COVID-19 tedavi süresi böbrek taşı olmayan hastalara göre anlamlı olarak daha uzundu (8,1±1,7 ve 6,8±2,2 gün, p=0,01). Grup 2 ve Grup 3'te ortalama hastanede kalış süresi böbrek taşı olan hastalarda böbrek taşı olmayanlara göre anlamlı olarak daha uzundu (sırasıyla 9,1±3,7 ve 6,2±2,1 gün, p=0,007; 19,1±8,1 ve 11,3±6,2 gün, p=0,001).Sonuç: Böbrek taşı olanlarda COVID-19 tedavi süresinin daha fazla ve COVID-19 enfeksiyonunun daha şiddetli olduğu saptandı., Purpose: To evaluate the relationship between the presence of kidney stones and COVID-19.Materials and methods: Patients, who were treated for COVID-19 as outpatients as well as inpatients in the ward and/or ICU of two different secondary and tertiary care centers between July 15, 2020, and December 31, 2020, and aged ≥18 years were retrospectively evaluated. The patients were divided into two subgroups based on the presence of kidney stones, and then the patients with kidney stone were categorized into three groups: those who were treated in an outpatient setting (Group 1), those who were treated in the ward (Group 2), and those who were treated in the intensive care unit (Group 3).Results: The total of 1,335 COVID-19 patients included in the study. Kidney stone was present in 31 (6.9%) of 450 outpatients, 41 (8.9%) of 460 inpatients treated in the ward, and 60 (14.1%) of 425 inpatients treated in the intensive care unit. In Group 1, the duration of COVID-19 treatment was significantly longer in patients with kidney stone than patients without kidney stone (8.1±1.7 vs. 6.8±2.2 days, p=0.01). In Group 2 and in Group 3, the mean hospitalization duration was significantly longer in patients with kidney stone than in those without kidney stone (9.1±3.7 vs. 6.2±2.1 days, p=0.007; 19.1±8.1 vs. 11.3±6.2 days, p=0.001, respectively). Conclusion: The duration of COVID-19 treatment was longer and the COVID-19 infection was more severe in those with kidney stones.

Sperm vitality testing is a basic semen examination that has been described in the World Health Organization (WHO) Laboratory Manual for the Examination and Processing of Human Semen from its primary edition, 40 years ago. Several methods can be used to test sperm vitality, such as the eosin-nigrosin (E-N) stain or the hypoosmotic swelling (HOS) test. In the 6th (2021) edition of the WHO Laboratory Manual, sperm vitality assessment is mainly recommended if the total motility is less than 40%. Hence, a motile spermatozoon is considered alive, however, in certain conditions an immotile spermatozoon can also be alive. Therefore, the differentiation between asthenozoospermia (pathological decrease in sperm motility) and necrozoospermia (pathological decrease in sperm vitality) is important in directing further investigation and management of infertile patients. The causes leading to necrozoospermia are diverse and can either be local or general, testicular or extra -testicular. The andrological management of necrozoospermia depends on its etiology. However, there is no standardized treatment available presently and practice varies among clinicians. In this study, we report the results of a global survey to understand current practices regarding the physician order of sperm vitality tests as well as the management practices for necrozoospermia. Laboratory and clinical scenarios are presented to guide the reader in the management of necrozoospermia with the overall objective of establishing a benchmark ranging from the diagnosis of necrozoospermia by sperm vitality testing to its clinical management. We aimed to investigate the relationship between premature ejaculation and the age when men had been circumcised before adulthood. A total of 2,768 sexually active male patients aged between 18 and 65 years were included in this study. A multicentre study was conducted prospectively with the participation of 20 centres. A survey consisting of 12 questions prepared by the researchers, as well as the validated Turkish versions of the five‐item Premature Ejaculation Diagnostic Tool, was administered to all participants. The study included 1,603 participants who met the inclusion criteria. There was no significant difference in the Premature Ejaculation Diagnostic Tool and self‐reported ejaculation time between the participants who had been circumcised at different ages during childhood. Remembering circumcision experience with fear or anxiety did not increase the risk of sexual dysfunction compared to the participants who described their experience with happiness or with no particular emotion. There was no significant difference in Premature Ejaculation Diagnostic Tool scores or the self‐reported ejaculation time of the participants circumcised at different ages. The age of childhood circumcision, having a fearful or anxious circumcision experience, does not affect the risk of premature ejaculation in adult life. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: To present a nation‐wide analysis of the workload of urology departments in Turkey week‐by‐week during Covid‐19 pandemic. Methodology: The centres participating in the study were divided into three groups as tertiary referral centres, state hospitals and private practice hospitals. The number of outpatients, inpatients, daily interventions and urological surgeries were recorded prospectively between 9‐March‐2020 and 31‐May‐2020. All these variables were recorded for the same time interval of 2019 as well. The weekly change of the workload of urology during pandemic period was evaluated, also the workload of urology and the distributions of certain urological surgeries were compared between the pandemic period and the same time interval of the year 2019. Results: A total of 51 centres participated in the study. The number of outpatients, inpatients, urological surgeries and daily interventions were found to be dramatically decreased by the 3rd week of pandemics in state hospitals and tertiary referral centres; however, the daily urological practice were similar in private practice hospitals throughout the pandemic period. When the workload of urology in pandemic period and the same time interval of the year 2019 were compared, a huge decrease was observed in all variables during pandemic period. However, temporary measures like ureteral stenting, nephrostomy placement and percutaneous cystostomy have been found to increase during Covid‐19 pandemic compared with normal life. Conclusions: Covid‐19 pandemic significantly affected the routine daily urological practice likewise other subspecialties and priority was given to emergent and non‐deferrable surgeries by urologists in concordance with published clinical guidelines. Amaç: Antibiyotik kullanılıp kullanılmamasının total prostat spesifik antijen (PSA) düzeyi ve serbest/total (s/t) PSA oranına etkisi ve bu değişimlerin prostat biyopsi sonuçlarına olan yansımalarının değerlendirilmesi.Materyal ve Metod: Kliniğimizde 2004 ile 2016 yılları arasında yaşa göre PSA değeri normalden yüksek olan ve prostat biyopsisi yapılan 1,062 hasta retrospektif olarak değerlendirildi. Rektal muayenede bulgusu olan veya 5-alfa redüktaz inhibitörü kullanan hastalar çalışma dışında tutuldu. Bu olgulardan, kontrol PSA değeri olan veya biyopsi öncesinde ve sonrasında PSA 4-10 ng/mL arasında iken s/t PSA oranı elde edilmiş toplam 303 hasta çalışmaya dahil edildi. Toplam 214 hastaya ortanca 4 hafta (2-8 hafta) süren antibiyotik tedavisi sonrasında, kalan 89 olguya antibiyotik verilmeksizin ortalama 1 ay sonraki kontrol PSA düzeylerindeki yükseklik nedeniyle biyopsi yapılmıştı. Her iki grupta da PSA ve s/tPSA oran değişimleri için %5 ve %10 sınır (cut off) değerler alınarak karşılaştırmalar yapıldı.Bulgular: Hastaların ortanca yaşı 63 yıl (45-86 yıl) idi. Antibiyotik verilen grupta (n= 214) her iki sınır değer için (%5 ve %10) anlamlı PSA düşüşü izlenmedi. Antibiyotik verilmeyen hastalarda (n= 89) ise her iki sınır değeri için kontrol PSA değerlerinde anlamlı artış saptandı (%5 sınır değer için p< 0.001, %10 sınır değer için p< 0.001). Antibiyotik alan ve almayan olgular birlikte değerlendirildiğinde (n= 303) PSA'daki azalma ve artışlar antibiyotik kullanılan grupta her iki sınır değer için de anlamlı olarak yüksek bulundu (%5 sınır değer için p< 0.001 ve p< 0.001; %10 sınır değer için p= 0.001 ve p= 0.001). Serbest/total PSA oran değişimleri açısından her iki sınır değer için antibiyotik verilip verilmemesinin anlamlı bir fark oluşturmadığı izlendi. Antibiyotik verilsin veya verilmesin total PSA değişimleri ile patoloji sonuçları arasında arasında anlamlı ilişki saptanmadı. Ancak antibiyotik verilen ve s/t PSA oranında değişim izlenen olgularda her iki sınır değer için patoloji sonuçları ile anlamlı ilişki gözlendi. Bu oranın azaldığı olgularda prostat kanseri, arttığı olgularda ise benign patolojiler daha yüksek oranda saptandı (%5 sınır değer için p değerleri sırasıyla p= 0.004 ve p= 0.014; %10 sınır değer için p değerleri sırasıyla p= 0.014 ve p= 0.026). Yine antibiyotik alan ve almayan olgular birlikte değerlendirildiğinde de oran değişimleri ile patoloji sonuçları arasında anlamlı farklılık saptanmadı.Sonuç: Rektal muayenesi normal, yaşa göre yüksek PSA değeri olan hastalarda önce antibiyotik verilerek ya da herhangi bir ilaç verilmeksizin makul bir süre sonra kontrol PSA değerine bakılarak prostat biyopsisine karar vermek sıklıkla başvurulan durumlardır. Biyopsi kararı verilmesinde s/t PSA oran değişimlerinin, özellikle antibiyotik kullanan olgularda total PSA'dan daha fazla yol gösterici olabileceği anlaşılmaktadır. Objectives: To evaluate the impact of antibiotic treatment to total prostate specific antigen (PSA) levels and free/total (f/t) PSA ratio and response of these changes to prostate biopsy results.Materials and Methods: We retrospectively evaluated 1,062 prostate biopsy performed patients with elevated age-adjusted serum PSA levels from 2004 to 2016. Patients treated with 5-alpha-reductase inhibitors and who has abnormalities on digital rectal examination were excluded. A total of 303 patients who met these criteria and had control levels of PSA or before and after biopsy f/t PSA ratio determinations because of PSA levels between 4 and 10 ng/mL were included into this study. There were 214 patients with persistent elevated serum PSA levels after a mean duration of 4 weeks (range; 2-8) of antibiotic treatment followed by prostate biopsy (treatment group) and 89 patients who had prostate biopsy after a mean followup of 1 month without antibiotherapy (control group). The groups were compared with regard to changes in serum PSA levels and f/t PSA ratios with a 5% and 10% cut off values.Results: The mean age of patients were 63 years (range; 45–86). In treatment group (n= 214), no significant difference was detected in serum PSA levels at both cut off values. In control group (n= 89), there was a significant rise in subsequent PSA levels at both cut off values (for 5% cut off value p< 0.001; for 10% cut off value p< 0.001). If we evelaute treatment and control group together (n= 303) for the change of PSA levels, raise and decrease in PSA values were significantly high in treatment group for both cut off values (for %5 cut off value p< 0.001, p< 0.001; for %10 cut off value p= 0.001, p= 0.001). There was no difference between f/t PSA ratio changes and antibiotic treatment or observation for both cut off values. For the treatment and control group particularly and combined together, there was no difference between serum PSA levels and prostate biopsy results in both cut off values. There was a statisctically significant relationship between patients with changed f/t PSA ratio in treatment group and pathology results for both cut off values. According to that, patients with increased f/t PSA ratio had more likely BPH in biopsy results and patients with decreased ratio were cancer as well (for %5 cut off value p= 0.004, p= 0.014; for %10 cut off value p= 0.014, p= 0.026). If we evaluate treatment and control group together, there was no significant difference between f/t PSA ratio changes and biopsy results.Conclusion: We decide to perform prostate biopsy with subsequent PSA levels with or without treatment to the patients with normal digital rectal examinatiorn and elevated age-adjusted serum PSA levels. To decide for prostate biopsy, it is suggested that f/t PSA ratio is more useful than PSA levels in serum especially for the patients treated with antibiotics. 93

Penile abscess is a rare clinical entity. We present the case of a 45-year-old paraplegic male who developed penile abscess due to self-administration of intracavernous papaverine injections for erectile dysfunction treatment. Penile magnetic resonance imaging revealed an abscess of 5x4.5 cm in size, located at the right side of the penis and the penile corpus deviated to the left side. Penile exploration, incision and drainage were performed and the patient was treated successfully with intravenous antibiotics. A penile prosthesis was implanted 6 months after the initial surgery. Ongün, Şakir (Balikesir Author), Objective To present a nation-wide analysis of the workload of urology departments in Turkey week-by-week during Covid-19 pandemic. Methodology The centres participating in the study were divided into three groups as tertiary referral centres, state hospitals and private practice hospitals. The number of outpatients, inpatients, daily interventions and urological surgeries were recorded prospectively between 9-March-2020 and 31-May-2020. All these variables were recorded for the same time interval of 2019 as well. The weekly change of the workload of urology during pandemic period was evaluated, also the workload of urology and the distributions of certain urological surgeries were compared between the pandemic period and the same time interval of the year 2019. Results A total of 51 centres participated in the study. The number of outpatients, inpatients, urological surgeries and daily interventions were found to be dramatically decreased by the 3rd week of pandemics in state hospitals and tertiary referral centres; however, the daily urological practice were similar in private practice hospitals throughout the pandemic period. When the workload of urology in pandemic period and the same time interval of the year 2019 were compared, a huge decrease was observed in all variables during pandemic period. However, temporary measures like ureteral stenting, nephrostomy placement and percutaneous cystostomy have been found to increase during Covid-19 pandemic compared with normal life. Conclusions Covid-19 pandemic significantly affected the routine daily urological practice likewise other subspecialties and priority was given to emergent and non-deferrable surgeries by urologists in concordance with published clinical guidelines.

- Abstract
Purpose: Non-obstructive azoospermia (NOA) is a common, but complex problem, with multiple therapeutic options and a lack of clear guidelines. Hence, there is considerable controversy and marked variation in the management of NOA. This survey evaluates contemporary global practices related to medical and surgical management for patients with NOA., Materials and Methods: A 56-question online survey covering various aspects of the evaluation and management of NOA was sent to specialists around the globe. This paper analyzes the results of the second half of the survey dealing with the management of NOA. Results have been compared to current guidelines, and expert recommendations have been provided using a Delphi process., Results: Participants from 49 countries submitted 336 valid responses. Hormonal therapy for 3 to 6 months was suggested before surgical sperm retrieval (SSR) by 29.6% and 23.6% of participants for normogonadotropic hypogonadism and hypergonadotropic hypogonadism respectively. The SSR rate was reported as 50.0% by 26.0% to 50.0% of participants. Interestingly, 46.0% reported successful SSR in <10% of men with Klinefelter syndrome and 41.3% routinely recommended preimplantation genetic testing. Varicocele repair prior to SSR is recommended by 57.7%. Half of the respondents (57.4%) reported using ultrasound to identify the most vascularized areas in the testis for SSR. One-third proceed directly to microdissection testicular sperm extraction (mTESE) in every case of NOA while others use a staged approach. After a failed conventional TESE, 23.8% wait for 3 months, while 33.1% wait for 6 months before proceeding to mTESE. The cut-off of follicle-stimulating hormone for positive SSR was reported to be 12-19 IU/mL by 22.5% of participants and 20-40 IU/mL by 27.8%, while 31.8% reported no upper limit., Conclusions: This is the largest survey to date on the real-world medical and surgical management of NOA by reproductive experts. 47. Changes in Testosterone Levels Following Surgical Sperm Retrieval in Men with Non-Obstructive Azoospermia: Systematic Review and Meta-Analysis.

Purpose: Surgical sperm retrieval (SSR) is used to extract spermatozoa for use with intracytoplasmic sperm injection in men with obstructive and non-obstructive azoospermia (NOA). The procedure may lead to segmental devascularization, postoperative fibrosis, and atrophy with a subsequent decrease in testosterone. The aim of the study is to investigate the impact of SSR on serum levels of total testosterone (TT), follicle-stimulating hormone (FSH), luteinizing hormone (LH) testicular volume, and sexual function in infertile azoospermic men., Materials and Methods: In this systematic review and meta-analysis (SRMA), we searched articles in "PubMed" and "Scopus" exploring the impact of SSR on TT, FSH, LH, and testicular volume. The full-text articles were screened to assess eligibility before data extraction, quality assessment, and meta-analysis., Results: Seventeen studies meeting the inclusion criteria were finally analyzed and included 1,685 infertile, azoospermic men. Patients underwent SSR and were followed in the postoperative period (one week to 32 months). The analysis showed a significant reduction in TT (mean difference [MD] 3.81 nmol/L, 95% confidence interval [CI] 0.55:7.06; p=0.02) compared to pre-SSR values. We also observed insignificant differences in serum FSH (MD 5.08 IU/L, 95% CI -5.6:15.8; p=0.35), LH (MD -2.96 IU/L, 95% CI -6.31:0.39; p=0.08), and no change in testicular volume (MD 0.07 mL, 95% CI -1.92:2.07; p=0.94) after SSR. Sexual dysfunction was associated with hypogonadism, depression, and anxiety, especially in men with unsuccessful SSR and Klinefelter syndrome., Conclusions: The results of this SRMA indicate a significant reduction in TT after SSR. - Abstract
The purpose of this study was to determine the average penis length and girth among the Turkish population, and to analyse the perspectives of couples. A total of 1703 males and 139 female partners participated in this study. All the participants completed a questionnaire and the flaccid and stretched penis length and girth were recorded. The results showed that the average flaccid penis length was 8.44 ± 2.28 cm and 12.27 ± 2.91 cm stretched, with girth of 8.23 ± 2.07 cm. Regarding the importance of penis size for a satisfying sexual experience, 18.5% of men thought penis length mattered, 9.9% considered thickness important, and 71.6% thought both were significant. A majority of participants, 67.8% of males and 76% of females, expressed that erect penis length was crucial for sexual pleasure. For a healthy sexual intercourse, 19.4% of the female partners emphasized the significance of penis length, 23.7% highlighted thickness, and 56.8% emphasized both factors for a fulfilling sexual relationship. A comparison between men's stretched penis length and the ideal length for partner satisfaction revealed no statistically significant difference (13 (4.8-21) cm vs. 12 (8-20) cm, p = 0.078). The average penis length and girth of Turkish males were found to be similar to the data in studies of this subject in literature. Both the males and their partners thought that erect penis length and girth were important for sexual intercourse.
