A patient with oat cell carcinoma of the lung which was responsive to radiotherapy developed jaundice. Endoscopic retrograde pancreatocholangiography (ERCP) revealed compression of the distal common bile duct. Radiotherapy to this area produced prompt and prolonged relief of the obstructive jaundice. The use of ERCP which led to significant palliation in a patient with known inoperable cancer makes this case worthy of note. [ABSTRACT FROM AUTHOR]

Eight children (aged 8 mo to 13 yr) with cystic fibrosis (CF) and growth failure were given home nocturnal nasogastric feeding of an elemental diet for 3 mo and re-evaluated 3 mo after cessation of tube feeding. An increase in energy intake (p <0.05) resulted in increased serum transferrin (p <0.005), retinol-binding protein (p <0.05), and clinical scores (p <0.05) during the study. Height and growth velocity increased by 60% (p <0.05), and weight growth velocity increased by 63%. Arm- muscle circumference, triceps skinfolds, serum albumin, vitamin A, vitamin E, zinc, and copper did not change significantly during tube feeding. Nocturnal feeding were safe and effective in promoting growth; however, most children with severe lung disease did not sustain the gains after cessation of tube feeding. Our findings indicate that continued nutritional support is needed to maintain the growth in malnourished children with severe lung disease. [ABSTRACT FROM AUTHOR]

A 40-year-old man with a history of insulin-dependent diabetes mellitus was admitted to the hospital because of jaundice and pruritis. During his evaluation the diagnosis of primary sclerosing cholangitis and "microscopic" ulcerative colitis were established. Massive intraabdominal lymphadenopathy was discovered on CT scan and histological examination eventually proved this to be follicular hyperplasia. The case herein reported documents the association of primary sclerosing cholangitis with diabetes mellitus and ulcerative colitis as well as reporting the occurrence of massive intraabdominal lymphadenopathy. Upper gastrointestinal polyps commonly occur in patients with familial polyposis coli; but the occurrence of gastric carcinoma is very rare in this disorder. A 66-year-old man with a past history of hyperparathyroidism was evaluated for chronic epigastric pain. An upper gastrointestinal x-ray examination revealed the presence of polypoid filling defects and endoscopy-polypectomy eventually proved these to be gastric carcinoids. [ABSTRACT FROM AUTHOR]

This report demonstrates that hepatic metastases can present with fulminant hepatic failure and that liver enzyme abnormalities may not become prominent until there has been massive replacement of the liver. The CT scan of liver may not demonstrate diffuse liver metastases as seen in this patient. An elderly woman with a history of cholelithiasis was seen for right upper quadrant pain and jaundice. During her evaluation, she was found to have extensive choledocholithiasis and a cholecystoduodenal fistula. Successful management was accomplished with cholecystectomy, removal of the common bile duet stones, and by creating a choledochoduodenostomy where the fistula had been present. A 46-year-old woman was seen for evaluation of dyspepsia of 8-wk duration. X-rays of the upper gastrointestinal tract revealed an intraluminally projecting diverticulum of the duodenal bulb. The pleuropulmonary complications of pancreatitis are protean. The subject of pancreatic pleural effusions has received avid attention in the recent literature with the demonstration of pancreaticopleural fistulization by endoscopic retrograde cholangiopancreatography. Surgical intervention has been efficacious when nonoperative management has failed.
