It is well known that a large portion of the population elevates their intake of high energy-dense foods during times of stress; however, it is understudied whether stress affects the reinforcing value of a food reward. Further knowledge of this relationship may help us better understand the positive correlation between reinforcing value of food and obesity. Therefore, it was tested if an acute stressor would increase the reinforcing value of low or high energy-dense food. Participants (N = 70) were randomized to a stress or no-stress condition after which they were allowed to work to gain access to a food reward and reading time. To determine if high energy-dense food was specifically affected, half the participants from each stress manipulation were randomly assigned to work for either grapes or chocolate candies. Participants in the stress condition worked less for food access than those in the no-stress condition, for both low and high energy-dense foods, but stress did not affect the reinforcing value of reading time. These results indicate that, contrary to our hypothesis, in a sample of college students, an acute stressor decreased reinforcing value of food, with no difference between a low and high energy-dense food item., Competing Interests: Declarations of competing interest None., (Copyright © 2024. Disparities in access to safe public drinking water are increasingly being recognized as contributing to health disparities and environmental injustice for vulnerable communities in the United States. As the Co-Directors of the Apsaálooke Water and Wastewater Authority (AWWWA) for the Crow Tribe, with our academic partners, we present here the multiple and complex challenges we have addressed in improving and maintaining tribal water and wastewater infrastructure, including the identification of diverse funding sources for infrastructure construction, the need for many kinds of specialized expertise and long-term stability of project personnel, ratepayer difficulty in paying for services, an ongoing legacy of inadequate infrastructure planning, and lack of water quality research capacity. As a tribal entity, the AWWWA faces additional challenges, including the complex jurisdictional issues affecting all phases of our work, lack of authority to create water districts, and additional legal and regulatory gaps-especially with regards to environmental protection. Despite these obstacles, the AWWWA and Crow Tribe have successfully upgraded much of the local water and wastewater infrastructure. We find that ensuring safe public drinking water for tribal and other disadvantaged U.S. communities will require comprehensive, community-engaged approaches across a broad range of stakeholders to successfully address these complex legal, regulatory, policy, community capacity, and financial challenges., Competing Interests: The authors declare no conflict of interest. An estimated 11 million people in the US have home wells with unsafe levels of hazardous metals and nitrate. The national scope of the health risk from consuming this water has not been assessed as home wells are largely unregulated and data on well water treatment and consumption are lacking. Here, we assessed health risks from consumption of contaminated well water on the Crow Reservation by conducting a community-engaged, cumulative risk assessment. Well water testing, surveys and interviews were used to collect data on contaminant concentrations, water treatment methods, well water consumption, and well and septic system protection and maintenance practices. Additive Hazard Index calculations show that the water in more than 39% of wells is unsafe due to uranium, manganese, nitrate, zinc and/or arsenic. Most families' financial resources are limited, and 95% of participants do not employ water treatment technologies. Despite widespread high total dissolved solids, poor taste and odor, 80% of families consume their well water. Lack of environmental health literacy about well water safety, pre-existing health conditions and limited environmental enforcement also contribute to vulnerability. A landfill gas to energy (LFGTE) facility in Ste. Sophie, Quebec was instrumented with sensors which measure temperature, oxygen, moisture content, settlement, total earth pressure, electrical conductivity and mounding of leachate. These parameters were monitored during the operating phase of the landfill in order to better understand the biodegradation and waste stabilization processes occurring within a LFGTE facility. Conceptual and numerical models were created to describe the heat transfer processes which occur within five waste lifts placed over a two-year period. A finite element model was created to simulate the temperatures within the waste and estimate the heat budget over a four and a half year period. The calibrated model was able to simulate the temperatures measured to date within the instrumented waste profile at the site. The model was used to evaluate the overall heat budget for the waste profile. The model simulations and heat budget provide a better understanding of the heat transfer processes occurring within the landfill and the relative impact of the various heat source/sink and storage terms. Aerobic biodegradation appears to play an important role in the overall heat budget at this site generating 36% of the total heat generated within the waste profile during the waste placement stages of landfill operations., (Copyright © 2015 Elsevier Ltd. Racial and ethnic minority communities, including American Indian and Alaska Natives, have been disproportionately impacted by environmental pollution and contamination. This includes siting and location of point sources of pollution, legacies of contamination of drinking and recreational water, and mining, military and agricultural impacts. As a result, both quantity and quality of culturally important subsistence resources are diminished, contributing to poor nutrition and obesity, and overall reductions in quality of life and life expectancy. Climate change is adding to these impacts on Native American communities, variably causing drought, increased flooding and forced relocation affecting tribal water resources, traditional foods, forests and forest resources, and tribal health. This article will highlight several extramural research projects supported by the United States Environmental Protection Agency (USEPA) Science to Achieve Results (STAR) tribal environmental research grants as a mechanism to address the environmental health inequities and disparities faced by tribal communities. The tribal research portfolio has focused on addressing tribal environmental health risks through community based participatory research. Specifically, the STAR research program was developed under the premise that tribal populations may be at an increased risk for environmentally-induced diseases as a result of unique subsistence and traditional practices of the tribes and Alaska Native villages, community activities, occupations and customs, and/or environmental releases that significantly and disproportionately impact tribal lands. Through a series of case studies, this article will demonstrate how grantees-tribal community leaders and members and academic collaborators-have been addressing these complex environmental concerns by developing capacity, expertise and tools through community-engaged research.

American Indians have unique vulnerabilities to the impacts of climate change because of the links among ecosystems, cultural practices, and public health, but also as a result of limited resources available to address infrastructure needs. On the Crow Reservation in south-central Montana, a Northern Plains American Indian Reservation, there are community concerns about the consequences of climate change impacts for community health and local ecosystems. Observations made by Tribal Elders about decreasing annual snowfall and milder winter temperatures over the 20 th century initiated an investigation of local climate and hydrologic data by the Tribal College. The resulting analysis of meteorological data confirmed the decline in annual snowfall and an increase in frost free days. In addition, the data show a shift in precipitation from winter to early spring and a significant increase in days exceeding 90° F (32° C). Streamflow data show a long-term trend of declining discharge. Elders noted that the changes are affecting fish distribution within local streams and plant species which provide subsistence foods. Concerns about warmer summer temperatures also include heat exposure during outdoor ceremonies that involve days of fasting without food or water. Additional community concerns about the effects of climate change include increasing flood frequency and fire severity, as well as declining water quality. The authors call for local research to understand and document current effects and project future impacts as a basis for planning adaptive strategies.

Elderly patients with cardiovascular disease require slightly different management strategies compared with younger patients. Diagnosis is more difficult. Procedures, both diagnostic and therapeutic, are more dangerous. Despite higher risks, elderly patients have much to gain from aggressive therapies such as angioplasty, surgery, and thrombolysis. Decisions regarding these therapies are based on an incomplete data base and an understanding of the patients' needs. A review of the available data on cardiac surgery, angioplasty, myocardial infarction, and heart failure in the elderly is presented.

Cancer incidence and mortality were ascertained in a cohort of 1910 male participants of the Albany Cardiovascular Health Center (CVHC). The New York State Cancer Registry, vital records files, CVHC follow-up records, New York State Retirement System files, and New York State Department of Motor Vehicles driver's license files were used. Serum cholesterol measurements as well as values for other exposure variables were obtained from records of medical examinations which began in 1953-1954. The study cohort was divided into two groups, based on initial serum cholesterol measurement (less than or equal to 190 mg/100 ml and less than or equal to 190 mg/100 ml). For total cancers, both incidence and mortality were similar in these groups. For digestive cancer, both incidence and mortality were slightly lower in the less than or equal to 190 mg/100 ml group. The deficit was not statistically significant. For respiratory cancer, relative risk and rate ratio estimates were in the range of 1.4-1.7 for incidence and mortality. The excess risk in the less than or equal to 190 mg/100 ml group was of borderline statistical significance. The association was concentrated in the lowest cholesterol quintile rather than suggesting a strong dose-response relationship. The estimates were not found to be confounded by cigarette smoking, body mass index, education or age. A reduction in the crude rate ratio estimate from 1.5 to 1.2 was observed when early cases were excluded, suggesting that part of the observed excess may be due to preclinical cancer.

Endoscopic retrograde sphincterotomy was performed on four sedated pigs, ages 3-4 months, using a standard human duodenoscope and papillotome. Sphincterotomies, 1 cm in length, were well-tolerated, and all animals recovered promptly, spontaneously regained gastrointestinal function, and gained weight. The first three animals were sacrificed after one week, and autopsy revealed no complications. The fourth animal was sacrificed immediately following the procedure, and no evidence of perforation was found. These observations demonstrate that the pig is a valid experimental model for endoscopic sphincterotomy. Its use in training is limited by technical and anatomic differences from humans. Potential uses of this technique in research are discussed.

Patients in long term studies of coronary heart disease may have different levels of risk during the course of study. Smoking habits, blood pressure, and obesity may change drastically during this period. The multiple logistic model, the most commonly used model for the analysis of coronary heart disease studies, does not consider survival time in assessment of the dependent covariates and does not account for the censoring which usually occurs in such studies. We propose a Cox model with time-dependent covariates to model the risk of coronary heart disease in the Albany study. The Cox model we fitted evaluates the patients' risk on the basis of the data at the last visit. With this methodology, we can evaluate whether it is advantageous for individuals to modify their risk of disease by their effecting changes in their covariates, that is to stop smoking, lose weight, change diet and so on. The important covariates that explain the risk of coronary heart disease were the same in our model as in the models used in the earlier reports. The estimated relative risks were slightly higher in most cases and lend more support to the need to encourage patients to achieve a better covariate state.

In a prospective study on Coronary Heart Disease (CHD) orthogonal electrocardiograms (Frank) were recorded annually for ten years from 1,444 asymptomatic, middle-aged males with a mean age of 57.4 +/- 10.6 years. Cases with overt or suspected CHD were excluded. The purpose of the study was to identify risk indicators in electrocardiograms and to compare them with other known risk factors used for prediction of acute CHD events such as myocardial infarction (MI) and/or cardiac death (CD). Such acute events occurred in 88 cases. Pre-event ECGs of these acute events were compared with all others without events, using logistic regression analysis. Identified ECG risk indicators were then compared with other known risk factors such as smoking, blood pressure, cholesterol, age, weight, etc. The predictive power of the ECG, derived mainly from the ST-T complex, exceeded all others by a wide margin. The amplitude of the first 1/8 of the ST-T complex in lead x (similar to V5-V6) together with relative body weight proved best when one pre-event record was available. Prediction improved when ECG changes between two pre-event recordings were included. Precision of measurements by computer appeared essential for improvements in CHD prediction.

The relation of alcohol consumption to coronary heart disease (CHD) was examined in a cohort of 1910 employed men aged 38 to 55 years enrolled in the Albany Study, a prospective investigation of factors related to cardiovascular disease. Two follow-up periods were examined, one between 1953-54 and 1971-72, the other after 1971-72. In the initial period, there was no clear evidence of a relation between the rate of alcohol consumption and CHD incidence. In the later period, men whose monthly consumption was 60 ounces or more had a lower than average CHD incidence rate. A negative relationship with drinking held for all manifestations of CHD. Other CHD risk factors were examined, special attention being given to cigarette smoking and HDL-cholesterol. The fact that drinking is a matter of choice may introduce some confounding factors. The absence of a relation between drinking and CHD risk in the earlier follow-up suggests the need for caution in interpreting the results.

Between 1953 and 1955, a still-ongoing prospective cardiovascular study enrolled 1,910 employed men. Men who drank more alcohol had higher levels of cigarette use, blood pressure, and high-density-lipoprotein cholesterol concentrations. After 18 years of follow-up, the survivors consumed twice as much alcohol, on the average, as they had at entry, weighed slightly more, and had substantially higher blood pressures. Formerly heavy drinkers who had quit, however, lost weight and had less than average blood pressure increases. There was no significant association between changes in smoking habits and changes in drinking habits.

The relation of alcohol consumption to mortality was examined in a cohort of 1,910 employed men aged 38-55 years, enrolled in the Albany Study, a prospective investigation of factors related to cardiovascular disease. Two follow-up periods were examined, one between 1953-1954 and 1971-1972 and the other after 1971-1972. In both periods, there was a positive relation between the rate of alcohol consumption and noncoronary heart disease death, not assignable to any specific cause. Coronary heart disease death was not associated with drinking during the initial follow-up but was negatively associated with drinking in the later follow-up. All-cause mortality was positively associated with alcohol consumption in the earlier follow-up, because of the greater cigarette use among drinkers, but not in the later follow-up. There was a significant positive relation of drinking to deaths from liver cirrhosis and diabetes but not to deaths from motor vehicle accidents.

In the Albany Study cohort of 1910 men, first examined between 1953 and 1955, 27-year mortality was least at relative weights between 100 and 109% of those considered desirable according to the 1959 Build and Blood Pressure Study. Mortality was greater at lower and higher weights. This association of weight and mortality was substantially stronger during the first 15 years after characterization than in the remaining 12 years. In contrast to the Albany Study, the 1979 insurance study and a study by the American Cancer Society reported minimum mortality for men at average weight by height, which is 15-20 pounds above the 1959 insurance standards. Other studies have reported minimum mortality at above-average weights. Perhaps the concept of an unvarying 'desirable' weight should be abandoned.

The relation between coronary heart disease (CHD) prevalence and fasting lipid levels was assessed by a case-control study in five populations with a total of 6859 men and women of black, Japanese and white ancestry drawn from subjects aged 40 years and older from populations in Albany, Framingham, Evans County, Honolulu and San Francisco. In each major study group mean levels of high density lipoprotein (HDL) cholesterol were lower in persons with CHD than in those without the disease. The average difference was small -- typically 3-4 mg/dl -- but statistically significant. It was found in most age-race-sex specific groups. The inverse HDL cholesterol-CHD association was not appreciably diminished when adjusted for levels of low density lipoprotein (LDL) cholesterol and triglyceride. LDL, totoal cholesterol and triglycerides were directly related to CHD prevalence; surprisingly, these findings were less uniformly present in the various study groups than the inverse HDL cholesterol-CHD association.

This statement is an update of the 1970 Inter -Society Commission for Heart Disease Resources (ICHD) report, Primary Prevention of the Atherosclerotic Diseases. The charge to the Study Group was to assess relevant new data and, where the evidence is less than definitive, to formulate conclusions and recommendations based on best judgment. Current developments are reviewed, and issues raised in response to the earlier recommendations are considered. Recommendations are intended to serve as a guide for individual behavior, physician practice, and the formulation of public health policy.

Data from five study populations participating in the Cooperative Lipoprotein Phenotyping Study indicate strong relations between reported alcohol consumption and blood-lipids. Alcohol consumption was positively associated with high-density-lipoprotein cholesterol level in all populations (r from 0-16 to 0-30), the lipid level appearing to be a graded response even over the low levels of alcohol consumption reported. Less strong but consistently negative correlations were found with low-density-lipoprotein cholesterol. Plasma-triglycerides showed a modest positive correlation with alcohol. The five populations were those of the Albany, Evans County, Framingham, Honolulu, and San Francisco Studies.

Precursors of sudden death were sought in men--1838 civil servants in Albany, New York, and 2282 residents of Framingham, Massachusetts--under continuous surveillance for 16 years. In men 45-74 years old there were 234 deaths attributed to coronary heart disease (CHD) of which 109 occurred within one hour of onset of symptoms. More than half of all deaths due to CHD occurred outside the hospital and about 80 per cent of these were sudden. Most were unheralded by prior symptoms of CHD. Persons at high risk of death from CHD, including sudden death, can be identified long before the terminal unexpected catastrophe. The same precursive stigmata exist in persons subject ot coronary attacks whether or not immediately fatal. The risk of sudden death in these two populations was positively correlated with high blood pressure, the electrocardiographic pattern of left ventricular enlargement, obesity, and heavy cigarette usage. Sudden death is a common and possibly incidental expression of lethal coronary heart disease. The potential candidate for sudden death cannot be confidently distinguished from the individual who succumbs more slowly of myocardial infarction. The inescapable conclusion is that the prevention of sudden death requires the prevention of coronary attacks.

Correlations between 276 orthogonal electrocardiographic measurements and constitutional variables were made in 450 normal women, aged 18 to 90 years. Advancing age led to decreases in amplitudes, left QRS axis shift, rightward and superior displacement of the ST segment, and anterior shift of the T wave. QZ was absent in 1% of normal women over age 40. In the oldest subjects, Ry amplitude was 71% and RZ amplitude was 80% of the respective values in the youngest group. Whereas QRS amplitude decreases with age leveled off at the sixth decade of life, they continued to old age for ST-T measurements. Men revealed steeper age trends than women. Blacks had larger QRS amplitudes and smaller Q/R ratios than whites. Stratification of electrocardiographic criteria according to age, sex, and race appears essential for routine interpretations and for epidemiological studies where new events, such as myocardial infarcts, need to be differentiated from normal age trends.

When assessing patients' serial ECG changes, the clinician implicitly compares those changes to the limits of change expected in a healthy population. Prospective epidemiological studies, too, develop their criteria from the limits of normal serial ECG changes. Surprisingly then, few studies have reported normal limits for changes between serial ECGs taken six months or longer apart, and all are based on small samples. The present study has a large sample size: 243 white middle-aged and older males, after exclusions for heart disease. Each had at least four consecutive annual examinations with ECGs. Limits of serial variability were computed for 52 measurements. The ECG measurements included durations, amplitudes, ratios, angles and spatial magnitudes. Clinical measurements included blood pressure, cholesterol relative weight and hemoglobin. Year-to-year ECG variabilities were compared to day-to-day variabilities of the same measurements reported earlier. Year-to-year variation was virtually identical to the reported day-to-day variation in most measurements. In only two measurements was year-to-year variation over 25% greater than the reported day-to-day variation.
