Abstract

Protein-coding genetic variants that strongly affect disease risk can yield relevant clues to disease pathogenesis. Here we report exome-sequencing analyses of 20,791 individuals with type 2 diabetes (T2D) and 24,440 non-diabetic control participants from 5 ancestries. We identify gene-level associations of rare variants (with minor allele frequencies of less than 0.5%) in 4 genes at exome-wide significance, including a series of more than 30 SLC30A8 alleles that conveys protection against T2D, and in 12 gene sets, including those corresponding to T2D drug targets (P = 6.1 × 10-3) and candidate genes from knockout mice (P = 5.2 × 10-3). Within our study, the strongest T2D gene-level signals for rare variants explain at most 25% of the heritability of the strongest common single-variant signals, and the gene-level effect sizes of the rare variants that we observed in established T2D drug targets will require 75,000-185,000 sequenced cases to achieve exome-wide significance. We propose a method to interpret these modest rare-variant associations and to incorporate these associations into future target or gene prioritization efforts.

Abstract

Objectives. Cardiovascular disease (CVD) is the leading cause of death and disability among persons with diabetes. Early intervention on cardiovascular risk factors (CRFs) is important in reducing CVD burden. The SEARCH for Diabetes in Youth study assessed CRFs in incident cohorts of youth aged <20 years established from 2002 to 2016. Research Design and Methods. Regression models assessed trends over each incident year for lipids (total cholesterol (TC), HDL‐c, LDL‐c, triglycerides (TG), VLDL‐c, and non‐HDL‐c), kidney function (albumin/creatinine ratio (ACR) ≥30 and ≥300, cystatin C, serum creatinine and estimated glomerular filtration rate (eGFR)), systolic and diastolic blood pressure (BP) z‐scores, BMI z‐score, waist circumference (WC), and an inflammatory marker (C‐reactive protein (CRP)). Models were stratified by diabetes type (type 1 diabetes (T1D), N = 4,600; type 2 diabetes (T2D), N = 932) and adjusted for age at diagnosis, sex, race/ethnicity, and diabetes duration. An interaction analysis assessed differential time trends by type. Results. For youth with T1D, all CRFs significantly improved over time, with the exception of ACR > 300, cystatin C, serum creatinine, eGFR, and CRP. For youth with T2D, TC, LDL‐c, and non‐HDL‐c significantly improved, while eGFR, BMI z‐score, and CRP significantly worsened. Significant differences in trends over time by type were seen for TC, HDL‐c, BMI z‐score, BP z‐scores, WC, and CRP. Conclusions. Overall, improvements in CRFs were more often observed in youth with T1D. Youth with T2D had worsening trends over time in BMI z‐score, CRP, and kidney function. Abstract

Few studies have explored durability of insulin pump use, and none have explored the link between depression and pump discontinuation. To examine the relationship between depressive symptoms [measured by the Children's Depression Inventory (CDI)], method of insulin delivery, and hemoglobin A1c (A1c), mixed models were used with data from 150 adolescents with type 1 diabetes (T1D) and visits every 6 months for 2 years. Of the 63% who used a pump, compared with multiple daily injections (MDI) at baseline, there were higher proportions who were non-minorities, had caregivers with a college degree, private insurance, and two caregivers in the home (p ≤ 0.01). After adjusting for time, sex, age, T1D duration, frequency of blood glucose monitoring, ethnicity, insurance, and caregiver number and education, baseline pump use was associated with -0.79% lower mean A1c [95% confidence interval (CI): -1.48, -0.096; p = 0.03]. For those using a pump at baseline, but switching to MDI during the study (n = 9), mean A1c was 1.38% higher (95% CI: 0.68, 2.08; p < 0.001) than that for those who did not switch method of delivery. A 10-point increase in CDI was associated with a 0.39% increase in A1c (95% CI: 0.16, 0.61; p = 0.001), independent of pump use. Regarding the temporal relationship between CDI score and changing method of insulin delivery, prior higher CDI score was associated with switching from pump to MDI (odds ratio = 1.21; 95% CI: 1.05, 1.39; p = 0.007). Clinicians should be aware of the associations between depressive symptoms, change in insulin delivery method, and the effect on glycemic control.

20. Removing Barriers to Learning: Factors That Affect Participation and Dropout in Parent Interventions. Report No. 27.

Abstract

Even the most well developed and well executed school-based parent interventions frequently have low rates of participation and high rates of dropout. A three-stage study employed focus groups and interviews to examine why parents did or did not participate in school-based programs designed to involve them, and why they either continued to participate or dropped out. Two design conditions were developed to make participation easier: one with low support elements--workshops offered twice a week, a meal provided, public relations in the form of flyers, banners, and phone calls; and one with high support elements--child care, transportation, home visits. The study found that even with high support program elements, parental participation did not increase greatly, 48 percent compared to 38 percent for the low-support groups. However, the increased number of parents who did participate were primarily those considered the most difficult to reach. Dropouts were the result of factors that were not within the control of the support mechanism, such as time constraints and personal problems that interfered with program completion. Reasons for continuing parental nonparticipation in programs containing elements to increase participation included lack of time, belief that the program would not help them or their children, lack of interest, and other factors. Transportation and child care were not considered barriers by those not participating. (HTH)

21. Project Self Help: A First-Year Evaluation of a Family Literacy Program. Report No. 8.

Abstract

This report describes the first year evaluation of the implementation and effects of the Project SELF HELP, a family literacy program located in two urban elementary schools and run by a community organization. The program was available to 15 adults from each site who had lower than a fifth grade reading level and whose children were experiencing academic difficulty in school. Parents were given counseling and instruction in parenting skills, life skills, and other basic skills. Each parent regularly participated in school activities. Older children participated in an after-school tutorial program, while younger children received day care services. Opportunities for joint adult-child interactions were regularly provided. Preliminary findings indicated that elementary students benefitted from summer reading activities. According to results from two measures, parents' literacy improved. Parents also had high scores on the home environment index. Teachers rated students as significantly improved in social behavior, attention, and control of disruptive behaviors. These findings should be interpreted cautiously because of the limited sample size and the fact that this was the first year of the program. 22. The Role of Family Support and Integrated Human Services in Achieving Success for All in the Elementary School.

Abstract

The Success for All elementary school restructuring program attempts to ensure that all students in high poverty schools will be at or near grade level in basic skills by the end of the third grade and will maintain or better that standing through the rest of their elementary school careers. The program emphasizes prevention and early intervention, research-based interventions in curriculum and instruction at all grade levels, one-to-one tutoring, and other components, including the Family Support and Integrated Human Services (FSIHS) components. This report provides a research base and rationale for the FSIHS component, describes the structure and function of the FSIHS component, and illustrates how family support teams in five Baltimore (Maryland) elementary schools promote attendance, school-based interventions, parent involvement, and integration of services. The Family Support Teams are composed of all of the school's resource personnel, including guidance counselors, social workers, parent liaisons, administrators, facilitators or master teachers, and, in some sites, staff from city social service and health departments. Ten bar graphs are presented on the attendance and retention rates of each school since it began the use of Success for All and family support teams. Included are 16 references. (Author/RLC)

23. Models for Integrating Human Services into the School.

Abstract

This report examines five models of school-based integrated human service programs to evaluate the effects of the programs in light of the growing support for and implementation of these programs. The study examined the following programs: (1) school-based health clinics in Baltimore (Maryland); (2) Success for All (an elementary school-level program at 35 sites nationwide); (3) the New Jersey School-Based Youth Services Program (human services); (4) the New Beginnings program in San Diego (California) providing health and social services; and (5) the Comer School Development Model based on the model developed by J. Comer. The study sought to document some of the characteristics of programs perceived to be effective and to outline some of the evaluation strategies that might lead toward increased understanding of the impact of these programs on the children and families they serve. The report describes each of the programs in detail. A section on lessons from successful programs lists the following seven components important to success: (1) collaborative planning; (2) ownership by the school; (3) principal's role; (4) case manager; (5) shared resources; (6) gradual phase-in; and (7) training and staff development. A section on evaluation issues notes the need for current data and the high burden of data collection as well as the resistance to evaluation of still developing programs. Included are 16 references. Abstract

OBJECTIVE: To examine the association between diabetes stigma, socioeconomic status, psychosocial variables, and substance use in adolescents and young adults (AYAs) with type 1 or type 2 diabetes. RESEARCH DESIGN AND METHODS: This is a cross-sectional analysis of AYAs from the SEARCH for Diabetes in Youth study who completed a survey on diabetes-related stigma, generating a total diabetes stigma score. Using multivariable modeling, stratified by diabetes type, we examined the relationship of diabetes stigma with variables of interest. RESULTS: Of the 1,608 AYAs who completed the diabetes-related stigma survey, 78% had type 1 diabetes, and the mean age was 21.7 years. Higher diabetes stigma scores were associated with food insecurity (P = 0.001), disordered eating (P < 0.0001), depressive symptoms (P < 0.0001), and decreased health-related (P < 0.0001) and diabetes-specific quality of life (P < 0.0001). Abstract

Aims. Celiac disease (CD) in adults with type 1 diabetes has been associated with increased cardiovascular risk and the earlier occurrence of diabetes-associated complications. In the Search for Diabetes in Youth study, we aimed to assess the frequency of CD and the potential for undiagnosed CD among youth with childhood onset type 1 diabetes. In addition, we assessed the burden of cardiovascular risk factors and diabetes-associated complications in youth with type 1 diabetes by CD status and IgA tissue transglutaminase autoantibody (tTGA) levels. Methods. 2,444 youths with type 1 diabetes completed a CD questionnaire and underwent tTGA testing. Integrating the celiac disease questionnaire and tTGA results for this cross-sectional analysis, participants were categorized as follows: (1) reported CD; (2) seropositive for CD (no reported CD and seropositive tTGA); and (3) type 1 diabetes only (comparison group: no reported CD and seronegative tTGA). Subanalyses were performed on those with no reported CD and tTGA ≥10x ULN, designated potentially undiagnosed CD. Cardiovascular risk factors and diabetes-associated complications were evaluated by CD status and tTGA levels utilizing a Poisson model to estimate relative risk. Results. Reported CD in youths with type 1 diabetes was 7%. Seropositivity for tTGA with no reported CD was present in 4%, and 1.2% had potentially undiagnosed CD.
