Introduction There are scarce data on the routine latent tuberculosis infection treatment (LTBIT) and factors associated with a non-completion in high tuberculosis burden countries. Therefore, in this study we aimed to evaluate the factors associated with non-completion of LTBIT. Materials and methods This was a non-matched case control study conducted at a University Hospital in Rio de Janeiro, Brazil. A total of 114 cases and 404 controls were enrolled between January/1999 and December/2009. Cases were close contacts who did not complete the LTBIT and controls were the contacts that completed it. Multivariate analysis was used to investigate risk factors associated with non-completion of LTBIT among contacts in two different periods of recruitment. Results Factors associated with non-completion LTBIT included: drug use (OR 23.33, 95% CI 1.83–296.1), TB treatment default by the index case (OR 16.97, 95% CI 3.63–79.24) and drug intolerance. TB disease rates after two years of follow up varied from 0.4% to 1.9%. The number necessary to treat to prevent one TB case among contacts was 116. Conclusions Non-completion treatment by the index case and illicit drug use were associated with not completing latent tuberculosis infection treatment and no tuberculosis disease was identified among those who completed latent tuberculosis infection treatment.

Introduction Human herpesvirus 8 (HHV-8) is associated with the pathogenesis of Kaposi Sarcoma and interstitial pneumonitis in adults. This study aims to evaluate association between HHV-8 and interstitial lung disease in HIV-infected children. Methods HIV-infected children with interstitial pneumonitis underwent lung biopsies in a tertiary hospital and were investigated for HHV-8, Epstein-Barr virus (EBV) and cytomegalovirus (CMV) using polymerase chain reaction (PCR) and immunohistochemistry in lung tissue. Peripheral blood PCR was also performed for HHV-8. Results From six patients included, PCR for HHV-8 was positive in lung samples in four children and in peripheral blood in one. PCR for EBV and CMV and immunohistochemical study for HHV-8, EBV and CMV in lung were negative in all patients. Conclusion No previous cases of HHV-8-associated interstitial pneumonitis was described in HIV-infected children. An immunological disorder and an infectious agent might influence development of the lymphoid interstitial pneumonitis. HHV-8 may be this infectious trigger.

A síndrome da cimitarra, embora rara, é importante de ser conhecida por pediatras e pneumologistas, pois a imagem radiológica, apesar de sugerir o diagnóstico, pode ser mal-interpretada, levando a falsos diagnósticos. A partir da suspeita diagnóstica, a investigação com ecocardiograma e angiografia é fundamental, mesmo no paciente assintomático, pois pode-se evidenciar hipertensão pulmonar importante, de tal forma que o tratamento cirúrgico é indicado para evitar complicações futuras irreversíveis. Criança, apesar das dificuldades diagnósticas iniciais, foi operada precocemente, havendo excelente evolução durante o seguimento ambulatorial.Scimitar syndrome is rare, but knowledge about it is important to pediatricians and pneumologists. X-ray can suggest diagnosis, but the interpretation can be wrong. If a diagnosis is made, further anomalies must be investigated with echocardiography and angiography, even if the patient is asymptomatic. This child showed evidence of pulmonary hypertension and required surgery. Postoperative evolution was excellent.

Dissertação de Mestrado em Comunicação em Saúde apresentada à Universidade Aberta O presente estudo procura analisar a comunicação que se estabelece entre os Técnicos de Saúde – Cardiopneumologistas e os doentes, na prática de exames complementares de diagnóstico e terapêutica da área da Pneumologia – Provas de Função Respiratória (PFR) – Espirometrias. Este estudo desenvolve-se na área da comunicação em saúde, centrando-se na importância dos factores comunicacionais na relação entre o doente e os prestadores de cuidados, na satisfação percebida pelo doente, enquanto indicadores da qualidade dos serviços. Insere-se no âmbito do Estudo Funcional Respiratório (EFR) e circunscreveu-se apenas à técnica da Espirometria, que sendo uma das técnicas mais utilizadas, é também muito influenciada pelo desempenho do doente e do CPL. O estudo teve lugar no laboratório de função respiratória do Hospital Garcia de Orta onde a autora desempenha a sua actividade profissional. Partindo de uma contextualização teórica em que se analisa o processo de comunicação e toda a sua envolvente. Foi dado especial relevo ás relações interpessoais, ás diferentes formas de comunicação, ás atitudes e crenças, ás emoções e à tecnologia de diagnóstico nomeadamente à correspondente à Cardiopneumologia (CPL). O modo de comunicar e de agir de um indivíduo com outro promove o êxito ou o fracasso da relação interpessoal. Durante a interacção humana, gestos e voz são simultaneamente utilizados para expressar não só informação verbal, mas também aspectos comunicativos importantes que enriquecem, complementam e clarificam a conversação. De entre estes aspectos comunicativos não linguísticos encontra-se o estado emocional do falante, que se manifesta através da modulação de diferentes canais de comunicação, incluindo a expressão facial e a entoação. Foram estudados doze doentes durante a realização de Espirometrias, através de um estudo exploratório. Os objectivos do estudo foram: - Verificar se o acolhimento, a integração e a explicação dada ao doente no laboratório de função respiratória, influenciam os resultados, - Verificar se durante a realização da Espirometria, sendo a comunicação/explicação, transmitida pelo CPL considerada uma parte importante deste, influencia o desempenho e a satisfação do doente e por sua vez, a fidedignidade dos resultados, - Verificar se para além da comunicação, existem outros factores que influenciam no desempenho e na satisfação do doente, sendo assim necessário alterar a forma de comunicar com os doentes para promover uma melhor colaboração nestes exames, - Verificar se o tipo de acolhimento dado aos doentes pode ser influenciado pelo número de exames diários realizados no laboratório. Foi utilizada uma metodologia qualitativa e os resultados obtidos confirmaram maioritariamente as expectativas The present study aims at analyzing the communication process taking place between health professional in cardiopneumology (CPL) and the patients, during routine tests for diagnosing purposes, and for complementary exams and treatment in the field of Pulmonary Respiratory Function - Spirometry. It was hypothesized that the quality of communication between patient and professionals would have a positive impact on the satisfaction of the patient as well as on the level of the understanding of the practice, particularly in the case of spirometry where the cooperation of the client is of the utmost importance The study was conducted in the Hospital Garcia de Orta, where the author works, using an observational technique followed by questionnaires that the patients were asked to answer about their satisfaction and level of anxiety experimented Five sessions out of twelve exams were video recorded for further examination. Verbal and non verbal communication and technical skills of the professionals were the independent variables to be related with the attitudes and the behaviours of the patients The analysis of the results confirmed the hypothesis. Although, in broad terms, and due to the skills of the well trained professionals, no particularly negative results were to be expected, the technique of recording the exams permited a more detailed assessment of the practices that contributes to improve the performance mostly in what concerns the way of communicating with the patients

OBJETIVO: Contribuir para o melhor conhecimento da atividade de pesquisa em Pneumologia no Brasil. MÉTODOS: Estudo retrospectivo observacional dos resumos publicados nos Anais dos Congressos Brasileiros de Pneumologia e Tisiologia de 1986 a 2004, para descrição quantitativa da distribuição geográfica e do tipo das instituições de origem dos resumos, dos tipos de estudos e dos temas abordados. RESULTADOS: Foram publicados 6.467 resumos, sendo 3.870 originários de instituições do Sudeste, 1.309 do Sul, 783 do Nordeste, 267 do Centro-Oeste e 84 do Norte. Houve 94 resumos provenientes de instituições estrangeiras, em especial portuguesas (56,3%) e norte-americanas (13,8%). A maioria dos trabalhos (5.825) foi realizada em instituições públicas brasileiras. Quanto aos tipos de estudos, foram observados 4.234 estudos clínicos, 1.994 relatos de caso e 239 estudos experimentais, com elevação progressiva acentuada do número de estudos clínicos e de relatos de caso no período. No cômputo geral, tuberculose e outras doenças infecciosas foram os temas mais freqüentes (25,2%), seguidos por oncologia (11,6%), doenças pulmonares intersticiais (8,8%) e cirurgia torácica (8,5%). Entretanto, houve grandes variações no número de resumos sobre cada tema no decorrer do período. CONCLUSÃO: As instituições públicas brasileiras são as principais responsáveis pela atividade de pesquisa em Pneumologia no Brasil, a qual está concentrada na Região Sudeste. Um terço desta atividade é representada por relatos de caso e, embora haja grande variação de assuntos abordados, as doenças com elevada prevalência no país, como tuberculose e outras infecções pulmonares, são os temas mais freqüentes. OBJECTIVE: To increase the knowledge base regarding pulmonology research in Brazil. METHODS: A retrospective, observational study of the abstracts published in the Annals of the Brazilian Pulmonology and Phthisiology Conferences held from 1986 to 2004, quantifying the institutions of origin by geographic distribution and type, as well as categorizing the abstracts by study design and topic. RESULTS: A total of 6467 abstracts were published. The institutions of origin were located, variously, in the Southeast (3870 abstracts), South (1309), Northeast (783), Central-West (267) and North (84). There were 94 abstracts originating from foreign institutions, especially from institutions in Portugal (56.3%) and the United States (13.8%). Most of the studies (5825) were conducted in public Brazilian institutions. There were 4234 clinical studies, 1994 case reports and 239 original research articles. A marked, progressive increase was observed in the number of clinical studies and case reports during the period evaluated. Overall, the most common themes were tuberculosis and other infections diseases (25.2%), following by oncology (11.6%), interstitial lung diseases (8.8%) and thoracic surgery (8.5%). Nevertheless, the number of abstracts on each topic varied widely from year to year. CONCLUSION: Public Brazilian institutions are the principal sources of pulmonology research in Brazil. Such research activity is concentrated in the southeastern part of the country. Case reports account for one-third of this activity. Although there was great variability in the subjects addressed, diseases that are highly prevalent in Brazil, such as tuberculosis and other infections diseases, were the most common topics.

Introduction This study sought to assess the prognostic value of the kinetics of procalcitonin (PCT), C-reactive protein (CRP) and clinical scores (clinical pulmonary infection score (CPIS), Sequential Organ Failure Assessment (SOFA)) in the outcome of ventilator-associated pneumonia (VAP) at an early time point, when adequacy of antimicrobial treatment is evaluated. Methods This prospective observational cohort study was conducted in a teaching hospital. The subjects were 75 patients consecutively admitted to the intensive care unit from October 2003 to August 2005 who developed VAP. Patients were followed for 28 days after the diagnosis, when they were considered survivors. Patients who died before the 28th day were non-survivors. There were no interventions. Results PCT, CRP and SOFA score were determined on day 0 and day 4. Variables included in the univariable logistic regression model for survival were age, Acute Physiology and Chronic Health Evaluation (APACHE) II score, decreasing ΔSOFA, decreasing ΔPCT and decreasing ΔCRP. Survival was directly related to decreasing ΔPCT with odds ratio (OR) = 5.67 (95% confidence interval 1.78 to 18.03), decreasing ΔCRP with OR = 3.78 (1.24 to 11.50), decreasing ΔSOFA with OR = 3.08 (1.02 to 9.26) and APACHE II score with OR = 0.92 (0.86 to 0.99). In a multivariable logistic regression model for survival, only decreasing ΔPCT with OR = 4.43 (1.08 to 18.18) and decreasing ΔCRP with OR = 7.40 (1.58 to 34.73) remained significant. Decreasing ΔCPIS was not related to survival (p = 0.59). There was a trend to correlate adequacy to survival. Fifty percent of the 20 patients treated with inadequate antibiotics and 65.5% of the 55 patients on adequate antibiotics survived (p = 0.29). Conclusion Measurement of PCT and CRP at onset and on the fourth day of treatment can predict survival of VAP patients. A decrease in either one of these marker values predicts survival.

Asma é a doença crônica mais freqüente entre as crianças e representa um sério problema de saúde pública. Há evidência de aumento na prevalência de sibilância recorrente e asma, bem como de sua morbidade e mortalidade em muitos países nos últimos anos. Em nosso meio contribui com altos índices de utilização dos serviços de saúde e é causa importante de limitação da qualidade de vida das crianças. Com a finalidade de estudar a ocorrência de sibilância recorrente e os principais fatores de risco e proteção associados, foi constituída uma coorte de base populacional com 775 crianças menores de 13 anos, na cidade do Rio Grande, RS. Sibilância recorrente foi definida como três ou mais episódios de sibilos que necessitaram de medida terapêutica com bronco-dilatadores e/ou antiinflamatórios, nos 12 meses anteriores à entrevista. Questionários padronizados e validados em diversos estudos, com questões relativas à sibilância recorrente e doenças respiratórias, foram administrados por entrevistadores treinados, em 1997 e 2004. A análise dos dados compreendeu duas etapas: análise bivariada, com o cálculo dos odds ratio (OR) e análise multivariada, utilizando um modelo hierarquizado para o controle dos fatores de confusão, por meio de regressão logística não-condicional. A ocorrência de sibilância recorrente atual foi de 27,9%. As perdas do estudo foram de 11,6%. Os principais fatores de risco significativamente associados à sibilância recorrente, após a análise ajustada, foram: rinite atual com OR=45,7 (IC95% 24,2-86,5), uso de fogão à lenha com OR=2,7 (IC95% 1,4-4,9), antecedente pessoal de infecção respiratória aguda (IRA) com OR=2,1 (IC95% 1,3- 3,5), aleitamento artificial com OR=2,1 (IC95% 1,1-3,8), antecedente de asma em irmãos com OR=1,9 (IC95% 1,2-3,2), antecedente de asma na mãe com OR=1,8 (IC95% 1,1-2,9) e menos de seis consultas de pré-natal com OR=1,6 (IC95% 1,1-2,4). Escolaridade do pai ≤ 8 anos foi fator de proteção, com OR=0,6 (IC95% 0,4-0,9). Este estudo mostrou que sibilância recorrente tem uma alta prevalência em Rio Grande/RS, constitui um importante problema de saúde pública e, como tal, deve-se direcionar ações apropriadas para o seu controle e tratamento. Recurrent wheezing is highly prevalent in our region and ranks with high rates of demand of health care. In their most serious forms these diseases are the main causes of morbidity and hospitalization in children. The present study was aimed to determine the prevalence and risk factors for recurrent wheezing. A longitudinal cohort was carried out in a representative sample of children under 13 years old living in the urban area of Rio Grande, southern Brazil. The overall participation rate for the originally selected sample was 88,4%, amount to 775 subjects. The statistic analysis of data comprised two phases: bivariate analysis, with calculation of Odds Ratio (OR) of each risk factor, and multivariate analysis, based on the results of the previous analysis, through a hierarchy model for coufounders control with logistic regression. The overall prevalence of recurrent wheezing was 27,9%. In the multivariate model, the following risk factors remained significant: personal rhinitis (OR=45.7, 95%CI=24.2 to 86.5); firewood stove (OR=2.7, 95%CI=1.4 to 4.9); previous acute respiratory disease (ARI) (OR=2.1, 95%CI=1.3 to 3.5); no breast feeding (OR=2.1, 95%CI=1.1 to 3.8); asthma in the siblings (OR=1.9, 95%CI=1.2 to 3.2); maternal asthma (OR=1.8, 95%CI=1.1 to 2.9); prenatal visit frequence less (OR=1.6, 95%CI=1.1 to 2.4). Low parental instruction was protective factor (OR=0.6, 95%CI=0.4 to 0.9). This study shows: personal rhinitis, firewood stove, previous ARI, no breast feeding, asthma in siblings and mothers and prenatal visit frequence less are important risk factors for recurrent wheezing in this population.
