Abstract

Background and Objectives: The European Medicine Agency extended the use of Comirnaty, Spikevax, and Nuvaxovid in paediatrics; thus, these vaccines require additional real-world safety evidence. Herein, we aimed to monitor the safety of COVID-19 vaccines through Covid-19 Vaccine Monitor (CVM) and EudraVigilance surveillance systems and the published pivotal clinical trials. Methods: In a prospective cohort of vaccinees aged between 5 and 17 years, we measured the frequency of commonly reported (local/systemic solicited) and serious adverse drug events (ADRs) following the first and second doses of COVID-19 vaccines in Europe using data from the CVM cohort until April 2022. The results of previous pivotal clinical trials and data in the EudraVigilance were also analysed. Results: The CVM study enrolled 658 first-dose vaccinees (children aged 5–11 years; n = 250 and adolescents aged 12–17 years; n = 408). Local/systemic solicited ADRs were common, whereas serious ADRs were uncommon. Among Comirnaty first and second dose recipients, 28.8% and 17.1% of children and 54.2% and 52.2% of adolescents experienced at least one ADR, respectively; injection-site pain (29.2% and 20.7%), fatigue (16.1% and 12.8%), and headache (22.1% and 19.3%) were the most frequent local and systemic ADRs. Results were consistent but slightly lower than in pivotal clinical trials. Reporting rates in Eudravigilance were lower by a factor of 1000. Conclusions: The CVM study showed high frequencies of local solicited reactions after vaccination but lower rates than in pivotal clinical trials. Injection-site pain, fatigue, and headache were the most commonly reported ADRs for clinical trials, but higher than spontaneously reported data.

Abstract

Fatal Adverse Events (FADEs) are a major public health problem, and some FADEs could be preventable. The aim of the present study is to describe the frequency, the drugs involved and the preventability in the FADEs collected through the MEREAFaPS Study between 2012 and 2018. All cases including the outcome “death” have been examined. We excluded cases with vaccine-related ADEs, overdose or suicide, and ADEs occurred during the hospitalisation. Two trained assessors evaluated all cases fulfilling the inclusion criteria. ADEs’ preventability was evaluated applying the Schumock and Thornton algorithm. During the study period, we observed 429 cases of death, 92 of which were excluded. The remaining 337 cases involved 187 women and 150 men, with a mean age of 79 and of 77years, respectively. For each report, the suspected drugs and concomitant ones were 1.26 and 4.20, respectively. Anticoagulants and antiplatelet agents account for more than 40% of FADE cases and the most frequent reactions are haemorrhages (37.5%). The 25% of the FADEs were preventable. This study confirms that FADEs are still a relevant clinical occurrence, and are often caused by widely used old drugs associated with adverse events. The death of one in four patients was preventable. Further efforts should be done to improve the appropriateness of the therapy, especially in older patients who are treated with anticoagulants.

Abstract

To date, four vaccines have been authorised for emergency use and under conditional approval by the European Medicines Agency to prevent COVID-19: Comirnaty, COVID-19 Vaccine Janssen, Spikevax (previously COVID-19 Vaccine Moderna) and Vaxzevria (previously COVID-19 Vaccine AstraZeneca). Although the benefit–risk profile of these vaccines was proven to be largely favourable in the general population, evidence in special cohorts initially excluded from the pivotal trials, such as pregnant and breastfeeding women, children/adolescents, immunocompromised people and persons with a history of allergy or previous SARS-CoV-2 infection, is still limited. In this narrative review, we critically overview pre- and post-marketing evidence on the potential benefits and risks of marketed COVID-19 vaccines in the above-mentioned special cohorts. In addition, we summarise the recommendations of the scientific societies and regulatory agencies about COVID-19 primary prevention in the same vaccinee categories. Abstract

Different natural and anthropogenic hazards can affect critical infrastructures (CI) causing physical damages to infrastructure assets, and the possible interruption of the services provided by these infrastructures. Since CI are recognized, by definition, as vital for the society (Kröger and Zio, 2011), their interruption can lead to serious and heavy socio-economic consequences. CI are complex systems with multiple assets that contribute to the ability to provide a service. These assets are interconnected between each other, so that an impact on an infrastructure can propagate to other interconnected. The processes that can affect CI are very different in nature, magnitude and frequency. The European Project Threvi2 tackles these problems by defining a comprehensive ontology for the infrastructures systems, the interconnections among infrastructures and the hazards that can affect these infrastructures. An important component of this ontology is the vulnerability model, which allows to connect the hazard ontology with the infrastructure ontology by assessing if, how and to what extent a specific hazard can affect an infrastructure system.

Abstract

The paper is focused on a phenomenon – the so called “back-shoring” (often indicated also as “on-shoring”, “in-shoring”, “re-shoring”, “reverse-shoring”, “international re-concentration”, “reverse-globalization”, even if they are not always considered synonymous) – which is becoming more and more diffused in the last years. Notwithstanding this increasing diffusion, it is still rarely investigated by academics and often discussed only in economic newspapers and white papers by management consulting firms. Manufacturing “back-shoring” is the process by which previously off-shored production are moved back to the domestic location (Kinkel and Maloca, 2009; Leibl, Morefield and Pfeiffer, 2011) an earlier off-shored or off-shore outsourced manufacturing activity is relocated in the firm’s home country. Based on a comprehensive literature review and evidences regarding 87 western companies, the objectives of this paper are: a) to analyze the relevance of such a managerial phenomenon; b) to identify and classify its motivations; c) to highlight some research questions.

Abstract

Different approaches are being developed to improve the differentiation of Leishmania genus using biochemical and molecular methods. In this study, 11 independent polymorphic microsatellites were used for the typing of strains of L. infantum isolated in Sicily. Polymerase chain reaction was employed to amplify the microsatellites contained in 12 DNA regions selected from among more investigated loci. A total of 51 isolates of L. infantum from dogs were tested by using the same locus panel. The products were successively analysed using an automatic sequence detector (ABI PRISM 3130 AB), to discover relevant microsatellite polymorphisms. It was possible to discriminate between MON-1 and non-MON-1 groups. Moreover, the method permitted to distinguish various genotypes of L. infantum isolates within each zymodema. Model- and distance-based analyses of the data set showed comparable results. The frequency of heterozygosity in the alleles analysed varied extremely between the different groups of isolates. As the method exhibits a high level of discrimination, it is suitable for characterization of closely related strains in epidemiological studies.

However, the communication of risk is a complex activity involving many different types of communicators and receivers from scientists, to the media, to government agencies, industry and consumer groups, each of which having its own agendas to fulfil. Such variation across the communication of the same risk can lead to confusion, misunderstandings, misreporting in the media and subsequent conflicts. The goal of risk communication is to produce an informed public. The personal nature of risk issues and the uncertainty associated with estimating risk can provoke considerable anxiety for the public and make adopted risk management strategies less acceptable. Lombardy is one of the most densely populated and industrialized regions in Europe and nearly 280 Seveso sites interest it. The Seveso Directive requires specific measures on risk communication to the population. Nevertheless, the Lombardy Region Authorities consider that the implementation of such provisions is too weak. Therefore an exploratory research has been concluded, in order to estimate the gaps in risk communication and to improve the participation of the population in the emergency preparedness activities. The paper reports the main results of the research and illustrates the potential strategies to improve the risk communication and the population participation and preparedness (ÉupolisLombardia, 2011).

Abstract

Different natural and anthropogenic hazards can affect critical infrastructures (CI) causing physical damages to infrastructure assets, and the possible interruption of the services provided by these infrastructures. Since CI are recognized, by definition, as vital for the society (Kröger and Zio, 2011), their interruption can lead to serious and heavy socio-economic consequences. CI are complex systems with multiple assets that contribute to the ability to provide a service. These assets are interconnected between each other, so that an impact on an infrastructure can propagate to other interconnected. The processes that can affect CI are very different in nature, magnitude and frequency. The European Project Threvi2 tackles these problems by defining a comprehensive ontology for the infrastructures systems, the interconnections among infrastructures and the hazards that can affect these infrastructures. An important component of this ontology is the vulnerability model, which allows to connect the hazard ontology with the infrastructure ontology by assessing if, how and to what extent a specific hazard can affect an infrastructure system.
