Redox imbalance is reported to play a pivotal role in tumorigenesis, cancer development, and drug resistance. Severe oxidative damage is a general consequence of cancer cell responses to treatment and may cause cancer cell death or severe adverse effects. To maintain their longevity, cancer cells can rescue redox balance and enter a state of resistance to anticancer drugs. Therefore, targeting redox signalling pathways has emerged as an attractive and prospective strategy for enhancing the efficacy of anticancer drugs and decreasing their adverse effects. Over the past few decades, natural products (NPs) have become an invaluable source for developing new anticancer drugs due to their high efficacy and low toxicity. Increasing evidence has demonstrated that many NPs exhibit remarkable antitumour effects, whether used alone or as adjuvants, and are emerging as effective approaches to enhance sensitivity and decrease the adverse effects of conventional cancer therapies by regulating redox balance. Among them are several novel anticancer drugs based on NPs that have entered clinical trials. In this review, we summarize the synergistic anticancer effects and related redox mechanisms of the combination of NPs with conventional anticancer drugs. We believe that NPs targeting redox regulation will represent promising novel candidates and provide prospects for cancer treatment in the future. NPs can selectively act as pro-oxidants, inducing cytotoxicity in cancer cells, while also serving as antioxidants and protectors in normal cells for detoxification. NPs play a dual role in increasing the sensitivity and reversing the drug resistance of cancer cells, primarily through pro-oxidant action and, in some cases, through antioxidant action. The heterogeneities in cellular and molecular biology provide the potential basis for the diverse regulation of NPs on the redox state. Background: Management of patients with migraine who have concomitant medication overuse (MO) or medication overuse headache (MOH) is a major problem in clinical practice. Detoxification of acute analgesics before or during initiation of prophylactic therapy has long been recommended although this concept has recently been questioned. Additionally, relapse after detoxification is a common problem. This real-world study analyses the initial and sustained effectiveness of prophylactic migraine therapy with CGRP (receptor) antibodies without prior detoxification in patients with comorbid MO or MOH for up to one year. Methods: A retrospective real-world analysis was performed on 291 patients (episodic migraine (EM) with MO (EM-MO; n = 35), EM without MO (EM-noMO; n = 77), chronic migraine (CM) with MOH (CM-MOH; n = 109), CM without MOH (CM-noMOH; n = 70). All patients began treatment with either erenumab (n = 173), fremanezumab (n = 70) or galcanezumab (n = 48) without prior detoxification. Data were available for up to 12 months of treatment. Responder rates for monthly headache days (MHD), monthly migraine days (MMD) and monthly acute medication intake (AMD) were analysed. Results: All groups showed a significant reduction in MHD, MMD and AMD at the last observed time point compared to baseline. In patients with CM and MOH, 60.6% (66/109) no longer fulfilled the definition of MO or MOH and a further 13.8% (15/109) had only EM-MO. In the EM cohort, 89% (31/35) of MO patients lost their MO during therapy. MHD and AMD 30% responder rates were comparable for CM-MOH and CM-noMOH (MHD: CM-MOH: 56.0% vs. CM-noMOH: 41.4%, p = 0.058, AMD: CM-MOH: 66.1% vs. CM-noMOH: 52.9%, p = 0.077). MMD responder rate did not differ significantly (after Bonferroni adjustment) (CM-MOH: 62.4% vs. CM-noMOH: 47.1%, p = 0.045, α = 0.017). After successful initiation of therapy, 15.4% of the initial CM-MOH patients relapsed and met the criterion for CM-MOH at the end of follow-up. There were no antibody specific differences in response to therapy. Conclusions: Our data confirms the effectiveness of CGRP antibody treatment in migraine patients with additional MOH or MO in a real-world setting. Low relapse rates after initial successful therapy support an early start of CGRP antibody treatment in patients with MOH or MO. Trial registration: No registration, retrospective analysis. Background: Alterations in higher‐order social cognition are well documented in individuals with severe alcohol use disorder (SAUD). However, the basic mechanisms underpinning them are not well understood. This knowledge gap hampers the development of targeted therapeutic interventions. Here, we investigated whether individuals with SAUD show abnormalities in social episodic memory processes, which may represent relevant candidate mechanisms for alterations in social cognition. Methods: Recently detoxified patients with SAUD and matched healthy controls (HCs) completed two experimental tasks. We first used a Social Recognition Task in 40 SAUD patients and 40 HCs to measure the participants' ability to implicitly memorize the facial identity and emotion of novel interpersonal cues (i.e., dynamic facial expressions of anger and happiness). We then used a Social Memory Accessibility Task in 29 SAUD patients and 30 HCs) to measure participants' access to and fluency for already existing social memories by asking them to retrieve as many specific positive and negative interpersonal events as possible within equal time limits. Results: In the Social Recognition Task, we found that, compared to HCs, patients with SAUD had a globally lower recognition performance for the facial identities of novel social stimuli, but a preserved bias toward positive information. Conversely, in the social memory accessibility task, patients showed greater access to and fluency for negative interpersonal memories than controls (no group differences were observed for positive ones), resulting in a negative accessibility bias. Conclusions: This exploration of episodic social memory in individuals with SAUD showed (1) a preserved bias for the encoding of positive versus negative novel social information, and (2) greater access to negative than positive interpersonal memories. These results enhance our understanding of socio‐affective processing in individuals with SAUD and identify social memory alterations that may contribute to social cognition and interpersonal difficulties. Triple negative breast cancer (TNBC) is an aggressive subtype of breast cancer. Conventional treatment options for TNBC often have limited efficacy and significant side effects. In recent years, traditional Chinese medicine (TCM) has shown promising results in the treatment of TNBC. TCMs include herb combinations that have synergistic effects to regulate homeostasis in the body, reduce tumor resistance, and improve patient quality of life. At present, three main TCM methods are used to treat TNBC in the clinic: strengthening the body's resistance, dispelling phlegm, and removing cancer toxins. This paper reviews the theories and mechanisms of each in TNBC treatment. The method of strengthening the body's resistance emphasizes enhancing the body's original Qi to fight against pathogenic factors; the method of dispelling phlegm seeks to eliminate phlegm stagnation and alleviate the burden on affected organs; the method of removing cancer toxins focuses on detoxification and detumescence to remove the toxic elements associated with TNBC. Although these methods treat TNBC from different etiologies, they have achieved good therapeutic effects and represent an important academic approach: That is, to cure the disease with a comprehensive view of the body and restore the balance of Yin and Yang. Background: Homelessness is a traumatic experience, and can have a devastating effect on those experiencing it. People who are homeless often face significant barriers when accessing public services, and have often experienced adverse childhood events, extreme social disadvantage, physical, emotional and sexual abuse, neglect, low self‐esteem, poor physical and mental health, and much lower life expectancy compared to the general population. Rates of problematic substance use are disproportionately high, with many using drugs and alcohol to deal with the stress of living on the street, to keep warm, or to block out memories of previous abuse or trauma. Substance dependency can also create barriers to successful transition to stable housing. Objectives: To understand the effectiveness of different substance use interventions for adults experiencing homelessness. Search Methods: The primary source of studies for was the 4th edition of the Homelessness Effectiveness Studies Evidence and Gaps Maps (EGM). Searches for the EGM were completed in September 2021. Other potential studies were identified through a call for grey evidence, hand‐searching key journals, and unpacking relevant systematic reviews. Selection Criteria: Eligible studies were impact evaluations that involved some comparison group. We included studies that tested the effectiveness of substance use interventions, and measured substance use outcomes, for adults experiencing homelessness in high income countries. Data Collection and Analysis: Descriptive characteristics and statistical information in included studies were coded and checked by at least two members of the review team. Studies selected for the review were assessed for confidence in the findings. Standardised effect sizes were calculated and, if a study did not provide sufficient raw data for the calculation of an effect size, author(s) were contacted to obtain these data. We used random‐effects meta‐analysis and robust‐variance estimation procedures to synthesise effect sizes. If a study included multiple effects, we carried out a critical assessment to determine (even if only theoretically) whether the effects are likely to be dependent. Where dependent effects were identified, we used robust variance estimation to determine whether we can account for these. Where effect sizes were converted from a binary to continuous measure (or vice versa), we undertook a sensitivity analysis by running an additional analysis with these studies omitted. We also assessed the sensitivity of results to inclusion of non‐randomised studies and studies classified as low confidence in findings. All included an assessment of statistical heterogeneity. Finally, we undertook analysis to assess whether publication bias was likely to be a factor in our findings. For those studies that we were unable to include in meta‐analysis, we have provided a narrative synthesis of the study and its findings. Main Results: We included 48 individual papers covering 34 unique studies. The studies covered 15, 255 participants, with all but one of the studies being from the United States and Canada. Most papers were rated as low confidence (n = 25, or 52%). By far the most common reason for studies being rated as low confidence was high rates of attrition and/or differential attrition of study participants, that fell below the What Works Clearinghouse liberal attrition standard. Eleven of the included studies were rated as medium confidence and 12 studies as high confidence. The interventions included in our analysis were more effective in reducing substance use than treatment as usual, with an overall effect size of –0.11 SD (95% confidence interval [CI], −0.27, 0.05). There was substantial heterogeneity across studies, and the results were sensitive to the removal of low confidence studies (−0.21 SD, 95% CI [−0.59, 0.17] − 6 studies, 17 effect sizes), the removal of quasi‐experimental studies (−0.14 SD, 95% CI [−0.30, 0.02] − 14 studies, 41 effect sizes) and the removal of studies where an effect size had been converted from a binary to a continuous outcome (−0.08 SD, 95% CI [−0.31, 0.15] − 10 studies, 31 effect sizes). This suggests that the findings are sensitive to the inclusion of lower quality studies, although unusually the average effect increases when we removed low confidence studies. The average effect for abstinence‐based interventions compared to treatment‐as‐usual (TAU) service provision was –0.28 SD (95% CI, −0.65, 0.09) (6 studies, 15 effect sizes), and for harm reduction interventions compared to a TAU service provision is close to 0 at 0.03 SD (95% CI, −0.08, 0.14) (9 studies, 30 effect sizes). The confidence intervals for both estimates are wide and crossing zero. For both, the comparison groups are primarily abstinence‐based, with the exception of two studies where the comparison group condition was unclear. We found that both Assertative Community Treatment and Intensive Case Management were no better than treatment as usual, with average effect on substance use of 0.03 SD, 95% CI [−0.07, 0.13] and –0.47 SD, 95% CI [−0.72, −0.21] 0.05 SD, 95% CI [−0.28, 0.39] respectively. These findings are consistent with wider research, and it is important to note that we only examined the effect on substance use outcomes (these interventions can be effective in terms of other outcomes). We found that CM interventions can be effective in reducing substance use compared to treatment as usual, with an average effect of –0.47 SD, 95% CI (−0.72, −0.21). All of these results need to be considered in light of the quality of the underlying evidence. There were six further interventions where we undertook narrative synthesis. These syntheses suggest that Group Work, Harm Reduction Psychotherapy, and Therapeutic Communities are effective in reducing substance use, with mixed results found for Motivational Interviewing and Talking Therapies (including Cognitive Behavioural Therapy). The narrative synthesis suggested that Residential Rehabilitation was no better than treatment as usual in terms of reducing substance use for our population of interest. Authors' Conclusions: Although our analysis of harm reduction versus treatment as usual, abstinence versus treatment as usual, and harm reduction versus abstinence suggests that these different approaches make little real difference to the outcomes achieved in comparison to treatment as usual. The findings suggest that some individual interventions are more effective than others. Background: Studies on early abstinence suggest that cognitive function is significantly reduced in the first year of abstinence, which raises the question of whether it is relevant to early relapse in patients with substance use disorders. This study investigates the extent to which impairments in executive function and memory predict alcohol relapse in patients with alcohol use disorder (AUD). Understanding these relationships is crucial for improving therapeutic approaches to prevent relapse in patients with AUD. Methods: We selected 116 adult patients (79 male and 37 female) diagnosed with AUD based on DSM‐5 criteria, all of whom were undergoing alcohol detoxification treatment. A comprehensive array of neuropsychological tests was administered to assess global cognition, memory, and executive functions. Patients' alcohol use was monitored monthly during a 6‐month follow‐up period. Logistic regression and Cox regression were used to explore the relationship between cognitive function and the likelihood of alcohol relapse. Results: Impairments in global cognition, semantic and phonemic fluency, cognitive flexibility, and learning ability during detoxification were significant predictors of relapse in AUD patients, showing similar predictive values at both 3 and 6 months post‐treatment. An abnormal Montreal Cognitive Assessment (MoCA) score increased the risk of relapse by 123% (HR: 2.227), and impairments in both semantic and phonemic fluency each increased the risk by 142% (HR: 2.423). Additionally, abnormal performance on the MoCA, Trail Making Test Part B (TMT‐B), and California Verbal Learning Test (CVLT) was associated with a higher number of drinking days at 3 months (IRR: 3.764; IRR: 2.237; IRR: 2.738, respectively) and abnormal MoCA and TMT‐B scores at 6 months (IRR: 2.451; IRR: 1.859, respectively). Conclusions: The MoCA test is a valuable tool for predicting relapse risk in AUD patients undergoing detoxification treatment, with similar predictive value for relapse at 3 or 6 months. Learning ability needs to be assessed and their impairments considered in the treatment of AUD patients. Environmental pollution and exposure to toxic metals such as cadmium (Cd) can cause severe and chronic diseases and have significant side effects on vital organs. The present study aimed to evaluate the effect of pomegranate peel on biochemical factors and lipid peroxidation in intoxication by Cd in Japanese quail. Two hundred seventy quails in different groups were fed diets containing Cd and pomegranate peel from 6 to 35 days old. Then, serum biochemical parameters were assessed, including liver enzymes, urea, and thiobarbituric acid. In the quails, Cd significantly increased MDA, urea, and AST (P < 0.05). Adding pomegranate peel at 1.5 and 2% levels decreased these parameters significantly (P < 0.05). In conclusion, dietary enrichment using pomegranate peel reduced the adverse effects of Cd by improving lipid peroxidation, aspartate aminotransferase (AST), and urea in Japanese quail. Objective Maternal smoking is associated with as much as a 50% reduced risk of preeclampsia, despite increasing risk of other poor pregnancy outcomes that often co-occur with preeclampsia, such as preterm birth and fetal growth restriction. Researchers have long sought to understand whether this perplexing association is biologically based, or a result of noncausal mechanisms. We examined whether smoking-response genes modify the smoking-preeclampsia association to investigate potential biological explanations. Study Design We conducted a nested case–control study within the Norwegian Mother, Father and Child Birth Cohort (1999–2008) of 2,596 mother–child dyads. We used family-based log-linear Poisson regression to examine modification of the maternal smoking-preeclampsia relationship by maternal and fetal single nucleotide polymorphisms involved in cellular processes related to components of cigarette smoke (n = 1,915 with minor allele frequency ≥10%). We further investigated the influence of smoking cessation during pregnancy. Results Three polymorphisms showed overall (p < 0.001) multiplicative interaction between smoking and maternal genotype. For rs3765692 (TP73) and rs10770343 (PIK3C2G), protection associated with smoking was reduced with two maternal copies of the risk allele and was stronger in continuers than quitters (interaction p = 0.02 for both loci, based on testing 3-level smoking by 3-level genotype). For rs2278361 (APAF1) the inverse smoking-preeclampsia association was eliminated by the presence of a single risk allele, and again the trend was stronger in continuers than in quitters (interaction p = 0.01). Conclusion Evidence for gene–smoking interaction was limited, but differences by smoking cessation warrant further investigation. We demonstrate the potential utility of expanded dyad methods and gene–environment interaction analyses for outcomes with complex relationships between maternal and fetal genotypes and exposures. Objective: This research aimed to assess the effectiveness of combination therapy with amantadine and clonidine against clonidine alone on subjective opioid withdrawal symptoms in patients treated with buprenorphine. Methods and Materials: Patients were recruited from outpatient addiction treatment centers for this double-blind, randomized control trial. They were divided into two groups randomly assigned to receive either clonidine (0.15–0.3 mg/day) or clonidine with amantadine (100–200 mg/day). We assessed withdrawal symptoms with the Subjective Opiate Withdrawal Scale (SOWS) on the first day of admission and 3, 7, 14, and 21 days afterward. Results: Forty-three patients completed the trial in each group. The impact of adding-on amantadine on the mean SOWS compared to control was significant (p < .001) at any time of the study period. However, there were no significant differences in mean SOWS within groups during follow-up (p > .05). Also, there were no significant differences in mean SOWS between the two groups during follow-up time (day 3rd, 7th, 14th, and 21st) (p > .05). Conclusion: Amantadine and clonidine combined treatment would result in fewer opioid withdrawal symptoms than clonidine alone, with a decrease commencing on the third day. [ABSTRACT FROM AUTHOR]

Background: Music therapy can positively impact craving, treatment readiness, and motivation in adults with substance use disorder (SUD) on a detoxification unit. However, the existing research is primarily comprised of studies with a single pre- or posttest and there is a need for randomized controlled studies that compare within-session changes resultant of various music therapy interventions to determine best practice. Objective: The purpose of this single-session study was to compare within-session changes between group motivational and educational songwriting (MESW) and group recreational music therapy (RMT) on craving and commitment to sobriety in adults with SUD on a detoxification unit. Method: Participants (N = 100) were cluster-randomized to group MESW or group RMT conditions and completed established psychometric instruments measuring craving and commitment to sobriety at pre- and posttest. Results: Within-group changes were significant in all measures, indicating that music therapy was effective within the temporal parameters of a single session. Although between-group differences were not significant, mean within-session improvements in expectancy, compulsivity, emotionality, total craving, and commitment to sobriety were larger in the MESW condition than the RMT condition. Additionally, the MESW group tended to have slightly more favorable posttest scores than the RMT group in all measures. Conclusion: Despite the temporal limitations of single-session therapy common on detoxification units, both MESW and RMT protocols resulted in significant within-session changes in measures of craving and commitment to sobriety. The main objective of this study is to check the efficacy of the combinational chemotherapy regimen and Qing Gong Detoxification Soup on the epithelial-mesenchymal transition gene expression, immune homeostasis mechanism, and tumor markers in patients with cervical cancer. In the present study, 88 individuals with cervical cancer were selected as experimental and control groups, 44 cases each, using the random number table method. The clinical efficacy, expression of epithelial-mesenchymal transition genes (Vimentin, N-cadherin, E-cadherin, α-catenin), Th1/Th2 (T helper cell 1/T helper cell 2), cytokines (IFN-γ, IL-4, IL-2, IL-6), tumor markers (CEA, SCCA, CA125), TCM (Traditional Chinese Medicine) symptom score and adverse reactions were investigated throughout the entire study. Our results clearly indicated the significant effectiveness of our combinational therapy in the treatment of cancer by 84.09% in comparison to the control group (63.64%). No significant differences in the primary and secondary symptom scoresÍ? i.e., IFN-γ (Interferon-gamma), IL-2 (Interleukin-2), IL-4 (Interleukin-4), IL-6 (Interleukin-6), CEA (carcinoembryonic antigen), SCCA (squamous cell carcinoma antigen), and expression of several epithelialmesenchymal transition genes, IFN-γ, as well as IL-2 between experimental and control groups were observed before starting the therapy. IFN-γ, IL-2, IL-4, IL-6, CEA, SCCA and CA125 (cancer antigen 125) scores were significantly improved after treatment in both groups, although the study group outperformed the control group and the differences were statistically significant (p < 0.05). We also found that the incidence of adverse reactions was 9.09% in the experimental group which was lower than 25.00% in the control group, and the differences were statistically significant (p < 0.05). This study confirmed that combinational therapy of Qing Gong Detoxification Soup and TP (paclitaxel-cisplatin) chemotherapy significantly enhanced the expression of the epithelial-mesenchymal transformation gene, repaired immune homeostasis, inhibited tumor markers, and reduced the occurrence of adverse reactions. Background • Appendicitis is a common acute abdominal disease. Traditional Chinese medicine believes that acute appendicitis is caused by the accumulation of heat and toxin, and the formation of carbuncle and pus in the colon due to stasis. Therefore, treatment should be carried out to clear heat and detoxify, clear the organs, and eliminate carbuncle. Dahuang Mudan Tang contains various traditional Chinese medicines for clearing heat and detoxifying, which can be used to treat appendicitis. This study observes the therapeutic effect of Dahuang Mudan Tang on patients undergoing laparoscopic surgery for acute appendicitis. Methods • Eight databases were searched by computer and inclusion criteria were pre determined before evaluation: (1) patients with appendicitis; (2) 18-70 years old; (3) Agree to this study and obtain randomized controlled trials at home and abroad on the combined treatment of appendicitis with caesarean section and rhubarb peony testing. Using RevMan 5.3 software, conduct a comprehensive evaluation of the cultivation quality and conduct data analysis. Results • The meta-analysis ultimately included 16 papers. They are all considered randomized controlled trials. The overall efficiency of the test unit and control unit was reported in 12 surveys. The total effective rate of the experimental group was significantly higher than that of the control group (Odds Ratio (OR): (1.16; 95% Cl: 1.11,1.20; P < .001), and the duration of bowel sounds was also significantly higher than that of the control group. Standardized mean deviation (SMD): (-7.39; 95% Cl: -8.48, -6.30; P < .01), defecation time SMD: (-1.60; 95% Cl: -2.07, -1.12; P < .01). Conclusion • Based on the total effective rate, defecation time, defecation time, CRP, IL-6, and adverse reactions of participants in this study, the combination of Dahuang Mudan Tang and laparoscopy in the treatment of appendicitis may be beneficial, which can improve clinical efficacy, inhibit inflammatory reactions, and promote postoperative recovery of patients. It is worth promoting and applying in clinical practice. However, these findings still require more high-quality research to confirm. Background: Traditional Polynesian medicine for children has been poorly documented, and few data are available on their efficacy and safety. In this context, the aim of this study was to identify traditional practices used for treating children and then assess the efficacy and safety of the most cited remedies by reviewing the literature. Methods: In 2022, a semi-structured survey was carried out on five islands from the Society archipelago (Bora Bora, Huahine, Moorea, Raiatea, and Tahiti). A total of 86 participants were interviewed including 19 experts in herbalism. A thorough literature review was performed on the most cited plant species to gather the relevant ethnobotanical, pharmacological, and clinical data of each remedy. Results: Participants mentioned using 469 remedies to treat 69 health disorders. The most represented health categories were digestive system, skin disorders, infectious diseases, and respiratory system. A total of 67 plant species (representing 731 use-reports) were mentioned and Annona muricata, Gardenia taitensis, and Hibiscus rosa-sinensis were the main plants reported. Regarding the safety of cited remedies, one plant (Microsorum grossum) showed high risk of toxicity, and its use should be avoided in infants and children. Conclusion: Our survey confirms the importance of traditional medical practices for children in the Society Islands. A lack of data in children for most cited remedies demonstrate the need for more pharmacological and toxicological research on Polynesian medicinal plants. Individuals with alcohol dependence often experience cognitive dysfunction after detoxification, which can persist even after sustained abstinence. Transcranial direct current stimulation (tDCS) has shown promise in improving cognitive functioning in these patients. The main objective of this research was to assess the impact of ten sessions of transcranial direct-current stimulation (tDCS) targeting the dorsolateral prefrontal cortex (DLPFC) on the neuropsychological functioning of hospitalized individuals with alcohol dependence who have undergone detoxification. This single-center, parallel, and single-blind randomized controlled trial assessed the effects of ten sessions of add-on tDCS treatment on neuropsychological functions. 44 recently detoxified alcohol-dependent inpatients were randomly assigned to either treatment as usual (TAU) plus ten sessions of active tDCS over left DLPFC, or TAU plus ten sessions of sham tDCS and assessed the effect of tDCS sessions and time on neuropsychological functions. Neuropsychological functions were evaluated in inpatients undergoing treatment for alcohol dependence at T1 (9.95 + 2.42 days abstinence) and T2 (19 ± 1.48 days abstinence), and while time had a significant impact on mean scores of various neuropsychological tests, neither active tDCS treatment nor its interaction with time displayed a significant effect on neuropsychological functioning, as revealed by two-way ANOVAs with repeated measurements. In detoxified inpatients with alcohol dependence, tDCS treatment was not found to have a significant impact on neuropsychological functioning when compared to sham treatment, whereas abstinence from alcohol for approximately three weeks resulted in significant improvements in various cognitive domains including mental speed, verbal learning & memory, visual memory, verbal fluency, and response inhibition. Drug overdoses are the leading cause of injury death in the United States with an estimated 105,752 individuals dying from an overdose in the United States in a 12-month period ending October 2021. Given that people who have opioid use disorder (OUD) are at an increased risk of death, it is crucial to assess risk factors associated with opioid overdose to improve interventions. We examine factors associated with non-fatal overdose among a suburban/exurban population with OUD in Southern California. Participants were recruited by convenience sampling (n = 355) and were interviewed between November 2017 to August 2018. Participants were eligible for the study if they had a history of pharmaceutical opioid use. A total of 198 (55.8%) participants reported at least one overdose in their lifetime. A total of 229 participants identified as male, 124 identified as female, and 2 identified as non-binary. When controlling for demographic factors, non-oral opioid administration at first opioid use (AOR 2.82, 95% CI 1.52-5.22), having a history of methadone detoxification, (AOR 2.23, 95% CI 1.27-3.91), history of buprenorphine detoxification (AOR 1.77, 95% CI 1.02-3.07), and history of 12 step attendance (AOR 1.89, 95% CI 1.12-3.20) were found to be independently and positively associated with lifetime opioid overdose. Detoxification with buprenorphine and methadone was found to be associated with having a non-fatal opioid overdose. Buprenorphine and methadone should not be prescribed as a detoxification medication as long-term use of medication for OUD results in better outcomes than medication that is used short-term. Aedes albopictus (Skuse, 1895) (Diptera: Cullicidae) is an aggressive and invasive mosquito species. It has been shown that Ae. albopictus, which can be a vector in spreading some diseases such as zika, dengue, and chikungunya, develops resistance to some insecticides. This study investigated enzyme level alterations, considered among the physiological resistance mechanisms. Two groups which are Ae. albopictus samples that were collected from the city of Aydın in Turkey (study group) and insecticide-free and laboratory-grown Ae. albopictus samples (control group) were the subjects of the study. The acetylcholinesterase (AChE), esterase, oxidase, and glutathione-s-transferase (GST) enzyme levels and specific activity assays were carried out in both groups. All enzyme analyses were conducted according to World Health Organization’s protocol, and protein determination was done by the Bradford method. In conclusion, the two groups had significant differences in all enzyme levels. This study, which revealed such enzyme differences in insecticide-resistant Aedes albopictus, might guide future genetic studies that will explain these enzyme differences and new studies for selecting insecticides that will not harm the environment and humans. As the accurate and sensitive measurement of withdrawal constitutes a crucial component of detoxification, there is a need to apply a recovery-oriented lens and critical review to substance withdrawal tools. Adhering to PRISMA guidelines and applying a recovery-oriented lens, I conducted a critical interpretive synthesis on articles that used substance withdrawal tools published in the Journal of Substance Use. I used a recovery-oriented lens to critically assess the strengths and limitations of the substance withdrawal tools. A total of 10 studies met eligibility criteria. These 10 studies used 13 different tools to measure withdrawal. Results of the analysis indicated that most tools were relatively brief, were substance-specific, did not contain subscales, and framed items in a negative manner. Perhaps most noteworthy, the tools were not created with input from service users. The diminutive number of studies measuring withdrawal is indicative of a consequential gap in the literature. While most withdrawal tools contained relatively few items, they should be substance-specific and contain subscales to assess multiple domains. There is a need for service user involvement in the design of withdrawal tools. Implications for clinical practice, limitations of the study, and suggestions for future research are provided. The field of nutrigenomics aims to explain how common nutrients, food additives and preservatives can change the body's genetic balance towards either health or sickness. This study reviews the effects of SNPs on detoxification, antioxidant capacity, and longevity. SNPs are mutations that only change one nucleotide at a specific site in the DNA. Specific SNPs have been associated to a variety of biological processes, including detoxification, antioxidant capacity, and longevity. This article mainly focuses on the following genes: SOD2, AS3MT, CYP1A2, and ADORA2A (detoxification); LEPR, TCF7L2, KCNJ11, AMY1, and UCP3 (antioxidant capacity); FOXO3 and BPIFB4 (longevity). This review underlines that many genes--among which FOXO3, TCF7L2, LEPR, CYP1A2, ADORA2A, and SOD2--have a unique effect on a person's health, susceptibility to disease, and general well-being. Due to their important roles in numerous biological processes and their implications for health, these genes have undergone intensive research. A feeding trial was conducted on commercial broilers for a period of 35 days to determine the individual effects of aflatoxin B1 (AFB1) and Fumonsinin B1 (FB1) toxins on performance, organ weights and immune status. The efficacy of dietary grape seed extract (GSE) and hydrated sodium calcium aluminosilicate (HSCAS) in preventing the adverse effects of AFB1 and FB1 toxin was also evaluated. Seven dietary treatments were tested on 420 commercial broiler chickens divided randomly into 42 replicates of 10 chicks each. the nutritional treatments: (1) control (non-contaminated diet); (2) non contaminated diet + 1mg/kg of AFB1; (3) non-contaminated diet + 400 mg/kg FB1 ; (4) 1 mg/kg AFB1 + 500 mg/kg GSE; (5) 1 mg/kg AFB1 + 5 g/kg HSCAS; (6) 400 mg/ kg FB1 + 500 mg/kg GSE; and (7) 400 mg/kg FB1 + 5 g/kg HSCAS. Feed intake and body weight gain were recorded weekly. On the 35th day, organ morphology and antibody titers for infectious bursal disease (IBD) and Newcastle disease (ND) were assessed. Individually, AFB1 and FB1 reduced weight gain while raising the feed conversion ratio (FCR) and mortality rate (p ≤ 0.05). AFB1 alone (p ≤ 0.05) raised the weights of the liver, kidney, gizzard, and spleen while decreasing the weights of the thymus and bursa. FB1 increased liver and gizzard weights and decreased thymus weight. Individual doses of AFB1 and FB1 substantially impacted NDV and IBD titers. GSE supplementation increased weight gain, enhanced feed conversion efficiency, and restored organ weights (p ≤ 0.05). With the addition of GSE, antibody titers against NDV and IBD were dramatically increased. Adding HSCAS increased body weight gain and restored organ weights in birds fed AFB1 alone but not in birds given FB1 (p ≤ 0.05). FCR in groups fed toxins was unaffected by HSCAS addition. Adding HSCAS (p ≤ 0.05) increase antibody titers against NDV and IBD. In conclusion, mycotoxins cause adverse effects, especially aflatoxin B1, which is more severe than Fumonsinin B1. What is Known and Objective? In China, patients with drug-induced liver injury (DILI) are commonly treated with one or more types of hepatoprotective drugs, despite a lack of evidence. We performed this study to investigate the association between the treatment pattern of DILI, including withdrawal of suspected drugs and use of hepatoprotective drugs, and recovery following DILI. Methods. A retrospective study was conducted at a tertiary hospital in Central China. Data of patients with a diagnosis of DILI hospitalized between January 2015 and December 2020 were collected through the Electronic Medical Records System. We excluded cases that did not meet the biochemical criteria of DILI and had a Roussel Uclaf Causality Assessment Method score of less than 3. Univariate and multivariate logistic regression models were used to analyze the association between treatment patterns and clinical outcomes. Results and Discussion. In total, 699 patients were included. Suspected drugs were discontinued in 619 patients (88.6%). 693 patients (99.1%) were treated with hepatoprotective drugs, among whom only 14.7% patients received monotherapy with hepatoprotective drugs. Recovery following DILI was seen in 593 cases (84.8%). By multivariate analysis, the number of hepatoprotective drugs combined did not show significance (p = 0.363), while the withdrawal of suspected drugs was associated with recovery following DILI (p = 0.015). What is New and Conclusion. The withdrawal of suspected drugs is associated with the recovery following DILI, and hepatoprotective drug combinations do not contribute to better outcomes than monotherapy. The article, "Activated Charcoal and Poisoning: Is It Really Effective?" by Aksay et al. (2022), examines whether the usage of activated charcoal (AC) in current treatment protocols for ingested poisonings adds benefits, given recent controversies in its use. Study variables included clinical findings in relation to the drug being ingested, the frequency and usage of an antidote, the rate of being intubated, and the duration of being hospitalized comparing poisoned patients who received AC with those who did not. APRNs need to be aware of the current guidelines to help establish the appropriateness of use when administering AC and be able to evaluate patients during and after the administering of AC. Improved awareness and education regarding the different treatment modalities for toxicology patients such as AC can help with certain kinds of poisonings in the emergency department. The present study was conducted to estimate the association between measures of hope for the future and time to substance use relapse among people living with opioid dependence following their discharge from opioid detoxification programs. The data for this investigation were collected from a prospective cohort study of patients who completed detoxification in Ireland (N = 142). After baseline, participants completed follow-up visits at three, six and nine months. Cox regression models were used to analyze the association between measures of hope and time to relapse. Of the 142 participants included in this study, 43 (30.3%) were female and the mean duration of substance use was 14.3 years (standard deviation: 5.8). In the multivariable analysis, a five-unit increase in mean Trait Hope Scale scores was associated with a mean decrease of 23% in the Hazard Ratio (HR) of relapse following detoxification (HR = 0.77, 95% Confidence Interval [CI]: 0.63–0.94; p = 0.011). In the sub-analysis, the hope-agency subdomain was significantly associated with lower rates of relapse (HR = 0.59, 95% CI: 0.41–0.83; p = 0.009), while the hope-pathway subdomain was not significantly associated with relapse rates (HR = 0.74, 95% CI: 0.50–1.11; p = 0.146). In the present study, increased mean levels of hope were associated with slower rates of relapse among people living with opioid dependence following discharge from detoxification programs. These findings suggest that empowering people in recovery and providing additional support following services such as detoxification may be valuable strategies to reduce relapse rates among people living with opioid dependence. The present study aims to determine mental health conditions, service use determinants, and drug use behaviors associated with non-fatal overdose among people who use illicit drugs (PWUIDs). We searched for studies in English published before February 1, 2021, on PubMed, Scopus, Cochrane, and Web of Science to identify primary studies on the factors associated with non-fatal overdose among PWUIDs. After reviewing for study duplicates, the full-text of selected articles were assessed for eligibility using Population, Intervention, Comparator, and Outcomes (PICO) criteria. The present study applied OR to measure the effect size with 95% CI. For data analysis, R 3.5.1 with the "meta" package was used to conduct the meta-analysis. After a detailed assessment of more than 13,845 articles, a total of 60 studies met the eligibility criteria. We found that non-fatal overdose was independently and positively associated with various factors considered and was, as expected, most strongly associated with being male, needs help injecting, needle sharing, overdose experience in past 12 months, alcohol abuse, had mental health diagnosis, depression, and anxiety disorders, suicide (ideation or attempt), inpatient detoxification, benzodiazepine use, and emergency department visit or hospitalization. The findings of the current meta-analysis support the requirement to improve suitable harm reduction strategies for drug users, such as peer-based overdose management. Simple Summary: Cisplatin is recognized as the standard agent for head and neck squamous cell carcinoma therapy, despite the relevant risk of permanent hearing damage. The aim of this study was to evaluate the possible associations of the clinicopathological features and inherited genotypes encoding cisplatin metabolism in eighty-nine patients undergoing chemoradiation with the risk of hearing loss. We were able to confirm race, body mass index, and cumulative cisplatin dose as independent clinical risk factors. Patients with specific isolated and combined genotypes encoding cisplatin efflux (GSTM1, GSTP1 c.313A>G), DNA repair (XPC c.2815A>C, XPD c.934G>A, EXO1 c.1762G>A, MSH3 c.3133A>G), and apoptosis-related proteins (FASL c.-844A>T, P53 c.215G>C) presented up to 32.22 higher odds of moderate or severe ototoxicity. These findings reinforce the importance of inherited nucleotide variants involved in cisplatin metabolism as candidate variables for predictive models of adverse events. Background: Cisplatin (CDDP) is a major ototoxic chemotherapy agent for head and neck squamous cell carcinoma (HNSCC) treatment. Clinicopathological features and genotypes encode different stages of CDDP metabolism, as their coexistence may influence the prevalence and severity of hearing loss. Methods: HNSCC patients under CDDP chemoradiation were prospectively provided with baseline and post-treatment audiometry. Clinicopathological features and genetic variants encoding glutathione S-transferases (GSTT1, GSTM1, GSTP1), nucleotide excision repair (XPC, XPD, XPF, ERCC1), mismatch repair (MLH1, MSH2, MSH3, EXO1), and apoptosis (P53, CASP8, CASP9, CASP3, FAS, FASL)-related proteins were analyzed regarding ototoxicity. Results: Eighty-nine patients were included, with a cumulative CDDP dose of 260 mg/m2. Moderate/severe ototoxicity occurred in 26 (29%) patients, particularly related to hearing loss at frequencies over 3000 Hertz. Race, body-mass index, and cumulative CDDP were independent risk factors. Patients with specific isolated and combined genotypes of GSTM1, GSTP1 c.313A>G, XPC c.2815A>C, XPD c.934G>A, EXO1 c.1762G>A, MSH3 c.3133A>G, FASL c.-844A>T, and P53 c.215G>C SNVs had up to 32.22 higher odds of presenting moderate/severe ototoxicity. Conclusions: Our data present, for the first time, the association of combined inherited nucleotide variants involved in CDDP efflux, DNA repair, and apoptosis with ototoxicity, which could be potential predictors in future clinical and genomic models. Aims Alcohol-associated liver disease (ALD) is a global health problem caused, among other factors, by oxidative stress from the formation of reactive oxygen species (ROS). One important source of ROS is microsomal ethanol metabolism catalyzed by cytochrome P450 2E1 (CYP2E1), which is induced by chronic ethanol consumption. Inhibition of CYP2E1 by clomethiazole (CMZ) decreases oxidative stress in cell cultures and improves ALD in animal studies. Our study aimed to assess the benefits of a CYP2E1 inhibitor (clomethiazole) in detoxification of patients with ALD. Methods Open label, randomized controlled clinical trial to study whether CYP2E1 inhibition improves ALD in the patients with alcohol use disorders admitted for alcohol detoxification therapy (ADT). Patients had to have a serum aspartate aminotransferase (AST) activity exceeding twice the upper normal limit at time of admission and be non-cirrhotic defined by fibroscan value <12 kPa. Sixty patients were randomly assigned to ADT with either CMZ or clorazepate (CZP) for 7–10 days in a 1:1 ratio. The chlorzoxazone test of CYP2E1 activity was performed at enrolment and at 2 points during the study. Results ADT improved hepatic steatosis (controlled attenuation parameter) in both groups significantly. A trend towards a greater improvement in hepatic fat content during ADT (−21.5%) was observed in the CMZ group (252 ± 48 dB/m vs. 321 ± 38 dB/m; P  < 0.0001) compared with the CZP group (−13.9%; 273 ± 38 dB/m vs. 317 ± 39 dB/m; P  < 0.0001). As already reported, serum AST (P  < 0.004) and alanine aminotransferase (ALT) activities (P  < 0.0006) significantly decreased in CMZ patients as compared with patients on CZP by the end of hospitalization. A significant correlation was found between AST (P  = 0.023), ALT (P  = 0.009), GGT (P  = 0.039) and CAP. Conclusion This study demonstrates that CMZ improves clinical biomarkers for ALD in humans most likely due to its inhibitory effect on CYP2E1. Whether acute or chronic, liver failure is a state of liver dysfunction that can progress to multiorgan failure. Mortality in liver failure patients is approximately 80–90% and is caused by detoxification failure, which triggers other immediate complications, such as encephalopathy, coagulopathy, jaundice, cholestasis, and acute kidney failure. The ideal treatment for liver failure is liver transplantation, but the long waiting period for the right donor match causes unavoidable deaths in most patients. Therefore, new therapies, such as tissue engineering, hepatocyte transplantation, and stem cells, are now being studied to anticipate the patient's condition while waiting for liver transplantation. This literature review investigated the effectiveness of some bio-artificial liver support systems using review methods systematically from international publication sites, including PubMed, using keywords, such as bio-artificial liver, acute and chronic liver failure, extracorporeal liver support system (ECLS), MARS, single-pass albumin dialysis (SPAD). Artificial and bioartificial liver systems can show specific detoxification abilities and pathophysiological improvements in liver failure patients but cannot reach the ideal criteria for actual liver function. The liver support system must provide the metabolic and synthetic function as in the actual liver while reducing the pathophysiological changes in liver failure. Aspects of safety, cost efficiency, and practicality are also considered. Identifying the technology to produce high-quality hepatocytes on a big scale is essential as a medium to replace failing liver cells. Aims To assess recovery of alcohol-related neuropsychological deficits in a group of patients with pure severe alcohol use disorder (AUD) during a detoxification program using the Brief Evaluation of Alcohol-Related Neuropsychological Impairment (BEARNI) test. Methods Thirty-two patients with severe AUD using DSM-IV criteria (24 men, mean age = 45.5 ± 6.8 years old) were assessed using the BEARNI 8 ± 2 days after alcohol cessation (T1) and then were reassessed within 18 ± 2 days after alcohol cessation (T2). The primary study endpoint was the number of patients initially impaired at T1 who recovered cognitive functions at T2 assessment. Results At T1, 59% (n = 19) patients with pure severe AUD had at least one impaired cognitive function assessed by the BEARNI. At T2, 63% of the patients with AUD with deficits at T1 had normal BEARNI cognitive scores (χ2 = 7.7, P  = 0.005); specifically, the percentages of participants with normal subtest scores were 63% on memory (χ2 = 12.4, P  = 0.0004), 100% on verbal fluency (χ2 = 16; P  = <0.0001), 60% on alphabetical span (χ2 = 12.8; P  = 0.0003) and 67% on visuospatial (χ2 = 15, P  = 0.0001). Conclusions The cognitive impairments of two-thirds of patients with pure AUD included in the present study recovered within 18 days of abstinence, earlier than reported in previous studies. Consumers increasingly seek out the spiritual to enlighten their inner emptiness and find their inner selves. We add a physiological, embodied perspective, which has been commonly overlooked in extant research as a valuable opportunity for individual consumer spirituality. Interpretative investigation of body-transforming consumers uncovers a powerful reincarnation process that eventually leads to the self-renewal and reunion of body and mind. We find that the consumption of body-transforming substances initiates a mindful, spiritual consumer journey allowing consumers to actively develop and experience their inner spirituality through recurring cycles of reduction, reflection and release. These findings allow us to develop implications for a broader understanding of consumer spirituality where the active consumer seeks unity in the self and beyond. [ABSTRACT FROM AUTHOR]

The article discusses a presentation entitled "Mood Over Matter: Literature Review on Benzodiazepine Tapering, Current Practices and Updates on Adjunct Mood Stabilizers," during the 2024 American Psychiatric Association's Annual Meeting. The presentation focused on a literature review of benzodiazepine tapering practices, outpatient detoxification challenges, mood stabilizer adjunct use and potential barriers to discontinuation.

Introduction: Heat-clearing and detoxifying Chinese medicines have been documented to have anti-Alzheimer’s disease (AD) activities according to the accumulated clinical experience and pharmacological research results in recent decades. In this study, Fibraurea recisa Pierre (FRP), the classic type of Heat-clearing and detoxifying Chinese medicine, was selected as the object of research. Methods: 12 components with anti-AD activities were identified in FRP by a variety of methods, including silica gel column chromatography, multiple databases, and literature searches. Then, network pharmacology and molecular docking were adopted to systematically study the potential anti-AD mechanism of these compounds. Consequently, it was found that these 12 compounds could act on 235 anti-AD targets, of which AKT and other targets were the core targets. Meanwhile, among these 235 targets, 71 targets were identified to be significantly correlated with the pathology of amyloid beta (Ab) and Tau. Results and discussion: In view of the analysis results of the network of active ingredients and targets, it was observed that palmatine, berberine, and other alkaloids in FRP were the key active ingredients for the treatment of AD. Further, Kyoto encyclopedia of genes and genomes (KEGG) pathway enrichment analysis revealed that the neuroactive ligandreceptor interaction pathway and PI3K-Akt signaling pathway were the most significant signaling pathways for FRP to play an anti-AD role. Findings in our study suggest that multiple primary active ingredients in FRP can play a multitarget anti-AD effect by regulating key physiological processes such as neurotransmitter transmission and anti-inflammation. Besides, key ingredients such as palmatine and berberine in FRP are expected to be excellent leading compounds of multitarget anti-AD drugs. [ABSTRACT FROM AUTHOR]

Ranunculales, comprising of 7 families that are rich in medicinal species frequently utilized by traditional medicine and ethnomedicine, represents a treasure chest of biodiversity and chemodiversity. The phylogenetically related species often have similar chemical profile, which makes them often possess similar therapeutic spectrum. This has been validated by both ethnomedicinal experiences and pharmacological investigations. This paper summarizes molecular phylogeny, chemical constituents, and therapeutic applications of Ranunculales, i.e., a pharmacophylogeny study of this representative medicinal order. The phytochemistry/metabolome, ethnomedicine and bioactivity/pharmacology data are incorporated within the phylogenetic framework of Ranunculales. The most studied compounds of this order include benzylisoquinoline alkaloid, flavonoid, terpenoid, saponin and lignan, etc. Bisbenzylisoquinoline alkaloids are especially abundant in Berberidaceae and Menispermaceae. The most frequent ethnomedicinal uses are arthritis, heat-clearing and detoxification, carbuncle-abscess and sore-toxin. The most studied bioactivities are anticancer/cytotoxic, antimicrobial, and anti-inflammatory activities, etc. The pharmacophylogeny analysis, integrated with both traditional and modern medicinal uses, agrees with the molecular phylogeny based on chloroplast and nuclear DNA sequences, in which Ranunculales is divided into Ranunculaceae, Berberidaceae, Menispermaceae, Lardizabalaceae, Circaeasteraceae, Papaveraceae, and Eupteleaceae families. Chemical constituents and therapeutic efficacy of each taxonomic group are reviewed and the underlying connection between phylogeny, chemodiversity and clinical uses is revealed, which facilitate the conservation and sustainable utilization of Ranunculales pharmaceutical resources, as well as developing novel plant-based pharmacotherapy. Introduction: Herbs or plants are used for a variety of purposes, including nutrition, medicinal and in the beverage industry, beverages, coloring, food preservatives, insect repellants and cosmetics. Methods: Many plants were obtained from local markets and the oil was extracted with hexane at a concentration of 75%. Green silver nanoparticles were prepared. The nanoparticles were characterized using various techniques, including scanning electron microscopy, UV visible spectroscopy, Fourier transform infra-red, energy dispersive spectroscopy, and zeta potential analyzer. Results: The results showed small, smooth spherical nanoparticles, ranging between 57.41–88.00 nm, as well as the distribution of electric charges evenly on the surface of the nanoparticles, which acquired effective agents for nanoparticles against aflatoxin-b1. The effectiveness of green nanoparticles against aflatoxin-b1 by using high-performance liquid chromatography technology detected its concentration. The standard concentration of aflatoxin-b1 was (20) ppb. The results of the activity of plant oil extracts of T-thyme, rosemary-R, mint-M and eucalyptus-E reached (0.104, 1.586, 1.083 and 1.067) ppb, while it appears in the nanoparticle activity of T, R, M and E were as (0.065, 0.226, 0.377 and 0.702) ppb respectively. Conclusion: We concluded that green Ag nanoparticles are efficient in processing or eliminating aflatoxin-b1 and can be produced at very low concentrations compared to the concentrations of plant extracts prepared. The steady increase in the employment of silver nanoparticles (AgNPs) in consumer products entails the determination of the aquatic toxicity of AgNPs. Various AgNP characteristics including particle size, and shape, surface charge, and material have prominent effects on ecotoxicity. In the present study, we investigated the aquatic toxicity of chemically-synthesized AgNPs (Che-AgNPs) and green synthesis AgNPs (Gr-AgNPs) to Daphnia magna as a model organism. In each case, Che-AgNPs and Gr-AgNPs showed dose-dependent toxicity in the range of 5–50 ppb. It was also detected that the size and surface coverage material of AgNPs has a significant impact on the survival rate of D. magna. We also analyzed the expression of some genes related to detoxification and the reproductive system. These observations presented that in both NP types the significant alterations were detected in genes of the model organism in a dose-dependent manner. [ABSTRACT FROM AUTHOR]
