5. Factors Influencing Pneumococcal Vaccination-Induced Total IgG, IgG2, and IgA Levels in Vaccinated Splenectomized Patients

Abstract

Splenectomy is closely associated with a lifetime risk of pneumococcal and other encapsulated bacterial infections. In this study, it was aimed to investigate the change of antibody levels after vaccination against Streptococcus pneumoniae according to age, gender, years after splenectomy and the possible effect of splenectomy on IgG avidity. In addition the education and awareness levels of the participants about post-splenectomy vaccination and infectious diseases were also analyzed. In the first of the three phases of this study, 32 individuals with splenectomy were enrolled. The awareness of the patients about the possible risks after splenectomy was investigated with a simple questionnaire. Routine laboratory test results were obtained and clinical examinations were performed. In the second stage, total Ig values of 29 splenectomy patients were determined. In the third phase, 14 splenectomy and 5 healthy volunteers were vaccinated according to the Vaccination Practices Advisory Committee (ACIP) guidelines. Pneumococcal-specific antibody levels and IgG avidity were detected by enzyme linked immunosorbent assay (ELISA). It was determined that 68.8% of the splenectomized patients were unaware of their vaccination status and 78.2% of them were unaware of the increased risk of infectious diseases in asplenic conditions. . According to the hospital information management system, all 31 (96.87%) patients, except one, were vaccinated with PPV23. As expected, vaccinated patients exhibited high levels of vaccine-specific antibody production with IgG, IgG2, and IgA antibody concentrations of 321 ± 76.68 mg/l, 73.07 ± 8.273 mg/l, and 117.8 ± 14.94 mg/l, respectively, but unvaccinated patients had very low antibody (IgG, IgG2 and IgA antibody concentrations were 11.5 mg/l, 1.3 mg/l and 1.2 mg/l, respectively) levels. Abstract

Objectives: The use of central venous catheters as hemodialysis (HD) vascular access is associated with worsemorbidity and mortality in HD patients. This occasion is often attributed to comorbidities of the patients withcentral venous catheters. Studies reveal that a biofilm layer occurs on most of the tunneled-cuffed centralvenous catheters (TC-CVCs). This study aimed to determine the oxidative stress (OS) and systemicinflammation (SI) status in patients with TC-CVCs as HD vascular access without clinical signs and symptomsof infection.Methods: The study is composed of eighty-five patients with a minimum HD vintage of one year. Patientswith a history of infection or a cardiovascular event within six months, malignancy, systemic inflammatorydiseases, or malnutrition were excluded. OS indices and SI markers were studied and compared in patientswith arteriovenous fistula (AVF) and TC-CVCs.Results: Mean native thiol/total thiol (NT/TT) ratio was significantly higher and mean disulphide/total thiol(DT/TT) ratio was significantly lower in AVF group comparing TC-CVC group (0.46 ± 0.17 and 0.36 ± 0.17,p = 0.03 for NT/TT; 0.27 ± 0.08 and 0.31 ± 0.08, p = 0.04 for DS/TT; respectively). Mean OS index wassignificantly lower in the AVF group comparing TC-CVC group (0.15 ± 0.14 and 0.24 ± 0.23, p = 0.04;respectively]. Median hs-CRP levels and median IL-6 levels were significantly lower in AVF group comparingTC-CVC group (5.8 [min: 3.0-max: 82.5] mg/L and 9.7 [min: 3.0-max: 45.4] mg/L, p = 0.004 for hs-CRP; 6.2[min: 2.0-max:159.0] pg/mL and 12.2 [min: 2.6-max: 41.3) pg/mL, p = 0.01 for IL-6; respectively).Conclusions: TC-CVCs inversely affect OS and systemic inflammatory status in HD patients, presumablydue to foreign body reactions and biofilm layers.Keywords: Arteriovenous fistula, central venous catheters, hemodialysis vascular access, oxidative stress,systemic inflammation&nbsp

Abstract

Objective: Physiological and psychological conditions can cause female sexual dysfunction (FSD). It is called vaginismus when sexual dysfunction occurs with vaginal cramps and spasms. The aim of our research is to present the research topics and trends about FSD and vaginismus to the reader in a global contextMaterial and Methods: We used the terms ‘female sexual dysfunction’ and ‘vaginismus’ as keywords and we determined the articles indexed in the Web of Science database. We included all the articles in the database between 1980 and 2019. Documents of 2020 were excluded from this study. Gunnmap free open web-based application was used to visualize global research productivity. Vos-viewer 2019 program was used to determine the scientific relevance of the data.Results: We reached 2212 documents in total, 95.6 % of these articles were written in English, and 64.4% of the documents were original articles. Articles about vaginismus and FSD increased in the 1995s, the most productive year was 2019. The most productive country in genital cosmetics and aesthetics was the United States (USA), the most productive university was The University of British Columbia. The most productive writer was Goldstein I.Conclusion: Studies on vaginismus and FSD have increased over the past decade, and and non-invasive psychiatric interventions appear to be popular. Araştırmamızın amacı, CİB ve vajinismus ile ilgili araştırma konularını ve eğilimlerini okuyucuya küresel bir bağlamda sunmaktır.Gereç ve Yöntemler: "Kadın cinsel işlev bozukluğu" ve "vajinismus" terimlerini anahtar kelime olarak kullandık ve Web of Science veritabanında dizine eklenen makaleleri belirledik. Veri tabanında 1980-2019 yılları arasında yayınlanmış tüm makaleleri dahil ettik. 2020 tarihli belgeler bu çalışmadan çıkarıldı. Gunnmap ücretsiz açık web tabanlı uygulama, küresel araştırma verimliliğini görselleştirmek için kullanıldı. Verilerin bilimsel uygunluğunu belirlemek için Vos-viewer 2019 programı kullanılmıştır.Bulgular: Toplam 2212 belgeye ulaştık, bu makalelerin% 95,6'sı İngilizce yazılmıştır ve belgelerin % 64,4'ü orijinal makalelerdir. 25. Relationship between Serum Adipocyte Fatty AcidBinding Protein Levels and Systemic Inflammation in Hemodialysis Patients.

Abstract

Objective: Adipocyte fatty acid-binding protein (A-FABP) is expressed in adipose tissue and macrophages. It regulates cholesterol trafficking and is involved in atherosclerosis formation. A-FABP levels are associated with cardiovascular diseases (CVDs) in patients with or without chronic kidney disease. In this study, we evaluated A-FABP levels in healthy controls and hemodialysis (HD) patients and compared the results with C-reactive protein (CRP) and interleukin-6 (IL-6) levels to determine their relationship with systemic inflammation. Materials and Methods: The study comprised 23 healthy controls and 70 HD patients, excluding individuals with an active infection, malignancy, anorexia, obesity, and hypo- or hyperthyroidism. Demographic features, laboratory findings, A-FABP levels, and levels of inflammatory markers were evaluated between and within the groups. Results: Levels of A-FABP and inflammatory markers were significantly higher in HD patients. In the HD group, 20% of the patients had documented CVD. Levels of A-FABP and inflammatory markers were similar in nondiabetic and diabetic HD patients. Age was negatively correlated with A-FABP levels. Presence of diabetes was not correlated with A-FABP. Serum CRP and IL-6 levels were significantly correlated with A-FABP levels (r=0.354, p=0.003 and r=0.393, p=0.001, respectively). Conclusion: A-FABP levels are elevated in HD patients. Systemic inflammation is significantly related to A-FABP levels in both nondiabetic and diabetic HD patients and decreases with age. Abstract

Lichens and their secondary metabolite are still among the many unexamined natural sources in the drug industry. This study was designed to evaluate the cytotoxic effects of vulpinic acid lichen secondary metabolite and 6.25, 12.5, 25, 50, 100, 200 and 400 μM concentrations that treat cancer cell lines (CaCo2, HepG2, Hep2C, RD Wehi) and normal cells (Vero and L929) by MTT assay. The aim of this study was to determine the apoptotic effect of vulpinic acid on a molecular level. The determination of apoptotic molecular patterns of vulpinic acid was performed on western blot and quantitative realtime PCR (qRT-PCR) analysis. In our study, transcriptome changes on both gene and protein levels showed similar results. The determination of, mRNA levels of Bax, Bcl-2 and P53 genes were showed by qRT-PCR in cancer and normal cell lines. The results of the study showed that IC50 value of vulpinic acid altered the mRNA levels of Bax, Bcl- 2 and P53 genes in all examined cancer cells when compared to the untreated cells. When the mRNA levels of the examined genes were compared, it was observed that Bax gene showed more expression increase in all cell lines when compared to Bcl-2 and P53 genes. This is the first evaluation of the apoptotic effect of vulpinic acid secondary metabolite on mRNA levels. The current study highlights some points regarding vulpinic acid and its use for cancer treatment. [ABSTRACT FROM AUTHOR]

Abstract

Nitric oxide (NO) is unique molecule involved in many intracellular or intercellular signaling pathways and its importance varies from cell to cell. It is demonstrated that heterogeneous regulating responses to various NO levels have been observed in in different types of tumors. Constitutively expressed NO plays an important role in drawing up the phenotype of tumor cells. The roles of NO in tumor growth cover the tumor penetration into the surrounding tissues to spread to remote areas and the development of the neovascularization to obtain the highest blood flow for the necessary resources. Generally, effects of NO at low concentrations considered as protumoral. In very high concentrations of NO, it serves as a potent anti-tumor agent by inhibiting angiogenesis or inducing apoptosis and/or necrosis. While NOS activation is expected to show anti-tumor effect, it has been demonstrated that NO may increase the progression and propagation of cancer due to the effects on metastasis process. The effect of NO on metastasis is affected by different factors such as cell type, dose, the types of participating organs or the period in which NO involved in the tumor development. These different characteristic effects of NO expression are used therapeutically in many preclinical models of cancer in order to increase the inhibition of tumor growth and chemotherapy/radiotherapy efficiency. 84 ÖZET Nitrik Oksitin Sıçan Sağ Atriyumu Ve Sol Papiller Kası Üzerine Etkisinde Protein Kinazların ve Potasyum Kanallarının Rolü Nitrik oksit (NO), endotel hücresinden salgılanan ve hem fizyolojik hem de patolojik durumlarda kardiyak işlevlerin düzenlenmesinde rol alan önemli bir moleküldür. Ancak NO'in miyokardiyal kasılmalar üzerindeki etkileri henüz tam olarak ortaya konmamıştır. Bu çalışmanın amacı, NO'in izole sıçan sağ atriyumu ve sol ventrikül papiller kası üzerine olan etkilerini ve protein kinazların ve potasyum kanallarının bu etkiler üzerindeki olası rollerini ortaya koymaktır. Bu amaçla L-arginin (NO prekürsörü), 8-Br- cGMP (stabil cGMP analoğu), DEA-NONOATE (NO donörü), oksadiazolo kuinoksalin (ODQ, selektif guanilat siklaz inhibitörü), KT5823 (protein kinaz G inhibitörü), ve KT5720 (protein kinaz A inhibitörü) kullanılmıştır. Buna ek olarak NO'in kardiyak etkilerinde ATP-duyarlı K+ kanallarının (Katp) ve kalsiyuma bağımlı K kanallarının (KCa) rolleri, kanal blokörleri glibenklamid ve tetraetilamonyum (TEA) kullanılarak araştırılmıştır. Wistar sıçanlardan izole edilen sağ atriyum ve sol ventrikül papiller kası 37°C de Krebs solüsyonu içeren ve 5% C02 95% 02 ile gazlanan organ banyolarına asılmıştır. Kendiliğinden kasılan sağ atriyum ve dışarıdan elektriksel uyarıyla kasılan sol papiller kasların kasılmaları ve kalp hızı deney süresince kaydedilmiştir. Doku cAMP ve cGMP düzeyleri RIA metodu ile ölçülmüştür. Sonuçlar Student'ın t testi ile istatistiksel olarak değerlendirilmiştir. L-arginin (10`6 M-10`4 M) sağ atriyum tonüsünü azaltırken papiller kas kasılmaları ve kalp hızını etkilememiştir. 8-Br-cGMP (10`7M- 1(T*M) hem atriyum hem de papiller kas kasılmaların baskılamış ancak kalp ritmini etkilememiştir. DEA (10`7 M-10`4 M) papiller kas kasılmalarını etkilemezken atriyum kasılmalarını ve kalp hızını anlamlı olarak baskı lam ıştır. DEA'nın atriyum kasılmaları üzerindeki azaltıcı etkisi ODQ (1fJ5M) ve TEA (5x1 0-2 M) ile tamamen, glibenklamid (3x1 0`6 M) ile kısmen geriye çevrilebilmiş ancak KT5823 ve KT5720'den etkilenmemiştir. DEA'nın negatif kronotropik etkisi ise yanlızca süperoksit dismutaz (SOD, 25u/ml) ile anlamlı olarak geri dönmüştür. DEA (10`4 M) ile doku cGMP düzeyleri sağ atriyum ve papiller kasda anlamlı olarak yükselmiştir. Diğer yandan DEA'nın doku cAMP düzeylerine atriyumda bir etkisi olmazken papiller kasda cAMP'yi arttırdığı gözlenmiştir. Bu bulgular ekzojen NO'in atriyum kasılmaları ve sinüs ritmi üzerine depresif etkileri olduğunu, ventriküler kasılmaları ise etkilemediğini göstermiştir. DEA'nın kasılmalar üzerindeki etkisi cGMP ve potasyum kanalları ile ilişkili iken, sinüs üzerinde oksidan stres oluşturarak etki ettiği düşünülmektedir. Anahtar Kelimeler: Nitrik oksit, Kalp kasılmaları, Kalp hızı, Protein kinazlar, Potasyum Kanalları 85 SUMMARY The Role of Protein Kinases and Potassium Channels on the Effect of Nitric Oxide on Right Atrium and Left Papillary Muscle from the Rat Heart Nitric oxide (NO), an important molecule released from endothelium, plays a role in modulating cardiac functions both pathologic and physiologic conditions. But the effects of NO on myocardial contractility are a matter of controversy. The aim of this study was to examine the effects of NO on myocardial contractility and rhythmicty and role of protein kinases and K+ channels by using L-arginine (NO precursor), 8-Br-cGMP (stable analogue of cGMP), DEA-NONOATE (NO donor), oxodiazolo quinoxalin (ODQ.selective inhibitor of soluble guanylyl cyclase), KT5823 (inhibitor of protein kinase G), KT5720 (inhibitor of protein kinase A), glybenclamide (Katp bloker) and tetraetylamonium (TEA, Kca bloker). Right atrium and left ventricle papillary muscles were isolated from Wistar rats (200-300 g). Tissues were mounted in water-jacketed organ baths containing Krebs solution aerated with 5% CO2 95% 02 at 37°C. Papillary muscles were stimulated with a square-wave DC impulse. Rhytmic activity and contractions of tissues were recorded on a polygraph. Tissue levels of cAMP and cGMP were measured by RIA method. Results are statistically evaluated using Student's t-test. Although L-arginine (10`6 M - 10`* M) decreased only resting force, 8-Br-cGMP (10`7M - lO^M) decreased the contractions of cardiac tissues but did not affect the sinus rate significantly. DEA (10`7 M - 10`4 M) decreased both developed and resting force and sinus rate of right atrium but not papillary muscle contractions. Effect of DEA on right atrium contraction was blocked by ODQ (10`5M), TEA (5x10` 3 M) and by glybenclamide. Negative chronotropic effect of DEA was not changed by ODQ, glybenclamide (3x 10`6 M), TEA, KT5823 and KT5720 whereas totally inhibited by superoxide dismutase (25u/ml). DEA (lO^M) elevated cGMP level in the right atrium and papillary muscle. However cAMP level was elevated by DEA only in the papillary muscle. These findings demonstrated that exogenous NO has depressive effects on sinus rate and contractions of right atrium. K+ channels and cGMP have an important role on negative inotropic action of NO in right atrium. The depressive effect of DEA on heart rate depends on NO derivated oxidative stress. Abstract

Objective: The cytotoxic effects of natural rubber gloves and urinary catheters on different cells have been earlier reported. In order to reduce these effects, various coating techniques such as silver or polymer (hydrogel, silicone, polytetrafluoroethylene (PTFE)) coatings have been developed. However, during manufacturing process of silicone-coated rubber catheters, similar to latex catheters, zinc compounds, which have cytotoxic effects are added to the natural rubber base. The aim of this study was to compare the in vitro cytotoxic effects of rubber catheters with the silicone-coated rubber and to investigate the causes of the toxic effect. Method: The cytotoxic effects of total 48 urinary catheters in three different structure (natural rubber, silicone, silicone-coated rubber) and 8 different brands were analysed in mouse connective tissue fibroblast cell culture (L-929) by qualitative (imaging and scoring) and quantitative (MTT) methods. The levels of the elements in catheter extracts were examined in an Atomic Absorption Spectrophotometer (AAS). Results: Compared with standard polyethylene used as a negative control, the extracts from 100% silicone did not change vitality of the fibroblast cells (95.43% ± 5.39), while the natural rubber (8.57% ± 0.54) and the silicone-coated rubber (21.0% ± 2.52) decreased this significantly (p <0.01). The analysis of the catheter extracts by the AAS method showed that the zinc levels of rubber and silicon-coated catheter extracts were 4.78 ± 0.66 mg/L and 2.78 ± 0.33 mg/L, respectively and correlated with the observed toxic effects. No significant differences were found in the levels of elements such as Cr, Ag, Cu, Bi, Pb, Fe, Co and Cd. The addition of the metal chelating agent ethylenediaminetetracetic acid (EDTA) in a non-toxic level (5 µM) for the cells, reversed the latex cytotoxicity (101.05% ± 5.86 and 100.24 ± 4.9, respectively). Conclusion: Our study demonstrated that both the rubber and the silicone-coated rubber catheters have cytotoxic effects. Zinc compounds, which are added to rubber in the production stage for a variety of purposes, have proven to be responsible for this toxicity. The significant difference between the cytotoxic effects of the silicone-coated rubber and latex catheters suggests that the production technology is an important factor for the quality. Different coating methods have to be investigated or the products proved to be non-toxic should be used instead of the rubber catheters, in order to reduce complications in urinary catheterization in clinical practice. Abstract

Splenectomy is closely associated with a lifetime risk of pneumococcal and other encapsulated bacterial infections. In this study, it was aimed to investigate the change of antibody levels after vaccination against Streptococcus pneumoniae according to age, gender, years after splenectomy and the possible effect of splenectomy on IgG avidity. In addition the education and awareness levels of the participants about post-splenectomy vaccination and infectious diseases were also analyzed. In the first of the three phases of this study, 32 individuals with splenectomy were enrolled. The awareness of the patients about the possible risks after splenectomy was investigated with a simple questionnaire. Routine laboratory test results were obtained and clinical examinations were performed. In the second stage, total Ig values of 29 splenectomy patients were determined. In the third phase, 14 splenectomy and 5 healthy volunteers were vaccinated according to the Vaccination Practices Advisory Committee (ACIP) guidelines. Pneumococcal-specific antibody levels and IgG avidity were detected by enzyme linked immunosorbent assay (ELISA). It was determined that 68.8% of the splenectomized patients were unaware of their vaccination status and 78.2% of them were unaware of the increased risk of infectious diseases in asplenic conditions. . According to the hospital information management system, all 31 (96.87%) patients, except one, were vaccinated with PPV23. As expected, vaccinated patients exhibited high levels of vaccine-specific antibody production with IgG, IgG2, and IgA antibody concentrations of 321 ± 76.68 mg/l, 73.07 ± 8.273 mg/l, and 117.8 ± 14.94 mg/l, respectively, but unvaccinated patients had very low antibody (IgG, IgG2 and IgA antibody concentrations were 11.5 mg/l, 1.3 mg/l and 1.2 mg/l, respectively) levels. Although there was no correlation between antibody titers and gender, age groups or presence of fever history, the decrease in total IgG, IgG2 and IgA titers were strongly correlated with the time since splenectomy. Antibody titers were found to be significantly lower in splenectomized patients vaccinated more than 10 years ago. Routine laboratory results were at normal levels except for low platelet count. On the other hand, both splenectomized and healthy control subjects displayed similar IgG avidity index values (%61.8 ve %64.4% inhibition in control and splenectomized subjects, respectively) after the vaccination schedule. It was shown that post-splenectomy vaccination with PPV23 induced high levels of pneumococcus-specific antibody production that can last for more than five years. It was determined that more efforts should be made to increase the level of knowledge about pneumococcal and other overwhelming post-splenectomy infections (OPSI) as the awareness of the patients about the risks of infection after splenectomy was poor. In particular, patients with splenectomy operation more than 10 years ago should be very careful about being asplenic as they were determined to have significantly lower level of vaccine-specific antibody production. Our study was also the first to show that splenectomy does not alter IgG avidity induced by pneumococcal vaccination.
