Background: A number of mechanical decontamination methods have been proposed, however, there is no agreed-upon gold standard among them. This study aims to conduct a meta-analysis to assess the differences in the management of an inflammatory reaction around dental implants between air abrasion and mechanical decontamination. Methods: A comprehensive search strategy was employed, incorporating controlled vocabulary (MeSH) and free-text terms. This search was conducted by two reviewers to identify published systematic reviews. Three major electronic databases, namely, Medline via PubMed, the Cochrane database, and Embase, were searched up to May 2024. Results: Initially, 300 articles were identified. After conducting a comprehensive search and applying strict inclusion criteria, a total of 13 studies were deemed eligible for inclusion in the meta-analysis. The results showed that the mean difference in probing depth between air abrasion and other mechanical decontamination was 0.28 (95% confidence interval, −0.20 to 0.76). The mean difference in probing depth of air abrasion compared with other mechanical decontamination in maintenance purposes was 1.05 (95% confidence interval, 0.18 to 1.91). The mean difference in bleeding on probing between air abrasion and other mechanical decontamination was 0.51 (95% confidence interval, 0.07 to 0.95). The mean difference in alveolar bone loss between air abrasion and other mechanical decontamination was −0.14 (95% confidence interval, −0.77 to 0.48). The mean difference in alveolar bone loss for surgical approaches of air abrasion compared with other mechanical decontamination was 0.32 (95% confidence interval, 0.03 to 0.61). Conclusions: The findings of the study indicate that the use of air abrasion was just as effective as other mechanical decontamination methods in reducing probing depth and alveolar bone loss. The subgroup analysis showed that air abrasion was less effective in reducing probing depth in maintenance purposes. Purpose: Bioceramic coatings have been shown to promote bone repair, which aids in the early integration of implants. This study aimed to evaluate the influence of air abrasion with a bioceramic abrasive on the surface characteristics of different implant materials and surfaces. The dissolution of the applied treatment from the surfaces over 3 weeks was also assessed. Materials and Methods: Discs of three alloys used for dental implants were studied and compared: two types of commercially pure titanium (CpTi)/ (CpTi SLActive) and titanium‐zirconia (TiZr). The tested surfaces were: CpTi control (CpC), sandblasted (SB), sandblasted and acid‐etched (SBE), and CpTi SLActive®, (TiZr) Roxolid®. Three discs from each group underwent air abrasion with apatite bioceramic powders, 95% hydroxyapatite (HA)/5% calcium oxide (CaO), and 90% hydroxyapatite (HA)/10% calcium oxide (CaO). The treated discs were surface characterized by optical profilometry to obtain surface roughness, scanning electron microscopy (SEM), and energy dispersive X‐ray spectroscopy (EDS) to compare element weight percentages of titanium, calcium, and phosphate. Dissolution was assessed using inductively coupled plasma optic emission spectrometry (ICP‐OES). Results: Bioceramic powders were deposited on all tested surfaces leading to changes in surface characteristics. The only statistically significant differences between the material groups for surface roughness were found with 95% HA/5% CaO powder in the Sp and Rp parameters (p = 0.03 and 0.04, respectively). There were no significant differences in the Ca and P wt% between all groups and powders 95% HA/5% CaO and 90% HA/10% CaO (p = 0.14, 0.18, and p = 0.15, 0.12, respectively). A non‐uniform dispersion of the treatment on the surface layer was visible on all treated surfaces. The bioceramic powder continued to dissolute from the tested surfaces for 3 weeks. Conclusion: Bioceramic abrasion modifies implant surface characteristics, although the change in surface characteristics resulting from such treatment was not influenced by the implant material or surface treatment. Air abrasion with hydroxyapatite and calcium oxide bioceramics leaves powder deposits on the treated implant surfaces that could potentially influence the healing of implants affected by peri‐implantitis. Dental fluorosis is a subsurface hypomineralization condition, characterized by yellow to dark brown stains, with or without enamel porosity and pitting. Due to aesthetic concerns, children with discolored anterior teeth often experience negative impacts on their psychological, emotional, and social development. Treatment options include porcelain laminates or veneers, direct resin restorations, and crowns. However, these methods are invasive and require significant enamel reduction, making them unsuitable for children due to their enlarged pulp chambers. Microabrasion is the least invasive and safest treatment for children with fluorosis. This technique involves using a combination of 37% phosphoric acid gel with extra fine grain pumice, 18% hydrochloric acid in pumice, or 6.6% and 10% hydrochloric acid with silica carbide particles. The mixture is applied with a slowly rotating rubber cup to the areas of discoloration or white opacities. An additional benefit of microabrasion is that it leaves the enamel surface highly polished, reducing the growth of cariogenic bacteria and increasing resistance to demineralization. This case report describes two instances of dental fluorosis treated with microabrasion using different acidic compounds in two pediatric patients, resulting in clinically acceptable and aesthetically pleasing outcomes. Objectives: Comparative evaluation of surface changes and bond strength of Microabrasion Bleached enamel before and after Remineralization with Nano-hydroxyapatite paste, an In vitro study. Methods: Enamel samples were divided into - Group I - Conventional bleaching (HB), Group II - Conventional bleaching with remineralization (HB+N-Hp), Group III - Microabrasion-assisted bleaching(M+HB), and Group IV - Microabrasion-assisted bleaching with remineralization(M+HB+NHp). Surface roughness was measured using a profilometer. Samples were stored in artificial saliva for two weeks, then bonded with nanofilled composite resin and subjected to shear bond strength (SBS) analysis. Results: Group IV (M+HB+N-Hp) showed higher surface roughness than Groups I (HB) and II (HB+N-Hp), while Group II had significantly lower surface roughness compared to Groups III (M+HB) and IV. In terms of shear bond strength (SBS), Group III exhibited significantly higher SBS than Group I, whereas Group II showed significantly reduced SBS compared to Group I. All values were statistically significant. Conclusions: Remineralization with N-Hp reduces the surface roughness caused by bleaching and microabrasion, potentially decreasing bacterial biofilm adherence. Despite increased surface roughness correlating with better bond strength, microabrasion-assisted bleaching followed by remineralization and direct composite veneering after two weeks is a promising minimally invasive treatment for severe tooth discolorations. Background: Different methods for removing dental carious lesions exist, including conventional rotary caries removal and new advanced technology like polymer-based burs, chemomechanical agents, air abrasion, and laser. Objectives: This study shows the differences in features of dentin (smear layer, patency of dentinal tubules, surface irregularities, intertubular micro porosities, and exposed dentinal tubules) among different types of caries removal techniques. Materials and Methods: An in vitro study was done on 60 primary molars with occlusal class I active caries. Teeth were divided into three groups according to a method of caries removal (G1: chemomechanical, G2: mechanical with a smart bur, and G3: air-abrasion). After complete caries excavation, the teeth were examined under a scanning electronic microscope (SEM) with the power of magnification 4,000x and 8,000x to show the morphological dentinal features with SEM microphotographs. Data obtained was analyzed using the SPSS program where Fisher exact, Kruskal–Wallis and multiple Wilcoxon sum rank tests were used. The level of significance is when the p-value is less than 0.05. Results: Generally, SEM showed the highest ratio of score 1 of smear layer presence, surface irregularities, and microporosity in all groups in both magnifications. The patency of tubules showed the highest ratio of score 1 in G1, scores 2 in both G2 and G3 in magnification 4,000x, while 8,000x there was the highest ratio of its score 1 in G1 and G2 while the G3 has score 2 as the highest score. The exposed dentinal tubules showed the highest ratio in G1 in score 3, in G2 in score 2, and in G3 in score 1 in magnification 4,000x, while 8,000x there was the highest ratio of its score 2 in both G1 and G3 while the G3 has highest score 1. The study with magnification 4,000x showed a significant difference (S) among three groups in exposed dentinal tubules with a p-value (0.012), and there was S between chemomechanical and smart, chemomechanical and air-abrasions groups with a p-value (0.041, 0.001 subsequentially). Other dentin features showed non-significant differences (NS) among or between groups in both magnifications (4,000x, 8,000x). Conclusions: All groups were effective in removing caries and can successfully treat young, scared or stressed patients. Objective: To investigate the effect of printing material and air abrasion of bracket pads on the shear bond strength of 3D‐printed plastic orthodontic brackets when bonded to the enamel of extracted human teeth. Materials and Methods: Premolar brackets were 3D‐printed using the design of a commercially available plastic bracket in two biocompatible resins: Dental LT Resin and Dental SG Resin (n = 40/material). 3D‐printed brackets and commercially manufactured plastic brackets were divided into two groups (n = 20/group), one of which was air abraded. All brackets were bonded to extracted human premolars, and shear bond strength tests were performed. The failure types of each sample were classified using a 5‐category modified adhesive remnant index (ARI) scoring system. Results: Bracket material and bracket pad surface treatment presented statistically significant effects for shear bond strengths, and a significant interaction effect between bracket material and bracket pad surface treatment was observed. The non‐air abraded (NAA) SG group (8.87 ± 0.64 MPa) had a statistically significantly lower shear bond strength than the air abraded (AA) SG group (12.09 ± 1.23 MPa). In the manufactured brackets and LT Resin groups, the NAA and AA groups were not statistically significantly different within each resin. A significant effect of bracket material and bracket pad surface treatment on ARI score was observed, but no significant interaction effect between bracket material and pad treatment was found. Conclusion: 3D‐printed orthodontic brackets presented clinically sufficient shear bond strengths both with and without AA prior to bonding. Introduction: Pit and fissure sealants are a reliable and safe method of occlusal caries prevention. Accurate preconditioning of the enamel surface is essential for long-term sealant retention. Aim: To assess the retention rate of pit and fissure sealants applied to first permanent molars treated with air abrasion and acid etching in children aged 6 to 9 years. Materials and Methods: This split-mouth randomised study was conducted in the Department of Paediatric Dentistry, Vishnu Dental College, Bhimavaram, Andhra Pradesh, India from March 2019 to March 2020. The study included 45 children aged 6-9 years with 180 completely erupted maxillary and mandibular first permanent molars. The children were randomly divided into two groups: Group-I (Air Abrasion) and Group-II (Conventional Acid Etch). After pretreatment of the enamel surface with either air abrasion or acid etching, sealant was applied. Sealant retention was evaluated at 6- and 12-month intervals using Tonn and Ryge's scoring criteria under a dental operating microscope. Statistical analysis was performed using the Wilcoxon Signedrank Test and the Mann-Whitney U Test. Results: The mean age of the study participants was 7.6±0.5 years. At the 12-month interval, complete sealant retention was observed in 23% of teeth in the air abrasion group and 21% in the acid etch group. The p-value of 0.657 indicates that the difference was not statistically significant. The comparison of sealant retention rates in maxillary and mandibular molars treated with both air abrasion (p=0.191, p=0.881) and acid etching (p=1.000, p=0.574) showed no statistically significant differences at both the 6- and 12-month intervals. Conclusion: The retention rate of fissure sealants on enamel surfaces treated with air abrasion was comparable to acid etching. The objective of this study was to assess the effect of airborne-particle abrasion with alumina particles of various sizes (50 μm, 110 μm, 250 μm) on the surface properties of 3Y-TZP (3 mol% yttria-stabilized tetragonal zirconia polycrystals). The analysis comprised the Vickers hardness test (HV1kp), a surface roughness evaluation (Ra, Rq, Rz, Rsk, Rsm, Rt, Vo, profile displays), and SEM–EDS microanalysis (scanning electron microscopy–energy-dispersive X-ray spectroscopy). Any statistical relationships were assessed using the Kruskal–Wallis one-way analysis-of-variance-by-rank test (p = 0.05). While airborne-particle abrasion with 50 µm alumina significantly increases the Vickers hardness of 3Y-TZP (1678.8 HV1kp), 110 μm and 250 μm alumina particles do not appear to have any such effect. The surface roughness of 3Y-TZP increases with the size of the alumina particles (Ra, Rq, Rz, Rt, Vo: vertical plane and retention volume assessment); a higher Rsm (horizontal plane assessment) was noted for specimens air-abraded with 250 µm alumina particles; air abrasion with 50 μm alumina particles yielded a symmetrical surface roughness profile (Rsk). The major topographic pattern of the surface of 3Y-TZP is altered by alumina airborne-particle abrasion, with larger alumina particles causing more severe changes. Aluminum traces on the 3Y-TZP surface are observed. The shape of the alumina particles is irregular, with rough edges; the size of the alumina particles plays a significant role in the air abrasion of zirconia-based dental ceramics, affecting their morphology and properties. The surface of zirconia-based dental ceramic restorations require preparation prior to adhesive cementation. The purpose of this study was to assess the influence of airborne-particle abrasion with different sizes of alumina particles (50 μm, 110 μm, or 250 μm) on the mechanical strength of zirconia-based ceramics' frameworks and on the extent of phase transformations. A fracture resistance test was performed. The central surface of the frameworks was subjected to a load [N]. The identification and quantitative determination of the crystalline phase present in the zirconia specimens was assessed using X-ray diffraction. The Kruskal–Wallis one-way analysis of variance was used to establish significance (α = 0.05). The fracture resistance of zirconia-based frameworks significantly increases with an increase in the size of alumina particles used for air abrasion: 715.5 N for 250 μm alumina particles, 661.1 N for 110 μm, 608.7 N for 50 μm and the lowest for the untreated specimens (364.2 N). The X-ray diffraction analysis showed an increase in the monoclinic phase content after air abrasion: 50 μm alumina particles—26%, 110 μm—40%, 250 μm—56%, and no treatment—none. Air abrasion of the zirconia-based dental ceramics' surface with alumina particles increases the fracture resistance of zirconia copings and the monoclinic phase volume. Objectives: Analysis of the in vitro efficacy of non‐surgical and surgical dental implant surface decontamination with or without suprastructure. Materials and Methods: Three hundred and sixty implants were dipped in indelible red and distributed to 30°, 60°, or 90° angulated bone defect models. One hundred and twenty implants were used for each bone defect, 40 of which were assigned to a decontamination method (CUR: curette; SOSC: soundscaler; APA: air powder abrasion). Of these, 20 were subjected to a simulated non‐surgical (NST) or surgical treatment (ST), with/without mucosa mask, of which 10 were carried out with (S+) or without (S−) suprastructure. Uncleaned implant surface was assessed by both‐sided implant surface photography. Surface morphology changes were analyzed using scanning electron microscopy (SEM). Results: Cleaning efficacy was significantly better within NST if the suprastructure was removed (p < 0.001). No significant difference was found within ST (p = 0.304). Overall, cleaning efficacy in the order APA > SOSC>CUR decreased significantly (p < 0.0001) for both S+ and S− in NST as well as ST. Separated by NST/ST, S+/S−, defect angulation and decontamination method, only isolated significant differences in cleaning efficacy were present. Linear regression analysis revealed significant associations of remnants with the treatment approach, decontamination method, and defect angle (p < 0.0001). SEM micrographs showed serious surface damage after use of CUR and SOSC. Conclusions: Suprastructure removal is an additional option to improve cleaning efficacy of non‐surgical implant surface decontamination in this in vitro model. [ABSTRACT FROM AUTHOR]

Purpose: Drill less dentistry is painless, riskless, soundless and heatless and is very suitable for dental-related concerns where children are the most affected fraternity. Removing enamel from the teeth at the affected region by conventional drilling mechanism is challenging. The processed region is filled using amalgam or other sources for the occupation. The proceedings are a painful experience for the patients due to heat generation while drilling, which also induces vibrations and related noises. There are higher possibilities for tissue damage and disturbances in the unaffected regions. Air-abrasion-based drill-less dentistry handles such problems in a novel way and provides a comparatively pleasant treatment experience to patients. Design/methodology/approach: The enamel removal rate influences the drill-less dentistry as it empowers to predict the quantum of material that can be abraded while executing the process. The mathematical expression of the enamel removal rate has been estimated based on the basic laws of physics and assumptions. Findings: The current work exhibits mathematical modelling to predict the enamel removal. The expression also reveals that the velocity, density and mass flow rate of abrasive particles has a crucial role in deciding the rate of enamel removal from the tooth. The present mathematical expression provides beneficial inputs to the research fraternity in the dental field. Research limitations/implications: The current mathematical expression has arrived through basic laws of physics and assumptions. The enamel removal rate is estimated using an analytical model, and the current mathematical expression can be improvised through fine-tuning fine. The present preliminary studies could be helpful in developing an accurate predictive model in future. Practical implications: The present research supports drill-less dentistry and provides a mathematical solution in terms of derived formulations in predicting the enamel removal rate, as enamel removal rate plays an essential role in drill-less dentistry. Originality/value: The mathematical expression facilitates the problem handling more practically and efficiently. The mathematical expression is helpful in studying and deciding the processing conditions such as stream velocity, particle density and mass flow rate on effective enamel removal rate from the tooth structure. Background: This study investigated the effect of exposure of enamel surfaces to dried fruit suspension including dates, raisins, and dried apricot on their microhardness changes. Methods: In this in vitro study, fifty enamel sections of bovine incisor teeth were mounted inside the acrylic resin so that the enamel surface would not be exposed to the acrylic. After surface polishing, the initial micro-hardness was measured by a Vickers hardness-testing machine. The samples were randomly placed into five different solutions including apricot, raisin and date suspension, citric acid (positive control) or sorbitol (negative control), 5 times a day, each time for 5 minutes, and then in artificial saliva for 60 minutes. This process was repeated for 20 days. Eventually, the final micro-hardness of the samples was measured. Micro-hardness changes between groups were compared through ANOVA and TUKEY test using SPSS 23 software with a significance level of P<0.05. Results: After exposure, the micro-hardness of the teeth was significantly reduced in all three suspensions prepared from dried fruit (P<0.05). Apricot and date had the highest and lowest effects on reducing the micro-hardness of teeth, respectively. Sorbitol solution did not have a significant effect on changing the micro-hardness of teeth (P=0.13). Conclusion: The suspension of studied fruits (apricot, raisin, date) causes a significant reduction in micro-hardness of the dental enamel surface, indicating the negative effect of frequent consumption of dried fruits over long periods of time on dental health. Enamel hypoplasia occurs because of a defect in formation of the organic matrix during the development of tooth enamel. Minimally invasive procedures of the slightly altered enamel contribute to a greater longevity of teeth and prevent them from relapsing into the repetitive restorative cycle. This case history report aimed to show a sequential technique of minimally invasive procedures for esthetic resolution in anterior teeth. Prior to microabrasion, anterior teeth were bleached in office with 37% hydrogen peroxide. Afterwards, hypoplastic spots on the buccal incisal thirds of the maxillary central incisors were treated with two sessions of microabrasion using phosphoric acid and pumice stone and one session using resin infiltrant. Besides a slight remaining white spot on tooth 21, the masking of spots was done with this sequence of treatments, re-establishing color harmony. Aim: Air powder polishing (APP) can cause roughness on composite surfaces, and consequently the color change that occurs over time affects the aesthetic negatively. Here, we aim to investigate the effect of repolishing on the roughness and discoloration of resin composites after APP to avoid its negative effects for the first time. Methodology: A total of 39 composite discs were randomly allocated into three groups: Group I: Control (n = 13), Group II: Air Powder Polished (n = 13), and Group III: Air Powder Polished + Re-polished (n = 13). Color and surface roughness of the discs were measured before and after immersion in coffee for 1 week. Color was measured using a colorimeter (CR-400, Konica Minolta, Osaka, Japan), and roughness was measured using a surface roughness analyzer (SJ-400, Mitutoyo, Japan). Results: All specimens in the groups, except those in Group III, showed significant changes in roughness, and all materials showed significant color changes after immersion in the coffee relative to the baseline. There was no statistically significant difference between the groups in terms of roughness and color. Dental caries is the most common chronic disease, whose aetiology is multifactorial. It results from chemical dissolution and demineralisation of tooth structure due to the organic acids produced by microbes. If these lesions are left untreated, it can result in pulpal and periapical infection. Hence careful conservative management of these carious lesions are done to prevent further destruction. There are various concepts on caries excavation, from complete removal of the carious lesion to treatment where the carious lesion is not touched. There are also few non-invasive methods to arrest these lesions which are described in this article. Other than hand instrumentation, both rotary and nonrotary techniques were utilized in caries removal. Rotary includes dental burs of varying shapes and sizes running on various speeds whereas non-rotary includes air abrasives, ultrasonic, laser, chemo-mechanical techniques. Introduction: Bioactive glass is a material that finds more and more applications every year. It has the potential to be widely used in air abrasion because of its' special qualities. Objectives: This study aimed to present the application and therapeutic possibilities of using bioactive glass as an abrasive. Material and methods: Databases of PubMed/Medline, EBSCOhost, and Scopus were searched with the following combination of keywords: "(air abrasion or air-abrasion or sandblasting) and (bioactive glass or sylc)". Only original articles from the years 2010-2021 were included into the study. Results: After removing duplicates, 41 articles were obtained. As a result of the screening, 30 articles were used in this work. Bioactive glass air abrasion can be widely used in dentistry. It promotes re-mineralization of hard tissue, enabling a conservative approach to treatment of both carious and non-carious lesions. This material is also useful in implantology, by supporting bone regeneration, or in orthodontics by removing orthodontic adhesive without enamel damage. In the era of minimally invasive dentistry, frugal tooth preparation and minimal intervention, and not high efficiency of work, have become the canon of management. Bioactive glass is a material, that perfectly fits into this standard. Conclusions: Bioactive glass has unique advantages, such as being antibacterial or having the ability to re-mineralize tissues. Objective: To compare the in vitro decontamination efficacy of two electrolytic cleaning methods to diode laser, plasma, and air-abrasive devices. Material and methods: Sixty sandblasted large-grit acid-etched (SLA) implants were incubated with 2 ml of human saliva and Tryptic Soy Broth solution under continuous shaking for 14 days. Implants were then randomly assigned to one untreated control group (n = 10) and 5 different decontamination modalities: air-abrasive powder (n = 10), diode laser (n = 10), plasma cleaning (n = 10), and two electrolytic test protocols using either potassium iodide (KI) (n = 10) or sodium formate (CHNaO2) (n = 10) solution. Implants were stained for dead and alive bacteria in two standardized measurement areas, observed at fluorescent microscope, and analyzed for color intensity. Results: All disinfecting treatment modalities significantly reduced the stained area compared to the untreated control group for both measurement areas (p < 0.001). Among test interventions, electrolytic KI and CHNaO2 treatments were equally effective, and each one significantly reduced the stained area compared to any other treatment modality (p < 0.001). Efficacy of electrolytic protocols was not affected by the angulation of examined surfaces [surface angulation 0° vs. 60° (staining %): electrolytic cleaning-KI 0.03 ± 0.04 vs. 0.09 ± 0.10; electrolytic cleaning-CHNaO2 0.01 ± 0.01 vs. 0.06 ± 0.08; (p > 0.05)], while air abrasion [surface angulation 0° vs. 60° (staining %): 2.66 ± 0.83 vs. 42.12 ± 3.46 (p < 0.001)] and plasma cleaning [surface angulation 0° vs. 60° (staining %): 33.25 ± 3.01 vs. 39.16 ± 3.15 (p < 0.001)] were. Conclusions: Within the limitations of the present in vitro study, electrolytic decontamination with KI and CHNaO2 was significantly more effective in reducing bacterial stained surface of rough titanium implants than air-abrasive powder, diode laser, and plasma cleaning, regardless of the accessibility of the contaminated implant location. Clinical relevance: Complete bacterial elimination (residual bacteria < 1%) was achieved only for the electrolytic cleaning approaches, irrespectively of the favorable or unfavorable access to implant surface. Purpose: This in vitro study aims to assess the impact of various surface treatments on the shear bond strength (SBS) of two types of artificial teeth and denture base resins (DBRs). Materials and Methods: Two types of DBRs (CAD/CAM‐milled and heat‐polymerized) and two types of denture teeth (acrylic and composite) were investigated. Teeth were cut into slices (5 × 5 × 2 mm) and divided according to surface treatment into four subgroups (n = 10): no treatment (control), air abrasion (Alumina‐blasting; AB), bur roughening, and dichloromethane (DCM) subgroups. According to manufacturer recommendations, the treated tooth slices were bonded to the acrylic disk of DBRs. The SBS test was performed using a universal testing machine. ANOVA was used for results analysis followed by Tukey's post hoc tests (α = 0.05). Results: DCM and AB increased the SBS of acrylic teeth to heat‐polymerized DBR compared with other groups (p < 0.001). All surface treatments showed no significant difference in CAD/CAM DBR with acrylic teeth (p = 0.059; AB, p = 0.319; bur roughening, p = 0.895; DCM), while there was a significant decrease in SBS with composite teeth (p ˂ 0.001). Between teeth, acrylic teeth showed a statistically significant increase in SBS compared to composite teeth (p < 0.001). Conclusion: AB and DCM application improved the SBS for acrylic teeth with the heat‐polymerized DBR when compared with the untreated group, but none of the surface treatment agents showed significant improvement with CAD/CAM DBR. OBJECTIVE: To assess the enamel surface damage and residual adhesive remnant [adhesive remnant index (ARI)] on extracted premolars after debonding the ceramic brackets using three different debonding techniques, with optical coherence tomography (OCT). METHODS: Ninety extracted premolars were bonded with ceramic brackets and divided into three groups of 30 teeth each based on debonding techniques used. Twenty-four hours later, they were debonded using three different debonding techniques: debonding pliers, ultrasonic scalers, and Er-YAG laser. A baseline scan was obtained prior to bonding using OCT. The teeth were evaluated for the adhesive remnant on the tooth surface using ARI score, and the amount of enamel surface damage was evaluated using OCT. RESULTS: We observed that the use of ultrasonic scalers as a debonding technique led to greater incidence of enamel surface damage as measured in OCT. The ARI scores with debonding pliers and laser were significantly greater than that of scaler debonding. CONCLUSIONS: Results of this in vitro study confirmed that use of ultrasonic scalers as a debonding technique led to significantly greater incidence of enamel surface damage when compared to the other two debonding techniques. Microabrasion is a technique to improve the appearance of teeth with fluorosis, which is based on the chemical (acid) and mechanical (abrasive) removal of opacities using a gel. This study evaluated changes in enamel roughness and wear by means of a focal variation microscope when using a microabrasion system. Ten extracted third molars were used. Two sessions (t1 and t2) of the microabrasive agent were carried out for 60s and 30s respectively, on the vestibular surface of all teeth and a final polishing with the rubber cup recommended by the manufacturer for 15s. A parallelometer with adapted micro-motor was used for standardized microabrasion. A focus variation microscope was used to measure roughness (Sa, Sq, Sp and Sv) and surface wear. For statistical analysis Spearman’s correlation test were used. The results expressed in µm were: t1 (Sa: 2.31, Sq: 3.0, Sp: 11.31, Sv: 12.7) and t2: (Sa: 1.6, Sq: 2.1, Sp: 11.0, Sv: 8.5). The wear with respect to initial values was t1: -11.86 and t2: -33.24. Significant correlations were not observed. It can be inferred that although wear could increase with a greater number of sessions, but an increase in roughness would not necessarily be expected. [ABSTRACT FROM AUTHOR]

Introduction: With the increasing demand for aesthetic treatments, tooth bleaching is becoming a common procedure in dentistry. Contemporary bleaching agents are typically composed of either hydrogen peroxide (HP) or carbamide peroxide (CP). Tooth colour can be influenced by various factors, like intrinsic stains, extrinsic stains or systemic causes. Tooth bleaching treatment options include whitening toothpastes, professional cleaning by scaling and polishing to remove stain and tartar, internal bleaching of non-vital teeth, external bleaching of vital teeth, microabrasion of enamel with abrasives and acid, placement of crowns and veneers. Aim: To analyze the literature on bleaching and to provide a broad overview of the bleaching techniques, of their efficacy and adverse effects on soft and hard tissues, as well as of its effects on biological structures, and to provide literature review. Materials and methods: An online literature analysis was done using PubMed and Google Scholar for studies reporting bleaching techniques in dentistry until 1st July 2021, using relevant keywords like tooth-whitening, tooth bleaching, hydrogen peroxide, etc. Over 190 articles were initially reviewed, based on their applicability to the present topic and then studied in detail. Results and discussion: In vitro and in vivo studies, comparing various bleaching techniques, have demonstrated the efficacy of vital and non-vital tooth bleaching. Bleaching treatments are affected by several factors, including the actual causes of tooth discoloration. Also, patient compliance, sincerity and dedication in following the treatment plan plays a huge role in the success. Conclusions: This review article will help clinicians elevate their knowledge about various bleaching options available, its processing and understanding of the effects of bleaching on teeth, to reduce the risks for patients. Background. Enamel microabrasion can eliminate enamel irregularities and discoloration. This study evaluated the staining susceptibility of enamel after microabrasion using different clinical protocols. Methods. A total of 210 extracted bovine incisors were used in this study. The teeth were randomly divided into five groups of 42 teeth each (n = 42), including group 1: control (no treatment), group 2: microabrasion, group 3: microabrasion + fluoride polishing, group 4: macroabrasion (fine-grit diamond bur) + microabrasion, and group 5: macroabrasion (finegrit diamond bur) + microabrasion + fluoride polishing. The groups were then randomly divided into two subgroups for discoloration procedures: coffee and distilled water (n = 21). A spectrophotometric evaluation was carried out at baseline and on the 1st, 7th, 14th, and 28th days of the staining period. Statistical analyses were performed using repeated-measures ANOVA and the post hoc Bonferroni test at a significance level of 0.05. Results. The greatest color change was observed in group 2 specimens, which were immersed in coffee solutions. The color change values for groups 3, 4, and 5 specimens, which were immersed in distilled water, were higher than those in group 1 specimens (P < 0.05). The fluoride polishing + enamel microabrasion treatment groups (groups 3 and 5) exhibited greater resistance to color changes than the matched non-fluoride-polished groups (groups 2 and 4) (P < 0.05). Conclusions. The teeth that underwent enamel microabrasion treatment and were polished with fluoride gel became more resistant to color changes. Objectives: To develop and test the cutting efficiency of a novel degradable glass as an alternative media to alumina powder for air abrasion. Materials and methods: A zinc-based glass (QMZK2) was designed, produced, and evaluated with a multi-modality imaging analysis. The glass dissolution study was carried out in three acids, using ICP-OES (inductively coupled plasma optical emission spectroscopy) at 5 different time points: 2.5, 5, 10, 60, and 240 min. The cutting efficiency of both materials was tested under the same parameters on slabs of elephant enamel. A stained fissure of a molar tooth was air abraded with the glass and evaluated with X-ray micro-tomography before and after air abrasion. Results: The particle size distribution of the glass was similar to that of alumina 53 µm but with a slightly greater dispersion of particle size. The shape of the particles was angular, appropriate for cutting purposes. The dissolution study showed that the glass dissolved rapidly in acidic conditions at all time points. Between the two variables, pressure and powder flow, pressure was found to influence the cutting speed to a greater extent than powder flow. Conclusions: Alumina powder was found to perform significantly better in 4 of the 9 conditions tested on elephant enamel, QMZK2 in one, and no significant differences were found for the rest of the 4 conditions. The QMZK2 seems to offer promising results as an alternative material to alumina. Clinical relevance. Macroabrasion is a technique for the removal of localized white spots using a high-speed, intermittent high speed turbine finishing diamond tip. It is fast, safe, efficient, and an alternative to enamel microabrasion. However, when the stain is deeper, these localized intrinsic stains or defects can be treated with partial direct veneers. A conservative preparation should be done and that allows stratification of the resin to mask the hypoplasia and provide naturalness to the tooth. Thus, the objective of this work is to demonstrate, through a clinical case, macroabrasion and a partial veneer from the preparation, restoration, to the finishing and polishing to remove hypoplastic stains. Fluorosis is a dental disease caused by excessive intake of fluoride, which can result in white patches or lines on teeth. It can also lead to skeletal fluorosis, which affects the bones and can cause pain and deformities. Diagnosis of fluorosis can be done by a dentist through visual examination. Treatment options for dental fluorosis include tooth bleaching, enamel microabrasion, and composite fillings. There are currently no established treatments for skeletal fluorosis, but reducing fluoride intake and consuming calcium, vitamin C, and vitamin D can help protect against fluoride toxicity. Fluorosis can be prevented by using alternative sources of drinking water or by removing excessive fluoride from water. Pregnant women and nursing mothers should be particularly cautious about fluoride intake to avoid potential harm to the developing child. [Extracted from the article]

Purpose: To measure the softening via swelling of a composite resin by evaluating its surface hardness after the application of four monomer systems.Materials and Methods: A total of 40 specimens made of a hybrid composite resin were evaluated. Four monomer systems were used as surface-conditioning primers during five exposure times. Surface microhardness measurements (n = 8) were made after application of the monomer systems.Results: In all exposure times, Scotchbond Universal showed the highest influence on the composite resin surface softening (P < .05), followed by Stick Resin (P < .05).Conclusion: Surface softening of composite resins might be achieved with monomer systems, especially with Scotchbond Universal, which may have a positive influence when repairing aged composite resin restorations. • Surface effects of alumina particle air abrasion depends strongly on particle morphology. • Air abrasion with 85 μm round-shaped alumina particles is minimally invasive to enamel. • Round-shaped alumina particles might be suitable when removing composite remnants. • Air abrasion with 50 μm sharp-edged alumina particles results in significant volume loss of enamel. To evaluate the volume loss after air abrasion with alumina particles with different morphology on bovine enamel and luting composite resin. Air abrasion was performed on 12 unscathed bovine teeth and 72 luting composite resin discs with 85 μm round-shaped and 50 μm sharp-edged alumina particles applied for 20 s, 40 s and 60 s (n = 12). Air abrasion was standardized by uniform areas of 2 mm diameter, a pressure of 0.25 MPa and a distance of 3 mm. The volume loss was determined by using a laboratory scanner. Air abrasion with round-shaped alumina particles was mostly not measurable with the laboratory scanner, while sharp-edged alumina air abrasion resulted in significant loss of enamel. The median of volume loss by sharp-edged alumina particles ranged from 0.78 mm3 (20 s) to 2.52 mm3 (60 s). In contrast to round-shaped alumina the removal caused by sharp-edged alumina increased significantly with increasing application times (p ≤ 0.05). For air abrasion on luting composite resin the median of the removed volume ranged from 2.25 mm3 (20 s) to 6.18 mm3 (60 s), while round-shaped alumina showed a range from 0.45 mm3 (20 s) to 1.40mm3 (60 s). The round-shaped alumina produced a statistically significant lower volume loss than sharp-edged alumina for all three air abrasion times. The 85 μm round-shaped alumina particles removed less composite resin than 50 μm sharp-edged alumina particles but barely any enamel, making it an option for removing composite resin residues from enamel. Objective: Treatment of implants with peri-implantitis is often unsuccessful due to residual microbial biofilm hindering re-osseointegration. The aim of this study was to treat biofilm-grown titanium (Ti) implants with different modalities involving air abrasion (AA) and cold atmospheric plasma (CAP) to compare the effectiveness in surface decontamination and the alteration/preservation of surface topography. Materials and methods: Saliva collected from a peri-implantitis patient was used to in vitro develop human biofilm over 35 implants with moderately rough surface. The implants were then mounted onto standardized acrylic blocks simulating peri-implantitis defects and treated with AA (erythritol powder), CAP in a liquid medium, or a combination (COM) of both modalities. The remaining biofilm was measured by crystal violet (CV). Surface features and roughness before and after treatment were assessed by scanning electron microscope (SEM). The data were statistically analyzed using Kruskal-Wallis followed by Tukey's multiple comparison test. Results: In the present peri-implantitis model, the human complex biofilm growth was successful as indicated by the statistical significance between the negative and positive controls. All the treatment groups resulted in a remarkable implant surface decontamination, with values very close to the negative control for AA and COM. Indeed, statistically significant differences in the comparison between the positive control vs. all the treatment groups were found. SEM analysis showed no post-treatment alterations on the implant surface in all the groups. Conclusions: Decontamination with AA delivering erythritol with or without CAP in liquid medium demonstrated compelling efficacy in the removal of biofilm from implants. All the tested treatments did not cause qualitative alterations to the Ti surface features. No specific effects of the CAP were observed, although further studies are necessary to assess its potential as monotherapy with different settings or in combination with other decontamination procedures. Clinical relevance: CAP is a promising option in the treatment of peri-implantitis because it has potential to improve the elimination of bacterial plaque from implant surfaces, in inaccessible pockets or during open-flap debridement, and should stimulate the process of the re-osseointegration of affected dental implants by not altering surface features and roughness. To evaluate in vitro transenamel/transdentinal of penetration of H2O2 after microabrasive. Computational models were performed to verify peroxide penetration and evaluate if geometric modifications could affect in vitro results. Enamel/dentin blocks from bovine incisors were prepared and randomly divided into groups (n = 15) according with abrasive (35% phosphoric acid and pumice or 6.6% hydrochloric acid and silica) and bleaching agents (16% carbamide peroxide and 35% H2O2). From artificial pulp chambers, the H2O2 concentration was measured and SEM was used for surface morphology. Numerical models were performed (Abaqus® v6.12) modifying slightly enamel/dentin thickness based on experimental data. All groups presented H2O2 penetration, although no significant difference was noted between the control and experimental groups (p > 0.05). The numerical analysis demonstrated the role of dental tissue thickness in the H2O2 penetration. Microabrasion is recommended as its association with bleaching procedures and small modifications in thicknesses of enamel/dentin can cause substantial changes in HP penetration. Such diffusion aspect is clinically relevant for the bleaching procedures since very thin enamel could present more peroxide propagation through dental tissues. [ABSTRACT FROM AUTHOR]

Aims and objectives: The aim of this study was to assess the efficacy of 18% hydrochloric acid and 37% phosphoric acid by an in vivo comparison. Methods: Sixty fluorotic permanent maxillary central incisors from 30 patients were divided into 3 categories. The teeth received 5 seconds (mild fluorosis), 20 seconds (moderate fluorosis) and 30 seconds (severe fluorosis) application of 18% hydrochloric acid on 11 and 37% phosphoric acid on 21. Standardized intraoral photographies were taken immediately before, after, and one month after treatment. Vinyl polysiloxane impression of the patient were made before and after the treatment. A scanning electron microscopic (SEM) evaluation was carried out on the models to judge the surface alterations. Wilcoxon and Mann-Whitney tests were used to verify the hypothesis. Results: A statistically significant result was obtained in the reduction of white spot opacities, intensity of stains and the total area occupied by the stains in mild and moderate fluorosis teeth. Results of severe fluorosis had an unpredictable outcome. An SEM evaluation revealed good improvement in the surface texture of mild and moderate fluorosis teeth. Teeth with severe fluorosis showed only a slight improvement. Conclusion: A microabrasion procedure is effective for treating mild and moderate fluorosis cases. [ABSTRACT FROM AUTHOR]

Enamel white spot lesions are frequent and can impact patients' quality of life. The most conservative treatment in such cases is microabrasion, a technique that presents some drawbacks. The proposed strategy is not based on the elimination of dysplastic enamel, but on masking the lesion by infiltrating the porous subsurface enamel with a hydrophobic resin that has a refraction index closer to that of sound enamel, after permeating the non-porous surface enamel through hydrochloric acid erosion. Erosion-infiltration approaches have been proposed to treat initial caries, but this report suggests extending it to two novel indications: fluorosis and traumatic hypo-mineralization lesions. Four cases were treated by erosion infiltration following the original protocol. They were followed up clinically at several intervals during a period of 19 months of clinical service. The clinical results, although not perfect, satisfied the patients entirely. Erosion-infiltration could be a promising alternative for minimally invasive treatment in similar situations.
