Abstract In this paper we treat some aspects of the LARES 2 space experiment to test the general relativistic phenomenon of dragging of inertial frames, or frame-dragging, in particular we discuss some aspects of its relative accuracy which can approach one part in a thousand. We then, once again respond to the criticisms of the author of a recent paper about the accuracy in the measurement of frame-dragging with LARES 2. The claims of such a paper are not reproducible in any independent analyses. Indeed, it claims that the accuracy in the test of frame-dragging, which can be reached by the LARES 2 space experiment, is several orders of magnitude larger than previously estimated in a number of papers. Here we show that such a paper is based on a number of significant misunderstandings and conceptual mistakes. Furthermore, it is puzzling to observe that previous papers by the same author contained completely opposite statements about the accuracy which can be reached using two satellites with supplementary inclinations, such as in the LARES 2 space experiment, and in general with laser-ranged satellites.

The integration of WhatsApp (WhatsApp Inc., Mountain View, CA, USA) into the health-care sector is transforming the dynamics of medical communication. Launched in 2009 and incorporated into the Facebook suite of companies in 2014, WhatsApp has become a pivotal platform for health-care professionals. It allows for quick, personal, and group messaging, which is increasingly used to exchange patient information, discuss cases, and make timely decisions. The app’s end-to-end encryption is critical for maintaining patient confidentiality, ensuring that sensitive data are only accessible to the intended recipients. This security feature and the group sharing format enable health-care teams to communicate confidentially and efficiently, bolstering the oversight capabilities of attending senior consultants or physicians over their junior peers. The real-time exchange of information is crucial in providing continuous supervision and facilitating immediate feedback, which is indispensable in clinical training and patient care. However, the widespread adoption of WhatsApp also comes with challenges and potential drawbacks, which have been the focus of past research. These studies assessed the impact of WhatsApp on health-care delivery, considering both the enhancement of communication it offers and the issues it may present, such as data security beyond encryption and the blurring of professional and personal boundaries. In this “medical opinion” from an academic tertiary care center in Muscat, we explored how emergency medicine physicians can effectively use WhatsApp to expedite or bypass referral processes for emergency cases, particularly those involving vascular and other surgical emergencies.

Despite documented cases of baclofen toxicity in individuals with kidney disease, the drug is widely prescribed for various medical conditions, primarily spasticity, hiccups, and multiple sclerosis. Baclofen, a gamma-aminobutyric acid derivative, relies on renal excretion, rendering those with impaired kidney function susceptible to toxicity – a concern often underestimated by health-care providers. Adverse reactions, including single or double doses, are well documented in addition to multi-dose toxicity. This report discusses a case of baclofen-induced neurotoxicity in an end-stage renal disease patient undergoing dialysis, highlighting the subsequent management with continuous venovenous hemodialysis. In addition, it provides a comprehensive review of existing literature on baclofen toxicity in cases of renal insufficiency. Strikingly, the literature lacks clear guidelines regarding baclofen safety, dose adjustments, or renal function thresholds for contraindication. This contribution aims to augment understanding of this critical issue, emphasizing the need for heightened awareness and careful consideration of baclofen use in patients with kidney disease.

Abstract While alterations in nucleus size, shape, and color are ubiquitous in cancer, comprehensive quantification of nuclear morphology across a whole-slide histologic image remains a challenge. Here, we describe the development of a pan-tissue, deep learning-based digital pathology pipeline for exhaustive nucleus detection, segmentation, and classification and the utility of this pipeline for nuclear morphologic biomarker discovery. Manually-collected nucleus annotations were used to train an object detection and segmentation model for identifying nuclei, which was deployed to segment nuclei in H&E-stained slides from the BRCA, LUAD, and PRAD TCGA cohorts. Interpretable features describing the shape, size, color, and texture of each nucleus were extracted from segmented nuclei and compared to measurements of genomic instability, gene expression, and prognosis. The nuclear segmentation and classification model trained herein performed comparably to previously reported models. Features extracted from the model revealed differences sufficient to distinguish between BRCA, LUAD, and PRAD. Furthermore, cancer cell nuclear area was associated with increased aneuploidy score and homologous recombination deficiency. In BRCA, increased fibroblast nuclear area was indicative of poor progression-free and overall survival and was associated with gene expression signatures related to extracellular matrix remodeling and anti-tumor immunity. Thus, we developed a powerful pan-tissue approach for nucleus segmentation and featurization, enabling the construction of predictive models and the identification of features linking nuclear morphology with clinically-relevant prognostic biomarkers across multiple cancer types.

BackgroundDigital mental health tools, designed to augment traditional mental health treatments, are becoming increasingly important due to a wide range of barriers to accessing mental health care, including a growing shortage of clinicians. Most existing tools use rule-based algorithms, often leading to interactions that feel unnatural compared with human therapists. Large language models (LLMs) offer a solution for the development of more natural, engaging digital tools. In this paper, we detail the development of Socrates 2.0, which was designed to engage users in Socratic dialogue surrounding unrealistic or unhelpful beliefs, a core technique in cognitive behavioral therapies. The multiagent LLM-based tool features an artificial intelligence (AI) therapist, Socrates, which receives automated feedback from an AI supervisor and an AI rater. The combination of multiple agents appeared to help address common LLM issues such as looping, and it improved the overall dialogue experience. Initial user feedback from individuals with lived experiences of mental health problems as well as cognitive behavioral therapists has been positive. Moreover, tests in approximately 500 scenarios showed that Socrates 2.0 engaged in harmful responses in under 1% of cases, with the AI supervisor promptly correcting the dialogue each time. However, formal feasibility studies with potential end users are needed. ObjectiveThis mixed methods study examines the feasibility of Socrates 2.0. MethodsOn the basis of the initial data, we devised a formal feasibility study of Socrates 2.0 to gather qualitative and quantitative data about users’ and clinicians’ experience of interacting with the tool. Using a mixed method approach, the goal is to gather feasibility and acceptability data from 100 users and 50 clinicians to inform the eventual implementation of generative AI tools, such as Socrates 2.0, in mental health treatment. We designed this study to better understand how users and clinicians interact with the tool, including the frequency, length, and time of interactions, users’ satisfaction with the tool overall, quality of each dialogue and individual responses, as well as ways in which the tool should be improved before it is used in efficacy trials. Descriptive and inferential analyses will be performed on data from validated usability measures. Thematic analysis will be performed on the qualitative data. ResultsRecruitment will begin in February 2024 and is expected to conclude by February 2025. As of September 25, 2024, overall, 55 participants have been recruited. ConclusionsThe development of Socrates 2.0 and the outlined feasibility study are important first steps in applying generative AI to mental health treatment delivery and lay the foundation for formal feasibility studies. This case report elucidates the presentation and management of a 54-year-old man with type A aortic dissection (TAAD) in the emergency department. The patient, a known diabetic with uncontrolled hypertension and a history of heavy smoking, presented with severe central chest pain radiating to the back, accompanied by diaphoresis. Clinical evaluation revealed tachycardia and normotension. An immediate computed tomography angiogram confirmed the diagnosis of TAAD. The patient received timely medical intervention with intravenous beta-blockers and was prepared for a surgical consultation. This report underscores the critical importance of rapid diagnosis, effective blood pressure control, and multidisciplinary collaboration in the management of TAAD in the emergency setting, emphasizing contemporary evidence-based practices. While the article omits details on operative procedures, it notes that the patient was discharged in stable condition postsurgery, and at the 3-month follow-up, he is progressing well.

Background: Kidney transplantation is widely recognized as the primary treatment for those with end-stage renal disease. Our goal was to provide an overview of the clinical characteristics of postrenal transplant patients (commercial vs. noncommercial) seeking care in the emergency department (ED). Methodology: The study was conducted at a leading Omani ED from 2015 to 2021; this study included all postrenal transplant patients. Various variables were categorized, coded, and analyzed. Results: A total of 55 postrenal transplant patients were included in our study. Of these, 60.0% were male (n = 33). Among the participants, 14.5% (n = 8) had undergone noncommercial renal transplants, whereas the remaining 85.5% (n = 47) had undergone commercial renal transplants. Among noncommercial postrenal transplant patients, the most common reasons for ED visits were fever (5), gastrointestinal symptoms (4), and cardiac symptoms (3). They also presented with pain at the surgical site (2) and urological symptoms such as dysuria and hematuria (2). In contrast, the most frequent clinical presentations among commercial postrenal transplant patients were cardiac symptoms (13) and pain at the surgical site (13), followed by fever (12). Noncommercial renal transplant patients had a high discharge rate of 62.5% (5) and a low admission rate of 37.5% (3). On the other hand, commercial renal transplant patients necessitated a high admission rate of 59.6% (28) and a low discharge rate of 40.4% (19). Conclusion: The majority of patients had received commercial kidney transplants, and the most common complaints upon their presentation to the ED were cardiac symptoms and pain at the surgical site. Patients undergoing commercial transplants had higher rates of admission.

Cancer-associated cognitive deficits following chemotherapy have received increased attention in clinical research. Exercise has been shown to preserve cognitive function in cancer patients, though the overall effect is mixed. Here we present a scoping review of the published literature summarizing methods used to assess cognitive function in exercise oncology trials. Methods: PubMed, PsycINFO and CINAHL databases were searched using keywords “cognition,” “cancer” OR “neoplasm” OR “tumor,” “chemotherapy” and “exercise” OR “physical activity.” Studies eligible for inclusion include prospective studies that were published in English in peer-reviewed journals that include a method of assessing cognitive function in adult cancer patients, in which an exercise modality or method of quantifying exercise habits was evident. Studies were excluded if they included a pediatric population, patients that were not diagnosed with cancer, or were systematic/narrative/scoping reviews, protocol papers or dissertation/theses. Results: A total of 29 studies met the inclusion criteria. In total, 29 unique assessments were used to evaluate cognitive function, including patient-reported outcomes (PROs; n = 8) and objective (n = 21) methods. More than half (n = 17) of included studies relied on PROs while 12 studies utilized objective measures of cognitive function Cognitive domains of the PROs were limited in scope, focusing on memory and attention/concentration while the objective measures were broader and inclusive of multiple domains. Conclusion: The results of this review indicate that mixed approaches to evaluating cognitive function in cancer patients pose a major limitation to understanding the role of exercise as an integrative approach. The evidence demonstrates a need for more uniform assessment of cognitive function in exercise oncology trials.

This report presents a compelling case of remote site hemorrhage (RSH), a rare but severe complication associated with neurosurgery. RSH involves cerebral bleeding away from the surgical site and was first documented in 1937 by Van Gehuchten. Despite its rarity, RSH remains challenging, affecting less than 1% of cases, with an unclear cause. The case involves a 67-year-old male who developed severe symptoms within 24 hours post-surgery. The initial computed tomography scan showed an acute subdural hematoma. Surgical evacuation was followed by rapid deterioration, leading to multiple RSH and brain stem infarctions. Unfortunately, the patient did not survive. RSH poses significant morbidity and mortality risks. Potential factors include volume loss, dural opening, and blood pressure fluctuations. Management ranges from conservative approaches to surgery, with poor prognosis post-RSH intervention. This case highlights the need for thorough preoperative assessment and careful intraoperative management. It emphasizes the complexities of neurosurgery and underscores the importance of ongoing research for managing rare complications like RSH, ultimately improving patient outcomes.

OBJECTIVES: In the context of acute hypoxemic respiratory failure (AHRF), ensuring effective preoxygenation and apneic oxygenation emerges as the pivotal approach ensuring for averting hypoxemic adverse events during endotracheal intubation. To investigate this, we conducted an open-label randomized controlled trial, aiming to assess the comparative effectiveness of nasopharyngeal high-flow oxygenation in conjunction with Bag-Valve-Mask (BVM) versus standard BVM preoxygenation in patients experiencing AHRF within the emergency department (ED). METHODS: This prospective single-center, open-labeled, randomized controlled trial enrolled patients aged 18 years and above requiring rapid sequence intubation due to AHRF in the ED. Participants were randomly assigned in a 1:1 ratio to either the intervention arm (involving nasopharyngeal high-flow oxygenation and BVM preoxygenation) or the control arm (involving BVM preoxygenation alone). RESULTS: A total of 76 participants were enrolled in the study, evenly distributed with 38 individuals in each arm. Median (interquartile range [IQR]) SpO2 at 0 min postintubation was 95.5 (80%–99%) versus 89 (76%–98%); z-score: 1.081, P = 0.279 in the intervention and control arm, respectively. The most common postintubation complications included hypoxia (intervention arm: 56.7% vs. control arm: 66.7%) and circulatory/hypoxic arrest (intervention arm: 39.5% vs. control arm: 44.7%). There were no adverse complications in 36.7% (n = 11) of patients in the intervention arm. Despite the best possible medical management, almost half (52.6%) of patients in the intervention arm and 47.4% of patients in the control arm succumbed to their illnesses in the ED. CONCLUSION: The primary outcome revealed no statistically significant difference between the two arms. However, patients in the intervention arm exhibited fewer intubation-related adverse effects.

Abstract Breast cancer is one of the most common cancers in women and the second foremost cause of cancer death in women after lung cancer. Recent technological advances in breast cancer treatment offer hope to millions of women in the world. Segmentation of the breast’s Dynamic Contrast-Enhanced Magnetic Resonance Imaging (DCE-MRI) is one of the necessary tasks in the diagnosis and detection of breast cancer. Currently, a popular deep learning model, U-Net is extensively used in biomedical image segmentation. This article aims to advance the state of the art and conduct a more in-depth analysis with a focus on the use of various U-Net models in lesion detection in women’s breast DCE-MRI. In this article, we perform an empirical study of the effectiveness and efficiency of U-Net and its derived deep learning models including ResUNet, Dense UNet, DUNet, Attention U-Net, UNet++, MultiResUNet, RAUNet, Inception U-Net and U-Net GAN for lesion detection in breast DCE-MRI. All the models are applied to the benchmarked 100 Sagittal T2-Weighted fat-suppressed DCE-MRI slices of 20 patients and their performance is compared. Also, a comparative study has been conducted with V-Net, W-Net, and DeepLabV3+. Non-parametric statistical test Wilcoxon Signed Rank Test is used to analyze the significance of the quantitative results. Furthermore, Multi-Criteria Decision Analysis (MCDA) is used to evaluate overall performance focused on accuracy, precision, sensitivity, F $$_1$$ 1 -score, specificity, Geometric-Mean, DSC, and false-positive rate. The RAUNet segmentation model achieved a high accuracy of 99.76%, sensitivity of 85.04%, precision of 90.21%, and Dice Similarity Coefficient (DSC) of 85.04% whereas ResNet achieved 99.62% accuracy, 62.26% sensitivity, 99.56% precision, and 72.86% DSC. ResUNet is found to be the most effective model based on MCDA. On the other hand, U-Net GAN takes the least computational time to perform the segmentation task. Both quantitative and qualitative results demonstrate that the ResNet model performs better than other models in segmenting the images and lesion detection, though computational time in achieving the objectives varies.

Background: Due to a myriad of risk factors, epistaxis is a very frequent presentation to the emergency room (ER). This study aims to ascertain the prevalence of epistaxis in our population, risk factors, effectiveness of ER treatment, complications, and ER outcome. Materials and Methods: This was a prospective observational study performed in the ER of a referral tertiary care center in south India. Data were categorized, coded, and analyzed to determine the objective of the study. Results: During the study's six-month duration, 188 (0.6%) patients presented with epistaxis. The mean age was 42.9 (SD: 16.49) years, with a male preponderance of 143 (76.1%). A majority of these patients (n: 156; 82.9%) were triaged as priority II, with hypertension (n: 53, 28.2%) as the commonest comorbidities. Trauma-related epistaxis (n: 107, 56.9%) was the most frequent cause. Anterior nasal packing was carried out for 85 (45.2%) patients, posterior nasal packing was carried out for one (0.5%) patient, and bleeding had spontaneously resolved in the majority (n: 102; 54.3%) patients. Seven (3.7%; p-value: 0.001) patients had recurrent epistaxis, and of those, three (1.6%) required urgent resuscitation with crystalloid fluid and blood products. Two of these patients had bleeding dyscrasias, four had history of trauma, and one patient presented with uncontrolled hypertension. Two (1.1%) patients came back to us with recurrent bleeding within 12 h of discharge. Majority (69.2%; 130) were discharged stable, (23.9%; 45) were admitted for observation and (6.9%; 13) were discharged against medical advice. There was no mortality among these study populations. Conclusion: Middle-young, aged males most commonly presented with epistaxis. Most of them were secondary to trauma. Anterior nasal bleeding was the most common source and hemostasis could be obtained by anterior nasal packing. Majority could be discharged stable from the ER. However, this cohort had seen patients in life-threatening conditions, so the severity cannot be overlooked.

Introduction: Bone tumors, characterized by diverse locations and shapes, often necessitate surgical excision followed by custom implant placement to facilitate targeted bone reconstruction. Leveraging additive manufacturing, patient-specific implants can be precisely tailored with complex geometries and desired stiffness, enhancing their suitability for bone ingrowth.Methods: In this work, a finite element model is employed to assess patient-specific lattice implants in femur bones. Our model is validated using experimental data obtained from an animal study (n = 9).Results: The results demonstrate the accuracy of the proposed finite element model in predicting the implant mechanical behavior. The model was used to investigate the influence of reducing the elastic modulus of a solid Ti6Al4V implant by tenfold, revealing that such a reduction had no significant impact on bone behavior under maximum compression and torsion loading. This finding suggests a potential avenue for reducing the endoprosthesis modulus without compromising bone integrity.Discussion: Our research suggests that employing fully lattice implants not only facilitates bone ingrowth but also has the potential to reduce overall implant stiffness. This reduction is crucial in preventing significant bone remodeling associated with stress shielding, a challenge often associated with the high stiffness of fully solid implants. The study highlights the mechanical benefits of utilizing lattice structures in implant design for enhanced patient outcomes.

Objective: This study investigates the effects of a behavioral lifestyle intervention on inflammatory cytokines and frailty in older adults (≥ 65 years) with type 2 diabetes (T2D). Background: Conflicting data are demonstrating the superiority between Clonidine and Magnesium Sulphate in attenuating hemodynamic response to pneumoperitoneum. Methods and Methodology: This was a prospective randomized blinded trial comparing the efficacy of injection Clonidine 1.5 μg/kg (group A) and Magnesium Sulphate (MgSO4) 50 mg/kg (group B) done over six. Results: Sixty-eighty patients were included and assigned randomly to two groups of 34 (50.0%) patients. The mean (SD) age in groups A and B was 34.32 (11.35) and 37.20 (10.17) years respectively. Comparison of systolic blood pressure and mean arterial pressure were similar in both groups. The heart rate (HR) of patients in group A with group B, was almost the same at pre-induction and the end of infusion while HR was noted to be higher in group B, compared to group A at 5 minutes. Comparisons of EtCO2 (mm Hg) and SpO2 (%) between group A and group B were comparable in both the groups at pre-induction, at 5 min, 10 min, 15 min, 20 min, 25 min, 30 min, 35 min and 40 min. However, at the end of the infusion SpO2 levels were higher in Group A (p = 0.004). According to Modified Ramsay Sedation Score, half of the patients (50.0%) from group A were cooperative, oriented, and tranquil. In Group B, about one-third (38.2%) were cooperative, oriented, and tranquil. Immediate postoperative complications included nausea (8.8%) and vomiting (2.9%) which were comparable between both groups. Conclusion: Overall observations suggested that Clonidine (1.5 μg/kg) was equally effective as MgSO4 (50 mg/kg) in attenuating hemodynamic responses during pneumoperitoneum in laparoscopic cholecystectomy.

Functional electrical stimulation (FES) is a vital method in neurorehabilitation used to reanimate paralyzed muscles, enhance the size and strength of atrophied muscles, and reduce spasticity. FES often leads to increased muscle fatigue, necessitating careful monitoring of the patient’s response. Ultrasound (US) imaging has been utilized to provide valuable insights into FES-induced fatigue by assessing changes in muscle thickness, stiffness, and strain. Current commercial FES electrodes lack sufficient US transparency, hindering the observation of muscle activity beneath the skin where the electrodes are placed. US-compatible electrodes are essential for accurate imaging and optimal FES performance, especially given the spatial constraints of conventional US probes and the need to monitor muscle areas directly beneath the electrodes. This study introduces specially designed body-conforming US-compatible FES (US-FES) electrodes constructed with a silver nanowire/polydimethylsiloxane (AgNW/PDMS) composite. We compared the performance of our body-conforming US-FES electrode with a commercial hydrogel electrode. The findings revealed that our US-FES electrode exhibited comparable conductivity and performance to the commercial one. Furthermore, US compatibility was investigated through phantom and in vivo tests, showing significant compatibility even during FES, unlike the commercial electrode. The results indicated that US-FES electrodes hold significant promise for the real-time monitoring of muscle activity during FES in clinical rehabilitative applications.

Introduction: Stress shielding is a common complication following endoprosthetic reconstruction surgery. The resulting periprosthetic osteopenia often manifests as catastrophic fractures and can significantly limit future treatment options. It has been long known that bone plates with lower elastic moduli are key to reducing the risk of stress shielding in orthopedics. Inclusion of open space lattices in metal endoprostheses is believed to reduce the prosthesis modulus potentially improving stress shielding. However, no in vivo data is currently available to support this assumption in long bone reconstruction. This manuscript aims to address this hypothesis using a sheep model of extraarticular bone defect.Methods: Initially, CT was used to create a virtual resection plan of the distal femoral metaphyses and to custom design endoprostheses specific to each femur. The endoprostheses comprised additively manufactured Ti6Al4V-ELI modules that either had a solid core with a modulus of ∼120 GPa (solid implant group) or an open space lattice core with unit cells that had a modulus of 3–6 GPa (lattice implant group). Osteotomies were performed using computer-assisted navigation followed by implantations. The periprosthetic, interfacial and interstitial regions of interest were evaluated by a combination of micro-CT, back-scattered scanning electron microscopy (BSEM), as well as epifluorescence and brightfield microscopy.Results: In the periprosthetic region, mean pixel intensity (a proxy for tissue mineral density in BSEM) in the caudal cortex was found to be higher in the lattice implant group. This was complemented by BSEM derived porosity being lower in the lattice implant group in both caudal and cranial cortices. In the interfacial and interstitial regions, most pronounced differences were observed in the axial interfacial perimeter where the solid implant group had greater bone coverage. In contrast, the lattice group had a greater coverage in the cranial interfacial region.Conclusion: Our findings suggest that reducing the prosthesis modulus by inclusion of an open-space lattice in its design has a positive effect on bone material and morphological parameters particularly within the periprosthetic regions. Improved mechanics appears to also have a measurable effect on the interfacial osteogenic response and osteointegration.

Background: During the COVID-19 pandemic, many patients presented to the emergency department (ED) with features of Influenza-like illnesses (ILI) and with other atypical presentations. This study was done to determine the etiology, co-infections, and clinical profile of patients with ILI. Methods: This prospective observational study included all patients presenting to the ED with fever and/or cough, breathing difficulty, sore throat, myalgia, gastrointestinal complaints (abdominal pain/vomiting/diarrhea), loss of taste and altered sensorium or asymptomatic patients who resided in or travelled from containment zones, or those who had contact with COVID-19 positive patients during the first wave of the pandemic between April and August 2020. Respiratory virus screening was done on a subset of COVID-19 patients to determine co-infection. Results: During the study period, we recruited 1462 patients with ILI and 857 patients with the non-ILI presentation of confirmed COVID-19 infection. The mean age group of our patient population was 51.4 (SD: 14.9) years with a male predominance (n-1593; 68.7%). The average duration of symptoms was 4.1 (SD: 2.9) days. A sub-analysis to determine an alternate viral etiology was done in 293 (16.4%) ILI patients, where 54 (19.4%) patients had COVID 19 and co-infection with other viruses, of which Adenovirus (n-39; 14.0%) was the most common. The most common symptoms in the ILI-COVID-19 positive group (other than fever and/or cough and/or breathing difficulty) were loss of taste (n-385; 26.3%) and diarrhea (n- 123; 8.4%). Respiratory rate (27.5 (SD: 8.1)/minute: p-value < 0.001) and oxygen saturation (92.1% (SD: 11.2) on room air; p-value < 0.001) in the ILI group were statistically significant. Background: Emergency department (ED) revisits are an important measure of health-care quality provided by any medical center. This study analyzes the profile and outcome of patients revisiting the ED within 72 h at a tertiary care center in South India. Methods: This was a retrospective cohort study done on all patients presenting to ED within 72 h of their first visit over 5 months. Unavoidable revisits and the factors associated with them were categorized, coded, and analyzed. Results: The ED revisit rate was 2.9% (900/30409). Based on standard triaging criteria, patients during their first visit were triaged as follows: 8.2% as priority 1, 32% as priority 2, and 58.2% as priority 3. During the revisit, they were triaged as follows: 12.9% (+3.7% increase) as priority 1, 36.9% (+4.9% increase) as priority 2 and 49.1% (-9.1% decrease) as priority 3. Approximately a quarter (27%) of the patients had to be prioritized higher on their revisit. Abdominal pain (18.3%), vomiting/diarrhea (11.7%), bleeding from the previous wound site (10.5%), and fever (9%) were the most common presenting complaints. Avoidable revisits included 13.4% (n = 121) patients. Multivariate logistic regression analysis showed age >40 years (adjusted odds ratio [OR]: 1.60; 95% confidence interval [CI]: 1.04–2.46; P: 0.031) and abdominal pain (adjusted OR: 2.07;95% CI: 1.40–3.08; P: 40 years (adjusted OR: 3.53; 95% CI: 2.12–5.88; P: 40 years were found to be associated with higher odds of an unavoidable revisit. In addition, age >40 years and fever were associated with sicker patients on revisits.

Objectives: The prevalence of burnout is progressively rising among medical students. The objective of this study was to evaluate the occurrence and determinants of burnout among preclinical medical students in Muscat. Methods: This cross-sectional analytical research was conducted at one of Oman's top medical universities from October 2019 to April 2020. A cohort of 350 preclinical medical students in their 2nd, 3rd, and 4th years of study participated in this study. The assessment of burnout was based on self-reports from the participants, using the Maslach Burnout Inventory-Student Survey. Various variables were coded and analyzed. Results: A total of 350 students completed the survey (response rate: 83.3%). Burnout was reported by 125 (35.7%) students. Regarding specific domains of burnout, most students demonstrated higher rates of cynicism (n: 236; 67.4%) and emotional exhaustion (n: 221; 63.1%) and lower rates of academic efficacy (n: 197; 56.3%). Burnout was significantly more common among older students (p-value < 0.001), students living apart from their family (p-value: 0.042), students who did not participate in regular exercises (p-value < 0.001), those who spent fewer hours studying per day (p-value < 0.001), those who regretted their decision to study medicine (p-value: 0.023), those who were dissatisfied with examination results (p-value < 0.001), teaching strategies (p-value: 0.029), those with lower cumulative grade point averages (p-value < 0.001), and those with a history of academic probation (p-value < 0.001). Conclusions: Preclinical medical students displayed high rates of burnout. Continuous evaluation programs and educational initiatives are recommended to help such students to develop better burnout management and coping strategies.

Background: Health-related quality of life is rapidly becoming recognized as an important indicator of how a disease affects patient lives and for evaluating the quality of care, especially for chronic conditions such as chronic kidney disease (CKD). Objectives: This study is an attempt to assess the quality of life in patients with chronic kidney disease at MMIMSR and also identify characteristics that may be associated with their worsening quality of life. Materials and Methods: This cross-sectional investigation was conducted at the in-patient department (IPD) of the MMIMSR hospital. This study included 105 CKD patients and used a systematic random sampling method for quantitative analysis. This study utilized a 36-item short-form SF-36 (v1.3) questionnaire to assess HRQoL in CKD patients. Descriptive statistics were employed at the baseline. Chi square and ANOVA were used to draw comparisons between two groups or more than two groups, respectively. Logistic regression analysis was utilized to identify the potential QoL determinants. A p value of 0.05 or lower was used to determine statistical significance. Results: Among a total of 105 participants, the mean (±standard deviation) age was found to be 54.53 ± 13.47 years; 48 were male patients, and 57 were female patients. Diabetes Mellitus (61.9%), hypertension (56.2%), chronic glomerulonephritis (7.6%), chronic pyelonephritis (6.7%), and polycystic kidney disease (5.7%) were identified to be the most frequent disorders associated with CKD. The current study also demonstrated that the HRQoL score domains such as symptom problem list, the effect of kidney disease, and the burden of kidney disease decline significantly and progressively as the patient advances into higher stages of CKD (p = 0.005). A similar pattern was observed in work status, sleep, and general health (p < 0.005). Additionally, a statistically significant difference was noted for cognitive function, quality of social interaction, overall health, dialysis staff encouragement, patient satisfaction, social support, physical functioning, role of physical health, pain, emotional well-being, role of emotional health, social functioning, and energy fatigue (p < 0.005). The mean difference for PCS and MCS based on CKD stages was found to be statistically significant (p < 0.005). The PCS and MCS showed a positive correlation with GFR (r = 0.521), and Hb (r = 0.378), GFR (r = 0.836), and Hb (r = 0.488), respectively. Conclusions: The findings of this study demonstrated that a significant decrease in HRQoL was observed among CKD patients, with a progressive deterioration of HRQoL dimensions as the patient advances to end-stage renal disease. This study also revealed that CKD imposes various restrictions on patients’ day-to-day lives, particularly in terms of their physical and mental functioning, even in the initial stages of the disease.

Takayasu arteritis (TAK) could cause a stroke or transient ischemic attack (TIA) in young individuals due to inflammatory vascular occlusion or intracerebral hemorrhage. We compared the clinical presentation, angiographic features, longitudinal patterns of disease activity, medical treatments, and survival in 34 TAK patients with stroke/TIA and 157 without stroke/TIA from a single-center retrospective cohort. TAK patients with stroke/TIA were older (p = 0.044) with a greater proportion of males (p = 0.022), more frequent vision loss (odds ratio (OR) for stroke/TIA vs. without stroke TIA 5.21, 95% CI 1.42–19.14), and less frequent pulse or blood pressure inequality (OR 0.43, 95% CI 0.19–0.96) than TAK patients without stroke/TIA. Hata’s angiographic type IIa was more common in TAK patients with stroke/TIA (OR 11.00, 95%CI 2.60–46.58) and type V in TAK patients without stroke/TIA (OR 0.27, 95% CI 0.12–0.58). Cyclophosphamide was used more often in TAK patients with stroke/TIA (p = 0.018). Disease activity at baseline, 6, 12, and 24 months of follow-up was mostly similar for both groups. Risk of mortality was similar in TAK patients with or without stroke/TIA (hazard ratio unadjusted 0.76, 95% CI 0.15–3.99; adjusted for gender, age of disease onset, delay to diagnosis, baseline disease activity, and the number of conventional or biologic/targeted synthetic immunosuppressants used 1.38, 95% CI 0.19–10.20) even after propensity score-matched analyses. Stroke or TIA does not appear to affect survival in TAK patients adversely.

The excessive depletion of fossil fuels and increasing environmental concerns have led to the need to explore alternative sources of power for aircraft. This has spurred various stakeholders in the aerospace industry to explore hybrid electric propulsion technology and fully electric vehicles. Airships are aerial platforms based on lighter-than-air systems technology. They have several unique features compared to other vehicles, chiefly their being more environmentally friendly due to low fuel consumption. Among airships, lifting-body dynastats are the most suitable configuration for implementing different levels of hybridization in propulsion systems owing to their large surface-to-volume ratio. The present study deals with the relevance of a hybrid propulsion (conventional engine + electric motor) system and its comparison to conventional ones. An objective function based on envelope volume is formulated to achieve an optimal configuration of a tri-lobed dynastat to carry 10 tons of payload over a 500 km range for specified operating conditions powered by conventional fuel and batteries. The design space is explored assuming a predicted future battery technology level with specific energies ranging from 250 to 750 Wh/kg. Three case studies based on the source of power are investigated: fuel alone, fuel + batteries, and fuel + batteries + solar array. It is seen that the airship can be fully electric with zero carbon emissions but at the expense of a longer length (+18%) and higher envelope volume (+63%) compared to the baseline model.

The practice of Deep Convolution neural networks in the field of medicine has congregated immense success and significance in present situations. Previously, researchers have developed numerous models for detecting abnormalities in musculoskeletal radiographs of upper extremities, but did not succeed in achieving respectable accuracy in the case of finger radiographs. A novel deep neural network-based hybrid architecture named ComDNet-512 is proposed in this paper to efficiently detect the bone abnormalities in the musculoskeletal radiograph of a patient. ComDNet-512 comprises a three-phase pipeline structure: compression, training of the dense neural network, and progressive resizing. The ComDNet-512 hybrid model is trained with finger radiographs samples to make a binary prediction, i.e., normal or abnormal bones. The proposed model showed phenomenon outcomes when cross-validated on the testing samples of arthritis patients and gives many superior results when compared with state-of-the-art practices. The model is able to achieve an area under the ROC curve (AUC) equal to 0.894 (sensitivity = 0.941 and specificity = 0.847). The Precision, Recall, F1 Score, and Kappa values, recorded as 0.86, 0.94, 0.89, and 0.78, respectively, are better than any of the previous models’. With an increasing appearance of enormous cases of musculoskeletal conditions in people, deep learning-based computational solutions can play a big role in performing automated detections in the future.

Glioblastoma multiforme (GBM) is the most aggressive glioma of the primary central nervous system. Due to the lack of effective treatment options, the prognosis for patients remains bleak. Fibroblast activation protein alpha (FAP), a 170 kDa type II transmembrane serine protease was observed to be expressed on glioma cells and within the glioma tumor microenvironment. To understand the utility of targeting FAP in this tumor type, the immuno-PET radiopharmaceutical [89Zr]Zr-Df-Bz-F19 mAb was prepared and Lindmo analysis was used for its in vitro evaluation using the U87MG cell line, which expresses FAP endogenously. Lindmo analysis revealed an association constant (Ka) of 10−8 M−1 and an immunoreactivity of 52%. Biodistribution studies in U87MG tumor-bearing mice revealed increasing radiotracer retention in tumors over time, leading to average tumor-to-muscle ratios of 3.1, 7.3, 7.2, and 8.3 at 2, 24, 48 and 72 h, respectively. Small animal PET corroborated the biodistribution studies; tumor-to-muscle ratios at 2, 24, 48, and 72 h were 2.0, 5.0, 6.1 and 7.8, respectively. Autoradiography demonstrated accumulated activity throughout the interior of FAP+ tumors, while sequential tumor sections stained positively for FAP expression. Conversely, FAP− tissues retained minimal radioactivity and were negative for FAP expression by immunohistochemistry. These results demonstrate FAP as a promising biomarker that may be exploited to diagnose and potentially treat GBM and other neuroepithelial cancers.

Introduction: Compound fractures of tibia have been conventionally managed by debridement and external fixation, followed by intra medullary nailing or Ilizarov technique after few months. Definitive surgery is delayed in the early stages, as it may cause infections and additional soft tissue damage. But external fixators have their own set of complications. Hence a technique of antibiotic coated intramedullary nail (locked or unlocked) has been advocated. Methodology: Total 30 patients with grade 2 & 3 A (Gustilo-Anderson) compound tibia fractures were selected for the study. Group 1 was operated with antibiotic coated intramedullary nails. Group 2 was operated with external fixators application and later with intra medullary nailing. Results: The infection rate after nailing was 6.67% in group 1 (1out of 15) and 40% (6 out of 15) in group 2 (p=0.02 i.e. statistically significant).The timing to healing (cortical bridging) was 4.5 months (2 - 8.5months) in group 1 and 7.5 (3-18) months in group 2. (p=0.02i.e. statistically significant).The overall complication rate was 60% (9/15 cases) for group 2 and 26.67% (4/15 cases) for group 1 (p=0.03 i.e. statistically significant).Results of primary nailing with antibiotic impregnated cement are superior to delayed nailing after initial external fixation. Conclusion: Primary nailing with antibiotic impregnated cement method is biologically active with greater advantages compared with existing methods of treatment in grade 2 & 3A compound tibia fractures. Clinical results are better with a lower infection rate and a less hospital stay.

Background: Tibial condyle fractures in young adults or in old age are difficult to manage conservatively and often require operative management. The present study was conducted with an objective to compare the results of Open Reduction Internal Plate Fixation (ORIF) Technique and Minimally Invasive Percutaneous Plate Osteosynthesis (MIPPO) technique for Schatzker IV-VI tibial plateau fractures. Methods: The case records of 42 cases of schatzker IV-VI type tibial fracture were analyzed retrospectively. Among these 42 patients 21 received surgical treatments in the form of ORIF with plate fixation and other 21 cases received MIPPO after 7-10 days after injury. The surgical time, complication, length of hospital stay and cost of hospitalization were analyzed and compared between two groups. The purpose of this paper is to report the variance in institutional review board (IRB) reviews as part of the implementation of a multisite, quality improvement study through the Improvement Science Research Network (ISRN) and recommend strategies successful in procuring timely IRB approval. Using correspondence documents as data sources, the level of review was identified and time to submission, time to approval, and time to study start were analyzed. Thirteen of the 14 IRBs conducted independent reviews of the project. Twelve IRBs approved the study through expedited review while two IRBs reviewed the project at a full board meeting. Lastly, 11 of the 14 sites required documented consent. The greatest delay in approval was seen early on in the IRB process with site PIs averaging 45.1 ± 31.8 days to submit the study to the IRB. IRB approvals were relatively quick with an average of 14 ± 5.7 days to approval. The delay in study submission may be attributed to a lack of clear definitions and differing interpretations of the regulations that challenge researchers.

We report a case of an 18-year-old girl diagnosed to have Moyamoya disease (MMD), who underwent bilateral encephalo-duro-arterio-myo-synangiosis. Literature search has clearly inferred that in comparison to an adult patient, children with MMD can have a good prognosis if early diagnosis and active surgical intervention are achieved. Evidence has demonstrated that active surgical management, including indirect bypass surgery, may improve the cerebral circulation on a relatively larger scale than direct bypass surgeries, which can only improve the cerebral circulation in the area of the vascular reconstruction.

Complete thrombosis of the aorta in patients with abdominal aneurysms is extremely rare. The risk factors, natural history, and rupture risk are not well known. We report a case of a complete aortic thrombosis in a ruptured infrarenal mycotic aneurysm caused due to infection with Burkholderia pseudomallei.

This study investigate the sources, seasonal variation, and health risks of ten elements in water collected at ten sites in Yamuna, Delhi. Mean concentration of the elements ranged from 0.8 ± 0.8 µg/L for Co to 9455 ± 6379 µg/L for Fe, and followed in the order Co>Cd >As>Zn>Ni>Cu>Cr>Pb>Mn>Fe. Mean Cr and Pb levels exceed the WHO prescribed limit. Spatial variation was insignificant except for Mn. Seasonal variation of Pb, Cr, Fe, Zn, and As was significant, and in the order summer≥monsoon>winter. Cluster analysis identified five groups of elements. Principal component analysis indicates industrial, agricultural, and domestic wastewater effluents as the chief sources of the observed level of elements. Probability of non-carcinogenic health risk was moderate for Cr (69%), Mn (62%), As (44%), and Fe (29%) and high for Pb (96%). Cr, Ni, Cd, As and Pb pose carcinogenic risk with high probability (84–100%) except for Pb (4.8%). [ABSTRACT FROM AUTHOR]
