Objectives: To investigate remission with ultrasound (US) in patients with Rheumatoid arthritis (RA) according to different clinical remission criteria. Methods: A total of 105 patients with RA who were in remission for at least 6 months according to disease activity score in the 28 joints using C-reactive protein (DAS28-CRP) were included in the study. US remission rates were analyzed according to different remission criteria [DAS28-CRP, DAS28 using erythrocyte sedimentation rate (DAS28-ESR), clinical disease activity index (CDAI), simplified DAI (SDAI), and the 2011 American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) Boolean remission criteria]. US remission was determined as power doppler (PD) US score = 0. Results: Remission rates achieved for each remission criteria were 100%, 82.9%, 55.2%, 58.1% and 42.9% and US remission rates were 57.1%, 57.5%, 53.4%, 55.7%, 57.7% for DAS28 CRP, DAS 28 ESR, CDAI, SDAI, 2011 ACR/EULAR remission criteria, respectively. When the patients compared for the US findings between remission and non-remission patients according to the different clinical remission criteria, no difference was found (p > 0.05). Conclusions: This study shows that clinical remission criterias are not sensitive enough to accurately detect remission and there was no increase in the US remission rates as per the stricter remission criteria. Using US in addition to the clinical criteria would prove to be more useful in evaluating remission.

Objectives The aim of this study was to evaluate the impact of obesity on the treatment response to secukinumab and drug survival rate in patients with ankylosing spondylitis (AS). Methods We performed an observational cohort study that included AS patients based on the biological drug database in Turkey (TURKBIO) Registry between 2018 and 2021. The patients were divided into three groups: normal [body mass index (BMI) < 25 kg/m(2)], overweight (BMI: 25-30 kg/m(2)), and obese (BMI & GE; 30 kg/m(2)). Disease activity was evaluated at baseline, 3, 6, and 12 months. Drug retention rates at 12 months were also investigated. Results There were 166 AS patients using secukinumab (56.6% male, mean age: 44.9 & PLUSMN; 11.6 years). The median follow-up time was 17.2 (3-33.2) months. Forty-eight (28.9%) patients were obese. The mean age was higher in the obese group than in others (P = .003). There was no statistically significant difference in Bath Ankylosing Spondylitis Disease Activity Index 50, Assessment of SpondyloArthritis international Society 20 (ASAS20), ASAS40, Ankylosing Spondylitis Disease Activity Score (ASDAS) low disease activity, and ASDAS clinically important improvement responses between the three groups at 3, 6, and 12 months, although they were numerically lower in obese patients. Drug retention rates at 12 months were similar in all groups (P > .05). Conclusions This study suggested that obesity did not affect secukinumab treatment response and drug retention in AS patients.

Relapsing polychondritis (RP) is an immune-mediated, systemic inflammatory and degenerative disease that affects cartilaginous structures particularly the ears, nose, eyes, joints, and respiratory tract and other tissues in the body. RP targets non-cartilaginous structures such as skin, kidney, heart, and the central nervous system in addition to cartilage-containing structures. Since it is a rare disease, data on its epidemiology are insufficient. It is prevalent between the ages of 40 and 60. Men and women are affected equally. Clinical characteristics vary among patients. The disease's rarity and broad clinical spectrum frequently result in misdiagnosis or delayed diagnosis. To prevent related complications and death, and to improve prognosis, early diagnosis and timely treatment of RP are crucial. Glucocorticoids, dapsone, disease-modifying antirheumatic drugs (DMARDs), and biologics are available as treatment options. The prognosis, such as the clinical presentation, varies depending on the extent of organ damage. 10 patients with RP had a mean onset age of 49.5±4.1. The median time between symptom onset and diagnosis was 3 months (2-60). 80% of the patients were male. The most frequent clinical manifestation is auricular chondritis (100%). During treatment, oral prednisolone was administered to all patients at least once. Intravenous methylprednisolone was administered to two patients. Due to ineffectiveness of DMARDs, one patient was switched to infliximab. A patient died due to pneumosepsis. This article aims to increase clinicians' awareness of this rare disease that can affect multiple systems by providing an overview of its pathogenesis, clinical course, diagnosis, and treatment., Tekrarlayan polikondrit (TP), özellikle kulaklar, burun, gözler, eklemler ve solunum yolu olmak üzere vücuttaki kıkırdaklı yapıları ve diğer dokuları etkileyen, immun aracılı, sistemik inflamatuar, dejeneratif bir hastalıktır. TP'nin hedef dokuları sadece kıkırdak içeren yapılar olmayıp, deri, böbrek, kalp ve merkezi sinir sistemi gibi kıkırdak içermeyen yapılar da etkilenir. Nadir bir hastalık olması nedenli epidemiyolojisi konusunda veriler yetersizdir. Sıklıkla 40-60 yaşları arasında görülür. Erkekler ve kadınlar eşit olarak etkilenir. Klinik özellikleri hastalar arasında değişkenlik göstermektedir. Hastalığın nadir görülmesi ve geniş klinik yelpazesi sıklıkla yanlış tanıya veya tanıda gecikmeye yol açar. TP'nin erken teşhisi ve hızlı tedavisi, ilişkili komplikasyonları ve ölümü önlemek, prognozu iyileştirmek için kritik öneme sahiptir. Tedavi seçenekleri arasında glukokortikoidler, dapson, hastalık modifiye edici antiromatizmal ilaçlar (DMARDs) ve biyolojik ajanlar yer alır. Prognoz, organ hasarının ciddiyetine bağlı olarak klinik tablo kadar heterojendir. TP tanısı ile izlediğimiz 10 hastanın ortalama hastalık başlama yaşı 49,5±4,1 idi. Semptom başlangıcı ile tanı arası süre ortanca 3 aydı (2-60). Hastaların %80’i erkekti. En sık rastlanan klinik bulgu aurikuler kondritti (%100). Hastaların tümü tedavileri sırasında en az bir kez oral prednizolon aldı. İki hastaya intravenöz metilprednizolon uygulandı. Bir hastada DMARDs yanıtsız olması nedenli infliksimaba geçildi. Bir hasta pnömosepsis nedenli kaybedildi. Bu yazıda, TP'nin patogenezi, klinik seyri, teşhisi ve tedavisi ile ilgili mevcut bilgilere genel bir bakış sunarak nadir görülen ancak pek çok sistemi etkileyebilen bu hastalıkla ilgili hekimler arasındaki farkındalığı artırmayı amaçladık.

Psoriatic arthritis (PsA) is a chronic inflammatory disease that can involve axial and peripheral joints. It is known that chronic inflammation in PsA plays a role in atherogenesis and peripheric insulin resistance. The aim of this study is to determine the relationship between carotid intima- media thickness (CIMT), which is an indicator of subclinical atherosclerosis, and metabolic syndrome components in patients with PsA according to joint involvement types. In our study, we included 74 PsA, 38 rheumatoid arthritis (RA) patients and 53 healthy people. Patients with PsA was divided into two types as peripheral and axial joint involvement. Age, gender, laboratory values, drugs were obtained from hospital files and waist circle, blood pressure, CIMT were measured in all subject. RA patients had significantly higher CIMT levels than PsA patients and both patient groups compared to the healty control group. Bu çalışmanın amacı, PsA eklem tutulum tiplerine göre subklinik aterosklerozun göstergesi olan karotis intima media kalınlığı (KİMK) ile metabolik sendrom komponentleri arasındaki ilişkiyi araştırmaktadır. 74 PsA, 38 Romatoid Artrit (RA) ve 53 sağlıklı kişi çalışmaya dahil edildi. PsA’lı hastalar RA benzeri periferik eklem tutulumu ve spondiloartrit benzeri eklem tutulumu olmak üzere iki tipe ayrıldı. Hasta dosyalarından yaş, cinsiyet, laboratuvar verileri, tedavide kullanılan ilaçlar kayıt edildi. Hastaların bel çevresi ve kan basıncı ölçümleri yapıldı. Ultrasonografi ile KİMK’na bakıldı. In this study, we aimed to evaluate the clinical and demographic data of the patients followed up with the diagnosis of Vascular Behçet's Disease (BD), and to examine the frequency of relapse and its relationship with the treatments used. The data of 512 patients diagnosed with BD were reviewed retrospectively, and 68 BD with vascular involvement were detected. Demographic characteristics, first vascular event, and recurrence, if any, and treatment protocols were recorded. The frequency of vascular involvement was 13.28%. 85% of the patients were man. Venous involvement was most common in the lower extremities. (77.9%) The first vascular relapse occurred in 29.4% of the patients, and the second vascular relapse occurred in 8.8%. After the first vascular event, 73.5% of the patients received systemic immunosuppressive therapy and 45.5% received anticoagulant therapy. The risk of relapse was found to be significantly higher in the group that did not receive immunosuppressive therapy. (p=0.001) Although the relapse rate was higher in the group receiving anticoagulant treatment, no statistically significant difference was found. (p=0.61) Vascular involvement often occurs in male patients. Immunosuppressive therapies and anticoagulants are used in the treatment. The use of immunosuppressive therapy can reduce the risk of vascular relapse. No additional benefit of anticoagulant therapy was observed. Multicenter studies with larger numbers of patients are needed for a better understanding in this issue., Bu çalışmada, vasküler Behçet Hastalığı (BH) tanısı ile izlediğimiz hastaların klinik, demografik verilerinin değerlendirilmesi, relaps sıklığı ve kullanılan tedavilerle olan ilişkisinin irdelenmesi amaçlanmıştır. BH tanılı 512 hastanın dosyası geriye dönük incelenerek 68 vasküler tutulumlu Behçet hastası tespit edildi. Demografik özellikler, birinci vasküler olay ve varsa nüksü, tedavi protokolleri kaydedildi. Vasküler tutulum sıklığı %13,28’idi. Hastaların %85’i erkekti. En sık alt ekstremitelerde venöz tutulum görüldü (%77,9). İlk vasküler relaps, hastaların %29,4’inde, ikinci vasküler relaps ise %8,8’inde gelişti. Vasküler tutulumlu Behçet hastalarında vasküler tutulumun tespit edilmesini takiben hastaların %73,5’i sistemik immünsüpresif (İS) tedavi, %45,5’i antikoagülan tedavi almıştı. İS tedavi almayan grupta relaps riski anlamlı olarak yüksek bulundu. (p=0.001) Antikoagülan tedavi alan grupta relaps oranı daha fazla olmakla birlikte istatistiksel olarak anlamlı bir fark saptanmadı (p=0.61). Vasküler tutulum sıklıkla erkeklerde görülmektedir. Tedavide İS'ler ve antikoagülanlar kullanılmaktadır. İmmünsupresif tedavi kullanımı vasküler relaps riskini azaltabilir, ancak antikoagülan tedavinin ek faydası gösterilememiştir. Bu konuda daha fazla sayıda hasta ile yapılacak çok merkezli çalışmalara ihtiyaç vardır.

The aim of the study is to evaluate the rheumatoid arthritis (RA) patients' knowledge about methotrexate (MTX), the standard drug in the treatment of RA, through a questionnaire. In August-October 2017, patients who used MTX with a diagnosis of RA in a university hospital were asked to answer questionnaire about the mechanism of action of MTX, administration method, drug interaction, side effects, frequency of follow-up of patients and changes to the lifestyle that should be done. One hundred and sixty-two patients (144 women) were included in the study. The mean patient age was 52 (18-77) years, the mean disease duration was 10.3 (1-40) years, and the mean duration of MTX use was 7.1 (1-38) years. 50% of the patients correctly answered that MTX is an anti-inflammatory drug that regulates the disease. 85% of patients correctly responded to the weekly application frequency correctly.; 19% of the patients stated that the reason for using folic acid was to reduce the toxicity of the treatment and again only 11% knew that trimethoprim was contraindicated. 11% were aware of the hematological risks and 11% were aware of the risk of hypersensitivity pneumonia. 51% of them had knowledge about laboratory tests. 20% of women and men of childbearing age knew that they should use contraception while using MTX. 34% stated that alcohol consumption should be limited. Men and those with a high level of education gave a statistically higher rate of correct answers. (p=0.019, p=0.000) It was observed that the patients had insufficient knowledge about MTX, which is frequently used in RA patients in daily practice. There are shortcomings on important issues, especially serious side effects, drug interactions, and birth control. Ağustos-Ekim 2017 tarihinde bir üniversite hastanesinde RA tanısı ile MTX kullanan hastalardan, MTX’in etki mekanizması, uygulama şekli, ilaç etkileşimi, yan etkileri, hastaların izlenme sıklığı ve yapılması gereken yaşam tarzı değişiklikleri ile ilgili soruları içeren anketi yanıtlamaları istendi. Yüz altmış iki hasta (144 kadın) çalışmaya dahil edildi. Ortalama hasta yaşı 52 (18-77), ortalama hastalık süresi 10.3 (1-40) yıl ve ortalama MTX kullanma süresi 7,1 (1-38) yıl idi. MTX’in hastalığı düzenleyen antiromatizmal bir ilaç olduğunu %50 hasta doğru yanıtladı. Doğurganlık çağında kadın ve erkeklerin MTX kullanırken kontrasepsiyon kullanması gerekliliğini bilenler %20 idi. %34’ü alkol tüketiminin sınırlı olması gerektiğini belirtti.
