Objective: A key contributor to neonatal early-onset sepsis, postpartum maternal sepsis, and infant late-onset invasive infections is Streptococcus agalactiae (group B Streptococcus; GBS). GBS may also result in bone and joint infections, bacteremia, endocarditis, pneumonia, and infections of the skin and soft tissues in adults. The bacterial polysaccharide capsule may distinguish between ten GBS serotypes (Ia, Ib, and II–IX). In order to reduce maternal colonization and prevent transmission to neonates, GBS capsular polysaccharide vaccines have been investigated. We aimed to detect the differences of the polysaccharide capsules among GBS isolates. Methods: This study used GBS isolates from several clinical specimens in the Microbiology Laboratory at Marmara University Hospital in Istanbul. Fifty of the isolates were colonized (37 genital site, 13 throat swab), and 50 of the isolates were infectious (33 urine, 7 blood, 2 respiratory specimens, and 4 wound swab specimens, 4 sterile body fluids). The serotypes of GBS isolates were determined by detecting visible agglutination when specific GBS capsule antigens reacted with serotype monospecific antibodies. Results: Colonized strain distribution values for serotype Ia, serotype II, serotype III, serotype Ic, serotype R, serotype V, serotype IV, serotype II/R, serotype III/R, and non-serotyping stains were 12 (24%), 11 (22%), 7 (14%), 3 (6%), 3 (6%) , 2 (4%), 1 (2%), 2 (4%), 1 (2%), 8 (16%) respectively. Causative agents distribution values for serotype II, serotype Ia, serotype III, serotype R, serotypes IV and serotype VIII, II / R, III / R, and non-serotyping stains were 15 (30%), 13 (26%), 10 (20%), 2 (4%), 1 (2%), 1 (2%), 3 (6%), 2 (4%) and 2 (4%) respectively. Conclusion: Serotype Ia, serotype II, and serotype III were the common serotypes in our clinical isolates but molecular-based studies associated with GBS population are needed in our country.

Ayrıca örgüt yapısı biçimsel iletişim sistemini belirlediğinden kültürün önemli bir kaynağı haline gelmektedir. Örgüt yapısı, bir işletmenin iskeletidir. Örgüt kültürü gibi, örgüt yapısı da bir organizasyon içindeki davranışı şekillendirir ve kontrol eder. Bu araştırmanın amacı, örgüt kültürünün örgüt yapıları üzerindeki etkisini incelemek ve arasındaki ilişkileri belirlemektir. Çalışmanın örneklem kümesini Çanakkale Merkez’de yer alan küresel otomotiv firmalarının servis çalışanları oluşturmaktadır. Otomotiv Sektörü’ne yönelik gerçekleştirilen çalışmada, ağırlıklı olarak satış ve teknik destek elemanlarının çalıştıkları işletmelerdeki kültür-yapı algıları ortaya konulmuştur. Araştırmanın sonuçlarına göre örgüt kültürünün alt boyutları olan misyon ve uyum ile mekanik örgüt yapısı arasında anlamlı ilişkilerin varlığı tespit edilmiştir., Organizational culture in a business consists of common norms and values that control the interaction of employees with each other, their suppliers, customers and other people. The foundations of culture in an organization are the organizational structure, the code of ethics, customers, employees and the value and rights given to them. It is expected that the organizational structure will be in harmony with the goals and culture of the organization, and thus be able to keep up with the changing and developing conditions. In addition, since the organizational structure determines the formal communication system, it becomes an important source of culture. Organizational structure is the backbone of a business. Similarly, organizational culture and structure shape and control behavior within an organization. The main purpose of this research is to examine the effects of organizational culture on organizational structures and to determine the relationships between them. The sample set of the study consists of service employees of global automotive companies located in the city center of Çanakkale. In the study performed for the Automotive Sector, the culture-structure perceptions of the businesses were revealed, where predominantly sales and technical support personnel work. According to the results revealed in the research, there are significant relationships between mission and adaptability, which are the sub-dimensions of organizational culture, and mechanical organizational structure.

GİRİŞ ve AMAÇ: Koruyucu hekimliğin oldukça önemli bir parçasıolan bağışıklama uygulamalarında en temel görevler aile hekimlerinedüşmektedir.Batman ilinde aile hekimlerinin bağışıklama konusundakieğitim ve güncelleme ihtiyaçlarına yönelik Aralık 2019’da birsempozyum düzenlenmiştir. Bu çalışmanın amacı gerçekleştirilensempozyuma katılan birinci basamak hekimlerinin aşı uygulamalarıkonusundaki bilgi düzeylerini değerlendirmektir.YÖNTEM ve GEREÇLER: Çalışma tanımlayıcı nitelikte bir gözlemçalışması olarak planlanmıştır. Aile hekimi olarak görev yapmaktaolan 155 hekim bu sempozyuma katılmış olup 48 hekim anketidoldurmuştur. Sempozyum öncesinde aile hekimlerine demografiksorular ile birlikte aşı uygulamaları konusunda bilgi düzeyinietkileyebileceği düşünülen sorular ve bilgi tespitine yönelik 34 sorudanoluşan bir anket uygulanmıştır. Bilgi sorularının yanıtları doğru,yanlış veya bilmiyorum olarak seçeneklendirilmiştir.BULGULAR: Anket sorularını cevaplayanların 40’ı erkek, meslekteortalama çalışma süreleri 8±7,3 yıldır.Kişilerin yarısı bilgi ölçümüneyönelik sorulan 34 sorudan 23 ve üzerinde soruyu doğru cevaplamıştır,en az 9 soru (soruların %26,5’i) doğru cevaplanmıştır. Bir kişi isesoruların tamamını doğru cevaplamıştır.Meslekte çalışma süresi iledoğru cevap verilen soru sayısı arasında herhangi bir korelasyonsaptanmamıştır (r=-0,142 p=0,337).TARTIŞMA ve SONUÇ: Aşılar konusunda son dönemde giderek artantereddütleri gidermede en etkin yollardan biri hekim ve diğer sağlıkçalışanlarının, aşı uygulanacak bireyler ve ebeveynler ile iyi biriletişim kurması ve güven sağlamasıdır. Etkin bir iletişimisağlayabilmek için özellikle birinci basamak sağlık hizmeti veren ailehekimlerimizin bu konudaki bilgilerinin yeterli olması gerekmektedir.Bu nedenle aile hekimlerine yönelik bilgilendirici nitelikte toplantılarındüzenlenmesi güncel bilgi erişimi konusunda kolaylık sağlayacaktır.&nbsp

Carbapenem-Resistant Klebsiella pneumoniae (CRKp) infections are worrying health problems due to decreasing treatment options. This study investigates the carbapenemase (OXA-23,24, 48, 51, 55, 58, KPC, NDM-1, VIM, IMP) and mcr-1 genes of the CRKps isolates A total of 33 CRKp isolates isolated from patient blood samples from the Dicle University Medical Faculty Hospital, intensive care units (ICUs) between February 2020 and June 2020, were included in the study. The presence of carbapenemase encoding genes -including all CRKp isolates, bla OXA-23, 24, 48, 58, bla KPC, blaNDM-1, bla VIM, bla IMP- were investigated by multiplex Polymerase Chain Reaction (PCR). CRKp isolates were tested for mcr-1 gene and bla OXA-51, bla OXA-55 genes by monoplex PCR. All CRKp isolates studied with Kirby Bauer Disc Diffusion Method (DDM) (100%) were resistant to ertapenem, 9 (27.27%) resistant to imipenem, and 23 (69.70%) were resistant to meropenem. 20 (60.61%) of the isolates were found resistant to colistin. bla OXA-48, bla NDM-1 and bla OXA-24 genes were found in 75.76% (n = 25), 6.06% (n = 2) and 3.03% (n = 1) isolates, respectively. Both bla OXA-48 and bla NDM-1 genes were detected in two (6.06%) isolates and mcr-1 gene in 16 (48.48%) isolates. While the mean hospitalization was 20.3 days in 13 patients with a colistin minimum inhibitory concentration (MIC) of 2 μg/ml, it was 33.9 days in 20 patients with a colistin MIC of > 2 μg/ml. The average length of stay in the hospital was 21.8 days in mcr-1 negative patients and 35.7 days in mcr-1 positive patients. Carbapenemase and mcr-1 positivities were found at dramatically high rates in Diyarbakır, Turkey. It was indicated that plasmid-mediated antimicrobial resistance in Kp isolates was problematic. Each hospital should monitor the colistin and carbapenem resistance mechanisms by molecular methods. Colistin resistance should be confirmed by the broth microdilution method (BMD). Karbapenem Dirençli Klebsiella pneumoniae (KDKp) enfeksiyonları azalan tedavi seçenekleri nedeniyle endişe verici sağlık sorunları oluşturmaktadır. Bu çalışmada, KDKp izolatlarının karbapenemaz (OXA-23,24, 48, 51, 55, 58, KPC, NDM-1, VIM, IMP) ve mcr-1 genleri araştırılmaktadır. Dicle Üniversitesi Tıp Fakültesi Hastanesi yoğun bakım ünitelerinden (YBÜ) Şubat 2020 ile Haziran 2020 tarihleri arasında alınan hasta kan örneklerinden izole edilen toplam 33 KDKp izolatı çalışmaya dahil edildi. Tüm KDKp izolatları, bla OXA-23, 24, 48, 58, bla KPC, blaNDM-1, bla VIM, bla IMP dahil olmak üzere karbapenemaz kodlayan genlerin varlığı multipleks Polimeraz Zincir Reaksiyonu (PZR) ile araştırıldı. KDKp izolatları monoplex PZR ile mcr- 1 geni ve bla OXA-51, bla OXA-55 genleri için test edildi. Kirby Bauer Disk Difüzyon Test (DDT) (%100) ile çalışılan tüm KDKp izolatları ertapenem’e dirençliydi; 9’u (%2.27) imipeneme, 23’ü (%69.70) meropeneme dirençliydi. İzolatların 20’si (%60,61) kolistine dirençli bulundu. bla OXA-48, bla NDM-1 ve bla OXA-24 genleri sırasıyla %75.76 (n=25), %6.06 (n=2) ve %3.03 (n=1) izolatında bulundu. İki (%6.06) izolatta hem bla OXA-48 hem de bla NDM-1 genleri, 16 (%48,48) izolatta mcr-1 geni saptandı. Kolistin minimum inhibitör konsantrasyonu (MİK) değeri 2 μg/ml olan 13 hastada ortalama yatış süresi 20.3 gün iken, kolistin MİK değeri > 2 μg/ml olan 20 hastada 33.9 gündü. Hastanede ortalama kalış süresi mcr-1 negatif hastalarda 21.8 gün, mcr-1 pozitif hastalarda 35.7 gündü. Karbapenemaz ve mcr- 1 pozitiflikleri Diyarbakır, Türkiye’de çarpıcı biçimde yüksek oranlarda bulundu. Kp izolatlarında plazmit aracılı antimikrobiyal direncin sorunlu olduğu belirtildi. Her hastane moleküler yöntemlerle kolistin ve karbapenem direnç mekanizmalarını izlemelidir. Kolistin direnci sıvı mikrodilüsyon yöntemi ile doğrulanmalıdır.

Real-life experience of tenofovir alafenamide fumarate: The pythagorean cohort

WOS:000635211600001 Background: Chronic hepatitis B (CHB) is a viral infection that can result in life-threatening conditions, such as hepatocellular carcinoma and cirrhosis. Tenofovir, which is used for the treatment of CHB, is a nucleotide analog that inhibits HBV-DNApolymerase and has two formulations: disoproxil and alafenamide. In contrast to tenofovir disoproxil fumarate (TDF), tenofovir alafenamide fumarate (TAF) penetrates the whole hepatocyte without being eliminated due to its longer plasma half-life and greater plasma stability. As a result, side effects such as proximal renal tubulopathy and loss of bone density are less common in the treatment of TAF and have similar efficacy to TDF. Objectives: The purpose of the study was to evaluate the effectiveness and reliability of TAF using real-life data. Methods: This retrospective cohort study was carried out in secondary or tertiary healthcare centers in southern Turkey. A total of 480 patients aged 18 years and older were administered TAF for an appropriate indication by the infectious diseases and gastroenterology clinics of the healthcare centers participating in this study. The data collected at t = 0, t =3, and t = 6 months of treatment were analyzed. The chi-square, Mann-Whitney U, Friedman, Wilcoxon, Cochran's Q, and McNemar's tests were used. Results: The mean age of the patients was 47.40 +/- 14.5, and 327 of them (68.1%) were male. A total of 78.1% of the 480 patients who underwent the TAF treatment had previous antiviral therapy experience (TDF, n =340; 70.8 %), and 21.9% were treatment-naive. The most common reasons for the initiation of TAF treatment were the use of drugs affecting bone mineral density (BMD) (42.9%) and osteoporosis (22.3%). Patients who had taken TDF experienced a significant improvement in glomerular filtration rate (GFR), hip and spine T-scores, and phosphorus levels from t = 0 months tot = 6 months after switching to TAF (P < 0.05). For this group, no statistically significant difference was observed concerning LDL and cholesterol levels from t= 0 months to t = 6 months. Side effects were reported by 5.7% of patients in the third month and 7.1% in the sixth month, with the most common side effect being hair loss (1%). Conclusions: TAF was found to be an effective and safe alternative to TDF with lower incidences of its long-term effects, such as nephrotoxicity and decreased bone density.

Giriş: Sağlık sistemi Covid-19 pandemisi sürecinde derinden etkilenmiştir. Sağlık çalışanları yüksek risk altında hizmet vermektedir. Bu dönemde tıp öğrencileri uzaktan eğitimlerine devam etmektedirler. Bu çalışmada, tıp fakültesi öğrencilerinin kendi sağlıklarının yanı sıra meslekleri hakkındaki kaygıları ve tıp fakültesine başlama kararları ile ilgili düşüncelerinin incelenmesi amaçlanmıştır. Yöntem: Bu çalışma 10-17 Mayıs 2020 tarihleri arasında Ankara Yıldırım Beyazıt Üniversitesi Tıp Fakültesi'nde yapılmıştır. Demografik bilgiler, mesleki kaygı, sağlık kaygısı ve sağlık biliş düzeyi hakkında soruları içeren çevrimiçi anket 2281 öğrenciye gönderilmiştir. Veriler elverişli örneklem yöntemi ile elde edilmiştir. Verilerin dağılımı medyan (minimum-maksimum) kullanılarak sunulmuştur. Ankete verilen yanıtlar arasındaki ilişkiyi değerlendirmek için çift serili korelasyon katsayısı, ki-kare testi ve polikorik korelasyon katsayısı kullanıldı. Bulgular: Anketi toplam 885 öğrenci tamamlamıştır. Ortalama yaş 21.47 (± 2.62) olarak bulunmuştur. Çalışmaya katılanların 545'i (%61,9) kadındı. 809 öğrenci (%91,8) tıp fakültesini kendi isteğiyle, 72'si (%8,2) ise istemeyerek seçtiğini belirtmiştir. Tıp fakültesini istemeyerek seçenlerin meslek değişikliği yapma isteği yüksek bulunmuştur. Uzaktan eğitim 168 (%19,2) öğrenciye göre yeterli bulunurken 435 öğrenci için kısmen yeterli olarak değerlendirilmiştir. Öğrencilerin okumakta olduğu yarıyıl arttıkça mesleki kaygıları, mesleklerinin avantaj ve dezavantajlarına ilişkin farkındalıkları, sağlık sistemine güvenleri, farklı mesleki kaynakları gözden geçirme yetenekleri artmıştır. Kız öğrencilerin, erkek öğrencilere göre mesleki ve genel sağlıkları ile daha fazla endişe duyduğu bulunmuştur. Sonuç: Pandeminin tıp öğrencilerinin meslek seçimi ve kaygı düzeyleri üzerinde etkisi olmasına rağmen, genel olarak bu etkiler çok yüksek bulunmamıştır. Mesleki kaygı, mesleğe ilişkin farkındalık, sağlık sistemine güven, farklı mesleki kaynakları gözden geçirme becerisi okumakta oldukları yarıyıl ile ilişkiliydi. Uzaktan eğitim ile ilgili iyileştirilmeler yapılmadır. Pandemi sürecinde öğrencilere psikolojik destek sağlanması faydalı olabilir. Ayrıca sonuçlar, araştırmanın yapıldığı zaman ve pandeminin kısa sürede sona ereceği beklentisi nedeniyle iyimser olarak cevaplandırılmış olabileceği göz önüne alınarak değerlendirilmelidir., Aim: The Covid-19 pandemic has affected healthcare system deeply. The healthcare workers served at high risk. In this period, medical students continued their education remotely. This study was aimed to evaluate the concerns of the medical students about their own health as well as their profession and thoughts about their decision to start medical school. Methods: This study was conducted in Ankara Yildirim Beyazit University, Faculty of Medicine between 10-17 May 2020. The online questionnaire including demographic information, occupational anxiety, health anxiety, health cognition level were sent to 2281 students. The data were obtained using a convenience sampling. The distribution of the data is presented using median (minimum-maximum). Double-series correlation coefficient, chi-square test and polychoric correlation coefficient were used to evaluate the relationship between responses to the questionnaire. Results: A total of 885 students completed the questionnaire. The mean age was 21.47 (±2.62); 545 (61.9%) of them were female. A number of 809 (91.8%) students stated that they chose medical school willingly while 72 (8.2%) students unwillingly. Those who unwillingly chose medicine had a high desire to change their profession. The distance education was sufficient for 168 (19.2%) students; partially sufficient for 435 students. As the semester increased, their professional anxiety, awareness of the advantages and disadvantages of their professions, trust to the health system, ability of reviewing different professional resources increased. Female students were more concerned about their professional and general health than male students. Conclusions: Although the pandemic had effects on medical students' profession choice and anxiety levels, in general, these effects was not higher. The professional anxiety, awareness of the profession, trust to the health system, ability of reviewing different professional resources were related to semester year. Distance education systems should be improved. Psychological support can be provided for the students. Moreover the results should be evaluated regarding the time of study and expectations the end of pandemic in a short time that may be the reason more optimistic answers.

Amaç: Aşı tereddüdü hem Türkiye hem de dünya için toplumsal tehdit oluşturan bir tutumdur. Bu konuda Türkçeye validasyonu yapılmış bir ölçek bulunmamaktadır. Bu çalışmanın amacı Opel ve ark. tarafından 2011 yılında geliştirilip 2013 yılında revize edilen ve ‘Parent Attitudes About Childhood Vaccines Survey ‘isimli ölçeğin Türkçeye dil validasyonunun yapılmasıdır.Gereç ve Yöntem : Çalışma bir dil validasyonu çalışması olarak gerçekleştirilmiştir. Opel DJ ve arkadaşları tarafından geliştirilen ölçek için ‘Seri yöntem’ kullanılarak dil validasyonu çalışması yapılmıştır. Ölçeğin çeviri, gözden geçirme, geri çeviri, açıklığın ve aynılığın değerlendirmesi ile çevirisi sağlanmış ve Modifiye Lawshe yöntemiyle kapsam geçerlilik analizi yapılarak son hali verilmiştir. Uzmanların ölçek maddelerine yönelik içerik uygunluğu yönünden belirtmiş oldukları görüşlere göre her madde için kapsam geçerlilik oranı ve kapsam geçerlilik indeksi hesaplanmıştır.Bulgular. Tüm ölçek soruları için kapsam geçerlilik oranı kritik değer olan 0,455’in üzerinde saptanmıştır. Ölçeğin kapsam geçerlilik indeksi değeri ise 0,630 olup ölçek sorularının kapsam geçerliliğinin istatistiksel olarak anlamlı olduğu tespit edilmiştir.Çıkarımlar: Çocukluk çağı aşılarına yönelik ebeveyn tutumları ölçeğinin dil validasyonunun tamamlanması ve kapsam geçerlilik testlerinden istatistik olarak anlamlı sonuçlar alınmış olması bu ölçeğin Türkçe anlaşılırlığının sağlandığını göstermektedir. Sonrasında yapılacak geçerlilik ve güvenilirlik çalışmaları ile ölçeğin Türkiye’de aşı tereddüdü konusunda ebeveyn tutumlarını ölçebilir hale gelmesi hedeflenmektedir. Böylece daha geniş kitlelerde çalışılarak aşı tereddüdü ve mücadelesi konusunda yapılacak çalışmalara katkı sağlayacağı düşünülmektedir.

The clustered regularly interspaced short palindromic repeats –CRISPR-associated (CRISPR-Cas) system, as a bacterial adaptive immune system, is recognized as one of the new strategies for controlling antibiotic-resistant strains. The programmable Cas nuclease of this system used against bacterial genomic sequences could be lethal or could help reduce resistance of bacteria to antibiotics. Therefore, this study aims to review using the CRISPR-Cas system to promote sensitizing bacteria to antibiotics. We envision that CRISPR-Cas approaches may open novel ways for the development of smart antibiotics, which could eliminate multidrug-resistant (MDR) pathogens and differentiate between beneficial and pathogenic microorganisms. These systems can be exploited to quantitatively and selectively eliminate individual bacterial strains based on a sequence-specific manner, creating opportunities in the treatment of MDR infections, the study of microbial consortia, and the control of industrial fermentation.Keywords: antibiotic-resistant bacteria, CRISPR-Cas system, sequence-specific manner, resensitization, genome editing

Bruselloz enfekte hayvanlarla doğrudan temas, süt ve süt ürünlerinin taze olarak tüketilmesi, enfekte damlacıkların inhalasyonu ile bulaşabilen, brusella türlerinin neden olduğu bir hastalıktır. Özellikle çocuk hastaların tedavisinde güçlüklere ve nükslere neden olmaktadır. Bu tezde brusellozun endemik olduğu Diyarbakır yöresinde izole edilen Brucella suşlarının tür düzeyinde tanımlanması, antibiyotik duyarlılıklarının belirlenmesi multilocus variable number tandem repeat analysis (MLVA) yöntemiyle moleküler epidemiyolojik analizlerinin yapılması amaçlandı. Çalışmaya 2000-2013 yıllarında Diyarbakır Çocuk Hastalıkları Hastanesi’nde, brusella enfeksiyonu tanısı alan çocuklardan izole edilmiş toplam 77 Brucella suşu dahil edildi. Konvansiyel yöntemler ile suşların tür düzeyine tayini ve antibiyotik duyarlılık testleri gerçekleştirildi. Polimeraz zincir reaksiyonu yöntemiyle Brusella türleri tanımlandı ve ardından MLVA yöntemi ile suşların genotip tayini yapıldı. 77 hastanın 40’ı erkek, 37’si kız olup yaş aralığı 2-16 yaş (ortalama 9.14 ± 3.4 yaş) arasındaydı. Bütün hastalarda yakın zamanda pastörize edilmemiş süt veya süt ürünleriyle beslenme hikayesi mevcuttu. Karın ağrısı (58%), artralji (%56), miyalji (%40) en sık şikayetlerdi. En sık klinik bulgular arasında ateş, artrit, hepatomegali ve splenomegali yer alıyordu. Mevsimsel dağılım açısından sonbahar sonu ve yaz aylarında sıklık gösteriyordu. Suşların tamamının B.melitensis biovar 3 olduğu saptandı. İki izolat seftriaksona dirençli bulunurken, suşların tamamı doksisiklin, streptomisin ve trimetoprim-sülfametoksazole duyarlıydı. Tigesiklin ve Rifampisin için MİK aralıkları 0,016-0,23 ve 0,38-1,5 µg/ml olarak belirlendi. Moleküler epidemiyolojik analizler sonucunda 77 suşta 42 farklı MLVA-16 profili belirlendi. Bu 42 profilin 18 tanesi en az iki veya daha fazla suş içermekteydi. Geriye kalan 24 suş ise özgü profil gösterdi. Suşlar arasındaki kümeleşme oranı %66.7 olarak tespit edildi. Lokuslardan ayırım gücü en yüksek lokus Bruce 30 iken, bunu sırasıyla Bruce 16, Bruce 9, Bruce 7, Bruce 4 takip etti. Suşlarının tamamının MLVA-11'e göre genotip 122, MLVA-8'e göre ise genotip 43 olduğu ve Doğu Akdeniz genotipinde yer aldığı belirlendi. Sonuç olarak yörede çocuklarda bruselloz halen önemli bir halk sağlığı sorunudur. Suşlar arasında çapraz bulaş oranı yüksektir. Çalışmamız bölgede hayvan hareketliliğin kontrol altına alınmasının, et ve süt ürünlerinin denetim altına alınmasının, çocukların beslenmesinde kontamine ürünlerden kaçınılmasına yönelik eğitim faaliyetlerinin gerekliliğini ortaya koymuştur. Anahtar Sözcükler: Brusella, MLVA, bulaş dinamiği Brucellosis is an infection disease caused by brucella species and is transmitted by direct contact with infected animals, consumption of fresh milk and dairy products, inhalation of bacteria of infected droplets. It causes difficulties and recurrences especially in the treatment of child patients. This study was aimed to identify the brucella species, to determine their antibiotic susceptibilities and to perform molecular epidemiological analyzes of the isolates by multilocus variable number tandem repeat analysis (MLVA) method. A total of 77 brucella strains isolated from children with brucella infection in Diyarbakır Children’s Diseases Hospital between 2000 and 2013 were included in the study. Genotying of the strains were done by MLVAOrsay 16 method. Of the 77 patients, 40 patients were boys and 37 patients were girls, with ages ranging from 2 years to 16 years (mean, 9.14 ± 3.4 years). All patients had a history of recently consuming unpasteurized milk or dairy products. Abdominal pain (58%) and arthralgia (%56), and myalgia (40%) were the most frequent complaints. The most frequent clinical findings were fever, arthritis, hepatomegaly, and splenomegaly. The seasonal distribution showed an increase in late spring and summer. All of the Brucella isolates were found to be B. melitensis biovar 3. Two isolates were resistant to ceftriaxone, while all isolates were susceptible to doxycycline, streptomycin and trimethoprim-sulfamethoxazole. Ranges of minimal inhibitory concentration (MIC) of tigecycline and rifampicin were 0.016-0.23 and 0.38-1.5 μg/ml, respectively. Molecular epidemiological analyzes revealed 42 different MLVA-16 profiles. Eighteen of these 42 profiles included two or more strains. The clustering rate was 66.7%. In three strains four locus variants, in one strain five locus variants were found. Bruce 30 was the highly discriminatory locus and it was followed by Bruce 16, Bruce 9, Bruce 7, and Bruce 4, respectively. All of the strains were defined as genotype 122 according to MLVA-11 and genotype 43 according to MLVA-8, and were in the Eastern Mediterranean genotype. In conclusion, brucellosis is still a significant public health problem in children in the region and high clustering rate indicates that most of the cases were related to each other and most probable originated from common source. Controlling of animal movements, meat and milk products and training activities about avoiding contaminated products of the children were necessary. The aim of this review is to investigate the relationship of oral microbiota with many oral and related diseases, mainly dental caries and periodontitis, and their effects on general health. We shared the results of the studies and the results of the research which are still in the research stage. Specifically, we aimed to introduce and discuss the course of oral microbiota transplantation (OMT) and the process ahead.Microbiota plays an important role in many metabolic events such as modulation of glucose and lipid homeostasis, satiety regulation, energy and vitamin production, as well as anti-carcinogenic and anti-inflammatory effects. The most intense colonization area in the body, which contains hundreds of different bacterial, viral and fungal species after the gastrointestinal tract, is the oral cavity. The diversity in the oral microbiota is shaped by the micro-environmental conditions according to the host's systemic condition, diet, genetic predisposition and the efficacy of antimicrobial agents in saliva. While the microorganisms in the oral microbiota continue their symbiotic relationship with the host, the variation in the proportion of certain species with the potential for pathogenicity in the ecological flora results in some oral and systemic diseases.In oral medicine, oral microbiota transplantation (OMT) has been hypothetically inspired by fecal microbiota transplantation. Oral microbiota transplantation aims to transfer oral biofilms from a healthy donor to the patient with caries or periodontitis. Ongoing and future studies should provide new information about oral microbiota and how a disbiotic microbiota can be successfully modified with a health-friendly flora., Özet: Bu derlemenin amacı, oral mikrobiyotanın, başta diş çürüğü ve periodontitis olmak üzere, birçok ağız içi ve ilişkili hastalıklarla ilişkisini ve bunların genel sağlık üzerine etkilerini araştırmaktır. Konuyla ile ilgili bugüne değin yapılan ve halen araştırma aşamasında olan çalışmaları ve sonuçlarını paylaştık. Spesifik olarak, oral mikrobiyota transplantasyonu (OMT)’nun kat ettiği yol ve önündeki süreci tanıtmayı ve tartışmayı amaçladık. Mikrobiyota, glikoz ve lipit homeostazisinin modülasyonu, tokluk regülasyonu, enerji ve vitamin üretimi gibi birçok metabolik olayda önemli bir rol oynamasının yanında anti-karsinojenik ve anti-inflamatuar etkilere de sahiptir. Gastrointestinal sistemden sonra yüzlerce farklı bakteriyel, viral ve fungal türü barındıran, vücuttaki en yoğun kolonizasyon alanı ağız boşluğudur. Oral mikrobiyotadaki çeşitlilik, konakçının sistemik durumu, diyeti, genetik yatkınlığı ve tükürükteki antimikrobiyal ajanlarının etkinliğine göre, mikro-çevresel koşullar tarafından şekillendirilir. Oral mikrobiyotadaki mikroorganizmalar, konakçı ile simbiyotik ilişkilerini sürdürürken, ekolojik floradaki patojenite potansiyeli olan belirli türlerin oranlarındaki değişim, bazı ağız ve sistemik hastalıklarla sonuçlanmaktadır.Tıpta fekal mikrobiyota transplantasyondan esinlenerek, oral mikrobiyota transplantasyonu (OMT) hipotetik olarak önerilmiştir. Oral mikrobiyota transplantasyonu, sağlıklı bir donördeki oral biyofilmleri çürük ya da periodontitisli hastaya aktarmayı amaçlamaktadır. Devam eden ve gelecekteki çalışmalar, oral mikrobiyota konusunda ve disbiyotik bir mikrobiyotanın, sağlık açısından yararlı bir flora ile nasıl başarılı bir şekilde değiştirilebileceğine dair yeni bilgiler sunmalıdır.

Ektima gangrenozum Pseudomonas aeruginosa ile ilişkili bir deri lezyonudur. Daha önce sağlıklı olan bir yaşında erkek çocuk; başka bir merkezde su çiçeği tanısı aldıktan 10 gün sonra ishal, kulak akıntısı ve bez bölgesinde su çiçeğinden farklı lezyonlar gelişmesi üzerine hastanemize gönderildi. Fizik bakısında; her iki gluteal bölge ve perianal bölgede yeşilimsi, keskin sınırlı, nekrotik ülserler vardı. Ön tanıda ektima gangrenozum düşünüldü. Hastanın yara kültüründe Pseudomonas aeruginosa üredi. İzlemde şiddetli ishal ve ülserasyonların devam etmesi nedeni ile olguya kolostomi açıldı. Olgunun immünolojik değerlendirmesinde T lenfosit alt grupları ve immünglobulinleri normal saptandı. Bu olgu, bez bölgesinde Pseudomonas aeruginosa’ya bağlı ektima gangrenozum meydana gelebileceğine ve Pseudomonas sepsisinin araştırılması gerektiğine dikkat çekmek amacıyla sunulmuştur. (Turk Pediatri Ars 2016; 51: 46-8) Ecthyma gangrenosum is a skin lesion associated with Pseudomonas aeruginosa. A previously healthy one-year-old boy who had been diagnosed with varicella 10 days ago was admitted to our hospital with complaints of diarrhea, green ear discharge and new lesions in the diaper area. Intravenous meropenem and amikacin had been previously initiated. Physical examination revealed greenish, well bordered necrotic ulcers on both gluteal areas and the perianal area. Pseudomonas aeruginosa grew in wound culture. A colostomy was opened due to recalcitrant diarrhea and ulcers. The patient remitted in one month and was discharged. T lymphocyte subgroups and immunoglobulins were found to be normal on immunologic evaluation. We presented this case to draw attention to the fact that ecthyma gangrenosum due to Pseudomonas aeruginosa may occur in the diaper area and that sepsis due to Pseudomonas should be investigated.
