Purpose/Objective(s) We previously reported the results of a phase II clinical trial evaluating 30-36 Gy of adjuvant radiation therapy (RT) for selected patients with HPV+ OPSCC. Herein we report the two-year results of a phase III trial comparing this 30-36 Gy regimen with standard of care (SOC) adjuvant RT. Materials/Methods All patients (pts) received transoral robotic surgery (TORS) and neck dissection for a margin negative resection. Pts with pT4 disease or who required >2 attempts to clear margins were excluded. Pts with intermediate risk factors received 30 Gy/1.5 Gy b.i.d. + docetaxel 15 mg/m2 days 1 and 8, while pts with extranodal extension (ENE) simultaneously received 36 Gy/1.8 Gy b.i.d. to ENE+ nodal levels. Pts were randomized (2:1) to DART or SOC (60 Gy ± weekly cisplatin 40 mg/m2). Stratification was by risk group (intermediate risk vs ENE+) and smoking status ( Results Accrual was from 10/16 – 8/20 (n = 194, DART: 130, SOC: 64, ENE+: 115 (59%), Non-smokers: 139 (72%), median age 59.4 yrs (37.9-81.6), male 89%. Median follow-up as of 7/21 was 25.3 mo. 1.6% DART and 27.4% SOC pts (p 4 LN). For ENE+/pN0-1 pts, (DART vs SOC) 2 yr PFS was 89.6% vs 95.8%; LRC 95.8% vs 100.0%; DMFS 96.4% vs 95.8%. For ENE+/pN2 (DART vs SOC) PFS was 42.9% vs 100%; LRC 77.0% vs 100%; DMFS 59.4% vs 100%. Conclusion DART demonstrated less toxicity, improved swallowing function and QOL when compared to SOC. DART also had excellent LRC, PFS, and OS rates, particularly in the ENE negative cohort. Caution is advised for de-escalating ENE+ pts with pN2 disease.

Purpose/Objective(s) We previously reported the two-year results of MC1273, a phase II trial evaluating 30-36 Gy of adjuvant radiation therapy (RT) for selected patients (pts) with HPV+ OPSCC. Herein we report the long-term disease control and toxicity rates for this de-escalated regimen. Materials/Methods Details for the MC1273 regimen have previously been published. All pts received surgery & neck dissection for a margin negative resection. Eligibility criteria included pts with HPV+OPSCC, ≤10 pack-year smoking history, and negative margins. Cohort A (≥T3, ≥N2, lymphovascular invasion, or perineural invasion) received 30 Gy delivered in 1.5 Gy b.i.d. over 12 days along with weekly docetaxel (15 mg/m2, days 1 & 8). Pts with +ECE were enrolled in Cohort B and received the same treatment plus a simultaneous integrated boost to nodal levels with ECE to 36 Gy in 1.8 Gy b.i.d. The primary endpoint was local/regional control (LRC) at 2 years with each cohort powered to detect a 10% locoregional failure rate with 85% confidence. Secondary endpoints included 2-yr progression free survival (PFS), overall survival (OS), toxicity, swallow function, and pt reported QOL. Follow-up was q3 mos for the first 2 years, q6 mo for year 3, and yearly until year 5. Based upon the 2-year MC1273 data, pts with pT4 disease were excluded from MC1675, the follow-up phase III trial randomizing pts to the 30-36 Gy regimen versus a standard 60 Gy regimen. Results Accrual was from September 2013 to June 2016 (n = 80, 1 ineligible, Cohort A: 37, Cohort B: 43, median (range) age 61 years (25 - 77), male n = 71 (89.9%). Median follow-up for alive pts as of 2/22/21 was 52 mos (30 – 67). Late grade 2 or higher toxicity rates at 2, 3, and 5 years were 6.7%, 6.8%, and 5.0%, respectively, with the most common being dry mouth (1.7%, 3.4%, and 0.0% at years 2, 3, and 5) and dysphagia (3.3%, 0.0%, and 5.0% at years 2, 3, and 5). Grade 3 rates were 0.0%, 0.0%, and 1.2% at years 2, 3, and 5, respectively, with one pt who developed a malignant peripheral nerve sheath tumor in the brachial plexus at the edge of the neck volume 51 mo after RT. The 2, 3, and 5 year disease metrics for the entire study population, Cohort A, and Cohort B can be found in the Table, along with an unplanned subset analysis for pts qualifying for MC1675 (n = 72). Conclusion MC1273 maintained excellent long-term disease control rates comparable to historical controls, particularly in the intermediate risk cohort and pts qualifying for MC1675. Toxicity rates remained low long-term.

This paper describes the design of a baseband processor for IS-54 North American cellular telephony standard. The effect of diverse circuit impairments on the error vector in digital mode and on the parasitic amplitude modulation in analog mode are analyzed. An analog offset compensation scheme, which takes advantage of the TDMA operation, is presented. The device incorporates a Manchester data decoder for data transmission in analog mode. The messages can be sent via two interfaces to the DSP or /spl mu/-processor. The architecture of the digital signal processing chain is discussed. The device is fabricated in a 0.9 /spl mu/ CMOS technology with an area of 40 mm/sup 2/.

The glass, liquids and amorphous materials diffractometer (GLAD) is a new instrument at the intense pulsed neutron source (IPNS) at Argonne National Laboratory. The GLAD currently has 218 linear position sensitive detectors arranged in five banks. Raw data collected from the instrument are typically split into 1000–1500 angular groups each of which contains approximately 2000 time channels. In order to obtain a meaningful overview of such a large amount of data, an interactive system to view the data has been designed. The system was implemented in C using the graphical kernel system (GKS) for portability. The system treats data from each bank of detectors as a three-dimensional data set with detector number, position along detector and time of flight as the three coordinate axes. The software then slices the data parallel to any of the coordinate planes and displays the slices as images. This approach has helped with the detailed analysis of detector electronics, verification of instrument calibration and resolution determination. In addition, it has helped to identify low-level background signals and provided insight into the overall operation of the instrument.

The structure of molten FeCl[sub 3] at 320[sup degrees]C has been measured with neutron diffraction at the Intense Pulsed Neutron Source. Results indicate that melting in FeCl[sub 3]is accompanied by a change in local structure from the octahedral environment of the Fe[sup 3[plus]] in the solid to an Fe[sub 2]Cl[sub 6] molecular liquid. This behavior is similar to that observed in AlCl[sub 3] and in contrast to that of YCl[sub 3] where an octahedral coordination is preserved on melting. 3 figs, 1 tab, 12 refs.

By means of the new technique of neutron Compton scattering we have determined the kinetic energy of hydrogen in β-V2H. The resulting value agrees with that calculated from the vibrational ground state wavefunction which was derived from previously measured vibrational excitation energies.

An ab initio itinerant-electron calculation for a class of cerium and uranium compounds was performed over the full frequency range studied experimentally. The band structure is obtained from a self-consistent spin-polarized full-potential linearized muffin-tin orbital calculation with a true interstitial. It was found that, for the compounds that display giant Kerr rotations, such a conventional calculation provides magnetooptic behavior very different from that obtained experimentally. This poor agreement was attributed to the presence of a large orbitally driven magnetism totally unlike the magnetism of Ni and Fe, and to the possible importance of explicit correlation effects on approaching heavy fermion behavior. Therefore, explicit orbital polarization was incorporated into the band calculation. This brings about a much better agreement for the ordered magnetic moments but fails to provide adequate agreement for the magnetooptic behavior; furthermore, unlike a calculation including explicit correlation effects, it does not provide agreement with the experimental ordered moment for the incipient heavy fermion CeTe system. >

A brief review of the structure of molten salts is given with special emphasis on structural features in reciprocal space. Selected diffraction results on halide salts containing monovalent and divalent cations are presented, along with recent results on salts containing trivalent cations. Different models and computational techniques, such as random packing of structural units, the reference interaction site model, and reverse Monte Carlo are used to derive structural information at the partial level. 15 refs., 3 figs.

Architectural and circuit innovations resulting in a 260mW single-chip ISDN U-interface transceiver wii h a range of more than 21Kft of AWG26 cable are described. These include a new scheme for jitter compensation using a two-phase decimator that results in faster phase adjustment and jitter tracking, and a fastconverging nonlinear echo canceler that can track time-dependent components of nonlinear distortion.

Describes the technique of two-phase decimation. When combined with the previously known technique of jitter compensation, it allows the performance of the echo canceler in a full-duplex data transceiver to be preserved in the presence of both the phase steps generated by the timing recovery digital phase locked loop and rapid changes in the sampling phase of the input signal. This results from being able to train the jitter canceler continuously, instead of restricting the training to some initial startup period. This technique provides an efficient way to preserve the time invariance of the echo path during a phase step, a precondition for the jitter compensation technique to perform properly. >

Giant magneto-optic Kerr rotations occur in a number of cerium and uranium chalcogenides and pnictides. Some of these materials (e.g. CeTe, UTe) show distinct evidence of correlated-electron behavior, including incipient heavy- fermion behavior. The giant magneto-optic behavior originates in the coupling of light to the large partially itinerant orbital polarization present in these materials. The approach to the heavy-fermion state is characterized by a washout (broadening away) of what band theory would predict to be strong structure in the magneto-optic dispersion; and magneto-optic behavior appears to be a very sensitive probe of the approach to the heavy-fermion state.

Purpose/Objective(s): To report outcomes of patients with laryngeal or hypopharyngeal carcinoma treated with definitive intensity-modulated radiation therapy (IMRT). Materials/Methods: The clinical records of all patients with laryngeal and hypopharyngeal squamous cell carcinomas treated with definitive IMRT at our institution between January 2004 and December 2011 were retrospectively reviewed. Fifty patients were reviewed. All patients would have required a total laryngectomy. Forty-two (84%) patients received concurrent chemotherapy, 6 (12%) received no chemotherapy, and 2 (4%) received adjuvant chemotherapy. Of patients who received concurrent chemotherapy, the majority of them (24 patients, 48%) received cisplatin alone; 6 patients (12%) received cetuximab alone, 8 (16%) received both cetuximab and cisplatin, and 6 (12%) received other chemotherapy regimens. Kaplan-Meier survival analysis and univariate Log-rank statistics were applied. Results: Median follow-up was 26.0 months (range, 4.0-106.0 months). Median age was 61 years (range, 35-83 years); 39 (78%) were male. There were 35 (70%) laryngeal and 15 hypopharyngeal cancers. Twelve (24%), 29 (58%), and 2 (4%) patients were diagnosed clinical stages III, IVA, and IVB, respectively. All patients were prescribed 7000 cGy in 35 daily fractions; however, 2 patients were unable to complete the full treatment. There were 16 local, 7 regional, and 15 distant failures. For all patients, local control (LC), locoregional control (LRC), and overall survival (OS) at 2 years were 73%, 68%, and 63%, respectively. The 2 year LC, LRC and OS for laryngeal cancer were 77%, 75%, and 70%, respectively. For hypopharyngeal cancer, LC, LRC, and OS at 2 years were 54%, 45%, and 47%, respectively. LC and LRC were not significantly different between sites, but there was a trend toward improved OS in patients with laryngeal cancer, (P Z 0.06). Eleven patients underwent salvage laryngectomy (9 laryngeal and 2 hypopharyngeal). Altogether, 34 (68%) patients had a feeding tube placed; 6 (12.5%) before, 25 (50%) during, and 3 (6%) after radiation therapy. At the time of last follow up, only 14 patients (28%) remained feeding tube dependent. Outcomes looking at tumor volume, CTV expansion, and other dosimetric properties will be reported later. Conclusions: The use of IMRT achieved encouraging local and regional tumor control in patients with hypopharyngeal and laryngeal cancer. There is a trend toward worse overall survival in patients with hypopharyngeal compared to laryngeal carcinoma. Conclusions: With an initial dose level of olaparib 50mg BID, this combined modality approach of olaparib, C225, and RT appears to be a tolerable targeted treatment regimen without evidence of increased toxicity compared to a standard cetuximab and radiation therapy approach. A new type of order is identified in complex glasses, characterized by diffraction peaks at values of the wave vector below those typical of intermediate-range order. Combined neutron and anomalous x-ray diffraction studies of one glass exhibiting this behavior, vitreous rubidium germanate, indicate it to be associated with chemical ordering of the two cations with respect to each other.

Review of Neutron Sources and Applications was held at Oak Brook, Illinois, during September 8--10, 1992. This review involved some 70 national and international experts in different areas of neutron research, sources, and applications. Separate working groups were asked to (1) review the current status of advanced research reactors and spallation sources; and (2) provide an update on scientific, technological, and medical applications, including neutron scattering research in a number of disciplines, isotope production, materials irradiation, and other important uses of neutron sources such as materials analysis and fundamental neutron physics. This report summarizes the findings and conclusions of the different working groups involved in the review, and contains some of the best current expertise on neutron sources and applications.

A base-band modem/codec chip for the dual-mode North American IS-54 standard is shown. In the digital mode of operation, this chip acts as a modem. On the transmit side, it performs pi /4-shifted DQPSK modulation, square-root raised-cosine filtering, and waveform synthesis. The receive section helps to demodulate the signal by minimizing the intersymbol interference using square-root raised-cosine filters. In the analog mode of operation, the chip behaves like a codec. The transmit section supports part of the frequency modulation process while the receive section acts as an analog-to-digital interface. In addition to integrating the signal-processing sections of the transmit and receive paths, the chip also offers auxiliary controls for such functions as automatic gain control, automatic frequency control, and transmit power control. A wideband data detector is provided to decode the Manchester-encoded forward analog control channel messages in stand-by mode or forward analog voice channel messages. >

The East Spar gas condensate field is located in production licence WA-13-L in the offshore Carnarvon Basin. Production commenced in 1996 with two subsea wells linked to processing facilities on Varanus Island via a multi-phase pipeline. The pressure performance of the field has been significantly different to predevelopment expectations. This prompted a reexamination of the seismic and well data to investigate the potential for alternative reservoir models.Integrated stratigraphic and seismic interpretation reveals that the Barrow Group reservoir sands were deposited within an incised valley of limited lateral extent. Sea level fall instigated erosion of a valley that on transgression was filled with successive fluvial, estuarine and marine sediments. Good quality sands are expected to be limited to this valley, the upper part of which can be mapped on seismic. Poor sand development in East Spar–2ST is consistent with its location at the edge of the incised valley.Before development, the primary production mechanism was expected to be a strong bottom water drive comparable with other Barrow Group fields in the Carnarvon Basin. The revised depositional model, however, and the observed decline in reservoir pressure, indicate that connection to this regional aquifer is limited. This implies that water influx will probably be later, and ultimate recovery higher, than previously anticipated.

Time-of-flight total neutron scattering experiments using pulsed neutrons with short wavelength produced by an electron linear accelerator have been carried out to measure the structure factors of SiO2 and M2O · 2 SiO2 (M = Li, Na and Li0.63Na0.37) glasses over a wide range of the scattering vector up to Q (=4π sin θ/λ) ∼480 nm−1. The Placzek correction has been considered up to the second order in the energy transfer. The Fourier transform of the structure factors yield the pair distribution functions highly resolved in real space, from which the stretching of the SiO and OO atomic distances and the number of bridging and non-bridging oxygen atoms in the alkali disilicate glass have been obtained.

Neutron diffraction data are reported for measurements of P 0.4 Se 0.6 and P 0.5 Se 0.5 glasse incorporating both naturally abundant Se and 76 Se isotope. In conjunction with previous EXAFS measurements, the nearest-neighbor configuration for the two compositions is determined. The analysis confirms the model of molecular units linked by selenium atoms or chains, proposed in a previous paper. The limit of the glass-forming region just above 50% P concentration corresponds to a situation where there is only one linking Se atom per P 4 Se 3 unit.
