BACKGROUND. There is enough evidence of the negative impact of excess weight on the formation and progression of res piratory pathology. Given the continuing SARS-CoV-2 pandemic, it is relevant to determine the relationship between body mass index (BMI) and the clinical features of the novel coronavirus infection (NCI).AIM. To study the effect of BMI on the course of the acute SARS-COV-2 infection and the post-covid period.MATERIALS AND METHODS. AKTIV and AKTIV 2 are multicenter non-interventional real-world registers. The АКТИВ registry (n=6396) includes non-overlapping outpatient and inpatient arms with 6 visits in each. The АКТИВ 2 registry (n=2968) collected the data of hospitalized patients and included 3 visits. All subjects were divided into 3 groups: not overweight (n=2139), overweight (n=2931) and obese (n=2666).RESULTS. A higher BMI was significantly associated with a more severe course of the infection in the form of acute kidney injury (p=0.018), cytokine storm (pCONCLUSION. Overweight and/or obesity is a significant risk factor for severe course of the new coronavirus infection and the associated cardiovascular and kidney damage Overweight people and patients with the 1st and 2nd degree obesity tend to have a high risk of death of SARS-CoV-2 infection in both acute and post-covid periods. On top of that, in case of morbid obesity patients this tendency is statistically significant. Normalization of body weight is a strategic objective of modern medicine and can contribute to prevention of respiratory conditions, severe course and complications of the new coronavirus infection.

Nitric oxide (NO) plays an important pathogenetic role in vascular relaxation and is a candidate molecule of a common pathogenetic link in the development of arterial hypertension (AH) and tension-type headache (TTH).Objective of the study was to study the association of the single nucleotide variant (SNV) rs2297518 of the NOS 2 gene with the risk of developing AH and clinical “AH + TTH” phenotype in adults living in a large industrial city of Eastern Siberia.Design and methods. All participants (N = 91) were divided into two groups: group 1 (patients with AH) — 60 people, including the main subgroup (patients with AH without headache) — 30 people and a comparable subgroup (patients with clinical phenotype “AH + TTH”) — 30 people; group 2 (control — healthy volunteers) — 31 people. Carriage of the SNV rs2297518 gene NOS 2 (locus 17q11.2) was determined using real-time polymerase chain reaction.Results. The minor allele A rs2297518 of the NOS 2 gene was statistically significantly associated with a high risk of developing AH (odds ratio (OR) = 8,43 [95 % confidence interval (CI): 2,33–30,46], p = 0,000223) and phenotype “AH + TTH” (OR = 5,44 [95 % CI: 1,46–20,21], p = 0,006) compared with the control group. The heterozygous genotype GA rs2297518 of the NOS 2 gene also was statistically significantly associated with a high risk of developing AH (OR = 8,17 [95 % CI: 2,03–32,79], p = 0,001).Conclusions. The study demonstrated that the minor allele A of the SNV rs2297518 (26096597 G > A) of the NOS 2 gene, which encodes the inducible NO-synthase (iNOS), can be considered as a clinically significant genetic biomarker, first of all, of AH in the Caucasian population of Eastern Siberia. At the same time, future studies may clarify the role of this SNV as a genetic biomarker of the “AH + TTH” phenotype.

Aim. The detection of cognitive deficiency after heart bypass surgery, the evaluation of the influence of piribedil on the higher cerebral function in the early postoperative period during the follow-up. Materials and methods. 64 patients with ischemic heart disease were examined. They were divided into two groups. Patients of the main group (n=32) were treated with pronoran (piribedil) at a daily dose of 50 mg for 3 months. 32 patients of the control group received standard therapy in the postoperative period. All patients had undergone heart bypass surgery. Cognitive functions were examined before operation, 10 days , three and six months after surgery. Results. Positive dynamics of all tested parameters was documented in the patients of the main group within 6 months after the onset of therapy. In control patients, cognitive disorders remained at the preoperative level based on the results of three tests. Conclusion. The positive results of piribedil therapy suggest that piribedil has positive effect on memory, attention and psychomotor activity of the patients

Objective. Pharmacoeconomic analysis of the use of combinations of antihypertensive drugs were olmesartan/lercanidipine and zofenopril/lercanidipine in patients with essential hypertension II-III stage. Materials and methods. The authors performed prospective randomized uncontrolled study. Were examined in 27 people essential hypertension with II-III stage age from 40 to 71 years before and after the use of combinations of drugs olmesartan/lercanidipine and zofenopril/lercanidipine. An ABC/VEN-analysis, сost-effectiveness analysis, comparative assessment of the incidence of side effects of the studied drugs were performed. Results. Anti-hypertensive effect of the studied combinations of drugs were not significantly different and equally effectively reduced blood pressure. But the cost of blood pressure reduction at 1 mm Hg was the smallest in the group zofenopril/lercanidipine. Conclusion . The results of pharmacoeconomic analysis to be more cost-effective combination for the treatment of patients with essential hypertension II-III stage is the combination of zofenopril/lercanidipine.

Chronic headache is a topical problem of neurology, psychiatry and general practice. The medication-overuse headache (MOH) is one of the leading pathologies in the structure of chronic headache. However, early diagnosis of the MOH is challenging. We analyzed potential proteomic biomarkers of serum and urine in patients with MOH. Methods: We searched PubMed, Springer, Scopus, Web of Science, ClinicalKey, and Google Scholar databases for English publications over the past 10 years using keywords and their combinations. Results: We found and analyzed seven studies that met the search criteria for the purpose of the review, including 24 serum proteomic biomarkers and 25 urine proteomic biomarkers of MOH. Moreover, the candidate genes and locus of the studied serum (vitamin D-binding protein, lipocalin-type prostaglandin D2 synthase, apolipoprotein E, etc.) and urine proteomic biomarkers (uromodulin, alpha-1-microglobulin, zinc-alpha-2-glycoprotein, etc.) of MOH are presented in this review. Conclusions: The serum and urine proteomic biomarkers of MOH can potentially help with the identification of patients with MOH development. Due to the relevance of the problem, the authors believe that further investigation of the MOH proteomic biomarkers in different ethnic and racial groups of patients with primary headache is necessary. In addition, it is important to investigate whether medications of different drug classes influence the levels of serum and urine proteomic biomarkers.

We evaluated effectiveness of using copyrighted computer-based stimulation programs in the correction of cognitive function in patients with coronary heart disease after coronary bypass surgery.A total of 74 patients were examined, all the patients underwent a course of drug therapy, 37 patients underwent a course of rehabilitation in addition to medical therapy using computer-based stimulation programs (1 time per day for 20 min within 10 days). A course of rehabilitation using computer-based stimulation programs in patients with coronary heart disease after coronary bypass surgery was proved to be an effective way of correcting cognitive function.

Aim. To reveal cognitive deficit after coronary artery bypass, the influence of citicoline, piribedil on the state of higher cerebral functions in the early and late periods after surgery.Material and methods. The study included 94 patients with ischemic heart disease. All patients were divided into 3 groups. Patients of the first group (n=30) were prescribed citicoline as a cerebral neuroprotective drug. Patients of the second group (n=32) had piribedil in addition to standard therapy. Patients of the control group (n=32) had only a standard treatment without any neuroprotective drugs. All patients underwent coronary artery bypass surgery. The cognitive function was assessed before, 10 days after and six months after coronary artery bypass.Results. Patients of group 1 and 2 had achieved pre-surgical levels of cognitive tests results 6 months after coronary artery bypass. The control group had achieved initial levels only in three tests: visual memory (immediate simulation; p=0.008), categorical association (p=0.002), clock drawing test (Wilcoxon test; p=0,005), while other indices were reduced in comparison with the initial ones.Conclusion. The obtained results allow considering the studied drugs as a protectors of cognitive function after surgery. Randomized controlled double-blind studies on large samples are needed to confirm these results.

This review deals with the etiology, pathogenesis and prevalence of cognitive impairment in patients undergoing coro-nary bypass surgery. The importance of early diagnosis and early treatment of cognitive disorders in patients after coronary ar tery bypass grafting is emphasized. Несмотря на существенные достижения последних лет в области снижения смертности и повышения рождаемо-сти, демографическая ситуация в Российской Федерации остается неблагоприятной. Среди причин смертности населения в России, как и в большинстве развитых стран, ведущее место занимают болезни системы кровообращения. Начиная с 1975 года, доля умерших от этой патологии прочно занимает первое место в структуре общей смертности населения. В 2009 году в России от болезней системы кровообращения умерло 1136,7 тыс. человек, на их долю приходилось 56,5% всех смертей [30,33]. Среди заболеваний сердечно-сосудистой системы (ССС), являющихся причиной смерти, основными остаются ИБС, в том числе инфаркт миокарда, церебро-васкулярные болезни и гипертоническая болезнь [35,39]. Известно, что до 60% сердечно-сосудистой смертности за-висит от распространенности в популяции так называемых факторов риска, к которым относятся артериальная гипер-тония, нарушения углеводного обмена, ожирение, курение, чрезмерное употребление алкоголя, низкая физическая активность, иными словами, – элементы нездорового об-раза жизни [25,41]. В настоящее время ИБС занимает ведущую позицию среди сердечно- сосудистых заболеваний как по при-чине смертности, так и по инвалидности трудоспособ-ного населения России. Консервативное лечение у этих пациентов зачастую малоэффективно и хирургическая реваскуляризация миокарда является единственным ме-тодом лечения, улучшающим качество жизни и прогноз [5,9,16,58]. В настоящее время доказано, что коронарное шунтирование (КШ) является лучшим методом хирурги-ческой реваскуляризации для таких пациентов [53,55]. При этом у такого рода пациентов наблюдается высокая частота периоперационных осложнений, среди которых особое внимание заслуживают цереброваскулярные [2,23,28, 38,50].Повреждение головного мозга после оперативного вмешательства на сердце является актуальной проблемой, несмотря на улучшение хирургических и внедрение ней-ропротективных методов. Тем не менее, на сегодняшний день пациенты, направляемые на кардиохирургические операции, стали старше и имеют больше коморбидной патологии [34,46]. Существуют две клинические формы послеопера-ционного цереброваскулярного осложнения после КШ: I тип – ОНМК и II тип – когнитивные расстройства. По данным разных авторов, послеоперационная когнитивная дисфункция (ПОКД) встречается от 5 до 80 % в послеопе-рационном периоде КШ [7,15,34, 45,56,59]. Однако до на-стоящего времени не сложилось единого мнения о частоте неврологических осложнений после операций на сердце в условиях искусственного кровообращения (ИК).

To evaluate the efficacy of computer-based stimulation programs in the correction of postoperative cognitive dysfunction in patients with ischemic heart disease in remote period after coronary artery bypass surgery.The study involved 74 patients. All patients underwent a drug therapy and the rehabilitation course using computer-based stimulation programs (1 time per day for 20 minutes during10 days). Coronary artery bypass surgery was performed for all patients. Patients were examined before operation, after 6 and 12 months.The significant improvement of higher cortical functions assessed with the FAB scale, the Schulte test, associated thinking test, ten-word retrieval test at the first attempt, the Clock drawing test) was revealed in the group using computer-based stimulation programs in comparison with the control group.It is found that rehabilitation course using computer-based stimulation programs in patients with ischemic heart disease after coronary artery bypass surgery is the effective method of cognitive function correction.Цель исследования. Оценить эффективность использования компьютерных стимулирующих программ в коррекции послеоперационных когнитивных дисфункций у больных с ишемической болезнью сердца в отдаленном периоде после операции коронарного шунтирования (КШ). Objective. To carry out a pharmacoeconomic analysis of the antihypertensive therapy in elderly patients of high car-diovascular risk by the means of the method assessing «expenses — benefi ts», cost minimization, the increment of cost effi ciency. Results. The cost of the daily effi cient dose of the original (Renitec, MSD) and generic Enalapril (Hemofarm) was 6,54 and 7,25 rubles, respectively. Minimal coeffi cient «expenses — benefi ts» was defi ned in the group of patients re-ceiving two drug combination therapy including original Enalapril (Renitec, MSD) compared to combination of three drugs including generic Enalapril (Hemofarm) while the antihypertensive effect was comparable. The cost effi ciency increment (CER incr) was 169,12 rubles regarding original Enalapril (Renitec, MSD) benefi t. Patients with ischemic heart disease (IHD) with depressive RAS disabilities are a special group of dispensary observation with a higher risk of cardiovascular complications than patients without depression. Timely detection of depressive disorders in IHD patients, the study of psychosocial factors, along with the known factors of cardiovascular risk, are essential components of modern diagnostic, medical and rehabilitation programs for patients with comorbid pathology in the polyclinic.

We undertook the analysis of cardiovascular risk factors and psycho-social factors in 230 patients with coronary heart disease (CHD) based at a city polyclinic. Ninety (31.1%) patients suffered depression (19-28 CES-D scores). Patients with and without depression were significantly different in terms of clinical and functional manifestations of stable angina of effort, cardiovascular risk factors, gender; and psycho-social factors. CHD with depression was associated with a higher FC of angina of effort, risk of adverse outcome (Duke index), frequency of concomitant pathology, psycho-social stress, loss of working capacity and a lower income levels. These patients treated with selective antidepressants (serotonin reuptake inhibitors) for 6 months experienced a marked decrease in the degree of depression and manifestation of its symptoms, improvedpsycho- social protection and quality of life compared with untreated patients. These data illustrate pathogenetic and behavioural mechanisms underlying the relationship between CHD and depression.

To work out the techniques of forecasting the development of postoperative cognitive dysfunction in early and late periods after coronary bypass surgery using fully connected multilayer neural networks.Based on the study of 264 patients with stroke, 9 neural network models for prognosis of cognitive dysfunction after coronary bypass surgery and 7 models for prognosis of cognitive dysfunction 12 months after surgery were built. Fully connected multilayer neural networks of direct propagation were used for working-out the prognostic techniques.The high prognostic significance of neural networks in relation to cognitive disorders in early and late periods after coronary bypass surgery under extracorporeal circulation was demonstrated. Possibilities of creating neural networks make their application in cardiac surgery clinics available and promising.Цель исследования. Разработать методику прогнозирования развития послеоперационной когнитивной дисфункции в раннем и отдаленном периодах после коронарного шунтирования с использованием полносвязной многослойной нейронной сети. Материал и методы. На основе данных обследования 264 пациентов были построены 9 моделей нейронной сети для прогнозирования развития когнитивной дисфункции после операции коронарного шунтирования и 7 моделей нейронной сети для прогнозирования развития когнитивной дисфункции через 12 мес после операции. Для разработки методики использовались полносвязные многослойные нейронные сети прямого распространения. Результаты и заключение. Показана высокая прогностическая ценность нейронных сетей в отношении когнитивных нарушений в раннем и более позднем периодах после операции коронарного шунтирования в условиях искусственного кровообращения. Возможности построения нейронных сетей делают доступным и перспективным их применение в условиях кардиохирургических стационаров.

The study of hemostasis and NAD(P)-dependent platelet dehydrogenases activity was made in 90 patients with cerebrovascular complications (development of ischemic stroke) of hypertension and coronary heart disease. On the basis of NADP-dependent platelet dehydrogenases activity we suggested a mode of prognostication a development of repeated cerebrovascular complications of thrombotic nature (the acute stroke, transitory ischemic attack, acute myocardial infarction, cardiovascular death). The proposed prognostic factor has heightened sensibility and specificity.

Neurorehabilitation of poststroke cognitive impairments is an important medical problem. The purpose of the present study was to estimate the efficacy of new methods of restoration of impaired cognitive functions using computer correction programs. 43 poststroke patients aged 57-69, (male - 23, female - 20) were randomized into two groups. First group patients have been treated with the standard methods and supplementary neuropsychological computer training for 14days, 25-35min of duration per day. Control group received standard treatment according to Federal and local medical recommendations. Initial and achieved levels of cognitive functioning were estimated with the use of Mini Mental State Examination, Frontal Assessment Battery, the Clock Drawing Test, the Montreal Cognitive Assessment, Schulte's test, Hospital Anxiety and Depression Scale. We found that including the computer correction programs into the complex protocol of rehabilitation of post-stroke patients confirmed their efficacy in both clinical aspects and the Patient Global Impression Scale. Although the results are encouraging, further studies are required with larger samples and longer follow-up to identify characteristics of those patients who are most likely to benefit from computer training of cognitive functions.

The purpose of the present research was the estimation of efficiency of new methods of neurorehabilitation of impairments of cognitive functions with the use of computer programs of correction. Twenty six post-stroke patients aged 57–69, (male - 12, female - 14) were examined and treated. In addition to usual restorative treatment all patients received a course of neuropsychological computer programs training within 14 days, 25–35 min of duration. Tasks included training of attention with use of the computed programs on the basis of Schulte's test, the task for training visual storing with a set of pictures and symbols, the switching test, correction optical and spatial gnosis with test of narrative images and the test of «arrangements of hands of the clock» with possibility of a feedback. Initial level cognitive impairments and results of restoration were estimated with the use of Mini Mental State Examination, Frontal Assessment Battery, the Clock Drawing Test, the Montreal Cognitive Assessment, Schulte's test. The first experience of inclusion of the computed programs of correction of cognitive impairments in schemes of neurorehabilitation has shown good effect concerning clinical displays, and concerning Patient Global Impression Scale. Although the results are encouraging, further studies are required with larger samples and longer follow-up to identify characteristics of those patients who are most likely to benefit from computed training of cognitive functions.

Железнодорожный транспорт для России является государствообразующей отраслью. На фоне неуклонного процесса интеграции России с мировой экономикой, российские железные дороги активно включаются в мировую транспортную систему. В 2010 году размер транзитных перевозок грузов возрос в 3 раза по сравнению с 2005 годом. Сегодня, несмотря на постоянную, планомерную работу по охране труда и профилактике нарушений здоровья трудящихся, в России отмечается усиление интенсивности стресса на производстве, не уменьшается число профессиональных болезней и все более расширяется спектр производственно зависимых заболеваний. На железнодорожном транспорте России проводится постоянная целенаправленная работа по охране труда и улучшению условий трудящихся отрасли, которая в настоящее время осуществляется в рамках отраслевой программы «Безопасность труда на железнодорожном транспорте». Артериальная гипертония (АГ) является одним из самых распространенных заболеваний, возникновение которого связано со значительным увеличением риска развития сердечно-сосудистых осложнений и смертности. В жизни человека трудовые процессы играют огромную роль, поэтому следует знать, как работа влияет на сердечно-сосудистую систему, в частности на заболеваемость гипертонической болезнью. Достаточно часто повышенное артериальное давление отмечается у лиц, профессии которых связаны с частыми нервно-психическими стрессами (например, у административных и научных работников, шоферов, рабочих со сдельной оплатой труда и т.д.). В настоящее время имеются редкие исследования, посвященные изучению состояния сердечно-сосудистой системы при влиянии производственных вредностей, изучению системного управления профессиональным риском и оценки уровня здоровья, степени приверженности лечению у работников такой сложной сферы, которой является железнодорожный транспорт.
