Urothelial carcinoma is one of the most common solid malignancies. Moreover, urothelial cancer of the ureter of a single kidney is a rare entity that requires nephron-sparing treatment and carries a high risk of postoperative recurrence. An analysis of the literature on this disease was conducted using the databases of PubMed, Scopus, and Web of Science for the period from 2011 to 2022. A clinical case of recurrence of urothelial cancer of the pyelocaliceal system of the kidney in a 73-year-old patient was presented. For the primary tumor, the patient underwent percutaneous laser en bloc resection on the narrow base of the tumor. In case of recurrence, given the acute angle of attack and the wide tumor base, bipolar loop resection became necessary. This clinical observation demonstrates the need for logistical equipment with laser, mono- and bipolar energy sources, and flexible nephroscopes for the most effective and personalized treatment.

Objective. This article describes the first experience of using an oxygenated salt intestinal solution (SIS) in the complex intensive care of functional intestinal failure (FIF) developed due to pancreonecrosis in a child aged 10 years 11 months after a blunt abdominal injury and burdened with concomitant alimentary factor. Complex treatment was aimed to eliminate increasing endotoxicosis, pain syndrome, suspected secondary ischemia of the affected organs of the pancreaticoduodenal zone, restoration of motor and barrier functions of the intestine. In this connection, adjustments were made to the early enteral nutrition and enteral correction method using an oxygenated salt enteral solution (with a description of the oxygenation method), which allowed for 12 h to eliminate intestinal paresis, with the production of stool. According to the intestinal lavage method, repeated use of SIR-RA led to the significant persistent improvement in the patients condition and a decrease in inflammatory markers, which subsequently allowed the transition to adequate enteral nutrition. The patient was subsequently transferred to a specialized surgical Department and discharged from the hospital, and recovered with no signs of endocrine pancreatic insufficiency. Positive treatment results of a patient with pancreonecrosis complicated by FIF were made possible by improving intensive therapy tactics using an oxygenated salt enteral solution. The scheme of gradual enteral correction developed and used by us is an essential component of complex therapy of pancreonecrosis and functional intestinal insufficiency. The use of an oxygenated salt enteral solution helps to restore the main functions of the gastrointestinal tract.

The study objective is to evaluate the effectiveness and safety of the original and non-original sevoflurane during urological operations in children.Materials and methods. Patients of urological profile 3—6 years old without comorbid pathology underwent surgery lasting no more than 40 min under inhalation anesthesia: in group 1 — original sevoflurane (n = 11), group 2 — non-original sevoflurane (n = 11). The depth of anesthesia was evaluated using bispectral index monitoring.Results. In the group of non-original sevoflurane, the excitation syndrome before the beginning of anesthesia and in its introductory period was noted in 54.5 and 72.7 % respectively, the reaction to the introduction of a laryngeal mask — in 36.4 % of patients in the absence of these reactions in the group of the original drug. There was a higher depth of anesthesia in non-original drug with the achievement of statistically significant differences in the indicator at 15 and 20 min: 44 (42; 46) vs 50 (49; 54; p = 0.0001) and 42 (40; 46) vs 50 (47; 56; p = 0.003) respectively, as well unstable level of depth of anesthesia of the group of non-original sevoflurane compared to the original during the period of maintaining anesthesia.Conclusion. Despite the same formula of the original and non-original sevoflurane, the non-original drug was characterized by a greater frequency of excitation syndrome and reaction to the introduction of a laryngeal mask and unstable depth of anesthesia.

IR spectroscopy and quantum chemical methods are used to study the features of hydration and determine concentration structural areas in the HCOOH-H2O system. Data on the composition and structure of stable heteroassociates (HA) formed in HCOOH aqueous solutions are obtained from the joint analysis of the IR spectra of these solutions and the results of quantum chemical calculations of (HCOOH)m·(H2O)n (m = 1−4, 6, n = 1−10, 14) and HCO 2 − ·(H2O)n (n = 2, 6) complexes. The sequence is established in which these complexes form when pure acid is diluted with water. For HCOOH concentrations from 100% to the ratio HCOOH:H2O = 1:1, all water molecules are involved in the most stable cyclic (HCOOH)2· (H2O)2 HAs with the C2 symmetry. The components of the equimolar solution are almost completely bound into such hydrates. For the HCOOH:H2O ratio ranging from 1:1 to 1:2, less stable (by ∼10%) cyclic HCOOH · (H2O)2 HAs form along with the (HCOOH)2 · (H2O)2 complexes. The solution with a twofold water excess consists of these hydrates. A further increase in the water concentration results in further hydration of HCOOH · (H2O)2 HAs.
