1. The effectiveness of prehospital hypertonic saline for hypotensive trauma patients: a systematic review and meta-analysis

Abstract: Abstract Background The optimal prehospital fluid for the treatment of hypotension is unknown. Hypertonic fluids may increase circulatory volume and mute the pro-inflammatory response of the body to injury and illness. The purpose of this systematic review is to determine whether in patients presenting with hypotension in the prehospital setting (population), the administration of hypertonic saline (intervention), compared to an isotonic fluid (control), improves survival to hospital discharge (outcome). Methods Searches were conducted in Medline, Embase, CINAHL, and CENTRAL from the date of database inception to November, 2016, and included all languages. Two reviewers independently selected randomized control trials of hypotensive human participants administered hypertonic saline in the prehospital setting. The comparison was isotonic fluid, which included normal saline, and near isotonic fluids such as Ringer’s Lactate. Assessment of study quality was done using the Cochrane Collaborations’ risk of bias tool and a fixed effect meta-analysis was conducted to determine the pooled relative risk of survival to hospital discharge. Secondary outcomes were reported for fluid requirements, multi-organ failure, adverse events, length of hospital stay, long term survival and disability. Results Of the 1160 non-duplicate citations screened, thirty-eight articles underwent full-text review, and five trials were included in the systematic review. All studies administered a fixed 250 ml dose of 7.5% hypertonic saline, except one that administered 300 ml. Two studies used normal saline, two Ringer’s Lactate, and one Ringer’s Acetate as control. Routine care co-interventions included isotonic fluids and colloids. Five studies were included in the meta-analysis (n = 1162 injured patients) with minimal statistical heterogeneity (I 2 = 0%). The pooled relative risk of survival to hospital discharge with hypertonic saline was 1.02 times that of patients who received isotonic fluids (95% Confidence Interval: 0.95, 1.10). There were no consistent statistically significant differences in secondary outcomes. Conclusions There was no significant difference in important clinical outcomes for hypotensive injured patients administered hypertonic saline compared to isotonic fluid in the prehospital setting. Hypertonic saline cannot be recommended for use in prehospital clinical practice for the management of hypotensive injured patients based on the available data. 2. Hospital and long-term care facility environmental service workers' training, skills, activities and effectiveness in cleaning and disinfection: a systematic review

Abstract: Multi-drug-resistant organisms (MDROs) (meticillin-resistant Staphylococcus aureus, vancomycin-resistant enterococcus, MDR Acinetobacter spp., extended-spectrum beta-lactamase-producing Gram-negative organisms), Clostridioides difficile, viral respiratory pathogens and healthcare-associated infections (HAIs) are major concerns in medical facilities.To assess environmental service workers' (ESWs) training, workload, numbers of surfaces and pieces of medical equipment cleaned per shift, how long they worked on each surface, how they cleaned, supervision, MDRO and HAI status, and measurement of effectiveness in reducing MDROs and HAIs in patients and on surfaces.CINAHL, Cochrane CENTRAL Register of Controlled Trials, EMBASE, Medline and Scopus were searched from inception to 28Fourteen cluster RCTs provided data on a series of interventions. Ten trials focused on reducing patient infections from MDROs/HAIs, and four found significant reductions in patient infection rates with strategies including bleach, quaternary ammonium detergents, ultraviolet light and hydrogen peroxide vapour. Minimal information was provided on ESW numbers, MDRO/HAI status, workload, numbers of surfaces and objects cleaned per shift, and effects of training on improving effectiveness. The findings suggest that there is a need for detailed evaluations of the effects of training and disinfection activities for individuals and teams of ESWs.

Abstract: We present an optical/near-IR survey of 11 variable young stars (EXors and EXor candidates) aimed at deriving and monitoring their accretion properties. About 30 optical and near-infrared spectra ($\Re$ $\sim$ 1500-2000) were collected between 2014-2019 with the Large Binocular Telescope (LBT). From the spectral analysis we have derived the accretion luminosity and mass accretion rate, the visual extinction, the temperature and density of the permitted line formation region, and the signature of the outflowing matter. Two sources (ASASSN-13db and iPTF15afq) have been observed in outburst and quiescence, three during a high-level of brightness (XZ Tau, PV Cep, and NY Ori), and the others in quiescence. These latter have accretion luminosity and mass accretion rates in line with the values measured in classical T Tauri stars of similar mass. All sources observed more than once present accretion variability. The most extreme case is ASASSN-13db, for which the mass accretion rate decreases by two orders of magnitude from the outburst peak in 2015 to quiescence in 2017. Also, in NY Ori the accretion luminosity decreases by a factor 25 in one year. In 80% of the sample we detect the [OI]630 nm line, a tracer of mass loss. From the variability of the H$\alpha$/[OI]630, ratio, we conclude that mass accretion variations are larger than mass loss variations. Abstract: Young stellar objects in their pre-main sequence phase are characterized by irregular changes in brightness, generally attributed to an increase of the mass accretion rate due to various kind of instabilities occurring in the circumstellar disk. In the era of large surveys aimed to monitor the sky, we present a pipeline to detect irregular bursts, in particular EXors-like ( EX Lupi type eruptive variables), in the light curves. Abstract: Interference-based forgetting occurs when new information acquired either before or after a learning event attenuates memory expression (proactive and retroactive interference, respectively). Multiple learning events often occur in rapid succession, leading to competition between consolidating memories. However, it is unknown what factors determine which memory is remembered or forgotten. Here, we challenge the snail, Lymnaea, to acquire two consecutive similar or different memories and identify learning-induced changes in neurons of its well-characterized motor circuits. We show that when new learning takes place during a stable period of the original memory, proactive interference only occurs if the two consolidating memories engage the same circuit mechanisms. If different circuits are used, both memories survive. However, any new learning during a labile period of consolidation promotes retroactive interference and the acquisition of the new memory. Therefore, the effect of interference depends both on the timing of new learning and the underlying neuronal mechanisms., Michael Crossley et al. use the snail Lymnaea to investigate the factors involved in determining which memories are remembered or forgotten when multiple learning events happen close together. They find that timing of events and whether the same neuronal mechanisms are used are key factors in determining whether interference will occur.

Abstract: In the Azienda Ospedaliera Universitaria Policlinico Umberto I in Rome, the Hospital Social Services (HSS) is located within the Directorate of Health, reporting directly to the Chief Medical Officer, providing counselling and supporting clinical services. The HSS is part of a network with its own technical, professional and assessment independence. It often serves as liaison between the hospital and the territory, facilitating the development of services and contributing to public health recovery and maintenance, therefore improving the citizens' standard of living thanks to aid projects and specific interventions.The present Report is based on two different studies carried out in 2008 and 2014, both examining the work of the Hospital Social Service in the "Azienda Ospedaliera Universitaria Policlinico Umberto I" in Rome. The purpose is to compare these surveys and work out the results. The data collection is based on a number of social records from the HSS archives (814 records in 2008 and 790 in 2014). The research project followed subsequent stages: planning a draft of the research, where ethnomethodology was used as empirical evaluation technique; collecting data from the HSS's paper and file archives (biographical, clinical and social data); revising, analysing and elaborating the data which showed relevant changes leading to interesting conclusions.The comparative analysis of data showed a higher demand of HSS healthcare services, despite a smaller number of beds and hospitalisations available in standard regime. Also, it indicated an increase of patients below 18 years and a decrease of the over-65s age group. As for the geographical origin of patients reported to the HSS, there was a decrease in the percentage of Italian citizens, while the percentage of irregular non-EU and EU patients increased by over 5%. Significant results were found comparing the days between the report to the HSS and patient discharge. Data concerning the 'more than 7 days' group was steady over the years, being more consistent for both the variables considered and increasing in 2014. In the comparative analysis, the interventions with more significant differences were those aimed at promoting homecare and entering sheltered housing. In the former cases, a considerable decrease was observed, if compared to the activation of the integrated health and social homecare services, while there were more requests for homecare assistance; in the latter ones the analysis highlighted a sharp decrease in the percentage of the variable called "assessment to enter Extended Care Units". The causes of these changes are remarkable and have to be found in the political, historical and cultural scenario: - a first factor is the increase in the more recent migratory flow from non-EU countries to Italy, which could explain the rising percentage of foreigners taken over by the HSS; - regional policies, economic cuts imposed on healthcare and higher income limits in order to calculate the patient's economic participation in the costs of institutionalisation have affected the above-mentioned changes; - the innovations in the regulatory field of Latium Region have brought structural changes in long-term care facilities and in the level of care in Extended Care Units (ECU).

Abstract: Eruptive variables are young stars that show episodic variations of brightness: EXors/FUors variations are commonly associated with enhanced accretion outbursts occurring at intermittent cadence of months/years (EXors) and decades/centuries (FUors). Variations that can be ascribed to a variable extinction along their line of sight are instead classified as UXors. We aim at investigating the long-term photometric behaviour of three sources classified as eruptive variables. We present data from the archival plates of the Asiago Observatory relative to the fields where the targets are located. For the sake of completeness we have also analysed the Harvard plates of the same regions that cover a much longer historical period, albeit at a lower sensitivity, however we are only able to provide upper limits. A total of 273 Asiago plates were investigated, providing a total of more than 200 magnitudes for the three stars, which cover a period of about 34 yr between 1958 and 1991. We have compared our data with more recently collected literature data. Our plates analysis of V2492 Cyg provides historical upper limits that seem not to be compatible with the level of the activity monitored during the last decade. Therefore, recently observed accretion phenomena could be associated with the outbursting episodes, more than repetitive obscuration. While a pure extinction does not seem the only mechanism responsible for the ASASSN-15qi fluctuations, it can account quite reasonably for the recent V350 Cep variations., Comment: 12 pages, accepted by A&A

Abstract: After a quiescence period of about 10 years, the classical EXor source V1118 Ori has undergone an accretion outburst in 2015 September. The maximum brightness (DV > 4 mag) was reached in 2015 December and was maintained for several months. Since 2016 September, the source is in a declining phase. Photometry and low/ high-resolution spectroscopy were obtained with MODS and LUCI2 at the {\it Large Binocular Telescope}, with the facilities at the Asiago 1.22 and 1.82 m telescopes, and with GIANO at the {\it Telescopio Nazionale Galileo}. The spectra are dominated by emission lines of \hi\ and neutral metallic species. From line and continuum analysis we derive the mass accretion rate and its evolution during the outburst. Considering that extinction may vary between 1.5 and 2.9 mag, we obtain m_acc= 0.3$-$2.0 10$^{-8}$ m_sun/yr, in quiescence and m_acc= 0.2$-$1.9 10$^{-6}$ m_sun/yr, at the outburst peak. The Balmer decrement shape has been interpreted by means of line excitation models, finding that from quiescence to outburst peak, the electron density has increased from $\sim$ 2 10$^9$ cm$^{-3}$ to $\sim$ 4 10$^{11}$ cm$^{-3}$. The profiles of the metallic lines are symmetric and narrower than 100 km s$^{-1}$, while \hi\, and \hei\,\,lines show prominent wings extending up to $\pm$ 500 km s$^{-1}$. The metallic lines likely originate at the base of the accretion columns, where neutrals are efficiently shielded against the ionizing photons, while faster ionized gas is closer to the star. Outflowing activity is testified by the detection of a variable P Cyg-like profile of the H$\alpha$ and \hei\, 1.08\,$\mu$m lines., Comment: Accepted by ApJ

Abstract: Although classical and operant conditioning are operationally distinct, it is unclear whether these two forms of learning are mechanistically distinct or similar. Feeding behavior ofAplysiaprovides a useful model system for addressing this issue. Both classical and operant appetitive behavioral training enhance feeding, and neuronal correlates have been identified. Behavioral training was replicated byin vitroanalogs that use isolated ganglia. Moreover, a single-cell analog of operant conditioning was developed using neuron B51, a cell important for the expression of the conditioned behavior. Here, a single-cell analog of classical conditioning was developed. Acetylcholine (ACh) mediated the conditioned stimulus (CS)-elicited excitation of B51 in ganglia and mimicked the CS in the single-cell analog of classical conditioning. Pairing ACh with dopamine, which mediates the unconditioned stimulus in ganglia, decreased the excitability of B51, and increased the CS-elicited excitation of B51, similar to results following bothin vivoandin vitroclassical training. Finally, a D1dopamine receptor (D1R) agonist failed to support classical conditioning in the cellular analog, whereas D1R mediates reinforcement in operant conditioning.

This observational, nonrandomized, multicenter study evaluated EBT as a post-surgical adjuvant radiation therapy for early stage breast cancer.Methods: This study included women aged 50 years or more with invasive carcinoma or ductal carcinoma in situ, tumor size ≤3 cm, negative lymph node status, and negative surgical margins. The endpoints were skin and subcutaneous toxicities, efficacy outcomes, cosmetic outcomes, and device performance. In this interim report, 1-month, 6-month, and 1-year follow-up data are available on 68, 59, and 37 patients, respectively.Results: The EBT device performed consistently, delivering the prescribed 34 Gy to all 69 patients (10 fractions/patient). Most adverse events were Grade 1 and included firmness, erythema, breast tenderness, hyperpigmentation, pruritis, field contracture, seroma, rash/desquamation, palpable mass, breast edema, hypopigmentation, telangiectasia, and blistering, which were anticipated. Breast infection occurred in 2 (2.9%) patients. No tumor recurrences were reported. Cosmetic outcomes were excellent or good in 83.9%–100% of evaluable patients at 1 month, 6 months, and 1 year.Conclusion: This observational, nonrandomized, multicenter study demonstrates that this EBT device was reliable and well tolerated as an adjuvant radiation therapy for early stage breast cancer.Keywords: radiation therapy, electronic brachytherapy, breast cancer

Abstract: This paper presents the observations of the Cloud D in the Vela Molecular Ridge, obtained with the IRAC camera onboard the Spitzer Space Telescope at the wavelengths \lambda = 3.6, 4.5, 5.8, 8.0 {\mu}m. A photometric catalog of point sources, covering a field of approximately 1.2 square degrees, has been extracted and complemented with additional available observational data in the millimeter region. Previous observations of the same region, obtained with the Spitzer MIPS camera in the photometric bands at 24 {\mu}m and 70 {\mu}m, have also been reconsidered to allow an estimate of the spectral slope of the sources in a wider spectral range. A total of 170,299 point sources, detected at the 5-sigma sensitivity level in at least one of the IRAC bands, have been reported in the catalog. There were 8796 sources for which good quality photometry was obtained in all four IRAC bands. For this sample, a preliminary characterization of the young stellar population based on the determination of spectral slope is discussed; combining this with diagnostics in the color-magnitude and color-color diagrams, the relative population of young stellar objects in the different evolutionary classes has been estimated and a total of 637 candidate YSOs have been selected. The main differences in their relative abundances have been highlighted and a brief account for their spatial distribution is given. The star formation rate has been also estimated and compared with the values derived for other star forming regions. Finally, an analysis of the spatial distribution of the sources by means of the two-point correlation function shows that the younger population, constituted by the Class I and flat-spectrum sources, is significantly more clustered than the Class II and III sources., Comment: Accepted by ApJ

Abstract: Introduction: EMS time factors such as total prehospital, activation, response, scene and transport intervals have been used as a measure of EMS system quality with the assumption that shorter EMS time factors save lives. The objective was to assess in adults and children accessing ground EMS (population), whether operational time factors (intervention and control) were associated with survival at hospital discharge (outcome). Methods: Medline, EMBASE, and CINAHL were searched up to January 2015 for articles reporting original data that associated EMS operational time factors and survival. Conference abstracts and non-English language articles were excluded. Two investigators independently assessed the candidate titles, abstracts, and full text with discrepant reviews resolved by consensus. Risk of bias was assessed using GRADE. Results: A total of 10,151 abstracts were screened for potential inclusion, 199 articles were reviewed in full-text, and 73 met inclusion criteria. Amongst included studies, 49 investigated response time, while 24 investigated other time factors. All articles were observational studies. Amongst the 14 (28.6%) studies where response time was the primary analysis, statistically significant associations between shorter response time and increased survival were found in 5 of 7 cardiac arrest, 1 of 5 general EMS population, and 0 of 2 trauma studies. Other time factors were reported in the primary analysis in 10 (41.7%) studies. One study reported shorter combined scene and transport intervals associated with increased survival in acute heart failure patients. Two studies in trauma patients had somewhat conflicting results with one study reporting shorter prehospital interval associated with increased survival whereas the other reported increased survival associated with longer scene and transport intervals. Study design, analysis, and methodological quality were of considerable variability, and thus, meta-analyses were not possible. Conclusion: There is a substantial body of literature describing the association between EMS time factors and survival, but evidence informing these relationships are heterogeneous and complex. Important details such as patient population, EMS system characteristics, and analytical approach must be taken into consideration to appropriately translate these findings to practice. These results will be important for EMS leaders wishing to create evidence-based time policies.

Abstract: Introduction: Clinical decision support (CDS) has been implemented in many clinical settings in order to improve decision-making. Their potential to improve diagnostic accuracy and reduce unnecessary testing is well documented; however, their effectiveness in impacting physician practice in real world implementations has been limited by poor physician adherence. The objective of this systematic review and meta-regression was to establish the effectiveness of CDS tools on adherence and identify which characteristics of CDS tools increase physician use of and adherence. Methods: A systematic review and meta-analysis was conducted. MEDLINE, EMBASE, PsychINFO, the Cochrane Central Register of Controlled Trials and Cochrane Database of Systematic Reviews were searched from inception to June 2017. Included studies examined CDS in a hospital setting, reported on physician adherence to or use of CDS, utilized a comparative study design, and reported primary data. All tool type was classified based on the Cochrane Effective Practice and Organization of Care (EPOC) classifications. Studies were stratified based on study design (RCT vs. observational). Meta-regression was completed to assess the different effect of characteristics of the tool (e.g. whether the tool was mandatory or voluntary, EPOC classifications). Results: A total of 3,359 candidate articles were identified. Seventy-two met inclusion criteria, of which 46 reported outcomes appropriate for meta-regression (5 RCTs and 41 observational studies). Overall, a trend of increased CDS use was found (pooled RCT OR: 1.36 [95% CI: 0.97-1.89]; pooled observational OR: 2.12 [95% CI: 1.75-2.56]).When type of tool is considered, clinical practice guidelines were superior compared to other interventions (p=.150). Reminders (p=.473) and educational interventions (p=.489) were less successful than other interventions. Multi-modal tools were not more successful that single interventions (p=.810). Lastly, voluntary tools may be supperior to than mandatory tools (p=.148). None of these results are statistically significant. Conclusion: CDS tools accompanied by a planned intervention increases physician utilization and adherence to the tool. Meta-regression found that clinical practice guidelines had the biggest impact on physician adherence although not statistically significant. Further research is required to understand the most effective intervention to maximize physician utilization of CDS tools.

Abstract: The aim of this paper is to identify the young protostellar counterparts associated to dust millimeter cores of the Vela Molecular Ridge Cloud D through new IR observations (H_2 narrow-band at 2.12 micron and N broad band at 10.4 micron) along with an investigation performed on the existing IR catalogues. The association of mm continuum emission with infrared sources from catalogues (IRAS, MSX, 2MASS), JHK data from the literature and new observations, has been established according to spatial coincidence, infrared colours and spectral energy distributions. Only 7 out of 29 resolved mm cores (and 16 out of the 26 unresolved ones) do not exhibit signposts of star formation activity. The other ones are clearly associated with: far-IR sources, H_2 jets or near-IR objects showing a high intrinsic colour excess. The distribution of the spectral indices pertaining to the associated sources is peaked at values typical of Class I objects, while three objects are signalled as candidates Class 0 sources. We remark the high detection rate (30%) of H_2 jets driven by sources located inside the mm-cores. They appear not driven by the most luminous objects in the field, but rather by less luminous objects in young clusters, testifying the co-existence of both low- and intermediate-mass star formation. The presented results reliably describe the young population of VMR-D. However, the statistical evaluation of activity vs inactivity of the investigated cores, even in good agreement with results found for other star forming regions, seems to reflect the limiting sensitivity of the available facilities rather than any property intrinsic to the mm-condensations., Comment: 38 pages. Abstract: V1118 Ori is an eruptive variable belonging to the EXor class of Pre-Main Sequence stars whose episodic outbursts are attributed to disk accretion events. Since 2006, V1118 Ori is in the longest quiescence stage ever observed between two subsequent outbursts of its recent history. We present near-infrared photometry of V1118 Ori carried out during the last eight years, along with a complete spectroscopic coverage from 0.35 to 2.5 um. A longterm sampling of V1118 Ori in quiescence has never been done, hence we can benefit from the current circumstance to determine the lowest values (i.e. the zeroes) of the parameters to be used as a reference for evaluating the physical changes typical of more active phases. A quiescence mass accretion rate between 1--3 $\times$ 10$^{-9}$ M$_{\sun}$ yr$^{-1}$ can be derived and the difference with previous determinations is discussed. From line emission and IR colors analysis a visual extinction of 1-2 mag is consistently derived, confirming that V1118 Ori (at least in quiescence) is a low-extinction T Tauri star with a bolometric luminosity of about 2.1 L$_{\sun}$. An anti-correlation exists between the equivalent width of the emission lines and the underlying continuum. We searched the literature for evaluating whether or not such a behaviour is a common feature of the whole class. The anti-correlation is clearly recognizable for all the available EXors in the optical range (H$\beta$ and H$\alpha$ lines), while it is not as much evident in the infrared (Pa$\beta$ and Br$\gamma$ lines). Abstract: The Einstein spontaneous rates (A-coefficients) of Fe+ lines have been computed by several authors with results that differ from each other by up to 40%. Consequently, models for line emissivities suffer from uncertainties that in turn affect the determination of the physical conditions at the base of line excitation. We provide an empirical determination of the A-coefficient ratios of bright [Fe II] lines that would represent both a valid benchmark for theoretical computations and a reference for the physical interpretation of the observed lines. With the ESO-Very Large Telescope X-shooter instrument between 3000 Å and 24700 Å, we obtained a spectrum of the bright Herbig-Haro object HH 1. We detect around 100 [Fe II] lines, some of which with a signal-to-noise ratios >=100. Among these latter lines, we selected those emitted by the same level, whose dereddened intensity ratios are direct functions of the Einstein A-coefficient ratios. From the same X-shooter spectrum, we got an accurate estimate of the extinction toward HH 1 through intensity ratios of atomic species, H I recombination lines and H2 ro-vibrational transitions. We provide seven reliable A-coefficient ratios between bright [Fe II] lines, which are compared with the literature determinations. In particular, the A-coefficient ratios involving the brightest near-infrared lines (λ12570/λ16440 and λ13209/λ16440) are in better agreement with the predictions by the Quinet et al. relativistic Hartree-Fock model. However, none of the theoretical models predict A-coefficient ratios in agreement with all of our determinations. We also show that literature data of near-infrared intensity ratios better agree with our determinations than with theoretical expectations. Abstract: As part of the POISSON project (Protostellar Optical-Infrared Spectral Survey on NTT), we present the results of the analysis of low-resolution NIR spectra 0.9-2.4 um) of two samples of YSOs in Lupus and Serpens (52 and 17 objects), with masses 0.1-2.0 Msun and ages from 10^5 to a few 10^7 yr. After determining the accretion parameters of the Lup and Ser targets by analysing their HI near-IR emission features, we added the results to those from previous regions (investigated in POISSON with the same methodology). We obtained a final catalogue (143 objects) of mass accretion rates (Macc) derived in a homogeneous fashion and analysed how Macc correlates with M* and how it evolves in time. We derived the accretion luminosity (Lacc) and Macc for Lup and Ser objects from the Br_gamma line by using relevant empirical relationships from the literature that connect HI line luminosity and Lacc. To minimise the biases and also for self-consistency, we re-derived mass and age for each source using the same set of evolutionary tracks. We observe a correlation MaccM*^2.2, similarly to what has previously been observed in several star-forming clouds. The time variation of Macc is roughly consistent with the expected evolution in viscous disks, with an asymptotic decay that behaves as t^-1.6. However, Macc values are characterised by a large scatter at similar ages and are on average higher than the predictions of viscous models. Although part of the scattering may be related to the employed empirical relationship and to uncertainties on the single measurements, the general distribution and decay trend of the Macc points are real. These findings might be indicative of a large variation in the initial mass of the disks, of fairly different viscous laws among disks, of varying accretion regimes, and of other mechanisms that add to the dissipation of the disks, such as photo-evaporation., Comment: 18 pages, 10 figures, accepted by A&A

Abstract: MOONS is a new Multi-Object Optical and Near-infrared Spectrograph selected by ESO as a third generation instrument for the Very Large Telescope (VLT). The grasp of the large collecting area offered by the VLT (8.2m diameter), combined with the large multiplex and wavelength coverage (optical to near-IR: 0.8μm - 1.8μm) of MOONS will provide the European astronomical community with a powerful, unique instrument able to pioneer a wide range of Galactic, Extragalactic and Cosmological studies and provide crucial follow-up for major facilities such as Gaia, VISTA, Euclid and LSST. MOONS has the observational power needed to unveil galaxy formation and evolution over the entire history of the Universe, from stars in our Milky Way, through the redshift desert, and up to the epoch of very first galaxies and re-ionization of the Universe at redshift z>8-9, just few million years after the Big Bang. On a timescale of 5 years of observations, MOONS will provide high quality spectra for >3M stars in our Galaxy and the local group, and for 1-2M galaxies at z>1 (SDSS-like survey), promising to revolutionise our understanding of the Universe. The baseline design consists of ~1000 fibers deployable over a field of view of ~500 square arcmin, the largest patrol field offered by the Nasmyth focus at the VLT. The total wavelength coverage is 0.8μm-1.8μm and two resolution modes: medium resolution and high resolution. In the medium resolution mode (R~4,000-6,000) the entire wavelength range 0.8μm-1.8μm is observed simultaneously, while the high resolution mode covers simultaneously three selected spectral regions: one around the CaII triplet (at R~8,000) to measure radial velocities, and two regions at R~20,000 one in the J-band and one in the H-band, for detailed measurements of chemical abundances. © (2014) COPYRIGHT Society of Photo-Optical Instrumentation Engineers (SPIE). 21. Far-IR Spectrophotometry of HH Flows with the ISO Long-Wavelength Spectrometer

Abstract: Full Iso-Lws spectral scans between about 45 to 190 μm of 17 individual HH objects in 7 star forming regions have revealed essentially only [O I] 63 μm line emission, implying that the Fircooling of these objects is totally dominated by this line alone. In this case, J-shock models can be used to determine the mass loss rates of the HH exciting sources. These mass loss rates are in reasonably good agreement with those estimated for the accompanying CO flows, providing first observational evidence that HH and molecular flows are driven by the same agent. The Lmech – Lbol relation, based on our results with the Lws, implies that young stellar objects of lower mass are loosing mass at relatively higher rates than their more massive counterparts.

Abstract: Fischer-Tropsch synthesis (FTS) has been studied at 5 · 103 Pa, at 508 K, an H2/CO=2/1 molar ratio on Ru/γ-Al2O3 catalysts (1% Ru wt./wt.), using Ru(AcAc)3 (AcAc standing for acetylacetonate), RuCl3 and K(AcAc) as precursors; K/Ru and Cl/Ru molar ratios ranged from 0.14 to 2.18 and from 1.31 to 3.71, respectively. As Ru catalysts for FTS undergo easy alteration resulting from coke deposition, the results from kinetic runs were collected using both fresh catalysts (at 5–10 min time-on-stream) and also when steady state conditions were reached (about 150 min time-on-stream). The influence of K/Ru and Cl/Ru ratios on CO conversion and on the selectivity of the formation of different products in the reaction has been examined. Changes in activity, during the regeneration procedures of the catalysts, differ significantly when the precursor is Ru(AcAc)3 (increase in activity after regeneration cycle) or RuCl3 (significant deactivation with a decrease in Ru surface concentration, as determined by XPS).

Abstract: A Ru/γ-Al 2 O 3 (0.87% wt./wt.) catalyst has been studied for the Fischer-Tropsch synthesis (FTS) at 5 · 10 3 Pa, 508 K with a H 2 /CO ratio of 2/1. The same catalyst was also sintered at 973 K for different times in order to change the metal dispersion D M of Ru ( D M from 18 to 93%). The aim of the present work is to verify which atoms of Ru crystallites are most active in FTS. For this study a ‘model’ of truncated hexagonal bipyramid for the crystalline agglomerates has been assumed. It was verified that atoms with higher nearest neighbors i ( i ,8 and 9) on flat faces and edges are more active than those on vertexes and edges at lower i values ( i ,5 to 7). We found that larger crystallites decompose CO more easily than smaller ones, and that the turnover number for CO disappearance and for CH 4 , C2, C3, C5–C15 formation does not depend strongly on metal dispersion. However, on the contrary, we found that light olefins (C2 and C3) are easily formed on large crystallites. The above findings reveal that FTS could be considered as a structure sensitive reaction for the initial step, but are structure insensitive for the overall reaction.

Abstract: Hereditary neuropathy with liability to pressure palsies (HNPP) is commonly associated with a 1.5-megabase deletion on chromosome 17p11.2-12. We analyzed the phenotypic expression of the deletion in 39 HNPP patients from 16 families carrying the deletion. Two-thirds of the individuals had episodes of acute mononeuropathy, often involving nerve territories of the upper limbs or brachial plexus; however, 41% of affected subjects were unaware of their disease, and 25% were almost or totally free of symptoms; one-third complained of chronic symptoms and four older patients had a picture of polyneuropathy. Electrophysiologic abnormalities differed among affected subjects, ranging from conduction abnormalities localized at common entrapment sites to diffuse conduction slowing, usually more evident at entrapment sites; patients from one family had preeminent proximal involvement. The spectrum of phenotypic expression of deletion-associated HNPP appears to be broader than previously thought. The prevalence of the disease is probably underestimated, and the availability of molecular diagnosis should increase disease detection.

25. Nanotechnology applied to silicon grism, for high-resolution infrared spectroscopy in astronomy

Abstract: We present the results of our project for the design, construction and on-sky tests of silicon grisms. Silicon grisms are optical devices, that provide a medium-high resolution spectroscopic mode in imaging instruments, without introducing major changes in instrument configuration. After some critical phases during the experimentation, a silicon grism has been eventually fabricated and inserted in the filter wheel of the Near Infrared camera NICS, at the focal plane of the National Galileo Telescope (TNG), the 3.5 m Italian facility at the Canary Islands (E). The results of the main phases of the grism fabrication and the results of the on-sky tests are detailed reported.

Abstract: As part of the Herschel guaranteed time key program 'HOBYS', we present the photometric survey of the star forming region Vela-C, one of the nearest sites of low-to-high-mass star formation in the Galactic plane. Vela-C has been observed with PACS and SPIRE in parallel mode between 70 um and 500 um over an area of about 3 square degrees. A photometric catalogue has been extracted from the detections in each band, using a threshold of 5 sigma over the local background. Out of this catalogue we have selected a robust sub-sample of 268 sources, of which 75% are cloud clumps and 25% are cores. Their Spectral Energy Distributions (SEDs) have been fitted with a modified black body function. We classify 48 sources as protostellar and 218 as starless. For two further sources, we do not provide a secure classification, but suggest they are Class 0 protostars. From SED fitting we have derived key physical parameters. Protostellar sources are in general warmer and more compact than starless sources. Both these evidences can be ascribed to the presence of an internal source(s) of moderate heating, which also causes a temperature gradient and hence a more peaked intensity distribution. Moreover, the reduced dimensions of protostellar sources may indicate that they will not fragment further. A virial analysis of the starless sources gives an upper limit of 90% for the sources gravitationally bound and therefore prestellar. We fit a power law N(logM) prop M^-1.1 to the linear portion of the mass distribution of prestellar sources. This is in between that typical of CO clumps and those of cores in nearby star-forming regions. Abstract: On a solid surface the effects of the implosion of the bubbles, created during a sonication run, are commonly used to clean or to erode the surface itself. In the present work, ultrasound was used in order to obtain a ruthenium on alumina catalyst distinguished by a very high value of the dispersion of the metal. The samples were characterized with different techniques in order to study the influence of the sonication on both the metal crystallites and the support.

This observational, nonrandomized, multicenter study evaluated EBT as a post-surgical adjuvant radiation therapy for early stage breast cancer.Methods: This study included women aged 50 years or more with invasive carcinoma or ductal carcinoma in situ, tumor size ≤3 cm, negative lymph node status, and negative surgical margins. The endpoints were skin and subcutaneous toxicities, efficacy outcomes, cosmetic outcomes, and device performance. In this interim report, 1-month, 6-month, and 1-year follow-up data are available on 68, 59, and 37 patients, respectively.Results: The EBT device performed consistently, delivering the prescribed 34 Gy to all 69 patients (10 fractions/patient). Most adverse events were Grade 1 and included firmness, erythema, breast tenderness, hyperpigmentation, pruritis, field contracture, seroma, rash/desquamation, palpable mass, breast edema, hypopigmentation, telangiectasia, and blistering, which were anticipated. Breast infection occurred in 2 (2.9%) patients. No tumor recurrences were reported. Cosmetic outcomes were excellent or good in 83.9%–100% of evaluable patients at 1 month, 6 months, and 1 year.Conclusion: This observational, nonrandomized, multicenter study demonstrates that this EBT device was reliable and well tolerated as an adjuvant radiation therapy for early stage breast cancer.Keywords: radiation therapy, electronic brachytherapy, breast cancer

Abstract: We present a study of the infrared variability of young stellar objects by means of two Spitzer-IRAC images of the Vela Molecular Cloud D (VMR-D) obtained in observations separated in time by about six months. By using the same space-born IR instrumentation, this study eliminates all the unwanted effects usually unavoidable when comparing catalogs obtained from different instruments. The VMR-D map covers about 1.5 square deg. of a site where star formation is actively ongoing. We are interested in accreting pre-main sequence variables whose luminosity variations are due to intermittent events of disk accretion (i.e. active T Tauri stars and EXor type objects). The variable objects have been selected from a catalog of more than 170,000 sources detected at a S/N ratio > 5. We searched the sample of variables for ones whose photometric properties are close to those of known EXor's. These latter are monitored in a more systematic way than T Tauri stars and the mechanisms that regulate the observed phenomenology are exactly the same. Hence the modalities of the EXor behavior is adopted as driving criterium for selecting variables in general. We selected 19 bona fide candidates that constitute a well-defined sample of new variable targets for further investigation. Out of these, 10 sources present a Spitzer MIPS 24 micron counterpart, and have been classified as 3 Class I, 5 flat spectrum and 2 Class II objects, while the other 9 sources have spectral energy distribution compatible with phases older than Class I. This is consistent with what is known about the small sample of known EXor's, and suggests that the accretion flaring or EXor stage might come as a Class I/II transition. Abstract: We present low resolution (R approximately 250) spectroscopy in the near-IR (0.8 to 2.5um) of the EXor variables. These are the initial results (obtained during the period 2007-2008) from a long term photometric and spectroscopic program aimed to study the variability in the accretion processes of pre-Main Sequence (PMS) stars, by correlating the continuum fluctuations with the spectroscopical properties. Eight sources have been observed in different epochs, for a total of 25 acquired spectra. EXor spectra show a wide variety of emission features dominated by HI recombination (Paschen and Brackett series). We have investigated whether line and continuum variability could be due to a variable extinction, but such hypothesis is applicable only to the peculiar source PV Cep. By comparing the observed spectra with a wind model, mass loss rates in the range (2-10)x10^(-8) M_sun} yr^(-1) are derived, along with other wind parameters. Consistent results are also obtained by assuming that HI lines are due to accretion. CO overtone is also detected in the majority of the sources both in absorption and in emission. It appears to come from regions more compact than winds, likely the stellar photosphere (when in absorption) and the circumstellar disk (when in emission). NaI and CaI IR lines behave as the CO does, thus they are thought to arise in the same locations. For some targets multiple spectra correspond to different activity stages of the source. Those exhibiting the largest continuum variation at 2um (DeltaK > 1 mag) present a significant line flux fading during the continuum declining phases. Abstract: Learning can lead to changes in the intrinsic excitability of neurons. However, it is unclear to what extent these changes persist and what role they play in the expression of memory. Here, we report that in vitro analogues of operant conditioning produce a long-term (24 h) increase in the excitability of an identified neuron (B51) critical for the expression of feeding in Aplysia. This increase in excitability, which is cAMP dependent, contributes to the associative modification of the feeding circuitry, providing a mechanism for long-term memory storage.

Abstract: SummaryOperant conditioning is a ubiquitous but mechanistically poorly understood form of associative learning in which an animal learns the consequences of its behavior. Using a single-cell analog of operant conditioning in neuron B51 of Aplysia, we examined second-messenger pathways engaged by activity and reward and how they may provide a biochemical association underlying operant learning. Conditioning was blocked by Rp-cAMP, a peptide inhibitor of PKA, a PKC inhibitor, and by expressing a dominant-negative isoform of Ca2+-dependent PKC (apl-I). Thus, both PKA and PKC were necessary for operant conditioning. Injection of cAMP into B51 mimicked the effects of operant conditioning. Activation of PKC also mimicked conditioning but was dependent on both cAMP and PKA, suggesting that PKC acted at some point upstream of PKA activation. Our results demonstrate how these molecules can interact to mediate operant conditioning in an individual neuron important for the expression of the conditioned behavior.

Abstract: We present the 12CO(1-0) and 13CO(2-1) line maps obtained observing with the SEST a $\sim 1\degr \times 1\degr$ region of the Vela Molecular Ridge, Cloud D. This cloud is part of an intermediate-mass star forming region that is relatively close to the Sun. Our observations reveal, over a wide range of spatial scales (from $\sim 0.1$ to a few parsecs), a variety of dense structures such as arcs, filaments and clumps, that are in many cases associated with far-IR point-like sources, recognized as young stellar objects and embedded star clusters. The velocity field analysis highlights the presence of possible expanding shells, extending over several parsecs, probably related to the star forming activity of the cloud. Furthermore, the analysis of the line shapes in the vicinity of the far-IR sources allowed the detection of 13 molecular outflows. Considering a hierarchical scenario for the gas structure, a cloud decomposition was obtained for both spectral lines by means of the CLUMPFIND algorithm. The CLUMPFIND output has been discussed critically and a method is proposed to reasonably correct the list of the identified clumps. We find that the corresponding mass spectrum shows a spectral index $\alpha\sim1.3 \div 2.0$ and the derived clump masses are below the corresponding virial masses. Abstract: Objective: To design a Health-related Quality of Life (HRQoL) instrument for HIV-infected people in the era of highly active antiretroviral therapy (HAART). Methods: The self-administered questionnaire was developed by an Italian network including researchers, physicians, people living with HIV, national institutions and community-based organizations (CBO) through several steps: (1) review of existing HRQoL literature and questionnaires for HIV-infected people; (2) selection of relevant domains measuring HRQoL in HIV-infected people, and identification of new domains related to new aspects of HRQoL concerning HAART-treated individuals; (3) conduction of two pre-test analyses in independent groups of Italian HIV-positive people (n≌100) distributed throughout the country. The objectives of the first pre-test were to verify the usefulness of the questionnaire, to construct a form easily understandable by everyone, to define the domains and their significance; the second pre-test aimed at evaluating and reshaping the questionnaire based on a statistical analysis of the outcomes of first pre-test; (4) validation analysis. A large cohort of people with HIV infection was recruited for the last step.Results: The internal consistence reliability (Cronbach’s α) was ≥0.70 for all domains. Most domains had Cronbach’s coefficient >0.80. All domains demonstrated convergent and discriminant validity. The final version of ISSQoL includes two sections: HRQoL Core Evaluation Form (9 domains) and Additional Important Areas for HRQoL (6 domains). The ISSQoL was administered together with two additional forms: a Daily Impact of Symptoms Form and a Demographic Information Form. The Additional Important Areas for HRQoL include social support, interaction with medical staff, treatment impact, body changes, life planning, and motherhood/fatherhood.Conclusion: The data reported in the present paper provide preliminary evidence of the reliability and validity of the ISSQoL questionnaire for the measurement of HRQoL in HIV-infected people. The direct involvement of HIV-positive people in all the phases of the project was a key aspect of our work.

Abstract: A long-standing debate in neuroscience is whether classical and operant conditioning are mechanistically similar or distinct. The feeding behavior of Aplysia provides a model system suitable for addressing this question. Here we report that classical and operant conditioning of feeding behavior differentially modify the intrinsic excitability of neuron B51, a critical element for the expression of the feeding response, thus revealing that these two forms of associative learning differ at the cellular level.

Abstract: The complete near infrared (0.9-2.5 micron) spectra in three different star forming regions (HH24-26, HH72 and BHR71) are presented and analyzed in the framework of shock excitation models. The spectra are dominated by H_2 rovibrational emission (vibrational state nu up to 5, excitation energy T_ex up to 35000 K), while emission from ionized material, recognizable from [Fe II] and [S II] lines, is significantly fainter. The analysis of the H_2 excitation diagrams points to the existence of two different excitation regimes: whilst condensations observed only in the infrared appear to have temperatures rarely exceeding 3000 K and can be modelled in the framework of steady-state C-shock models, the infrared counterparts of Herbig Haro (HH) objects exhibit a temperature stratification with components up to more than 5000 K. The H_2 emission from representative HH objects (HH26A, HH72A and HH320A) has been successfully modelled by planar J-shocks with magnetic precursors, for which the main parameters (pre-shock density, speed) are derived. However, these same models are unable to reproduce the observed atomic and ionic emission, which probably arises from a distinct and perhaps more embedded region with respect to that traced by the H_2. Abstract: Operant conditioning is a form of associative learning through which an animal learns about the consequences of its behavior. Here, we report an appetitive operant conditioning procedure in Aplysia that induces long-term memory. Biophysical changes that accompanied the memory were found in an identified neuron (cell B51) that is considered critical for the expression of behavior that was rewarded. Similar cellular changes in B51 were produced by contingent reinforcement of B51 with dopamine in a single-cell analog of the operant procedure. These findings allow for the detailed analysis of the cellular and molecular processes underlying operant conditioning.

Abstract: We present a [60--100] vs. [100--170] $��$m two colour diagram for a sample of 61 young stellar objects (YSOs) observed with the Long Wavelength Spectrometer (LWS) on-board the Infrared Space Observatory (ISO). The sample consists of 17 Class 0 sources, 15 Class I, 9 Bright Class I ($L_{\rm bol}>10^4 L_\odot$), 20 Class II (14 Herbig Ae/Be stars and 6 T Tauri stars). We find that each class occupies a well defined region in our diagram with colour temperatures increasing from Class 0 to Class II. Therefore the [60--100] vs. [100--170] two colours diagram is a powerful and simple tool to derive from future (e.g. with the Herschel Space Observatory) photometric surveys the evolutionary status of YSOs. The advantage over other tools already developed is that photometry at other wavelengths is not required: three flux measurements are enough to derive the evolutionary status of a source. As an example we use the colours of the YSO IRAS 18148$-$0440 to classify it as Class I. The main limitation of this work is the low spatial resolution of the LWS which, for some objects, causes a high uncertainty in the measured fluxes due to the background emission or to the source confusion inside the LWS beam., 7 pages, 2 figures, requires mn2e document class. Abstract: Understanding how an animal learns to make associations is an important goal of the behavioral sciences. Many advances have been made in understanding the cellular mechanisms underlying the two major forms of associative learning: operant conditioning and classical conditioning. Operant conditioning is a process by which an organism learns to associate consequences to its own behavior. The operant conditioning of Aplysia feeding behavior shows this association through changes in the membrane properties of a cell responsible for mediation of the reinforced behavior. These changes in the membrane properties alter the likelihood that the cell will be electrically active and hence alter the expression of the behavior, accordingly. Classical conditioning occurs when an organism learns to associate a typically neutral stimulus with a later salient event. The classical conditioning of an Aplysia withdrawal reflex is expressed as an increase in the efficacy of a specific sensorimotor synapse. The stimuli form an association by converging on a second messenger cascade in the sensory neuron. Both presynaptic and postsynaptic processes contribute to this association. Associative modifications in other species are also considered.

Abstract: This research was undertaken to inform future telehealth policy directions regarding the socioeconomic impact of telehealth. Fifty-seven sources were identified and analyzed through a comprehensive literature search of electronic databases, the Internet, journals, conference proceedings, as well as personal communication with consultants in the field. The review revealed a focus on certain socioeconomic indicators such as cost, access, and satisfaction. It also identified areas of opportunity for further research and policy analysis and development (e.g., social isolation, life stress, poverty), along with various barriers and challenges to the advancement of telehealth. These included confidentiality, reimbursement, and legal and ethical considerations. To become fully integrated into the health care system, telehealth must be viewed as more than an add-on service.
