Abstract

Differences in mortality between critically ill patients with severe alcohol‐associated hepatitis (sAH) and acute‐on‐chronic liver failure (ACLF) and non‐sAH ACLF (i.e., ACLF not precipitated by sAH) are unknown. Such differences are important, as they may inform on prognosis and optimal timing of liver transplantation (LT). Thus, we aimed to compare short‐term and longer‐term mortality between patients with sAH ACLF and patients with non‐sAH ACLF who were admitted to the intensive care unit. Patients with ACLF admitted from 2016‐2018 at two tertiary care intensive care units were analyzed. SAH was defined by the National Institute on Alcohol Abuse and Alcoholism’s Alcoholic Hepatitis Consortium and Model for End‐Stage Liver Disease score >20. Mortality without LT was compared between sAH ACLF and non‐sAH ACLF using Fine and Gray’s competing‐risks regression. A total of 463 patients with ACLF (18% sAH and 82% non‐sAH) were included. 5. The Effect of Two Interventions to Increase Breast Cancer Screening in Rural Women

Abstract

Guideline-based mammography screening is essential to lowering breast cancer mortality, yet women residing in rural areas have lower rates of up to date (UTD) breast cancer screening compared to women in urban areas. We tested the comparative effectiveness of a tailored DVD, and the DVD plus patient navigation (PN) intervention vs. Usual Care (UC) for increasing the percentage of rural women (aged 50 to 74) UTD for breast cancer screening, as part of a larger study. Four hundred and two women who were not UTD for breast cancer screening, eligible, and between the ages of 50 to 74 were recruited from rural counties in Indiana and Ohio. Consented women were randomly assigned to one of three groups after baseline assessment of sociodemographic variables, health status, beliefs related to cancer screening tests, and history of receipt of guideline-based screening. The mean age of participants was 58.2 years with 97% reporting White race. After adjusting for covariates, 54% of women in the combined intervention (DVD + PN) had a mammogram within the 12-month window, over 5 times the rate of becoming UTD compared to UC (OR = 5.11; 95% CI = 2.57, 10.860; p < 0.001). Interactions of the intervention with other variables were not significant. Significant predictors of being UTD included: being in contemplation stage (intending to have a mammogram in the next 6 months), being UTD with other cancer screenings, having more disposable income and receiving a reminder for breast screening. Women who lived in areas with greater Area Deprivation Index scores (a measure of poverty) were less likely to become UTD with breast cancer screening. For rural women who were not UTD with mammography screening, the addition of PN to a tailored DVD significantly improved the uptake of mammography. Attention should be paid to certain groups of women most at risk for not receiving UTD breast screening to improve breast cancer outcomes in rural women.

Abstract

ImportanceWomen living in rural areas have lower rates of breast, cervical, and colorectal cancer screening compared with women living in urban settings.ObjectiveTo assess the comparative effectiveness of (1) a mailed, tailored digital video disc (DVD) intervention; (2) a DVD intervention plus telephonic patient navigation (DVD/PN); and (3) usual care with simultaneously increased adherence to any breast, cervical, and colorectal cancer screening that was not up to date at baseline and to assess cost-effectiveness.Design, Setting, and ParticipantsThis randomized clinical trial recruited and followed up women from rural Indiana and Ohio (community based) who were not up to date on any or all recommended cancer screenings. Participants were randomly assigned between November 28, 2016, and July 1, 2019, to 1 of 3 study groups (DVD, DVD/PN, or usual care). Statistical analyses were completed between August and December 2021 and between March and September 2022.InterventionThe DVD interactively assessed and provided messages for health beliefs, including risk of developing the targeted cancers and barriers, benefits, and self-efficacy for obtaining the needed screenings. Patient navigators counseled women on barriers to obtaining screenings. The intervention simultaneously supported obtaining screening for all or any tests outside of guidelines at baseline.Main Outcomes and MeasuresReceipt of any or all needed cancer screenings from baseline through 12 months, including breast, cervical, and colorectal cancer, and cost-effectiveness of the intervention. Binary logistic regression was used to compare the randomized groups on being up to date for all and any screenings at 12 months.ResultsThe sample included 963 women aged 50 to 74 years (mean [SD] age, 58.6 [6.3] years). The DVD group had nearly twice the odds of those in the usual care group of obtaining all needed screenings (odds ratio [OR], 1.84; 95% CI, 1.02-3.43; P = .048), and the odds were nearly 6 times greater for DVD/PN vs usual care (OR, 5.69; 95% CI, 3.24-10.5; P P Conclusions and RelevanceIn this randomized clinical trial of rural women who were not up to date with at least 1 of the recommended cancer screenings (breast, cervical, or colorectal), an intervention designed to simultaneously increase adherence to any or all of the 3 cancer screening tests was more effective than usual care, available at relatively modest costs, and able to be remotely delivered, demonstrating great potential for implementing an evidence-based intervention in remote areas of the midwestern US.Trial RegistrationClinicalTrials.gov Identifier: NCT02795104

Abstract

Dihadron azimuthal correlations containing a high transverse momentum ( ) trigger particle are sensitive to the properties of the nuclear medium created at RHIC through the strong interactions occurring between the traversing parton and the medium, i.e. jet-quenching. Previous measurements revealed a strong modification to dihadron azimuthal correlations in Au+Au collisions with respect to p+p and d+Au collisions. The modification increases with the collision centrality, suggesting a path-length or energy density dependence to the jet-quenching effect. This paper reports STAR measurements of dihadron azimuthal correlations in mid-central (20%-60%) Au+Au collisions at GeV as a function of the trigger particle's azimuthal angle relative to the event plane, . The azimuthal correlation is studied as a function of both the trigger and associated particle . The subtractions of the combinatorial background and anisotropic flow, assuming Zero Yield At Minimum (ZYAM), are described. The correlation results are first discussed with subtraction of the even harmonic (elliptic and quadrangular) flow backgrounds. The away-side correlation is strongly modified, and the modification varies with , with a double-peak structure for out-of-plane trigger particles. The near-side ridge (long range pseudo-rapidity correlation) appears to drop with increasing while the jet-like component remains approximately constant. The correlation functions are further studied with the subtraction of odd harmonic triangular flow background arising from fluctuations. It is found that the triangular flow, while responsible for the majority of the amplitudes, is not sufficient to explain the -dependence of the ridge or the away-side double-peak structure. The dropping ridge with could be attributed to a -dependent elliptic anisotropy; however, the physics mechanism of the ridge remains an open question. Even with a -dependent elliptic flow, the away-side correlation structure is robust. These results, with extensive systematic studies of the dihadron correlations as a function of , trigger and associated particle , and the pseudo-rapidity range , should provide stringent inputs to help understand the underlying physics mechanisms of jet-medium interactions in high energy nuclear collisions.

11. Circumscribed Mass: Invasive Cancer

Abstract

Most invasive breast cancers present as focal asymmetries, areas of architectural distortion, or irregular masses with indistinct or spiculated margins. Some present as round or oval masses with circumscribed margins; however, most round or oval masses have microlobulated, indistinct, or spiculated margins and may be associated with characteristic malignant calcifications. This chapter, appearing in the section on circumscribed mass, reviews the key clinical and imaging features, differential diagnosis, and management recommendations for invasive breast cancers, which can present as circumscribed masses on mammography and sonography. Examples include medullary carcinoma, mucinous carcinoma, invasive papillary carcinoma, and high-grade invasive ductal carcinoma. These lesions may present as solid or complex masses with circumscribed margins and may be confused with several benign breast conditions.

Abstract

Fibroepithelial lesions account for the overwhelming majority of solid breast masses affecting women younger than age 20. Nearly all present as self-detected or provider-detected palpable masses. All fibroepithelial lesions are composed of stromal (fibrous) and glandular (epithelial) elements and variable histology. Rapidly growing mobile breast masses in girls or female adolescents may represent juvenile fibroadenomas, which have different but benign histological features when compared to typical fibroadenomas. Benign phyllodes tumors closely resemble usual fibroadenomas and juvenile fibroadenomas on imaging. Decisions whether to biopsy these tumors are made clinically; the diagnosis of phyllodes tumor depends on histological assessment.This chapter, appearing in the section on circumscribed mass, reviews the key clinical and imaging features, differential diagnosis, and management recommendations of large solid breast masses affecting young women, including typical fibroadenomas, giant fibroadenomas, juvenile fibroadenomas, and phyllodes tumors.

13. Circumscribed Mass: Cysts, Complicated Cysts, Clustered Microcysts

Abstract

Benign cysts can occur in women of all ages, but they have a predilection for women in their 40s and 50s, and are far less common in younger and older women. Most cysts arise at the level of the terminal duct-lobular unit (TDLU); less commonly, cysts are caused by ectasia of central ducts. This chapter, appearing in the section on circumscribed mass, reviews the key clinical and imaging features, differential diagnosis, and management recommendations for primary cystic masses, including simple cysts, complicated cysts, and clustered microcysts. Careful assessment of sonographic features usually allows a definitive diagnosis of these benign entities that do not typically require tissue sampling. Some complex masses containing fluid and/or cystic-appearing components may require tissue sampling for diagnosis.

Abstract

A fibroadenoma is a benign fibroepithelial breast tumor arising from the terminal duct-lobular unit (TDLU), composed of epithelial and stromal elements. The overwhelming majority of fibroadenomas present as palpable or imaging-detected circumscribed masses showing sharp demarcation between the lesion and the adjacent breast tissue. Fibroadenomas are the most common benign breast tumor occurring in women, with a peak incidence in the third and fourth decades, but they can occur from childhood through the eight decade of life. This chapter, appearing in the section on asymmetry, mass, and distortion, reviews the key clinical and imaging features, differential diagnosis, and management recommendations for fibroadenomas.

Atonement. For some this word is the heart of the Christian faith. For others, it is irrelevant for Christianity and how they live their Christian lives. Often we do not see'the breadth and depth'of the atonement in the gospel. Christian D. Kettler, in his fourth book on'the vicarious humanity of Christ,'suggests that we consider that the atonement is not only a vicarious death in our place and on our behalf--whether in the form of a'penal substitute'or a'moral example'--but also his vicarious humanity, having atoning significance for all of our lives, indeed, all of reality: the church, the world, and the self. These essays are collected from several years of Kettler's thought on the doctrine of the vicarious humanity of Christ and its implications for all of life and reality, seeking to demonstrate that the doctrine of the atonement is not to be restricted to one small part of the doctrinal agenda, but has profound implications that theology sometimes does not explore. These issues include the nature of culture, aesthetics, creation out of nothing, spiritual formation, emotional weakness, the priesthood of Christ and creation, genetic engineering, and trusting in the'ideal'self.

Abstract

The male breast is susceptible to many of the same pathologic processes as the female breast. Many of these conditions have mammographic, ultrasonographic (US), and magnetic resonance imaging findings that allow differentiation between clearly benign conditions and those that require biopsy. Gynecomastia is the most common abnormality of the male breast and has characteristic imaging features that usually allow differentiation from malignancy. Mammography is the initial imaging modality for a clinically suspicious mass. A palpable mass that is occult or incompletely imaged at mammography mandates targeted US. Suspicious or indeterminate masses require biopsy, which can usually be performed with US guidance. Approximately 0.7% of breast cancers occur in men. Men with breast cancer often present at a more advanced stage than do women owing to a delay in diagnosis. Benign breast neoplasms that may occur in men include angiolipoma, schwannoma, intraductal papilloma, and lipoma. Benign nonneoplastic entities that may occur in the male breast include intramammary lymph node, sebaceous cyst, diabetic mastopathy, hematoma, fat necrosis, subareolar abscess, breast augmentation, venous malformation, secondary syphilis, and nodular fasciitis. Familiarity with the salient features of the classic benign male breast conditions will allow accurate imaging interpretation and avoid unnecessary and often invasive treatment. © RSNA, 2013.

Abstract

Introduction Breast ductography is ordinarily considered a safe diagnostic procedure, and to date, no serious complications after the procedure have been reported. In keeping with a rising incidence of methicillin-resistant Staphylococcus aureus (MRSA) infections in both the hospital and outpatient settings, this case report describes a serious MRSA complication after ductography. Case presentation A 45-year-old Caucasian woman presented to the interdisciplinary breast disease clinic for secondary evaluation of a spontaneous unilateral nipple discharge, which was evaluated with ultrasound and contrast ductography. About 2 weeks after the procedure, the patient developed an MRSA breast abscess. Conclusion As exemplified by this unusual complication from a minimally invasive procedure, heightened awareness of the potential for MRSA infections by clinical providers is prudent as colonization of patients and providers and the risks of clinically significant infections are increased.

Abstract

We present first measurements of the evolution of the differential transverse momentum correlation function, C, with collision centrality in Au + Au interactions at root s(NN) = 200 GeV. This observable exhibits a strong dependence on collision centrality that is qualitatively similar to that of number correlations previously reported. We use the observed longitudinal broadening of the near-side peak of C with increasing centrality to estimate the ratio of the shear viscosity to entropy density, eta/s, of the matter formed in central Au + Au interactions. We obtain an upper limit estimate of eta/s that suggests that the produced medium has a small viscosity per unit entropy. (C) 2011 Elsevier B.V. All rights reserved.

Abstract

Trotz aller Fortschritte der modernen Medizin zahlen die Erkrankungen des Herz-Kreislauf-Systems und der plotzliche Herztod nach wie vor weltweit zu den haufigsten Todesursachen. Dabei liegt einer Vielzahl der plotzlichen Herztodfalle Kammerflimmern zugrunde, was bei Fortbestehen die Uberlebenswahrscheinlichkeit der Notfallopfer dramatisch reduziert und einzig durch eine umgehende Defibrillation terminiert werden kann. Durch die Einbeziehung von Laienhelfern bei der auserklinischen kardiopulmonalen Reanimation und der zusatzlichen Anwendung von automatisierten externen Defibrillatoren (AEDs) vor Eintreffen des professionellen Rettungsdienstes im Rahmen von sog. Public Access Defibrillation (PAD) -Programmen konnte schon vor Jahren ein Meilenstein im Kampf gegen den plotzlichen Herztod gesetzt werden, was seither in den neuesten internationalen Leitlinien des European Resuscitation Council (ERC) und der American Heart Association (AHA) besondere Berucksichtigung findet. Dennoch bleibt es trotz dieser Empfehlungen zumeist unklar, an welchen Orten AEDs vorgehalten werden sollten. Somit setzt die Implementierung solcher Programme zunachst eine lokale Bedarfsbewertung voraus, anhand derer mogliche Hoch-Risiko-Lokalitaten fur eine AED-Vorhaltung identifiziert werden konnen. Mit der Schaffung des Gottinger AED-Modells konnte nun erstmals eine solche Bewertungsgrundlage vorgelegt werden. Nachfolgende Studien sollten folgen, um die Validitat dieses Modells zu prufen.

Abstract

We study the beam-energy and system-size dependence of phi meson production (using the hadronic decay mode phi -> K(+) K(-)) by comparing the new results from Cu + Cu collisions and previously reported Au + Au collisions at root s(NN) = 62.4 and 200 GeV measured in the STAR experiment at RHIC. Data presented in this Letter are from mid-rapidity (vertical bar y vertical bar ) and net strangeness = 2 (Xi). The enhancement for phi mesons is observed to be higher at root s(NN) = 200 GeV compared to 62.4 GeV. These observations for the produced phi(s (s) over bar) mesons clearly suggest that, at these collision energies, the source of enhancement of strange hadrons is related to the formation of a dense partonic medium in high energy nucleus-nucleus collisions and cannot be alone due to canonical suppression of their production in smaller systems. Abstract

Lumbar isokinetic strength and the influence of age, bodyweight and testing velocity in patients with chronic low back pain in comparison with persons without pain. Lumbar isokinetic strength parameters of 80 patients with chronic low back pain and 70 persons without pain were compared and the influence of age, bodyweight and testing velocity was evaluated. The patients with chronic low back pain showed less strength than the persons without pain. All parameters of extension discriminated between the two groups whereas only some of the flexion parameters did. The isokinetic strength of the lumbal extension muscles was higher than the strength of the flexion muscles. In patients with chronic low back pain, isokinetic strength of lumbar extension muscles was more reduced than the strength of flexion muscles in comparison with persons without pain. At 90 degrees/sec in comparison to 60 degrees/sec, lower extension forces, higher flexion forces and changed ratios of flexion and extension muscles were measured. Age had an influence only on women. There were no changes in ratios of flexion and extension muscles with increasing age. Bodyweight showed weak correlations with isokinetic flexion forces. The influence of different factors on isokinetic force varies between patients with chronic low back patients and healthy subjects.

Abstract

We study the energy dependense of the transverse momentum (PT) spectra for charged pions, protons and anti-protons for An + An collisions at root(NN)-N-s = 62.4 and 200 GeV. Data are presented at mid-rapidity (broken vertical bar y broken vertical bar 7 GeV/c) the modification is similar for both energies. The p/pi(+) and (p) over bar/pi(-) ratios for central collisions at root(NN)-N-s = 62.4 GeV peak at PT similar or equal to 2 GeV/c. In the PT range where recombination is expected to dominate, the p/pi(+) ratios at 62.4 GeV are larger than at 200 GeV, while the (p) over bar/pi(-) ratios are smaller. For PT > 2 GeV/c, the (p) over bar/pi(-) ratios at the two beam energies are independent of P-T and centrality indicating that the dependence of the (p) over bar/pi(-) ratio on P-T does not change between 62.4 and 200 GeV. Abstract

A theology of the vicarious humanity, not just death, of Christ provides a fresh way to ground theological aesthetics in the reconciling life of Christ. Theological aesthetics can also bring a new perspective on the atonement as including the “Godward” movement of the Son to the Father, a movement of beauty that humanity has been invited to participate in, an act of both solidarity and substitution. This response reveals the uniqueness of Christ's beauty, a vicarious not analogous knowledge of beauty, thus creating a wholeness and openness to the beauty of the grace of God's creation.

Abstract

Previous measurements of a quadrupole component of azimuth correlations denoted by symbol ${v}_{2}$ have been interpreted to represent elliptic flow, a hydrodynamic phenomenon conjectured to play a major role in noncentral nucleus-nucleus collisions. ${v}_{2}$ measurements provide the main support for conclusions that a ``perfect liquid'' is formed in heavy-ion collisions at the Relativistic Heavy Ion Collider. However, conventional ${v}_{2}$ methods based on one-dimensional (1D) azimuth correlations give inconsistent results and may include a jet contribution. In some cases the data trends appear to be inconsistent with hydrodynamic interpretations. In this study we distinguish several components of 2D angular correlations and isolate a nonjet (NJ) azimuth quadrupole denoted by ${v}_{2}{\text{2D}}$. We establish systematic variations of the NJ quadrupole on ${y}_{t}$, centrality, and collision energy. We adopt transverse-rapidity ${y}_{t}$ as both a velocity measure and a logarithmic alternative to transverse momentum ${p}_{t}$. Based on NJ-quadrupole trends, we derive a completely factorized universal parametrization of quantity ${v}_{2}{\text{2D}}({y}_{t},b,\sqrt{{s}_{NN}})$ which describes the centrality, ${y}_{t}$, and energy dependence. From ${y}_{t}$-differential ${v}_{2}({y}_{t})$ data we isolate a quadrupole spectrum and infer a quadrupole source boost having unexpected properties. NJ quadrupole ${v}_{2}$ trends obtained with 2D model fits are remarkably simple. The centrality trend appears to be uncorrelated with a sharp transition in jet-related structure that may indicate rapid change of Au-Au medium properties. The lack of correspondence suggests that the NJ quadrupole may be insensitive to such a medium. Several quadrupole trends have interesting implications for hydro interpretations.

Abstract

OBJECTIVE The German physician based emergency medical system (EMS) might confront physicians with advance directives in the field. A multi-question survey was used to evaluate emergency physicians' experience with advance directives in the prehospital setting and to assess their attitudes towards forms and statements of advance directives. METHODS A questionnaire was mailed to the members of the Association of Emergency Physicians of Northern Germany ("AGNN"), an interest group of emergency physicians, in 2001. RESULTS 511 emergency physicians (50,4 % of the AGNN members) filled in the questionnaire completely and sent it back for evaluation. 75 % of the participants were working as emergency physicians at present, 72 % had emergency experiences of more than 5 years. One third had previously dealt with advance directives in the prehospital setting. 77 % of these physicians thought advance directives generally helpful. Nevertheless 88 % based their management on the context of the individual circumstances (e. g. emergency conditions, underlying diseases, expected prognosis), only 7 % said they would always exactly follow the statements of the directive. In the view of the emergency physicians the advance directive should contain information on cardiopulmonary resuscitation (CPR: 88 %), intensive care-treatment (75 %) and preclinical emergency treatment (55 %). Information on underlying diseases (87 %) and a legal substitute (84 %) should be contained as well. As formal requirements, 47 % of the physicians wanted the family doctor to be involved, 49 % desired a notary authenticity confirmation, additionally or solely. Pragmatically, the advance directive should be kept with the personal documents (84 %). A regular reconfirmation was deemed necessary (twice to once a year: 64 %). The current legal situation was regarded as unclear by 81 % of the emergency physicians, 85 % favored a unique, officially authorized type of directive. CONCLUSION The high number of returned questionnaires shows the importance of the topic "advance directives" for emergency physicians. Despite some practical and legal problems, a big majority of the experienced emergency physicians in this survey thought the advance directives in the prehospital setting to be helpful. A clear statement on resuscitation as well as simplification of the many existing types of directives are the most essential requirements demanded by the emergency physicians. A solution could be the creation of an extra "emergency advance directive".

Schon jetzt ist der Anasthesist an zahlreichen Schnittstellen der operativen Versorgung tatig und kunftig fur die Rolle des Koordinationsarztes zur perioperativen Fuhrung von Patienten pradestiniert. Seine vielfachen Kontaktstellen und sein Querschnittswissen befahigen ihn zur Ubernahme einer interdisziplinaren strukturverbundenen Rolle im Krankenhaus - auch in einer entsprechenden Fuhrungsposition. Im Fach der Anasthesiologie ist eine differenzierte, qualifiziert aufgebaute Struktur erforderlich: Nur eine verbesserte Qualifizierung des Assistenzpersonals, die mit einer Erweiterung des Aufgaben- und Verantwortungsbereiches einhergeht, ist in der Lage, sowohl die humanen als auch die okonomischen Engpasse zu verringern. Der Anasthesist selbst aber muss kompetenter Partner aller operativen Disziplinen sein, angefangen vom praoperativen Assessment bis zur Entlassung aus dem Krankenhaus. Als perioperativer Manager begleitet er den Weg des Patienten durch alle Schritte der Diagnostik und Therapie. Hierdurch wird der Schritt vom Narkosearzt zum perioperativen Mediziner vollzogen. The anaesthetist is already actively involved in numerous interfaces of surgical care, and is thus predestined to take over the role of the coordinating physician for the peri-operative patient care. His numerous contacts and comprehensive skills render him particularly suited to take on the responsibilities of an interdisciplinary structure-binding role in the hospital - also in a managerial position. In the specialty of anaesthesiology, a differentiated qualified structure is needed: only an improvement in the qualifications of assistant staff, associated with an expansion of tasks and responsibility can reduce both personnel and economic bottlenecks. The anaesthetist himself, however, must be a competent partner for all operative disciplines ranging from pre-operative assessment to patient discharge from the hospital. As a peri-operative manager he accompanies the patient through all the diagnostic and therapeutic steps. In this way, the step from anaesthetist to peri-operative physician can be accomplished.

Abstract

OBJECTIVE It is very important to investigate the patient's disability and pain. Interviews of intubated and tracheotomised patients were neglected because of inadaequate measuring methods. This is the first prospective study that evaluates the disability and pain of intubated and tracheotomised patients. METHODS Disability, Hospital Anxiety and Depression Scale, Visuelle Analogue Scale, Glasgow Coma Scale and structured questions were used to investigate the disability and pain of ventilated patients. 26 patients of an interdisciplinary operative intensive care unit took part in the study. Sociodemographic parameters, ventilation, sedation and pain were evaluated. Doctors and nurses were asked to assess the patient's pain and disability. RESULTS 17 intubated and 9 tracheotomised patients were included in the study. Mean intensity of pain was 30.3 (SD = 31.4), anxiety 40.8 (SD = 31.4), disability 30.0 (SD = 11.5) and disability caused by ventilation 61.9 (SD = 28.5). 46.2 % of the patients had a pathological subscale of anxiety and 50 % of depression in the Hospital Anxiety and Depression Scale. Nurses assessed the patient's disability and pain better than the doctors. CONCLUSION A high disability has to be taken in account in the therapy of intubated and tracheotomised patients.

Abstract

Seit Einfuhrung moderner Verfahren der Herz-Lungen-Wiederbelebung Anfang der Sechziger Jahre des vergangenen Jahrhunderts konnten zahlreiche Patienten mit Herzkreislaufstillstand wiederbelebt und gerettet werden. In der Mehrzahl der Falle bleiben solche Bemuhungen jedoch immer noch erfolglos, nur wenige Patienten erreichen wieder ihre fruhere gesundheitliche Verfassung. Wann stellen Wiederbelebungsmasnahmen tatsachlich eine noch erwunschte Hilfe dar, wann ist ihre Anwendung nicht mehr gerechtfertigt? Wie bei anderen medizinischen Interventionen sollte die Entscheidung nicht nur auf medizinischen Uberlegungen beruhen, sondern allgemeine ethische Grundsatze einbeziehen und sich am Wohl und den Praferenzen des Patienten orientieren. Erleidet ein vermeintlich Gesunder unerwartet einen Kreislaufstillstand, so stellt der Beginn von Wiederbelebungsmasnahmen den anerkannten Standard der Behandlung dar. Der Zeitdruck bei einer Unterbrechung der Sauerstoffversorgung des Gehirns macht ein unverzugliches Handeln erforderlich. Jede Verzogerung erhoht die Gefahr eines bleibenden neurologischen Schadens oder eines irreversiblen Todes. Dennoch sollten Wiederbelebungsbemuhungen nicht reflexartig erfolgen. Eindeutige Grunde fur den Verzicht sind sichere Todeszeichen oder mit dem Leben nicht zu vereinbarende schwerste Verletzungen. Auch ein hohes Alter und schwere Vorerkrankungen konnen die Entscheidungen beeinflussen. Bei final erkrankten Patienten ist der Eintritt des Kreislaufstillstands das absehbare Ende eines Sterbeprozesses. Der Entschluss, von Wiederbelebungsbemuhungen abzusehen, bedeutet bei einem Sterbenden nicht den Verzicht auf Beistand und Leidenslinderung. Gelingt zwar die Wiederherstellung des Kreislaufs, doch der Patient bleibt bewusstlos und verstirbt Tage oder Wochen spater, so bedeutet ein solches Schicksal eine schwere Last fur alle Beteiligten. Allerdings ist zum Zeitpunkt des Kreislaufstillstandes die Prognose oftmals nicht abschatzbar. Die Notwendigkeit des schnellen Handelns begrundet die Empfehlung, im Zweifelsfall mit Wiederbelebungsbemuhungen zu beginnen: in dubio pro vita. Bei einem Kreislaufstillstand kann das Recht des Patienten auf Selbstbestimmung nur begrenzt berucksichtigt werden, da dieser sich nicht mehr ausern kann. Eine Aufklarung uber mogliche Konsequenzen der Wiederbelebungsbemuhungen kann nicht erfolgen, die Einwilligung wird unterstellt. Patientenverfugungen konnen bei der Abschatzung des Patientenwillens weiterhelfen. Der Wunsch des Patienten nach Verzicht auf eine Behandlung muss grundsatzlich respektiert werden. Unklar ist der konkrete Umgang mit Patientenverfugungen in Notfallsituationen. Oftmals sind solche Dokumente nicht sofort verfugbar. Unter Zeitdruck mussen Arzte stellvertretend fur den Patienten entscheiden, Fragen konnen erst spater gestellt werden. Angesichts der Bewahrung des Lebens als zentraler Aufgabe der Medizin muss das ethische Dilemma akzeptiert werden, dass gelegentlich Wiederbelebungsmasnahmen erfolgen, von denen sich spater herausstellt, dass sie nicht mehr indiziert oder unerwunscht waren.

Abstract

STAR measurements of dihadron azimuthal correlations (Lambda phi) are reported in midcentral (20-60%) Au + Au collisions at v root sNN = 200 GeV as a function of the trigger particle's azimuthal angle relative to the event plane, phi(s) = | phi(1) -psi(EP)|. The elliptic (v(2)), triangular (v(3)), and quadratic (v(4)) flow harmonic backgrounds are subtracted using the zero yield at mini + Au collisions. It is found that a finite near-side (| Delta phi | pi/2) correlation shows a modification from d+ Au data, varying with fs. The modification may be a consequence of path-length-dependent jet quenching and may lead to a better understanding of high-density QCD.

Abstract

Palliativmedizin sieht ihr Ziel in der Verbesserung der Lebensqualitat inkurabler Patienten und deren Angehorigen. Gemas der WHO besitzt die Beherrschung von Schmerzen, weiteren korperlichen Symptomen sowie psychosozialen und spirituellen Problemen hochste Prioritat. Das medizinisch-arztliche Aufgabenfeld ist untrennbar mit pflegerischen und psychosozialen Kompetenzen in einem multiprofessionellen Team verbunden. Besonders im fortgeschrittenen Verlauf von Krebserkrankungen konnen neben Schmerzen zahlreiche weitere, extrem beeintrachtigende Beschwerden auftreten wie Obstipation, Ubelkeit und Erbrechen, gastrointestinaler Verschluss und Luftnot. Zunachst ist es wichtig, die Ursachen der einzelnen Symptome zu ergrunden, da sich therapeutische Entscheidungen an den vorliegenden Pathomechanismen orientieren. Eine effektive medikamentose Symp-tomkontrolle setzt genaue Kenntnisse der Wirkmechanismen voraus. Die haufig schwierige Entscheidung zwischen kausalen und symptomatischen Therapieangeboten muss – wenn immer moglich – zusammen mit dem Patienten und haufig im interdisziplinaren medizinischen Konsil getroffen werden. Die Tumorschmerztherapie orientiert sich an den Leitlinien der Weltgesundheitsorganisation. Entscheidend sind Dauertherapie und Dosistitration der Analgetika sowie spezifische Therapieansatze fur neuropathische Schmerzanteile. Eine effektive Prophylaxe und Behandlung der Obstipation konnen die Ernahrungslage verbessern und Ileussituationen vermeiden. Neue Erkenntnisse uber die Auslosung von Ubelkeit und Erbrechen durch Neurotransmitter eroffnen spezifische Angriffspunkte. Die Therapie der Dyspnoe stellt hohe Anforderungen an das Behandlungsteam, da pflegerische Masnahmen die hierbei eingeschrankteren medizinischen Moglichkeiten effektiv erganzen mussen.

Abstract

In Deutschland ist wie in nahezu allen Industrielandern eine Zunahme der Zahl von Tumorerkrankungen und der Krebsmortalitat zu verzeichnen.Gemas den Angaben des Statistischen Bundesamtes starben 1998 nahezu 213.000 Personen infolge einer bosartigen Erkrankung. An Krebs Erkrankte leiden in der Regel an einer Vielzahl von physischen und psychischen Funktionsstorungen, die bei unzureichender oder fehlender Behandlung zu einem deutlichen Verlust an Lebensqualitat fuhren. Schmerzen sind dabei das dominierende Symptom mit entsprechend gravierenden Auswirkungen auf den Patienten und seine Angehorigen. Zwar gibt es von verschiedenen Organisationen Therapiekonzepte fur eine suffiziente Schmerztherapie, doch diese werden haufig nicht in ausreichendem Mase umgesetzt. Dieser Beitrag stellt daher umfassend die Schmerztherapie als einen entscheidenden Eckpfeiler in der Palliativmedizin dar.

Abstract

This article presents an overview of the current situation of euthanasia in Europe. Emphasis is given to the positions discussed in the Netherlands and in Germany. The current situation, the development of the legal positions, and the resulting debate are established by analysing English and German anesthesiological and medical-ethical journals. It has to be noted that many physicians are not satisfied with the terminology of euthanasia. The traditional concepts of euthanasia do not cover the aspect of accompanying terminally ill persons until they have died. The differentiation of active, passive, and indirect euthanasia does not correspond to the practical handling of the problem. Many physicians are in need of an open discussion of euthanasia-related issues. The way euthanasia is practiced in the Netherlands has strongly influenced the further development of the debate in Europe. Even though the Dutch model is rejected by the jurisdications of virtually all other countries, and official statements of medical corporations stick to the disapproval of active euthanasia, studies examining the attitudes towards euthanasia and the treatment of it in daily routine show that active interventions to shorten life are performed to different degrees outside of the Netherlands as well.

Abstract

Objective Thirty years ago, cardiopulmonary resuscitation (CPR) was primarily developed for otherwise healthy individuals who experienced sudden cardiac arrest. Today, CPR is widely viewed as an emergency procedure that can be attempted on any person who undergoes a cessation of cardiorespiratory function. Therefore, the appropriateness of CPR has been questioned as a matter of the outcome, the patient's preferences, and the cost. The objective of this article is to analyse ethical issues in prehospital resuscitation. Arguments CPR is bound by moral considerations that surround the use of any medical treatment. According to Beauchamp and Childress, the hierarchy of justification in biomedical ethics consists of ethical theories, principles, rules, and particular judgements and actions. The decision to start CPR is based on the medical judgement that a person is suffering from circulatory arrest. The decision is justified by the moral rule that the victim of a cardiac arrest has the right to survive and to receive CPR. Moral rules are more specific to contexts and are based on ethical principles. The principle of beneficence means the provision of benefits for the promotion of welfare. Talking about beneficence in resuscitation means once again reporting stories of success, as many victims of pre- and in-hospital sudden death have been saved in the past. Nevertheless, resuscitative efforts still remain unsuccessful in the majority of cases, involving the principle of nonmaleficence. There is potential harm in CPR. Survivors may recover cardiac function, but sustain severe hypoxic brain damage, at worst surviving without awakening for months or years. In particular, post-traumatic CPR is associated with an extremely poor outcome, leading to the issue of futility. However, futility should be defined in a strict fashion, as there might be an individual chance of survival. The principle of respect for autonomy means the right of a patient to accept or reject medical treatment, which continues in emergency conditions and after the patient has lost consciousness. The time frame in CPR requires medical decision-making within seconds, and CPR is usually initiated without the patient's involvement. If the patient's wish's can be ascertained later on, life-sustaining therapies might be withdrawn at the time. Terminally ill but still competent patients should be encouraged to write a no-CPR document, which does not deny patients relief from severe symptoms, but might facilitate withholding resuscitative efforts at the scene. The principle of justice affects priorities in the allocation of health care resources. The decision made for a particular patient might delay or prevent emergency treatment in other patients who could receive greater benefit. Conclusions The standard of care remains the prompt initiation of CPR. However, ethical principles such as beneficence, nonmaleficence, autonomy, and justice have to be applied in the unique setting of emergency medicine. Physicians have to consider the therapeutic efficacy of CPR, the potential risks, and the patient's preferences.

Abstract

AIM Magnesium deficiency was investigated in critically ill patients, comparing measurements of plasma concentrations with the results obtained by the magnesium tolerance test. DESIGN AND METHODS 20 critically ill patients (5 females, 15 males) between the ages of 27 and 86 were investigated. Magnesium plasma concentrations were determined before the magnesium tolerance test according to Ryzen was performed. For this purpose, magnesium sulfate (0.1 mmol/kg) was infused intravenously over four hours. Renal magnesium excretion was measured in the 24 h urine beginning at the start of the infusion. Magnesium concentrations in plasma and urine were determined using atomic absorption spectrophotometry. MAIN RESULTS In 12 patients magnesium plasma concentrations were decreased to 0.58-0.79 mmol/l. 6 patients showed values within the normal range of 0.80 to 1.0 mmol/l. In 2 patients the plasma concentration was increased to 1.07 and 1.27 mmol/l. Parenteral magnesium tolerance testing revealed a considerable magnesium deficiency by retention of 65-100% of the loading dose in 14 of the 20 patients. The remaining 6 patients retained 23-48% of the loading dose, thus demonstrating a moderate magnesium deficiency. CONCLUSION Determination of magnesium plasma concentration appears suitable as an informative preliminary survey, since low values are reliable indicating a magnesium deficiency. However, this study confirms that normal magnesium plasma concentrations do not exclude a considerable magnesium deficiency, which is more sensitively determined by the magnesium tolerance test.

Abstract

A systematic study is presented for centrality, transverse momentum (p(T)), and pseudorapidity (eta) dependence of the inclusive charged hadron elliptic flow (v(2)) at midrapidity (vertical bar eta vertical bar < 1.0) in Au + Au collisions at root s(NN) = 7.7, 11.5, 19.6, 27, and 39 GeV. The results obtained with different methods, including correlations with the event plane reconstructed in a region separated by a large pseudorapidity gap and four-particle cumulants (v(2){4}), are presented to investigate nonflow correlations and v(2) fluctuations. We observe that the difference between v(2){2} and v(2){4} is smaller at the lower collision energies. Values of v(2), scaled by the initial coordinate space eccentricity, v(2)/epsilon, as a function of p(T) are larger in more central collisions, suggesting stronger collective flow develops in more central collisions, similar to the results at higher collision energies. These results are compared to measurements at higher energies at the Relativistic Heavy Ion Collider (root s(NN) = 62.4 and 200 GeV) and at the Large Hadron Collider (Pb + Pb collisions at root s(NN) = 2.76 TeV). Abstract

We report measurements of charmed-hadron $({D}^{0},{D}^{*})$ production cross sections at midrapidity in $p+p$ collisions at a center-of-mass energy of 200 GeV by the STAR experiment. Charmed hadrons were reconstructed via the hadronic decays ${D}^{0}\ensuremath{\rightarrow}{K}^{\ensuremath{-}}{\ensuremath{\pi}}^{+}$, ${D}^{*+}\ensuremath{\rightarrow}{D}^{0}{\ensuremath{\pi}}^{+}\ensuremath{\rightarrow}{K}^{\ensuremath{-}}{\ensuremath{\pi}}^{+}{\ensuremath{\pi}}^{+}$ and their charge conjugates, covering the ${p}_{T}$ range of $0.6--2.0$ and $2.0--6.0\text{ }\text{ }\mathrm{GeV}/c$ for ${D}^{0}$ and ${D}^{*+}$, respectively. From this analysis, the charm-pair production cross section at midrapidity is $d\ensuremath{\sigma}/dy{|}_{y=0}^{c\overline{c}}=170\ifmmode\pm\else\textpm\fi{}45(\mathrm{stat}{)}_{\ensuremath{-}59}^{+38}(\mathrm{sys})\text{ }\text{ }\ensuremath{\mu}\mathrm{b}$. The extracted charm-pair cross section is compared to perturbative QCD calculations. The transverse momentum differential cross section is found to be consistent with the upper bound of a fixed-order next-to-leading logarithm calculation.

Abstract

Measurements of the differential cross section and the transverse single-spin asymmetry, A(N), vs x(F) for pi(0) and eta mesons are reported for 0.4 0.55, is consistent with a perturbative QCD prediction, and the eta/pi(0) cross-section ratio agrees with existing midrapidity measurements. For 0.55 < x(F) < 0.75, the average A(N) for eta is 0.210 +/- 0.056, and that for pi(0) is 0.081 +/- 0.016. The probability that these two asymmetries are equal is similar to 3%.

Abstract

We report STAR measurements of the longitudinal double-spin asymmetry A_LL, the transverse single-spin asymmetry A_N, and the transverse double-spin asymmetries A_Sigma and A_TT for inclusive jet production at mid-rapidity in polarized p+p collisions at a center-of-mass energy of sqrt{s} = 200 GeV. The data represent integrated luminosities of 7.6 /pb with longitudinal polarization and 1.8 /pb with transverse polarization, with 50-55% beam polarization, and were recorded in 2005 and 2006. No evidence is found for the existence of statistically significant jet A_N, A_Sigma, or A_TT at mid-rapidity. Recent model calculations indicate the A_N results may provide new limits on the gluon Sivers distribution in the proton. The asymmetry A_LL significantly improves the knowledge of gluon polarization in the nucleon.

Abstract

We report on the mid-rapidity mass spectrum of di-electrons and cross sections of pseudoscalar and vector mesons via e(+) e(-) decays, from root s = 200 GeV p + p collisions, measured by the large-acceptance experiment STAR at the Relativistic Heavy Ion Collider. The ratio of the di-electron continuum to the combinatorial background is larger than 10% over the entire mass range. Simulations of di-electrons from light-meson decays and heavy-flavor decays (charmonium and open charm correlation) are found to describe the data. The extracted omega -> e(+) e(-) invariant yields are consistent with previous measurements. The mid-rapidity yields (dN/dy) of phi and J/psi are extracted through their di-electron decay channels and are consistent with the previous measurements of phi -> K+ K- and J/psi -> e(+) e(-). Our results suggest a new upper limit of the branching ratio of the eta -> e(+) e(-) of 1.7 x 10(-5) at the 90% confidence level.
