We have demonstrated the effect of covering an N95 filtering facepiece respirator (FFR) with an overlying face mask. In total, 100 participants successfully completed quantitative fit testing wearing a 3M 1870+ FFR. Among them, 13 (13%; 95% CI, 7%–22%) failed subsequent fit testing when simultaneously wearing a Halyard 47117 procedural mask over the FFR.

New technology has facilitated survey research of anesthesia professional society members. We evaluated prevailing metrics of quality and impact of published research studies based on surveys of anesthesiologists. We hypothesized that adherence to recommended practices (such as use of reminders) would be associated with increased survey response rates, and that higher response rates would be associated with higher article impact. Using the MEDLINE database, we identified 45 English-language research articles published in 2010-2017 reporting original data from surveys of anesthesiologists. The median response rate was 37% (IQR: 25-46%). Recommended survey practices, including the use of reminders (p = 0.861) and validated questionnaires (p = 0.719), were not correlated with response rates. In turn, survey response rates were not associated with measures of article impact (p = 0.528). The impact of published research based on surveys of anesthesiologists, as measured by citation scores (p = 0.493) and Altmetrics (p = 0.826), may be driven primarily by the novel data or questions raised using survey methodology, but does not appear to be associated with response rates. Improving reporting of survey methodology and understanding possible sources of non-response bias are important for future studies in this area.

Background We examined the effects of integrated cognitive-behavioral therapy for depression and insomnia (CBT-D + CBT-I) delivered via videoconferening in rural, middle aged and older adults with depressive and insomnia symptoms. Method Forty patients with depressive and insomnia symptoms were randomized to receive either 10 sessions of CBT-D + CBT-I or usual care (UC). Patients in the integrated CBT condition were engaged in telehealth treatment through Skype at their primary care clinic. Assessments were conducted at baseline, post-treatment, and 3-month follow-up. Results CBT-D +CBT-I participants had significantly greater improvements in sleep at post-treatment and 3-month follow-up as compared to the UC participants. The time by group interaction for depression was not significant; both the CBT-D + CBT-I and UC conditions had a decrease in depressive symptoms over time. Conclusion While integrated CBT benefits both depression and insomnia symptoms, its effects on depression are more equivocal. Further research should consider expanding the depression treatment component of integrated CBT to enhance effectiveness.

BACKGROUND Understanding how survey methodology and quality measures are associated with progress from abstract presentation to manuscript publication can help optimize the design of survey research in anesthesiology, and enhance respondents' confidence in the value of survey participation. AIMS The aim of this study was to determine if adherence to survey method recommendations and attainment of high response rates are associated with faster progress to publication among abstracts initially presented at anesthesiology society meetings. METHODS Abstracts from the American Society of Anesthesiologists' (ASA) Annual Meeting, Association of Anaesthetists of Great Britain and Ireland (AAGBI) Annual Congress, and the International Anesthesia Research Society (IARS) Annual Meeting from 2011-2014 were reviewed. Abstracts reporting original survey data collection were included in a systematic search for resulting publications in peer-reviewed academic journals. Cox proportional hazards regression was used to analyze progress to publication. RESULT Ninety-nine ASA, 76 AAGBI, and 30 IARS abstracts met inclusion criteria. Among these abstracts, 43 (43%) from ASA, none from AAGBI, and 7 (23%) from IARS have been published as original research articles or brief reports. Surveying patients or caregivers, as opposed to medical professionals, was associated with increased likelihood of publication (hazard ratio [HR] = 4.4, 95% confidence interval [CI]: 1.6, 12.4, P = 0.005) as was a larger sample size (eg, >500 vs

Domino liver transplantation (DLT) utilizes the explanted liver of one liver transplant recipient as a donor graft in another patient. While there may be unique risks associated with DLT, it is unclear if DLT has less favorable long-term outcomes than deceased donor liver transplantation (DDLT). We used a propensity score matching approach to compare the outcomes of DLT recipients to DDLT recipients. The United Network for Organ Sharing (UNOS) registry was queried for patients undergoing DLT or DDLT in 2002-2016. Each DLT recipient was matched to a unique DDLT recipient to compare mortality and graft failure. There were 126 DLT and 62 835 DDLT recipients meeting inclusion criteria. After propensity score matching on recipient pre-transplant characteristics, 123 DLT cases were matched to DDLT controls from the same UNOS region. On stratified Cox proportional hazards regression, DLT incurred no increase in the hazard of mortality [hazard ratio (HR) = 1.4; 95% confidence interval (CI): 0.8, 2.7; P = 0.265] or graft failure (HR = 1.2; 95% CI: 0.7, 2.1; P = 0.556) compared to DDLT. Using a large national registry, a propensity-matched analysis found no increased risk of mortality or graft failure associated with DLT compared to DDLT.

Some students cope with the conflicting demands of work, family, health, and college by engaging in adjustment-seeking behavior, whereby students try to persuade instructors to make the kinds of personalized adjustments needed to avoid the potential consequences of conflicts between academic responsibilities and their other responsibilities. Hypothesized here is that students’ adjustment-seeking behaviors are predicated by knowledge of students’ political skill and adjustment-seeking self-efficacy. Participants completed a survey measuring adjustment-seeking behavior, adjustment-seeking self-efficacy, and political skill. Multiple regression analysis revealed that students’ political skill and adjustment-seeking self-efficacy contributed significantly to the prediction of adjustment-seeking behavior, as hypothesized. Adjustment-seeking behavior was found to be more frequent among participating students with higher levels of political and self-efficacy skills. Implications for research and practice are discussed.

Objective Restless legs syndrome (RLS) is a common neurological movement disorder occurring in approximately 10% of the general population. The prevalence of moderately severe RLS is 2.7% overall (3.7% for women and 1.7% for men). Epilepsy is also a common neurological disorder with significant associated morbidity and impact on quality of life. We evaluated the severity and frequency of primary RLS in patients with localization-related temporal lobe epilepsy (TLE) and investigated the role of prodromal RLS symptoms as a warning sign and lateralizing indicator. Methods All epilepsy patients seen in the outpatient clinic were screened for movement disorders from 2005 to 2015. Ninety-eight consecutive patients with localization-related TLE (50 right TLE and 48 left TLE) who met inclusion criteria were seen in the outpatient clinic. The control group consisted of 50 individuals with no history or immediate family history of epilepsy. Each patient was evaluated with the International Restless Legs Study Group (IRLSSG) questionnaire, NIH RLS diagnostic criteria, ferritin level, and comprehensive sleep screening including polysomnography. Furthermore, patients with obstructive sleep apnea or a definite cause of secondary restless legs syndrome such as low serum ferritin or serum iron levels were also excluded from the study. Results There was a significant association between the type of epilepsy and whether or not patients had RLS χ 2 (1) = 10.17, p 2 Goodness of Fit Test. Based on the odds ratio, the odds of patients having RLS were 4.60 times higher if they had right temporal epilepsy than if they had left temporal epilepsy, serving as a potential lateralizing indicator. A prodromal sensation of worsening RLS occurred in some patients providing the opportunity to intervene at an earlier stage in this subgroup. Significance We identified frequent moderate to severe RLS in patients with epilepsy. The frequency of RLS was much more common than would typically be seen in patients of similar age. The restlessness was typically described as moderately severe. The RLS symptoms were more common and somewhat more severe in the right TLE group than the left TLE group.

Although the transport of neonates is generally safe, adverse events can occur where equipment is a contributing factor. The aims of the study were to explore how the types of neonatal intensive care unit bed in use could impact a simulated emergency endotracheal intubation and to identify future areas for training and education.The efficiency of endotracheal intubation performed during simulated neonatal transport using 3 different transport modalities (closed incubator bed, open incubator bed, and open radiant warmer bed) was assessed. Twenty participants were enrolled. Outcomes included time to intubation, intubation success, and ease of mannequin access and were compared using Wilcoxon signed-rank tests and McNemar exact tests.Median times to intubation were 59, 44, and 37 seconds with the incubator top closed, with the top open, and with the open radiant warmer bed, respectively. Intubation was slowest and subjective ease of access was most difficult with the incubator top closed.Experienced anesthesia providers had significantly greater difficulty with simulated emergency endotracheal intubation when performing neonatal transport with the incubator top closed compared with available alternative modes.

Purpose The purpose of this paper is to describe and define the concept of student adjustment-seeking behavior, differentiating it from the related behaviors of compliance-seeking, and help-seeking. Design/methodology/approach Based on a survey of college students, documentation is provided as to the frequency of occurrence of student adjustment-seeking behavior. Also presented is empirical, albeit exploratory, evidence of a statistically significant correlation between self-reports of prior adjustment-seeking behavior, and adjustment-seeking likelihood. Findings In support of the study hypothesis, a moderately high, bivariate correlation was observed between adjustment-seeking likelihood and prior adjustment-seeking behavior. Originality/value Further attention to the topic and measurement of adjustment-seeking behavior would seem to be justified and continued efforts to validate the measure of adjustment-seeking likelihood may be fruitful in efforts to better understand adjustment-seeking behavior. Research recommendations, practical applications, and study limitations are discussed.

Epilepsy is a relatively common neurologic disorder in children that has important implications for development, parents, and society. Making the correct diagnosis starts with an accurate and complete history that consequently leads to a directed diagnostic workup. This article outlines a diagnostic and management approach to pediatric seizures and epilepsy syndromes. Making the correct diagnosis of epilepsy or nonepileptic imitators allows the practitioner to prescribe appropriate therapy. Initial management for typical epileptic syndromes and seizures and potential adverse effects are discussed. Alternative treatment options for pharmacologically resistant patients such as ketogenic diet, vagal nerve stimulation, and surgery are also discussed. While most children favorably respond to antiepileptic medications, early identification of medication failure is important to ensure optimal neurodevelopment.

Introduction Obstructive Sleep Apnea (OSA) and Sleep Disordered Breathing (SDB) in children tend to be a more complex and multifactorial disease than in adults. Although adult screening tools, such as the STOP-BANG questionnaire, their application limited in pediatrics. We used our previously described 6-point questionnaire to identify OSA in children and evaluated its use for predicting post-operative respiratory events. Methods Children from 3 to 18 years of age presenting for surgery were eligible. Exclusion criteria were emergency surgery or refusal to participate. A 6-question survey regarding symptoms of OSA/SDB was administered preoperatively. Neck circumference was measured. Height and weight were recorded from preoperative data and the body mass index (BMI) percentile obtained. Results 749 patients were enrolled in the study. 707 patients were in the final analysis (359 boys and 348 girls, mean age 12 ± 4 years). The median 6-item questionnaire score was 1 (interquartile range: 0, 2) and 186 (26%) scored ≥ 2 of 6 points. Children with predicted OSA (yes on ≥ 2 questions) were more likely than without predicted OSA to require supplemental oxygen in the PACU (24% vs. 17%; 95% confidence interval [CI] of difference: −0.3%, 13%; p = 0.049). Amongst 681 patients with available data on Post Anesthesia Care Unit (PACU) length of stay (LOS), prolonged LOS (>1 h) was not more likely among children with predicted OSA (42%) compared to those without predicted OSA (39%; 95% CI of difference: −5%, 11%; p = 0.479). Outcomes assessed after PACU discharge noted no differences. Specifically, overnight hospital stay was required in 33% of patients with predicted OSA as compared to 29% of those without (95% CI of difference: −4%, 11%; p = 0.399). On POD 0, supplemental oxygen was used on the inpatient ward for 6% of patients with predicted OSA compared to 4% of patients without predicted OSA (95% CI of difference: −2%, 6%; p = 0.272). Conclusion The incidence of OSA/SDB is under-appreciated in children presenting for non-otolaryngological surgical procedures. Although patients judged to have OSA on the 6-item question may need for supplemental oxygen longer in the PACU, no other outcomes differences were noted.

Six loci, apoliproprotein B (including Ag(x) antigen), immunoglobulin kappa constant region (IGKC), luteinizing hormone/choriogonadotrophin receptor, avian myelocytomatosis viral related oncogene, neuroblastoma derived, ornithine decarboxylase, and proopiomelanocortin (adrenocorticotropin/beta-lipotropin) (POMC), were newly assigned to sheep chromosome 3p using a chromosomally characterized minipanel of sheep-hamster cell hybrids. Isotopic in situ hybridization of IGKC to sheep chromosome 3p22-p17 is reported, confirming the cell hybrid assignment. As these loci are all known to map to human chromosome 2p, this study demonstrates that this chromosomal segment is extensively conserved in sheep. Only POMC has been previously assigned to cattle chromosome 11, which is the equivalent of sheep chromosome 3p. Therefore, we predict that the other loci assigned in this study to sheep 3p are likely to be located on cattle 11. The provisional assignment of an additional locus, annexin-like to sheep chromosome 3p is also reported.

Following a discussion of the impact of sleep disorders, the clinical management of chronic obstructive pulmonary disorder is discussed, as are asthma and obstructive sleep apnea. Restrictive lung diseases are outlined. The behaviorial management of insomnia appears to improve sleep and the implications are detailed. Finally, the pharmacologic treatment of insomnia for patients who have chronic pulmonary disease is reviewed in detail.

BACKGROUND In the past, clinical decisions regarding treatment of neurovascular disorders leading to ischemia have been guided by the percentage of stenosis of the vessel in question. However, such an approach assumes a predictable and stable relationship between the percentage of stenosis and the degree of flow reduction it causes. Historically, this type of relationship has been difficult to document. Thus, a method for noninvasively measuring the absolute flow of specific cerebral arteries is of potential practical value. METHODS We set to quantify the mean blood flow (Qm, in mL/min) in the cerebral arteries using quantitative magnetic resonance angiography (QMRA), and to compare the findings in normal vessels with those found in vessels considered pathologically narrowed. Specific vascular segments were identified, studied, and the results entered into a database. Statistical analyses of the measurements were carried out using StatPlus for Microsoft Excel. It involved comparing of the Qm found in specific vessels, as well as those found in normal and abnormal vessels, using analysis of variance (ANOVA). The abnormal vessels were selected from specifically identified magnetic resonance angiography (MRA) studies. RESULTS A total of 57 patients, 26 men and 31 women, with ages ranging from 19 to 86 years (mean = 64.5), underwent MRA with subsequent QMRA of 157 arteries. The latter included 72 internal carotid (ICA), 45 vertebral (VA), 18 common carotid (CCA), 13 middle cerebral (MCA), and nine basilar (BA) arteries. The mean Qm obtained were CCA = 313.9 (±115.4), ICA = 231.1 (±83.7), VA = 90.5 (±45.8), MCA = 92.5 (±62.3) and BA = 120.1 (±64.5). ANOVA showed significant differences between individual vessels (P < .00001). The values obtained were consistent with those predicted mathematically, as derivatives of their proportional contributions to overall cerebral arterial flow (Qbrain) as a product of the normal cardiac output (CO) [ie, Qbrain = CO*.2]. Further ANOVA of the normal and abnormal vessels showed statistically significant differences between the two groups (186.5 ± 108.6 vs. 117.9 ± 76.1, respectively; P= .000514). CONCLUSIONS The use of QMRA provides the mean for direct measurement of absolute blood flow within the cerebral arteries in physiologic and pathologic states. This technique may be of future importance in characterizing absolute flow compromise in the cerebral arteries under a variety of clinically relevant circumstances.

The Antisocial Features (ANT) scale of the Personality Assessment Inventory (PAI) was subjected to taxometric analysis in a group of 2,135 federal prison inmates. Scores on the three ANT subscales—Antisocial Behaviors (ANT-A), Egocentricity (ANT-E), and Stimulus Seeking (ANT-S)—served as indicators in this study and were evaluated using the following taxometric procedures: mean above minus below a cut (MAMBAC), maximum eigenvalue (MAXEIG), and latent mode factor analysis (L-Mode). Objective and subjective evaluation of the results revealed consistent support for a dimensional interpretation of latent structure across the di ferent taxometric procedures as well as across gender, race, and security level. As a dimensional construct, antisocial personality disorder arranges respondents along one or more quantitative dimensions (degree of antisociality), rather than assigning them to qualitatively distinct categories (antisocial or not antisocial).

The construct validity of the Psychological Inventory of Criminal Thinking Styles's Fear-of-Change scale (FOC) was evaluated by correlating it with scales from the Personality Assessment Inventory (PAI). As expected, the FOC displayed a positive association with the PAI Anxiety (ANX) scale and negative associations with the PAI Positive Impression Management and Treatment Rejection scales among 136 male inmates. The FOC also forged a stronger relationship with the ANX's Cognitive subscale than with the Affective or Physiological subscales. It was further surmised that a subgroup of 54 participants retested after 3 months of program participation witnessed significant FOC gains. An independent sample of 239 medium-security male inmates participated in a second study in which significant FOC accretions were found in offenders who discontinued programming but not in offenders who continued or graduated. A third study conducted on 46 nonprogram participants effectively ruled out the alternative hypothesis that FOC scores rise in nonprogram offenders.

Methode: Systematische Literaturrecherche, Expertenbewertung und Konsensuskonferenzen. Ergebnis: Postakute Behandlungen bestehen in Maßnahmen zur Entwöhnung von Alkohol, d. h. zum Erhalt, der Verbesserung oder der Wiederherstellung der Funktions- und Leistungsfähigkeit des chronisch Alkoholkranken in Alltag und Beruf. Suchtspezifische Maßnahmen und allgemeine Methoden (z. B. Psychotherapie, Ergo-/Arbeitstherapie) werden nach Evidenzklassen (nach SIGN 1999) und Empfehlungsstärken (nach APA 1995) bewertet dargestellt und im Rahmen integrierter Programme als Sequenzbehandlungen empfohlen. Schlussfolgerung: Diese evidenzbasierte Leitlinie kann dem praktisch Tätigen in Klinik und Praxis im medizinischen, psycho- und soziotherapeutischen Bereich als Orientierung dienen und dazu beitragen, die Langzeitbehandlung der Alkoholabhängigen auf eine allgemeine empirische Grundlage zu stellen.

Thirty-four male white-collar offenders without a prior history of non-white-collar crime, 23 male white-collar offenders with at least one prior arrest for a non-white-collar crime, and 66 male non-white-collar offenders housed in a minimum security federal prison camp completed the Psychological Inventory of Criminal Thinking Styles and Social Identity as a Criminal scale and were rated on the Lifestyle Criminality Screening Form-Revised. Significant group differences were noted on the Psychological Inventory of Criminal Thinking Styles Self-Assertion/Deception scale, Social Identity as a Criminal Centrality subscale, Social Identity as a Criminal In-Group Ties subscale, and Lifestyle Criminality Screening Form-Revised, which showed that white-collar offenders with no prior history of non-white-collar crime registered lower levels of criminal thinking, criminal identification, and deviance than white-collar offenders previously arrested for non-white-collar crimes.

The records of 185 inmates rated on the Psychopathy Checklist-Revised (PCL-R) as part of a forensic evaluation for the US federal courts were reviewed for evidence of subsequent disciplinary reports in a 2-year follow-up. The Personality Assessment Inventory (PAI) was also administered to 149 of these inmates. The PCL-R Factor 2 score added significant predictive variance at the second step of a logistic regression analysis of disciplinary report outcome after demographics (age, education, ethnic status) and scores on the PAI Aggression Scale had been entered at step 1. The PAI Aggression Scale also displayed incremental validity by successfully predicting the presence of a subsequent disciplinary report at step 2 of a logistic regression analysis after age, education, ethnic status, and the PCL-R Factor 2 score had been entered at step 1. These and other results suggest that self-report measures may be just as effective as non-self-report rating scales in predicting forensic and correctional outcomes.

This article attempts to fill a void in the professional literature about career advancement of workers with disabilities. It reports on the results of a study of 240 deaf and hard-of-hearing college graduates, most of whom were supported by vocational rehabilitation when they were students. The success of these alumni 15 years into their careers was reflected in their overall satisfaction with their careers, prospects for promotions, and relations with their supervisors. These findings exemplify that career enhancement and productivity skills can be obtained and used by workers with disabilities. Implications for rehabilitation consumers and counselors are discussed.

The measurement of organic carbon in soils has traditionally used dichromate oxidation procedures including the Wakley and Black and the Heanes methods. The measurement of carbon in soils by high temperature combustion is now widely used providing a rapid automated procedure without the use of toxic chemicals. This procedure however measures total carbon thus requiring some means of correction for soil samples containing carbonate and charcoal forms of carbon. This paper examines the effects of known additions of charcoal to a range of soil types on the results obtained by the Walkley and Black, Heanes and combustion methods. The results show, that while the charcoal carbon does not react under Walkley and Black conditions, some proportion does so with the Heanes method. A comparison of six Australian Soil and Plant Analysis Council reference soil samples by the three methods showed good agreement between the Heanes method, the combustion method and only slightly lower recoveries by the Walkley and Black procedure. Carbonate carbon will cause an overestimation of soil organic carbon by the combustion method thus requiring a separate determination of carbonate carbon to be applied as a correction. This work shows that a suitable acid pre-treatment of alkaline soils in the sample boats followed by a drying step eliminates the carbonate carbon prior to combustion and the need for an additional measurement. The measurement of carbon in soils by high temperature combustion in an oxygen atmosphere has been shown to be a rapid and reliable method capable of producing results in good agreement with one of the established dichromate oxidation procedures.

This open-label, randomized study in 36 healthy postmenopausal women, investigated the pharmacokinetics of Fem7® (an oestradiol matrix-type patch), at doses of 25, 50, 75 and 100 μg/24 h, applied once weekly. Maximum plasma concentrations of oestradiol and oestrone were observed 14–20 h after patch application, remaining within the therapeutic range until removal, and returning to baseline within 12 h thereafter. Plasma oestradiol concentrations increased in a dose-dependent manner for all four doses, and plasma oestrone increased for the three highest doses. Fem7 treatment was well tolerated at all four doses and no severe adverse reactions were reported. Fem7 is a convenient and well-tolerated form of hormone-replacement therapy that delivers oestradiol in a consistent and predictable manner.

The employee age/job satisfaction relationship in facility relocation settings was examined. In a sample of 172 relocated workers, one of two relocation consequences was found to be negatively associated with job satisfaction. As hypothesized, both consequence measures interacted with employee age so that responses to consequences were more negative for older workers.

We surveyed employees from seven relocating firms undergoing either an expansion or decline. Employees' judgments of procedural fairnes regarding the decision procedures used to implement the change showed a stronger effect on normative commitment for sites undergoing decline than for those undergoing growth. Procedural fairness concerns therefore seem to be more important to employees experiencing organizational decline. The finding has implications for research and for the management of organizational decline.

Study objectives: Prior research investigating co-occurring insomnia/obstructive sleep apnea (CIO) has mainly focused on comparing comorbid patients, obstructive sleep apnea (OSA), and insomnia (INS) to those with OSA alone. This approach is informative but omits the potentially interesting comparison of comorbid patients to those with INS alone. Our study used an incomplete factorial design, crossing OSA (present or absent) with INS (present or absent) to more clearly focus on the question, is comorbid INS an epiphenomenon of OSA or an independent disorder? Methods: Our study was an archival analysis from the database of a sleep center comparing consecutively diagnosed patients characterized as OSA or INS. A third group, CIO, was derived from the OSA group. Our study was conducted at an American Academy of Sleep Medicine–accredited sleep disorders center. We studied 299 patients, including 94 OSA, 97 INS, and 108 CIO. Patients ranged from ages 15 to 86 years. Results: Groups were compared on polysomnography (PSG), sleep pattern, sleep stages, sleep pathology, self-reported sleep concerns, and self-reported daytime functioning. From a consecutive group of OSA patients, we estimate the prevalence of CIO at 67.4%. Based mainly on multivariate analysis of covariance (MANCOVA) controlling for demographic differences between groups, we found few if any significant differences between CIO and INS alone or between CIO and OSA alone. Conclusions: The clinical presentation of CIO is indistinguishable from INS alone, both with respect to PSG findings and to self-reported sleep onset and sleep maintenance disturbance. We observed a weak relation between OSA severity and co-occurring INS. These data are consistent with the view that INS with co-occurring OSA is an independent, self-sustaining disorder. We hypothesized that in some unknown proportion of cases, OSA initially instigated the INS, but the INS was then perpetuated and reshaped by sleep concerns and self-defeating compensatory behaviors.

To determine the prevalence of personality disorders and their relation to insomnia parameters among persons with chronic insomnia with hypnotic dependence.Eighty-four adults with chronic insomnia with hypnotic dependence completed the SCID-II personality questionnaire, two-weeks of sleep diaries, polysomnography, and measures of insomnia severity, impact, fatigue severity, depression, anxiety, and quality of life. Frequencies, between-subjects t-tests and hierarchical regression models were conducted.Cluster C personality disorders were most prevalent (50%). Obsessive-Compulsive personality disorder (OCPD) was most common (n=39). These individuals compared to participants with no personality disorders did not differ in objective and subjective sleep parameters. Yet, they had poorer insomnia-related daytime functioning. OCPD and Avoidant personality disorders features were associated with poorer daytime functioning. OCPD features were related to greater fatigue severity, and overestimation of time awake was trending. Schizotypal and Schizoid features were positively associated with insomnia severity. Dependent personality disorder features were related to underestimating time awake.Cluster C personality disorders were highly prevalent in patients with chronic insomnia with hypnotic dependence. Features of Cluster C and A personality disorders were variously associated with poorer insomnia-related daytime functioning, fatigue, and estimation of nightly wake-time. Future interventions may need to address these personality features.

Organizational transitions may be difficult to implement if employees resist the change. ‘Participation’ and ‘education’ (Kotter and Schlesinger, 1979) are hypothesized to enhance commitment to change, however mechanisms for those effects are unclear. In a sample of employees from seven relocated organizations, a test of our model showed that the effects of justification (a form of education) on intent to remain are mediated by outcome and procedural fairness judgements. Voice (a form of participation) showed no effects on fairness judgments, perhaps because employees did not expect voice in relocation decisions. Supplementary analyses yielded no evidence for direct effects of voice and justification on intention to remain. The findings extend our understanding of fairness to transition settings.

The relationship between age and the perceived value of pay (valence) was investigated for a sample of 453 nonunionized female employees at an American apparel plant. Pay valence was determined using a “pay addressing needs” format, suggested by Lawler (1971). For workers up to age 40, pay valence appeared higher for younger workers, but for workers older than 40 pay valence was unrelated to age. The ability of pay to satisfy specific Maslowian needs decreased with age (except with respect to physiological needs) for workers up to age 40.

Epilepsy, one of the most common neurological diseases, affects over 50 million people worldwide. Epilepsy can have a broad spectrum of debilitating medical and social consequences. Although antiepileptic drugs have helped treat millions of patients, roughly a third of all patients have seizures that are refractory to pharmacological intervention. The evolution of our understanding of this dynamic disease leads to new treatment possibilities. There is great interest in the development of devices that incorporate algorithms capable of detecting early onset of seizures or even predicting them hours before they occur. The lead time provided by these new technologies will allow for new types of interventional treatment. In the near future, seizures may be detected and aborted before physical manifestations begin. In this chapter we discuss the algorithms that make these devices possible and how they have been implemented to date. We also compare and contrast these measures, and review their individual strengths and weaknesses. Finally, we illustrate how these techniques can be combined in a closed-loop seizure prevention system.
