1. Recurrent maternal virilization during pregnancy in patients with PCOS: two clinical cases

Abstract

Abstract Background Maternal virilization during pregnancy is a rare phenomenon. Polycystic ovary syndrome (PCOS), luteoma and luteinic cysts are the most frequent and benign etiologies. This article presents two cases of recurrent maternal virilization during pregnancy. Clinical cases Our reported cases were young women with Afro-Caribbean and Nigerian origins. Data were collected by history-taking, clinical examination, laboratory investigations, transabdominal ultrasonographic examination and Magnetic Resonance Imaging. Both patients were diagnosed with PCOS according to the Rotterdam criteria. During each of their pregnancies they both developed an explosive hirsutism, a deepening in the voice, a clitoromegaly. Gestational diabetes occurred during pregnancies. There was no fetal virilization, despite raising androgen levels, more than tenfold to normal. Improvement of hirsutism and normalization of androgens were described in postpartum. Conclusion Only few cases of maternal virilization during pregnancy were reported in literature and even fewer concern recurrent and bilateral ovarian etiology. Hyperplasia of ovarian theca cells seems to be the most likely explanation, which would suggest that PCOS belongs to a spectrum of abnormal reactivity of the ovary to human Chorionic Gonadotrophin (hCG) stimulation along with luteoma and luteinic cyst of pregnancy. Insulin resistance could worsen hyperandrogenism but is not enough to explain virilization. Treatment should focus on protecting the fetus of possible virilization as well as its mother, but also on preserving the subsequent fertility in both.

Abstract

STUDY QUESTION What are the changes in serum concentration of total and cleaved anti-Muüllerian hormone (AMH) molecular forms and of androgens before and throughout pregnancy in women with and without polycystic ovary syndrome (PCOS) in a longitudinal follow-up investigation? SUMMARY ANSWER Serum levels of total and cleaved AMH are higher from preconception to the third trimester of pregnancy in women with PCOS as compared to controls, whereas testosterone and androstenedione levels are higher in women with PCOS than in control women before pregnancy and during the second and third trimester of pregnancy. WHAT IS KNOWN ALREADY Cross-sectional or partial longitudinal studies have shown higher AMH and androgen levels in pregnant women with PCOS as compared with non-PCOS women. To date, no complete longitudinal dynamic monitoring of the circulating forms of AMH and androgens from pre-conception to the third trimester of pregnancy have compared women with and without PCOS. STUDY DESIGN, SIZE, DURATION This systematic prospective quarterly longitudinal monocentric study was a comparative follow-up of 30 women with PCOS and 29 controls before and during pregnancy from April 2019 to July 2022. PARTICIPANTS/MATERIALS, SETTING, METHODS Women aged 18–43 years with a pre-conception measurement of AMH were included during the first trimester of a singleton pregnancy. The PCOS group was defined according to the Rotterdam diagnostic criteria. The control group patients included in the study had normal ovarian reserves. Circulating total and cleaved AMH, and serum estradiol, LH, and androgen levels were measured during the first, second, and third trimester of pregnancy in all study participants. MAIN RESULTS AND THE ROLE OF CHANCE Before pregnancy, patients with PCOS had higher levels of AMH than controls. The total and cleaved AMH forms were significantly higher in women with PCOS than controls from pre-conception to the third trimester of pregnancy (all P LIMITATIONS, REASONS FOR CAUTION Our control population was a population of infertile women with no ovarian problems but most of them had undergone ART treatments to achieve pregnancy. WIDER IMPLICATIONS OF THE FINDINGS These results strengthen the hypothesis that gestational hyperandrogenism as well as exposure to elevated AMH levels in utero could be driving forces predisposing female progeny to develop PCOS. Abstract

Study question Are circulating levels of molecular forms of AMH throughout pregnancy still higher in women with PCOS than in controls? Summary answer AMH and its molecular forms decrease significantly throughout pregnancy, remaining higher in women with PCOS, without modification in the proportion of AMH molecular forms. What is known already AMH injection in late gestational mice causes a “PCOS” phenotype in the offspring’s, possibly via a gestational hyperandrogenism. AMH could therefore play a role in the heritability of PCOS in women. Circulating AMH exists in different molecular forms, cleaved (active) and uncleaved. In normal women, serum AMH decreases during pregnancy. In women with PCOS, cross-sectional or partial longitudinal studies have shown higher AMH levels in the second trimester of pregnancy and at the time of delivery in women with PCOS compared to controls. To date, no longitudinal dynamic monitoring of AMH throughout pregnancy in women with PCOS has been performed. Study design, size, duration Systematic prospective quarterly longitudinal monocentric comparative follow up of 30 women with PCOS and 29 controls before and during pregnancy from April 2019 to September 2021. To demonstrate a high effect size with 90% power and 5% first-species risk, it was necessary to include 23 subjects per arm. With a calculated loss of follow-up rate of 20%, 29 patients must be recruited per group. Participants/materials, setting, methods Women aged 18 to 43y, with a pre-conception dosage of AMH, were included during the first trimester of a singleton pregnancy. PCOS group was defined according to the modified Rotterdam criteria. In the control group, the patients had normal ovarian reserve. During the first, second and third trimester of pregnancy, all women were tested for serum AMH and its molecular forms with a Beckman automatic analyser, and for serum estradiol, LH and androgen levels. Main results and the role of chance Before pregnancy, patients with PCOS had higher levels of AMH (58.9 vs. 18.3pmol/L; p = Limitations, reasons for caution Only free testosterone is active, and only TT was measured. Additional SHBG and albumin measurements are needed. Our control population is a population of infertile women who have no ovarian problem but who have undergone, for most of them, ART treatments to obtain pregnancy with a possible impact on implantation. Wider implications of the findings Elevated serum AMH level could be instrumental in the modification of the in-utero androgenic environment that is believed to promote the transmission of the syndrome. The impact of AMH could be different depending on the clinical or metabolic phenotype of the patients and needs to be explored further. Trial registration number NCT03483792

Abstract

Resumen Desde su descripcion inicial por Stein y Leventhal en 1935, el sindrome de ovarios poliquisticos (SOP) ha visto evolucionar su definicion. Desde 2003, tras la conferencia de consenso de Rotterdam, la definicion se ha basado en la presencia de criterios clinicos, de laboratorio y ecograficos, tras descartar otras etiologias que pudieran producir cuadros clinicos similares. La fisiopatologia de este sindrome sigue siendo objeto de mucha controversia en la comunidad cientifica, pero la mayoria de los especialistas parecen estar de acuerdo en que el hiperandrogenismo de origen ovarico es el elemento «fundador». El tratamiento, tanto en lo que respecta al hiperandrogenismo como a la infertilidad y los trastornos del ciclo, esta cada vez mejor codificado. La frecuente asociacion de la resistencia a la insulina con este sindrome deberia llevar a su deteccion ante el diagnostico de SOP. En efecto, en este caso, las medidas higienico-dieteticas y el uso de agentes sensibilizantes a la insulina, cuyo lugar queda por definir, parecen potenciar la accion de los diversos tratamientos.

La prevalence de l’obesite augmente massivement depuis plusieurs dizaines d’annees dans les pays industrialises. Les femmes obeses ont une activite sexuelle, non differente des femmes de poids normal, et beneficieraient d’une couverture contraceptive moindre et d’un risque de grossesse non desiree plus eleve. En outre, l’obesite constitue un facteur de risque important d’accident thromboembolique veineux et arteriel (infarctus du myocarde et infarctus cerebral). L’ensemble de ces facteurs est donc a integrer dans la strategie de choix d’une methode contraceptive chez une femme obese. L’objectif de cette revue est de reprendre de maniere exhaustive toutes les methodes contraceptives disponibles en France en 2017 et d’evaluer la balance benefices/risques de chacune d’entre elles chez les femmes obeses.

Abstract

Cyproterone acetate is actually the first-line anti-androgenic treatment for severe hyperandrogenism in women. However, as this treatment is currently controversial, the objective of the study was to evaluate the safety and efficacy of spironolactone as a relay for cyproterone acetate.This is a monocentric retrospective study conducted between December 2002 and January 2018 at the Jeanne de Flandre Hospital at the University Hospital Center of Lille. Patients with clinical hyperandrogenism who received treatment with cyproterone acetate and then spironolactone were included. A total of 37 patients were clinically and biologically evaluated before treatment, followed by cyproterone acetate and spironolactone.Clinically, the vast majority of patients were satisfied with the relay by spironolactone and found no difference between the two treatments. Biologically, testosterone and delta-4 androstenedione levels were significantly decreased with cyproterone acetate and spironolactone compared to no treatment. No significant differences were found when comparing levels under cyproterone acetate and under spironolactone. In addition, 87,5% of patients were free of side effects.The data collected show the clinical and biological efficacy of spironolactone as a relay for cyproterone acetate in the treatment of hyperandrogenism. This anti-androgen therefore appears as an effective and well-tolerated alternative, as a relay for cyproterone acetate in patients with hyperandrogenism.

Abstract

The main objective of this study was to describe the ovulation rate in patients with polycystic ovary syndrome, treated with ovulation induction/intra-uterine insemination and follitropin alfa by gonadotrophins at a second attempt.An observational, national and multicentre study was carried out: 51 French physicians (endocrinologists, gynaecologists) participated. Eligible patients were followed according to the usual clinical practices. The primary endpoint was the number of ovulations (spontaneous or triggered). Quality of life evaluation (by FertiQoL), compliance, and patient satisfaction were secondary endpoints.A total of 202 patients (mean age: 29.9 years; mean infertility: 2.9 years) were included: 78.4% met the Rotterdam definition. The ovulation rate was 93.3% (95% confidence interval [89.8; 96.8]%). At 12 weeks of gestation, 38 patients had an ongoing pregnancy. A difference of 10 points of the mean total FertiQoL score was observed between the two attempts. No patient reported missing injection. More than 9 in 10 patients said they were satisfied to very satisfied with the use of the pen injector for administration of follitropin alfa. Eight patients (4.0%) had hyperstimulation leading to cycle cancellation, and two patients (1.1%) reported ovarian hyperstimulation syndrome.At the second cycle of follitropin alfa stimulation, a high rate of ovulations, satisfactory compliance and tolerance profile associated with a change in quality of life were reported.

Abstract

In women, the anti-Mullerian hormone (AMH) is secreted by the granulosa cells of growing follicles. Its measurement is strongly correlated with antral follicle count and represents a reliable marker of ovarian reserve. It also has the advantage of being highly reproducible since it has little variation within and between cycles. However, although it seems to be a good quantitative reflection of the ovarian reserve, it does not assess the oocyte quality. This drawback precludes any good prediction of female fertility in the general population. However, the AMH assay can become an indirect marker of female fertility in some situations at risk for premature ovarian failure or in the polycystic ovary syndrome. Its interest is no more to be proven in assisted reproductive technology where it is a valuable aid to the choice of the proposed techniques, ovarian stimulation protocols and gonadotropin doses. AMH is finally very informative in monitoring cancer patients having received ovariotoxic drugs or having undergone mutilating ovarian surgeries. In conclusion, although it cannot be considered itself as a reliable predictor of pregnancy in women, AMH is now a must in the management and treatment of female infertility.

Abstract

This Consensus Opinion summarizes the main aspects of several techniques for performing ovarian antral follicle count (AFC), proposes a standardized report and provides recommendations for future research. AFC should be performed using a transvaginal ultrasound (US) probe with frequency ≥ 7 MHz. For training, we suggest a minimum of 20-40 supervised examinations. The operator should be able to adjust the machine settings in order to achieve the best contrast between follicular fluid and ovarian stroma. AFC may be evaluated using real-time two-dimensional (2D) US, stored 2D-US cine-loops and stored three-dimensional (3D) US datasets. Real-time 2D-US has the advantage of permitting additional maneuvers to determine whether an anechoic structure is a follicle, but may require a longer scanning time, particularly when there is a large number of follicles, resulting in more discomfort to the patient. 2D-US cine-loops have the advantages of reduced scanning time and the possibility for other observers to perform the count. The 3D-US technique requires US machines with 3D capability and the operators to receive additional training for acquisition/analysis, but has the same advantages as cine-loop and also allows application of different imaging techniques, such as volume contrast imaging, inversion mode and semi-automated techniques such as sonography-based automated volume calculation. In this Consensus Opinion, we make certain recommendations based on the available evidence. However, there is no strong evidence that any one method is better than another; the operator should choose the best method for counting ovarian follicles based on availability of resources and on their own preference and skill. Abstract

Serum AMH measurement became a key element in clinical practice, especially before using Assisted reproductive techniques (ART). However, many AMH kits exist giving different AMH results, leading to a confusion in their interpretation. Until recently, only manual ELISA kits existed (mainly Gen II Beckman, EIA/AMH Immunotech and two Anshlab kits) reporting non-interchangeable results. High and low AMH cut-off values, mainly useful to adapt therapeutics in ART, were different between kits. Since the end of 2014, the arrival of two automatic assays (Access Dxi Beckman and AMH Elecsys Roche) seems to improve the sensitivity and the reproducibility of AMH measurement. It could simplify the interpretation of AMH values and improve our clinical choices. This review synthetizes the main comparisons between the different AMH kits available in 2017 to help clinicians in their daily clinical practice.

Abstract

Data on the incidence and severity of ovarian hyperstimulation syndrome (OHSS) in France are limited.Prospective observational multicentric study (23 French centers of IVF) in a cohort of 421 women treated with highly purified hMG (HP-hMG) for the first or second cycle of IVF with or without ICSI. The primary objective was to assess the incidence of moderate to severe OHSS in this cohort.At inclusion, 172 patients (40.9%) were considered at risk of OHSS by the physicians. The main measures for risk minimization taken by the physicians rested on initial dose of HP-hMG and protocol choice. At the end of the follow-up (4 months in average), the rate of OHSS moderate to severe was 2.4% (confidence interval 95%: 1.1-4.3%) for the studied IVF cycle. OHSS was severe for 3 women (0.7%) and moderate for 7 women (1.7%).This rate of OHSS after IVF is at the lower limit of the rates reported in the literature for OHSS. This study brings reassuring epidemiological data on the rate of OHSS in women at risk. The measures taken by the physicians to minimize the risk of OHSS could have contributed to this low incidence.

Abstract

La infertilidad femenina de origen endocrino es la primera causa de infertilidad de la mujer en los paises desarrollados. Entre las afecciones predominantes ocupa el primer lugar el sindrome de los ovarios poliquisticos, que afecta al 5-10% de las mujeres en edad de procrear. Por la disovulacion frecuentemente asociada a este sindrome, la fertilidad se encuentra disminuida. Para remediar esta disovulacion pueden aplicarse medidas terapeuticas simples; la primera de ellas es un tratamiento metabolico en caso de sobrepeso. La otra afeccion frecuentemente causal es la hiperprolactinemia, cuyo tratamiento esta bien codificado con los agonistas dopaminergicos. Las amenorreas hipotalamicas funcionales, en relacion con una restriccion alimentaria, y los demas casos infrecuentes de hipogonadismo hipogonadotropo pueden tratarse mediante la administracion con bomba pulsatil de hormona liberadora de gonadotropinas (GnRH). En cambio, ante el diagnostico de insuficiencia ovarica prematura, que afecta a alrededor del 1% de las mujeres, el pronostico sobre la fertilidad es sombrio. La unica tecnica que aumenta las posibilidades de embarazo es la donacion de ovocito, compleja desde los puntos de vista etico, humano y tecnico. Descubrimientos recientes de nuevas moleculas han permitido comprender mejor la fisiopatologia de la amenorrea hipotalamica funcional y la espaciamiento de las pulsaciones secretoras de la GnRH. Asi mismo, la demostracion de nuevas mutaciones geneticas ha reducido la proporcion de casos de hipogonadismos hipogonadotropos congenitos idiopaticos y ha ayudado a explicar algunas situaciones de insuficiencia ovarica prematura. Por ultimo, el diagnostico de infertilidad de origen endocrino no exime de efectuar una valoracion completa, ya que otra causa de infertilidad (obstruccion tubarica, alteraciones espermaticas, etc.) puede modificar radicalmente la conducta practica.

Abstract

Introduction: Weight loss amongst women with polycystic ovary syndrome (PCOS) is crucial to reduce the risk of endocrine, reproductive and metabolic complications including hirsutism, menstrual disturbances and cardiovascular disease. With approximately 50% of women with PCOS being overweight or obese, effective dietary management of weight in PCOS is essential. However, there is inconsistent evidence as to whether specifically modified diets (e.g. reduced carbohydrate diets) are more effective at achieving weight loss amongst women with PCOS than are conventional healthy hypocaloric diets. Material and Methods: A systematic review and meta-analysis of randomized controlled trials that had compared weight and BMI between women with PCOS who had undergone either a specifically modified diet or a conventional healthy hypocaloric diet were performed. Six electronic databases were searched, a manual search of the reference lists of the included studies was carried out and authors were contacted for additional information. Nine studies with a total of 395 participants (all with a body mass index [BMI] ≥30) were included in the meta-analysis. The effect size used was the mean difference in post-intervention weight and BMI between participants who had undergone a specifically modified diet and participants who had undergone a conventional healthy hypocaloric diet. Results: There were no differences between groups in post-intervention weight (mean difference 1.26, 95% confidence interval (CI) -0.92 to 3.43, p = .26; heterogeneity I2 = 50%, p = .04) or BMI (mean difference 0.15, 95% CI -0.93 to 1.23, p = 0.79; heterogeneity I2 = 44%, p = 0.10). Subgroup analyses according to the presence of a dietary run-in period (a period at the start of the study during which all participants are placed on an identical diet in order to equalize them on variables influenced by diet), intervention duration and type of diet and a sensitivity analysis according to study quality were not significant. Conclusions: Whilst the results should be interpreted in light of the moderate heterogeneity observed, they suggest that specifically modified diets offer no added benefit for weight loss in women with PCOS over conventional healthy hypocaloric diets. The findings of this meta-analysis may promote the unification of guidelines for the dietary management of PCOS and allow clinicians to be confident in prescribing conventional healthy hypocaloric diets for weight loss amongst their PCOS patients.

21. Hiperandrogenismo en la adolescente

Abstract

La fisiopatologia del hiperandrogenismo, que se manifiesta desde el punto de vista clinico sobre todo por la presencia de hirsutismo, acne y/o alteracion menstrual, se debe esencialmente a la hiperproduccion ovarica o suprarrenal de androgenos. Su caracter patologico resulta dificil de establecer en el periodo peripuberal debido a la existencia de inmadurez fisiologica del eje hipotalamo-hipofisario. Su etiologia mas frecuente es el sindrome de ovarios polimicroquisticos (SOPMQ). Su diagnostico en la mujer adulta se basa en criterios precisos que requieren su adaptacion al caso particular de la adolescente. La evaluacion clinica de la rapidez de instalacion y de la intensidad de los signos de hiperandrogenismo asi como el estudio bioquimico sistematico de inicio permiten descartar las demas causas, particularmente los tumores ovaricos y suprarrenales, el deficit de 21-hidroxilasa, las endocrinopatias y las causas iatrogenicas. Dejando aparte el tratamiento etiologico, la actuacion terapeutica consiste en la administracion de antiandrogenos, principalmente acetato de ciproterona. En caso de SOPMQ, cuando se asocian alteraciones metabolicas, la educacion sanitaria respecto a las normas higienicodieteticas resulta fundamental.

Abstract

Resume Introduction Les troubles du comportement des patients ayant une maladie d’Alzheimer ou une affection apparentee sont la principale cause d’epuisement de leur environnement et conduisent frequemment a la decision d’institutionnalisation en unite de soins de longue duree. Ces troubles comportementaux sont plus frequents chez l’homme que chez la femme. Or, avec l’augmentation de l’esperance de vie des hommes, le nombre d’hommes ayant une demence de type Alzheimer ou apparentee va s’accroitre de facon exponentielle. Patients et methodes L’acetate de cyproterone, progestatif anti-androgene, fut propose dans un contexte d’impasse therapeutique a 19 patients dements ayant des troubles comportementaux importants ou majeurs, maintenus le plus souvent en unite de soins de longue duree geriatrique. L’evaluation clinique et comportementale reposait sur l’echelle d’independance fonctionnelle et sur l’echelle d’evaluation des troubles comportementaux en institution de Cohen-Mansfield, avant traitement et apres stabilisation sous traitement. Sept patients etaient atteints d’une maladie d’Alzheimer, sept d’une demence vasculaire, deux d’une degenerescence fronto-temporale, deux d’une maladie d’Huntington et un d’une maladie a corps de Lewy probable. Quinze patients avaient des troubles comportementaux de type agressif ou impulsif et quatre patients des comportements moteurs aberrants predominants associes a une agressivite moderee. Resultats L’acetate de cyproterone, a la dose moyenne de 92,5 mg/j (50 a 100 mg/j) ameliora de facon tres significative le comportement agressif et/ou impulsif des patients ayant une maladie d’Alzheimer ou une demence vasculaire. Il n’eut aucun effet sur les comportements moteurs aberrants. L’efficacite sur l’agressivite et l’impulsivite s’est maintenue durant la phase de traitement et l’agressivite reapparut a l’arret du traitement, plus rapidement chez les patients ayant une demence vasculaire. Conclusion L’acetate de cyproterone est donc une alternative interessante aux psychotropes sedatifs chez le dement agressif. Cependant, avant d’y avoir recours, il est indispensable de mener une analyse clinique fine du trouble comportemental. Les resultats de cette etude preliminaire justifient la realisation d’une etude avec tirage au sort en double insu, afin de confirmer l’interet de ce traitement. Le mode d’action central de l’acetate de cyproterone reste a preciser : effets cumules du blocage de la synthese des androgenes et des recepteurs centraux aux androgenes, effet sedatif de type progestatif, possible fixation sur les recepteurs aux opioides.

Abstract

Resume Le syndrome des ovaires polykystiques est le dereglement ovarien le plus frequent chez la femme avant la menopause. Il associe en general une hyperandrogenie et une anovulation chronique a une hyperinsulinemie, une insulino-resistance et une dyslipidemie. L’hypothese physiopathologique du syndrome des ovaires polykystiques semble etre une anomalie intrinseque a la steroidogenese au niveau des cellules thecales, influencee par l’environnement, l’insuline, le surpoids. Il s’agirait d’une maladie multigenique et environnementale. La connaissance des genes impliques dans le syndrome des ovaires polykystiques permettrait de proposer de nouvelles strategies therapeutiques et une eventuelle prevention chez les femmes predisposees. Les genes des gonadotrophines, de la steroidogenese et de l’insulino-resistance ne semblent pas etre directement impliques sauf peut-etre le CYP 11a. Par contre, l’etude des facteurs de transcription de ces genes constitue une piste interessante qui pourrait expliquer egalement les autres manifestations cliniques du syndrome des ovaires polykystiques (arret de la maturation folliculaire, insulino-resistance, obesite, cancer de l’endometre…).

Resume Le syndrome des ovaires polykystiques (SOPK) est le dereglement ovarien le plus frequent chez la femme avant la menopause. Il associe en general une hyperandrogenie et une anovulation chronique a une hyperinsulinemie, une insulinoresistance et une dyslipidemie. L'hypothese physiopathologique du SOPK semble etre une anomalie intrinseque des cellules thecales sur la steroidogenese, influencee par l'environnement, l'insuline, le surpoids. Il s'agirait d'une maladie multigenique et environnementale. La connaissance des genes du SOPK permettrait de proposer de nouvelles strategies therapeutiques et une eventuelle prevention chez les femmes predisposees. Les genes des gonadotrophines, de la steroidogenese et de l'insulinoresistance ne semblent pas etre directement impliques sauf peut-etre le CYP 11a. Par contre, l'etude des facteurs de transcription de ces genes constitue une piste interessante qui pourrait expliquer egalement les autres manifestations cliniques du SOPK (arret de la maturation folliculaire, insulinoresistance, obesite, cancer de l'endometre, etc.).

Abstract

In Assisted Reproductive Technologies (ART), impaired ovarian reserve represents a therapeutic challenge. The Anti-Mullerian Hormone (AMH) serum level would be a good marker of ovarian reserve and a predictor of response to stimulation. The objective of this study is to assess into a population of infertile couples where the woman has at least one patent tube and where the man has sperm parameters compatible with insemination, whether AMH level less than 12pmol/L can be used to establish a strategy supporting the couple's infertility by comparing their chances of pregnancy after Intra-uterine insemination (IUI) or in vitro fertilization (IVF).This single-center retrospective study of 1012 patients over 28months compared the pregnancy rates of 2011 ART attempts (1385 IUI and 626 IVF, ICSI excluded) according to the value of serum AMH, either reduced if≤12pmol/L or non-reduced if greater.In IVF, a low AMH reduced pregnancy rate (18.4% vs. 32.9% in the normal AMH group, P0.0001). Conversely, the AMH value did not influence the success in IUI cycles (14.2% vs. 14.5%, respectively, NS). In cases with low AMH, the pregnancy rate per initiated cycle in IVF (18.4%) was not significantly greater than in IUI cycles (14.2%). Converting an IVF attempt in IUI did not impair the pregnancy rate (13.5% vs. 14.5% after immediate IUI, NS).When the serum AMH level is less than 12pmol/L, IUI may be an interesting option in case of IVF failure. However, its place remains to be defined: converting IVF in IUI, IUI in relay of failed IVF, or even as first line therapy when the chances with IVF appear to be minimal.

Abstract

Steroid 21-hydroxylase deficiency is the most common adrenal genetic disease and is also named congenital adrenal hyperplasia. Depending on the severity of CYP21A2 gene mutations, there are severe or "classical" forms and moderate or "nonclassical" forms of 21-hydroxylase deficiency. The enzyme deficiency causes a disruption of adrenal steroidogenesis, which induces hyperandrogenism and elevated plasma levels of progesterone and 17-hydroxyprogesterone, the two substrates of 21-hydroxylase. These endocrine abnormalities will disrupt gonadal axis, endometrial growth and maturation and finally secretion of cervical mucus. All these phenomena contribute to a female hypofertility. Infertility is more severe in classical forms. When to become pregnant, treatment with hydrocortisone or dexamethasone can limit the production of adrenal androgens and progesterone and improves spontaneous pregnancy rates while minimizing the risk of miscarriage, which is usually relatively high in this disease. When planning pregnancy in patients with a 21-hydroxylase deficiency, genotyping the partner is required to screen for heterozygozity (1/50) and to assess the risk of transmission of a classical form in the progeny.

Abstract

Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in women of reproductive age and the leading cause of female infertility. This condition is frequently associated with significant metabolic disorders, including obesity and hyperinsulinemia. Therefore, it seems essential to focus on the pregnancy of these patients and possible obstetric complications. Many studies suggest an increase in the risk of obstetric pathology: early miscarriage, gestational hypertension, preeclampsia, gestational diabetes mellitus diagnosed during early pregnancy, prematurity, low birthweight or macrosomia, neonatal complications and cesarean sections. However, it is difficult to conclude clearly about it, because of the heterogeneity of definition of PCOS in different studies. In addition, many confounding factors inherent in PCOS including obesity are not always taken into account and generate a problem of interpretation. However it seems possible to conclude that PCOS does not increase the risk of placental abruption, HELLP syndrome, liver disease, postpartum hemorrhage, late miscarriage and stillbirth.

Abstract

To evaluate fertiloscopy's results and to redefine its place in the management of female infertility.Retrospective study including 262 cases of fertiloscopy and 260 patients with primary or secondary idiopathic infertility. Analysis of infertility assessment's findings (hysterosalpingography), perioperative data (operating technique, laparoscopic conversion, failures and complications). Comparison of hysterosalpingography's findings and peroperative data; comparison of fertiloscopy and laparoscopy's findings.Access to peritoneal cavity was possible for 248 fertiloscopies (95%), and pelvic exploration was considered as complete for 226 cases (86%). Laparoscopic conversion was necessary in 54 cases (21%) and indicated by surgical pathology in more of one third of the cases (n=20). Our failure rate was only 5,3% (n=14), partially thanks to posterior colpotomie (70% of failures avoided). We deplored 8 complications (3.05%) which were not severe (no bowel injury), among which half were linked with the hysteroscopy (uterus perforation). In the cases of laparoscopic conversion, laparoscopic findings confirmed per-fertiloscopic data, considering adhesions and tubal patency. Hysterosalpingography had poor sensibility and positive predictive value.Fertiloscopy is a safe, reproducible and not much invasive procedure. It can be substituted to laparoscopy in infertility assessment when there is no obvious surgical indication. Moreover, it could be considered as a first line way of investigation in female infertility management, instead of hysterosalpingography which has poor sensibility and positive predictive value.

Abstract

Le devenir du Syndrome de Klinefelter (SK) au dela de l'âge du depistage dans l'enfance ou lors de l'incorporation au Service Militaire est mal connu. 33 observations reunies dans des services divers chez des sujets de 20 a 65 ans servent de support a une revue de la litterature; celle-ci permet d'identifier deux groupes de donnees. Le premier groupe concerne des facteurs de risque indetermines car les obervations rapportees restent isolees (hemopathies, processus tumoraux). Les troubles de la regulation glucidique et un hyperinsulinisme paraissent neanmoins plus apparents. Le deuxieme groupe de faits correspond a un risque relatif plus significatif. On y retient le cancer du sein, l'osteoporose, les ulceres de jambes. Une pathologie dysimmunitaire pourrait etre un facteur determinant de plusieurs circonstances pathologiques. Les tumeurs testiculaires que l'on pouvait craindre sont en fait des evenements rares.

Abstract

Introduction et but de l’etude La prevalence de l’obesite feminine est en constante augmentation et peut se compliquer d’une diminution de la fertilite. Les centres d’assistance medicale a la procreation (AMP) sont de plus en plus souvent confrontes a la prise en charge de patientes obeses, alors qu’une perte de poids preliminaire (voire l’obtention d’un IMC Materiel et methodes 83 patientes, obeses, inclues entre le 1er septembre 2010 et le 30 septembre 2011, beneficiaient d’un suivi conjoint en consultation d’AMP et en consultation de nutrition. Le suivi nutritionnel reposait principalement sur une prise en charge educative nutritionnelle (individuelle ou en groupe, visant a modifier les comportements alimentaires et d’activite) associee ou non a d’autres strategies (Regime tres basses calories, therapie cognitivo-comportementale (TCC), traitement medicamenteux (metformine). L’efficacite de la prise en charge nutritionnelle a ete evaluee a partir des parametres anthropometriques ainsi que sur la survenue d’une grossesse. Les variables quantitatives ont ete comparees par un test de Student ou de Kruskal-wallis, selon les effectifs. Les variables qualitatives ont ete comparees selon un test de Chi-2 ou le test de Fisher Exact, selon les effectifs. La significativite a ete retenue pour une valeur de p inferieure a 0,05. Resultats et Analyse statistique L’IMC initial des patientes etait de 37,5 ± 5,24 kg/m2, et 53 % d’entre elles avaient un IMC > 35. La duree de la prise en charge nutritionnelle etait de 4,6 ± 6,9 mois. 50,6 % des patientes avaient beneficie d’une education nutritionnelle seule, en complement 15,7 % d’une TCC, 13,3 % d’un regime tres basses calories, 8,4 % d’un traitement par metformine, 8,4 % d’une prise en charge combinee et 3,6 % ont ete orientees vers une chirurgie bariatrique. 37,8 % des patientes ont obtenu une perte de poids superieure a 5 % et 27,2 % des patientes en obesite severe ont obtenu un IMC 35 (p = 0,07). Abstract

The polycystic ovary syndrome (PCOS) is the most frequent endocrine disease in women of reproductive age. Hyperandrogenism, anovulation and metabolic syndrome are the cardinal features of PCOS. Hyperandrogenism results from a diffuse enzymatic hyperactivity at the theca-interstitial cell level. Anovulation is due to an impairment of the selection of a dominant follicle, while the number of smaller follicles is exaggerated. The molecular grounds of insulin resistance could be an increased Serine phosphorylation of the insulin receptor. The clinical classification of PCOS distinguishes three forms: the classic PCOS, where the three above mentioned features are present, the non classic PCOS and the asymptomatic PCOS, revealed by ultrasonography. Only the increased ovarian volume or surface (11ml and5.5cm(2), respectively) must be viewed as a specific ultrasonic sign of PCOS. Cyproterone acetate remains the basic treatment of hyperandrogenism. The treatment of anovulation and infertility follows a consensual strategy. The insulin sensitizing treatment allows to decrease hyperandrogenism, to reverse the menstrual cycle irregularity and to obtain spontaneous or induced pregnancies.
