1. Role of electrogastrography in detecting motility disorders in children affected by chronic intestinal pseudo-obstruction and Crohn's disease

Des troubles de transit sont observes dans un grand nombre de situations pathologiques. Le but de ce travail etait d'etudier par electrogastrographie le transit gastrointestinal chez les enfants atteints de la maladie de Crohn ou d'une pseudo-occlusion intestinale chronique (POIC) a la recherche d'une anomalie de la motricite gastrointestinale. Patients et Methodes: L'etude portait sur 34 sujets: 20 temoins (M = 15, âge moyen 10 ± 3,5 ans), 8 patients atteints de la maladie Crohn en phase quiescente (M = 4, âge moyen 18 ± 7 ans), et 6 patients presentant une POIC (M = 6, âge moyen 10 ± 3,5 ans). Resultats: L'analyse de l'activite electrique gastrique a demontre une modification post-prandiale dans le groupe temoin representee par une augmentation du pourcentage de 3 cycles par minute (3CPM), de la puissance dominante de la periode (PDP) et de la frequence de la periode dominante (PDF) et par une reduction de la bradygastrie. Les patients atteints de la maladie de Crohn ou d'une POIC montraient une variation anormale de 3CPM, de PDF, et de la bradygastrie post-prandiale. Neanmoins, les patients atteints de la maladie de Crohn montraient une difference significative de la PDP en post-prandiale compare aux temoins. Les patients ayant une POIC presentaient un profil different, soit de leur PDF preprandiale avec tachygastrie, soit de leur 3CDP post-prandial compares aux temoins. Conclusion: L'electrogastrographie est une methode non invasive pour etudier la motricite gastrique en rapport avec les alterations de l'activite electrique gastrique observees dans la pseudo-occlusion intestinale chronique et la maladie de Crohn.
