One of the most common complications in breast conserving surgery is seroma formation. The origin of seroma formation remains unclear. While intraoperative radiotherapy (IORT) has been shown to be an alternative to whole breast irradiation, the influence on seroma production is unclear. Therefore, this analysis compares seroma production in patients with breast conserving surgery with or without IORT as tumour bed boost during breast conserving surgery. A retrospective analysis of seroma production in patients with nodal-negative (pN0sn) pT1/2 primary breast cancer treated between September 2010 and October 2013 at the Breast Cancer Centre, University Hospital Ulm was performed. Patients with neoadjuvant chemotherapy, previous breast/axillary surgery or more than one intervention were excluded. IORT was applied as a tumour bed boost with 50-kV X-rays (Intra beam®) delivering 9 Gy at the applicator surface. Seroma formation was measured using wound drains placed in breast and in axilla. Data of 152 patients (99 −IORT; 53 +IORT) were available for analysis. No significant differences between patients with or without IORT with regard to seroma production and number of days until drain removal were found (all p > 0.05). Patients with IORT encountered no increased seroma production and removal of the drains was not delayed compared to patients with breast conserving surgery only. Our results indicate that IORT does not increase the seroma production compared to surgery alone.

Hintergrund Es gibt zahlreiche randomisierte Studien zum Management von Patienten mit lokalisiertem oder metastasiertem Prostatakarzinom, aber nur wenige, meist retrospektive Studien haben das Vorgehen bei nodal-positiven Patienten untersucht. Folglich ist die optimale Therapie in dieser Situation unklar. Aktuelle Leitlinien empfehlen eine alleinige Androgendeprivationstherapie (ADT) oder eine Strahlentherapie plus eine ADT.

5. Die perkutane Strahlentherapie als primäre Option der Therapie des lokal begrenzten Prostatakarzinoms

Zur Behandlung des lokal begrenzten Prostatakarzinoms (PCa) werden die radikale Prostatektomie (RP), perkutane Strahlentherapie (RT), die permanente Seed-Implantation (PSI) oder die Active Surveillance (AS) als kurative Therapie eingesetzt. Unter Berucksichtigung der aktuellen deutschen S3-Leitlinie und der EAU-Guideline fassen wir das stadienabhangige Prozedere aus der Sicht der Strahlentherapie zusammen. Beim lokal begrenzten niedrig-Risisko PCa ist bisher nicht geklart, welche Behandlung die besten Resultate erbringt, wenn Heilungschance, Nebenwirkungsrisiko und Patientenwunsche berucksichtigt werden. Fur diese Patientengruppe werden in der PREFERE Studie die RP, RT, PSI und AS vergleichend untersucht. Beim PCa mit intermediarem oder mit hohem Risiko-Profil sind die kombinierte RT+hormonelle Behandlung (HT) oder die RP indiziert. Mit der fruhen Salvage-RT, bei einem PSA unter 0,5 ng/ml, besteht nach post-RP biochemischem Rezidiv eine zweite Heilungschance.

Patients with metastatic and locally advanced bladder or prostate cancer may suffer from pelvic symptoms such as pain, obstruction, and hemorrhage. Local tumor growth is associated with significant morbidity and systemic therapy is often ineffective. Local therapies such as bladder irrigation, transurethral resection of the prostate, and fulguration of bleeding vessels provide relief but often require repeated treatments.The aim of this work was to review the current status of palliative pelvic radiotherapy for metastatic bladder and prostate cancer.The available literature was evaluated and treatment recommendations are proposed depending on different clinical scenarios.To date, no standard regimen exists for the delivery of palliative pelvic radiotherapy. Various radiotherapy schedules manage successful and long-term palliation of pelvic symptoms in most patients and result in acceptable toxicity. For bladder cancer, the most common dose and fractionation regimens range from 20 Gy in 5 fractions to 40 Gy in 20 fractions. Some retrospective studies evaluated 6 weekly fractions of 6 Gy to a total dose of 36 Gy. For prostate cancer, the most common dose and fractionation regimes range from 30 Gy in 10 fractions to 50 Gy in 25 fractions. The symptomatic response rate is between 70 and 95%.Pelvic radiotherapy for patients with metastatic and locally advanced bladder or prostate cancer provides effective and long-term palliation of a variety of symptoms such as pain, obstruction, and hemorrhage, with acceptable toxicity. Future studies should investigate the optimal target dose and fractionation schedule.

Standard der postoperativen Strahlentherapie des brusterhaltend operierten Mammakarzinoms ist die Bestrahlung der gesamten Restbrust mit einer anschliesenden Aufsattigung des Tumorbetts (sog. „boost“). Aufgrund der haufigeren Diagnose kleinerer Mammakarzinome bei postmenopausalen Patientinnen durch das Mammographie-Screening ergibt sich die Notwendigkeit, uber mogliche Veranderungen der bisherigen Therapiealgorithmen nachzudenken. Der Grosteil der Lokalrezidive tritt im Bereich der ehemaligen Tumorhohle auf. Infolgedessen wird aktuell diskutiert, ob eine alleinige Bestrahlung des Tumorbetts die Radiotherapie der gesamten Restbrust ersetzen kann.

Das Skelettsystem ist als Metastasierungsort beim fortgeschrittenen Mammakarzinom am haufigsten betroffen. Die wichtigsten physiologischen Aufgaben des Knochens sind seine Stutzfunktion und die Kalziumspeicherung. Am Knochenstoffwechsel sind vornehmlich zwei Zellpopulationen mit gegenlaufiger Funktion beteiligt, Osteoblasten und Osteoklasten. Wird deren Gleichgewicht gestort, beispielsweise durch die Invasion von Tumorzellen und Freisetzung osteolytischer Faktoren, kommt es in einer Art Circulus vitiosus zur Destruktion des Knochens. Daraus ergeben sich die unter dem Begriff „skeletal related events“ (SRE) zusammengefassten Komplikationen. Zur Diagnostik von Knochenmetastasen stehen neben der Anamnese verschiedene Verfahren der Bildgebung zur Verfugung (Skelettszintigraphie, Rontgen, Computer- und Magnetresonanztomographie). Die Therapie ist interdisziplinar zu entscheiden, sie richtet sich vor allem nach dem Benefit fur die Patientin. Im Vordergrund stehen dabei Schmerzreduktion und Funktionserhalt. Neben der Radiatio und der operativen Intervention zur lokalen Metastasenkontrolle sind Systemtherapien mit Bisphosphonaten oder einem RANKL-Inhibitor indiziert.

There are numerous randomized trials to guide the management of patients with localized (and metastatic) prostate cancer, but only a few (mostly retrospective) studies have specifically addressed node-positive patients. Therefore, there is uncertainty regarding optimal treatment in this situation. Current guidelines recommend long-term androgen deprivation therapy (ADT) alone or radiotherapy plus long-term ADT as treatment options.This overview summarizes the existing literature on the use of radiotherapy for node-positive prostate cancer as definitive treatment and as adjuvant or salvage therapy after radical prostatectomy. In this context, we also discuss several PET tracers in the imaging evaluation of patients with biochemical recurrence of prostate cancer after radical prostatectomy. As for definitive treatment, retrospective studies suggest that ADT plus radiotherapy improves overall survival compared with ADT alone. These studies also consistently demonstrated that many patients with node-positive prostate cancer can achieve long-term survival - and are likely curable - with aggressive therapy.The beneficial impact of adjuvant radiotherapy on survival in patients with pN1 prostate cancer seems to be highly influenced by tumor characteristics. Men with ≤ 2 positive lymph nodes in the presence of intermediate- to high-grade disease, or positive margins, and those with 3 or 4 positive lymph nodes are the ideal candidates for adjuvant radiotherapy (plus long-term ADT) after surgery.There is a need for randomized trials to further examine the potential role of radiotherapy as either definitive or adjuvant treatment, for patients with node-positive prostate cancer.

Biochemical recurrence after radical prostatectomy (RP) is associated with risk indicators, including Gleason score, preoperative PSA level, tumor stage, seminal vesicle invasion, and positive surgical margins. The 5-year biochemical progression rate among predisposed patients is as high as 50–70%. Post-RP treatment options include adjuvant radiotherapy (ART, for men with undetectable PSA) or salvage radiotherapy (SRT, for PSA persisting or re-rising above detection threshold). Presently, there are no published randomized trials evaluating ART vs. SRT directly. Published data on ART and SRT were reviewed to allow a comparison of the two treatment approaches. Three randomized phase III trials demonstrated an almost 20% absolute benefit for biochemical progression-free survival after ART (60–64 Gy) compared to a “wait and see” policy. The greatest benefit was achieved in patients with positive margins and pT3 tumors. SRT can be offered to patients with elevated PSA after RP. In 30–70% of SRT patients, PSA will decrease to an undetectable level, thus giving a second curative chance. The rate of side effects for both treatments is comparably low. The role of irradiation of pelvic lymph nodes and the additional use of hormone therapy and radiation dose are discussed. It remains unclear whether early SRT initiated after PSA failure is equivalent to ART. Where SRT is indicated, it should be started as early as possible.

In dieser Ubersicht werden die Moglichkeiten der palliativen Strahlentherapie beim metastasierten Nierenzellkarzinom dargestellt. Wahrend sich uber viele Jahre die Tatsache erhartet hat, dass ein adjuvanter, neoadjuvanter oder gar definitver Einsatz beim Primartumor keinen Benefit bringt, ist die Strahlentherapie eine wichtige Stutze in der Palliation der disseminierten Erkrankung. Lokale Schmerzen bei Knochenmetastasen konnen zu uber 70% gemildert werden, nahezu unabhangig von der Fraktionierung, d. h. fur den palliativen Fall konzentriert sich die Therapie unter Umstanden auf nur eine Sitzung. Die zweite wichtige Indikation ergibt sich bei der Behandlung von Hirnmetastasen, sei es als Ganzhirnbestrahlung nach Operation und bei multiplen Hirnmetastasen oder als stereotaktische Einzeitbestrahlung (Radiochirurgie) bei bis zu 3 synchronen Hirnmetastasen mit lokalen Kontrollraten von uber 90%, die bei neurochirurgischen Eingriffen kaum erreicht werden.

Hierfur wurden strategische Umentwicklungen begonnen, die dem Patienten eine moderne, optimale Versorgung ermoglichen sollen. Bisher bestehende Abgrenzungen der klinischen Facher losen sich ebenfalls teilweise methodenabhangig auf. Fur Patienten mit gastrointestinalen Tumoren wird heute zunehmend die stationare Versorgung unter Integration der niedergelassenen betreuenden Arzte in viszeralmedizinischen Organzentren durchgefuhrt. Interdisziplinaritat soll der Schlussel zum Erfolg sein. Am Beispiel der gastrointestinalen Tumoren wird auf spezielle Aspekte v. a. der kolorektalen Karzinome eingegangen und ein Ausblick insbesondere fur die universitare Viszeralmedizin skizziert.

Unklarheit besteht bezuglich des Stellenwertes des operativen Lymphknotenstagings und der adjuvanten Strahlentherapie beim fruhen Endometriumkarzinom. Der zunehmende Einsatz der pelvinen und paraaortalen Lymphadenektomie sowie der nicht nachweisbare Uberlebensvorteil durch die adjuvante Strahlentherapie nahren die Diskussion daruber, wie Patientinnen mit einem Endometriumkarzinom im Stadium IB behandelt werden sollten. Derzeit ist das Risiko fur ein pelvines Rezidiv nach Endometriumkarzinom im Stadium IB G1 und 2 als so gering einzustufen, dass eine generelle Beckenbestrahlung nicht zu rechtfertigen ist. Dennoch sollten Patientinnen ≥60 Jahre und solche mit einer Lymphangioinvasion eine Radiatio des Scheidenstumpfes im Sinne einer vaginalen Einlage erhalten. Im Stadium IB G3 sollte die pelvine und paraaortale Lymphadenektomie durchgefuhrt werden, die Beckenbestrahlung als alternative Therapieoption stellt lediglich die zweite Wahl dar. Da die meisten Patientinnen mit einem Karzinom im Stadium IB eher an Begleiterkrankungen als am Tumorleiden versterben, erscheint das Gesamtuberleben als alleiniges Entscheidungskriterium fur oder wider eine adjuvante Strahlentherapie nicht ideal. Sicher stellt die adjuvante Beckenbestrahlung fur viele Patientinnen eine Ubertherapie dar, dennoch darf der prognostische Wert eines unauffalligen Lymphknotenstagings nicht dahingehend uberinterpretiert werden, bei allen Patientinnen auf eine adjuvante Strahlentherapie verzichten zu konnen.

Salvage radiotherapy (SRT) is commonly used to treat patients with biochemical failure after radical prostatectomy (RP). Retrospective series have demonstrated biochemical response in approximately 60-75% of patients, but only a significantly lower rate of patients achieves a response with a decrease of the prostate-specific antigen (PSA) to a value below the limits of detectability. Therefore, long-term response at 10 years is only about 20-25% in all of these patients. The purpose of this study was to determine prognostic factors with impact on achieving the undetectable PSA range after SRT and to define the role of this end point.Between 1997 and 2004, 162 patients received SRT at the Charité Universitätsmedizin, Berlin. No patient had hormonal treatment before SRT and 90% of the patients (143) had a SRT dose of 66 Gy. We analyzed the impact of nine potential risk factors on achieving an undetectable PSA after RT and on biochemical relapse-free survival (bNED) after SRT.Median follow-up time was 41.5 months and median PSA pre-RT was 0.33 ng/mL. Calculated bNED for 3.5 years was 54%. A total of 60% of the patients achieved an undetectable PSA after SRT. Univariate analysis demonstrated statistically significant predictors of biochemical progression after SRT: Gleason score (p = 0.01), PSA pre-SRT (p = 0.031), tumor stage (p = 0.047), and persistent detectable PSA after RT (p0.00005). In multivariate analysis, margin status (p = 0.017) and PSA pre-SRT (p = 0.002) were significant predictors of an undetectable PSA after SRT. The most significant independent predictor of bNED was "PSA undetectable after RT" (p0.0005) with a hazard ratio of 8.4, thus leading to a calculated bNED at 3.5 years of 75% compared with only 18% for those patients, who did not achieve an undetectable PSA after SRT. The rate of severe Grade 3-4 side effects was below 2.5%.The study represents one of the largest retrospective single-institution series of SRT for increasing PSA after RP in patients without any hormonal treatment before the initiation of SRT. Our findings suggest that achieving an undetectable PSA after RT is an important prognosticator for a high chance of cure and patients with a low PSA pre-SRT, positive surgical margins, and low tumor stage at the time of RP are best candidates for SRT.

In a retrospective analysis, we examined factors influencing the outcome of prostate cancer (PCa) patients receiving salvage radiotherapy (SRT) for PSA recurrence after radical prostatectomy (RP).306 patients received 3D-conformal SRT at a median pre-SRT PSA of 0.298 ng/ml. Post-SRT progression was defined as PSA ⩾0.2 ng/ml above nadir and rising further, or hormone treatment, or clinical recurrence. Data were analyzed with the Kaplan-Meier method and multivariable Cox regression.Application of SRT at a PSA0.2 ng/ml correlated significantly with achieving a post-SRT PSA nadir0.1 ng/ml and with improved freedom from progression (median follow-up 7.2 years). The post-SRT nadir0.1 ng/ml correlated significantly with less recurrences and with better overall survival. In multivariable Cox analysis restricted to pre-SRT parameters, a pre-SRT PSA ⩾0.2 ng/ml had the strongest impact (hazard ratio 2.4) on progression. If the post-SRT PSA nadir was included in the model, then failing the nadir was the most important risk factor (hazard ratio 8.1).Early SRT at a PSA0.2 ng/ml is a favorable treatment option for post-RP biochemical recurrence. It correlated with a post-SRT PSA-nadir0.1 ng/ml which was associated with improved freedom from progression and overall survival.

Introduction. The anatomic position of the sentinel lymph node is variable. The purpose of the following study was to assess the dose distribution delivered to the surgically marked sentinel lymph node site by 3D conformal radio therapy technique.Material and Method. We retrospectively analysed 70 radiotherapy (RT) treatment plans of consecutive primary breast cancer patients with a successful, disease-free, sentinel lymph node resection.Results. In our case series the SN clip volume received a mean dose of 40.7 Gy (min 28.8 Gy/max 47.6 Gy).Conclusion. By using surgical clip markers in combination with 3D CT images our data supports the pathway of tumouricidal doses in the SN bed. The target volume should be defined by surgical clip markers and 3D CT images to give accurate dose estimations.
