The aims of our work were to determine normal aging rates for structural and functional arterial parameters in healthy children, adolescents, and young adults and to identify gender-related differences in these aging rates. Methods. 161 subjects (mean: 15 years (range: 4–28 years), 69 females) were studied. Subjects included had no congenital or chronic diseases, nor had they been previously exposed to traditional cardiovascular risk factors. Arterial parameters assessed were (1) central blood pressure (BP) and aortic pulse wave analysis, (2) arterial local (pressure-strain elastic modulus) and regional (pulse wave velocity, PWV) stiffness, and (3) arterial diameters and carotid intima-media thickness (CIMT). Simple linear regression models (age as the independent variable) were obtained for all the parameters and the resulting rates of change were compared between genders. Results. No gender-related differences were found in mean values of arterial structural and functional parameters in prepubertal ages (4–8 years), but they started to appear at ~15 years. Boys showed a greater rate of change for central systolic BP, central pulse pressure, CIMT, and carotid-femoral PWV. Conclusion. Gender-related differences in arterial characteristics of adults can be explained on the basis of different growing-related patterns between boys and girls, with no existing differences in prepubertal ages.

Nitrous oxide (N2O) is a long-lived potent greenhouse gas and stratospheric ozone-depleting substance that has been accumulating in the atmosphere since the preindustrial period. The mole fraction of atmospheric N2O has increased by nearly 25 % from 270 ppb (parts per billion) in 1750 to 336 ppb in 2022, with the fastest annual growth rate since 1980 of more than 1.3 ppb yr−1 in both 2020 and 2021. According to the Sixth Assessment Report of the Intergovernmental Panel on Climate Change (IPCC AR6), the relative contribution of N2O to the total enhanced effective radiative forcing of greenhouse gases was 6.4 % for 1750–2022. As a core component of our global greenhouse gas assessments coordinated by the Global Carbon Project (GCP), our global N2O budget incorporates both natural and anthropogenic sources and sinks and accounts for the interactions between nitrogen additions and the biogeochemical processes that control N2O emissions. We use bottom-up (BU: inventory, statistical extrapolation of flux measurements, and process-based land and ocean modeling) and top-down (TD: atmospheric measurement-based inversion) approaches. We provide a comprehensive quantification of global N2O sources and sinks in 21 natural and anthropogenic categories in 18 regions between 1980 and 2020. We estimate that total annual anthropogenic N2O emissions have increased 40 % (or 1.9 Tg N yr−1) in the past 4 decades (1980–2020). Direct agricultural emissions in 2020 (3.9 Tg N yr−1, best estimate) represent the large majority of anthropogenic emissions, followed by other direct anthropogenic sources, including fossil fuel and industry, waste and wastewater, and biomass burning (2.1 Tg N yr−1), and indirect anthropogenic sources (1.3 Tg N yr−1) . For the year 2020, our best estimate of total BU emissions for natural and anthropogenic sources was 18.5 (lower–upper bounds: 10.6–27.0) Tg N yr−1, close to our TD estimate of 17.0 (16.6–17.4) Tg N yr−1. For the 2010–2019 period, the annual BU decadal-average emissions for both natural and anthropogenic sources were 18.2 (10.6–25.9) Tg N yr−1 and TD emissions were 17.4 (15.8–19.20) Tg N yr−1. The once top emitter Europe has reduced its emissions by 31 % since the 1980s, while those of emerging economies have grown, making China the top emitter since the 2010s. The observed atmospheric N2O concentrations in recent years have exceeded projected levels under all scenarios in the Coupled Model Intercomparison Project Phase 6 (CMIP6), underscoring the importance of reducing anthropogenic N2O emissions. To evaluate mitigation efforts and contribute to the Global Stocktake of the United Nations Framework Convention on Climate Change, we propose the establishment of a global network for monitoring and modeling N2O from the surface through to the stratosphere. Microplastics (MPs) are recognized as a major environmental problem due to their ubiquitous presence in ecosystems and bioaccumulation in food chains. Not only humans are continuously exposed to these pollutants through ingestion and inhalation, but recent findings suggest they may trigger vascular inflammation and potentially worsen the clinical conditions of cardiovascular patients. Here we combine headspace analysis by needle trap microextraction-gas chromatography-mass spectrometry (HS-NTME-GC-MS) and biological assays to evaluate the effects of polystyrene, high- and low-density polyethylene MPs on phenotype, metabolic activity, and pro-inflammatory status of Vascular Smooth Muscle Cells (VSMCs) the most prominent cells in vascular walls. Virgin and artificially aged MPs (4 weeks at 40 °C and 750 W/m2 simulated solar irradiation) were comparatively tested at 1 mg/mL to simulate a realistic exposure scenario. Our results clearly show the activation of oxidative stress and inflammatory processes when VSMCs were cultured with aged polymers, with significant overexpression of IL-6 and TNF-α. In addition, volatile organic compounds (VOCs), including pentane, acrolein, propanal, and hexanal as the main components, were released by VSMCs into the headspace. Type-specific VOC response profiles were induced on vascular cells from different MPs.

The trace metal iron (Fe) is an essential micronutrient that controls phytoplankton productivity, which subsequently affects organic matter cycling with feedback on the cycling of macronutrients. Along the continental margin of the US West Coast, high benthic Fe release has been documented, in particular from deep anoxic basins in the Southern California Borderland. However, the influence of this Fe release on surface primary production remains poorly understood. In the present study from the Santa Barbara Basin, in situ benthic Fe fluxes were determined along a transect from shallow to deep sites in the basin. Fluxes ranged between 0.23 and 4.9 mmol m−2 d−1, representing some of the highest benthic Fe fluxes reported to date. To investigate the influence of benthic Fe release from the oxygen-deficient deep basin on surface phytoplankton production, we combined benthic flux measurements with numerical simulations using the Regional Ocean Modeling System coupled to the Biogeochemical Elemental Cycling (ROMS-BEC) model. For this purpose, we updated the model Fe flux parameterization to include the new benthic flux measurements from the Santa Barbara Basin. Our simulations suggest that benthic Fe fluxes enhance surface primary production, supporting a positive feedback on benthic Fe release by decreasing oxygen in bottom waters. However, a reduction in phytoplankton Fe limitation by enhanced benthic fluxes near the coast may be partially compensated for by increased nitrogen limitation further offshore, limiting the efficacy of this positive feedback.

Nitrogen (N) plays a central role in marine biogeochemistry by limiting biological productivity in the surface ocean; influencing the cycles of other nutrients, carbon, and oxygen; and controlling oceanic emissions of nitrous oxide (N2O) to the atmosphere. Multiple chemical forms of N are linked together in a dynamic N cycle that is especially active in oxygen minimum zones (OMZs), where high organic matter remineralization and low oxygen concentrations fuel aerobic and anaerobic N transformations. Biogeochemical models used to understand the oceanic N cycle and project its change often employ simple parameterizations of the network of N transformations and omit key intermediary tracers such as nitrite (NO2-) and N2O. Here we present a new model of the oceanic N cycle (Nitrogen cycling in Oxygen Minimum Zones, or NitrOMZ) that resolves N transformation occurring within OMZs and their sensitivity to environmental drivers. The model is designed to be easily coupled to current ocean biogeochemical models by representing the major forms of N as prognostic tracers and parameterizing their transformations as a function of seawater chemistry and organic matter remineralization, with minimal interference in other elemental cycles. We describe the model rationale, formulation, and numerical implementation in a one-dimensional representation of the water column that reproduces typical OMZ conditions. We further detail the optimization of uncertain model parameters against observations from the eastern tropical South Pacific OMZ and evaluate the model's ability to reproduce observed profiles of N tracers and transformation rates in this region. We conclude by describing the model's sensitivity to parameter choices and environmental factors and discussing the model's suitability for ocean biogeochemical studies.

The aim of this research is to develop a multiparametric downscaling analysis for the detection of abandoned waste in the environment. This methodology, using a multi-technological approach, involves the adoption VHR satellite images, Unmanned Aircraft System (UAS) and Unmanned Ground Vehicles (UGV). The identified Warning Areas (WA) will be investigated through an in-situ analysis with air quality measurement devices based on advanced sensors mounted on drones. The creation of a Cadastre Accumulation of Abandoned Materials (CAMA) and the related APP will allow the administrations to monitor the phenomenon. Finally, the waste product analysis, retrieved by means of UAS dataset computation, allows to retrieve some interesting prospects regarding Waste to Energy framework. Here, preliminary results obtained by the on-going INTESA Project are presented.

The Dark Energy Spectroscopic Instrument (DESI) completed its 5 month Survey Validation in 2021 May. Spectra of stellar and extragalactic targets from Survey Validation constitute the first major data sample from the DESI survey. This paper describes the public release of those spectra, the catalogs of derived properties, and the intermediate data products. In total, the public release includes good-quality spectral information from 466,447 objects targeted as part of the Milky Way Survey, 428,758 as part of the Bright Galaxy Survey, 227,318 as part of the Luminous Red Galaxy sample, 437,664 as part of the Emission Line Galaxy sample, and 76,079 as part of the Quasar sample. In addition, the release includes spectral information from 137,148 objects that expand the scope beyond the primary samples as part of a series of secondary programs. Here, we describe the spectral data, data quality, data products, Large-Scale Structure science catalogs, access to the data, and references that provide relevant background to using these spectra.

The Dark Energy Spectroscopic Instrument (DESI) was designed to conduct a survey covering 14,000 deg ^2 over 5 yr to constrain the cosmic expansion history through precise measurements of baryon acoustic oscillations (BAO). The scientific program for DESI was evaluated during a 5 month survey validation (SV) campaign before beginning full operations. This program produced deep spectra of tens of thousands of objects from each of the stellar Milky Way Survey (MWS), Bright Galaxy Survey (BGS), luminous red galaxy (LRG), emission line galaxy (ELG), and quasar target classes. These SV spectra were used to optimize redshift distributions, characterize exposure times, determine calibration procedures, and assess observational overheads for the 5 yr program. In this paper, we present the final target selection algorithms, redshift distributions, and projected cosmology constraints resulting from those studies. We also present a One-Percent Survey conducted at the conclusion of SV covering 140 deg ^2 using the final target selection algorithms with exposures of a depth typical of the main survey. The SV indicates that DESI will be able to complete the full 14,000 deg ^2 program with spectroscopically confirmed targets from the MWS, BGS, LRG, ELG, and quasar programs with total sample sizes of 7.2, 13.8, 7.46, 15.7, and 2.87 million, respectively. These samples will allow exploration of the Milky Way halo, clustering on all scales, and BAO measurements with a statistical precision of 0.28% over the redshift interval z < 1.1, 0.39% over the redshift interval 1.1 < z < 1.9, and 0.46% over the redshift interval 1.9 < z < 3.5.

Throughout the course of their lives fish ingest food containing essential elements, including nitrogen (N), phosphorus (P), and iron (Fe). Some of these elements are retained in the fish body to build new biomass, which acts as a stored reservoir of nutrients, while the rest is excreted or egested, providing a recycling flux to water. Fishing activity has modified the fish biomass distribution worldwide and consequently may have altered fish-mediated nutrient cycling, but this possibility remains largely unassessed, mainly due to the difficulty of estimating global fish biomass and metabolic rates. Here we quantify the role of commercially targeted marine fish between 10 g and 100 kg (CTF10g100kg) in the cycling of N, P, and Fe in the global ocean and its change due to fishing activity, by using a global size-spectrum model of marine fish populations calibrated to observations of fish catches. Our results show that the amount of nutrients potentially stored in the global pristine CTF10g100kg biomass is generally small compared to the ambient surface nutrient concentrations but might be significant in the nutrient-poor regions of the world: the North Atlantic for P, the oligotrophic gyres for N, and the high-nutrient, low-chlorophyll (HNLC) regions for Fe. Similarly, the rate of nutrient removal from the ocean through fishing is globally small compared to the inputs but can be important locally, especially for Fe in the equatorial Pacific and along the western margin of South America and Africa. We also estimate that the cycling rate of elements through CTF10g100kg biomass was on the order of 3 % of the primary productivity demand for N, P, and Fe globally, prior to industrial fishing. The corresponding export of nutrients by egestion of fecal matter by CTF10g100kg was 2.3 % (N), 3.0 % (P), and 1 %–22 % (Fe) of the total particulate export flux and was generally more significant in the low-export oligotrophic tropical gyres. Our study supports a significant, direct role of the CTF10g100kg fraction of the ichthyosphere in global nutrient cycling, most notably for Fe, which has been substantially modified by industrial fishing. Although we were not able to estimate the roles of smaller species such as mesopelagic fish because of the sparsity of observational data, fishing is also likely to have altered their biomass significantly through trophic cascades, with impacts on biogeochemical cycling that could be of comparable magnitude to the changes we assess here.

Abstract Radiographs (XR), computed tomography (CT) or magnetic resonance imaging (MRI) are regularly analyzed to determine whether a bone lesion is benign or malignant. An online quiz was created providing 15 cases with a clinical summary, MRI, CT, and XR. After each image, participants were asked to rate the probability (0–100%) the bone tumor was malignant. Order and difficulty of the images were randomly determined. Probability statements regarding the diagnosis were actualized along the sequence of exam, to quantify how the degree of belief changed to account for evidence from those exams. 64 physicians participated and provided 154 assessments from 1 (n = 18) to 3 (n = 44) different cases. After the first image, participants favored the correct malignancy status at 70%; 80% after the second and 80% after the third one. Participants were more likely to favor the correct malignancy status when the lesion was malignant and when first confronted with XR or CT, rather than MRI, though the most predictive factor of correct diagnosis was the difficulty of the case. In conclusion, the additional information provided by successive imaging studies was moderate. XR or CT seemed more appropriate than MRI as first imaging study. Bypassing XR should be discouraged.

The scientific community has shown increasing interest in laser scribing for the direct fabrication of conductive graphene-based tracks on different substrates. This can enable novel routes for the noninvasive analysis of biofluids (such as sweat or other noninvasive matrices), whose results can provide the rapid evaluation of a person's health status. Here, we present a wearable sensing platform based on laser induced graphene (LIG) porous electrodes scribed on a flexible polyimide sheet, which samples sweat through a paper sampler. The device is fully laser manufactured and features a two layer design with LIG-based vertical interconnect accesses. A detailed characterization of the LIG electrodes including pore size, surface groups, surface area in comparison to electroactive surface area, and the reduction behavior of different LIG types was performed. The bare LIG electrodes can detect the electrochemical oxidation of both uric acid and tyrosine. Further modification of the surface of the LIG working electrode with an indoaniline derivative [4-((4-aminophenyl)imino)-2,6-dimethoxycyclohexa-2,5-dien-1-one] enables the voltammetric measurement of pH with an almost ideal sensitivity and without interference from other analytes. Finally, electrochemical impedance spectroscopy was used to measure the concentrations of ions through the analysis of the sweat impedance. The device was successfully tested in a real case scenario, worn on the skin during a sports session. In vitro tests proved the non-cytotoxic effect of the device on the A549 cell line.

Eastern boundary upwelling systems (EBUSs) are physically and biologically active regions of the ocean with substantial impacts on ocean biogeochemistry, ecology, and global fish catch. Previous studies have used models of varying complexity to study EBUS dynamics, ranging from minimal two-dimensional (2-D) models to comprehensive regional and global models. An advantage of 2-D models is that they are more computationally efficient and easier to interpret than comprehensive regional models, but their key drawback is the lack of explicit representations of important three-dimensional processes that control biology in upwelling systems. These processes include eddy quenching of nutrients and meridional transport of nutrients and heat. The authors present the Meridionally Averaged Model of Eastern Boundary Upwelling Systems (MAMEBUS) that aims at combining the benefits of 2-D and 3-D approaches to modeling EBUSs by parameterizing the key 3-D processes in a 2-D framework. MAMEBUS couples the primitive equations for the physical state of the ocean with a nutrient–phytoplankton–zooplankton–detritus model of the ecosystem, solved in terrain-following coordinates. This article defines the equations that describe the tracer, momentum, and biological evolution, along with physical parameterizations of eddy advection, isopycnal mixing, and boundary layer mixing. It describes the details of the numerical schemes and their implementation in the model code, and provides a reference solution validated against observations from the California Current. The goal of MAMEBUS is to facilitate future studies to efficiently explore the wide space of physical and biogeochemical parameters that control the zonal variations in EBUSs.

In the current era of rapid climate change, accurate characterization of climate-relevant gas dynamics – namely production, consumption, and net emissions – is required for all biomes, especially those ecosystems most susceptible to the impact of change. Marine environments include regions that act as net sources or sinks for numerous climate-active trace gases including methane (CH4) and nitrous oxide (N2O). The temporal and spatial distributions of CH4 and N2O are controlled by the interaction of complex biogeochemical and physical processes. To evaluate and quantify how these mechanisms affect marine CH4 and N2O cycling requires a combination of traditional scientific disciplines including oceanography, microbiology, and numerical modeling. Fundamental to these efforts is ensuring that the datasets produced by independent scientists are comparable and interoperable. Equally critical is transparent communication within the research community about the technical improvements required to increase our collective understanding of marine CH4 and N2O. A workshop sponsored by Ocean Carbon and Biogeochemistry (OCB) was organized to enhance dialogue and collaborations pertaining to marine CH4 and N2O. Here, we summarize the outcomes from the workshop to describe the challenges and opportunities for near-future CH4 and N2O research in the marine environment.

To preserve the Europe consumers' health, the use of glucocorticoids as growth promoters is prohibited in cattle fattening. In 2008, the Italian Ministry of Health associated to the official control a national monitoring plan based on the histological thymus analysis to identify animals illegally treated with corticosteroids. However, since corticosteroids are authorized and widely used for therapeutic purposes, it is necessary to verify whether the thymus histological test and some physicochemical traits in meat are able to discriminate doped calves from dexamethasone therapeutic treated ones. The aims of this study were (i) to establish whether the therapeutic and illicit corticosteroid treatments of calves could be differentiated through histological evaluation of thymus and by physicochemical meat traits; (ii) to identify a restricted number of physicochemical traits that could differentiate dexamethasone treated from untreated calves. Three groups of 15 calves each were included in this study: group dexamethasone therapeutic treatment treated with dexamethasone 21-phosphate disodium salt at a therapeutic dose (2 mg/kg of live weight for three consecutive days); group dexamethasone anabolic treatment orally treated with dexamethasone 21-phosphate disodium salt according to a presumed anabolic protocol (0.4 mg/day per animal for 20 days); group placebo control treated with a placebo served as control. Results demonstrated that groups could be easily discriminated by thymus microscopy as well as by two meat markers, namely, cooking loss and shear firmness or Warner-Bratzler shear force. The combination of thymus microscopic features and meat physicochemical traits could be used as a practical, economic and accurate screening strategy to discriminate between meat from illegally and therapeutically treated calves. This new reliable and simple tool could contribute to identify animals treated with dexamethasone in those countries where glucocorticoids are illegally used as growth promoters. More in general, this system could be included in the framework of official controls, and applied to verify suppliers' reliability by the meat industry.

Although it has long been assumed that the glacial–interglacial cycles of atmospheric CO2 occurred due to increased storage of CO2 in the ocean, with no change in the size of the active carbon inventory, there are signs that the geological CO2 supply rate to the active pool varied significantly. The resulting changes of the carbon inventory cannot be assessed without constraining the rate of carbon removal from the system, which largely occurs in marine sediments. The oceanic supply of alkalinity is also removed by the burial of calcium carbonate in marine sediments, which plays a major role in air–sea partitioning of the active carbon inventory. Here, we present the first global reconstruction of carbon and alkalinity burial in deep-sea sediments over the last glacial cycle. Although subject to large uncertainties, the reconstruction provides a first-order constraint on the effects of changes in deep-sea burial fluxes on global carbon and alkalinity inventories over the last glacial cycle. The results suggest that reduced burial of carbonate in the Atlantic Ocean was not entirely compensated by the increased burial in the Pacific basin during the last glacial period, which would have caused a gradual buildup of alkalinity in the ocean. We also consider the magnitude of possible changes in the larger but poorly constrained rates of burial on continental shelves, and show that these could have been significantly larger than the deep-sea burial changes. The burial-driven inventory variations are sufficiently large to have significantly altered the δ13C of the ocean–atmosphere carbon and changed the average dissolved inorganic carbon (DIC) and alkalinity concentrations of the ocean by more than 100 µM, confirming that carbon burial fluxes were a dynamic, interactive component of the glacial cycles that significantly modified the size of the active carbon pool. Our results also suggest that geological sources and sinks were significantly unbalanced during the late Holocene, leading to a slow net removal flux on the order of 0.1 PgC yr−1 prior to the rapid input of carbon during the industrial period.

Introduction Breast cancer is a severe pathology that once detected completely changes the patient’s perception of life. Objectives Evaluating the relationship that is established between the level of stress, the type of surgery applied and, the instructive level of women. Methods We selected 67 patients which were divided into 2 groups: group I(31) women who benefited from immediate reconstruction and group II(36) subjects who benefited from a late reconstruction. We split each group into two subgroups: women with secondary education and women with higher education. A socio-demographic questionnaire and the DASS-21 scale were applied. Results Comparing the two groups we noticed that stress level was more present in group I(38,7%) than in group II(25%). The differences were not statistically significant(p>0,05). In the subgroup of women with higher education in group I, high levels of stress were observed at 23,08% and, in the subgroup of patients with secondary education, 50% had high levels of stress. The differences were not statistically significant (p>0,05). We also analyzed the two subgroups of group II and we identified increased levels of stress in 20% of patients with higher education compared to those with secondary education where 26,93% had high levels of stress. The Dark Energy Spectroscopic Instrument (DESI) embarked on an ambitious 5 yr survey in 2021 May to explore the nature of dark energy with spectroscopic measurements of 40 million galaxies and quasars. DESI will determine precise redshifts and employ the baryon acoustic oscillation method to measure distances from the nearby universe to beyond redshift z > 3.5, and employ redshift space distortions to measure the growth of structure and probe potential modifications to general relativity. We describe the significant instrumentation we developed to conduct the DESI survey. This includes: a wide-field, 3.°2 diameter prime-focus corrector; a focal plane system with 5020 fiber positioners on the 0.812 m diameter, aspheric focal surface; 10 continuous, high-efficiency fiber cable bundles that connect the focal plane to the spectrographs; and 10 identical spectrographs. Each spectrograph employs a pair of dichroics to split the light into three channels that together record the light from 360–980 nm with a spectral resolution that ranges from 2000–5000. We describe the science requirements, their connection to the technical requirements, the management of the project, and interfaces between subsystems. DESI was installed at the 4 m Mayall Telescope at Kitt Peak National Observatory and has achieved all of its performance goals. Some performance highlights include an rms positioner accuracy of better than 0.″1 and a median signal-to-noise ratio of 7 of the [O ii ] doublet at 8 × 10 ^−17 erg s ^−1 cm ^−2 in 1000 s for galaxies at z = 1.4–1.6. We conclude with additional highlights from the on-sky validation and commissioning, key successes, and lessons learned.

Abstract Background To investigate the efficacy of tamsulosin in patients with lower urinary tract symptoms (LUTS) and benign prostatic enlargement (BPE) with intravesical prostatic protrusion (IPP). Ultrasound measurement of the IPP has been previously described as an effective instrument for the evaluation of benign prostatic obstruction (BPO) and could help in clarifying the role of alpha-blockers in patients with (BPE). Methods Patients with BPE and LUTS were enrolled in this observational study. Intravesical prostatic protrusion was graded as grade 1 ( 10 ml). Patients were treated with tamsulosin for twelve weeks. Evaluation was performed before and at the end of treatment by means of International Prostate Symptom Score (IPSS) and uroflowmetry. Patients were considered responders if a reduction of IPSS > 3 points was reported. Results One hundred forty-two patients were enrolled. Twelve patients were excluded because of incomplete data. Fifty patients showed an IPP grade 1 (group A), 52 a grade 2 (group B) and 28 a grade 3 (group C). Treatment success was obtained in 82%, 38,5% and 7,1% of patients respectively; these differences (group A vs B-C and group B vs C) were highly significant. The odd ratio to obtain a treatment success was of 59 and 8.1 in group A and group B respectively, in comparison to group C. After a multivariate regression, the relationship between IPP grade and treatment success remained significant. Improvement of uroflowmetry parameters has been reported in all the groups especially in patients with a low grade IPP (p value = 0,016 group A vs group B; p value = 0,005 group A vs group C). Prostate volume seems not to influence this relationship. Conclusions Intravesical prostatic protrusion has found to be significantly and inversely correlated with treatment success in patients with LUTS and BPE under alpha-blockers therapy. Alpha blockers odd ratio of success is 59 times higher in patients with a low grade IPP in comparison to patients with a high grade.

Global marine fish harvest increased over the 20th century, reaching a peak in the 1990s. Here, Galbraith and colleagues analyse a model combining both ecological and economic drivers to weigh the factors most likely to contribute to long-term changes in fish harvests.

Environmental change and the exploitation of marine resources have had profound impacts on marine communities, with potential implications for ocean biogeochemistry and food security. In order to study such global-scale problems, it is helpful to have computationally efficient numerical models that predict the first-order features of fish biomass production as a function of the environment, based on empirical and mechanistic understandings of marine ecosystems. Here we describe the ecological module of the BiOeconomic mArine Trophic Size-spectrum (BOATS) model, which takes an Earth-system approach to modelling fish biomass at the global scale. The ecological model is designed to be used on an Earth-system model grid, and determines size spectra of fish biomass by explicitly resolving life history as a function of local temperature and net primary production. Biomass production is limited by the availability of photosynthetic energy to upper trophic levels, following empirical trophic efficiency scalings, and by well-established empirical temperature-dependent growth rates. Natural mortality is calculated using an empirical size-based relationship, while reproduction and recruitment depend on both the food availability to larvae from net primary production and the production of eggs by mature adult fish. We describe predicted biomass spectra and compare them to observations, and conduct a sensitivity study to determine how they change as a function of net primary production and temperature. The model relies on a limited number of parameters compared to similar modelling efforts, while retaining reasonably realistic representations of biological and ecological processes, and is computationally efficient, allowing extensive parameter-space analyses even when implemented globally. As such, it enables the exploration of the linkages between ocean biogeochemistry, climate, and upper trophic levels at the global scale, as well as a representation of fish biomass for idealized studies of fisheries.

We analyse simulations of the Pacific Ocean oxygen minimum zones (OMZs) from 11 Earth system model contributions to the Coupled Model Intercomparison Project Phase 5, focusing on the mean state and climate change projections. The simulations tend to overestimate the volume of the OMZs, especially in the tropics and Southern Hemisphere. Compared to observations, five models introduce incorrect meridional asymmetries in the distribution of oxygen including larger southern OMZ and weaker northern OMZ, due to interhemispheric biases in intermediate water mass ventilation. Seven models show too deep an extent of the tropical hypoxia compared to observations, stemming from a deficient equatorial ventilation in the upper ocean, combined with too large a biologically driven downward flux of particulate organic carbon at depth, caused by particle export from the euphotic layer that is too high and remineralization in the upper ocean that is too weak. At interannual timescales, the dynamics of oxygen in the eastern tropical Pacific OMZ is dominated by biological consumption and linked to natural variability in the Walker circulation. However, under the climate change scenario RCP8.5, all simulations yield small and discrepant changes in oxygen concentration at mid depths in the tropical Pacific by the end of the 21st century due to an almost perfect compensation between warming-related decrease in oxygen saturation and decrease in biological oxygen utilization. Climate change projections are at odds with recent observations that show decreasing oxygen levels at mid depths in the tropical Pacific. Out of the OMZs, all the CMIP5 models predict a decrease of oxygen over most of the surface and deep ocean at low latitudes and over all depths at high latitudes due to an overall slow-down of ventilation and increased temperature.

The aim of the study was to evaluate the adherence of systemic sclerosis (SSc) female patients to cervix and breast cancer screening procedures, as suggested by local guidelines. A cohort of 84 SSc women was asked if they had undergone mammography and Pap test during the previous 2- and 3-year intervals, as indicated according to the Italian recommendations. The results were compared with those collected in patients affected by other chronic rheumatic disorders and in the general population. More than 85% of SSc women declared to comply with an age-related cervix and breast cancer screening schedule. The data were similar to those collected in patients affected by other chronic rheumatic disorders, whereas the subjects belonging to the general population reported to undergo breast cancer screening more frequently. Among SSc women, neither the educational level, nor the lung and skin involvement influenced their cancer screening program compliance. Only a positive history of ischemic digital ulcers seemed to interfere with mammography. Our study reported a very high percentage of SSc female patients who adhered to programs for the early detection of cervical and breast cancer. The significant adherence to guidelines may be due to the schedule adopted by the local health public service, which regularly invites eligible subjects by mail to undergo cancer screening at no charge.

The paper reports the results from the observational retrospective-prospective RUBINO study conducted in Italy to assess the safety of rituximab in the treatment of rheumatoid arthritis (RA) in routine clinical practice. The percentage of patients who manifested at least one grade 3 or 4 adverse event (AE) assessed by the Common Terminology Criteria for Adverse Events version 3.0 (CTCAE v.3) during the observation period (primary objective) was evaluated. The percentage of patients manifesting a severe AE (SAE), clinical response to rituximab treatment, clinical remission according to disease activity score for 28 joints (DAS28) criteria, markers of disease and quality of life were also assessed. Fifty-three Italian rheumatology centers took part in the study. Patients with a diagnosis of RA and inadequate response to anti-tumor necrosis factor b (anti-TNFa) drugs were enrolled. Participating patients had previously received at least one cycle of rituximab, and treatment was still ongoing at the time of recruitment. Out of 205 patients enrolled, 60% manifested no form of AE, 14.2% had at least one grade 3 or 4 AE, and 11.2% patients reported an SAE. The overall percentage of patients manifesting AEs (40%) was lower compared to the DANCER (81% and 85%), REFLEX (85%) and RESET (85% and 69%) studies, but higher than that observed in the CERERRA registry (from 10.2% to 13.9%). This difference may be due to the shorter observation period applied in the CERERRA registry (only 12 months) compared to the RUBINO study (up to 3 years). All parameters of RA activity (erythrocyte sedimentation rate, C-reactive protein, health assessment questionnaire score, DAS28) improved significantly during the study.

The Apparent Oxygen Utilisation (AOU) is a classical measure of the amount of oxygen respired in the ocean's interior. We show that AOU systematically overestimates True Oxygen Utilisation (TOU) in 6 coupled circulation-biogeochemical ocean models. This is due to atmosphere–ocean oxygen disequilibria in the subduction regions, consistent with previous work. We develop a simple, new, observationally-based approach which we call Evaluated Oxygen Utilisation (EOU). In this approach, we take into account the impact of the upper ocean oxygen disequilibria into the interior, considering that transport takes place predominantly along isopycnal surfaces. The EOU approximates the TOU with less than half of the bias of AOU in all 6 models despite large differences in the physical and biological components of the models. Applying the EOU approach to a global observational dataset yields an oxygen consumption rate 25% lower than that derived from AOU-based estimates, for a given ventilation rate.

Phosphate distributions simulated by seven state-of-the-art biogeochemical ocean circulation models are evaluated against observations of global ocean nutrient distributions. The biogeochemical models exhibit different structural complexities, ranging from simple nutrient-restoring to multi-nutrient NPZD type models. We evaluate the simulations using the observed volume distribution of phosphate. The errors in these simulated volume class distributions are significantly larger when preformed phosphate (or regenerated phosphate) rather than total phosphate is considered. Our analysis reveals that models can achieve similarly good fits to observed total phosphate distributions for a~very different partitioning into preformed and regenerated nutrient components. This has implications for the strength and potential climate sensitivity of the simulated biological carbon pump. We suggest complementing the use of total nutrient distributions for assessing model skill by an evaluation of the respective preformed and regenerated nutrient components.

Tree ring Δ14C data (Reimer et al., 2004; McCormac et al., 2004) indicate that atmospheric Δ14C varied on multi-decadal to centennial timescales, in both hemispheres, over the period between AD 950 and 1830. The Northern and Southern Hemispheric Δ14C records display similar variability, but from the data alone is it not clear whether these variations are driven by the production of 14C in the stratosphere (Stuiver and Quay, 1980) or by perturbations to exchanges between carbon reservoirs (Siegenthaler et al., 1980). As the sea-air flux of 14CO2 has a clear maximum in the open ocean regions of the Southern Ocean, relatively modest perturbations to the winds over this region drive significant perturbations to the interhemispheric gradient. In this study, model simulations are used to show that Southern Ocean winds are likely a main driver of the observed variability in the interhemispheric gradient over AD 950–1830, and further, that this variability may be larger than the Southern Ocean wind trends that have been reported for recent decades (notably 1980–2004). This interpretation also implies that there may have been a significant weakening of the winds over the Southern Ocean within a few decades of AD 1375, associated with the transition between the Medieval Climate Anomaly and the Little Ice Age. The driving forces that could have produced such a shift in the winds at the Medieval Climate Anomaly to Little Ice Age transition remain unknown. Our process-focused suite of perturbation experiments with models raises the possibility that the current generation of coupled climate and earth system models may underestimate the natural background multi-decadal- to centennial-timescale variations in the winds over the Southern Ocean.

Chikungunya is an arboviral disease transmitted by Aedes mosquitoes. The disease typically consists of an acute illness characterised by fever, rash, and incapacitating arthralgia, that can persist for months. Chikungunya virus, a member of the genus Alphavirus, has recently caused a large outbreak on islands in the Indian Ocean and on the Indian subcontinent. The ongoing outbreak has involved more than 1.5 million patients, including travellers who have visited these areas. We describe our casistic of six travellers with Chikungunya arthropathy. All patients experienced fever and rash of short term during a travel in areas of epidemicity. All patients had peripheral poliarthralgias, which duration was >2 months in 4 cases (66%) and >6 months in 1 case (16%).

Acute renal failure due to bilateral ureteral obstruction is a rare complication after appendectomy in children. We report a case of bilateral ureteric obstruction in a 14-year-old boy nine days after surgery for an acute appendicitis. After saline-filling of the urinary bladder, transabdominal ultrasound demonstrated bilateral hydronephrosis of moderate degree. No abscess was found with CT but presence of millimetric stones on both distal ureters was shown, with bilateral calyceal dilatation. Cystoscopy revealed inflammatory changes in the bladder base. Following introduction of bilateral ureteric stents, there was rapid normalisation of urinary output and serum creatinine.

The authors report two cases of active seropositive rheumatoid arthritis who were treated in an early phase of the disease with infliximab plus methotrexate obtaining a clinical remission. The benefit was maintained after the discontinuation of the anti-TNF alpha inhibitor for adverse events, indicating that the early administration of the drug may be followed by a sustained remission.

Calves castration represents an old and traditional practice to obtain animal work force, but for technical, economic and social reasons in the last half of the 20th century it was left. The recent renewal of Piemontese steers rearing in the breed’s native diffusion area, for quality meat production (Biagini et al., 2001), has been spurred also for the interest in local typical dishes. Traditionally, in Piedmont calves were castrated before puberty (about 6 month of age), but now also later (12-14 month of age) to benefit by the bull higher growing rate.

A 42-year-old man with a cardiac tamponade underwent an urgent pericardiotomy that showed tumoral tissue, covering the surface of the right atrium. The tumor was then partially excised, and the histological examination revealed the presence of a moderately-differentiated angiosarcoma. The patient was then referred to the oncology unit and scheduled for a chemotherapy schedule including Epirubicin (60 mg/m2, on days 1 and 2) plus Ifosfamide (2000 mg/m2, on days 1 to 3) and Uromitexan (2000 mg/m2 at hours 0, 4, 8 after IFO). All drugs were administered every three weeks. After two cycles, a restaging work-up revealed a partial remission. The treatment was continued for another two cycles. A new evaluation by cardiac MRI evidenced a local and distant (lung) progression of disease. The patient died after three months. This paper confirms that cardiac angiosarcoma is a fatal disease, and the prognosis is usually 6–11 months from time of diagnosis.
