To identify the clinical characteristics and patterns of ultrasound use amongst pregnancies with antenatally unidentified SGA, compared to those in which it is identified, to understand how to better design interventions that improve antenatal SGA identification.A prospective cohort study of singleton, non-anomalous, small for gestational age (SGA, birthweight10th centile) babies born after 24+0 gestational weeks, from 13 UK sites, collected for the baseline period and control arm of the DESiGN trial. We define pregnancies with antenatally unidentified SGA where there was no scan or a final scan with estimated fetal weight, EFW, at 10Of the 15,784 SGA babies included, SGA was not identified antenatally in 78.7%. Of pregnancies with unidentified SGA, 47.1% had no recorded growth scan. Amongst 9,410 pregnancies with complete data on key maternal co-morbidities and antenatal complications, the risk of unidentified SGA was lower for women with any indication for serial scans (aOR 0.56, 95% CI: 0.49-0.64), Asian ethnicity (aOR 0.80 compared to white, 95% CI: 0.69-0.93) and non-cephalic presentation (aOR 0.58, 95% CI: 0.46-0.73). To determine whether the Growth Assessment Protocol (GAP), as implemented in the DESiGN trial, is cost-effective in terms of antenatal detection of small-for-gestational-age (SGA) neonate, when compared with standard care.This was an incremental cost-effectiveness analysis undertaken from the perspective of a UK National Health Service hospital provider. Thirteen maternity units from England, UK, were recruited to the DESiGN (DEtection of Small for GestatioNal age fetus) trial, a cluster randomized controlled trial. Singleton, non-anomalous pregnancies which delivered after 24 + 0 gestational weeks between November 2015 and February 2019 were analyzed. Probabilistic decision modeling using clinical trial data was undertaken. The main outcomes of the study were the expected incremental cost, the additional number of SGA neonates identified antenatally and the incremental cost-effectiveness ratio (ICER) (cost per additional SGA neonate identified) of implementing GAP. Secondary analysis focused on the ICER per infant quality-adjusted life year (QALY) gained.The expected incremental cost (including hospital care and implementation costs) of GAP over standard care was £34 559 per 1000 births, with a 68% probability that implementation of GAP would be associated with increased costs to sustain program delivery. GAP identified an additional 1.77 SGA neonates per 1000 births (55% probability of it being more clinically effective). The ICER for GAP was £19 525 per additional SGA neonate identified, with a 44% probability that GAP would both increase cost and identify more SGA neonates compared with standard care. The probability of GAP being the dominant clinical strategy was low (11%). The expected incremental cost per infant QALY gained ranged from £68 242 to £545 940, depending on assumptions regarding the QALY value of detection of SGA.The economic case for replacing standard care with GAP is weak based on the analysis reported in our study. Intermittent claudication can be neurogenic or vascular. Physicians use a profile based on symptom attributes to differentiate the 2 types of claudication, and this guides their investigations for diagnosis of the underlying pathology. We evaluated the validity of these symptom attributes in differentiating neurogenic from vascular claudication.Patients with a diagnosis of lumbar spinal stenosis (LSS) or peripheral vascular disease (PVD) who reported claudication answered 14 questions characterizing their symptoms. We determined the sensitivity, specificity and positive and negative likelihood ratios (PLR and NLR) for neurogenic and vascular claudication for each symptom attribute.We studied 53 patients. The most sensitive symptom attribute to rule out LSS was the absence of "triggering of pain with standing alone" (sensitivity 0.97, NLR 0.050). Pain alleviators and symptom location data showed a weak clinical significance for LSS and PVD. Constellation of symptoms yielded the strongest associations: patients with a positive shopping cart sign whose symptoms were located above the knees, triggered with standing alone and relieved with sitting had a strong likelihood of neurogenic claudication (PLR 13). Patients with symptoms in the calf that were relieved with standing alone had a strong likelihood of vascular claudication (PLR 20.0).The classic symptom attributes used to differentiate neurogenic from vascular claudication are at best weakly valid independently. However, certain constellation of symptoms are much more indicative of etiology. These results can guide general practitioners in their evaluation of and investigation for claudication.La claudication intermittente peut avoir une étiologie neurogène ou vasculaire. Les médecins utilisent un profil fondé sur les particularités des symptômes pour distinguer l’une de l’autre et ceci oriente leur choix des méthodes de diagnostic de la pathologie sous-jacente. Nous avons évalué la validité de ces particularités des symptômes utilisées pour distinguer la claudication d’origine neurogène de la claudication d’origine vasculaire.Des patients atteints d’une sténose spinale lombaire (SSL) ou d’une maladie vasculaire périphérique (MVP) avérées qui se plaignaient de claudication ont réponduà 14 questions afin de caractériser leurs symptômes. Nous avons déterminé la sensibilité, la spécificité et les rapports de probabilité positifs et négatifs (RPP et RPN) à l’égard de la claudication neurogène ou vasculaire pour chacune des particularités des symptômes.Notre étude a regroupé 53 patients. La particularité des symptômes dotée de la sensibilité la plus élevée pour ce qui est d’écarter le diagnostic de SSL a été l’absence de « déclenchement de la douleur à la simple station debout » (sensibilité 0,97; RPN 0,050). Les données sur ce qui soulageait la douleur et sur la localisation des symptômes ont eu une faible portée clinique en ce qui a trait à la SSL et à la MVP. La présence d’une constellation de symptômes a donné lieu aux associations les plus solides : les patients qui manifestaient un signe du « panier d’épicerie » positif et dont les symptômes étaient localisés au-dessus du genou, déclenchés par la station debout seule et soulagés en position assise présentaient une forte probabilité de claudication d’origine neurogène (RPP 13). Chez les patients dont les symptômes étaient localisés au mollet et qui étaient soulagés par la station debout, on notait une forte probabilité de claudication d’origine vasculaire (RPP 20,0).Considérés individuellement, les attributs classiques des symptômes utilisés pour distinguer la claudication d’origine neurogène de la claudication d’origine vasculaire sont au mieux faiblement valides. Toutefois, certaines constellations de symptômes éclairent bien davantage l’étiologie. Ces résultats peuvent guider l’omnipraticien dans son examen et dans son diagnostic de la claudication.

Introduction Le succes du traitement endovasculaire des anevrysmes de l’aorte abdominale (AAAs) requiert l’existence d’une anatomie sous-renale specifique au niveau du collet pour permettre un ancrage et une fixation durable. Il s’agit d’une etude unicentrique revoyant les resultats de l’endoprothese Zenith en fonction de la longueur du collet anevrysmal. Methodes Une revue unicentrique retrospective de l’ensemble des AAAs traites electivement avec l’endoprothese Zenith sur une periode recente de 5 ans. Les patients etaient separes entre ceux ayant des collets sous-renaux compris entre 4 et 15 mm de longueur et ceux ayant un collet >15 mm en utilisant des mesures faites en ligne centrale de flux. Les resultats cliniques et le suivi tomodensitometrique etaient revus. Resultats De 2003 a 2008, 318 patients avaient un traitement electif pour AAA sous-renal avec l’endoprothese Zenith. Parmi les 318 patients, 68 (21,4%) avaient un collet de 4-15 mm de long et 250 (79,5%) avaient un collet >15 mm. La mortalite globale etait de 0,9% ( p = 0,11) et le taux d’endofuite de type II etait de 19% ( p = 0,11); sans difference entre les groupes. Quatre patients de chaque groupe avaient une endofuite de type I proximale, de resolution spontanee chez deux patients de chaque groupe. Les deux autres patients de chaque groupe necessitaient une intervention secondaire (deux interventions endovasculaires et deux conversions chirurgicales ouvertes). Les endofuites de type I et les reinterventions ne differaient pas statistiquement entre les groupes ( p = 0,06). Lors de l’analyse complementaire, les patients ayant necessite une reintervention ou une conversion pour endofuite de type I avaient d’autres caracteristiques defavorables (large diametre, angulation). Aucune nouvelle endofuite de type I n’etait decouverte sur un suivi de 5 ans. Conclusion Ces resultats a moyen terme indiquent que les patients ayant des collets sous-renaux plus courts peuvent etre traites avec la meme efficacite que ceux ayant des collets plus longs avec l’endoprothese Zenith, pour peu que ces collets ne soient pas tortueux ou de large diametre.

Purpose: To determine whether endovascular repair (EVAR) offers a survival advantage over open repair (OAR) with ruptured abdominal aortic aneurysms (RAAA). Methods: Retrospective analysis of RAAA patients treated between 2003 and 2008. Univariate and multivariate analyses were performed. Results: 167 patients presented with RAAA (OAR = 135, 80.8%, EVAR = 32, 19.2%). On univariate analysis, EVAR was associated with a decreased mortality relative to OAR, (15.6% vs 43.7%, P = .004). Patients who survived were younger (P < .0005), had a higher blood pressure (P < .0005), level of consciousness (P < .0005), and hemoglobin (P = .018), and a lower urea (P = .005) and international normalized ratio (INR; P = .001). On multivariate analysis, type of repair remained an independent predictor of 30-day mortality (OR: 0.121; 95% CI: 0.021-0.682, P = .017). Conclusion: Controlling for preoperative factors, EVAR is an independent predictor of lower 30 day mortality relative to open repair after RAAA. This supports the wider use of endovascular repair in all patients with RAAA.

Actuellement seule l'hydratation et la N-acetylcysteine se sont montrees efficaces pour diminuer l'incidence de la nephropathie au produit de contraste radiologique. Nous avons etudie le role de la N-acetylcysteine et de divers protocoles d'hydratation chez des malades de chirurgie vasculaire soumis a une arteriographie. Un essai monocentrique randomise, controle contre placebo, a ete mene chez des malades ayant une dysfonction renale pre-existante stable et subissant une arteriographie elective en externe. Les malades ont ete randomises entre hydratation orale en externe et N-acetylcysteine, hydratation plus N-acetylcysteine ou notre traitement standard par hydratation intra-veineuse en hospitalisation seule. Deux malade sur 28 (7 %) qui avaient recu une hydratation orale en externe et de la N-acetylcysteine ont fait une nephropathie au produit de contraste alors que deux sur 25 (8 %) qui avaient eu une hydratation en hospitalisation et de la N-acetylcysteine ont fait une nephropathie au produit de contraste et que deux sur 25 (8 %) qui ont eu le traitement standard par hydratation intra-veineuse seule en hospitalisation ont fait une nephropathie au produit de contraste. Il n'y avait pas de difference significative dans l'incidence de la nephropathie au produit de contraste entre les differents groupes. Aucun facteur de risque independant statistiquement significatif n'a ete identifie parmi les malades qui ont fait une nephropathie au produit de contraste. La N-acetylcysteine n'a pas apporte un benefice supplementaire aux malades traites par hydratation intra-veineuse en hospitalisation. L'hydratation orale associee a la N-acetylcysteine chez les malades externes etait aussi efficace pour prevenir la nephropathie au produit de contraste que les traitements chez les malades hospitalises et a evite une admission hospitaliere couteuse.

Bacillus Calmette-Guérin (BCG) is a live attenuated strain of Mycobacterium bovis that has proven effective in the treatment of early-stage bladder cancer. Although intravesical therapy with BCG is generally considered safe, serious complications including hematuria, granulomatous pneumonitis, hepatitis, and life-threatening BCG sepsis are well known. BCG-related vascular infections are rarely reported. We present a case of a ruptured abdominal aortic aneurysm secondary to M bovis infection 2 years after intravesical instillation of BCG and review the related literature.

The purpose of the present study was to determine whether an institution’s prior endovascular experience influenced the learning curve of subsequent surgeons. A prospective analysis of the initial 70 endovascular abdominal aortic aneurysm repair (EVAR) cases attempted by an individual surgeon was performed with the primary outcome variable being achievement and 30-day maintenance of initial clinical success. Along with standard statistical analyses, the cumulative sum failure method (CUSUM) was used to analyze the learning curve, with a predetermined acceptable failure rate of 10%. Seventy elective EVAR cases were performed by this surgeon during a 4-year period (2000-2004) (mean age, 73.7 ∓ 5.4 years; mean aneurysm diameter 63.3 ∓ 7.2 mm). Initial clinical success was achieved in 68 of 70 cases (97%), which differed significantly with that of our initial surgeon (88.5%, P = .01). Causes of failure in the present series included 1 early mortality (1.4%) and 1 case of conversion to open repair with no instances of type I endoleak or endograft limb thrombosis. Both surgeons’ cases were plotted sequentially with CUSUM curves revealing a significantly shorter learning curve for the second surgeon. Optimal results were achieved following 10 to 20 EVAR cases, as opposed to 60 cases in the initial series. Such an analysis confirms that as an institution’s experience with EVAR increases, an individual surgeon’s learning curve shortens considerably.

Objectives Worldwide, increasing proportions of aortic aneurysms are repaired electively via the endovascular route. The purpose of this study was to report the recent utilization of endovascular repair in Canada by reviewing a national administrative database. Methods The Canadian Institute for Health Information database (a collection of all acute care hospitalizations) was reviewed to identify patients who received nonemergent repair of an abdominal aortic aneurysm (AAA) between April 1, 2003 and March 31, 2004. During this 1-year period, differentiation between endovascular (EVAR) and open repair was possible using ICD-10-CA procedural codes in eight of ten provinces. Case volumes, patient age, length of hospitalization, and mortality were stratified by method of repair, province, and size and teaching status of hospitals. Results In this 1-year period, 1996 patients in eight provinces (representing 72% of Canada's population) underwent open repair (n = 1818, 91.1%) or EVAR (n = 178, 8.9%) of a nonruptured AAA. National utilization rates were 8.4 and 0.8 per 100,000 population for open repair and EVAR. These rates were more constant for EVAR (0 to 1.3) then for open repair (4 to 18.3) when analyzed on a provincial basis. Mean patient age did not differ between EVAR and open repair (73.7 vs 71.9 years, P = 0.4) while mean length of stay (5.8 vs 11.9 days, P = 0.03) and in-hospital mortality (0.6% vs 4.6%, P = .025) were significantly lower for EVAR than for open repair. Most EVAR (96%) and more than half of open repairs (56%) were performed in academic teaching centers. Conclusions Although EVAR results in significant reductions in length of hospitalization and early mortality, it continues to be underutilized in Canada compared with other national reports involving administrative databases.

The purpose of these authors' study was to analyze their center's experience with orthotopic heart transplantation (OHT) and abdominal aortic aneurysms (AAA) with particular attention to corticosteroid dosing, hemodynamic parameters, and aneurysm growth rate. A retrospective review of all patients (453) who underwent OHT at their university-affiliated medical center over an 18-year period (1981–1999) was undertaken. Nine (2%) patients who developed AAAs were identified and aneurysm growth was correlated with corticosteroid immunosuppression and hemodynamic parameters. The mean age of OHT patients was 44.5 ±15 years and the majority were males (371 males, 82%). Median follow-up was 5.7 years. Ischemic cardiomyopathy (IC) was the most common indication for transplantation (45.5% of patients). All AAA patients were male (p= 0.157), with a mean age of 58.4 ±4.8 years (p= 0.001), and had undergone OHT for IC (p= 0.001). Significant vascular complications are rare following systemic infections with Mycobacterium tuberculosis (TB). This report describes a 33-year-old man who presented with a short history of abdominal discomfort and febrile episodes with no prior history of infection with TB. Ultrasound, CT scan, and aortography confirmed the presence of a pseudoaneurysm originating from the posterior aspect of the supraceliac aorta at the level of the diaphragm. Via a full thoracoabdominal approach, periaortic inflammatory tissue and the aortic wall itself were debrided, and repair of the pseudoaneurysm was achieved with a synthetic patch. Mycobacterium tuberculosis was isolated from the aortic wall, and anti-TB medications were instituted. Postoperatively the patient did well and was discharged after 14 days. As illustrated by this case, tuberculous mycotic aneurysms of the aorta are optimally treated with a combination of medical and surgical therapy, and early diagnosis is essential to ensure survival.

We report a case of a 52-year-old male who presented with acute leg ischemia and underwent successful femoral embolectomy and fasciotomies. Investigations revealed a pedunculated mass in the aortic arch, floating under the innominate and left common carotid arteries. Urgent resection was performed through a longitudinal aortotomy with deep hypothermic circulatory arrest and axillary artery perfusion to reveal a 2.5 × 1.5 cm pedunculated mass attached to the posterior aspect of the arch that was resected. Histology revealed thrombus material prompting lifelong systemic anticoagulation. On 3 months follow-up, the patient had returned to normal activities and computed tomography confirmed complete resection without recurrence. This case study demonstrates that spontaneous thrombus formation is possible in high-flow vascular regions such as the aortic arch and also confirms the importance of evaluating central sources of thromboemboli in patients presenting with acute limb ischemia. Urgent surgical removal is recommended and can be performed safely.

Respiratory failure that requires endotracheal intubation is an uncommon but potentially fatal complication of scald burns in children. Because scalds are rarely associated with a direct pulmonary injury, the pathophysiology of respiratory failure is unclear. A possible mechanism may be upper airway edema, diminished pulmonary compliance secondary to fluid resuscitation, or both. To identify an at-risk population for intubation after a scald injury, the hospital courses of 174 consecutive patients under the age of 14 years who were admitted after a scald injury to a single burn center during a 6-year period were examined. Seven of these patients (4%) required endotracheal intubation. No patient older than 2.8 years or who had a scald injury that covered less than 19% of the total body surface area required intubation. Severe trauma may initiate a systemic inflammatory response, which in turn may result in remote organ injury. After limb ischemia/reperfusion (I/R), intravital fluorescence microscopy was applied to the livers of normotensive rats to investigate the initiation of remote injury to the liver. Additionally, we determined whether Kupffer cell activation and tumor necrosis factor-alpha (TNF-alpha) were involved, via perfusion deficits, in such injury. TNF-alpha, measured by immunoassay, peaked at 30 minutes of reperfusion, but returned to baseline within 60 minutes. Limb I/R resulted in significant increases to global hepatocellular injury measured by alanine transaminase (ALT) and lethal hepatocyte injury as seen with intravital fluorescence microscopy. Although the number of perfused sinusoids went unchanged, a significantly augmented perfusion heterogeneity was measured. After 1.5 hours of reperfusion, both TNF-alpha and Kupffer cells were shown to contribute to global hepatocellular injury (e.g., ALT). After 3 hours, TNF-alpha was no longer essential for this injury, suggesting that some other mechanism(s) activated Kupffer cells and initiated hepatocellular injury. Using propidium iodide and fluorescence microscopy, we found that both TNF-alpha and Kupffer cell activation were necessary to drive hepatocytes toward lethal injury. No additional benefits were observed with a combination of TNF-alpha inhibition and Kupffer cell suppression. These results not only implicate both Kupffer cells and TNF-alpha in the initiation of remote hepatic injury, but suggest that sinusoidal perfusion deficits are not essential for the initiation of such injury. Other mechanism(s) are likely involved in the pathogenesis of remote hepatic parenchymal injury.

While surgery for stenotic lesions of the internal carotid artery (ICA) remains a common procedure performed by vascular surgeons, aneurysmal disease in this area continues to present less frequently. It has been estimated that operations for carotid aneurysms may account for at most 2% of all carotid artery interventions. Although pseudoaneurysms following endarterectomy continues to be the most common pathology (in up to 58% of cases) several other aetiological processes are possible. These include atherosclerosis, fibromuscular dysplasia, trauma and carotid dissection. The natural history of these aneurysms is not entirely known as few centres have accumulated sufficient experience. However, in keeping with most published series, 43% of patients with ICA aneurysms presenting to the Texas Heart Institute suffered from symptoms related to cerebral ischemia. This relatively high risk of cerebral events has led many vascular surgeons to advocate intervention for most of these aneurysms. This report describes the presentation and subsequent successful surgical treatment of a large asymptomatic, atherosclerotic internal carotid aneurysm in an elderly women. Case Report

To report the early results of a multicenter registry of endovascular aneurysm repair (EVAR) using the Endurant stent-graft.Patients having elective treatment of infrarenal abdominal aortic aneurysm (AAA) with the Endurant stent-graft at 3 Canadian centers were enrolled in a prospective registry between September 2008 and January 2010. In the 16-month period, 111 patients (90 men; mean age 75 years, range 53-93) were registered. Thirty-seven (33.3%) patients had challenging anatomy: short proximal aortic necks (n=17), large diameter (28 mm) aortic necks (n=4), angulated (60°) necks (n=3), and small (15 mm) external iliac arteries (n=21). Outcomes evaluated included survival, endoleak, aneurysm expansion5 mm, secondary intervention, stent-graft migration, and graft thrombosis.The overall technical success rate was 100%. Nineteen (17.1%) patients experienced perioperative complications. After a mean follow-up of 6 months (range 0.1-16), mortality in the series was 4.5%: 1 perioperative death (multisystem organ failure) and 4 (3.6%) late deaths (3 cardiac, 1 cancer). Clinical and imaging follow-up past the perioperative period were available in 107 (96.4%) and 99 (89.2%) patients, respectively. Among the latter, 9 (9.1%) had a type II endoleak on the first scan; 4 resolved spontaneously. Three (3.0%) patients developed graft limb thrombosis in follow-up; one required an intervention. There was no graft migration, aneurysm expansion, secondary intervention for endoleak, aneurysm rupture, or conversion.Early results from this prospective multicenter registry indicate that the Endurant stent-graft is a safe option for elective EVAR in selected AAA patients. Longer follow-up is required to determine the durability of these outcomes.

The objective was to determine whether incision application of platelet-rich plasma (PRP) will decrease postoperative wound complications in vascular surgery patients. A prospective, randomized trial randomized 81 incisions in 51 patients who underwent femoral artery exposure for elective revascularization procedures or endovascular abdominal aneurysm repairs. Incidence of diabetes, chronic renal failure, prosthetic grafts, body mass index (BMI), and steroid use did not differ. Using the ASEPSIS wound classification system, we found no difference in incidence of wound infection. Wound complications occurred in 9 (23%) of 40 of PRP group and 9 (22%) of 41 of non-PRP. Severe wound complications developed in 5 (13%) PRP and 6 (5%) of non-PRP (P = NS). In multivariate analysis, there were no predictors for wound infection. Groin wound complications rates are common in this patient group. Platelet-rich plasma did not decrease the incidence of groin wound complications in our patients

To develop strategies based on simple clinical assessment and blood markers to identify older individuals at high risk for Type 2 diabetes.A prospective study of non-diabetic men (n = 3523) and women (n = 3404) aged 60-79 years followed for 7 years, during which there were 297 incident cases of Type 2 diabetes. Logistic regression was used to develop scores to predict incident cases, starting with clinical predictors and adding blood markers that predicted the incidence of diabetes. Receiving operating characteristic analyses were used to assess improvement in prediction.The area under the curve for a simple clinical assessment score, which included age, sex, family history of diabetes, smoking status, BMI, waist circumference, hypertension and recall of doctor diagnosis of coronary heart disease was 0.765 (0.740, 0.791); sensitivity and specificity in the top quintile of the score were 50.3 and 81.4%, respectively. Addition of simple fasting blood markers HDL cholesterol, triglyceride and glucose improved prediction [area under the curve = 0.817 (0.793, 0.840), P0.0001; sensitivity 63.8%; specificity 82.0%]. An alternative model adding blood markers not dependent on fasting yielded similar results. Further addition of C-reactive protein made no improvement. Blood measurements made small differences to reclassification of risk in those in the lowest three quintiles of the non-laboratory score.In large population settings, simple clinical assessments could be used in the first instance to identify older adults who would benefit from further testing with routine (non-fasting) blood markers to identify those at most likely to be at elevated diabetes risk.

The following is a case of a 22-year-old male with recurrent thoracic aneurysms with several constitutional symptoms, including gastrointestinal discomfort, irritable bowel syndrome, lactose intolerance, and a 2-week history of severe lower back pain. The patient underwent an initial thoracoabdominal repair of a visceral aneurysm followed by endovascular repair of a recurrent thoracic pseudoaneurysm. The etiology of the visceral aneurysm was initially hypothesized to be mycotic; however, further information revealed signs and symptoms consistent with the diagnostic criteria for Behçet disease (BD). We suggest that BD be considered in younger patients who present with an aortic aneurysm. Although open repair is the traditional approach for arterial lesions in BD, the role for endovascular intervention should be considered as it represents a surgical repair with a significant reduction in morbidity.

Background Successful endovascular repair of abdominal aortic aneurysms (AAAs) requires specific infrarenal neck anatomy to allow for a durable seal and fixation. This is a single-center study reviewing outcomes in relation to neck length after placement of a Zenith endograft. Methods Retrospective single-center review of all AAAs electively repaired with a Zenith endograft during a recent 5-year period. Patients were divided into those with infrarenal necks 4-15 mm in length and those >15 mm using center line measurements. Clinical outcomes and follow-up computed tomography scans were reviewed. Results Between 2003 and 2008, 318 patients underwent elective repair of an infrarenal AAA with the Zenith endograft. Of 318 patients, 68 (21.4%) had necks measuring 4-15 mm in length and 250 (79.5%) had necks measuring >15 mm. Overall early mortality was 0.9% ( p = 0.11) and the rate of type II endoleaks was 19% ( p = 0.11); neither differed between the groups. Four patients in each group had immediate proximal type I endoleaks, which resolved spontaneously in two patients in each group. The remaining two in each group required further intervention (two endovascular and two conversion to open repair). Type I endoleaks and reinterventions did not differ statistically between groups ( p = 0.06). On further analysis, those patients requiring reintervention or conversion for type I endoleaks had other unattractive neck features (large diameter, angulation). There have been no instances of new type I endoleaks during 5-yearfollow-up period. Conclusion These midterm results indicate that patients with shorter infrarenal necks can be treated as effectively as those with longer necks with the Zenith endograft unless these necks are tortuous or wide.

It is important to know how increasing levels of atmospheric CO2 will affect native vegetation. The objective of this study was to determine the effect of elevated CO2 concentrations on species composition in a tallgrass prairie kept at a high water level (730 mm of water in a 2000 mm soil profile) and a low water level (660 mm of water in 2000 mm). 16 cylindrical plastic chambers were placed on the prairie to maintain two levels of CO2 (ambient or twice ambient) during two growing seasons in 1989 and 1990. Frequency of species was determined on 25 July 1989 and on 5 and 10 October 1990. At the beginning of the study, Poa pratensis (Kentucky bluegrass), the dominant C3 species, had the highest frequency of 43.3 %, but decreased with time. However, at the end of the experiment and under the high soil-water level, there were more P. pratensis plants in the elevated CO2 treatment (fre- quency: 13.5%) than in the ambient CO2 treatment (1.0 %).

With the atmospheric concentration of CO 2 increasing, it is important to know how this will affect crop growth. The objective of the study was to determine the effect of elevated CO 2 on big bluestem (Andropogon gerardii Vitman) growing in a tallgrass prairie on a Tully silty clay loam (fine, mixed, mesic Pachic Argiustoll) kept at a high water level (field capacity) or a low water level (half field capacity)

Aims We investigated pathways linking offspring birth weight to maternal diabetes risk in later life by taking into account a range of prospective early-life and adult maternal factors. Methods In a national birth cohort study, we examined the relationship between offspring birth weight and maternal glycated haemoglobin (HbA1c) at age 53 years in 581 mothers who had a first birth between age 19 and 25 years, and had data on potential confounders or mediators. Results Mean age at first birth was 21.5 years. After adjustment for maternal body mass index (BMI), mean percentage change in maternal HbA1c per kilogram increase in offspring birth weight was −1.8%[95% confidence interval (CI) −3.5, −0.1; P = 0.03]. This relationship was mostly accounted for by gestational age that was inversely related to maternal HbA1c (−0.9%; 95% CI −1.5, −0.4; P = 0.001). Other risk factors for high HbA1c were smoking and high BMI at 53 years. There was a significant interaction between offspring birth weight and maternal childhood social class (P = 0.01). Mothers from a manual background with higher birth weight offspring had lower HbA1c (BMI adjusted: −3.1%; 95% CI −5.0, −1.1); this was not observed for mothers from a non-manual background (BMI adjusted: 1.9%; 95% CI −1.3, 5.0). Conclusions Short gestational age and low offspring birth weight may be part of a pathway linking impaired early maternal growth to diabetes risk in later life. A second possible pathway linking higher offspring birth weight to later maternal glucose status was also identified. These potential pathways require further investigation in cohorts with a wider maternal age range so that the early targeting of public health initiatives can be assessed. Diabet. (...) The effect of location (environment) on the base temperature for growth of sorghum [Sorghum bicolor (L.) Moench] genotypes was studied. Seeds of six sorghum genotypes were obtained from three locations: Manhattan, KS, College Station and Lubbock, TX. The seeds were placed on moist germination blotter paper on a one-way thermal gradient plate. Observations were made at least daily for 3 wk to determine the number of days required for (i) germination (i.e., protrusion of the radicle from the seed), (ii) root elongation to 10 mm or longer, and (iii) shoot elongation to 10 mm or longer. Short-term and midterm clinical outcomes after endovascular repair of abdominal aortic aneurysms (AAAs) have been well documented. Evaluation of longer term outcomes is now possible. Here we describe our initial 100 high-risk patients treated with endovascular aneurysm repair (EVAR), all with a minimum of 5 years of follow-up. A retrospective review of prospectively recorded data in a departmental database was undertaken for the first 100 consecutive EVAR patients with a minimum of 5 years (range, 60-105 months) of follow-up performed between December 1997 and June 2001. Information was obtained from surgical follow-up visits and family doctors' offices. Endovascular repair of AAA in high-risk patients can be achieved with acceptably low postoperative mortality and morbidity. Longer term results in this high-risk cohort suggest that EVAR is effective in preventing aneurysm-related deaths at 5 years and beyond. All late mortalities were due to patients' comorbid diseases.

Thoracic aorta disease remains a challenging problem, and despite improvements, open repair techniques are still associated with significant morbidity and mortality. This is a retrospective review of 53 consecutive patients with thoracic aortic pathology who were treated with endovascular repair between September 1998 and December 2004 at a tertiary-care hospital. Endovascular stent graft placement was performed on 23 elective and 30 emergent patients (34 male patients, mean age 66 years, 21 to 85 years). Completion angiography revealed no endoleak in 47 (89%) patients, a type I endoleak in 4 patients, and a type II endoleak in 2 patients. Operative 30-day mortality for elective aneurysms (n = 22), emergent aneurysms (n = 10), dissection (n = 3), penetrating aortic ulcers (n = 7), and trauma (n = 11) was 0%, 40%, 0%, 29%, and 9%, respectively. In total, 46 (87%) patients survived 30 days, and 36 (78.3%) of the survivors were discharged home free of complications. Two patients (4%) experienced paraplegia. Median follow-up was 22 months (1 to 72 months). Intermediate-term results revealed 41 (89%) patients free of endoleak, stent migration, or aneurysmal expansion. Two (4%) patients required reintervention with an additional stent graft. There were 2 (4%) patients with late aortic-related deaths and four (9%) patients with non—aorticrelated late deaths. Endovascular stent graft placement for thoracic aorta disease can be performed successfully and safely with good perioperative and intermediate-term outcomes. Stent graft complication and reintervention rates are low, whereas intermediate survival rates are good. Long-term efficacy still needs to be evaluated.

Presently, only hydration and N-acetylcysteine have been shown to be effective in decreasing the incidence of radiographic contrast-induced nephropathy. We investigated the role of N-acetylcysteine and various hydration protocols in vascular surgery patients undergoing angiography. A single-center, randomized, placebo-controlled trial was conducted in patients with stable, preexisting renal dysfunction undergoing elective, outpatient angiography. Patients were randomized to outpatient oral hydration and N-acetylcysteine, inpatient hydration plus N-acetylcysteine, or our standard therapy of inpatient intravenous hydration alone. Two of twenty-eight (7%) patients who received outpatient oral hydration and N-acetylcysteine developed contrast-induced nephropathy, while two of 25 (8%) who recieved inpatient hydration plus N-acetylcysteine developed contrast-induced nephropathy and two of 25 (8%) who received standard therapy of inpatient intravenous hydration alone developed contrast-induced nephropathy. There was no statistical difference in incidence of contrast-induced nephropathy between the groups. No statistically significant independent risk factors were identified among the patients who developed contrast-induced nephropathy. N-Acetylcysteine did not confer additional benefit to patients treated with inpatient intravenous hydration. Outpatient oral hydration plus N-acetylcysteine was as effective at preventing contrast-induced nephropathy as inpatient therapies and avoided costly hospital admission.

Objective Recently, practice guideline documents have recommended the completion of different levels of interventional experience and 5 or 10 thoracic endovascular aortic cases prior to surgeon credentialing. This study's purpose was to determine whether these requirements are valid by reviewing three surgeons' learning curves with thoracic aortic endovascular repairs. Methods Between 1998 and 2006, 67 patients underwent emergent or elective endovascular repair of thoracic aortic pathologies by one of three vascular surgeons with extensive experience with catheter manipulation and abdominal aortic endografts. Following standard retrospective review, each surgeon's learning curve was analyzed using the cumulative sum failure method with a target success rate of 95% derived from the literature. The main outcome variable was primary technical success. Results These 67 patients presented with several pathologies including elective (n = 31) and ruptured (n = 11) thoracic aortic aneurysms, acute dissections or aortic ulcers (n = 10), and acute blunt thoracic aortic trauma (n = 15). The mean age was 65 (range: 20 to 90) and the early (30 day) mortality rate was 19.4% in urgent cases (n = 36) and 0% in elective cases (n = 31). Paraplegia occurred in two patients (3%). Primary technical success was achieved in 62 cases (92.5%) and did not differ between surgeons (92.6%, 91.3%, 94.1%, respectively; P = .9). Each surgeon's cases were plotted sequentially and the resulting learning curves were similar. Although acceptable outcomes were obtained throughout the study period, improved results, compared with the target success rate, were not achieved until each surgeon treated 5 to 10 patients. Conclusion This study supports the case volume requirements of the Society for Vascular Surgery credentialing guidelines, which also requires extensive catheter and guidewire experience. With this background in catheter manipulation and endovascular abdominal aortic repair, surgeons can achieve optimal outcomes with thoracic aortic lesions following 5 to 10 cases.

It has been postulated that gender differences in abdominal aortic aneurysm (AAA) rupture risk are due to variation in proportional dilatation. This study's purpose was to examine differences in relative dilatation of AAAs as related to rupture risk and repair thresholds. During a recent 20-month period (August 2003-March 2005), all patients receiving elective endovascular repair of an AAA underwent computer-generated computed tomographic measurements, including suprarenal aortic diameter (D1) and maximum aneurysm diameter (D3). As a measure of proportional dilatation, a D3/D1 ratio was calculated for each patient and compared between genders. Values for males and females were plotted separately (D3 vs. D3/D1), and the resulting linear regression equations allowed for the comparison of proportional dilatation of aneurysms between genders. In 129 patients (108 males, 21 females), mean D3 did not differ between genders (62 +/- 9.0 vs 63.4 +/- 10.8 mm, p = 0.89), while relative dilatation (D3/D1) was significantly greater in females compared to males (2.82 +/- 0.52 vs. 2.55 +/- 0.42, p = 0.02). Linear regression analysis allowed for comparison of D3 as a function of D3/D1 and correlated a 55 mm AAA in males with one of 52 mm in females. These differences became greater with increasing AAA diameters (e.g., 60 and 70 mm AAAs in a male were proportionally similar to 56 and 62 mm AAAs in a female). AAAs of equal diameter represent a greater proportional dilatation in females than in males. This supports the recommendation for a smaller aneurysm diameter (52 mm) threshold for repair in females rather than the commonly used 55 mm threshold in males. Also, these data suggest that the gender difference in rupture risk widens exponentially as aneurysm diameter, and thereby proportional dilatation, increases above this repair threshold.

Unlike in the United States, endovascular aneurysm repair (EVAR) has not been widely disseminated in Canada but has remained limited to large-volume vascular surgery units. Since the development of the endovascular program at our hospital, we have experienced a growth in our aneurysm practice and the area of referral. The purpose of this study was to compare the geographic referral area of our aneurysm practice between 1997 (prior to the introduction of EVAR) and 2003 (EVAR and open surgery). Our prospective database was reviewed to identify patients who underwent elective open aneurysm repair in 1997 and 2003 and those who underwent EVAR in 2003. Each patient’s county of residence was identified, allowing for grouping of patients into one of four geographic regions (I-IV) increasingly more distant from our hospital. Proportions were compared with the χ2 test. In 1997, 105 patients underwent open abdominal aortic aneurysm repair, with the majority of patients originating from the two regions in closest proximity to our hospital (I, 34%; II, 46%; III, 18%; IV, 2%). This contrasts with the 2003 EVAR group (n = 63), which had a higher proportion of patients referred from greater distances (I, 13%; II, 27%; III, 27%; IV, 33%) (p < 0.001). The 2003 open group (n = 165) did not differ statistically with respect to region of origin (I, 18%; II, 41%; III, 21%; IV, 19%) compared to their 2003 EVAR counterparts (p = 0.075) but did have a higher proportion of patients from the more distant regions compared to the 1997 open group (p < 0.001). During the last 5 years, we have experienced a doubling of our elective aneurysm case volumes as well as a trend for patients to be referred from greater distances for both EVAR and open repair. This suggests a trend in Canada toward increased centralization of aneurysm care in centers providing both endovascular and open surgical alternatives.

As hemodialysis-dependent patients have a shorter life expectancy it has been recently questioned whether they benefit from procedures such as abdominal aortic aneurysm (AAA) repair. The purpose of this study was to review our results with elective AAA repair in hemodialysis-dependent patients. During a recent 6-year period (1998-2003), 7 such patients underwent elective repair of their infrarenal aortic aneurysms. Mean age and aneurysm diameter were 71 years and 69 mm, respectively. Mean length of stay was 28.6 days including routine intensive care unit admission. Three patients underwent standard open repair, and 3 underwent endovascular repair. In the seventh patient iliac calcification precluded endograft delivery and resulted in conversion to open repair. Another endovascular patient suffered from perforation of her sigmoid colon. One and 3-year survival rates in the entire cohort were 100% and 75%, respectively. The authors conclude that aneurysm repair in hemodialysis patients is warranted and life prolonging in appropriate risk patients, despite lengthy hospital stays. However, successful endovascular repair may be prevented by the severe arterial calcification in these patients.

The role of endovascular therapy in the management of inflammatory aneurysms of the infrarenal abdominal aorta has been controversial. Review of our endovascular database identified six patients who have undergone treatment for preoperatively diagnosed inflammatory abdominal aortic aneurysms. Outcomes measured were primary success of the procedure, variation in computed tomographic (CT) scan-defined perianeurysmal fibrosis, change in aneurysm size, development of endoleak, requirement of reintervention, aneurysm rupture, and progression or resolution of symptoms. At a median follow-up of 20 months (range 4-56 months), endovascular repair has been successful in all six patients. All patients demonstrated CT reduction of perianeurysmal fibrosis, with a median of 47% absolute reduction (range 33–69%, p = 0.014). All patients had aneurysm sac shrinkage, with a mean of 41% (range 6–86%, p = 0.04). There were no aneurysm ruptures or persistent endoleaks. Of the three patients who presented with abdominal or back pain, all are now symptom-free. One patient required reintervention for limb thrombosis of a bifurcated graft after 2 years. In conclusion, endovascular treatment of an inflammatory abdominal aortic aneurysm is safe and effective and the treatment of choice in anatomically suitable patients.

Purpose Ruptured abdominal aortic aneurysms (RAAAs) continue to result in early mortality in up to 50% of patients. Additionally, it remains difficult to compare outcomes given the variability in patient comorbidities and presentation. The purpose of this study was to describe an instrument that permits the prospective analysis of outcomes after RAAA repair while adjusting for the variability in preoperative risk. Methods Consecutive patients undergoing attempted open RAAA repair over a 5-year period (1999 to 2003) at our center were reviewed. Thirty-day or in-hospital mortality was the main outcome variable. Preoperative mortality risk was estimated for each patient by using a validated modification of the POSSUM scoring system (V-POSSUM). A risk-adjusted cumulative sum method (RA-CUSUM) was used to compare observed versus predicted outcomes by assigning a risk-adjusted score, based on log-likelihood ratios, to each patient. These scores were sequentially plotted with preset control limits to allow for "signaling" when results were substantially different from expected (doubling or halving of odds ratios). Results A total of 136 patients were reviewed, with an early mortality rate of 45.6%. V-POSSUM scores were accurate in predicting mortality for the entire cohort, with an observed-to-predicted mortality ratio of 0.92 ( P = .80). Each patient's risk-adjusted score was plotted sequentially. In one segment of the resulting plot, the graph adopted a negative slope and crossed the lower control limit, indicating improved results compared with predicted. Conclusions V-POSSUM scores in this series accurately predicted early mortality after RAAA surgery. The RA-CUSUM method allows for the prospective evaluation of outcomes, while taking into account patient variability. In the current study, this resulted in the identification of a series of patients who had improved outcomes compared with predicted.
