Background. The purpose was to determine the effect of drug therapy upon genetically determined disorders of bone tissue metabolism in patients with vitamin D-resistant rickets. Materials and methods. In the consultative out-patient department of the Institute of Traumatology and Orthopedics of the NAMSU, 39 patients who had been diagnosed with vitamin D-resistant rickets, phosphate diabetes (VDRR), were examined and treated. Medical treatment of the patients was carried out in several stages. Stage 1 included a patient complete examination to determine the level of calcium and phosphorus in blood and urine, calcidiol and calcitriol in blood, parathyroid hormone and osteocalcin parameters as well as the marker of bone formation P1NP and that of osteoresorption B-CTx. At the first stage, children were obliged to undergo a genetic study. The aim of the study was to detect changes (polymorphism) in receptor alleles to vitamin D (VDR) and collagen type 1 (COL1). The examination at the next stages was conducted entirely, in addition to genetic studies. Results. Comprehensive study of vitamin D metabolism and biochemical parameters of bone function in patients with VDRR, including the formation of the organic basis of bone, allowed examine thoroughly some issues of pathogenesis and the essence of osteomalacia and later osteoporotic changes of different degrees. Depending on these changes, it was possible to develop different regimens of medical correction of bone metabolism disorders in phosphate-diabetes. Conclusions. The treatment of orthopedic VDRR should begin with 60 000 units of vitamin D, 12 μg of alpha-calcidol and 18 g of calcium glycerophosphate per month. In 3 months of treatment, a re-examination of bone metabolism should be performed, resulting in alterations of vitamin D or the hormonal form of vitamin D alpha-calcidol intake. The calcium-phosphorus mixture should be consumed by children at an average dose of 600–800 mg per day., Цель: определить влияние консервативной терапии на генетически обусловленные нарушения метаболизма костной ткани у больных витамин-D-резистентным рахитом. Материалы и методы. На базе консультативно-поликлинического отдела ГУ «Институт травматологии и ортопедии НАМНУ» было обследовано и пролечено 39 пациентов с диагнозом «витамин-D-резистентный рахит, фосфат-диабет» (ВДPР). Консервативное лечение пациентов проводилось в несколько этапов. Первый этап включал полное обследование пациента на определение уровня кальция и фосфора в крови и моче, кальцидиола и кальцитриола крови, показателей паратиреоидного гормона и остеокальцина, а также маркера костеобразования P1NP и остеорезорбции B-CTx. На первом этапе в обязательном порядке дети проходили генетическое исследование. Целью исследования было выявление изменений (полиморфизма) в аллелях рецепторов к витамину D (VDR) и коллагену первого типа (COL1). Обследование на следующих этапах проводилось полностью, кроме генетических исследований. Результаты. Всестороннее изучение метаболизма витамина D и биохимических показателей жизнедеятельности костной ткани у больных ВДРР, включая формирование органической основы кости, позволило основательно изучить некоторые вопросы патогенеза и сущности остеомаляционных и в дальнейшем остеопоротических изменений разной степени. В зависимости от этих изменений удалось разработать различные схемы медикаментозной коррекции нарушений костного метаболизма при фосфат-диабете. Выводы. Лечение ортопедических проявлений витамин-D-резистентного рахита должно начинаться с 60 000 единиц витамина D, 12 мкг альфакальцидола и 18 г кальция глицерофосфата в месяц. После 3 месяцев лечения должно проводиться повторное изучение показателей костного метаболизма, в результате чего доза витамина D или гормональной формы витамина D (альфакальцидола) может меняться. Кальций-фосфорную смесь дети должны потреблять в средней дозе 600–800 мг в сутки., Мета: визначити вплив консервативної терапії на генетично зумовлені порушення метаболізму кісткової тканини у хворих на вітамін-D-резистентний рахіт. Матеріали та методи. На базі консультативно-поліклінічного відділу ДУ «Інститут травматології та ортопедії НАМНУ» були обстежені та проліковані 39 пацієнтів із діагнозом «вітамін-D-резистентний рахіт, фосфат-діабет» (ВДPР). Всебічне вивчення метаболізму вітаміну D та біохімічних показників життєдіяльності кісткової тканини у хворих на ВДРР, включаючи формування органічної основи кістки, дозволило ґрунтовно вивчити деякі питання патогенезу та сутності остеомаляційних і в подальшому остеопоротичних змін різного ступеня. Залежно від цих змін вдалось розробити різні схеми медикаментозної корекції порушень кісткового метаболізму при фосфат-діабеті. Висновки. Лікування ортопедичних проявів вітамін D-резистентного рахіту повинно розпочинатись з 60 000 одиниць вітаміну D, 12 мкг альфакальцидолу та 18 г кальцію гліцерофосфату на місяць. Після 3 місяців лікування повинно проводитись повторне вивчення показників кісткового метаболізму, внаслідок чого доза вітаміну D чи гормональної форми вітаміну D (альфакальцидолу) може змінюватись. Кальцій-фосфорну суміш діти повинні споживати в середній дозі 600–800 мг на добу.

Цель исследования: улучшение результатов хирургического лечения многоплоскостных деформаций у детей с сохранившимися зонами роста при рахитических деформациях. Материалы и методы. В ГУ «Институт травматологии и ортопедии НАМН Украины» с 2013 по 2019 год пролечено 6 пациентов с витамин-D-зависимым (2) и витамин-D-резистентным (4) рахитом в возрасте от 3 до 9 лет с многоплоскостными деформациями нижних конечностей, в значительной мере нарушающими функцию ходьбы и биомеханику нижних конечностей. С целью коррекции деформаций нижних конечностей у всех пациентов проведены сегментарные коррекционные остеотомии каждого из сегментов нижних конечностей (24). Фиксация проводилась интрамедуллярным «растущим» стержнем, который давал возможность зафиксировать конечность после оперативного вмешательства, не влияя в дальнейшем на рост ребенка. В 2 случаях использовался запатентованный стержень украинского производства, в 4 — прототип интрамедуллярного телескопического стержня Fassier-Duval. Результаты. У оперированных пациентов до начала лечения функция ходьбы и опоры была значительно ухудшена, после оперативных вмешательств с применением интрамедуллярных «растущих» конструкций восстановлена функция ходьбы и опоры, устранены многоплоскостные деформации. Всем пациентам проводилось консервативное лечение метаболических нарушений костной ткани. Во всех случаях остеотомий получено сращение в соответствующие сроки, рост бедренной и большеберцовой костей в длину не нарушался, раздвижение интрамедуллярных конструкций проходит удовлетворительно, ни у одного пациента за период наблюдения не было повторных переломов и деформаций длинных костей нижних конечностей, миграции элементов металлоконструкций. Максимальный срок наблюдения послеоперационного периода у наших пациентов составлял 1,5 года с начала оперативных вмешательств. Выводы. При хирургическом лечении многоплоскостных деформаций нижних конечностей мы рекомендуем использовать многоуровневые остеотомии с металлоостеосинтезом телескопическим интрамедуллярным стержнем, позволяющим провести одномоментную коррекцию рахитической деформации и избежать повреждений зоны роста., Мета дослідження: покращення результатів хірургічного лікування багатоплощинних деформацій у дітей зі збереженими зонами росту при рахітичних деформаціях. З метою корекції деформацій нижніх кінцівок у всіх пацієнтів проведені сегментарні корекційні остеотомії кожного із сегментів нижніх кінцівок (24). В оперованих пацієнтів до початку лікування функція ходьби й опори була значно погіршена, після оперативних втручань із застосуванням інтрамедулярних «ростучих» конструкцій відновлено функцію ходьби й опори, усунуті багатоплощинні деформації. Усім пацієнтам проводилось консервативне лікування метаболічних порушень кісткової тканини. В усіх випадках остеотомій отримано зрощення у відповідні терміни, ріст стегнової та великогомілкової кісток у довжину не порушувався, розсування інтрамедулярних конструкцій відбувається задовільно, у жодного пацієнта за період спостереження не було повторних переломів і деформацій довгих кісток нижніх кінцівок, міграції елементів металоконструкцій. Максимальний термін спостереження післяопераційного періоду в наших пацієнтів становив 1,5 року від початку оперативних втручань. Висновки. При хірургічному лікуванні багатоплощинних деформацій нижніх кінцівок ми рекомендуємо використовувати багаторівневі остеотомії з металоостеосинтезом телескопічним інтрамедулярним стрижнем, що дозволяє провести ­одномоментну корекцію рахітичної деформації і уникнути пошкоджень зони росту., Background. The purpose was to improve the results of surgical treatment for multidimensional deformities in children with open growth plates in rachitic diseases. Materials and methods. In the State Institution “Institute of Traumatology and Orthopedics of the National Academy of Medical Sciences of Ukraine” from 2013 till 2019, we have treated 6 patients aged 3 to 9 years with vitamin D-dependent (2) and vitamin D-resistant (4) rickets with multidimensional deformities of lower limbs that significantly affected gait function and lower limb biomechanics. In order to correct the deformities of the lower extremities, all patients underwent segmental corrective osteotomies of every segment of lower limbs (24). Fixation was performed by an intramedullary telescopic rod, which gave the possibility to fix the limb after surgery, without affecting the further growth of the child. In 2 cases, a patented rod of Ukrainian production was used, in four cases — a prototype of Fassier-Duval intramedullary telescopic rod. Results. In operated patients, walking and support function was significantly impaired prior to treatment, after surgery with the use of intramedullary telescopic rods, the function of gait and support is restored, multidimensional deformities are eliminated. All patients underwent conservative treatment for bone metabolic disorders. In all cases of osteotomy, healing occurred at proper time, the growth of the femur and tibia is not disturbed in length, moving of the intramedullary structures is satisfactory, no patient had recurrent fractures and deformities of the lower extremity long bones during the observation period, no migration of the elements of metal structures was detected. The maximum postoperative follow-up period in our patients was 1.5 years from the start of surgery. Conclusions. In the surgical treatment for multidimensional deformities of the lower extremities, we recommend using multilevel osteotomies with metal osteosynthesis by intramedullary telescopic rod, which allows for a one-time correction of rickets and helps avoid the damage to the growth plate.

X-linked hypophosphataemia (XLH) is the most common cause of inherited phosphate wasting and is associated with severe complications such as rickets, lower limb deformities, pain, poor mineralization of the teeth and disproportionate short stature in children as well as hyperparathyroidism, osteomalacia, enthesopathies, osteoarthritis and pseudofractures in adults. The characteristics and severity of XLH vary between patients. Because of its rarity, the diagnosis and specific treatment of XLH are frequently delayed, which has a detrimental effect on patient outcomes. In this Evidence-Based Guideline, we recommend that the diagnosis of XLH is based on signs of rickets and/or osteomalacia in association with hypophosphataemia and renal phosphate wasting in the absence of vitamin D or calcium deficiency. Whenever possible, the diagnosis should be confirmed by molecular genetic analysis or measurement of levels of fibroblast growth factor 23 (FGF23) before treatment. Owing to the multisystemic nature of the disease, patients should be seen regularly by multidisciplinary teams organized by a metabolic bone disease expert. In this article, we summarize the current evidence and provide recommendations on features of the disease, including new treatment modalities, to improve knowledge and provide guidance for diagnosis and multidisciplinary care., In this Evidence-Based Guideline on X-linked hypophosphataemia, the authors identify the criteria for diagnosis of this disease, provide guidance for medical and surgical treatment and explain the challenges of follow-up.

ᅟOBJECTIVES: Vitamin-D-dependent rickets type 1A (VDDR1A) is a rare inherited disease caused by defective activation of vitamin D. The aim of the study was to describe the craniofacial characteristics and the dental phenotype of patients with genetically confirmed VDDR1A. The VDDR1A findings were compared to findings in patients with X-linked hypophosphatemia (XLH) and healthy controls.MATERIAL AND METHODS: Ten patients with VDDR1A were identified. The reference group for the comparison of cephalometric findings was 49 adults without chronic disease. The reference group for the comparison of dental findings was 30 adults with XLH. Clinical examination, clinical photos, and radiographs were obtained. Cephalometric analysis was performed. Photos and radiographs were visually evaluated.RESULTS: The depth of the posterior cranial fossa (d-p and d-s-iop) in VDDR1A adults was reduced compared to the reference group (p < 0.05). Five (83%) of six adults with VDDR1A and one (4%) of 25 adults with XLH had enamel hypoplasia on several incisors and/or canines (p < 0.001). Three (75%) of four adults with VDDR1A and none of 16 adults with XLH had several first molars with enamel hypoplasia (p = 0.004). Five of 7 (71%) adults with VDDR1A and 24 of 30 (80%) adults with XLH had endodontically affected teeth.CONCLUSIONS: The dental aberration of VDDR1A is more in line with the dental aberration of nutritional rickets than with the dental aberrations in XLH, suggesting the combination of low availability of both calcium and phosphate to be critical in periods of enamel formation.CLINICAL RELEVANCE: Knowledge on craniofacial and dental aberration in patients with rare diseases, e.g., inherited rickets, is of importance to the dental practitioner, especially during diagnostics and treatment in special care units.

An autosomal recessive form of hypophosphatemia (ARHP) was recently shown to be caused by homozygous mutations in DMP1, the gene encoding dentin matrix protein-1 (DMP-1), a non-collagenous bone matrix protein with an important role in the development and mineralization of bone and teeth. Here, we describe a previously not reported consanguineous ARHP kindred in which the three affected individuals carry a novel homozygous DMP-1 mutation. The index case presented at the age of 3 years with bowing of his legs and showed hypophosphatemia due to insufficient renal phosphate retention. Serum alkaline phosphatase activity was elevated, with initially normal PTH. FGF23 was inappropriately normal at an older age while being treated with oral phosphate and 1,25(OH)(2)D. Similar clinical and biochemical findings, except for elevated FGF23 levels, were present in his 16-month-old brother and his 12.5-year-old female cousin; the parents of the three affected children are first-degree cousins. Nucleotide sequence analysis was performed on PCR-amplified exons encoding DMP-1 and flanking intronic regions. A novel homozygous frame-shift mutation (c.485Tdel; p.GLu163ArgfsX53) in exon 6 resulting in a premature stop codon was identified in all effected individuals. The parents and available unaffected siblings were heterozygous for c.485Tdel. Tooth growth and shape were normal for the index case, his affected brother and cousin, but their permanent and deciduous teeth displayed enlarged pulp chambers. The identified genetic mutation underscores the importance of DMP-1 mutations in the pathogenesis of ARHP. Furthermore, DMP-1 mutations appear to contribute, through yet unknown mechanisms, to tooth development. (C) 2009 Elsevier Inc. All rights reserved.
