In this paper we study stellar variability in the globular cluster NGC 3201 in the magnitude range V = 16–17, corresponding to the subgiant branch and blue stragglers region of the cluster. Our aim is to expand the sample of new type of variables with low amplitude and a short period found in previous papers. We used observations obtained at the Complejo Astronómico El Leoncito. We applied statistical tests and analyzed periodograms obtained by generalized Lomb–Scargle and phase dispersion minimization methods. We found five stars considered as variables and one as a possible variable. According to their periods (fluctuating between 0.33 and 0.57 days), amplitudes (between 0.03 and 0.87 V magnitudes), the position in the color–magnitude diagram, and the shape of their phased light curves, they do not resemble any known variable star. Because stellar variability could be produced by more than one process, we propose to complement this work with a spectroscopic analysis to confirm our results.

The presence of rare earth elements (REE) in the southern hemisphere, particularly marine ecosystems of Patagonia, have received little attention. The Magellanic penguin, which is also known as the Patagonian penguin, inhabits only in austral regions of South America. Although seabird feathers have been used extensively as a bio-monitoring tool, no studies have addressed the effect of age on REE accumulation in Magellanic penguins. In this study, the concentrations of REE were determined by ICP-MS to detect La, Ce, Pr, Nd, Sm, Eu, Gd, Tb, Dy, Ho, Er, Tm, Yb, Sc, Lu and Y in the feathers of Magellanic penguins from Magdalena Island, an important rookery in the Strait of Magallan. Age-related differences were studied to investigate the different patterns of REE bioaccumulation between adults and juveniles. The data showed that juvenile penguins exhibit higher REE-levels than adult individuals (p < 0.05). Mean REE-values (μg g -1 d.w) differed several orders of magnitude, ranging from 0.002 for Lu in adults to 1.15 for Ce in juvenile individuals. Objective: This systematic review and meta-analysis aimed to (I) evaluate the evidence on the effects of high-intensity interval training (HIIT) on systolic blood pressure (SBP) and diastolic blood pressure (DBP) in hypertensive patients; (II) determine whether HIIT impacts SBP and DBP differently; and (III) assess the clinical relevance of these effects., Methods: A comprehensive search was conducted across multiple electronic databases, resulting in the inclusion of seven randomized clinical trials in the meta-analysis. The outcomes were analyzed using random-effects models to compute mean differences (MD) and standardized mean differences (SMD) for SBP and DBP., Results: A small reduction in SBP was observed with HIIT interventions (MD -3.00; 95% CI -4.61 to -1.39; p < 0.0001; SMD -0.28; 95% CI -0.42 to -0.13; p = 0.0003). However, no statistically significant reductions were detected for DBP (MD -0.70; 95% CI -1.80 to 0.39; p = 0.21; SMD -0.07; 95% CI -0.22 to 0.08; p = 0.35). Despite demonstrating statistical significance for SBP, the effects did not reach clinical relevance., Conclusions: HIIT interventions yield small reductions in SBP, with minimal impact on DBP. These findings suggest limited clinical relevance in the management of hypertension. Further randomized controlled trials are necessary to standardize HIIT protocols, with specific emphasis on intensity control and manipulation, to better understand their potential role in hypertensive populations.

The objective of this study is to analyse the kinetic effects of acute fatigue during a 45° change of direction executed with the non-dominant limb, emulating a typical defensive action during pressing in soccer. Seventeen male professional soccer players (age: 21.7 ± 5.4 years) performed a 45° change of direction before and after a fatigue protocol. Participants were instructed to execute an approach run as quickly as possible, to change direction with their non-dominant foot on a force platform, and then continue running with the aim of stopping against an opponent. Times, forces, impulses, and force rates for different subphases during support were compared. Speeds remained consistent across pre-fatigue (4.53 ± 0.55 m s- 1) and post-fatigue (4.58 ± 0.70 m s- 1 ) trials. The results indicated a decrease in braking time (p = 0.01), impulse associated with the braking phase (p < 0.01), and impulse during body weight lifting (p = 0.01) under acute fatigue conditions. These changes suggest that, under acute fatigue, soccer players may not decelerate sufficiently in the initial direction of the run and may raise their body less in the first step after the change of direction. Background: Systematizing reliable protocols and procedures for strength assessment in handball has allowed for a more thorough kinetic analysis and increased precision in detecting training-induced changes in muscular strength. The evaluation of upper limb strength with a bilateral standing press (BSP) exercise in handball players approximates blocking actions in the defensive phase, pushing and fixation., Aim: The aim of this study was to analyze the relative and absolute reliability of intra-session comparisons in an isometric peak muscle strength protocol in a bilateral standing press (BSP) exercise among female handball players., Methods: Sixteen young female handball players at an international level, aged between 22 ± 4 years, with no prior experience in using functional electromechanical dynamometers (FEMD) participated in this study. The participants initiated the assessment of maximum isometric force in a bipedal stance with a forward projection of the dominant foot between 20 and 30 cm. The knees were kept semi-flexed, and the hip extension of the non-dominant limb ranged from 15 to 20°. Bilateral anterior push of the upper limb was performed with shoulder abduction and elbow flexion at 90°, maintaining a pronated hand position. Participants were instructed to exert three sets of maximal force for 5 s in BSP exercise. Relative reliability was assessed using the model intraclass correlation (ICC) and absolute reliability was assessed using the coefficient of variation (CV) and standard error of measurement (SEM). For this study, the parameters of maximum and mean muscle strength were considered., Results: The results demonstrated high relative reliability (ICC 0.93-0.97) and absolute reliability (SEM 0.19-2.79) y (CV 4.78-9.03) for both mean force and peak force, with no significant differences between the sets ( p > 0.05), indicating a negligible effect size (0.01-0.12)., Conclusion: The mean and peak isometric muscle strength for the BSP exercise controlled with FEMD in female handball players exhibits high relative and absolute reliability between series., Competing Interests: The authors declare that they have no competing interests., (© 2024 Cifuentes-Zapata et al.)

Many group-living animals coordinate social behaviours using contact calls, which can be produced for all group members or targeted at specific individuals. In the disc-winged bat, Thyroptera tricolor , group members use 'inquiry' and 'response' calls to coordinate daily movements into new roosts (furled leaves). Rates of both calls show consistent among-individual variation, but causes of within-individual variation remain unknown. Here, we tested whether disc-winged bats produce more contact calls towards group members with higher kinship or association. In 446 experimental trials, we recorded 139 random within-group pairs of one flying bat (producing inquiry calls for roost searching) and one roosting bat (producing response calls for roost advertising). Using generalized linear mixed-effect models (GLMM), we assessed how response and inquiry calling rates varied by sender, receiver, genetic kinship and co-roosting association rate. Calling rates varied consistently across senders but not by receiver. Response calling was influenced by inquiry calling rates, but neither calling rate was higher when the interacting pair had higher kinship or association. Rather than dyadic calling rates indicating within-group relationships, our findings are consistent with the hypothesis that bats produce contact calls to maintain contact with any or all individuals within a group while collectively searching for a new roost site. The purpose of the study was to analyze the test-retest reliability of an isometric and isometric/vibratory muscular strength protocol in the bilateral seated bench press (BSBP), bilateral seated rowing (BSR), unilateral seated right knee extension (USKER), and left knee extension (USKEL) tests controlled using functional electromechanical dynamometry (FEMD) in healthy young adults. A repeated measures design was used to determine the reliability of a muscular strength protocol in isometric and isometric vibration modes with FEMD. No significant differences were found in test-retest analysis ( p > 0.05; ES < 0.20); and high reliability (CV = 4.65-5.02%; ICC = 0.99-0.98) was found for BSBP measures, and acceptable reliability (CV = 3.71-9.61%; ICC = 0.98-0.95) was found for BSR, USKER, and USKEL. Furthermore, the coefficients between the two measures were strong (r = 0.963-0.839) and highly significant ( p = 0.001) for maximal strength in the isometric and maximal isometric/vibratory assessment of muscle strength in all muscle strength tests. This study demonstrates that isometric and maximal isometric/vibratory strength in the BSBP, BSR, USKER, and USKEL tests can be measured with high reliability and reproducibility using the FEMD.

Gallbladder cancer (GBC) is a very aggressive malignant neoplasm of the biliary tract with a poor prognosis. There are no specific therapies for the treatment of GBC or early diagnosis tools; for this reason, the development of strategies and technologies that facilitate or allow an early diagnosis of GBC continues to be decisive. Phage display is a robust technique used for the production of monoclonal antibodies (mAbs) involving (1) the generation of gene libraries, (2) the screening and selection of isoforms related to an immobilized antigen, and (3) the in vitro maturation of the affinity of the antibody for the antigen. This research aimed to construct a human immune library from PBMCs of GBC patients and the isolation of scFv-phage clones with specificity against the larger extracellular loop belonging to claudin 18.2, which is an important biomarker overexpressed in GBC as well as gastric cancer. The immune-library-denominated GALLBLA1 was constructed from seven GBC patients and has a diversity of 6.12 × 10 10 pfu mL -1 . After three rounds of panning, we were able to identify clones with specificity against claudin 18.2. GALLBLA1 can contribute to the selection, isolation, and recombinant production of new human mAbs candidates for the treatment of gastrointestinal cancers.

Cancer is one of the leading global causes of death and disease, and treatment options are constantly evolving. In this sense, the use of monoclonal antibodies (mAbs) in immunotherapy has been considered a fundamental aspect of modern cancer therapy. In order to avoid collateral damage, it is indispensable to identify specific molecular targets or biomarkers of therapy and/or diagnosis (theragnostic) when designing an appropriate immunotherapeutic regimen for any type of cancer. Furthermore, it is important to understand the currently employed mAbs in immunotherapy and their mechanisms of action in combating cancer. To achieve this, a comprehensive understanding of the biology of cancer cell antigens, domains, and functions is necessary, including both those presently utilized and those emerging as potential targets for the design of new mAbs in cancer treatment. This review aims to provide a description of the therapeutic targets utilized in cancer immunotherapy over the past 5 years, as well as emerging targets that hold promise as potential therapeutic options in the application of mAbs for immunotherapy. Additionally, the review explores the mechanisms of actin of the currently employed mAbs in immunotherapy.

Purpose: This study aimed to examine the reliability of trunk extensor strength assessment with a functional electromechanical dynamometer (FEMD)., Methods: Thirty-one men performed strength assessment at different velocities (V) (V1 = 0.15 m⋅s-1 , V2 = 0.30 m⋅s-1,V3 = 0.45 m⋅s-1) and range of movement (R) (R1 = 25% cm; R2 = 50% cm), and isometric contraction at 90°. Reliability was obtained through the intraclass correlation coefficient (ICC), typical error (TE), and coefficient of variation (CV)., Results: The absolute reliability provided stable repeatability of the average eccentric strength in the V1R1 condition (CV = 9.52%) and the maximum eccentric strength in V1R1 (CV = 9.63%) and V2R2 (CV = 9.66%). The relative reliability of the trunk extensor's average strength was good (ICC = 0.77-0.83) for concentric and good (ICC = 0.78-0.85) and moderate (ICC = 0.67-0.74) for eccentric contraction. Also, good (ICC = 0.77-0.81) and moderate (ICC = 0.55-0.74) reliability of the maximum strength were obtained for concentric and eccentric contraction. The most reliable manifestation to evaluate the concentric (CV = 11.33%) and eccentric (CV = 9.52%) strength was the average strength in the V1R1 condition and the maximum strength (CV = 10.29%) to isometric assessment. The average concentric strength in the V2R2 condition (r = 0.69) and the maximum eccentric strength in the V1R1 condition (r = 0.65) were the best related to the maximum isometric strength., Conclusions: FEMD is a highly reliable device to evaluate trunk extensors strength.

For centuries, numerous treatments were used, of the most varied origins and based on the most unlikely foundations designed to hasten recovery and improve the survival of patients with whooping cough. In 1906, when the bacterial origin of whooping cough was identified, the hope for a potential treatment arose, however, only decades later, humanity would face a significant change with the con solidation of an effective vaccination. This article provides a historical review from the use of some of the most frequent treatments devised between the 18th-20th centuries for the cure of whooping cough. Some of the therapies offered in Chile in the first half of the last century are emphasized and detailed.

Throwing velocity is one of the most important factors for scoring goals in handball. This study aimed to identify the type of throw and procedure for selecting the final test outcome that provide throwing velocity with the greatest reliability. Fifteen experienced handball players and 33 non-experienced participants were tested in two sessions. Each session consisted of 4 trials of 3 different throwing tests (unspecific, 7-meters, and 3-steps). The maximum value of 4 trials, average value of 4 trials, and average value of the 3 best trials were considered. Throwing velocity was highly reliable (coefficient of variation [CV]≤3.3%, intraclass correlation coefficient≥0.89) with the exception of the unspecific throw for the non-experienced group (CV≥5.9%, intraclass correlation coefficient≤0.56). The 3-steps throw (CV=1.7%) was more reliable than the 7-meters throw (CV=2.1%) (CV ratio =1.19) and unspecific throw (CV = 3.8%) (CV ratio =2.18), the 3 procedures provided a comparable reliability (CV range=2.4-2.6%; CV ratio ≤1.07), and the experienced group (CV=1.0%) presented a higher reliability than the non-experienced group (CV=4.0%) (CV ratio =3.83). Background: The evaluation of the force in internal rotation (IR) and external rotation (ER) of the shoulder is commonly used to diagnose possible pathologies or disorders in the glenohumeral joint and to assess patient's status and progression over time. Currently, there is new technology of multiple joint isokinetic dynamometry that allows to evaluate the strength in the human being. The main purpose of this study was to determine the absolute and relative reliability of concentric and eccentric internal and external shoulder rotators with a functional electromechanical dynamometer (FEMD)., Methods: Thirty-two male individuals (21.46 ±  2.1 years) were examined of concentric and eccentric strength of shoulder internal and external rotation with a FEMD at velocities of 0.3 m s -1 and 0.6 m s -1 . Relative reliability was determined by intraclass correlation coefficients (ICC). Absolute reliability was quantified by standard error of measurement (SEM) and coefficient of variation (CV). Systematic differences across velocities testing circumstances, were analyzed with dependent t tests or repeated-measures analysis of variance in case of 2 or more than 2 conditions, respectively., Results: Reliability was high to excellent for IR and ER on concentric and eccentric strength measurements, regardless of velocity used (ICC: 0.81-0.98, CV: 5.12-8.27% SEM: 4.06-15.04N). Concentric outcomes were more reliable than eccentric due to the possible familiarization of the population with the different stimuli., Conclusion: All procedures examined showed high to excellent reliability for clinical use. Acquisition of mobile elements by horizontal gene transfer can play a major role in bacterial adaptation and genome evolution by providing traits that contribute to bacterial fitness. However, gaining foreign DNA can also impose significant fitness costs to the host bacteria and can even produce detrimental effects. The efficiency of horizontal acquisition of DNA is thought to be improved by the activity of xenogeneic silencers. These molecules are a functionally related group of proteins that possess affinity for the acquired DNA. Binding of xenogeneic silencers suppresses the otherwise uncontrolled expression of genes from the newly acquired nucleic acid, facilitating their integration to the bacterial regulatory networks. Even when the genes encoding for xenogeneic silencers are part of the core genome, homologs encoded by horizontally acquired elements have also been identified and studied. In this article, we discuss the current knowledge about horizontally acquired xenogeneic silencer homologs, focusing on those encoded by genomic islands, highlighting their distribution and the major traits that allow these proteins to become part of the host regulatory networks.

Background: The functional fitness of older people may be associated with their nutritional status., Aim: To assess the association between of anthropometric measures with functional fitness in older people., Material and Methods: Cross-sectional study conducted in 75 participants aged 65 to 89 years. Body mass index (BMI), waist-to-height ratio (WHtR), fat mass (FM) and skeletal muscle mass index (SMI) were calculated from anthropometric measures. The functional fitness was determined using the Senior Fitness Test battery., Results: BMI and FM indicated obesity, and WHtR indicated cardiometabolic risk in 49%, 55% and 83% of participants, respectively. SMI indicated a low muscle mass in 91% of females. Performance standards of chair stand, arm curl, 2-min step test and 8-foot up-and-go tests were met in 1%, 8%, 1% and 89% of participants, respectively. Significant negative correlations were found between 2-min step test and BMI, WHtR and FM (r = -0.26, -0.31 and -0.48 respectively). Back scratch had a negative correlation with BMI (r = -0.23) and SMI (rho = -0.28). Significant positive correlations were found between 8-foot up-and-go, WHtR (rho = 0.28) and FM (rho = 0.23), and between 2-min step test and SMI (rho = 0.28). The coefficient of determination (R2) between 2-min step test with BMI, WHtR and FM were 0.05, 0.08 and 0.22, respectively, while the R2 between back scratch and BMI was 0.04. Multiple regression models indicated that FM affected the 2-min step test independently of BMI and WHtR (adjusted R2 = 0.22), however age and sex negatively influenced these associations., Conclusions: Functional fitness of older adults is influenced by nutritional anthropometric measures, particularly BMI, WHtR and FM for aerobic capacity, and BMI for upper limb flexibility.

Aim: To determine the absolute and relative reliability of functional trunk tests, using a functional electromechanical dynamometer to evaluate the isokinetic strength of trunk flexors and to determine the most reliable assessment condition, in order to compare the absolute and relative reliability of mean force and peak force of trunk flexors and to determine which isokinetic condition of evaluation is best related to the maximum isometric., Methods: Test-retest of thirty-seven physically active male student volunteers who performed the different protocols, isometric contraction and the combination of three velocities (V 1 = 015 m s -1 , V 2 = 0.30 m  s -1 , V 3 = 0.45 m s -1 ) and two range of movement (R 1 = 25% cm ; R 2 = 50% cm) protocols., Results: All protocols to evaluate trunk flexors showed an absolute reliability provided a stable repeatability for isometric and dynamic protocols with a coefficient of variation (CV) being below 10% and a high or very high relative reliability (0.69 < intraclass correlation coefficient [ICC] > 0.86). The more reliable strength manifestation (CV = 6.82%) to evaluate the concentric contraction of trunk flexors was mean force, with 0.15 m  s -1 and short range of movement (V 1 R 1 ) condition. The most reliable strength manifestation to evaluate the eccentric contraction of trunk flexors was peak force, with 0.15 m  s -1 and a large range of movement (V 1 R 2; CV = 5.07%), and the most reliable way to evaluate isometric trunk flexors was by peak force (CV = 7.72%). This study examined the reliability and validity of three methods of estimating the one-repetition maximum (1RM) during the free-weight prone bench pull exercise. Twenty-six men (22 rowers and four weightlifters) performed an incremental loading test until reaching their 1RM, followed by a set of repetitions-to-failure. Eighteen participants were re-tested to conduct the reliability analysis. The 1RM was estimated through the lifts-to-failure equations proposed by Lombardi and O'Connor, general load-velocity (L-V) relationships proposed by Sánchez-Medina and Loturco and the individual L-V relationships modelled using four (multiple-point method) or only two loads (two-point method). The direct method provided the highest reliability (coefficient of variation [CV] = 2.45% and intraclass correlation coefficient [ICC] = 0.97), followed by the Lombardi's equation (CV = 3.44% and ICC = 0.94), and no meaningful differences were observed between the remaining methods (CV range = 4.95-6.89% and ICC range = 0.81-0.91). The lifts-to-failure equations overestimated the 1RM (3.43-4.08%), the general L-V relationship proposed by Sánchez-Medina underestimated the 1RM (-3.77%), and no significant differences were observed for the remaining prediction methods (-0.40-0.86%). The individual L-V relationship could be recommended as the most accurate method for predicting the 1RM during the free-weight prone bench pull exercise.

An adequate drug dosage at treatment initiation is particularly relevant for critically ill patients. An inadequate dosage may result in therapeutic failure, potentially severe adverse events, and unnecessary health expenditures. At present, due to the higher incidence of childhood obesity, primary care physicians are more commonly faced with this population, so they need to make appropriate therapeutic decisions. Knowledge of the resulting pharmacokinetic alterations caused by increased body fat is critical. The optimal drug dosage is not completely defined and the correct body descriptor should be used, although there is no consensus on which is the most adequate one. This aims of this study were (I) to determine the velocity variable and regression model which best fit the load-velocity relationship during the free-weight prone bench pull exercise, (II) to compare the reliability of the velocity attained at each percentage of the one-repetition maximum (1RM) between different velocity variables and regression models, and (III) to compare the within- and between-subject variability of the velocity attained at each %1RM. Eighteen men (14 rowers and four weightlifters) performed an incremental test during the free-weight prone bench pull exercise in two different sessions. General and individual load-velocity relationships were modelled through three velocity variables (mean velocity [MV], mean propulsive velocity [MPV] and peak velocity [PV]) and two regression models (linear and second-order polynomial). The main findings revealed that (I) the general (Pearson's correlation coefficient [r] range = 0.964-0.973) and individual (median r = 0.986 for MV, 0.989 for MPV, and 0.984 for PV) load-velocity relationships were highly linear, (II) the reliability of the velocity attained at each %1RM did not meaningfully differ between the velocity variables (coefficient of variation [CV] range = 2.55-7.61% for MV, 2.84-7.72% for MPV and 3.50-6.03% for PV) neither between the regression models (CV range = 2.55-7.72% and 2.73-5.25% for the linear and polynomial regressions, respectively), and (III) the within-subject variability of the velocity attained at each %1RM was lower than the between-subject variability for the light-moderate loads. No meaningful differences between the within- and between-subject CVs were observed for the MV of the 1RM trial (6.02% vs. 6.60%; CVratio = 1.10), while the within-subject CV was lower for PV (6.36% vs. 7.56%; CVratio = 1.19). These results suggest that the individual load-MV relationship should be determined with a linear regression model to obtain the most accurate prescription of the relative load during the free-weight prone bench pull exercise., Competing Interests: The authors have declared that no competing interests exist.

Background: The aim of this study was to examine whether the addition of very light loads for modeling the force-velocity (F-V) relationship during the bench press (BP) exercise can confirm its experimental linearity as well as to increase the reliability and concurrent validity of the F-V relationship parameters (maximum force ( F 0 ), maximum velocity ( V 0 ), F-V slope, and maximum power ( P max ))., Method: The F-V relationship of 19 healthy men were determined using three different methods: (I) 6-loads free method: six loads performed during the traditional free-weight BP exercise (≈ 1-8-29-39-49-59 kg), (II) 4-loads free method: four loads performed during the traditional free-weight BP exercise (≈ 29-39-49-59 kg), and (III) 4-loads Smith method: four loads performed during the ballistic bench press throw exercise in a Smith machine (≈ 29-39-49-59 kg)., Results: The linearity of the F-V relationship was very high and comparable for the three F-V methods ( p = 0.204; median Pearson's correlation coefficient ( r ) = 0.99). The three methods were ranked from the most to the least reliable as follows: 6-loads free (coefficient of variation (CV) range = 3.6-6.7%) > 4-loads Smith (CV range = 4.6-12.4%) > 4-loads free (CV range = 3.8-14.5%). The higher reliability of the 6-loads free method was especially pronounced for F-V slope (CV ratio ≥ 1.85) and V 0 (CV ratio ≥ 1.49) parameters, while the lowest difference in reliability was observed for F 0 (CV ratio ≤ 1.27). The 6-loads free and 4-loads free methods showed a very high concurrent validity respect to the 4-loads Smith method for F 0 and P max ( r ≥ 0.89), a moderate validity for the F-V slope ( r = 0.66-0.82), and a low validity for V 0 ( r ≤ 0.37)., Discussion: The routine testing of the F-V relationship of upper-body muscles through the BP exercise should include trials with very light loading conditions to enhance the reliability of the F-V relationship., Competing Interests: The authors declare that they have no competing interests.

Genomic Islands (GIs) are DNA regions acquired through horizontal gene transfer that encode advantageous traits for bacteria. Many GIs harbor genes that encode the molecular machinery required for their excision from the bacterial chromosome. Notably, the excision/integration dynamics of GIs may modulate the virulence of some pathogens. Here, we report a novel family of GIs found in plant and animal Enterobacteriaceae pathogens that share genes with those found in ROD21, a pathogenicity island whose excision is involved in the virulence of Salmonella enterica serovar Enteritidis. In these GIs we identified a conserved set of genes that includes an excision/integration module, suggesting that they are excisable. Indeed, we found that GIs within carbapenem-resistant Klebsiella pneumoniae ST258 KP35 and enteropathogenic Escherichia coli O127:H6 E2348/69 are excised from the bacterial genome. In addition to putative virulence factors, these GIs encode conjugative transfer-related proteins and short and full-length homologues of the global transcriptional regulator H-NS. Phylogenetic analyses suggest that the identified GIs likely originated in phytopathogenic bacteria. Taken together, our findings indicate that these GIs are excisable and may play a role in bacterial interactions with their hosts.

Down syndrome is the most common chromosomal abnormality in newborns, with a high incidence in Chile. This condition presents unique physiological aspects that should be known, which can affect the child during their stay in an Intensive Care Unit, beyond the neonatal period This review is focused on the respiratory, cardiovascular, infectious and neurological disorders. Anesthetic management and postoperative analgesia considerations, weaning from mechanical ventilation, cervical spine instability and prognosis of the critically ill child with Down syndrome are also analyzed. The evaluation of these conditions should be performed when the patient is admitted to the intensive care unit. The purpose of this update is to update the knowledge of the diagnosis and treatment of potential complications of children with Down syndrome during their stay in the unit of critical patient.

Background: There is no conclusive evidence about the association between physical fitness (PF) and health related quality of life (HRQOL) in older adults., Aim: To seek for an association between PF and HRQOL in non-disabled community-dwelling Chilean older adults., Material and Methods: One hundred and sixteen subjects participated in the study. PF was assessed using the Senior Fitness Test (SFT) and hand grip strength (HGS). HRQOL was assessed using eight dimensions provided by the SF-12v2 questionnaire. Binary multivariate logistic regression models were carried out considering the potential influence of confounder variables., Results: Non-adjusted models, indicated that subjects with better performance in arm curl test (ACT) were more likely to score higher on vitality dimension (OR > 1) and those with higher HGS were more likely to score higher on physical functioning, bodily pain, vitality and mental health (OR > 1). The adjusted models consistently showed that ACT and HGS predicted a favorable perception of vitality and mental health dimensions respectively (OR > 1)., Conclusions: HGS and ACT have a predictive value for certain dimensions of HRQOL.

Respiratory monitoring plays an important role in the care of children with acute respiratory failure. Therefore, its proper use and correct interpretation (recognizing which signals and variables should be prioritized) should help to a better understanding of the pathophysiology of the disease and the effects of therapeutic interventions. In addition, ventilated patient monitoring, among other determinations, allows to evaluate various parameters of respiratory mechanics, know the status of the different components of the respiratory system and guide the adjustments of ventilatory therapy. In this update, the usefulness of several techniques of respiratory monitoring including conventional respiratory monitoring and more recent methods are described. Moreover, basic concepts of mechanical ventilation, their interpretation and how the appropriate analysis of the information obtained can cause an impact on the clinical management of the patient are defined., (Copyright © 2016 Hospital Infantil de México Federico Gómez. Background: The regular practice of physical activity contributes to weight control and improves maximal oxygen uptake (VO2max), socialization and quality of life., Aim: To determine the effect of moderate-to-vigorous physical activity on body composition and VO2max in a group of overweight school-age males., Material and Methods: We studied fifty-five overweight males aged 15.6 ± 0.7 years. Participants were divided in two groups (experimental and control groups). The intervention in the experimental group was the practice of small-sided games during 60 minutes, two days per week and during 11 weeks. At baseline and the end of the intervention, body composition was measured using bioimpedance and VO2max was calculated using the Course Navette test., Results: A 7% reduction in the percentage of body fat and a 9% increase in VO2max were observed in the experimental group (p < 0.01). No changes were recorded in the control group., Conclusions: Eleven weeks of small-sided exercises performed twice per week increased VO2max and reduced fat mass in overweight adolescents.

We report the case of a patient whose main complaint was swelling on the right side of the mandible when he presented to the Ear, Nose and Throat (ENT) Service. Imaging studies revealed a large homogeneous, multilocular, expansive lesion in the body of the right mandibular ramus. The lesion was poorly enhanced following intravenous contrast injection. The patient was treated with hemimandibular surgical resection, fibula free flap reconstruction and adjuvant radiotherapy. Currently, the patient is disease free and free of posttreatment complications.

We report on a patient who presented to the Ear, Nose and Throat (ENT) clinic with swelling of the neck, dysphagia, headache, dyspnea and stridor. Imaging studies revealed an expansive heterogeneous process to the left retropharyngeal region. The mass was ovoid in shape, displaying moderate enhancement after intravenous contrast administration. Subsequently, a biopsy revealed the presence of undifferentiated sarcoma. The patient was treated with chemotherapy followed by radiation therapy, but follow-up exams at 6 months posttreatment revealed that while the tumor was stable, it persisted. Consequently, the patient was enrolled in a palliative care and pain control program and is currently being followed.

We report a patient who presented to the ENT service complaining of nasal obstruction, exophthalmos, edema and ipsilateral facial congestion. Imaging studies revealed an aggressive noncalcified solid mass centered in the left nasoethmoidal region and heterogeneous avid enhancement following contrast media injection. Subsequently, a biopsy confirmed the presence of solid alveolar rhabdomyosarcoma. The patient was treated with chemoradiation therapy for 7 weeks. Due to the advanced stage of the disease, the patient was enrolled in a palliative care and pain control program.

We report the case of a patient who presented to the ENT service with left facial swelling of 5 months duration. Imaging studies revealed a dense expansive mass confined to the inside of the left deep parotid lobule and moderate enhancement following contrast media injection. Subsequently, a biopsy confirmed the presence of an oncocytoma. The patient was treated with total parotidectomy, complete tumor resection and sparing facial nerve surgery. Today, the patient is disease free and has no complications.

Introduction: Physical exercise is a key modulator of the maximum fat oxidation rate (MFO). However, the metabolic transition zones in the MFO-exercise relationship are not generally considered for training prescription. Objective. To examine the effects of training in different metabolic transition zones on the kinetics of MFO and its localization (Fatmax) in young physically active men., Methods: 97 men were divided into 4 similar sized groups, 3 experimental groups and a control group (CG). Subjects in each experimental group undertook an 8-week running program. Training was continuous at the intensity of the aerobic threshold or VT1 (CCVT1); or performed as intervals at the intensity of the anaerobic threshold or VT2 (ITVT2); or at maximum aerobic power VO2max (ITVO2max). Before and after the training intervention, expired gases were monitored in each subject to determine VO2max, VT1, VT2, MFO (by indirect calorimetry) and Fatmax., Results: In response to training, experimental groups showed an increase in MFO (from 16,49 to 18,51%; p<0,01) and a mean reduction in Fatmax of 60,72±10,52 to 52,35±7,61 %VO2max (p<0,01). No changes of interest were observed in the control subjects. Intergroup comparisons revealed no differences in MFO and Fatmax among the experimental groups, though compared to the CG, a greater reduction in Fatmax was observed in CCVT1 (p<0,05). No changes were detected in performance except a drop in VO2max in the GC (p<0,05)., Conclusion: 8 weeks of training led to an increase in MFO and reduction in Fatmax irrespective of training intensity., (Copyright AULA MEDICA EDICIONES 2014. We report on a patient who presented with cranial nerve VI bilateral paresis, absence of pharyngeal reflex, dysarthria, right tongue deviation, and right facial paralysis. Imaging studies showed an expansive process in the cranial base with clivus and petrous apex osteolysis. A biopsy confirmed the presence of clear cell adenocarcinoma and suspicion of renal tumor metastases. Abdominal imaging studies revealed a mass in the right kidney. Consequently, radiotherapy was performed, and the patient was enrolled in a palliative care and pain control program.

Acute gastric volvulus in children is uncommon. This paper reports a case presented in a 14-year-old female whose diagnosis was suspected preoperatively taking into account the Brouchardt triad (vomiting, epigastric pain and inability to pass the nasogastric tube into the stomach). The child underwent laparatomy and the final diagnosis was mesenteroaxial gastric volvulus complicated with gastric perforation. Closure of the perforation and gastropexy were performed and the operative results were satisfactory.
