In the conditions of rapid qualitative changes in socio-economic relations in the Russian society and cardinal transformation of the world structure, the authors set the goal to present a conceptual approach and theoretical foundations of the national economy model transformation. The methods of content and logical analysis, as well as general scientific methods of abstraction, generalization, and synthesis were used in the problem formulation within the framework of historical and systemic approach to studying complex issues and challenges characteristic of socio-economic formations development transitional periods. The political and economic aspects of the influence of the world structure transformation on the tasks and foundations of the national state economic mechanism adaptation have been considered. The interrelation of the social structure evolution and the increasing role of national wealth effective management to ensure sovereign development and increase public welfare has been outlined. The preconditions have been analyzed and the necessity of updating the modern nation-state economic model substantiated, as well as the conceptual provisions and basic principles of updating the economic model formulated. Possible ensuring solutions in the sphere of public power, including improving the mechanism of state control over the effective use of national assets for implementing the justice principle in the economic relations system have been defined.

Introduction. The paper deals with relaxation and creep coefficients, long-term ultimate bending deformation under long-term exposure to moisture and chemically active environment (10,000 hours) of sewer pipes.Materials and methods. 400, 1,200 and 1,400 mm diameter sewer pipes made of glass-reinforced polyester. A compressive load was applied to the specimen at a constant rate until a specific annular strain of (3.0 ± 0.5) % was achieved in (60 ± 10) s. The obtained specific annular strain was kept constant for 2 minutes, after this time the compressive load and annular strain were determined and recorded. The essence of the method is to load a test specimen, placed horizontally and immersed in water, at a given temperature, with a diametrically compressive load for a time during which the annular strain remains constant. The load is measured at specified intervals until 10,000 h is reached. According to the test results, the long-term specific annular stiffness and relaxation coefficient under the influence of moisture are determined in accordance with the methodology of GOST R 57008–2016. Two specimens shall be used for testing, unless otherwise specified in the normative document. Long-term specific ring stiffness and relaxation coefficient are the arithmetic mean obtained for two specimens.Results. It was found that the performance characteristics (long-term ultimate bending deformation, relaxation and creep coefficients when exposed to moisture) of the investigated sewer pipes are determined by the specific ring stiffness and their diameter. For polyester pipes with initial ring stiffness of 5,000 N/m2 increasing their diameter from 400 to 1,400 mm leads to linear increase of relaxation coefficient (from 0.79 to 0.96) and decrease of long-term ultimate bending strain (from 0.84 to 0.75 %) when exposed to moisture during 10,000 hours.Conclusions. The durability of the investigated sewer pipes made of glass-reinforced polyester exceeds 50 years.

The article analyzes the interaction between administrative discretion and the main social regulators – religion, ethics and law. It is shown that historically discretion arises as a religious institution, consisting in the trust of public authorities and their officials in prophets – people who are able to contact with the divine forces. The procedure of discretio spirituum, used in the Middle Ages to determine whether a person is a prophet or a false prophet, is described separately. The significance of this procedure is expressed in the partial secularization of discretion, that is, the assumption by church authorities that the divine injunctions can be interpreted, supplemented, or ignored not only by God, but also by the aforementioned persons (prophets). This recognizes that discretion can be both negative (negative) and positive (positive); that the amounts of discretion granted to prophets must be in direct proportion to the degree of trust in them; and that there are specific criteria according to which to establish what amounts of discretion will be granted to prophets. After many centuries, this concept of administrative discretion has remained virtually unchanged. Ethical science reproduces it almost literally: it again refers to the trust, which this time is granted to executive authorities and their officials, as well as to the degree of such trust, which determines the scope of discretionary powers and is determined by certain criteria. Legal science borrows the above theory less explicitly, but its echoes can be detected there as well. In particular, according to one of the concepts of law understanding the law embodies a distrust (degree of distrust) between the people, and since the law and discretion are usually opposed to each other, discretion is again defined as trust (degree of trust). In the conclusion the conclusion is made about the special relevance of this approach to the administrative discretion because since 2020 the trust is a constitutional category.

Treatment of patients with iatrogenic injuries of the biliary tract is the most difficult and important section in hepatobiliary surgery. When analyzing the causes, it was found that in 70–94 % of cases this type of injury is observed during cholecystectomy. We present a rare clinical case of the development of secondary biliary cirrhosis due to iatrogenic trauma of the biliary tract. This injury caused long-term suffering for the patient due to the further development of complications of cirrhosis, specifically of portal hypertension. The latter caused repeated recurrent profuse bleeding from varicose veins of the esophagus. The use of a minimally invasive transjugular intrahepatic portosystemic shunt procedure for the first time in the Udmurt Republic was of particular relevance in solving this problem. This procedure has become a key one in solving the abovementioned problems and will become the preventive measure for the cirrhosis progression in the future. It is also important to focus on prophylactic measures aimed at preventing iatrogenic injuries of the biliary tract, as this problem can cause irreversible complications. Prevention should include adequate examination and visualization of the bile ducts and gallbladder before surgery, their careful mobilization, compliance with the rules of operation with electrosurgical instruments, as well as the use of additional minimally invasive techniques such as choledoscopy, cholangiography and intraoperative ultrasound. Besides that, all manipulations should be carried out under strict control and clear visualization of instruments and anatomical structures of organs. The article provides a detailed description of the technique of transjugular intrahepatic portosystemic shunt surgery, as well as presents X-ray images obtained during this operation.

Frequent development SARS-CoV-2 in patients with a surgical profile of a planned and emergency order indicate the need to study the features of the course of the postoperative period and histological examination of autopsy material of lung tissues in patients with coronavirus infection in these individuals.

Aim. To determine the prognostic significance of N-terminal pro-brain natriuretic peptide (NT-proBNP) and soluble ST2 (sST2) in patients with post-myocardial infarction heart failure after a coronavirus disease 2019 (COVID-19)Material and methods. Three following groups were formed: in patients of group I (main), who underwent inpatient treatment for COVID-19, the cause of heart failure (HF) was prior Q-wave myocardial infarction; in patients of group II (comparison) without COVID-19, the cause of HF was prior Q-wave myocardial infarction; Group III (comparison) — patients with HF of ischemic origin without postinfarction cardiosclerosis and COVID-19. Statistical processing was carried out using the software package IBM SPSS Statistics Version 26.0.Results. The patients of all three groups included in the present study were comparable. After 6 months in group I, HF course worsening was recorded. In patients of groups I and II, an increase in NYHA HF class from 2 to 3 and 4 was recorded, in contrast to patients of group III, among whom the number of patients with class 3 HF decreased by 25% due to compensation to class 2. Patients of the studied groups, who had elevated serum levels of NT-proBNP and sST2, had more pronounced structural and functional myocardial disorders than patients with normal levels of these biomarkers. Using analysis of adverse cardiovascular events, such as recurrent myocardial infarction, cerebrovascular accident, hospitalization due to coronary artery disease, decompensated HF, which required hospitalization, a predictive model was developed using a neural network. There were following most important factors of the developed model: age, level of NT-proBNP and sST2, lung involvement according to computed tomography.Conclusion. Modern biomarkers NT-proBNP and sST2 have sufficient predictive value to determine the risk of adverse events in HF.

The OBJECTIVE was to show a potential and efficiency of endovascular methods in treatment of patient with portal vein stenosis after pancreatoduodenal resection complicated by the development of varices in the portomesenteric basin and recurrent gastrointestinal bleeding.METHODS AND MATERIALS. We presented the results of treatment of two patients who previously underwent pancreatoduodenal resection for pancreatic head cancer. 2 patients were admitted to the Clinic of the Research Institute of Surgery and Emergency Medicine of Pavlov University with a clinical picture of recurrent gastrointestinal bleeding.RESULTS. Both patients had no visible bleeding site with endoscopy, but had recurrent melena and hemoglobin loss. On radiological imaging we found a portal vein stenosis with a significant varicose transformation of the mesenteric basin. Endovascular stent placement for portal vein stenosis was performed in both cases with no recurrence of bleedings and no complications.CONCLUSION. Portal vein stenosis after pancreatoduodenal resection is a rare complication, which can manifest in recurrent gastrointestinal bleeding with no visible bleeding site on endoscopy. Endovascular methods are feasible and safe in treatment of portal vein stenosis with no relapse. Multidisciplinary approach in diagnostic and treatment of such patients allow to identify and remove the cause of recurrent bleeding.

The article presents the results of the sociological analysis of the temporal aspects of social care in the new conditions of the pandemic reality. The pandemic is considered as a set of conditions and factors that determine the specific dynamics of societal transformations that affect almost all aspects of the life of society including its temporality - the present and future. Digital technologies, distant models of social relations, mobility and cohesion become priorities for social policy and social care. The pandemic, on the one hand, exacerbates social inequality, on the other hand, levels it out in the ways of self-realization (network technologies), ways of obtaining benefits, primarily from the state (digital social, financial and other services). The pandemic has a binary impact on the practices of social care, programs and technologies for social services, social support, social assistance, and social protection. These practices are most effectively carried out in urbanized areas, where the quality of social care is partly determined by the quality and availability of the relevant urban infrastructure. Based on the results of the new Russian and foreign research presented at the conference under review, the forms, values and resources of social care in the pandemic society are described together with the specifics of distant culture and social inequality in the contemporary city, barriers to mobility (digital, social-perceptual, social-legal), challenges, actors and discourses of social work, lockdowns of social life.

The article presents the results of a review of the data of Russian and international research of social-urban development as presented at the scientific events in the Saratov region. In contemporary urbanism, there is a number of trends: temporal, of universal design, and social-cultural. The Russian urban development follows agglomeration trends that are increasingly evident in the processes of settlement, which means active development of suburban areas, changes in their landscape characteristics, cultural spaces, and mobility of citizens. The covid-19 pandemic had a complex impact on the social-urban features of cities in Russia and the world by transforming the structure and functionality of many urban locations, creating conditions for the emergence of a post-coronavirus city. The empirical data show that such a city is the most socially sensitive to negative and positive aspects of social life and to manifestations of inclusive practices that unite people. Under the low, fragmented accessibility of social, cultural and other infrastructure of cities that are designed for healthy people, there is a synchronization of urban infrastructure elements in the context of inclusion due to the social demand for a coronavirus transformation of the architectural and urban environment in terms of social distancing. Based on the research data from different regions of Russia, the authors identify priority directions of the inclusive development of social urbanism: models of the inclusive culture of urban communities; monitoring of the city accessible environment for citizens of different age and mobility (in particular, with the tracing and walk along approaches); model of participatory urban planning and social expertise of the inclusiveness of the urban space; educational model of professional training in the field of social urbanism and universal design.

Objective. In this paper, we show the work of the emergency surgical department on the example of a large repurposed multidisciplinary hospital, also its necessity even for infectious hospitals not accepting patients with acute surgical pathology.Methods and Materials. The work of surgical department of Pavlov First Saint Petersburg State Medical University repurposed for COVID-19 patients was analyzed. About 1500 COVID-19 patients were hospitalized during 4-month period.Results. All patients undergoing surgery were in severe condition (at intensive care unit). Most common surgery in abdomen was diagnostic laparoscopy. Spontaneous bleedings of different localization occurred in these patients, what required endovascular and endoscopic treatment. We observed only Clavien – Dindo 1-degree postoperative complications. Mortality rate was high and amounted to 75 %.Conclusion. The work of surgical department in an infectious hospital for the treatment of a new coronavirus infection is extremely difficult. An infectious hospital should have a 24-hour diagnostic service (US, CT, MRI). Patients severity and the working conditions necessitate the availability of the trained surgical personnel. Given the high risks of bleedings in patients with COVID-19, it is necessary to have a 24-hour endoscopy and endovascular surgery departments with the ability to perform diagnostic and therapeutic interventions.

The article presents the results of the regional sociological study of the student youth civil activism and predicts the development of its destructive manifestations in the Volga Region. Civil activity is an integral component of renewal and social development, which is based on the multi-format and multi-vector activity of young people who strive for social transformations here and now. For some young people, activism became a kind of employment in offline and online formats. In some cases, destructive (even delinquent) civil activism of the youth is a response to an unfair, unlawful action or attitude, primarily of the authorities. Based on the theoretical ideas of structural functionalism, conflictological and interpretive approaches, the article reveals peculiarities of the youth activism in the Volga Region and presents their interpretation. The expert survey allowed to identify relationships between social injustice and civil activity. The authors also identified active dissemination of radical ideas at the grassroots level of everyday life, within everyday forms of interaction and under the decrease in public civil activity, which determines latent conflicts between the population and the government. The risk of an increase in social destructiveness is determined by the crisis of expectations of changes, unsolved social-economic problems, renaissance of socialist ideas that become popular among the youth, and an increase in the number of nonresident and especially foreign students. The student civil activity in the Volga Region focuses on the most pressing challenges at the federal and local levels. Sanctions against students participating in rallies and protest groups in social networks only stimulate the youths interest in protest activities. The region needs a youth policy responsive to the needs and demands of the youth and capable of creating a multi-format space for positive youth activism.

The article presents the results of the regional sociological study of the parameters of the social-economic well-being in the Volga Region. The well-being category consists of social satisfaction, trust, tension and security. Social well-being reflects the efficiency of the social system, its quality, the authorities’ competence in the development of social-economic processes and of a socially sensitive (accessible, comfortable) social environment. The study identified connections between social-economic well-being and employment, financial situation and the dynamics of migration; and empirically proved the low likelihood of ethnic or religious conflicts in the region, the high level of social well-being as mentioned by the younger generations and the average one among other age groups. The level of ethnic and confessional tension is influenced by the age of the respondents: a third of the younger generations and of the working age are more concerned with the criminal situation and with conflicts on national and religious grounds than pensioners. The able-bodied population of the Volga Region is concerned about their professional well-being due to perceiving migrants as competitors: in some cases, an increase in the share of migrants contributes to conflicts in the interethnic interaction. With an increase in the educational level the degree of social trust increases, which is a positive factor for the tolerant attitude towards others. In general, there are no reasons for concerns about ethnic conflicts in the region. The multi-ethnicity of the Russian society explains the relatively high tolerance to migrants despite many risk factors.

Aim. To assess the ultrasonography role for diagnosis of atherosclerotic lesions of the extracranial carotid arteries.Material and methods. The study was conducted on the basis of the St. Petersburg Mariinsky Hospital. A total of 855 patients who were admitted to the regional vascular center due to ischemic stroke or transient ischemic attack were examined. The mean age of the patients was 68 years (min 18, max 107). All patients underwent ultrasound of the carotid arteries.Results. In 570 (66,6%) patients with neurological symptoms (according to clinical and neurological examination), hemodynamically insignificant carotid artery lumen stenosis up to 70-75% was revealed. Ultrasound imaging in all patients revealed signs of atherosclerotic plaque (ASP) instability in various combinations, except for hemorrhage. It was not possible to clearly visualize the finished hemorrhage in the ASP structure.Conclusion. Despite the high informative value of ultrasound, it is not possible to answer all clinical questions with its single use. At the moment, there is no classification of instability signs, reflecting the ASP embolism risk. It needs for paternal change the management of asymptomatic patients. More detailed study of ASP destabilization using various visualization methods is necessary.

Aim. To study the myocardial morpho-functional abnormalities, the incidence and nature of cardiac arrhythmias in patients 3 months after the coronavirus disease 2019 (COVID-19).Material and methods. The study included 77 patients (mean age, 35,9 years) treated for coronavirus infection, which underwent echocardiography and 24-hour Holter monitoring 3 months after COVID-19. The patients were divided into 3 groups: group 1 — 31 patients with upper respiratory tract involvement; group 2 — 27 patients with bilateral pneumonia (CT grade 1, 2), 3 — 19 patients with severe pneumonia (CT grade 3, 4). Statistical processing was carried out using Statistica 10.0.Results. The clinical case of successful cure of the victim with severe closed chest injury is presented, as a result of which the life-threatening acute respiratory failure could not be compensated by artificial lung ventilation and, in connection with which the use of extracorporeal membrane oxygenation as a «life-saving technology» was required.

We present an observation from practice, which describes a multidisciplinary approach to the treatment of patient with acute mesenteric ischemia.

Aim. Based on the autopsy data, to analyze mortality patterns of the Zabaykalsky Krai population over a three-year period in the group of out-of-hospital sudden cardiac death (SCD).Material and methods. The protocols of deceased persons without evidence for violent death were analyzed with distribution into groups depending on age, sex and cause of death. Descriptive statistics were used for statistical processing.Results. The leading positions (58% of cases) in mortality patterns are occupied by various types of coronary artery disease (CAD). Chronic coronary syndromes were detected in 21%, cardiomyopathy — in 11%, decompensated heart failure — in 7%, myocarditis — in 1% of cases. Acute types of CAD were found in 68,4% in men and 31,6% in women. Among men, the number of such cases increases from 31 to 70 years of age and decreases over 70 years old. Among women, there is an increase in the SCD prevalence in the group over 70 years old.Chronic coronary syndromes were found in 46,4% in men and 53,6% in women. In both groups the number of cases increases with age. The maximum sex differences are observed in the group over 70 years old. The mean age for men is 72,2±8,8 years, for women — 77,2±10,4 years. Blood alcohol was detected in 10,2% of cases. The mean age of the deceased in all age groups of persons with identified blood alcohol was 66,2±12 years.In 1,89% of cases, I46 code (ICD-10) was established. The largest number of deaths among persons of both sexes was registered in the group of 31-40 years old, accounting for 36,8% among men and 13,2% among women. The mean age of the deceased was 35,8±8,4 years. In 28,6% of cases, ethyl alcohol was found in the biological media of the deceased in this group.Conclusion. Acute and chronic types of CAD make a significant contribution to out-of-hospital mortality. The number of SCD in men is higher than in women and is directly proportional to the age increase, reaching a maximum in the group over 70 years old. Ethyl alcohol, an important trigger of SCD, was detected in 10,8% of SCD cases in 2017, and in 15% in 2018 and 2019.

An electrochemical hydrophone based on the principles of molecular electronic transfer (MET) has been described. The paper presents theoretical and experimental results for the sensitivity and the level of self-noise determination for the MET hydrophone (METH) in the frequency range of 0.02–200 Hz, which determines the fields of acceptance of the devices being developed. An experimental model has been developed by using a force-balancing feedback. Different methods and techniques for its calibration have been developed. The experimental device with 0.75 mV Pa−1 sensitivity flat in the frequency band 0.02–200 Hz has been presented. It has been demonstrated that in the ultra-low-frequency range METH noise could be much lower than the standard Wenz noise model. Easy to produce, cheap and suitable for mass production, the MET hydrophone could be in demand in marine and land acoustic research.

Relevance of research: There are two sizes (several microns and nanodimensional) of strengthening j'-phase in single-crystal heat resisting nickel and intermetallic alloys, used for making blades of modern gas turbine engines (GTD). For in-depth study of structural and phase condition of such alloys not only qualitative description of created structure is necessary, but quantitative analysis of alloy components geometrical characteristics. Purpose of the work: Development of reference material sets of heat resisting nickel and intermetallic alloy composition and structure. Research methods: To address the measurement problem of control of structural and geometrical characteristics of single-crystal heat resisting and intermetallic alloys by analytical microscopy and X-ray diffraction analysis the research was carried out using certified measurement techniques on facilities, entered in the Register of Measurement Means of the Russian Federation. The research was carried out on microsections, foils and plates, cut in the plane {100}. Results: It is established that key parameters, defining the properties of these alloys are particle size of strengthening j' -phase, the layer thickness of j-phase between them and parameters of phases lattice. Metrological requirements for reference materials of composition and structure of heat resisting nickel and intermetallic alloys are formulated. The necessary and sufficient reference material set providing the possibility to determine the composition and structure parameters of single-crystal heat resisting nickel and intermetallic alloys is defined. The developed RM sets are certified as in-plant reference materials. Conclusion: The reference materials can be used for graduation of spectral equipment when conducting element analysis of specified class alloys; for calibration of means of measuring alloy structure parameters; for measurement of alloys phases lattice parameters; for structure reference pictures receiving by SEM and TEM.

OBJECIVE. The authors would like to design an original device for aerosol laparoscopic chemotherapy and propose the most effective and safe mode. MATERIALS AND METHODS. The experimental and clinical studies were carried out and included 6 series. Male rabbits (n=71) were used for experimental part and the method was applied on 4 patients in clinical part. RESULTS. The original device and the effective mode of its application were developed for aerosol laparoscopic chemotherapy. The supposed method of chemotherapy appeared to be more effective than the standard method. There weren’t noted any complications or side effects. The method proved its safety. CONCLUSIONS. The aerosol laparoscopic intraperitoneal chemotherapy using the proposed regime is the effective and safe method of peritoneal carcinomatosis treatment.

The colon microflora was investigated by PCR method in real time before and after planned operation in 38 patients with colon cancer. A multifactor analysis showed absence of statistically significant dependence of revealed changes in microflora from sex, age of patients, volume and method of surgery, presence of tumor stenosis and infiltrate. An anaerobic imbalance was often registrated in postoperative period, especially after antibiotic prophylaxis and therapy. Введены и исследованы специальные подмножества уравнений линейной системы, именуемые кланами. Предложено использовать декомпозицию на кланы для ускорения решения линейной системы. Сложность декомпозиции равна кубу от размера системы. Поэтому ускорение вычислений получено для методов, сложность которых превышает кубическую. Для целочисленных систем, решаемых в целой неотрицательной области, ускорение вычислений является экспоненциальным.

Предложена организация последовательного процесса композиции кланов линейных систем для реализации дополнительных ускорений вычислений при их решении. Получено ускорение вычислений путем решения последовательности систем композиции кланов существенно меньшей размерности. Использован граф декомпозиции системы на кланы. Выполнен сравнительный анализ последовательной композиции подграфов и реберной (парной) композиции. Задача построения последовательности систем наименьшей размерности названа оптимальным коллапсом взвешенного графа. Приведены оценки верхней и нижней границ ширины коллапса, которая соответствует размерности систем. Построен и статистически обоснован эвристический алгоритм оптимального коллапса.

With the aim being evaluation of psychological functioning in children while benign epileptiform discharges of childhood are being formed (BEDC) (in the EEG results) a group of 13 children with organic brain lesions - neuroimaging verified and neuroimaging non-verified -has been formed. All of the patients were diagnosed with various focal epilepsies and showed different levels of cognitive deficits. All of the patients underwent electroencephalograph assessment with at least once-a-month frequency. Patients with verified diagnosis of benign epilepsy of childhood with central-temporal spikes were not included into the study. Upon EEG dynamic assessment administration all the children included in the study showed (which was no longer present at the time of control assessment in 1-2-3 months) transitory complexes in the form of benign epileptifom discharges of childhood (BEDC) which in its turn coincided with a clinical improvement of cognitive aspects in all children (attention, hyperactivity, speech) regardless of whether they had any brain organic lesion or not and severity thereof.

A technique of the computing grid verification using invariants of infinite Petri nets was presented. Models of square grid structures in the form of parametric Petri nets for such edge conditions as connection of edges and truncated devices were constructed. Infinite systems of linear algebraic equations were composed on parametric Petri nets for calculating p-invariants; their parametric solutions were obtained. P-invariant Petri nets are structuraly conservative and bounded that together with liveness are the properties of ideal systems. Liveness investigation based on siphons and traps can be implemented by using p-invariants of modified nets.

Aim. Studies on the role of autonomic disorders, changes of serum NEFA, glycerol, adenine nucleotides and violation fraction of the fatty acids of erythrocyte membranes in the development of cardiac arrhythmias in patients with chronic prostatitis. Methods. The study included 50 patients with chronic prostatitis, mean age 35 ± 5,6 years. Vegetative state is defined by: clinical evaluation of autonomic disorders, autonomic profiles and vegetative index Kerdo. Performed Holter ECG monitoring with analysis of heart rate variability. The blood was determined NEFA, glycerol, macroergs concentration and size distribution of higher fatty acids in red blood cells. Results. The majority of patients with chronic prostatitis on the questionnaire data revealed sympathotony manifested reduced the basic parameters of heart rate variability. In these patients have the syndrome disorders utilization of fatty acids, characterized by the accumulation in the blood NEFA while reducing the level of glycerol, and decreased ATP content in red blood cells and increased AMP. The greatest changes in the level and composition of free fatty acids in erythrocyte membranes as raising saturated and polyunsaturated reduction deal with chronic prostatitis with the activation of the sympathetic autonomous nervous system level.
