Objectives: Acetate (ACE) and lactate (LAC)-containing balanced resuscitation fluids are commonly used for fluid therapy in cats. These fluids can influence acid-base and electrolyte status. This prospective randomised study compared two balanced crystalloid solutions regarding their effect on acid-base status, electrolytes and LAC concentrations in dehydrated cats after intravenous fluid therapy. Methods: A total of 100 client-owned cats presenting to the emergency service with dehydration ⩾5% due to diarrhoea, vomiting or anorexia were included in this study. They were randomised to receive either ACE- (Sterofundin ISO; B Braun Vet Care) or LAC-containing (Ringer-Laktat nach Hartmann; B. Braun Vet Care) fluids for rehydration. Exclusion criteria were age <6 months, severe electrolyte abnormalities, severely increased creatinine or bolus therapy, and decompensated heart disease and liver diseases. Vital parameters were assessed and venous blood gas analysis was performed before and after fluid therapy. Data were analysed using the Mann–Whitney U-test and the Wilcoxon matched-pairs signed-rank test. The significance level was set at P ⩽0.05. Results: Post-rehydration pH normalised, and bicarbonate and base excess increased in both groups. Anion gap and LAC and potassium concentrations decreased in both groups. No difference in electrolyte, acid-base status and LAC was observed between cats receiving ACE and cats receiving LAC. Conclusions and relevance: Both fluids have similar effects on dehydrated cats' acid-base status and electrolyte and LAC concentrations. A three-year-old, intact female mix-breed dog, weighing 30 kg, was presented due to vomitus and diarrhea. At presentation, the patient had a slightly reduced general condition and moderately enlarged mandibular and popliteal lymph nodes. The initial blood work showed severe azotemia and hypoalbuminemia. In the urinalysis, marked proteinuria with a urine protein/creatinine ratio (UPC) of 4.69 was found. Further workup showed a high leishmania antibody titer. The dog was diagnosed with leishmaniosis and glomerulonephritis. Initial treatment consisted of intravenous fluid therapy, allopurinol, miltefosine, amlodipine, clopidogrel, and a diet with a low purine content. Creatinine temporarily decreased but increased again after three days. For further supportive treatment, intermittent hemodialysis in combination with hemoperfusion with the cytosorb® adsorber was performed. A total blood volume of 17.7 L was processed within three hours. Thereafter, immunoadsorption (IA) was performed with the COM.TEC® and ADAsorb® platforms and a LIGASORB® adsorber to eliminate circulating immunocomplexes. Treatment time for IA was two hours with a blood flow of 50 mL/min. A total plasma volume of 2.4 L was processed. Over the following days, creatinine declined, and the patient improved significantly. UPC decreased to 1.74 on day 17 after IA. The patient was discharged after two and a half weeks. Two years after the initial event, the patient is still in excellent condition, with creatinine, UPC, and albumin levels in the reference range. Therefore, IA might be an additional therapeutic option for dogs with leishmaniosis-induced glomerulonephritis and subsequent severe azotemia to improve immunocomplex-mediated glomerulonephritis. Simple Summary: Acquired myasthenia gravis is an immune-mediated neuromuscular disease caused by autoantibodies directed against the neuromuscular junction, which leads to skeletal muscle weakness. The acute fulminant form is defined as rapidly progressive generalized weakness and can lead to a life-threatening condition caused by the paralysis of respiratory muscles. Traditional management consists of acetylcholinesterase inhibitors and nutritional management. This case report shows a new therapeutic option for this acute life-threatening condition. The dog was mechanically ventilated for respiratory failure due to acquired myasthenia gravis and immunoadsorption, an extracorporeal blood purification technique, was performed. During immunoadsorption, blood is purified in an extracorporeal circuit using an adsorber that binds circulating autoantibodies. After immunoadsorption, the dog was successfully weaned from mechanical ventilation and further treated with medical management. A one-year-old, female intact Samoyed, 12.5 kg, was presented with coughing for 2 weeks, progressive appendicular and axial muscle weakness, megaesophagus and labored breathing for 5 days. There was no improvement with standard treatment. Acquired myasthenia gravis was suspected and the dog was referred with increasing dyspnea. At presentation, the dog showed a severely reduced general condition, was non-ambulatory and showed abdominal and severely labored breathing. A marked hypercapnia (PvCO2 = 90.1 mmHg) was present in venous blood gas analysis. The serum anti-acetylcholine receptor antibody test was consistent with acquired myasthenia gravis (2.1 nmol/L). The dog was anesthetized with propofol and mechanically ventilated with a Hamilton C1 ventilator. Immunoadsorption was performed with the COM.TEC® and ADAsorb® platforms and a LIGASORB® adsorber to eliminate anti-acetylcholine receptor antibodies. Local anticoagulation was performed with citrate. Treatment time for immunoadsorption was 1.5 h with a blood flow of 50 mL/min. A total plasma volume of 1.2 L was processed. Further medical treatment included intravenous fluid therapy, maropitant, esomeprazole, antibiotic therapy for aspiration pneumonia and neostigmine 0.04 mg/kg intramuscularly every 6 h for treatment of acquired myasthenia gravis. Mechanical ventilation was stopped after 12 h. A percutaneous gastric feeding tube was inserted under endoscopic control on day 2 for further medical treatment and nutrition. A second treatment with immunoadsorption was performed on day 3. Again, a total plasma volume of 1.2 L was processed. Immediately after this procedure, the dog regained muscle strength and was able to stand and to walk. After 6 days, the dog was discharged from the hospital. This is the first report of immunoadsorption for emergency management of a dog with acute-fulminant acquired myasthenia gravis. Objectives: The aim of this study was to compare the failure rate of two new generation pulse oximeters at different probe positions, and with and without vasoconstriction, in anaesthetised cats. Methods: This prospective clinical study included 103 cats in which the new generation pulse oximeters, the Rad-5 (Masimo) and EDAN H100N (EDAN), were evaluated. Premedication consisted of the vasoconstrictive drug combination butorphanol (0.2 mg/kg IV) and dexmedetomidine (5 µg/kg IV), or butorphanol only (0.2 mg/kg IV). Pulse oximeter failure rate at the tongue was compared between both groups. Pulse oximeter failure rate was also analysed at the alternative probe positions of the lip, pinna, knee fold and toe in the butorphanol group. Student's t -test, Wilcoxon matched pairs signed rank test, Mann−Whitney U-test, Friedman test and χ2 test were performed. A P value <0.05 was considered to be statistically significant. Results: Overall failure to achieve an adequate signal was 37.6% with the Masimo and 48.0% with the EDAN pulse oximeter (P <0.0001). At the standard probe position on the tongue, the Masimo failed in 4.5%, while the EDAN failed in 35.3% (P <0.0001). Vasoactive premedication increased the failure rate for the Masimo from 3.8% to 5.2% (P = 0.3414) and for the EDAN from 22.4% to 49.0% (P <0.0001). At the alternative probe positions of the lip and knee fold, failure rates for the Masimo were lower (39.7% and 81.4%) than with the EDAN (52.6% and 94.4%; P = 0.0231 and P = 0.0005, respectively), while the Masimo failed more often at the pinna (63.5%) than the EDAN (47.4%; P = 0.0044). At the alternative probe position of the toe, the failure rate for the Masimo (32.7%) was not different from the EDAN (38.5%; P = 0.7547). Conclusions and relevance: The Masimo pulse oximeter had lower signal failure rates at the standard probe position on the tongue and at 2/4 alternative probe positions. The standard probe position on the tongue had the lowest failure rate for both devices. Objectives: Airway management during anaesthesia in cats is always a demanding task and is associated with several complications. The aim of this study was to evaluate the practicability and complications during feline-specific laryngeal mask placement in anaesthetised cats as an alternative to endotracheal intubation. Methods: In this prospective clinical study, laryngeal masks were placed in 148 anaesthetised cats. Success of placement was evaluated by capnography. Results: Placement was possible at the first attempt in 136 cats, at the second attempt in eight cats and at the third attempt in one cat. In one cat, placement was not possible. Two cats were excluded. Failure to position the laryngeal mask at the first attempt was not different between laryngeal mask sizes (P = 0.313) or positioning during placement (P = 0.406). In nine cats, the laryngeal mask dislocated during the procedure. Dislocation occurred more often in the dorsal position than in the sternal (P = 0.018) and right lateral positions (P = 0.046). Mucous obstruction of the laryngeal mask occurred in one of these cats and regurgitation in another. Material-related issues, such as disconnection of the parts of the laryngeal mask and leakage of the balloon, were observed in 2/8 laryngeal masks. Conclusions and relevance: The placement of a feline-specific laryngeal mask was easy to perform. In about 7% of the cases, replacement of the device was required due to mispositioning or dislocation. Objectives: Ultrasonography of the caudal vena cava (CVC) has been previously established to assess fluid status in dogs but not in cats. The aim of this study was to determine CVC diameter changes during feline blood donation. Methods: Inter- and intra-observer variability were assessed in 11 client-owned cats. Minimal and maximal CVC diameters were assessed longitudinally in the subxiphoid view (SV) and right paralumbar view (PV), and transversely in the right hepatic intercostal view (HV). Eighteen client-owned, healthy, anaesthetised cats were evaluated during 21 blood donation procedures of 10 ml/kg in the same anatomical locations before (T0) and after (T1) blood donation, and after volume resuscitation with 30 ml/kg lactated Ringer's solution (T2). The CVC index was calculated. Results: Intra-observer variability was acceptable for all probe positions, except for the HV, whereas inter-observer variability was considered unacceptable for all probe positions. Complete measurements were obtained during 21 blood donations at T0, T1 and T2 at the SV, during 18/21 blood donations at the HV and during 16/21 blood donations at the PV. At the SV, the minimal CVC diameter between T1 and T2 (P <0.001), and the maximal CVC diameter between T0 and T1 and between T1 and T2 (P <0.001) were significantly different. At the HV, the minimal vertical diameter, maximal vertical diameter and minimal horizontal diameter were different between all timepoints (P <0.001). The maximal horizontal diameter was different between T1 and T2 (P = 0.002). At the PV, both diameters were different between all timepoints (P <0.001). The CVC index was not different between timepoints. Conclusion and relevance: Significant probe position dependent CVC diameter changes with marked overlap were observed before and after blood donation, and after fluid bolus. No absolute CVC diameter could be used to indicate hypovolaemia. Ultrasonographic assessment of the feline CVC is highly operator-dependent. The CVC index is not useful in cats. Purpose:Doxorubicin (DOX)-loaded phosphatidyldiglycerol-based thermosensitive liposomes (DPPG2-TSL-DOX) combined with local hyperthermia (HT) was evaluated in cats with locally advanced spontaneous fibrosarcomas (soft tissue sarcoma [STS]). The study was designed to evaluate the safety and pharmacokinetic profile of the drug. Results from four dose-levels are reported. Methods:Eleven client-owned cats with advanced STS were enrolled. Five cats received escalating doses of 0.1–0.4 mg/kg DOX (group I), three received 0.4 mg/kg constantly (group II) and three 0.6 mg/kg (group III) IV over 15 min. HT with a target temperature of 41.5 °C was started 15 min before drug application and continued for a total of 60 min. Six HT treatments were applied every other week using a radiofrequency applicator. Tumour growth was monitored by magnetic resonance imaging (MRI) and for dose level III also with18F-FDG PET. Results:Treatment was generally well tolerated and reasons for premature study termination in four cats were not associated with drug-induced toxicity. No DPPG2-TSL-DOX based hypersensitivity reaction was observed. One cat showed simultaneous partial response (PR) in MRI and positron emission tomography (PET) whereas one cat showed stable disease in MRI and PR in PET (both cats in dose level III). Pharmacokinetic measurements demonstrated DOX release triggered by HT. Conclusion:DPPG2-TSL-DOX + HT is a promising treatment option for advanced feline STS by means of targeted drug delivery. Objective: The aim of this retrospective study was to analyze the clinical signs, confirmed or suspected toxicants, treatments and outcomes of poisoning cases in dogs presented over a 5-year period to the emergency service of a small animal referral center., Material and Methods: Medical records of 634 dogs were evaluated for a history of confirmed or presumed poisoning, suspected toxicant, clinical signs, treatment, and patient outcome. The probability of poisoning was graded based on the patient history, clinical findings, toxicologic examination and - in some cases - investigation of gastrointestinal contents., Results: Most dogs were hospitalized (77%) due to poisoning with mostly unknown toxicants (33%), food residues (18%), rodenticides (10%), tremorgenic mycotoxins (8%), medications (7%) and various plants (7%), followed by recreational drugs (5%), chemicals (4%), molluscicides (3%), antiparasitics (2%), feces (2%), nuts (2%), or toxins of animal origin (1%). Patients were presented predominantly showing neurologic signs (56%), reduced general condition (39%), and cardiovascular or hydration status abnormalities (26%). The survival rate was 97%. Most dogs were clinically unremarkable at the time of hospital discharge (70%). An additional 18% of the survivors had no apparent complications by the time of discharge. Toxicant-related complications (20.5%) included hemorrhage (4%), hepatic (4%), renal (4%), respiratory (3%), gastrointestinal (3%), cardiovascular (3%), and/or central nervous system (3%) complications, or clinically relevant hypoglycemia (0.3%)., Conclusion and Clinical Relevance: In the present study, poisoning in dogs was mostly associated with the ingestion of food residues, but the causative toxicant remained unidentified in many cases. Canine parvovirus infection remains to represent a worldwide and commonly occurring infectious disease leading to severe morbidity especially in puppies. The main therapeutic approach is primarily based on symptomatic treatment, especially addressing acute gastrointestinal signs as well as treating and preventing potential sepsis due to bacterial translocation. Besides antibiotic and essential fluid therapy, the use of efficient antiemetic and pain medication is required. In addition, early enteral nutrition should be attempted as this has been shown to be associated with a shorter time to recovery. Modulation of the intestinal microbiome could improve clinical signs and possibly aide in avoiding long-term sequelae such as chronic gastrointestinal disease. The main causative agents of feline upper respiratory tract disease (FURTD) are feline herpesvirus-1 (FHV-1) and feline calicivirus (FCV). These viral infections are common, especially in multiple cat households. Severely affected cats often need to be hospitalized. Intensive symptomatic therapy is important in the management of cats with FURTD. The use of antiviral drugs is limited in cats, as they are often ineffective or toxic when given systemically. Antiviral drugs are, therefore, mainly used locally for the treatment of FHV-1-associated eye changes. Famciclovir, however, is an effective drug for systemic therapy in cats with FHV-1-related clinical signs. For FCV, only few antiviral drugs are available.
