Abstract

Background: We aimed to evaluate the relationship between serum leptin and the leptin/body mass index (BMI) ratio with prevalent cardiovascular disease (CVD), and their influence on all-cause and CVD-related mortality in patients on hemodialysis (HD). Methods: 118 stable HD patients (50 women, median [interquartile range] age, 65.1 [54.7-72.2] years) were studied. All patients had baseline measurement of serum leptin concentrations. Relationships between leptin and all-cause and CVD mortality were studied by means of survival analysis and Cox regression analysis. Results: The leptin/BMI ratio was similar in patients with and without CVD at baseline (0.65 [0.29-2.23] vs. 0.68 [0.29-1.49] ng·m²/ml·kg, respectively, NS). Multiple logistic regression analysis showed that there was not an independent association between leptin/BMI ratio and prevalent CVD. During the follow-up time, 52 (44.1%) patients died. CVD was the cause of death in 27 out of 52 (51.9%) deceased patients. Survival analysis and Cox proportional multivariate regression analysis showed that there were no significant relationships between leptin levels or the leptin/BMI ratio and all-cause and CVD-related mortality. Conclusion: These results do not support that, in stable HD patients, serum leptin concentrations and the leptin/BMI ratio are related with prevalent CVD. Leptin/BMI ratio seems not to be a risk factor for mortality in these patients.

Métodos: Se estudiaron 118 pacientes estables en HD (50 mujeres, edad mediana [recorrido intercuartílico], 65,1 [54,7-72,2] años). En todos los pacientes se cuantificó la concentración basal de leptina. La relación entre leptina y la mortalidad se evaluó mediante análisis de supervivencia y análisis de regresión de Cox. Resultados: El cociente leptina/IMC fue similar en pacientes con y sin ECV prevalente (0,65 [0,29-2,23] frente a 0,68 [0,29-1,49] ng·m²/ml·kg, respectivamente, NS). El análisis de regresión logística mostró que no existía una asociación independiente entre el cociente leptina/IMC y la enfermedad cardiovascular prevalente. Durante el seguimiento 52 pacientes fallecieron (44,1%). La ECV fue causa de muerte en 27 de 52 pacientes fallecidos (51,9%). El análisis de supervivencia y el análisis multivariante de Cox mostraron que no hubo relación significativa entre los niveles de leptina o el cociente leptina/IMC y la mortalidad global o por causa de ECV. Conclusión: Estos resultados no apoyan la hipótesis de que, en pacientes estables en HD, las concentraciones de leptina y el cociente leptina/IMC estén relacionados con la ECV prevalente. Más aún, el cociente leptina/IMC no parece ser un factor de riesgo de mortalidad en estos pacientes.

Abstract

Feeding responses by Scolytus scolytus were tested using elm twig bark extracts in a laboratory bioassay. One to 4- years-old elm twigs or small branches were sampled in spring and their bark extracted separately with methanol and with a mixture of petroleum ether and diethyl ether (1:1) as solvents. Bark extracts were tested in a two choice feeding bioassay consisting of two polyurethane discs placed in a 10 cm diameter Petri dish. Extracts were applied onto the discs and the amount of disc eaten by ten freshly emerged S. scolytus adults was recorded after 24 hours. Ten U. minor, two U. laevis, six U. glabra, three Dutch hybrids (European x Asiatic) and one U. pumila trees were tested in several comparisons. Discs with extracts from both U. laevis trees were significantly less eaten than those from U. pumila or from U. minor trees in two choice tests. Similarly, extracts from all U. glabra trees received less feeding than those from U. minor. On the contrary, S. scolytus showed no difference in feeding between U. pumila and U. minor extracts, and similarly for Dutch hybrids in comparison with two U. minor clones. Again, beetles preferred to feed on Dutch hybrid extracts better than in those from U. laevis. Significant intraspecific differences in feeding were obtained in U. minor. One of the U. minor clones resulted less chosen when compared to other four trees. Extracts from a dying U. minor tree received more feeding than those from a healthy tree. Comparisons were also made between bark extracts from 2-to 4-year-old vs. current-year twigs within the same trees. Los extractos del floema se evaluaron en un bioensayo de doble elección consistente en dos discos de poliuretano dispuestos en una placa Petri de 10 cm de diámetro. 43. Hypogonadism and Mortality in Aged Hospitalized Male Patients: A 5-Year Prospective Observational Study.

Abstract

Objective: To investigate the relationship between hypogonadism and mortality in aged hospitalized male patients. Design: A 5-year prospective observational study was conducted. Gonadal function was assessed at hospital admission and mortality was registered in the follow-up period. Patients and Methods: We studied all patients ≥ 65 years admitted for any reason during 2010 and 2011. Serum T concentrations were quantified in all patients. Hypogonadism was defined by the presence of serum T levels < 200 ng/dl. Number of deaths and all-cause and cardiovascular (CV) mortality were registered until December 31st, 2014. Results: During the study 150 patients were admitted and 103 (68.7 %) of them died during follow-up. Hypogonadism was positively associated with mortality (P = 0.036). The percentage of hypogonadal patients was significantly (P = 0.02) higher in the group of patients who died in hospital compared with those who died after hospital discharge and those who survived. CV disease was the main cause of death in 52 patients (50.5 %). Kaplan-Meier analysis showed a median survival time for all-cause mortality of 2.0 (0-16.5) months and 21.0 (5.0-33.2) months for patients with and without hypogonadism, respectively (P < 0.001). Similar findings were found when analyzing mortality due to CV disease (P = 0.009). Hypogonadism was a strong independent predictor for all-cause (adjusted multivariate analysis, HR 3.35; 1.55-7.23, P = 0.002) and CV mortality (HR 2.14; 1.18-3.86, P = 0.012). Conclusions: Hypogonadism discovered during hospitalization is associated with in-hospital and long-term mortality in elderly male patients and predicts both all-cause mortality and CV mortality in this population. 46. Macroprolactinoma: a diagnostic and therapeutic update.

Abstract

Prolactinomas are the most common type of pituitary adenomas. Macroprolactinomas are the name used for these tumors when their size is ≥1 cm. These tumors commonly cause symptoms due to the excessive production of prolactin as well as complaints caused by tumor mass and compression of neural adjacent structures. Clinical diagnosis and assessment of macroprolactinoma are based on the measurement of serum prolactin concentrations and the morphological evaluation of the pituitary gland by magnetic resonance imaging. Dopamine agonists are the first-line treatment modality, with cabergoline being preferred to bromocriptine, because of its better tolerance and feasibility of administration. Cabergoline therapy has been reported to achieve normalization of prolactin levels and gonadal function and reduction of tumor volume in >50% of patients with macroprolactinoma. Resistance or intolerance to dopamine agonists are the main indications for transsphenoidal adenomectomy in patients with macroprolactinoma. External radiation therapy has been used in patients with poor response to medical and surgical procedures. Clinically significant tumor growth may occur during pregnancy in women with macroprolactinomas, especially if they have not received prior surgical or radiation therapy. Visual fields should be assessed periodically during pregnancy and therapy with dopamine agonists is indicated if symptomatic tumor growth occurs. Cystic and giant prolactinomas as well as the rare cases of malignant prolactinomas have special peculiarities and entail a therapeutic challenge. [ABSTRACT FROM AUTHOR]

Abstract

Background: Several studies have reported that thyrotropin (TSH) levels are slightly increased in obese subjects. On the contrary, other studies have provided no evidence for an association between thyroid status and body mass index (BMI). Aim: Our aim has been to evaluate the relationship between TSH and BMI in a group of subjects with obesity, overweight and normal weight with serum TSH levels within the reference range, excluding subjects with hypothyroidism and hyperthyroidism. Subjects and methods: We evaluated a cohort of 778 euthyroid subjects (488 women, mean age 58.5 ± 15.9 yr) with TSH values between 0.4 and 5.0 mU / l. There were 321 subjects with obesity (BMI ≥ 30 kg /m2), 319 with overweight (25 - 29.9 kg / m2) and 138 with normal weight (18.5 - 24.9 kg /m2). Results: Serum TSH levels significantly increased with weight category in studied subjects (normal weight, median [interquartile range], 1.24 [0.94 - 1.72]; overweight, 1.31 [1.00 - 1.93]; obesity, 1.66 [1.15 - 2.44] mU / l; P < 0.001). Free thyroxine (T4) was not statistically different in subjects within the 3 weight categories. A significant correlation was found between TSH and BMI (r = 0.217, P < 0.001). Subjects with serum TSH levels in the upper tertile (1.84 - 5.0 mU / l) exhibited a BMI value (30.8 [26.9 - 34.9] kg / m2) significantly higher (P < 0.001) than that found in subjects with TSH levels in the middle (1.13 - 1.83 mU / l, 28.8 [25.8 - 32.3] kg / m2) or in the lower tertile (0.4 - 1.12 mU / l, 27.4 [25.4 - 30.6] kg / m2). There were no diff erences in BMI in patients classified according to tertiles of free T4. However, when studying a subgroup of patients with negative thyroid autoimmunity (n = 375), we could not observe any significant diff erence in TSH levels between obese (n = 106, 1.24 [0.90 - 1.81] mU / l) and non-obese subjects (n = 269, 1.23 [0.90 - 1.80] mU / l). No correlation was found between TSH and BMI in this subgroup of subjects with negative thyroid autoimmunity (r = − 0.035, NS). Conclusion: These results suggest that the relationship between TSH and BMI observed in euthyroid subjects is lost when subjects with negative autoimmunity are selected. [ABSTRACT FROM AUTHOR]

Abstract

Monochamus galloprovincialis Olivier (Col., Cerambycidae) is a vector of the pine wood nematode, Bursaphelenchus xylophilus, causing the destructive pine wilt disease. An effective lure for monitoring and/or mass-trapping would be of great interest in the management of this pine sawyer. Males and females of this species show an attractive kairomonal response to blends composed of four pheromone compounds used by Ips spp. bark beetles and two host volatiles from pines. This six-component lure is highly attractive but may to be too complex and costly for practical use as each component is released from a separate lure. The role of each component, ipsdienol, ipsenol, cis-verbenol, methyl butenol, α-pinene and ethanol as attractants for M. galloprovincialis was field tested in Spain to obtain a simpler but equally effective bait. Ipsenol was confirmed as the strongest kairomonal signal to M. galloprovincialis synergizing response to α-pinene by 95 times. The addition of methyl butenol to this blend doubled the number of males and females trapped. On the other hand, neither ipsdienol, cis-verbenol nor ethanol improved the results when incorporated into the above three-component blend. A lure consisting of ipsenol, methyl butenol and α-pinene may be very cost-efficient in operational monitoring or mass trapping of M. galloprovincialis. Three potentially repellent candidates, (−)verbenone, methyl cyclohexenone and trans-conophthorin, were also tested against the attractive three-component bait. trans-Conophthorin significantly reduced male catches of M. galloprovincialis; methyl cyclohexenone had no effect. In order to evaluate whether rhGH administration exerts any influence on GH, thyrotropin (TSH|| and prolactin (PRL|| responses to TSH-releasing hormone (TRH||, we assessed these responses before and after rhGH therapy.
