Background: To achieve a successful result, the orthodontist must use a systematic approach to plan the orthodontic treatment. Defining the correct position of the upper and lower incisors and evaluating their relationship with intermaxillary discrepancy and facial divergence have been recognized as the starting point for the diagnostic decision regarding extractions and anchorage requirements. The aim of our study was to analyze the relationship between intermaxillary discrepancy (ANPg^), mandibular inclination (SN^GoGn), lip incompetence, and the positioning of the upper and lower incisors (UIPs and LIPs) in a group of orthodontic patients. This retrospective study included 290 lateral cephalograms in 122 males (42.1%) and 168 females (57.9%) aged 8 to 53 years (median 14; interquartile range IQR 12–17). Data were analyzed by means of one-way Analysis of Variance (ANOVA) and linear regression analysis. Results: This study showed a statistically significant increase in LIP values in patients with lip incompetence (p < 0.001). Moreover, the distribution of LIPs in groups with various SN^GoGn and ANPg^ angles was significantly different (p < 0.001). The regression analysis also showed a positive association between the LIP and SN^GoGn and between the LIP and ANPg^. Conclusion: The LIP presented a statistically significant difference in patients with and without lip incompetence, which varied significantly in subjects with different sagittal malocclusions and vertical face patterns. Background: Early orthopaedic treatment with rapid maxillary expansion (RME) associated with facemask (FM) has been shown to be effective in correcting Class III malocclusions in children. Treatment with pushing splints 3 (PS3) has shown to correct skeletal discrepancies in Class III growing patients. However, the effects of orthopaedic treatment on the upper airways in children with Class III malocclusion is controversial. Objectives: The aim of this study was to evaluate the cephalometric changes in the airways of PS3 compared to the RME/FM protocol. Materials and Methods: In this study, 48 patients with Class III malocclusion (age range 5.5–8.5 years old) were selected for this study, and 24 were treated with PS3 appliance and 24 with RME/FM therapy. Lateral cephalograms before (T0) and at the end of the treatment (T1) were analysed to compare pharyngeal spaces. Paired and unpaired t tests were used for data analysis (P <.05). Results: A total of 41 patients (21 patients for the PS3 group, 11 males and 10 females, mean age 7.0 ± 1.2 years; 20 patients for the RME/FM group, 10 males and 10 females, mean age 7.2 ± 1.3 years) were included. The results showed a statistically significant (P <.05) increase in the nasopharyngeal space after both therapies. However, the effects were similar considering that there were no differences between groups for the assessed variables at T0–T1. Conclusions: Early treatment of Class III malocclusion with PS3 does not induce a statistically significant increase in the sagittal airway space compared to RME/FM. Featured Application: The use of a new and valid reference system for analysis of facial scans might improve the quality of the assessment of orthodontics and orthognathic outcomes. Background: The aim of this study was to evaluate the accuracy and the repeatability of two reference systems for three-dimensional facial scans (FSs) compared with a reference system based on cone-beam computed tomography (CBCT). Subjects and methods: A total of sixty subjects, thirty growing participants (8–17 years old) and thirty non-growing participants (older than 21 years old), with FSs and full-field-of-view CBCT were included. Two different reference systems on the same FS were constructed. To assess validity, the two reference systems based on FSs were compared with the reference systems built using the CBCT scans. After two weeks, all of the FSs were reassessed to evaluate intra-operator repeatability. Reliability and repeatability were evaluated by means of parametric and non-parametric tests, intra-class correlation, the Dahlberg formula, and Bland–Altman plots (p < 0.05). Results: Both reference systems presented good reliability and showed a small difference with respect to the CBCT-based sagittal plane (Sagittal FS1 = 1.90 ± 0.98°; Sagittal FS2 = 1.80 ± 1.13°; p = 0.232). Between them, the two reference systems showed a small difference in the position of the sagittal plane (Sagittal FS1^Sagittal FS2 = 1.39 ± 1.13°). Conclusions: Both reference systems assessed in this study showed good intra-operator repeatability and their use may be suggested as reliable reference systems for FSs. [ABSTRACT FROM AUTHOR]

Clear aligners are employed daily for the treatment of several malocclusions. Previous clinical studies indicated low accuracy for the correction of tooth rotations. The aim of this study was to evaluate the predictability of tooth rotations with clear aligners. The sample comprised 390 teeth (190 mandibular; 200 maxillary), measured from the virtual models of 45 participants (21 men, 24 women; mean age: 29.2 ± 6.6 years old). For each patient, pre-treatment (T0) digital dental models (STL files), virtual plan (T1) and post-treatment digital dental models (T2) of both the mandibular and maxillary arches were imported onto Geomagic Control X, a 3D metrology software which allows angular measurements. Rotations were calculated by defining reproducible vectors for all teeth in each STL file and superimposing both T0 with T1 to determine the prescribed rotation, and T0 with T2 to determine the achieved rotation. Prescribed and achieved rotations were compared to assess movement's accuracy. The Wilcoxon signed-rank test and paired t-test were used to assess differences between the prescribed and achieved movements (P < 0.05). The overall predictability of rotational movement was 78.6% for the mandibular arch and 75.0% for the maxillary arch. Second molar accuracy was the lowest in both arches. Aim: The objective of this research has been to apply a specific simulation to a patient to assess the biomechanical consequences of rotating an upper canine tooth through different attachment–aligner configurations and to predict the most efficient design using a three-dimensional finite element model of a full maxillary arch of a specific patient. Materials and methods: This was obtained by combining Cone-Beam Computed Tomography (CBCT) with the aim of reconstructing tooth roots and bone tissues, and Surface Structured-Light Scanning for creating digital tooth crown models from the patient's impressions. This model was imported into the finite element solver (Ansys® 17). Three different attachment–aligner combinations were created through the exploitation of computer-aided design (CAD) procedures, i.e., without attachments, with a couple of attachments and with an attachment and a pressure point. For each simulation, the resulting force–moment (MF) system applied by the aligner to the target tooth, as well as the tooth displacement and rotation, was computed using a workstation based on Intel Xeon CPU E3-1245 v3@3.40 GHz and 16 GB RAM. Simulations reported that by adding the pressure point and the attachment to the standard aligner the amount of Moment z (Mz) delivered to the tooth increased almost two times. Results and conclusions: The maximum tooth displacement (0.85 mm) was obtained with the attachment and pressure point aligner, while the lowest (0.058 mm) was obtained with use of a couple of attachments. Both the attachment and the pressure point have the potential to enhance the appliance's effectiveness. Particularly, the pressure point showed a higher influence on the load absolute value. The method applied in the present study should be used to retrieve the best design configuration for each patient and specific tooth movement. [ABSTRACT FROM AUTHOR]

(1) Background: Orthodontic treatment may be a potential predisposing factor for ECR. The affected tooth goes to ankylosis, which could lead to a malocclusion. Although teeth severely affected by ECR (class IV Heithersay) are usually extracted, this case report aims to present the use of an ECR class IV upper canine, both as ankylosed to solve the malocclusion and the occlusal plane canting, as well as not ankylosed to correct its ridge defect with orthodontic extrusion. (2) Methods: A 14-year-old male, complaining of an ugly smile and a failed orthodontic attempt to recover an impacted canine, was referred to the orthodontic clinic. He was diagnosed with class II right subdivision, midline deviation, both upper and lower occlusal plane canting, and an upper left canine, previously impacted, showing ECR class IV. The treatment first included canting resolution with a cantilever and a spring, exploiting the anchorage offered by the ankylosed ECR canine. Then, a coronectomy, endodontic treatment, and orthodontic extrusion of that canine were performed to obtain the implant site development. (3) Results: Clinical and radiographic outcomes showed normocclusion and better bony conditions for safer implant placement in the aesthetic zone. (4) Conclusions: The high aesthetics and the periodontal and bony conditions obtained are probably not achievable by other therapeutic alternatives. [ABSTRACT FROM AUTHOR]

Arch expansion might be used to correct buccal corridors, improve smile aesthetics, resolve dental cross bite, and gain space to resolve crowding. In clear aligner treatment, the predictability of the expansion is still unclear. The purpose of this study was to evaluate the predictability of dentoalveolar expansion and molar inclination with clear aligners. In the study, 30 adult patients (27 ± 6.1 years old) treated with clear aligners were selected (treatment time: 8.8 ± 2.2 months). The upper and lower arch transverse diameters were measured for canines, first and second premolars, and first molars on two different sides (gingival margins and cusp tips); moreover, molar inclination was measured. A paired t-test and Wilcoxon test were used to compare prescription (planned movement) and achieved movement. In all cases, except for molar inclination, a statistically significant difference was found between achieved movement and prescription (p < 0.05). Our findings showed a total accuracy of 64% for the lower arch, 67% at the cusp level, and 59% at the gingival level, with a total accuracy of 67% for the upper arch, 71% at the cusp level, and 60% at the gingival level. The mean accuracy for molar inclination was 40%. Average expansion was greater at cusps of canines than for premolars, and it was lowest for molars. The expansion achieved with aligners is mainly due to the tipping of the crown rather than bodily movement of the tooth. The virtual plan overestimates the expansion of the teeth; thus, it is reasonable to plan an overcorrection when the arches are highly contracted. [ABSTRACT FROM AUTHOR]
