1. Principal factors affecting senior management culture change for total quality metamorphosis

Abstract

The main objective of the thesis was to identify the major Quality parameters and key success factors worldwide (assigned as "whats") that are necessary to bring about change in the senior Management culture towards leadership in Quality, and in addition, the factors (assigned as "hows") affecting the implementation of the key success factors in enterprises. In parallel to "whats" and "hows", identification was made of specific instructions, techniques and methods, (assigned as 3rd level "hows") demonstrating how to achieve the "how" factors. The three-level investigation aims to develop a universal model addressing the key success factors, so that, through different morphes (steps or stages) a complete Total Quality metamorphosis can be achieved. Through an extensive literature search focusing on research and studies of academics and practitioners, 72 whats, and 165 hows addressed by 73 3rd level hows were identified. The 72 "what" requirements were used as the basis for the questionnaire of an international survey directed to over 200 Chief Executive Officers and practitioners from 15 countries worldwide. The grading of those requirements was used as a weighting factor in analysis of the factors, which was accomplished using the Quality Function Development technique (QFD). Through a series of QFD matrices (phase I and 11) a modified QFD matrix was developed relating the 72 "whats" and 165 "hows" containing the necessary information for analysis and identification of the principal "whats", "hows", 3rd level "hows" factors. The analysis reveals overall agreement on the most important success factors. This enables the development of an analytic model for a Chief Executive Officers culture change implementation process necessary to achieve Total Quality metamorphosis and Senior Manager leadership. 1. Understanding change, its meaning and implementation; 2. Influence or Convince management and gain support from Board and senior management for the Total Quality Management implementation; 3. Secure ownership and internalised strong top down commitment; 4. A communicated, clear and common vision of where company is going, its goals, values, ethics etc.; 5. Adopt a vision led strategic and holistic approach to change; 6. Measure and control the processes, using an internal coordinator; 7. Form an action/strategic plan with actionable steps and stages; 8. Enhance the company's perception of quality and management system; 9. Develop an effective decision, assessment, implementation and monitoring system; 10. Increase and maintain Senior Management effectiveness, development, and capabilities; 11. Senior Management must find time for changing and developing a strong culture; 12. Establish a top-level quality improvement committee; 13. Benefit from empowered, cooperative, motivated people; 14. Succeed Total Quality metamorphosis and SM leadership. The above implementations process steps are enhanced and achieved by the use of the "how· factors identified. The success of the whole model process revolves around the prime factor for management which is also the cornerstone for any Total Quality system; "Acceptance that the customer (internal and external) is paramount" and appreciate its implication for the company".

4. Acute perchloroethylene intoxication in an elderly woman: a case report

Abstract

Background: Perchloroethylene is a colorless, strong-smelling substance commonly used for dry cleaning. Liver and kidney toxicities and carcinogenicity are well-known occupational hazards caused by chronic perchloroethylene exposure. Acute intoxication by ingestion of nondiluted perchloroethylene is rare in the adult population owing to its strong smell and taste. Very few data are available to physicians managing patients in this situation. Case presentation: An 89-year-old Caucasian woman accidentally drank perchloroethylene while visiting her laundry, leading to a coma within a few minutes. The poison control center provided little information about perchloroethylene toxicity after ingestion, including an estimated long biological half-life (144 hour) and detrimental effects to liver and kidneys. A long intensive care unit stay was thus expected, potentially leading to several complications. After intubation, transitory hypoxemia appeared and rapidly resolved, while mild hemodynamic instability was managed with fluid resuscitation and anti-arrhythmic drugs. Twelve hours after perchloroethylene ingestion, the patient suddenly woke up and self-extubated. Less than 24 hours after ingestion, she was discharged from the intensive care unit, and 4 days later she was discharged home. Conclusion: The patient drank perchloroethylene from a bottle, which prevented her from smelling it, and owing to its taste, only a small sip was likely drunk. However, a much larger intake was presumed, given her rapid and profound central nervous system depression. This case was challenging owing to the paucity of information available regarding acute perchloroethylene ingestion and the duration and magnitude of its effect. The present report will hopefully be of support for clinicians managing patients with this rare acute intoxication.

5. Reporting on the Value of Artificial Intelligence in Predicting the Optimal Embryo for Transfer: A Systematic Review including Data Synthesis

Abstract

Artificial intelligence (AI) has been gaining support in the field of in vitro fertilization (IVF). Despite the promising existing data, AI cannot yet claim gold-standard status, which serves as the rationale for this study. This systematic review and data synthesis aims to evaluate and report on the predictive capabilities of AI-based prediction models regarding IVF outcome. The study has been registered in PROSPERO (CRD42021242097). Following a systematic search of the literature in Pubmed/Medline, Embase, and Cochrane Central Library, 18 studies were identified as eligible for inclusion. Regarding live-birth, the Area Under the Curve (AUC) of the Summary Receiver Operating Characteristics (SROC) was 0.905, while the partial AUC (pAUC) was 0.755. The Observed: Expected ratio was 1.12 (95%CI: 0.26–2.37; 95%PI: 0.02–6.54). Regarding clinical pregnancy with fetal heartbeat, the AUC of the SROC was 0.722, while the pAUC was 0.774. The O:E ratio was 0.77 (95%CI: 0.54–1.05; 95%PI: 0.21–1.62). According to this data synthesis, the majority of the AI-based prediction models are successful in accurately predicting the IVF outcome regarding live birth, clinical pregnancy, clinical pregnancy with fetal heartbeat, and ploidy status. This review attempted to compare between AI and human prediction capabilities, and although studies do not allow for a meta-analysis, this systematic review indicates that the AI-based prediction models perform rather similarly to the embryologists’ evaluations. While AI models appear marginally more effective, they still have some way to go before they can claim to significantly surpass the clinical embryologists’ predictive competence. © 2022 by the authors. Abstract

Almost 20% of Italy is characterized by the outcropping of carbonate massifs ranging in age from Cambrian to Quaternary. Coastal karst is present in many Italian regions: from North-East to South and West: the Gulf of Trieste, the Conero (South of Ancona, Marche), the Adriatic coast of Apulia including Gargano, Murge and Salento, Maratea in Basilicata, Cilento in Campania, Circeo and Gaeta in Latium, Argentario and Giannutri Island in Tuscany, the southernmost part of the Ligurian Alps, Palermo Mts., San Vito Lo Capo, Syracuse coast and Marettimo Island in Sicily, and, especially, in Sardinia, which has carbonate rocks touching the sea along the coast of Balai near Porto Torres, Capo Caccia-Punta Giglio (Alghero), Sinis and Buggerru along the western litoral, Capo Teulada and Capo Sant’Elia at Cagliari, Capo Figari, Tavolara Island and the Gulf of Orosei along the eastern mountainside. Recent researches have revealed several coastal cave systems that have a clear origin by mixing corrosion, in which the aggressive solution derives from the mixing between saline and fresh water at the watertable interface (the so-called flank margin caves). Glacioeustasy and tectonic movements can control the position of sea level with respect to coastal carbonate outcrops. For this reason these coastal caves represent useful records of sea-level stillstands. These caves are normally organized in sub-horizontal levels, and are characterized by the lack of high flow velocity markers (scallops) and alluvial sediments. Instead, they show rounded cave passage morphologies, often with horizontal wall notches, a characteristic swiss-cheese or sponge morphology, and passages that narrow going away from the coastline (due to the decreasing of sea water influence and mixing-corrosion effect). This paper describes some flank margin cave systems found in Apulia, Sicily, and Sardinia. In particular, five cave systems are illustrated: Sant’Angelo caves (Apulia), Pellegrino and Rumena caves (Sicily), and Giuanniccu Mene cave and Fico cave (Sardinia), explaining their relationship with past sea levels and local uplift rate.

Abstract

Italy is a country hosting a large number of hypogenic sulfuric acid (SAS) speleogenesis caves, mostly located along the Apennine chain, but also in Campania (along the coastline of Capo Palinuro), Apulia (along the coastline of Santa Cesarea Terme) and Sicily. Besides the typical morphologies related to their special geochemical origin (cupolas, replacement pockets, bubble trails, etc), these caves often host abundant secondary mineral deposits, mainly gypsum, being the result of the interaction between the sulfuric acid and the carbonate host rock. Native sulfur deposits are also well visible on the ceiling and roof, and peculiar sulfuric acid minerals such as jarosite, alunite, and other sulfates like copiapite, pickeringite, tschermigite, tamarugite (probably related to the weathering of native clay minerals) have been found in those caves. The presence of typical SAS minerals, together with the morphologies, testifies the influence of rising acidic waters, that likely interact with the deep-seated Triassic evaporite deposits (along the Apennine chain), with volcanic sources or hydrothermal springs in the Tyrrhenian sea (in Campania) and with marine waters that infiltrate on the sea bottom and rise through deep faults (in Apulia). This paper describes the secondary minerals discovered in several caves, and discusses their origin and possible use in reconstructing the evolution of these cave systems.

Abstract

Sulphuric acid speleogenesis (SAS) related to the upwelling of acid water enriched in H2S and CO2 represents an unusual way of cave development. Since meteoric infiltration waters are not necessarily involved in speleogenesis, caves can form without the typical associated karst expressions (i.e. dolines) at the surface. The main mechanism of sulphuric acid dissolution is the oxidation of H2S (Jones et al., 2015) which can be amplified by bacterial mediation (Engel et al., 2004). In these conditions, carbonate dissolution associated with gypsum replacement, is generally believed to be faster than the normal epigenic one (De Waele et al., 2016). In Italy several SAS caves have been identified, but only few systems have been studied in detail: Frasassi and Acquasanta Terme (Marche)(Galdenzi et al., 2010), Monte Cucco (Umbria) (Galdenzi & Menichetti, 1995), and Montecchio (Tuscany) (Piccini et al., 2015). Other preliminary studies have been carried out in Calabria (Galdenzi, 2007) and Sicily (De Waele et al., 2016). Several less studied SAS cave systems located in South Italy, and in particular in Apulia (Santa Cesarea Terme), Sicily (Acqua Fitusa, Acqua Mintina) and Calabria (Mt. Sellaro and Cassano allo Ionio) have been selected in the framework of a PhD thesis on SAS caves and their speleogenesis. Using both limestone tablet weight loss (Galdenzi et al., 2012) and micro erosion meter (MEM) (Furlani et al., 2010) methods the dissolution rate above and under water in the caves will be quantified. Geomorphological observations, landscape analysis using GIS tools, and the analysis of gypsum and other secondary minerals (alunite and jarosite) (stable isotopes and dating) will help to reconstruct the speleogenetic stages of cave formation. Preliminary microbiological analysis will determine the microbial diversity and ecology in the biofilms.

Abstract

Introduction: The microbial diversity in the Su Bentu Cave (Sardinia, Italy) was investigated by means of Illumina MiSeq analysis. The hypogean environment is of great interest for astrobiological research as cave conditions may resemble those in extra-terrestrial regions. Furthermore, they hold high potential to identify novel, extremely adapted organisms to severely oligo-trophic habitats. However, the influence of human is not neglectable and in-depth investigations are needed to determine the impact of exploration on an otherwise mostly pristine ecosystem. The cave investigated in this study develops for several kilometres into the mountain, two hundred metres below the topographic surface and is characterized by a strong air circulation. Its structure is composed of huge passages carved in limestone where an ephemeral underground stream creates some lakes, close to which seven samples of visible calcite rafts, manganese deposits and moonmilk (a hydrated calcium carbonate speleothem), were sampled during an expedition in 2014. Other samples were re-trieved from a frequently used campsite and from some dry cave passages leading deeper into the cave.

Abstract

Many morphological elements in Cuba's landscape (e.g. marine terraces, tidal notches) demonstrate that coastal uplift has taken place, but the rate at which this occurs is not known. Carbonate phreatic overgrowths on speleothems have been found in a cave in Central North Cuba, ~1 km from the present coastline at 16 m asl. They form exceptional and unique mushroom-shaped speleothems and balconies decorating the walls of the rooms. These phreatic overgrowths on speleothems (POS) formed at the oscillating air–water interface in sea-level controlled anchialine lakes. U/Th dating of these overgrowths suggests ages that are compatible with the Marine Isotope Stage 5e (i.e. 130–115 ka). These POS have fixed this sea-level highstand and demonstrate that this part of Cuba has been subjected to a much lower uplift rate than previously reported, that is, less than 0.1 mm/year since the last interglacial.
