Abstract

KM3NeT is a research infrastructure being installed in the deep Mediterranean Sea. It will house a neutrino telescope comprising hundreds of networked moorings - detection units or strings equipped with optical instrumentation to detect the Cherenkov radiation generated by charged particles from neutrino-induced collisions in its vicinity. In comparison to moorings typically used for oceanography, several key features of the KM3NeT string are different: the instrumentation is contained in transparent and thus unprotected glass spheres; two thin Dyneema ropes are used as strength members; and a thin delicate backbone tube with fibre-optics and copper wires for data and power transmission, respectively, runs along the full length of the mooring. Also, compared to other neutrino telescopes such as ANTARES in the Mediterranean Sea and GVD in Lake Baikal, the KM3NeT strings are more slender to minimise the amount of material used for support of the optical sensors. Moreover, the rate of deploying a large number of strings in a period of a few years is unprecedented. For all these reasons, for the installation of the KM3NeT strings, a custom-made, fast deployment method was designed. Despite the length of several hundreds of metres, the slim design of the string allows it to be compacted into a small, re-usable spherical launching vehicle instead of deploying the mooring weight down from a surface vessel. After being lowered to the seafloor, the string unfurls to its full length with the buoyant launching vehicle rolling along the two ropes.The design of the vehicle, the loading with a string, and its underwater self-unrolling are detailed in this paper.

Abstract

The KM3NeT research infrastructure being built at the bottom of the Mediterranean Sea will host water-Cherenkov telescopes for the detection of cosmic neutrinos. The neutrino telescopes will consist of large volume three-dimensional grids of optical modules to detect the Cherenkov light from charged particles produced by neutrino-induced interactions. Each optical module houses 31 3-inch photomultiplier tubes, instrumentation for calibration of the photomultiplier signal and positioning of the optical module and all associated electronics boards. By design, the total electrical power consumption of an optical module has been capped at seven watts. This paper presents an overview of the front-end and readout electronics system inside the optical module, which has been designed for a 1~ns synchronization between the clocks of all optical modules in the grid during a life time of at least 20 years., Comment: Prepared submission to Journal of Astronomical Telescopes, Instruments, and Systems (JATIS)

Abstract

KM3NeT is a research infrastructure located in the Mediterranean Sea, that will consist of two deep-sea Cherenkov neutrino detectors. With one detector (ARCA), the KM3NeT Collaboration aims at identifying and studying TeV-PeV astrophysical neutrino sources. With the other detector (ORCA), the neutrino mass ordering will be determined by studying GeV-scale atmospheric neutrino oscillations. The first KM3NeT detection units were deployed at the Italian and French sites between 2015 and 2017. In this paper, a description of the detector is presented, together with a summary of the procedures used to calibrate the detector in-situ. Finally, the measurement of the atmospheric muon flux between 2232-3386 m seawater depth is obtained., Comment: 15 pages, 9 figures

Abstract

Studying atmospheric neutrino oscillations in the few-GeV range with a multimegaton detector promises to determine the neutrino mass hierarchy. This is the main science goal pursued by the future KM3NeT/ORCA water Cherenkov detector in the Mediterranean Sea. In this paper, the processes that limit the obtainable resolution in both energy and direction in charged-current neutrino events in the ORCA detector are investigated. These processes include the composition of the hadronic fragmentation products, the subsequent particle propagation and the photon-sampling fraction of the detector. GEANT simulations of neutrino interactions in seawater produced by GENIE are used to study the effects in the 1 - 20 GeV range. It is found that fluctuations in the hadronic cascade in conjunction with the variation of the inelasticity y are most detrimental to the resolutions. The effect of limited photon sampling in the detector is of significantly less importance. Abstract

A prototype detection unit of the KM3NeT deep-sea neutrino telescope has been installed at 3500m depth 80km offshore the Italian coast. KM3NeT in its final configuration will contain several hundreds of detection units. Each detection unit is a mechanical structure anchored to the sea floor, held vertical by a submerged buoy and supporting optical modules for the detection of Cherenkov light emitted by charged secondary particles emerging from neutrino interactions. This prototype string implements three optical modules with 31 photomultiplier tubes each. These optical modules were developed by the KM3NeT Collaboration to enhance the detection capability of neutrino interactions. The prototype detection unit was operated since its deployment in May 2014 until its decommissioning in July 2015. Reconstruction of the particle trajectories from the data requires a nanosecond accuracy in the time calibration. A procedure for relative time calibration of the photomultiplier tubes contained in each optical module is described. This procedure is based on the measured coincidences produced in the sea by the 40K background light and can easily be expanded to a detector with several thousands of optical modules. The time offsets between the different optical modules are obtained using LED nanobeacons mounted inside them. A set of data corresponding to 600 hours of livetime was analysed. The results show good agreement with Monte Carlo simulations of the expected optical background and the signal from atmospheric muons. An almost background-free sample of muons was selected by filtering the time correlated signals on all the three optical modules. Abstract

The NEMO Phase-2 tower is the first detector which was operated underwater for more than one year at the "record" depth of 3500 m. It was designed and built within the framework of the NEMO (NEutrino Mediterranean Observatory) project. The 380 m high tower was successfully installed in March 2013 80 km offshore Capo Passero (Italy). This is the first prototype operated on the site where the italian node of the KM3NeT neutrino telescope will be built. The installation and operation of the NEMO Phase-2 tower has proven the functionality of the infrastructure and the operability at 3500 m depth. A more than one year long monitoring of the deep water characteristics of the site has been also provided. In this paper the infrastructure and the tower structure and instrumentation are described. The results of long term optical background measurements are presented. The rates show stable and low baseline values, compatible with the contribution of 40K light emission, with a small percentage of light bursts due to bioluminescence. Abstract

The results of the analysis of the data collected with the NEMO Phase-2 tower, deployed at 3500 m depth about 80 km off-shore Capo Passero (Italy), are presented. Cherenkov photons detected with the photomultipliers tubes were used to reconstruct the tracks of atmospheric muons. Their zenith-angle distribution was measured and the results compared with Monte Carlo simulations. An evaluation of the systematic effects due to uncertainties on environmental and detector parameters is also included. The associated depth intensity relation was evaluated and compared with previous measurements and theoretical predictions. Abstract

The first prototype of a photo-detection unit of the future KM3NeT neutrino telescope has been deployed in the deep waters of the Mediterranean Sea. This digital optical module has a novel design with a very large photocathode area segmented by the use of 31 three inch photomultiplier tubes. It has been integrated in the ANTARES detector for in-situ testing and validation. This paper reports on the first months of data taking and rate measurements. The analysis results highlight the capabilities of the new module design in terms of background suppression and signal recognition. The directionality of the optical module enables the recognition of multiple Cherenkov photons from the same $^{40}$K decay and the localization bioluminescent activity in the neighbourhood. The single unit can cleanly identify atmospheric muons and provide sensitivity to the muon arrival directions.

Abstract

In a cohort of subjects with asymptomatic carotid atherosclerosis (ACA), we have evaluated the neutrophil and lymphocyte count and their ratio (NLR), the gelatinases (MMP-2 and MMP-9) and their tissue inhibitors (TIMP-1 and TIMP-2). At baseline, no difference was observed between ACA subjects and subject control group regarding neutrophil and lymphocyte count while was evident in ACA subjects a significant increase in MMP-2, MMP-9 and TIMP-2 associated to a significant decrease in TIMP-1. Dividing the ACA according to the number of cardiovascular risk factors (CRFs) we have observed an increase in lymphocyte count in the subgroup with 3–5 CRFs. Evaluating the leukocyte subtypes according to all the surrogate markers of insulin resistance has been noted, in the subgroups that exceed the medians of these markers, a significant increase in neutrophil and lymphocyte count without any variation of the NLR. Effecting the same evaluation for the MMP/TIMP pattern we observed, instead, that the same subgroups tend to show a decrease in MMP-2 and an increase in MMP-9. No difference instead for TIMP-1 and TIMP-2. The abnormality of the MMP/TIMP pattern, bearing in mind the cardiometabolic clustering present in this cohort of ACA subjects, would induce to use drugs able not only to cure the cardiometabolic risk factors but also to influence the MMP/TIMP profile.

Abstract

The optical module of the KM3NeT neutrino telescope is an innovative multi-faceted large area photodetection module. It contains 31 three-inch photomultiplier tubes in a single 0.44m diameter pressure-resistant glass sphere. The module is a sensory device also comprising calibration instruments and electronics for power, readout and data acquisition. It is capped with a breakout-box with electronics for connection to an electro-optical cable for power and long-distance communication to the onshore control station. The design of the module was qualified for the first time in the deep sea in 2013. Since then, the technology has been further improved to meet requirements of scalability, cost-effectiveness and high reliability. The module features a sub-nanosecond timing accuracy and a dynamic range allowing the measurement of a single photon up to a cascade of thousands of photons, suited for the measurement of the Cherenkov radiation induced in water by secondary particles from interactions of neutrinos with energies in the range of GeV to PeV. A distributed production model has been implemented for the delivery of more than 6000 modules in the coming few years with an average production rate of more than 100 modules per month. Abstract

Remote sensing measurements offer great potential to quantify evapotranspiration (ET) more accurately. This work assesses several remotely-sensed ET datasets at high, medium and coarse scale through a comparison with eddy covariance (EC) measurements. In particular, daily retrievals obtained from the Sen-ET model and the ECOSTRESS ET product, both having resolutions of tens of meters, were considered. In addition, 3-km daily ET estimates derived from SEVIRI were analyzed, together with 8-day MODIS ET composites at 500-m spatial resolution. The remotely-sensed actual ET retrievals at medium and coarse scale were compared with EC observations at five forest and grassland sites over different climate zones in Italy. Additionally, for one of these sites (Monte Bondone), the high-resolution ET estimates within the EC footprint were also evaluated. Sen-ET retrievals demonstrated a remarkable ability to describe EC latent heat fluxes (e.g., Pearson correlation coefficient, R, equal to 0.83, and unbiased root-mean-square-difference, ubRMSD, equal to 0.85 mm/day), and therefore great potential for producing high-resolution ET estimates, while ECOSTRESS data were less performant (e.g., R = 0.46, and ubRMSD = 1.65 mm/day). Despite the spatial differences between EC footprint size and the medium along with coarse scale of remotely-sensed datasets, the latter both showed a good agreement with the measured EC fluxes. The SEVIRI ET, in particular, proved to be significantly suitable for the ET description also at a local scale (at daily time step, average R = 0.84, and average ubRMSD = 0.83 mm/day over the study sites), despite a tendency to underestimate in-situ measurements (negative biases ranging between 0.2 and 0.85 mm/day were observed). Abstract

To date, the virtual multidisciplinary tumor boards (vMTBs) are increasingly used to achieve high-quality treatment recommendations across health-care regions, which expands and develops the local MTB team to a regional or national expert network. This review describes the process of lung cancer-specific MTBs and the transition process from face-to-face tumor boards to virtual ones. The review also focuses on the project organization's description, advantages, and disadvantages. Semi-structured interviews identified five major themes for MTBs: current practice, attitudes, enablers, barriers, and benefits for the MTB. MTB teams exhibited positive responses to modeled data feedback. Virtualization reduces time spent for travel, allowing easier and timely patient discussions. This process requires a secure web platform to assure the respect of patients’ privacy and presents the same unanswered problems. The implementation of vMTB also permits the implementation of networks especially in areas with geographical barriers facilitating interaction between large referral cancer centers and tertiary or community hospitals as well as easier access to clinical trial opportunities. Studies aimed to improve preparations, structure, and conduct of MTBs, research methods to monitor their performance, teamwork, and outcomes are also outlined in this article. Analysis of literature shows that MTB participants discuss 5–8 cases per meeting and that the use of a vMTB for lung cancer and in particular stage III NSCLC and complex stage IV cases is widely accepted by most health professionals.Despite still-existing gaps, overall vMTB represents a unique opportunity to optimize patient management in apatient-centeredapproach.

Abstract

The modern revisitation of the local cultural and productive tradition is undoubtedly one of the key elements for the future development of a more modern and responsible economy, which accompanies a genetic identity that belongs to us and with which we identify. Sustainable fashion, despite being a driving sustainable need, takes us to a future based on the past where, for many decades, care for resources was a priority. The improvement methodology presented here is based on operational tools, contributing with their production to promote the adoption of new processes and more conscious production, with the objective of helping to change a mindset to quality and integrated ecosystems. The expected result of this research is a discussion to propose a series of recommendations that can contribute to adopting a change in the entrepreneurial system of SMEs, focusing on contradictions, persisting barriers and shared measures.

Abstract

BackgroundSince 2016, following the Italian “National Plan to Contrast Antimicrobial Resistance”, Campania Region has implemented an antimicrobial stewardship program, including the obligation to associate an appropriate International Classification of Diseases-9 code to each antibiotic prescription, the publication of schemes for empirical antibiotic therapy and educational interventions.MethodsTo evaluate the impact of these interventions on the prescribing habits of family pediatricians, we conducted a retrospective cohort study (January 2016–December 2020), including all patients registered in an associate practice of Primary Care Pediatricians. We collected data on antibiotic prescriptions through a specific study management software; our primary outcomes were the annual prescription rates, calculated for both the number of patients in follow-up and the number of medical consultations, and the annual prescription rates for selected antibiotic classes and molecules. To investigate the hypothesis that chronic conditions would be associated with an increased rate of prescription, we also tested the association between underlying conditions and the number of antibiotics received.ResultsDuring the study period, 2,599 children received 11,364 antibiotic prescriptions (mean 4.37, SD 4.28). From 2016 to 2020 we observed a substantial reduction in both the annual prescription rate per 100 patients (9.33 to 3.39; R2 = 0.927, p = 0.009), and the annual prescription rate per 100 medical consultations (25.49 to 15.98; R2 = 0.996, p R2 = 0.983, p = 0.001) and third generation Cephalosporins (28.43 to 5.43; R2 = 0.995, p DiscussionFollowing the implementation of the regional interventions on antimicrobial stewardship, we observed a substantial reduction in the overall antibiotic prescription per patients and per medical consultations, with a statistically significant reduction in the use of broad-spectrum molecules. Considering the results of our analysis, new guidance and training interventions addressed to specialists in the primary care sector should be implemented to further limit antibiotic resistance.

Abstract

In this article, we provide a comprehensive introduction to knowledge graphs, which have recently garnered significant attention from both industry and academia in scenarios that require exploiting diverse, dynamic, large-scale collections of data. After some opening remarks, we motivate and contrast various graph-based data models, as well as languages used to query and validate knowledge graphs. We explain how knowledge can be represented and extracted using a combination of deductive and inductive techniques. We conclude with high-level future research directions for knowledge graphs.

Abstract

In recent years, we saw the emergence of several approaches for producing machine-readable, semantically rich, interlinked description of the content of research publications, typically encoded as knowledge graphs. A common limitation of these solutions is that they address a low number of articles, either because they rely on human experts to summarize information from the literature or because they focus on specific research areas. In this paper, we introduce the Computer Science Knowledge Graph (CS-KG), a large-scale knowledge graph composed by over 350M RDF triples describing 41M statements from 6.7M articles about 10M entities linked by 179 semantic relations. It was automatically generated and will be periodically updated by applying an information extraction pipeline on a large repository of research papers. CS-KG is much larger than all comparable solutions and offers a very comprehensive representation of tasks, methods, materials, and metrics in Computer Science. It can support a variety of intelligent services, such as advanced literature search, document classification, article recommendation, trend forecasting, hypothesis generation, and many others. CS-KG was evaluated against a benchmark of manually annotated statements, yielding excellent results.

Abstract

Research publishing companies need to constantly monitor and compare scientific journals and conferences in order to inform critical business and editorial decisions. Semantic Web and Knowledge Graph technologies are natural solutions since they allow these companies to integrate, represent, and analyse a large quantity of information from heterogeneous sources. In this paper, we present the AIDA Dashboard 2.0, an innovative system developed in collaboration with Springer Nature to analyse and compare scientific venues, now also available to the public. This tool builds on a knowledge graph which includes over 1.5B RDF triples and was produced by integrating information about 25M research articles from Microsoft Academic Graph, Dimensions, DBpedia, GRID, CSO, and INDUSO. It can produce sophisticated analytics and rankings that are not available in alternative systems. We discuss the advantages of this solution for the Springer Nature editorial process and present a user study involving 5 editors and 5 researchers, which yielded excellent results in terms of quality of the analytics and usability.

Abstract

We present a cohort of 100 subjects [43 men and 57 women; median age 66.00(25)] who were tested using carotid ultrasound to identify subclinical carotid atherosclerosis (SCA). We have evaluated the behaviour of whole blood viscosity (WBV) at high (450 s–1) and low (0.51 s–1) shear rates, plasma viscosity (450–1), hematocrit and mean erythrocyte aggregation. When compared to normal control subjects, using the Mann-Whitney test, we observed in SCA patients a significant increase in WBV only. The results were substantial after having divided the SCA subjects according to the cardiovascular risk factors (CRFs) and the degree of insulin resistance; the research was performed using two surrogate indexes such as TG/HDL-C and TyG. With the division carried out according to CRFs, employing the Kruskal-Wallis test, results show a significant increase in WBV (at high and low shear rates), in plasma viscosity, in erythrocyte aggregation and plasma fibrinogen level. Whereas by dividing them into the median of TG/HDL-C and TyG, we noticed a significant increase in WBV (at high and low shear rates) and in erythrocyte aggregation in the two groups with high TG/HDL-C ratio and with high TyG; having found an increased level of plasma fibrinogen in the latter. The data underlines the role of the main hemorheologic aspects in subclinical carotid atherosclerosis being closely correlated to the CRFs and different degrees of insulin resistance.

Abstract

Acute aortic syndromes are life-threatening medical conditions that include classic acute aortic dissection (AAD), aortic intramural hematoma, penetrating aortic ulcer, and even aortic pseudoaneurysm and traumatic aortic injury. The European Society of Cardiology has designed a multiparametric diagnostic algorithm to provide stepwise diagnosis. All patients with AAD should receive aggressive medical therapy to control blood pressure and heart rate. Urgent surgical repair is recommended for type A AAD. Uncomplicated type B AAD requires aggressive medical therapy. In contrast thoracic endovascular repair is recommended for complicated type B. AAD should be considered a lifelong disease that affects the entire aorta.

Abstract

Background/aim Upper limb breast cancer-related lymphedema (BCRL) is a chronic and severe condition affecting a significant percentage of breast cancer survivors. Even though its physiopathology is well-known, there is no worldwide consensus on BCRL evaluation and a gold-standard treatment. This narrative review aims at providing a brief descriptive overview with regard to BCRL treatment modalities. Materials and methods We conducted a literature search within the PubMed database, and 33 articles out of 56 were selected, including reviews, systematic reviews, and meta-analyses aiming find the most updated evidence regarding BCRL treatment modalities. Results Physical exercise (aerobic exercise, resistance exercise, aquatic therapy), bandages, and intermittent pneumatic compression were shown to be most effective in BCRL patients, in terms of swelling reduction in the acute-intensive phase. Furthermore, physical exercise was beneficial also as a maintenance tool. Manual lymphatic drainage demonstrated efficacy in preventing secondary lymphedema if applied immediately after breast cancer surgery or in early phases of BCRL or as a maintenance tool. Complementary procedures such as acupuncture, reflexology, yoga and photo-biomodulation therapy did not show conclusive results in BCRL treatment. Surgery was shown effective in managing symptoms (liposuction), preventing (lymphaticovenular anastomosis) and treating BCRL (vascularized lymph node transfer). Conclusion BCRL is still a challenging condition either for breast cancer survivors and clinicians, deeply impacting patient functioning and quality of life. Due to the lack of globally accepted criteria in evaluating BCRL, to date a gold standard treatment for this widespread issue is still needed.
