Publisher Summary Unmodified extinction is an effective treatment for childhood insomnia. It is effective in reducing inappropriate bedtime behaviors (e.g., crying, repeatedly getting out of bed, and tantrums) that interfere with sleep onset and maintenance. These behaviors are often maintained by reinforcement in the form of parental attention and insufficient limit setting. These child behaviors often occur at bedtime and at waking up at night (night waking) or in the early morning. This treatment may also be effective in case of co-sleeping, whereby child and parent sleep in the same bed. There is no contraindication to the use of unmodified extinction. However, prior to starting an extinction procedure, any somatic, neurological, or other factor that may be responsible for bedtime and night-waking problems should be assessed and ruled out. There are many such factors, such as breathing difficulties, epilepsy, and melatonin synthesis disturbance.

Patients with obstructive sleep apnea (OSA) are likely to develop cognitive impairments, including problems with concentration, memory, and executive function. The etiology of these deficits is not fully resolved, although sleep fragmentation, excessive sleepiness, and hypoxemia appear to interact to produce neurologic consequences in patients with OSA. Disturbingly, a proportion of the cognitive dysfunction caused by hypoxic brain damage may be irreversible. Many patients with OSA will present in the primary care setting and may not know that they have a sleep disorder. Vigilance for the cognitive symptoms of OSA could assist in recognition by primary care physicians and aid in the prevention of lasting neurocognitive sequelae in this patient population. - Abstract
We investigated the relationship between neurocognitive functioning and quality of life/self-perceived health status (QOL) among cardiac rehabilitation (CR) patients to determine whether level of neurocognitive functioning is related to baseline QOL and improvement following CR. CR patients (n = 35) were given a neurocognitive screening before participation in CR, and also completed a behavioral inventory (SF-36) before and after CR to measure QOL associated with medical illness. At baseline, CR patients obtained relatively low SF-36 scores compared with published norms, and as reported previously, demonstrated inferior neurocognitive performance compared with healthy controls. Furthermore, neurocognitive performance was strongly positively correlated to SF-36 scores. Significant improvements were evident on many of the SF-36 subscales following rehabilitation. These improvements were relatively greater among SF-36 indexes of physical health status compared with SF-36 indexes of mental health status. Baseline neurocognitive performance also correlated strongly to the degree of improvement in SF-36 scores following rehabilitation. These findings indicate a strong relationship between baseline neurocognitive functioning and QOL before CR, and the degree to which QOL improves following this intervention. [ABSTRACT FROM AUTHOR]

Study objectives Obstructive sleep apnea (OSA) is commonly associated with cognitive and functional deficits, some of which are resolved after continuous positive airway pressure (CPAP) treatment. The investigation of brain structural changes before and after treatment could provide deep insights into the pathogenesis and the reversibility of this disorder. We hypothesized that severe OSA patients would have altered white matter (WM) integrity and cognition and that treatment would improve both the structural damage and the cognitive impairment. Design Prospective clinical study. Setting The Sleep Disorders Center and the Center of Excellence in High-Field Magnetic Resonance Imaging at Vita-Salute San Raffaele University, Milan, Italy. Participants Seventeen never-treated consecutive OSA patients were evaluated before and after treatment (after 3 and 12 months) and compared to 15 matched healthy controls. Intervention CPAP. Measurements WM integrity measured by diffusion tensor imaging (DTI) and cognitive performance (measured with neuropsychological testing) before and after 3 and 12 months of CPAP. Results Results in pre-treatment OSA patients showed impairments in most cognitive areas, mood and sleepiness that were associated with diffuse reduction of WM fiber integrity reflected by diminished fractional anisotropy (FA) and mean diffusivity (MD) in multiple brain areas. After 3 months of CPAP, only limited changes of WM were found. However, over the course of 12 months CPAP treatment, an almost complete reversal of WM abnormalities in all the affected regions was observed in patients who were compliant with treatment. Significant improvements involving memory, attention, and executive-functioning paralleled WM changes after treatment. Conclusions Changes of WM DTI "signatures" of brain pathology in OSA patients are appreciable over the course of 12-month treatment with CPAP in most of the regions involved. Recovery of cognitive deficits after treatment is consistent with the presence of a reversible structural neural injury in OSA in patients who were compliant with treatment.

"RATIONALE:. . Obstructive sleep apnea (OSA) is commonly associated with neurocognitive impairments that have not been consistently related to specific brain structure abnormalities. Knowledge of the brain structures involved in OSA and the corresponding functional implications could provide clues to the pathogenesis of cognitive impairment and its reversibility in this disorder.. OBJECTIVES:. . To investigate the cognitive deficits and the corresponding brain morphology changes in OSA, and the modifications after treatment, using combined neuropsychologic testing and voxel-based morphometry.. METHODS:. . A total of 17 patients treatment-naive to sleep apnea and 15 age-matched healthy control subjects underwent a sleep study, cognitive tests, and magnetic resonance imaging. After 3 months of treatment, cognitive and imaging data were collected to assess therapy efficacy.. MEASUREMENTS AND MAIN RESULTS:. . Neuropsychologic results in pretreatment OSA showed impairments in most cognitive areas, and in mood and sleepiness. These impairments were associated with focal reductions of gray-matter volume in the left hippocampus (entorhinal cortex), left posterior parietal cortex, and right superior frontal gyrus. After treatment, we observed significant improvements involving memory, attention, and executive-functioning that paralleled gray-matter volume increases in hippocampal and frontal structures.. CONCLUSIONS:. . The cognitive and structural deficits in OSA may be secondary to sleep deprivation and repetitive nocturnal intermittent hypoxemia. These negative effects may be recovered by consistent and thorough treatment. Our findings highlight the importance of early diagnosis and successful treatment of this disorder."

study objectives : Obstructive sleep apnea syndrome (OSAS) is asso- ciated with cognitive and functional deficits, most of which are corrected after positive airway pressure (PAP) treatment. Previous studies inves- tigating the neural underpinnings of OSAS failed to provide consistent results both on the cerebral substrates underlying cognitive deficits and on the effect of treatment on these anomalies. The aims of the study were a) to investigate whether never-treated OSA patients demonstrat- ed differences in brain activation compared to healthy controls during a cognitive task; and b) to investigate whether any improvements in cognitive functioning found in OSA patients after treatment reflected a change in the underlying cerebral activity. Design : OSA patients and healthy controls underwent functional mag- netic resonance imaging (fMRI) scanning. They were compared on per- formance and brain activation during a 2-back working-memory task. Patients were also re-evaluated after 3 months treatment with PAP. Cognitive functions were evaluated using neurocognitive tests. Sleepi- ness (ESS), mood (Beck Depression Inventory) and, quality-of-life (SF- 36) were also assessed. setting : The Sleep Disorders Center and CERMAC at the Vita-Salute San Raffaele University. Patients or Participants : 17 OSA patients and 15 age- and education- matched healthy controls. interventions : PAP treatment for 3 months. Measurements and results : Compared to controls, never-treated OSA patients showed increased activations in the left frontal cortex, medial precuneus, and hippocampus, and decreased activations in the caudal pons. OSA patients showed decreases in activation with treat- ment in the left inferior frontal gyrus and anterior cingulate cortex, and bilaterally in the hippocampus. Most neurocognitive domains, impaired at baseline, showed significant improvement after treatment. conclusions : OSA patients showed an overrecruitment of brain re- gions compared to controls, in the presence of the same level of performance on a working-memory task. - Abstract
The transition from wakefulness to sleep occurs when the core body temperature decreases. The latter is facilitated by an increase in the cutaneous blood flow, which dissipates internal heat into the micro-environment surrounding the sleeper's body. The rise in cutaneous blood flow near sleep onset causes the distal (hands and feet) and proximal (abdomen) temperatures to increase by about 1 °C and 0.5 °C, respectively. Characterizing the dynamics of skin temperature changes throughout sleep phases and understanding its relationship with sleep quality requires a means to unobtrusively and longitudinally estimate the skin temperature. Leveraging the data from a temperature sensor strip (TSS) with five individual temperature sensors embedded near the surface of a smart bed's mattress, we have developed an algorithm to estimate the distal skin temperature with a minute-long temporal resolution. The data from 18 participants who recorded TSS and ground-truth temperature data from sleep during 14 nights at home and 2 nights in a lab were used to develop an algorithm that uses a two-stage regression model (gradient boosted tree followed by a random forest) to estimate the distal skin temperature. A five-fold cross-validation procedure was applied to train and validate the model such that the data from a participant could only be either in the training or validation set but not in both. The algorithm verification was performed with the in-lab data. The algorithm presented in this research can estimate the distal skin temperature at a minute-level resolution, with accuracy characterized by the mean limits of agreement [-0.79 to +0.79 °C] and mean coefficient of determination R2=0.87. This method may enable the unobtrusive, longitudinal and ecologically valid collection of distal skin temperature values during sleep. Therelatively small sample size motivates the need for further validation efforts.

Introduction: The Digital Measures Development: Core Measures of Sleep project, led by the Digital Medicine Society (DiMe), emphasizes the importance of sleep as a cornerstone of health and the need for standardized measurements of sleep and its disturbances outside the laboratory. This initiative recognizes the complex relationship between sleep and overall health, addressing it as both a symptom of underlying conditions and a consequence of therapeutic interventions. It aims to fill a crucial gap in healthcare by promoting the development of accessible, nonintrusive, and cost-effective digital tools for sleep assessment, focusing on factors important to patients, caregivers, and clinicians., Methods: A central feature of this project was an expert workshop conducted on April 19th, 2023. The workshop convened stakeholders from diverse backgrounds, including regulatory, payer, industry, academic, and patient groups, to deliberate on the project's direction. This gathering focused on discussing the challenges and necessities of measuring sleep across various therapeutic areas, aiming to identify broad areas for initial focus while considering the feasibility of generalizing these measures where applicable. The methodological emphasis was on leveraging expert consensus to guide the project's approach to digital sleep measurement., Results: The workshop resulted in the identification of seven key themes that will direct the DiMe Core Digital Measures of Sleep project and the broader field of sleep research moving forward. These themes underscore the project's innovative approach to sleep health, highlighting the complexity of omni-therapeutic sleep measurement and identifying potential areas for targeted research and development. The discussions and outcomes of the workshop serve as a roadmap for enhancing digital sleep measurement tools, ensuring they are relevant, accurate, and capable of addressing the nuanced needs of diverse patient populations., Conclusion: The Digital Medicine Society's Core Measures of Sleep project represents a pivotal effort to advance sleep health through digital innovation. Human milk contains extracellular vesicles (HMEVs). Pre-clinical models suggest that HMEVs may enhance intestinal function and limit inflammation; however, it is unknown if HMEVs or their cargo survive neonatal human digestion. This limits the ability to leverage HMEV cargo as additives to infant nutrition or as therapeutics. This study aimed to develop an EV isolation pipeline from small volumes of human milk and neonatal intestinal contents after milk feeding (digesta) to address the hypothesis that HMEVs survive in vivo neonatal digestion to be taken up intestinal epithelial cells (IECs). Digesta was collected from nasoduodenal sampling tubes or ostomies. EVs were isolated from raw and pasteurized human milk and digesta by density-gradient ultracentrifugation following two-step skimming, acid precipitation of caseins, and multi-step filtration. EVs were validated by electron microscopy, western blotting, nanoparticle tracking analysis, resistive pulse sensing, and super-resolution microscopy. EV uptake was tested in human neonatal enteroids. HMEVs and digesta EVs (dEVs) show typical EV morphology and are enriched in CD81 and CD9, but depleted of β-casein and lactalbumin. HMEV and some dEV fractions contain mammary gland-derived protein BTN1A1. Neonatal human enteroids rapidly take up dEVs in part via clathrin-mediated endocytosis. 48. Fine tuned personalized machine learning models to detect insomnia risk based on data from a smart bed platform.

- Abstract
Introduction: Insomnia causes serious adverse health effects and is estimated to affect 10-30% of the worldwide population. This study leverages personalized fine-tuned machine learning algorithms to detect insomnia risk based on questionnaire and longitudinal objective sleep data collected by a smart bed platform., Methods: Users of the Sleep Number smart bed were invited to participate in an IRB approved study which required them to respond to four questionnaires (which included the Insomnia Severity Index; ISI) administered 6 weeks apart from each other in the period from November 2021 to March 2022. For 1,489 participants who completed at least 3 questionnaires, objective data (which includes sleep/wake and cardio-respiratory metrics) collected by the platform were queried for analysis. An incremental, passive-aggressive machine learning model was used to detect insomnia risk which was defined by the ISI exceeding a given threshold. Three ISI thresholds (8, 10, and 15) were considered. The incremental model is advantageous because it allows personalized fine-tuning by adding individual training data to a generic model., Results: The generic model, without personalizing, resulted in an area under the receiving-operating curve (AUC) of about 0.5 for each ISI threshold. The personalized fine-tuning with the data of just five sleep sessions from the individual for whom the model is being personalized resulted in AUCs exceeding 0.8 for all ISI thresholds. Interestingly, no further AUC enhancements resulted by adding personalized data exceeding ten sessions., Discussion: These are encouraging results motivating further investigation into the application of personalized fine tuning machine learning to detect insomnia risk based on longitudinal sleep data and the extension of this paradigm to sleep medicine., Competing Interests: All authors of this study are directly and indirectly employed by Sleep Number Corporation. Sleep regularity and chronotype can affect health, performance, and overall well-being. This observational study examines how sleep regularity and chronotype affect sleep quality and cardiorespiratory metrics. Data was collected from 1 January 2019 through 30 December 2019 from over 330 000 Sleep Number smart bed users across the United States who opted into this at-home study. A pressure signal from the smart bed reflected bed presence, movements, heart rate (HR), and breathing rate (BR). Participants (mean age: 55.69 years [SD: 14.0]; 51.2% female) were categorized by chronotype (16.8% early; 62.2% intermediate, 20.9% late) and regularity of sleep timing. Participants who were regular sleepers (66.1%) experienced higher percent restful sleep and lower mean HR and BR compared to the 4.8% categorized as irregular sleepers. Regular early-chronotype participants displayed better sleep and cardiorespiratory parameters compared to those with regular late-chronotypes. Significant variations were noted in sleep duration (Cohen's d = 1.54 and 0.88, respectively) and restful sleep (Cohen's d = 1.46 and 0.82, respectively) between early and late chronotypes, particularly within regular and irregular sleep patterns. This study highlights how sleep regularity and chronotype influence sleep quality and cardiorespiratory metrics. Irrespective of chronotype, sleep regularity demonstrated a substantial effect. Further research is necessary to confirm these findings.

Developing a vaccine to prevent congenital cytomegalovirus (CMV) infection and newborn disability requires an understanding of infection incidence. In a prospective cohort study of 363 adolescent girls (NCT01691820), CMV serostatus, primary infection, and secondary infection were determined in blood and urine samples collected at enrollment and every 4 months for 3 years. Baseline CMV seroprevalence was 58%. Primary infection occurred in 14.8% of seronegative girls. Among seropositive girls, 5.9% had ≥4-fold increase in anti-CMV antibody, and 23.9% shed CMV DNA in urine.
