Abstract

The Campi Flegrei caldera (southern Italy) is one of the most hazardous volcanic systems on Earth, having produced >60 eruptions in the past 15 ka. The caldera remains active and its potential for future eruptions is high, posing a danger to the dense population living nearby. Despite this, our understanding of pre-eruptive processes and the architecture of the sub-volcanic system are poorly constrained. Here, we combine established petrological techniques, geothermobarometric evaluation, and high resolution trace element crystal mapping, to present a multifaceted, coherent reconstruction of the complex pre eruptive dynamics and eruption timescales of Astroni volcano located in the eastern sector of Campi Flegrei caldera. The Astroni volcano is an important case study for investigating plumbing system processes and dynamics at Campi Flegrei caldera because it produced the most recent (ca. 4 ka ago) Plinian eruption within the caldera (Astroni 6); current long-term forecasting studies postulate that a similar sized event in this location is a probable future scenario. Geothermobarometric results indicate interaction between an evolved, shallow magma chamber, and a less evolved, deeper pocket of magma, in agreement with previous studies focused on the Astroni 6 eruption products. In addition, a range of textural and trace element zoning patterns point to a complex evolution of both magmas prior to their subsequent interaction. High resolution trace element crystal maps reveal discrete zonations in compatible elements. These zonations, combined with knowledge of Kfeldspar growth rates, highlight a recharge event in the shallow plumbing system a few hours to days before the Astroni 6 eruption.

Abstract

For active volcanoes, knowledge of the architecture of the plumbing system and the conditions of magma storage prior to an eruption are highly important, given their influence on the eruptive style and, thus, the management of future volcanic crises. Here, chlorine is used as a geobarometer for potassic alkaline magmas at the Campi Flegrei volcanic complex, revealing the shallowest depth of fluid-melt equilibration with respect to Cl. The results for representative fallout deposits of selected explosive eruptions show the existence of a multi-depth equilibration zone through time, including shallow magma storage. We describe evidence for the shallowest zone located at a depth equivalent to 65 MPa for the Agnano Monte Spina eruption (4482–4625 cal. yrs BP), at ~100 MPa for the Pomici Principali (11 915–12 158 cal. yrs BP), and the Astroni 6 (4098–4297 cal. yrs BP) eruptions, and close to 115 MPa for the last explosive eruption of Monte Nuovo (AD 1538). For comparison, the pressure estimated for a possible reservoir feeding the Cretaio eruption of Ischia island (AD 430), the only studied eruption on Ischia, is ~140 MPa. The pressure estimates for the two largest magnitude eruptions, the Campanian Ignimbrite (40 ka) and the Neapolitan Yellow Tuff (14.9 ka), are also discussed with respect to available magma withdrawal models. The pressures estimated using the Cl geobarometer for the magma leading to the fallout phases of these two eruptions provide evidence for a low-volume, shallow domain (~40 MPa) for the Plinian phase of the Campanian Ignimbrite eruption and a main, deeper reservoir (~130–165 MPa) for the Neapolitan Yellow Tuff eruption. The inferred shallowest equilibration pressures are interpreted here as corresponding to transitory, short-lived magma apophyses, whose eruption may have been facilitated by optimum tectonic stresses, rheological behavior of the crust, and efficiency of volatile exsolution. Alternatively, these magma apophyses may represent an evolved, crystal-rich ponded magma into which a volatile-rich magma ascending from depth was injected. The transient nature of such very shallow reservoirs is suggested by the short timescales inferred from diffusion modeling on crystals available in the literature for the studied Campi Flegrei eruptions. The influence of sulfur (S) on Cl solubility is assessed through Cl solubility modeling and applied to different eruptions. In addition, the pressure at which magmatic fluids and melts equilibrated with respect to Cl is shallower for the Campi Flegrei volcanic complex than the Somma-Vesuvio volcanic complex, erupting more homogeneous differentiated magma, of trachytic or phonolitic composition. Abstract

Background/Aims: Patients with genotype 3 hepatitis C virus (G3-HCV) cirrhosis are very difficult to treat compared to patients with other HCV genotypes. The optimal treatment duration and drug regimen associated with ribavirin (RBV) remain unclear. To evaluate the efficacy and safety of daclatasvir (DCV)/sofosbuvir (SOF) plus a flat dose of 800 mg RBV (flat dose) compared to DCV/SOF without RBV or DCV/SOF plus an RBV dose based on body weight (weight-based) in G3-HCV patients with compensated or decompensated cirrhosis. Methods: We analyzed data for 233 G3 cirrhotic patients. Of these, 70 (30%), 87(37%) and 76 (33%) received SOF/DCV, SOF/DCV/RBV flat dose, and SOF/DCV/RBV weight-based dose, respectively. Treatment duration was 24 weeks. Sustained virological response (SVR) was evaluated at week 12 posttreatment (SVR12). Results: Overall, SVR12 was achieved in 220 out of 233 patients (94.4%). The SVR12 rate was lower in the DCV/SOF group than in the DCV/SOF/RBV flat-dose group and the DCV/SOF/RBV weight-based group (87.1% vs 97.7% and 97.4%, respectively, p=0.007). Abstract

Celiac disease (CD) is an autoimmune disease with the destruction of small intestinal villi, which occurs in genetically predisposed individuals. At the present moment, a gluten-free diet (GFD) is the only way to restore the functionality of gut mucosa. However, there is an open debate on the effects of long-term supplementation through a GFD, because some authors report an unbalance in microbial taxa composition. Methods: For microbiome analysis, fecal specimens were collected from 46 CD individuals in GFD for at least 2 years and 30 specimens from the healthy controls (HC). Data were analyzed using an ensemble of software packages: QIIME2, Coda-lasso, Clr-lasso, Selbal, PICRUSt2, ALDEx2, dissimilarity-overlap analysis, and dysbiosis detection tests. Results: The adherence to GFD restored the alpha biodiversity of the gut microbiota in celiac people but microbial composition at beta diversity resulted as different to HC. The microbial composition of the CD subjects was decreased in a number of taxa, namely Bifidobacterium longum and several belonging to Lachnospiraceae family, whereas Bacteroides genus was found to be more abundant. Predicted metabolic pathways among the CD bacterial communities revealed an important role in tetrapyrrole biosynthesis. Conclusions: CD patients in GFD had a non-dysbiotic microbial composition for the crude alpha diversity metrics. We found significant differences in beta diversity, in certain taxon, and pathways between subjects with inactive CD in GFD and controls. Collectively, our data may suggest the development of new GFD products by modulating the gut microbiota through diet, supplements of vitamins, and the addition of specific prebiotics.

Abstract

For superficial colonic lesions, the NICE and Kudo classifications are used in the in vivo prediction of histology and as guide to therapy. The NICE system derives information from unmagnified NBI endoscopic images. The Kudo one necessitates a magnification, but, as this tool is not commonly available, it is applied also to characterize unmagnified pictures to compare their diagnostic performances. We conducted a prospective comparison of the NICE versus the Kudo classification for the differential diagnosis of colonic polyps taking histology as the gold standard. The inter-observer agreement for both classifications among 11 colonoscopists was also evaluated. Short unmagnified NBI videoclips of 64 colonic polyps were sent twice to the participants. In the first round, they classified the lesions according to the NICE classification; 4 months later, the same videos were assessed with the Kudo system. The diagnosis provided by the participants was grouped in non-neoplastic, non-invasive neoplasia, invasive neoplasia. Overall, the diagnostic accuracy was 82% (95%CI: 79–85) with the NICE system and 81% (95%CI: 78–84) with the Kudo one (ρ = 0.78). The accuracy of the NICE classification for non-neoplastic lesions was greater compared with the Kudo’s (ρ = 0.03). Sensitivity sub-analyses revealed a higher ability of the NICE in distinguishing between neoplastic vs. non-neoplastic lesions (ρ = 0.01). The overall inter-rater agreement did not differ when the classifications were compared. The NICE and the Kudo classifications might be considered comparable. Our data could allow the use of the NBI Kudo classification even in those centers where magnification is not available.

Abstract

Background and study aims The Paris classification of superficial colonic lesions has been widely adopted, but a simplified description that subgroups the shape into pedunculated, sessile/flat and depressed lesions has been proposed recently. The aim of this study was to evaluate the accuracy and inter-rater agreement among 13 Western endoscopists for the two classification systems. Methods Seventy video clips of superficial colonic lesions were classified according to the two classifications, and their size estimated. The interobserver agreement for each classification was assessed using both Cohen k and AC1 statistics. Accuracy was taken as the concordance between the standard morphology definition and that made by participants. Sensitivity analyses investigated agreement between trainees (T) and staff members (SM), simple or mixed lesions, distinct lesion phenotypes, and for laterally spreading tumors (LSTs). Results Overall, the interobserver agreement for the Paris classification was substantial (κ = 0.61; AC1 = 0.66), with 79.3 % accuracy. Between SM and T, the values were superimposable. For size estimation, the agreement was 0.48 by the κ-value, and 0.50 by AC1. For single or mixed lesions, κ-values were 0.60 and 0.43, respectively; corresponding AC1 values were 0.68 and 0.57. Evaluating the several different polyp subtypes separately, agreement differed significantly when analyzed by the k-statistics (0.08–0.12) or the AC1 statistics (0.59–0.71). Analyses of LSTs provided a κ-value of 0.50 and an AC1 score of 0.62, with 77.6 % accuracy. The simplified classification outperformed the Paris classification: κ = 0.68, AC1 = 0.82, accuracy = 91.6 %. Conclusions Agreement is often measured with Cohen’s κ, but we documented higher levels of agreement when analyzed with the AC1 statistic. The level of agreement was substantial for the Paris classification, and almost perfect for the simplified system.
