Einleitung: Zwangsmaßnahmen stellen einen schwerwiegenden Eingriff in die Freiheitsrechte Betroffener dar und sind deshalb ein kontrovers diskutierter Bestandteil der psychiatrischen Alltagspraxis. Auf Grund der zahlreichen negativen Aspekte von Zwangsmaßnahmen dürfen sie nur bei akuter Eigen- oder Fremdgefährdung, wenn alle weniger einschränkenden Maßnahmen ausgeschöpft wurden, angewandt werden. Im Dezember 2010 wurde an der Psychiatrischen Universitätsklinik der Charité im St. Hedwig-Krankenhaus ein neues multiprofessionelles, trialogisches und patientenzentriertes Behandlungskonzept, das „Weddinger Modell“, eingeführt. Dieses verfolgt als eines seiner Hauptziele die Reduktion von Zwangsmaßnahmen. Ziel der vorliegenden Studie war es, den Effekt des Weddinger Modells auf mechanische Zwangsmaßnahmen zu untersuchen. Methodik: In dieser retrospektiven Fallkontrollstudie wurden 357 Patienten untersucht. Die Patienten wurden an zwei Erhebungszeiträumen vor und an zwei Erhebungszeiträumen nach Implementierung des Weddinger Modells auf drei psychiatrischen Akutstationen behandelt. Die Studie analysierte zwei Patientengruppen, eine nach dem Weddinger Modell behandelte Interventionsgruppe (n = 122) und eine konventionell behandelte Kontrollgruppe (n = 235). Ausgewählte Parameter betreffend mechanische Zwangsmaßnahmen wurden zwischen den zwei Gruppen verglichen. Ergebnisse: Die Ergebnisse zeigen in der Interventionsgruppe, die nach dem Weddinger Modell behandelt wurde, eine signifikante Reduktion der kumulativen Dauer von Zwangsmaßnahmen, des Anteils der kumulativen Dauer von Zwangsmaßnahmen an der Verweildauer, der Fixierungsanzahl sowie der durchschnittlichen und kumulativen Isolierungsdauer. Schlussfolgerung: Die Ergebnisse weisen darauf hin, dass multiprofessionelle, trialogische und patientenzentrierte Behandlungskonzepte wie das Weddinger Modell einen positiven Beitrag zur Reduktion von Zwangsmaßnahmen leisten können. Das Weddinger Modell und ähnliche Behandlungsansätze zur Zwangsvermeidung können demnach verbreitet und umgesetzt werden., Introduction: Coercive measures pose a significant encroachment on the personal liberty of those concerned and are therefore a controversially discussed issue of everyday psychiatric practice. Due to numerous negative aspects, coercive measures may only be used if there is acute risk of self-endangerment or endangerment to others and only if all less restrictive measures have been exhausted. In December 2010 a new multiprofessional, trialogical and participatory treatment concept, the “Weddinger Modell”, was introduced at the Charité Psychiatric University Clinic at the St. Hedwig Hospital. One of its main goals is to reduce coercive measures. The aim of the present study was to investigate the effects of this novel treatment approach on mechanical coercive measures. Methods: The retrospective case-control study examined 357 patients. The patients were treated at three in-patient facilities for acute psychiatric care. They were investigated at two reporting periods before and at two reporting periods after implementation of the Weddinger Modell. The study analyzed two patient groups. The data on mechanical coercive measures were compared based on affiliation to an intervention group (n = 122) treated according to the Weddinger Modell or a control group treated conventionally (n = 235). Results: A significant reduction of the cumulative duration of coercive measures, the proportion of the cumulative duration of coercive measures to the length of stay, the frequency of restraint events and the average and cumulative duration of seclusion incidents was detected in the intervention group. Conclusion: The results indicate that multiprofessional, trialogical and patient centered concepts such as the Weddinger Modell can make a positive contribution to the reduction of coercive measures. The Weddinger Modell and similar treatment approaches to prevent coercion can therefore be disseminated and implemented.
