La hidrocefalia de presión normal (HPN) afecta a una gran proporción de la población anciana. En Colombia, se calcula que 10.000 personas podrían estar afectadas. Los síntomas de alteración en la marcha y caídas, urgencia e incontinencia urinaria y alteración cognoscitiva y demencia enfatizan la sospecha de HPN pero no son suficientes para el diagnóstico ya que muchas enfermedades en los ancianos tienen estos mismos síntomas. Además de la demostración del ensanchamiento de los ventrículos por TAC de cráneo o RM, es necesario realizar pruebas espe- cíficas para la HPN. Los centros de remisión casi siempre usan métodos especializados tales como la medición de flujo de LCR o el drenaje lumbar externo de LCR para realizar el diagnóstico; sin embargo en otras instituciones los médicos pueden identificar los pacientes con HPN y recomendarles el tratamiento evaluando la respuesta clínica a través de la punción lumbar evacuatoria. El tratamiento de la HPN con derivación ofrece como resultado una mejoría notoria de los síntomas, incluyendo la demencia. Por lo tanto, la HPN es un desorden que debe ser considerado para muchos pacientes ancianos de acuerdo a su historia, sintomatología, hallazgos neurologicos y hallazgos imagenológicos en la TAC o en la RM.

Abstract Background Adaptive strategies of emotion regulation are important for adolescents, as maladaptive strategies of such can manifest as psychopathology that is sometimes severe. Individual biological characteristics and influences from peers have been shown to have an effect on the development of emotion regulation strategies in adolescents. Maternal factors, however, have received less attention in this age group regarding how they might predict emotion regulation in adolescents. Given that prior work has demonstrated that certain maternal factors, like emotion regulation and personality, play a crucial role in the development of emotion regulation strategies in early childhood, we sought to examine these associations in adolescents in our current study. Methods Adolescents and their mothers (n = 123) both self-reported data on their own emotion regulation, and mothers also self-reported data on their own personality dysfunction. We operationalized maternal and adolescent emotion regulation as emotion suppression, a maladaptive emotion regulation strategy that is commonly used by adolescents. Results Our data demonstrated that both maternal emotion suppression and interpersonal personality dysfunction were positively associated with adolescent emotion suppression. No associations among maternal intrapersonal personality functioning and adolescent emotion suppression were detected. Conclusions Maternal personality dysfunction and emotion suppression both independently predicted adolescent emotion suppression use. These results support the idea that maternal characteristics play a role in shaping emotion regulation in adolescence.

The authors examined task-based (i.e., executive function), surveyed (i.e., effortful control), and physiological (i.e., resting cardiac respiratory sinus arrhythmia [RSA]) measures of child and maternal regulation as distinct moderators of longitudinal bidirectional links between child externalizing (EXT) behaviors and harsh parenting (HP) from 6 to 9 years. The sample size was 299 (50.9% female; 1% Asian, 4% multiple races; 14% Black; 78% White), and participants were recruited in the United States (a rural college town in Virginia and a midsized city in North Carolina). Higher child EXT at 6 years predicted higher HP at 7-8 years, which predicted higher EXT at 9 years. Also, this path was moderated by 6-year child effortful control, 6-year resting RSA, and 9-year executive function. In contrast, there was no moderating effect of any measure of maternal regulation. Findings suggest it is important to consider child self-regulation when examining bidirectionality in parent and child effects for HP and child EXT.

Abstract Background The longitudinal study, “Couples Lived Experiences,” focuses on whether and how relationship characteristics of older couples change with the cognitive decline of one member of the couple, and how these changes affect each individual’s emotional and physical health outcomes. Until now, most psychosocial research in dementia has focused either on the person with dementia (PWD) or the caregiver separately. The previous literature examining relationship characteristics and their role in outcomes for the caregiver and PWD is scant and suffers from methodological issues that limit the understanding of which relationship characteristics most influence outcomes for caregivers and care-receivers and what other factors may mitigate or exacerbate their effects. Methods We will enroll 300 dyads and collect information via online interviews of each member of the couple, every 6 months for 3 years. Relationship characteristics will be measured with a set of short, well-validated, and reliable self-report measures, plus the newly developed “Partnership Approach Questionnaire.” Outcomes include global quality of life, subjective physical health, mental health (depression and anxiety), and status change (transitions in levels of care; i.e., placement in a nursing home). Longitudinal data will be used to investigate how relationship characteristics are affected by cognitive, functional, and behavioral changes, and the impact of these changes on health outcomes. Qualitative data will also be collected to enrich the interpretation of results of quantitative analyses. Discussion Psychosocial interventions have demonstrated effectiveness in promoting the wellbeing of PWD and their caregivers. The knowledge gained from this study can lead to the development or enhancement of targeted interventions for older couples that consider the impact of cognitive and functional decline on the relationship between members of a couple and thereby improve their wellbeing. Although children’s negative affectivity is a temperamental characteristic that is biologically based, it is framed within and shaped by their emotional environments which are partly created by maternal emotion expressivity in the family. Children, in turn, play a role in shaping their family emotional context, which could lead to changes in mothers’ emotion expressivity in the family. However, these theorized longitudinal bidirectional relations between child negative affectivity and maternal positive and negative expressivity have not been studied from toddlerhood to early school-age. The current study utilized a cross-lagged panel model to examine the reciprocal relations between children’s negative affectivity and maternal expressivity within the family over the course of early childhood. Participants were 140 mother–child dyads (72 boys, mean age = 2.67 years, primarily White). Mothers reported the positive and negative expressivity in the family and children’s negative affectivity in toddlerhood (T1), preschool (T2), and school-age (T3). Maternal negative expressivity and child negative affectivity at T1 were significantly correlated. Maternal negative expressivity at T1 significantly predicted child negative affectivity at T3. Children’s negative affectivity at T2 significantly predicted mothers’ negative expressivity at T3. Mothers’ positive expressivity was not related to children’s negative affectivity at any of the three time points. The findings demonstrate the reciprocal relations between children’s negative affectivity and maternal negative expressivity in the family, suggesting the importance of the interplay between child temperament and maternal expressivity within the family emotional context.

Epigenetics, specifically DNA methylation, allows for the estimation of animal age from blood or remotely sampled skin. This multi-tissue epigenetic age estimation clock uses 110 longitudinal samples from 34 Navy bottlenose dolphins (Tursiops truncatus), identifying 195 cytosine-phosphate-guanine sites associated with chronological aging via cross-validation with one individual left out in each fold (R2 = 0.95). With a median absolute error of 2.5 years, this clock improves age estimation capacity in wild dolphins, helping conservation efforts and enabling a better understanding of population demographics.

The Deepwater Horizon (DWH) disaster resulted in large-scale contamination of bays, sounds, and estuaries in the northern Gulf of Mexico, home to multiple stocks of common bottlenose dolphins (Tursiops truncatus). Inhalation, aspiration, ingestion, and dermal absorption of oil and its toxic components were all considered possible routes of exposure for dolphins living within the oil spill footprint. To determine if dolphins were adversely impacted, catch-and-release health assessments were performed in heavily-oiled Barataria Bay (BB), Louisiana, and in Sarasota Bay (SB), Florida, a comparison site with no DWH oil contamination. Initial studies were conducted as part of a Natural Resource Damage Assessment (2011-2014) and follow-on studies were performed between 2016-2018 with support from the Gulf of Mexico Research Initiative. Ultrasound was used to evaluate the dolphins’ pulmonary health, including the presence/absence of pleural effusion, nodules, masses, consolidation, and alveolar interstitial syndrome (AIS). When present, AIS was further graded by severity (mild, moderate, or severe) and distribution. Based on the presence and severity of abnormalities, each dolphin was given an overall lung disease score (normal, mild, moderate, or severe). Normal to mild scores were considered within expected limits for a wild population, therefore the prevalence of normal-mild versus moderate-severe scores was compared between the oiled and unoiled sites. Separate analyses were conducted for dolphins alive in 2010 (and in BB, presumably exposed to DWH oil), and those born after 2010. For the dolphins alive in 2010, temporal trends were also examined using generalized additive models (GAMs). Results showed a strong difference (p=0.000357) in moderate to severe lung disease between the two sites for dolphins alive in 2010, but no significant difference (p=0.6745) between the sites for dolphins born after 2010. In BB dolphins, the prevalence of moderate to severe lung disease did not decrease in the years after the spill, and in fact, potentially worsened (p=0.0644 for trend over years), with the highest prevalence (0.61) being in 2018. Moderate to severe AIS remained a persistent finding in BB dolphins, and several animals had a pattern of AIS that was more severe ventrally than dorsally, with evidence of chronic, progressive disease states.

Abstract Blubber and serum testosterone levels were compared among 55 individual common bottlenose dolphins, Tursiops truncatus, in Sarasota Bay, FL during 2011–2019. A significant positive relationship between the matrices was found in male testosterone concentrations in 29 paired samples (r2 = 0.932). Mature males (n = 17) had 300 times greater mean testosterone concentration in serum than immature males (n = 17). A comparison of blubber samples, including 12 females, 24 immature males, and 19 mature males, revealed significant differences in mean blubber testosterone values among all three demographics. Immature males had greater than 6 times the average blubber testosterone concentration of females and mature males had almost 100 times that of immature males. Estimated testis volume was highly correlated with blubber testosterone concentration and mature males had 60 times greater average testis volume than immature males. We observed seasonal variation in blubber testosterone in mature males, consistent with known reproductive patterns. These data suggest males can be distinguished from females and designated as mature or immature via blubber testosterone concentrations, an observation that validates dart biopsy sampling as a means of obtaining demographic data.

Following the 2010 Deepwater Horizon disaster and subsequent unusual mortality event, adverse health impacts have been reported in bottlenose dolphins in Barataria Bay, LA including impaired stress response and reproductive, pulmonary, cardiac, and immune function. These conditions were primarily diagnosed through hands-on veterinary examinations and analysis of standard diagnostic panels. In human and veterinary medicine, gene expression profiling has been used to identify molecular mechanisms underlying toxic responses and disease states. Identification of molecular markers of exposure or disease may enable earlier detection of health effects or allow for health evaluation when the use of specialized methodologies is not feasible. To date this powerful tool has not been applied to augment the veterinary data collected concurrently during dolphin health assessments. This study examined transcriptomic profiles of blood from 76 dolphins sampled in health assessments during 2013-2018 in the waters near Barataria Bay, LA and Sarasota Bay, FL. Gene expression was analyzed in conjunction with the substantial suite of health data collected using principal component analysis, differential expression testing, over-representation analysis, and weighted gene co-expression network analysis. Broadly, transcript profiles of Barataria Bay dolphins indicated a shift in immune response, cytoskeletal alterations, and mitochondrial dysfunction, most pronounced in dolphins likely exposed to Deepwater Horizon oiling. While gene expression profiles in Barataria Bay dolphins were altered compared to Sarasota Bay for all years, profiles from 2013 exhibited the greatest alteration in gene expression. Differentially expressed transcripts included genes involved in immunity, inflammation, reproductive failure, and lung or cardiac dysfunction, all of which have been documented in dolphins from Barataria Bay following the Deepwater Horizon oil spill. The genes and pathways identified in this study may, with additional research and validation, prove useful as molecular markers of exposure or disease to assist wildlife veterinarians in evaluating the health of dolphins and other cetaceans.

Psychological science is struggling with moving forward in the midst of the COVID-19 pandemic, especially due to the halting of behavioral data collection in the laboratory. Safety barriers to assessing psychological behavior in person increased the need for remote data collection in natural settings. In response to these challenges, researchers, including our team, have utilized this time to advance remote behavioral methodology. In this article, we provide an overview of our group’s strategies for remote data collection methodology and examples from our research in collecting behavioral data in the context of psychological functioning. Then, we describe the design and development of our strategies for remote data collection of mother-infant interactions, with the goal being to assess maternal sensitivity and intrusiveness, as well as infants’ adaptive behaviors in several developmental domains. During these virtual visits over Zoom, mother-infant dyads watched a book-reading video and were asked to participate in peek-a-boo, toy play, and toy removal tasks. After the behavioral tasks, a semi-structured interview (Vineland Adaptive Behavior Scale – VABS III) was conducted to assess the infant’s adaptive behavior in communication, socialization, daily living skills, and motor domains. We delineate the specific strategies we applied to integrate laboratory tasks and a semi-structured interview into remote data collection in home settings with mothers and infants. We also elaborate on issues encountered during remote data collection and how we resolved these challenges. Lastly, to inform protocols for future remote data collection, we address considerations and recommendations, as well as benefits and future directions for behavioral researchers in developmental psychology research.

Abstract Ecosystems are experiencing elevated levels of disturbance, and species with narrower niches are often more vulnerable to disturbances. Niche breadth is often measured in terms of either diet or habitat use but diet and spatial use are infrequently considered in tandem. These different aspects of niche breadth potentially expose species to different types of disturbances; species with narrow dietary niches may be more affected by disturbances that alter trophic relationships, while species with narrow habitat niches may be more vulnerable to habitat loss and point‐source pollutants. We examined dietary and habitat niche breadth of common bottlenose dolphins, Tursiops truncatus truncatus, from three different nearshore sites in the Gulf of Mexico (GoM). Using stable isotopes, we determined proportional contributions of different prey groups to dolphin diets at each site and through time at one site. We used satellite‐linked telemetry at two sites to determine habitat use and site fidelity. Additionally, we examined the literature on cetacean diet, habitat use, movement, and IUCN status to determine relationships between niche breadth and population status for different species. Dolphin diets varied among sites as available prey varied, but Perciformes fish were the most frequently consumed prey. At the site for which we had temporal data, dolphins consumed more cephalopods in 2015 and 2018 but otherwise consumed primarily Perciformes fish. Dolphins had small utilization distributions and exhibited high site fidelity. Data from 31 cetacean species revealed that most species with vulnerable, threatened, or endangered IUCN statuses not only have specialized diets but also exhibit high site fidelity. Dolphins had diet characteristic of flexible generalists but were habitat specialists with high site fidelity. Dolphin populations in the GoM may have altered their diets in response to environmental changes that have altered community composition and trophic dynamics. On the other hand, their high site fidelity has exposed them to point‐source pollutants, such as oil spills, persistent organic pollutants, and freshwater. Our broader analysis of cetaceans confirmed that species with specialized diets and high site fidelity were the most vulnerable to disturbances, providing a framework to predict which nearshore dolphin populations, and cetaceans in general, are most vulnerable to environmental changes.

More than 2,000 common bottlenose dolphins (Tursiops truncatus) inhabit the Barataria Bay Estuarine System in Louisiana, USA, a highly productive estuary with variable salinity driven by natural and man-made processes. It was unclear whether dolphins that are long-term residents to specific areas within the basin move in response to fluctuations in salinity, which at times can decline to 0 parts per thousand in portions of the basin. In June 2017, we conducted health assessments and deployed satellite telemetry tags on dolphins in the northern portions of the Barataria Bay Estuarine System Stock area (9 females; 4 males). We analyzed their fine-scale movements relative to modeled salinity trends compared to dolphins tagged near the barrier islands (higher salinity environments) from 2011 to 2017 (37 females; 21 males). Even though we observed different movement patterns among individual dolphins, we found no evidence that tagged dolphins moved coincident with changes in salinity. One tagged dolphin spent at least 35 consecutive days, and 75 days in total, in salinity under 5 parts per thousand. Health assessments took place early in a seasonal period of decreased salinity. Nonetheless, we found an increased prevalence of skin lesions, as well as abnormalities in serum biochemical markers and urine:serum osmolality ratios for dolphins sampled in lower salinity areas. This study provides essential information on the likely behavioral responses of dolphins to changes in salinity (e.g., severe storms or from the proposed Mid-Barataria Sediment Diversion project) and on physiological markers to inform the timing and severity of impacts from low salinity exposure.

The Deepwater Horizon (DWH) oil spill profoundly impacted the health of bottlenose dolphins (Tursiops truncatus) in Barataria Bay, LA (BB). To comprehensively assess the cardiac health of dolphins living within the DWH oil spill footprint, techniques for in-water cardiac evaluation were refined with dolphins cared for by the U.S. Navy Marine Mammal Program in 2018 and applied to free-ranging bottlenose dolphins in BB (n = 34) and Sarasota Bay, Florida (SB) (n = 19), a non-oiled reference population. Cardiac auscultation detected systolic murmurs in the majority of dolphins from both sites (88% BB, 89% SB) and echocardiography showed most of the murmurs were innocent flow murmurs attributed to elevated blood flow velocity [1]. Telemetric six-lead electrocardiography detected arrhythmias in BB dolphins (43%) and SB dolphins (31%), all of which were considered low to moderate risk for adverse cardiac events. Echocardiography showed BB dolphins had thinner left ventricular walls, with significant differences in intraventricular septum thickness at the end of diastole (p = 0.002), and left ventricular posterior wall thickness at the end of diastole (p = 0.033). BB dolphins also had smaller left atrial size (p = 0.004), higher prevalence of tricuspid valve prolapse (p = 0.003), higher prevalence of tricuspid valve thickening (p = 0.033), and higher prevalence of aortic valve thickening (p = 0.008). Two dolphins in BB were diagnosed with pulmonary arterial hypertension based on Doppler echocardiography-derived estimates and supporting echocardiographic findings. Histopathology of dolphins who stranded within the DWH oil spill footprint showed a significantly higher prevalence of myocardial fibrosis (p = 0.003), regardless of age, compared to dolphins outside the oil spill footprint. In conclusion, there were substantial cardiac abnormalities identified in BB dolphins which may be related to DWH oil exposure, however, future work is needed to rule out other hypotheses and further elucidate the connection between oil exposure, pulmonary disease, and the observed cardiac abnormalities.

Cardiac auscultation is an important, albeit underutilized tool in aquatic animal medicine due to the many challenges associated with in-water examinations. The aims of this prospective study were to (1) establish an efficient and repeatable in-water cardiac auscultation technique in bottlenose dolphins (Tursiops truncatus), (2) describe the presence and characterization of heart murmurs detected in free-ranging and managed dolphins, and (3) characterize heart murmur etiology through echocardiography in free-ranging dolphins. For technique development, 65 dolphins cared for by the Navy Marine Mammal Program (Navy) were auscultated. The techniques were then applied to two free-ranging dolphin populations during capture-release health assessments: Sarasota Bay, Florida (SB), a reference population, and Barataria Bay, LA (BB), a well-studied population of dolphins impacted by the Deepwater Horizon oil spill. Systolic heart murmurs were detected at a frequent and similar prevalence in all dolphin populations examined (Navy 92%, SB 89%, and BB 88%), and characterized as fixed or dynamic. In all three populations, sternal cranial and left cranial were the most common locations for murmur point of maximal intensity (PMI). An in-water transthoracic echocardiogram technique was refined on a subset of Navy dolphins, and full echocardiographic exams were performed on 17 SB dolphins and 29 BB dolphins, of which, 40 had murmurs. Spectral Doppler was used to measure flow velocities across the outflow tracts, and almost all dolphins with audible murmurs had peak outflow velocities ≥1.6 m/s (95%, 38/40); three dolphins also had medium mitral regurgitation which could be the source of their murmurs. The presence of audible murmurs in most of the free-ranging dolphins (88%) was attributed to high velocity blood flow as seen on echocardiography, similar to a phenomenon described in other athletic species. These innocent murmurs were generally characterized as Grade I-III systolic murmurs with PMI in the left or sternal cranial region. This study is the first to describe an efficient technique for in-water dolphin cardiac auscultation, and to present evidence that heart murmurs are common in bottlenose dolphins.

Age is an important parameter to better understand wildlife populations, and is especially relevant for interpreting data for fecundity, health, and survival assessments. Estimating ages for marine mammals presents a particular challenge due to the environment they inhabit: accessibility is limited and, when temporarily restrained for assessment, the window of opportunity for data collection is relatively short. For wild dolphins, researchers have described a variety of age-determination techniques, but the gold-standard relies upon photo-identification to establish individual observational life histories from birth. However, there are few populations with such long-term data sets, therefore alternative techniques for age estimation are required for individual animals without a known birth period. While there are a variety of methods to estimate ages, each involves some combination of drawbacks, including a lack of precision across all ages, weeks-to-months of analysis time, logistical concerns for field applications, and/or novel techniques still in early development and validation. Here, we describe a non-invasive field technique to determine the age of small cetaceans using periapical dental radiography and subsequent measurement of pulp:tooth area ratios. The technique has been successfully applied for bottlenose dolphins briefly restrained during capture-release heath assessments in various locations in the Gulf of Mexico. Based on our comparisons of dental radiography data to life history ages, the pulp:tooth area ratio method can reliably provide same-day estimates for ages of dolphins up to about 10 years old.

The common bottlenose dolphin (Tursiops truncatus) is a global marine mammal species for which some populations, due to their coastal accessibility, have been monitored diligently by scientists for decades. Health assessment examinations have developed a comprehensive knowledge base of dolphin biology, population structure, and environmental or anthropogenic stressors affecting their dynamics. Bottlenose dolphin health assessments initially started as stock assessments prior to acquisition. Over the last four decades, health assessments have evolved into essential conservation management tools of free-ranging dolphin populations. Baseline data enable comparison of stressors between geographic locations and associated changes in individual and population health status. In addition, long-term monitoring provides opportunities for insights into population shifts over time, with retrospective application of novel diagnostic tests on archived samples. Expanding scientific knowledge enables effective long-term conservation management strategies by facilitating informed decision making and improving social understanding of the anthropogenic effects. The ability to use bottlenose dolphins as a model for studying marine mammal health has been pivotal in our understanding of anthropogenic effects on multiple marine mammal species. Future studies aim to build on current knowledge to influence management decisions and species conservation. This paper reviews the historical approaches to dolphin health assessments, present day achievements, and development of future conservation goals.

ObjectivesWhile patients' health priorities should inform healthcare, strategies for doing so are lacking for patients with multiple conditions. We describe challenges to, and strategies that support, patients' priorities-aligned decision-making.DesignParticipant observation qualitative study.SettingPrimary care and cardiology practices in Connecticut.ParticipantsTen primary care clinicians, five cardiologists, and the Patient Priorities implementation team (four geriatricians, physician expert in clinician training, behavioral medicine expert). The patients discussed were ≥ 66 years with >3 chronic conditions and ≥10 medications or saw ≥ two specialists.ExposureFollowing initial training and experience in providing Patient Priorities Care, the clinicians and Patient Priorities implementation team participated in 21 case-based, group discussions (10 face-to-face;11 telephonic). Using emergent learning (i.e. learning which arises from interactions among the participants), participants discussed challenges, posed solutions, and worked together to determine how to align care options with the health priorities of 35 patients participating in the Patient Priorities Care pilot.Main outcomesChallenges to, and strategies for, aligning decision-making with patient's health priorities.ResultsCategories of challenges discussed among participants included uncertainty, complexity, and multiplicity of problems and treatments; difficulty switching to patients' priorities as the focus of decision-making; and differing perspectives between patients and clinicians, and among clinicians. Strategies identified to support patient priorities-aligned decision-making included starting with one thing that matters most to each patient; conducting serial trials of starting, stopping, or continuing interventions; focusing on function (i.e. achieving patient's desired activities) rather than eliminating symptoms; basing communications, decision-making, and effectiveness on patients' priorities not solely on diseases; and negotiating shared decisions when there are differences in perspectives.ConclusionsThe discrete set of challenges encountered and the implementable strategies identified suggest that patient priorities-aligned decision-making in the care of patients with multiple chronic conditions is feasible, albeit complicated. Findings require replication in additional settings and determination of their effect on patient outcomes.

Model organisms are vital to uncovering the mechanisms of human disease and developing new therapeutic tools. Researchers collecting and integrating relevant model organism and/or human data often apply disparate terminologies (vocabularies and ontologies), making comparisons and inferences difficult. A unified disease ontology is required that connects data annotated using diverse disease terminologies, and in which the terminology relationships are continuously maintained. The Mouse Genome Database (MGD, http://www.informatics.jax.org), Rat Genome Database (RGD, http://rgd.mcw.edu) and Disease Ontology (DO, http://www.disease-ontology.org) projects are collaborating to augment DO, aligning and incorporating disease terms used by MGD and RGD, and improving DO as a tool for unifying disease annotations across species. Coordinated assessment of MGD's and RGD's disease term annotations identified new terms that enhance DO's representation of human diseases. Expansion of DO term content and cross-references to clinical vocabularies (e.g. OMIM, ORDO, MeSH) has enriched the DO's domain coverage and utility for annotating many types of data generated from experimental and clinical investigations. The extension of anatomy-based DO classification structure of disease improves accessibility of terms and facilitates application of DO for computational research. A consistent representation of disease associations across data types from cellular to whole organism, generated from clinical and model organism studies, will promote the integration, mining and comparative analysis of these data.
