Abstract

Autoimmune serology for the idiopathic inflammatory myopathies has over the years gained impact in the diagnostic work-up of these diseases. In The Netherlands the myositis line immuno-assay is offered by a limited number of laboratories, but the distinct phases from request to interpretation of results, are rather variable. These phases are explored by means of a questionnaire. Based on the results a consensus has been obtained about requesting, performing, reporting and interpreting the test results. The road to and the result of this harmonization initiative are reported in this article.

Abstract

Public relations (PR) and information to patients are key goals of the Dutch Society of Clinical Chemistry (NVKC). In this paper several initiatives will be discussed that have been undertaken to meet this statement. The Wikipedia project is one of those initiatives in which information concerning clinical chemistry is added or revised to the Dutch version of the online encyclopaedia Wikipedia. Over 100 tests and items of clinical chemistry have been added or revised and are recognized as a separate category within this online encyclopaedia. Statistical analysis showed an increase in the number of visitors to these articles of Wikipedia. Furthermore, by adding and improving the available information, the quality of the information provided online is enhanced. In conclusion: the attribution of health care professionals to this public domain leads to enhanced access of high quality information for the main public to clinical chemistry in general and to specific laboratory blood tests.

Abstract

Currently DNA-based analysis of blood groups is mainly used to improve transfusion safety by reducing alloantibody formation in multiply transfused patients and by monitoring pregnancies at risk for hemolytic disease of the fetus and newborn. We present a case in which genotyping was performed after massive transfusion with unmatched group O, D– blood in a trauma setting. Our patient was genotyped as O1A1and predicted to be D–, and we therefore transfused group A, D– red blood cell concentrates. This case demonstrates how the use of blood group genotyping in an acute setting can lead to a decrease in the unnecessary use of group O, D– blood products. Immunohematology 2012;28:85–7.

28. Monitoring direct thrombin inhibitors with routine and dedicated coagulation assays : which assay is helpful?

Abstract

The use of direct thrombin inhibitors (DTIs) for prophylactic or therapeutic anticoagulation is increasing because of the predictable bioavailability and short half-life of these DTIs. However, in certain situations, indication of the concentration is warranted. We investigated the effects of 3 DTIs (lepirudin, argatroban, and bivalirudin) in 6 pooled plasma specimens on routine coagulation assays (activated partial thromboplastin time [aPTT], prothrombin time [PT], and thrombin time [TT]) and dedicated DTI assays (Hemoclot, HemosIL, the ecarin clotting time, and a chromogenic ecarin clotting time) on 2 coagulation analyzers. We found routine tests to be nondiscriminative between concentrations of different DTIs in the aPTT. Moreover, for PT and TT, the responses for different DTIs differed. This was similar for ecarin clotting assays. The Hemoclot and HemosIL assays showed identical linear increases for all 3 DTIs. We conclude that dedicated calibrated assays based on a diluted TT (Hemoclot and HemosIL) appear to be the most suitable for monitoring purposes.

29. Allogene trombocytengel bij urologische patiënt met een verworven trombocytopathie

Abstract

Applicatie van een trombocytengel kan de wondheling versnellen en het risico op complicaties (nabloeden, wondinfectie) verminderen. Deze casus betreft een patiënt met een verworven trombocytopathie, die operatief wordt behandeld voor een vergrote prostaat en blaasstenen, waarbij na de eerste operatie een gecompliceerd beloop optreedt. De patiënt heeft last van persisterende bloedingen, waarvoor enkele malen operatief stolsels uit de blaas worden geëvacueerd. In totaal ontvangt de patiënt 14 eenheden erytrocytenconcentraat en 7 eenheden vers bevroren plasma. Uiteindelijk wordt bij deze patiënt gebruik gemaakt van een trombocytengel op basis van een allogeen product (5 donoren trombocytenconcentraat), waarna geen stolselretentie meer optreedt en de patiënt snel herstelt. Autologe donatie bij patiënten met een verworven trombocytopathie, levert geen goede trombocytengel, het gebruik van allogene trombocyten is dan een goed alternatief.

30. Autologe plaatjes-leukocytengel : toepassing bij diverse chirurgische indicaties

Abstract

Het therapeutisch gebruik van autoloog bereidde plaatjes-leukocytengel (PLG) is een nieuwe technologie welke de genezing van zachte weefsels en bot bevordert en versnelt. De doeltreffendheid van deze procedure ligt in de exogene aanvoer van diverse trombocytengroeifactoren, die vrijgemaakt worden uit de autoloog bereidde PLG. De reden om deze techniek toe te passen is, het op een suprafysiologische manier stimuleren van genezingsprocessen om sneller wondgenezing en weefselherstel te bewerkstelligen. Tevens kan de transfusie van allogene bloedproducten tijdens chirurgische ingrepen worden gereduceerd door verminderd bloedverlies als gevolg van snellere wonddichting. Ondanks een toename in klinische toepassingen van PLG is de structuur en kinetiek van dit biologisch materiaal niet volledig bekend. In dit artikel worden toepassingen voor PLG besproken, gebaseerd op publicaties in diverse chirurgische disciplines.

33. Evaluation of the Ves-Matic Cube 200 erythrocyte sedimentation method : comparison with Westergren-based methods

Abstract

The erythrocyte sedimentation rate (ESR) is still a widely used parameter for acute phase inflammation. Recently, new methods based on direct undiluted measurement of ESR in a standard EDTA tube have been developed. We evaluated the analytic performance of one of these new methods, the Ves-Matic Cube 200 (Diesse Diagnostica Senese, Siena, Italy), and compared it with several established Westergren-based diluted methods. The Ves-Matic Cube 200 showed a poor correlation (r = 0.83) with the International Council for Standardization in Haematology Westergren reference method, mainly caused by a considerable negative bias at low ESR levels. Moreover, a random bias was found at higher ESR levels that correlated with hematocrit levels, suggesting a differential influence of packed cell volume on the Ves-Matic Cube 200 results compared with Westergren results. We conclude that the Ves-Matic Cube 200 method is not interchangeable with Westergren-based diluted methods and generates ESR results that are too deviant to be clinically acceptable.

Contains fulltext : 69751.pdf (publisher's version ) (Closed access), In a clinical setting, fresh frozen plasma (FFP) is transfused to diluted patients with complicated surgery or trauma, as guided by prolonged conventional coagulation times or low fibrinogen levels. However, the limited sensitivity of these coagulation tests may restrict their use in measuring the effect of transfusion and hence predicting the risk of perioperative bleeding. We used the more sensitive, calibrated automated thrombogram (CAT) method to evaluate the result of therapeutic FFP transfusion to 51 patients with dilutional coagulopathy. Thrombin generation was measured in pre- and post-transfusion plasma samples in the presence of either platelets or phospholipids. For all patients, the transfusion led to higher plasma coagulation factor levels, a shortened activated partial thromboplastin time, and a significant increase in thrombin generation (peak height and endogenous thrombin potential). Interestingly, thrombin generation parameters and fibrinogen levels were higher in post-transfusion plasmas from patients who stopped bleeding (n = 32) than for patients with ongoing bleeding (n = 19). Plasmas from 15 of the 19 patients with ongoing bleeding were markedly low in either thrombin generation or fibrinogen level. We conclude that the thrombin generation method detects improved haemostatic activity after plasma transfusion. Furthermore, the data suggest that thrombin generation and fibrinogen are independent determinants of the risk of perioperative bleeding in this patient group.
