Abstract

Community concerns surrounding syringe waste are a common barrier to syringe services program (SSP) implementation. In Kanawha County, West Virginia, community opposition to SSPs resulted in the closure of needs-based SSPs prior to and during an HIV outbreak among persons who inject drugs (PWID). This qualitative analysis examines views of PWID and community partners on syringe waste and disposal associated with needs-based SSPs. Qualitative interviews with 26 PWID and 45 community partners (medical and social service providers, law enforcement personnel, policymakers, and religious leaders) were conducted. Interviews were recorded, transcribed, and coded. Code summaries described participants' views on syringe waste and disposal and needs-based SSPs. Community partners and PWID who favored needs-based SSPs reported that needs-based SSPs had not affected or reduced syringe waste. Conversely, community partners who favored one-to-one exchange models and/or barcoded syringes described needs-based SSPs increasing syringe waste. Community partners often cited pervasive community beliefs that SSPs increased syringe waste, risk of needlesticks, drug use, and crime. Community partners were unsure how to address syringe waste concerns and emphasized that contradictory views on syringe waste posed barriers to discussing and implementing SSPs. Participants' views on whether syringe waste was associated with needs-based SSPs often aligned with their support or opposition for needs-based SSPs. These differing views resulted in challenges finding common ground to discuss SSP operations amid an HIV outbreak among PWID. SSPs might consider addressing syringe waste concerns by expanding syringe disposal efforts and implementing community engagement and stigma reduction activities. [ABSTRACT FROM AUTHOR]

14. Understanding the barriers to, and impact of, men's engagement in physical activity and health related behaviours : an examination of an English Premier League 'football in the community' men's health programme

Abstract

This thesis outlines research undertaken by formal collaboration between Everton Football Club's Football in the Community (FitC) scheme; Everton in the Community (FitC) and Liverpool John Moores University, School of Sport and Exercise Sciences. In recent years, there has been recognition of the influence that English Premier league (EPL) football clubs can have in attracting men to physical activity and health engagement programmes. Despite attempts to align FitC programmes with suitable evaluation procedures, there still remains limited evaluative empirical evidence. '. Study 1 adopted ethnographic principles to explore the effectiveness of, and identify the barriers to, promoting positive health behaviours and messages to male football fans at an EPL football stadium on match days. Results showed that in general, men did not wish to engage in health related behaviours on match days however approaches that did not impose on, nor contaminate, the men's match day experience were more successful. Study 2 adopted a multi-method approach to explore the distinct barriers that hard-to-reach (HTR) male populations encounter when attempting to commit to regular participation in physical activity and health behaviours and to examine the impact of engaging in a 12 week FitC intervention. Economic, environmental and social barriers to engagement in regular physical activity and positive health behaviours are highlighted and specific biopsychosocial effects of engaging in the FitC programme are identified. Study 3 utilised informal semi structured interviews with programme participants to explore the contextual, environmental and psychosocial barriers experienced by men from HTR populations. Psychosocial motivations for programme uptake and the impact of regular engagement in the FitC men's health programme are discussed. It is recommended that commissioning agencies should endorse and fund men's health initiatives delivered in and by professional sports clubs. To maintain participant engagement and maximise improvements to men's health and wellbeing, alterations to current practice and research are discussed.

BackgroundStudy-based global health interventions, especially those that are conducted on an international or multi-site basis, frequently require site-specific adaptations in order to (1) respond to socio-cultural differences in risk determinants, (2) to make interventions more relevant to target population needs, and (3) in recognition of 'global health diplomacy' issues. We report on the adaptations development, approval and implementation process from the Project Accept voluntary counseling and testing, community mobilization and post-test support services intervention.MethodsWe reviewed all relevant documentation collected during the study intervention period (e.g. monthly progress reports; bi-annual steering committee presentations) and conducted a series of semi-structured interviews with project directors and between 12 and 23 field staff at each study site in South Africa, Zimbabwe, Thailand and Tanzania during 2009. Respondents were asked to describe (1) the adaptations development and approval process and (2) the most successful site-specific adaptations from the perspective of facilitating intervention implementation.ResultsAcross sites, proposed adaptations were identified by field staff and submitted to project directors for review on a formally planned basis. The cross-site intervention sub-committee then ensured fidelity to the study protocol before approval. Successfully-implemented adaptations included: intervention delivery adaptations (e.g. development of tailored counseling messages for immigrant labour groups in South Africa) political, environmental and infrastructural adaptations (e.g. use of local community centers as VCT venues in Zimbabwe); religious adaptations (e.g. dividing clients by gender in Muslim areas of Tanzania); economic adaptations (e.g. co-provision of income generating skills classes in Zimbabwe); epidemiological adaptations (e.g. provision of 'youth-friendly' services in South Africa, Zimbabwe and Tanzania), and social adaptations (e.g. modification of terminology to local dialects in Thailand: and adjustment of service delivery schedules to suit seasonal and daily work schedules across sites).ConclusionsAdaptation selection, development and approval during multi-site global health research studies should be a planned process that maintains fidelity to the study protocol.
