Background: The aim of this study was to investigate the effect of exercise during pregnancy on early childhood neurodevelopment (cognitive, motor, and language domains). Methods: A randomized controlled trial nested into the 2015 Pelotas (Brazil) Birth Cohort was conducted. Healthy pregnant women were enrolled between 16 and 20 weeks of gestation; 424 women and their children (intervention [n = 141]; control [n = 283]) were analyzed. An exercise-based intervention 3 times per week was delivered over 16 weeks. Child neurodevelopment and its domains were assessed at 1, 2, and 4 years. Standardized mean differences and 95% confidence intervals are presented. Results: No effects of exercise during pregnancy on child neurodevelopment and its domains at age 1 year were observed. Compared with the control group, children from women in the exercise group had higher language score at age 2 years (standardized mean differences = 0.23; 95% confidence intervals, 0.02 to 0.44) and higher cognitive score (standardized mean differences = 0.22; 95% confidence intervals, 0.03 to 0.41) at age 4 years. No effects of exercise during pregnancy were observed in the motor domain at 1, 2, and 4 years. Conclusions: No detrimental effects of exercise during pregnancy on child neurodevelopment were observed. Objective: To examine the joint associations of daily time spent in different intensities of physical activity, sedentary behaviour and sleep with all-cause mortality.Methods: Federated pooled analysis of six prospective cohorts with device-measured time spent in different intensities of physical activity, sedentary behaviour and sleep following a standardised compositional Cox regression analysis.Participants: 130 239 people from general population samples of adults (average age 54 years) from the UK, USA and Sweden.Main Outcome: All-cause mortality (follow-up 4.3-14.5 years).Results: Studies using wrist and hip accelerometer provided statistically different results (I2=92.2%, Q-test p<0.001). There was no association between duration of sleep and all-cause mortality, HR=0.96 (95% CI 0.67 to 1.12). The proportion of time spent in moderate to vigorous physical activity was significantly associated with lower risk of all-cause mortality (HR=0.63 (95% CI 0.55 to 0.71) wrist; HR=0.93 (95% CI 0.87 to 0.98) hip). A significant association for the ratio of time spent in light physical activity and sedentary time was only found in hip accelerometer-based studies (HR=0.5, 95% CI 0.42 to 0.62). In studies based on hip accelerometer, the association between moderate to vigorous physical activity and mortality was modified by the balance of time spent in light physical activity and sedentary time.Conclusion: This federated analysis shows a joint dose-response association between the daily balance of time spent in physical activity of different intensities and sedentary behaviour with all-cause mortality, while sleep duration does not appear to be significant. The strongest association is with time spent in moderate to vigorous physical activity, but it is modified by the balance of time spent in light physical activity relative to sedentary behaviour. Routine immunization during pandemics can be harmed. This study estimated the influenza vaccination coverage in older adults during the COVID-19 through the EPICOVID-19, a population-based study conducted in 133 cities from the 26 Brazilian states and Federal District. We selected 25 census tracts per city, with probability proportional to the tract's size, ten households by census tract, and one random individual interviewed. A total of 8,265 older adults (=60 years old) were interviewed and asked whether they had been vaccinated against flu in 2020. Vaccination coverage was 82.3% (95% CI: 80.1-84.2) with no difference by gender, age, and region; higher vaccination coverage was observed among the wealthiest (84.7% versus 80.1% in the poorest) and among the more educated (87.3% versus 83.2% less educated); lower coverage among indigenous (56.9% versus > 80% among other ethnic groups). A positive association was identified with the number of comorbidities among men but not among women. Most of the population was vaccinated (97.5%) in the public health system. The private network was chosen mainly in the South by the wealthiest and more educated. Vaccination coverage was seven percentage points lower than the government target (90%), and inequalities should be reversed in future campaigns. [ABSTRACT FROM AUTHOR]

Background: Most of physical activity surveillance data are derived from high-income countries. The aim of the current study was to report time trends in leisure-time physical activity. Methods: Population-based surveys were conducted in the city of Pelotas, Brazil in 2003 and 2010, including individuals aged 20+ years. Physical activity was assessed using the leisure-time section of the long version of the IPAQ. A cut-off point of 150 min/wk was used in the analyses. Methodologies were virtually identical in both surveys. Results: In 2003, 26.8% (95% CI 24.3; 29.2) of the participants were classified as active in leisure-time, as compared with 24.4% (95% CI 22.6; 26.2) in 2010. The proportion of subjects reporting 0 minutes per week of walking, moderate- and vigorous-intensity physical activity practice also did not vary between 2003 and 2010. However, the proportion of active adults decreased from 39.9% (95% CI 33.0; 42.7) in 2003 to 29.7% (95% CI 24.9; 34.5) in 2010 among high-income participants. Males were more active than females in both surveys. Conclusions: Leisure-time physical activity is stable among adults living in the South of Brazil, but high-income participants are becoming less active over time. This study aimed to describe leisure- time physical activity (LPA) during the COVID-19 pandemic in a municipality of Rio Grande do Sul state, southern Brazil, according to gender, level of education, and adherence to social distancing. A population-based and cross-sectional descriptive study was carried out in Bagé (RS), Brazil. LPA during the pandemic, place of activity, and Physical Education professional's supervision, were described. The sample included 377 adults, and 24.4% reported LPA during the pandemic. Marked inequalities were observed. LPA prevalence among men was 20 percentage points (pp) higher than women and 40 pp higher among those with higher schooling than those with lower schooling. Among those reporting LPA, 53.5% practiced at home, and 64.8% did not report Physical Education professional supervision. No differences were observed between LPA and level of social distancing. Besides the recurrent discourse that people should include physical activity in the pandemic context, in the light of the marked inequalities observed, this study addressed sociocultural aspects and emphasized that LPA promotion initiatives require humanized approaches that consider the unequal living conditions of Brazilians. The first case of COVID-19 was reported in China in December 2019, and, as the virus has spread worldwide, the World Health Organization declared it a pandemic. Estimates on the number of COVID-19 cases do not reflect it real magnitude as testing is limited. Population based data on the proportion of the population with antibodies is relevant for planning public health policies. We aim to assess the prevalence of SARSCoV- 2 antibodies, presence of signs and symptoms of COVID-19, and adherence to isolation measures. A random sample comprising 133 sentinel cities from all states of the country will be selected. Three serological surveys, three weeks apart, will be conducted. The most populous municipality in each intermediate region of the country, defined by the Brazilian Institute of Geography and Statistics, was chosen as sentinel city. In each city, 25 census tracts will be selected, and 10 households will be systematically sampled in each tract, totaling 33,250 participants. In each household, one inhabitant will be randomly selected to be interviewed and tested for antibodies against SARSCoV- 2, using WONDFO SARS-CoV-2 Antibody Test. By evaluating a representative sample of Brazilian sentinel sites, this study will provide essential information for the design of health policies. O COVID-19 é causado pelo vírus SARS-CoV-2, sendo o primeiro caso relatado na China em dezembro de 2019. O vírus se espalhou pelo mundo, levando a Organização Mundial da Saúde a declarar uma pandemia. As estimativas do número de casos de COVID-19 não refletem sua magnitude real, pois os testes são limitados em muitos países. Dados populacionais sobre a proporção da população com anticorpos são relevantes para o planejamento de políticas públicas de saúde. Nosso objetivo é avaliar a prevalência de anticorpos SARS-CoV-2, a presença de sinais e de sintomas de COVID-19 e a adesão a medidas de isolamento. Uma amostra aleatória composta por 133 cidades sentinelas de todos os estados do país será selecionada. Serão realizados três levantamentos sorológicos, com três semanas de intervalo. Em cada cidade, serão selecionados 25 setores censitários e 10 famílias serão amostradas aleatoriamente em cada setor. Em cada domicílio, um habitante será selecionado aleatoriamente para ser entrevistado e testado para anticorpos contra SARS-CoV-2, usando o Teste de Anticorpo WONDFO SARS-CoV-2, que foi validado antes do trabalho de campo. 18. Atividade física em adolescentes brasileiros: uma revisão sistemática sistemática

The present study aims to review the literature on studies in the area of physical activity and sedentary behaviors in Brazilian adolescents. Systematic review of the literature. The search was carried out in electronic databases and references of the papers identifi ed. The following keywords, in English and Portuguese, were used: “exercise”, “physical effort”, “physical fitness”, “sports”, “motor activity”, “physical activity”, “energetic metabolism”, “health behavior”, “lifestyle”, “risk factors”, “adolescent”, “adolescence”, “youth”, and “students”. The electronic search resulted in 653 manuscripts. The process of analysis included examination of titles, abstracts and full texts. After all these phases, 15 papers fulfi lled the inclusion criteria of the review. In the references of these papers, six extra relevant publications were identifi ed, totalizing 21 papers. In terms of the design, only two studies used the case-control approach; all others were cross-sectional. Twelve studies were classifi ed as having an adequate sampling strategy, and presented a description of the sampling methods. A single study provided information on the quality of the measurements (validity) and previous testing of the instruments. Six studies included simultaneously measurements of physical activity and sedentary behaviors. There is a wide methodological variability across the national studies on physical activity and sedentary behavior. However, there is a homogeneity in the designs used, highlighting the need of longitudinal studies in the area. O estudo teve como objetivo revisar a literatura sobre estudos na área de atividade física e comportamentos sedentários em adolescentes brasileiros. Revisão sistemática da literatura. A busca foi realizada em bases de dados eletrônicas e lista de referência dos artigos identifi cados. Os seguintes descritores, nas línguas portuguesa e inglesa, foram utilizados: “exercício”, “esforço físico”, “aptidão física”, “esportes”, “atividade motora”, “atividade física”, “metabolismo energético”, “conduta de saúde”, “estilo de vida”, “fatores de risco”, “adolescente”, “adolescência”, “juventude” e “estudantes”. A busca eletrônica inicial resultou em 653 manuscritos. O processo de análise dos estudos envolveu leitura de títulos, resumos e textos completos. Após todas essas fases, 15 manuscritos preencheram os critérios de inclusão da revisão. Nas referências destes artigos, foram identifi cadas outras seis publicações relevantes, totalizando 21 artigos. Em relação ao delineamento, apenas dois estudos utilizaram o desenho de casos e controles, sendo todos os demais transversais. Doze estudos foram classifi cados como tendo dimensionamento amostral adequado e apresentaram descrição do procedimento utilizado. Somente um estudo trazia informações sobre testagem prévia do instrumento e validade das medidas. Seis estudos incluíram simultaneamente medidas de atividade física e comportamentos sedentários. Há uma grande variabilidade metodológica entre os estudos nacionais sobre atividade física e comportamentos sedentários em adolescentes. No entanto, existe uma homogeneidade no delineamento, sendo necessários estudos longitudinais na área.

Background: Women are encouraged to be physically active during pregnancy. Despite available evidence supporting antenatal physical activity to bring health benefits for both the mother and child, the most effective way to prevent some maternal and fetal outcomes is still unclear. The purpose of this study was to evaluate the efficacy of an exercise intervention to prevent negative maternal and newborn health outcomes. Methods: A randomized controlled trial (RCT) nested into the 2015 Pelotas (Brazil) Birth Cohort Study was carried-out with 639 healthy pregnant women, 213 in the intervention group (IG) and 426 in the control (CG) group. An exercise-based intervention was conducted three times/week for 16 weeks from 16-20 to 32-36 weeks' gestation. The main outcomes were preterm birth and pre-eclampsia. Gestational age was calculated based on several parameters, including routine ultrassounds and/or last menstrual period and categorized as < 37 weeks and ≥ 37 weeks for evaluation of preterm birth. Pre-eclampsia was self-reported. Secondary outcomes were gestational weight gain, gestational diabetes, birth eight, infant length, and head circumference. Analyses were performed by intention-to-treat (ITT) and per protocol (70% of the 48 planned exercise sessions). Odds ratio were derived using unconditional logistic regression. Results: The IG and CG did not differ at baseline regarding their mean age (27.2 years ± 5.3 vs. 27.1 years ± 5.7) and mean pre-pregnancy body mass index (25.1 ± 3.9 vs. 25.2 ± 4.1 kg/m²). The mean adherence to the exercise intervention was 27 ± 17.2 sessions (out of a potential 48) with 40.4% attending > = 70% of the recommended exercise sessions. A total of 594 participants (IG:198; CG: 396) were included in the ITT and 479 (IG: 83; CG: 396) were included in the per protocol analyses. There were no significant differences in the incidence of preterm birth and pre-eclampsia between groups in the ITT and per protocol analysis. There were also no differences between the two groups in mean gestational weight gain, gestational diabetes, birth weight, infant length, and head circumference. Conclusions: While the RCT did not support the benefits of exercise performed during pregnancy on preeclampsia and preterm birth, the exercise program also did not present adverse impacts on newborn health. Our findings may contribute to promote intervention strategies that motivate health providers to encourage pregnant women to be more physically active. Background: Preterm birth is associated with most cases of neonatal deaths and negative health outcomes, and hypertensive disorders. Hypertension is influenced by maternal behavior, such as physical activity. Physical activity is associated with better outcomes for mother and fetus, besides healthier weight gains during pregnancy. Few women are physically active during pregnancy and few clinical trials have been carried out with pregnant women. The aim of this paper is to describe the protocol of a controlled trial evaluating whether regular exercise during pregnancy may result in improved maternal-child health and neonatal outcomes. Methods/Design: The PAMELA (Physical Activity for Mothers Enrolled in Longitudinal Analysis) trial is a randomized controlled trial nested in a birth cohort study. Eligible women belonging to the birth cohort will be invited (between the 16th and 20th week of gestation) to enroll in the trial. Baseline data (blood and urine samples, anthropometry and pulmonary function) will be collected at enrollment. The same assessments will be repeated eight and 16 weeks after baseline. After randomization, women will be allocated into either one of these groups: control, 426 women who will be advised to keep their usual daily activities; and intervention, 213 women who will engage in an exercise program, three sessions a week. At least 70% attendance over 16 weeks will be required to be considered compliant to the intervention. Exercise protocol will include aerobics, strength and flexibility training. Maternal and child outcomes will be measured at the 36th week of gestation, at birth and at three, 12, 24 and 48 months postpartum. An intention-totreat analysis will be performed. Discussion: Few women are active during pregnancy and a vast majority decrease their activities or even quit exercising. We present a population-based regular exercise intervention focused on the prevention of hypertension, pre-eclampsia and preterm birth. Data on the underlying cohort will allow future analysis using different outcomes with low probability of recall bias or misclassification of exposure status. Background: We investigated whether Body Mass Index (BMI) at 11 years old has a direct effect on bone mass at age 18 operating through alterations to bone growth and development, or whether the association is mediated by concurrent BMI, fat mass (FM), and fat free mass (FFM). Methods: Path analysis was used to explore the association between BMI at age 11 and whole-body bone mineral content (BMC) and bone mineral density (BMD) assessed by dual-energy x-ray absorptiometry (DXA) at age 18 in a prospective birth cohort study comprising 3,307 adolescents; we also evaluated the degree to which this association was mediated by BMI, FM (kg) and FFM (kg) assessed by plethysmography (BOD POD) at age 18. Results: We found a positive association between BMI at age 11 and BMC (males [β = 179.7 g, 95% CI 161.4; 198.0]; females [β = 179.9 g, 95% CI 165.3; 194.6]) and BMD (males [β = 0.030 g/cm², 95% CI 0.024; 0.035]; females [β = 0.029 g/cm², 95% CI 0.025; 0.033]) at age 18. This association was largely mediated by BMI and FFM at age 18 in both female and male adolescents. FM at age 18 was not an important mediator. Conclusions: Concurrent BMI and FFM were the main mediators of the association between BMC/BMD in late adolescence and BMI in early adolescence. [ABSTRACT FROM AUTHOR]

30. Time Trends of Physical Activity Among Brazilian Adolescents Over a 7-Year Period.

Purpose: To evaluate time trends in physical activity among adolescents aged 10 to 19 years living in southern Brazil over a 7-year period. Methods: Two population-based cross-sectional surveys with similar methodologies were carried out in the city of Pelotas, Brazil, in 2005 and 2012. Leisure-time and transport-related physical activity were measured using a validated questionnaire. A cut-off point of 300 minutes per week was used to classify adolescents as active or not. We also analyzed the two domains of physical activity (leisure time and transportation) separately. Results: The prevalence of physical inactivity was 69.6% (95% CI 66.5-73.2) in 2005 and 69.9% (95% CGI 66.5-72.7) in 2012. The percentage of active adolescents in leisure time also remained stable in the period (26.3% in 2005 [95% CI 23.3-29.2] vs. 28.1% in 2012 [95% CI 24.9-31.4]). Among boys we observed an increase in the practice of some leisure-time activities--weight lifting (87%) and running (78%)--and a decline in others--volleyball (61%) and basketball (56%). Among girls, the only significant difference was an increase in the practice of weight lifting (271%). The prevalence of active commuting to and from school declined from 69% (95% CI 65.6-72.4) in 2005 to 56.5 (95% CI 52.5-60.2) in 2012. Conclusions: There was a significant decline in active commuting to school among adolescents. Interventions promoting active commuting modes to school are urgently needed in Brazil. Background: The aim of this study was to investigate the association between the perceived environment and the use of public open spaces (POS). Methods: A cross-sectional study with household surveys was conducted in 1,461 adults from Curitiba, Brazil interviewed in person. The perceived environment was evaluated with the Neighborhood Environment Walkability Scale, and the POS use was evaluated using the ordinal scale (increased use). Results: The presence of interesting objects, heavy traffic, and the number of positive attributes of the environment was positively associated with POS use among men, and the presence of trees was associated with the use among women. Conclusions: Managers should invest in the architectural attractiveness of neighborhoods and should plant and conserve trees to encourage POS use. [ABSTRACT FROM AUTHOR]

Background: Neighborhood safety is one of the environmental aspects that can influence physical activity. We analyzed the association between perceived neighborhood safety and physical inactivity (PI) in adults and examined effect modification according to sociodemographic variables. Methods: A cross-sectional study was conducted with 1,261 adults (62% women), age 18-69 years from Curitiba, Brazil. Results: The perception of unsafe neighborhood was higher among women, older participants, those classified in the high socioeconomic (SES) group, overweighed and also among those reporting to have PA equipments and children. The association between perception safety of walking during the day and walking for leisure (women PR = 1.12 CI
95% = 1.02-1.22; men PR = 0.82 CI95% = 0.64-1.05; interaction term PR = 1.38 CI95% = 1.03-1.83) and safe perception was associated with PI, just in the highest SES group (PR = 1.09; CI95% = 1.00-1.19; p trend = 0.032) when compared with their counterparts (low SES PR = 0.99; CI95% = 0.90-1.04; p trend = 0.785; interaction term PR = 1.09; CI95% = 1.03-1.15; p trend = 0.007). To analyze COVID-19 vaccine uptake in children and to investigate factors associated with two outcomes variables: (a) not even beginning; (b) not completing the COVID-19 vaccine series. We used data of children aged 6–7 years from the 2015 Pelotas c Birth Cohort Study. COVID-19 vaccination status was collected from immunization cards and National Immunization Program Information System. Adjusted analyses were performed using a hierarchical model to identify factors associated with the two study outcomes. Among 3867 children, 20.7 % (95 % CI, 19.5 %–22.0 %) did not even begin the 2-dose primary COVID-19 vaccine series, and 28.2 % (95 % CI, 26.6 %–29.8 %) did not complete the series with the second dose. Children not even beginning the COVID-19 vaccine series were more likely to have a White mother, not to have obesity, to have a history of COVID-19 infection, to have received non-recommended drugs for COVID-19, to be afraid of needles, and to have an incomplete diphtheria-tetanus-pertussis (DTP) and poliovirus immunization schedule. Not completing the 2-dose series was associated with lower maternal age and education, mother's self-identification as White or Brown, lower household income, lack of access to health services, not having completed the DTP and poliovirus immunization schedule and living with a person with a history of infection with COVID-19. The results highlight a vaccine-hesitant parents' group who chose not beginning the COVID-19 vaccine series of their children and, another group of parents who failure to complete the child's series due to difficulty accessing health services. - Abstract
Objective. To describe the methods employed to assess the built environment and physical activity (PA) as part of a multicenter international study, and to discuss the challenges faced to obtain the necessary data in the Brazilian context. Methods. In 2010 a household survey was conducted with adults aged from 20 to 65 years in the city of Curitiba, Brazil. The study involved the used of geo-referenced information to measure walkability in all 2 125 census sectors in Curitiba. Census sectors were categorized by walkability and income, taking into consideration the average income of heads of the family in each sector. Physical activity was assessed by self-report and using an objective measure (accelerometers). Results. Sixteen high walkability and 16 low walkability sectors were studied, with eight high-income and eight low-income sectors in each category. A total of 699 subjects were interviewed and 381 wore accelerometers. The response rate was 66.4% for the interviews and the compliance with accelerometer use was 85.8% (n = 327). Conclusions. The results show that it is feasible to conduct high-quality studies on physical activity and built environment in the Brazilian context in accordance with international standards. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior O aumento da adiposidade corporal na população tornou-se um problema de saúde pública. Sabe-se que o excesso de adiposidade corporal está associado a diversas comorbidades, aumentando o gasto público com a saúde. Objetivou-se analisar a validade e os fatores associados do índice de adiposidade corporal em adultos. Um estudo transversal, de base populacional, foi realizado com uma amostra de 972 adultos, com idades entre 20 e 59 anos, na cidade de Viçosa, Minas Gerais, Brasil, em 2014. Foram aplicados questionários domiciliares para obter dados de condições socioeconômicas, demográficas, hábito de fumar, estilo de vida e morbidades autorreferidas. Medidas clínicas foram coletadas na Divisão de Saúde da Universidade Federal de Viçosa. O desfecho do estudo foi o índice de adiposidade corporal, obtido através da circunferência do quadril e da altura. As variáveis independentes incluídas foram sociodemográficas, comportamentos e hábitos relacionados á saúde, medidas antropométricas e morbidades autorrelatadas. As variáveis sociodemográficas foram: sexo; idade (20 a 29, de 30 a 39, 40 a 49 e 50 a 59 anos); nível de escolaridade ( 12 anos completos); classe econômica de acordo com bens de consumo (elevada, intermediária e baixa); cor da pele autorreferida (branca e não branca) e estado civil (com companheiro e sem companheiro). Comportamentos e hábitos relacionados à saúde incluíram o tabagismo (não-fumante, ex-fumante e fumante atual); nível de atividade física (inativo fisicamente < 150 minutos/semana e ativo fisicamente ≥ 150 minutos/semana), estimado pela versão longa, em português, do International Physical Activity Questionnaire; a (in)satisfação com a imagem corporal, avaliada pela escala de silhuetas para adultos (satisfeito e insatisfeito); a autoavaliação geral da saúde (regular/ruim e muito bom/bom) e doenças autorreferidas (diabetes, hipertensão e colesterol elevado). Os dados foram digitados em duplicata no software Epidata, por digitadores previamente treinados. Após verificar a consistência dos dados, realizou-se análises estatísticas utilizando o pacote estatístico Stata, versão 13,1. Todas as análises foram ajustadas para o efeito de delineamento amostral e ponderadas pela frequência de sexo, idade e escolaridade e atribuídos pesos pela razão entre as proporções da população, obtidos a partir do Instituto Brasileiro de Geografia e Estatística (IBGE), e da amostra. Foram realizadas estatísticas descritivas das variáveis da amostra populacional bem como testes qui-quadrado e de tendência linear, quando apropriado, para estimar as associações entre o desfecho e cada variável independente. Teste de Shapiro-Wilk foi utilizado para verificar a normalidade das variáveis. A correlação entre as variáveis foi avaliada pelo coeficiente de correlação de Pearson. Utilizou-se a abordagem gráfica de Bland-Altman para verificar a concordância entre o índice de adiposidade corporal e o absorciometria por dupla emissão de raios X. Para analisar o grau de concordância ou discordância, utilizou-se os gráficos de sobrevida de Kaplan-Meier. A diferença entre o índice de adiposidade corporal e a absorciometria por dupla emissão de raios X foi verificada por regressão de Cox.
