ImportanceDelays in screening programs and the reluctance of patients to seek medical attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced colorectal cancers at diagnosis.ObjectiveTo evaluate whether the SARS-CoV-2 pandemic was associated with more advanced oncologic stage and change in clinical presentation for patients with colorectal cancer.Design, Setting, and ParticipantsThis retrospective, multicenter cohort study included all 17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December 31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period), in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was 30 days from surgery.ExposuresAny type of surgical procedure for colorectal cancer, including explorative surgery, palliative procedures, and atypical or segmental resections.Main Outcomes and MeasuresThe primary outcome was advanced stage of colorectal cancer at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding, lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery, and palliative surgery. The independent association between the pandemic period and the outcomes was assessed using multivariate random-effects logistic regression, with hospital as the cluster variable.ResultsA total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years) underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142 (56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR], 1.07; 95% CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95% CI, 1.15-1.53; P P = .03).Conclusions and RelevanceThis cohort study suggests a significant association between the SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for these patients.

Locally Recurrent Rectal Cancer (LRRC) remains a major clinical concern; it rapidly invades pelvic organs and nerve roots, causing severe symptoms. Curative-intent salvage therapy offers the only potential for cure but it has a higher chance of success when LRRC is diagnosed at an early stage. Imaging diagnosis of LRRC is very challenging due to fibrosis and inflammatory pelvic tissue, which can mislead even the most expert reader. This study exploited a radiomic analysis to enrich, through quantitative features, the characterization of tissue properties, thus favoring an accurate detection of LRRC by Computed Tomography (CT) and 18 F-FDG-Positron Emission Tomography/CT (PET/CT). Of 563 eligible patients undergoing radical resection (R0) of primary RC, 57 patients with suspected LRRC were included, 33 of which were histologically confirmed. After manually segmenting suspected LRRC in CT and PET/CT, 144 Radiomic Features (RFs) were generated, and RFs were investigated for univariate significant discriminations (Wilcoxon rank-sum test, p < 0.050) of LRRC from NO LRRC. Five RFs in PET/CT (p < 0.017) and two in CT (p < 0.022) enabled, individually, a clear distinction of the groups, and one RF was shared by PET/CT and CT. As well as confirming the potential role of radiomics to advance LRRC diagnosis, the aforementioned shared RF describes LRRC as tissues having high local inhomogeneity due to the evolving tissue's properties. Pre-operative chemoradiotherapy (CRT) followed by surgical resection is still the standard treatment for locally advanced low rectal cancer. Nowadays new strategies are emerging to treat patients with a complete response to pre-operative treatment, rendering the optimal management still controversial and under debate. The primary aim of this study was to obtain a snapshot of tumor regression grade (TRG) distribution after standard CRT. Second, we aimed to identify a correlation between clinical tumor stage (cT) and TRG, and to define the accuracy of magnetic resonance imaging (MRI) in the restaging setting. Between January 2017 and June 2019, a cross sectional multicentric study was performed in 22 referral centers of colon-rectal surgery including all patients with cT3-4Nx/cTxN1-2 rectal cancer who underwent pre-operative CRT. Shapiro-Wilk test was used for continuous data. Categorical variables were compared with Chi-squared test or Fisher's exact test, where appropriate. Accuracy of restaging MRI in the identification of pathologic complete response (pCR) was determined evaluating the correspondence with the histopathological examination of surgical specimens. In the present study, 689 patients were enrolled. Complete tumor regression rate was 16.9%. The "watch and wait" strategy was applied in 4.3% of TRG4 patients. A clinical correlation between more advanced tumors and moderate to absent tumor regression was found (p = 0.03). Post-neoadjuvant MRI had low sensibility (55%) and high specificity (83%) with accuracy of 82.8% in identifying TRG4 and pCR. Our data provided a contemporary description of the effects of pre-operative CRT on a large pool of locally advanced low rectal cancer patients treated in different colon-rectal surgical centers.

Neuroendocrine tumours (NETs) of the midgut are often multifocal and have a noticeable attitude to metastasize to locoregional lymph nodes and liver. Surgery is the only curative treatment for metastatic NETs of the midgut, even though only a minority of patients are candidates to radical surgical resection. The optimal timing for surgical resection in case of synchronous presentation of primary intestinal neoplasms and resectable LM is still controversial, especially when LM are multiple and/or involve multiple liver segments. Even though a staged approach with initial intestinal resection followed by liver resection is still preferred, recent studies have shown favourable results for simultaneous procedures, which have the striking advantage of avoiding a second laparotomy, with morbidity and mortality rates comparable to staged resections. We report here the case of a patient with double midgut well-differentiated NET and thirty-two synchronous bilobar LM who received successful simultaneous curative right hemicolectomy and radical but conservative liver resection and radiofrequency thermal ablation with the guidance of intraoperative ultrasonography. He is alive without evidence of recurrence 48 months after surgery.I tumori neuroendocrini (NETs) dell’intestino (midgut) sono spesso multifocali ed hanno l’attitudine a dare metastasi ai linfonodi locoregionali ed al fegato. La chirurgia rappresenta attualmente l’unico trattamento ad intento curativo per i NETs dell’intestino con metastasi a distanza, sebbene solo una minoranza di pazienti possa essere di fatto candidata ad un intervento chirurgico ad intento curativo. La strategia chirurgica ottimale nel caso di una neoplasia dell’intestino con metastasi epatiche sincrone resecabili è tuttora controversa, specie nel caso in cui le metastasi epatiche siano multiple e/o coinvolgano multipli segmenti epatici. Sebbene nella maggioranza dei casi si preferisca procedere ad un intervento chirurgico in due tempi, di regola alla resezione intestinale seguita a distanza varabile di tempo dalla resezione epatica, alcuni recenti studi stanno ad indicare che un intervento simultaneo di resezione intestinale ed epatica è possibile in casi selezionati, con risultati clinici paragonabili a quelli descritti per gli interventi chirurgici in due tempi; il notevole vantaggio degli interventi simultanei è tuttavia rappresentato dalla possibilità di eseguire in un unico tempo un intervento potenzialmente radicale evitando una seconda laparotomia. Riportiamo il caso clinico di un paziente giunto alla nostra osservazione con una doppia neoplasia neuroendocrina dell’ileo terminale e del colon ascendente, rispettivamente, con multiple metastasi epatiche bilobari, sintomatica per la presenza di rush cutanei e diarrea. Una pancolonscopia aveva evidenziato un NET ben differenziato dell’intestino esprimente Sinaptofisina e Cromogranina A. La biopsia di una delle metastasi epatiche confermava che anche la neoplasia metastatica era ben differenziata. L’imaging preoperatrorio escludeva la presenza di metastasi extraepatiche. Il paziente veniva pertanto sottoposto ad un intervento chirurgico simultaneo di emicolectomia destra e resezione epatica ecoguidata “radicale ma conservativa”; nel complesso 32 metastasi epatiche bilobari venivano individuate e resecate in maniera radicale mediante l’impiego dell’ecografia intraoperatoria; un nodulo subcentimetrico posto nella profondità del parenchima epatico veniva sottoposto a termoablazione con radiofrequenza. La resezione epatica veniva condotta a termine con clampaggio intermittente dell’ilo epatico, della durata complessiva di 92 minuti. La durata complessiva dell’intervento chirurgico era di 680 minuti. Non si erano tuttavia rese necessarie emotrasfusioni intraoperatorie. Il paziente veniva dimesso in 15a giornata p.o. senza complicanze; il successivo decorso era complicato dalla comparsa, in 24a giornata postoperatoria, di una polmonite basale destra, regredita con terapia antibiotica. Successivamente il paziente veniva sottoposto a terapia con octreotide acetato (Sandostatina LAR). A distanza di 48 mesi dall’intervento chirurgico il paziente è in eccellenti condizioni cliniche e senza evidenza di recidive neoplastiche. Il caso descritto dimostra che, in casi selezionati di tumore neuroendocrino intestinale con metastasi limitate al fegato, una resezione simultanea intestinale ed epatica è possibile anche in presenza di metastasi epatiche multiple bilobari, a patto di eseguire una resezione epatica conservativa eco-guidata, ovvero con l’obiettivo di massimizzare il risparmio di parenchima epatico sano. Tale strategia chirurgica può consentire di ottenere risultati oncologici soddisfacenti anche a distanza.

The surgical management of locally recurrent rectal cancer is particularly challenging and it should be performed in specialist units characterized by multidisciplinary surgical team, appropriate expertise in anaesthesiology and intensive care and appropriate systems for rehabilitation. Although the mortality rate has been progressively reduced over the years, the complication rate is still not negligible. Patient selection is fundamental to prevent unnecessary morbidity in those whose survival is not likely to be significantly improved. Bleeding is the main intraoperative complication and it can be sudden, severe and life-threatening. Different strategies can be used to cope with massive intraoperative hemorrhage and postoperative complications.

- Abstract
AIM: The aim of this study was to evaluate the pathological response of locally advanced rectal cancer after preoperative concurrent two-drug chemotherapy and intensified radiation therapy (RT) with concomitant boost. PATIENTS AND METHODS: Patients with T4 tumor or local recurrence were included. A trial based on two-stage Simon's design was planned. RT was performed with 3D-conformal technique. The dose to the mesorectum and pelvic lymph nodes was 45 Gy (1.8 Gy/fraction). A concomitant boost was delivered to Gross Tumor Volume (GTV) 2 cm margin to a total dose of 55 Gy (2.2 Gy/fraction). The following concurrent chemotherapy was administered: Raltitrexed (3 mg/m(2)) and oxaliplatin (130 mg/m(2)) on days 1, 17, and 35 of RT. Pathological response was evaluated according to the Mandard classification. Toxicities were scored according to the Common Terminology Criteria for Adverse Events v3.0 scale. RESULTS: Eighteen patients (median age=64.5 years) were enrolled. The median follow-up was 22 months (range=2-36 months). After chemoradiation treatment, 16 patients underwent surgical resection (seven anterior resections and nine abdominal-perineal amputation); two patients did not undergo surgery due to early metastatic progression or refusal. R0 resection was achieved in all patients who underwent surgery. Five patients had pathological complete response [27.7%; 95% confidence interval (CI)=9.7-53.5%] and two patients showed only microscopic residual disease (11.1%; 95% CI=0.1-34.7%). Mandard grades 1 and 2 were detected in seven patients (38.9%; 95% CI=17.3-64.3%). Acute grade 3 or more toxicity was found in eight patients (44.4%; 95% CI=21.5-69.2%): one leucopenia-neutropenia, one liver, one skin and five cases of gastrointestinal toxicities. No patient had local tumor recurrence. One-, 2- and 3-year cumulative disease-free survival were 93.8%. One-, 2- and 3-year cumulative overall survival were 92.3%. Pelvic floor disorders (PFD), principally understood as pelvic organs prolapse (POP), faecal incontinence (FI) and urinary incontinence (UI), involve a group of pathologies which, even though benign, are extremely disabling. Moreover, their treatment is burdened by a significant percentage of functional failures since the physiopathology of these disorders is complex and still partially unknown. Therefore, it is important to evaluate the real impact of risk factors which, unlike age, ethnic group, pregnancy and delivery, can be modified. The relationship between the excess of fat mass and urinary disorders is that which has been the most investigated. In fact, studies have demonstrated how, with an increase in the body mass index (BMI), there is an increase in the prevalence and severity of UI. Furthermore, as a confirmation of the role of obesity, it has been demonstrated that weight loss determines a significant improvement and, in some cases, complete remission of the urinary symptomology. Instead, the relationship of obesity with FI and the morpho-functional alterations of the pelvic compartment have been studied less systematically. However, the prevalence of FI in obese patients seems to be greater than that in the general population, and weight loss, obtained after bariatric surgery, seems to result in improvement of FI and POP symptoms, as well as their impact on the Qol. Therefore, weight loss should be considered as the primary treatment for PFD in obese women.

46. Changes in Pelvic Floor Disorders After Gastric Bypass in Obese Women: Preliminary Results of a Prospective Study

Background This study was designed to assess the prevalence and severity of pelvic floor dysfunctions (PFDs) as well as their impact on quality of life in obese women before and after gastric bypass surgery. Methods 60 obese (BMI 30 kg/m2) females completed 6 validated specific and quality of life (QoL) questionnaires about PFDs. All patients were evaluated by pelvic floor physical examination, endoanal sonography and dynamic MRI. 29 patients were reassessed with the same questionnaires 6 months after bariatric surgery. Results Mean age was 42.5±10.8 yrs and mean BMI was 43.4±7.2 kg/m2. Prevalence of PFDs was 80.0% and 54.2% of patients reported that their symptoms adversely impacted QoL. Urinary incontinence (UI) was the most common disorder (63.3%), with linear association between BMI and UI (r2 =0.08; p=0.03). Prevalence of fecal incontinence (FI) was 23.3% and it concerned only loss of gas in 57.1% of cases. Urogenital prolapse and rectocele were documented in 68.3% and 78% of patients respectively. 44% of patients complained symptoms of obstructed defecation. With mean BMI reduction of 10 kg/m2, prevalence of PFDs decreased to 62.1% (p=0.04) with a significant improvement of QoL. Prevalence of UI decreased to 17.2% (p=0.003) and reduction was associated with postoperative BMI (p=0.04). On the whole 75% of patients who complained UI and 100% of patients with FI reported resolution of symptoms postoperatively. Conclusion In this sample of obese women, PFDs were more common than in the general population and adversely impacted QoL. A clear association was found between BMI and UI. Weight loss results in improvement of UI and FI.
