It is currently estimated that 80% of health data, such as that coming from medical transcripts, medical notes, lab results, email messages, attachments, is unstructured. Being unstructured, this information cannot be processed through automated PC procedures but requires the interpretation of human beings. We propose a cognitive computing system to extract automatically meaningful health information from the textual documents of the patient folder and merge this information into a structured data frame. The system was tested on the medical documents generated in an audiological outpatient hospital service; the data corpus consisted of the documents and reports generated longitudinally from the enrollment visit to the last available follow-up of hearing impaired aged patients treated with cochlear implants (CI). The system is based on an Information Extraction (IE) module to extract meaningful health information, a couple of ontologies to interpret the meaning and classify the extracted information into a logical hierarchy and an ad hoc developed structured data frame to gather the information. The system was designed to be compliant with the clinical best practices of the audiological/ENT (Ear-Nose-Throat) medical domains to ensure its ease of use in the real practice. The performance was assessed by measuring the percentage of information correctly extracted by the system against the one manual extracted by two experts. The accuracy of the system was very good (recall= 0.78; precision=3D0.95).

Scopo dello studio: La riabilitazione uditiva del paziente anziano presenta diverse criticità e deve tener conto di aspetti legati sia alla tecnologia che alla disabilità percepita. L'audiologia dispone di validi strumenti per la valutazione del quadro clinico globale del paziente con disabilità uditiva. Sfortunatamente, però, il personale medico non sempre riesce a far fruttare nella sua interezza la mole di informazioni teoricamente disponibili poiché, nella pratica, tali informazioni sono spesso frammentate in documenti e sorgenti diverse, sono parzialmente mancanti o contrastanti tra di loro. Una soluzione a questo problema è l'uso di sistemi computerizzati a supporto del clinico. Purtroppo, nonostante la crescente spinta verso la digitalizzazione nella sanità, attualmente circa l'80% dei dati medici è del tutto invisibile ai sistemi computerizzati poiché tali dati sono in un formato non strutturato; tra questi si citano dati e informazioni provenienti da referti e trascrizioni mediche scritte in linguaggio naturale, risultati di test clinici e anche messaggi ed allegati email. La rilevanza dei dati non strutturati nella pratica clinica è importante, poiché essi contengono informazioni relative allo stato di salute del paziente. Parecchie informazioni mediche, inoltre, devono comunque essere redatte in forma non strutturata poiché in alcune circostanze solo l'uso del linguaggio naturale consente la descrizione fedele, precisa e accurata del caso clinico. Questo è il primo lavoro scientifico nel dominio della audiologia in cui si è sviluppato un sistema intelligente di facile ed immediato utilizzo nella pratica clinica audiologica, basato sul 'cognitive computing', in grado di 'interpretare' testi non strutturati di rilevanza medica nella cartella clinica del paziente (quali i referti), di estrarre da essi dati e informazioni rilevanti per la riabilitazione uditiva del paziente e di dare un supporto per la individuazione del percorso riabilitativo più opportuno. Materiali e Metodi: La prima valutazione pilota del sistema è stata fatta su pazienti anziani (N=52) con disabilità uditive sottoposti ad impianto cocleare. Sono state elaborate le informazioni estratte ed opportunamente interpretate dal sistema di 'cognitive computing' sui dati e documenti non strutturati delle cartelle cliniche ambulatoriali contenenti la documentazione generata alla visita di accettazione e successivamente alle visite di follow-up post-impianto. I documenti elaborati includevano referti medici, risultati alle valutazioni audiometriche e psicoacustiche, schede tecniche con il set up degli impianti, risultati ai questionari di auto-valutazione e valutazioni logopediche. Risultati: Grazie al 'cognitive computing', il sistema è stato in grado di estrapolare dai documenti non strutturati le informazioni sia testuali che numeriche relative alla anamnesi medica, alla patologia attuale, alla diagnosi, ai possibili fattori di rischio per la sordità riscontrati, alla valutazione audiologica e logopedica, alla strategia di amplificazione in uso e alla valutazione delle capacità comunicative pre- e post-impianto. Tutte le informazioni così estratte sono state archiviate e poste in successione cronologica adeguata in un'unica piattaforma detta 'patient summary'. Discussione e Conclusioni: Il sistema ha fornito al personale medico un'unica piattaforma in cui convergono tutte le informazioni relative al singolo paziente in maniera coerente e strutturata, a partire da documenti originariamente redatti in linguaggio naturale. Tale sistema fornisce il quadro d'insieme dello stato del paziente, permette di monitorare i progressi nelle sue capacità comunicative e offre un valido aiuto nella definizione del miglior percorso di amplificazione del paziente. SUMMARY The need to optimize the use of all the information that modern technological tools have made available to the physician ENT/audiologist has increasingly emerged within the Italian scientific community. Towards this purpose, it is necessary to create a registry of the patients using cochlear implants (CIs). This registry will include a homogeneous summary of the information deriving from multiple sources related to daily clinical practice, in order to assess auditory benefits, safety and reliability in patients with cochlear implants, and organization over the national territory. The primary objectives relative to the above-mentioned analysis are to assess the impact of the use of cochlear implants on patient health, to ensure traceability of the devices currently used, monitoring their safety and reliability over time, to guarantee access of the technique in clinical and organizational conditions that can allow the best possible benefits. The aspects concerning implementation of the registry were discussed extensively during the first meetings of the Working Group (WG). In particular, owing to the complexity and high costs related mainly to the development of the technological aspects and the need to involve technological partners external to the WG, and to respect current privacy laws, the WG members decided that the project should be limited to proposal of a paper registry to be implemented at a later stage, possibly within the framework of successive research projects. During meetings, the WG members discussed various aspects of implementation of the registry, and in particular the scientific features connected to objectives, inclusion criteria, and structure of the forms needed for data collection and organizational aspects. A registry is proposed herein.

This is a case of successful cochlear implantation in a 50-year-old man who experienced sudden hearing loss and developed ipsilateral severe tinnitus at three years following conservative stage 1 vestibular schwannoma retrosigmoid surgery. After cochlear implantation, tinnitus improved from THI grade 4 to 2. Localisation skills improved. Hearing in noise (S/N + 7 dB) with target signal from the operated side improved from 38 to 100% of correct answers. A significant improvement of spatial and speech items of the "speech, spatial and qualities of sounds" questionnaire was also measured. In conclusion, cochlear implantation is a feasible and effective solution after conservative vestibular schwannoma surgery should delayed hearing loss occur.Presentiamo un caso di posizionamento di impianto cocleare in un uomo di 50 anni che, dopo aver subito tre anni prima un intervento per via retrosigmoidea per un neurinoma dell’acustico in stadio 1, è stato colpito da ipoacusia improvvisa ipsilaterale sviluppando un intenso acufene. Dopo l’attivazione dell’impianto l’acufene è sceso da un grado 4 ad un grado 2 secondo il THI. Sono migliorate le capacità di localizzazione. L’ascolto nel rumore (S/R + 7 dB) con il segnale proveniente dal lato operato è migliorato da 38 a 100%. Abbiamo inoltre verificato un significativo incremento dei punteggi relativi a spazialità e linguaggio del questionario “Speech, spatial and qualities of sounds”. In conclusione l’impianto cocleare è una soluzione praticabile ed efficace in caso di insorgenza di ipoacusia ritardata dopo chirurgia conservativa del neurinoma dell’acustico.

The association between hearing impairment, the diagnosis of dementia, and the role of sensory therapy has been proposed for some time, but further research is needed. Current understanding of this association requires the commitment of those experts who can integrate experience and research from several fields to be able to understand the link from hearing to dementia. A workshop whose panelists included experts from many areas, ranging from ear, nose and throat (ENT) to dementia's specialists, was promoted and organized by the Giovanni Lorenzini Medical Science Foundation (Milan, Italy; Houston, TX, USA) to increase the awareness of the relationship between hearing loss and dementia, and included questions and comments following a presentation from the clinical researcher, Frank Lin, who has been evaluating the relationship between hearing loss and cognitive decline since 2009.

We report a retrospective case of inner magnet migration, which occurred after 1.5 Tesla MRI scanning in an adult recipient of a bilateral cochlear implant (CI) despite a focused head dressing. The patient, bilaterally implanted with Nucleus 5 CIs (Cochlear LTD, Sydney, Australia), underwent a 1.5 Tesla cholangio-MRI scan for biliary duct pathology. In subsequent days, a focal skin alteration appeared over the left inner coil. Plain skull radiographs showed partial magnet migration on the left side. Surgical exploration confirmed magnet twisting; the magnet was effectively repositioned. Left CI performance was restored to pre-migration level. The wound healed without complications. Thus, focused dressing does not prevent magnet migration in CI recipients undergoing 1.5 Tesla MRI. All patients should be counselled on this potential complication. A minor surgical procedure is required to reposition the magnet. Nevertheless, timely diagnosis is necessary to prevent skin breakdown and subsequent device contamination. Plain skull radiograph is very effective in identifying magnet twisting; it should be performed systematically after MRI or minimally on all suspected cases.Segnaliamo il caso di un soggetto adulto con parziale migrazione del magnete dopo esecuzione di Risonanza Magnetica nucleare (RMN) a 1,5 Tesla, in un paziente adulto con impianto cocleare bilaterale nonostante le misure precauzionali adottate, quali la fasciatura compressiva in corrispondenza del magnete. Il paziente, portatore di impianto cocleare Cochlear Nucleus serie 5 (Cochlear LTD, Sydney, Australia) bilaterale, è stato sottoposto a colangio-RMN con apparecchiatura da 1,5 Tesla, poiché affetto da epatocoledocolitiasi. Nei giorni seguenti è comparsa una lesione cutanea in corrispondenza dell'antenna a sinistra. La radiografia del cranio ha evidenziato una parziale migrazione del magnete. L'esplorazione chirurgica ha confermato tale reperto e il magnete è stato correttamente riposizionato nella sua sede abituale. Le abilità percettivo-uditive del paziente con l'impianto cocleare di sinistra erano sovrapponibili a quelle pre-operatorie. Non vi sono state complicanze post-operatorie. In conclusione, la medicazione compressiva in corrispondenza dell'antenna non previene la possibile migrazione di magnete in pazienti sottoposti a RMN a 1,5 Tesla. Tutti i pazienti dovrebbero essere informati sui rischi e le misure da adottare ogni qualvolta si debba eseguire un esame di risonanza magnetica nucleare, anche in presenza di device compatibili con l'esecuzione della stessa. Il riposizionamento del magnete richiede un intervento chirurgico, pur trattandosi di una complicanza minore. È necessaria una tempestiva diagnosi della migrazione del magnete per evitare complicanze cutanee e la successiva contaminazione del ricevitore-stimolatore. La radiografia del cranio consente una corretta diagnosi di migrazione del magnete e dovrebbe essere eseguita dopo RMN o almeno nei casi sospetti.

Although still widely implanted, Pulsar© Med-El is rarely considered for small incision approach. Overall, 30 teen-age and adult patients were operated upon with a novel small incision (4-5 cm). Full insertion of the electrode array was achieved in all cases. No major intra-operative complications occurred. At follow-up, no flap-related complications and no migration of the receiver-stimulator were observed in the “device suture” (14 patients) or “no device suture” groups (16 patients). All patients are full-time users of the device. In conclusion, a small incision for the Pulsar© Med-El cochlear implant is feasible, safe and reproducible. Ligature fixation of the device is not critical with this operation. Also with this device, in adult and teen-age patients, it is, therefore, possible to retain several typical advantages of small incision approaches.

Future Internet packet switches and routers will have to overcome intrinsic limitations of current electronic switching fabrics, related to the extremely high amount and density of the information to be internally processed, and the consequent very large number of interconnections, and very high power consumption and dissipation. Motivated by this observation, we consider different architectures of optical switching fabrics, capable to interconnect the linecards of a packet switch with an overall switching capacity in the order of Tb/s, using both Wavelength Division Multiplexing (WDM) and space diversity. The physical-layer feasibility and the cost of the considered architectures are studied and compared using realistic models taken from commercially available optoelectronic devices.
