Insulin is a hormone responsible for breaking down glucose molecules in the blood and converting them into energy to keep the body in homeostasis. In this way, when it does not work properly, it can cause serious damage to health such as cardiovascular complications, damage to the kidneys and nervous system and in more serious cases it can be fatal. Based on this principle, the most frequent complications triggered by this lack of insulin control are hyperglycemia, hypoglycemia and the Hyperosmolar Hyperglycemic State. These complications are frequent practices present in the professional routine of Emergencies. Therefore, this study is justified by its high social and academic relevance, since diabetes is one of the chronic diseases that most affects the population, causing dysfunctions in the body and culminating in the development of diseases. Therefore, the objective of this research is to show through the scientific literature, which are the most frequent glycemic complications regarding emergency care. The construction of this research proceeded through an integrative literature review, through searches in LILACS and SCIELO databases. Therefore, it is evident that health education practices aim to promote self-care and prepare the return of diabetics, restore and promote health and prevent complications. La insulina es una hormona responsable de descomponer las moléculas de glucosa en la sangre y convertirlas en energía para mantener el cuerpo en homeostasis. De esta forma, cuando no funciona correctamente, puede causar graves daños a la salud como complicaciones cardiovasculares, daño a los riñones y al sistema nervioso y en casos más graves puede llegar a ser fatal. En base a este principio, las complicaciones más frecuentes desencadenadas por este descontrol de la insulina son la hiperglucemia, la hipoglucemia y el estado hiperglucémico hiperosmolar. Estas complicaciones son prácticas frecuentes presentes en la rutina profesional de Emergencias. Por tanto, este estudio se justifica por su alta relevancia social y académica, ya que la diabetes es una de las enfermedades crónicas que más afecta a la población, provocando disfunciones en el organismo y culminando en el desarrollo de enfermedades. Por lo tanto, el objetivo de esta investigación es mostrar a través de la literatura científica cuáles son las complicaciones glucémicas más frecuentes en la atención de urgencias. La construcción de esta investigación procedió a través de una revisión integrativa de la literatura, a través de búsquedas en las bases de datos LILACS y SCIELO. Por lo tanto, es evidente que las prácticas de educación en salud tienen como objetivo promover el autocuidado y preparar el retorno de los diabéticos, restablecer y promover la salud y prevenir complicaciones. Dessa forma, quando não funciona corretamente, pode ocasionar sérios danos à saúde como complicações cardiovasculares, danos aos rins e ao sistema nervoso e em casos mais graves pode ser fatal. Partindo deste princípio, as complicações mais frequentes desencadeadas por essa falta de controle insulínico são as hiperglicemias, hipoglicemia e o Estado Hiperglicêmico Hiperosmolar. Estas complicações são práticas frequentes presentes na rotina profissional dos Emergencistas. Portanto, a realização deste estudo, justifica-se pela sua elevada relevância social e acadêmica, visto que, a diabetes é uma das doenças crônicas que mais acomete a população, provocando disfunções no organismo e culminando no desenvolvimento de agravos. Diante disso, o objetivo desta pesquisa incide em evidenciar por meio da literatura científica, quais são as complicações glicêmicas mais frequentes no que tange os cuidados de emergência. A construção desta pesquisa procedeu-se através de uma revisão integrativa da literatura, por intermédio de buscas nas bases de dados. Portanto evidencia-se que as práticas de educação em saúde visam promover o autocuidado e preparar o retorno aos diabéticos, restaurar e promover a saúde e prevenir complicações.

A doença de Parkinson (DP) classifica-se como uma enfermidade neurodegenerativa, progressiva, que acomete, predominantemente, pacientes acima dos 50 anos e possui maior predileção pelo sexo masculino. Caracteriza-se, clinicamente, por disfunções motoras, posturais e, também, alterações cognitivo-emocionais significativas. Neste contexto, este estudo tem como objetivo correlacionar os domínios da qualidade de vida com a percepção da qualidade de vida total de indivíduos com DP. Participaram deste estudo 22 indivíduos com diagnóstico de doença de Parkinson, classificados entre os estágios um a cinco da Escala de Estadiamento Hoen e Yahr, tendo a sua qualidade de vida avaliada através do questionário Parkinson Disease Questionary-39(PDQ39). Dos 22 indivíduos (12 homens e 10 mulheres) apresentaram idade média de 70,82, variando entre 53 a 91 anos, e tempo de evolução da doença de 7,82±5,85, variando entre 1 e 20 anos de doença. Observou- se que as piores percepções sobre a qualidade de vida estão relacionadas ao domínio mobilidade, com média de 55,67% de comprometimento e ao domínio bem-estar emocional, com 47,16%. Introduction: The elderly patient with cancer has potential risk of polymedication due to the increase of comorbidity and complexity of the cancer treatment, which often includes multiple antineoplastic and supportive medications. Objective: The objective is to identify the frequency of polypharmacy, potentially inappropriate medication use and drug interaction in elderly patients with cancer. Method: It were collected sociodemographic, pharmacological and clinical information from 20 elderly patients (≥ 65 years) with cancer, polymedicated (≥five drugs), from July to August 2016, in an outpatient clinic in Sao Luis, MA through electronic chart and questionnaires applied in interviews. To analyze the data, it were used the: Anatomical Therapeutic Chemical (ATC), Micromedex®, Drugs®, Lexicomp® and the Anfarmag Phytotherapy Guideline. Beers Criteria 2015 was used to identify potentially inappropriate medication for the elderly. Results: Among the patients, 70% were males, with a mean age of 73 years (SD=7.9). The most prevalent types of cancer were prostate cancer and breast. Cardiovascular and endocrine comorbidities were the most reported. A total of 134 drugs prescribed and self-medication were identified, of which 41 were antineoplastic. Eighty percent of the patients were exposed to potential risk of drug interaction, totaling 90 (63.2% of moderate, 21.2% of significant and 8.8% of mild severity). There were 4 interactions involving medicinal plants and seven inappropriate medications in use by 20% e 65% of the elderly patients, respectively. Conclusion: Among the patients included in this study, it was verified the occurrence of polypharmacy, potential drug interactions and the use of potentially inappropriate medications for the elderly. Introducción: Un paciente con cáncer de edad avanzada tiene riesgo potencial de polmedicación debido a mayor comorbilidad y complejidad del tratamiento del cáncer, que incluye múltiples medicamentos antineoplásicos y de soporte. Objectivo: Identificar la frecuencia de polifarmacia, de uso de medicamentos potencialmente inapropiados y interacciones medicamentosa en personas mayores con cancro. Método: En el periodo de julio a agosto de 2016, en ambulatorio en Sao Luis – MA, se recolecto informaciones sociodemográficas, farmacológicas y clínicas de 20 pacientes ancianos (≥ 65 anos) cancro, polimedicados (≥ cinco medicamentos), por medio de recolección en registro medico electrónico y cuestionarios. Para análisis de datos, se utilizaron herramientas como Clasificación Anatómica Terapéutica Química, Micromedex®, Drugs®, Lexicomp® y Manual de fitoterápicos de la Anfarmag. Beers Criteria 2015 se utilizo para identificar los medicamentos potencialmente inapropiados para ancianos. Resultados: Entre los pacientes, 70% eran del sexo masculino, con edad promedio de 73 anos (DP=7,9). Los tipos de cancro mas prevalentes fueron de próstata y mama. En cuanto a las comorbilidades, las cardiovasculares y endocrinas fueron las mas relatadas. Se identificaron 134 prescripciones y automedicación, de los cuales 41 antineoplásicos. 80% de pacientes estaban expuestos a interacción medicamentosa, totalizando 90 (63,2% de gravedad moderada, 21,2% importantes y 8,8% leves). Se detectaron 4 interacciones que involucra plantas medicinales y siete medicamentos inapropriados para ancianos en el 20% y 65% de los pacientes, respectivamente. Conclusión: Entre los pacientes incluidos en estudio, verificamos aparición de polifarmacia, riesgos de posibles interacciones medicamentosas y el uso de medicamentos potencialmente inapropiados para los ancianos. Introdução: O paciente idoso com câncer possui risco potencial de polimedicação pelo aumento de comorbidade e complexidade do tratamento do câncer que, muitas vezes, inclui múltiplos antineoplásicos e medicamentos de suporte. Objetivo: Identificar a frequência de polimedicação, o uso de medicamentos potencialmente inapropriados e a interação medicamentosa em pacientes idosos com câncer. Behçet's disease is a rare form of systemic vasculitis that affects small to large vessels. It is characterized by mucocutaneous, pulmonary, cardiovascular, gastrointestinal, and neurological manifestations. Its clinical presentation is quite wide, ranging from milder cases to severe cases, with multisystemic involvement, characteristically with exacerbations and remissions. Its etiopathogenesis is still unclear, although there is evidence of genetic, environmental, and immunological factors, such as the association with the HLA-B51 allele. In conjunction, all of these point to an abnormal immunopathological process, with activation of cells of innate and adaptive immunity, such as NK cells, neutrophils, and T cells, which generate specific response patterns and cytokines capable of generating mediators that can damage and inflame blood vessels, resulting in venous and arterial occlusions and/or aneurysm formation. 17. Aspectos epidemiológicos da tuberculose multirresistente em serviço de referência na cidade de São Paulo

Com o objetivo de estudar algumas características epidemiológicas dos portadores de tuberculose pulmonar multirresistente e suas influências sobre o controle e o tratamento, foi avaliada uma coorte de 4 anos de pacientes selecionados pela recuperação do Mycobacterium tuberculosis no escarro, resistência à rifampicina, isoniazida e mais uma terceira droga usual ou falência do esquema de reserva, matriculados em uma referência na cidade de São Paulo. As variáveis estudadas foram: sexo e idade, tipo de multirresistência, contágio, condições associadas, perfil de resistência às drogas usuais e distribuição das lesões na radiologia convencional. Revistos 182 pacientes, 112 (61,5%) masculinos, com idade variando entre 16 e 64 anos (35,7±6,8). Com base na história terapêutica foram discriminados os seguintes tipos: MR-primária (com teste de sensibilidade inicial), 11 (6%), MR-pós-primária (irregularidade no tratamento anterior), 134 (74%) e MR-indeterminada (falência após uso regular informado dos esquemas usuais), 37 (20%). Contágio presente em 41 de 170 pacientes, predominando o intradomiciliar sobre o institucional. Identificados 4 surtos familiares e nenhum institucional. O abandono (45%) foi a mais freqüente condição associada, seguido do etilismo (27%), falência seqüencial aos esquemas de retratamento (23%), contágio com multirresistência (15%), reações adversas às drogas (6%), HIV-positivo (4%) e diabetes (3%). Resistência à rifampiina+isoniazida em 100%, 83% à estreptomicina e 47% ao etambutol. Todos com cavidades no Rx de tórax convencional, unilaterais em 35 (19%). Discutem-se os achados e apresentam-se sugestões. In order to study certain epidemiological features of multidrug-resistant tuberculosis (MDR-TB) carriers and their influence on the control and treatment, a group of patients was evaluated over a four-year period, selected by: Mycobacterium tuberculosis isolation from sputum; resistance to Rifampin, Isoniazid and one more drug, or, failure of reserve regimen, all cases were from a tuberculosis reference unit in the City of São Paulo. A total of 182 patients were reviewed, with a mean age of 35.7±6.8 years and 112 (61.5%) were male. These patients was classified according to therapeutic history, as: primary MDR-TB (with initial sensitivity test) 11 (6%); post primary MDR-TB (after irregular use previous treatment) 134 (74%), and indeterminate MDR-TB (failure after regular use of initial and reserve regimens) 37 (20%). Contagion was identified in 41/170 patients, acquired through domiciliary rather than institutional transmission. There were four familial outbreaks and none were institutional. The most frequent condition associated with these cases was abandonment of therapy (45%) followed by alcoholism (27%), sequential failure in the treatment regimens (23%), MDR contagion (15%), drug reaction (6%), HIV positive (4%) and diabetes (3%). There was resistance to Rifampin+Isoniazid in 100%, Streptomycin in 83% and Ethambutol in 47%. Conventional X-ray revealed cavities in all, though only 35 (19%) were unilateral. These cases are discussed and some suggestions presented.

Therefore, the contact between languages becomes a natural imperative. The relations established in the frontier space are conditioned by "a complex set of social correlations" MOLLICA (2015), which act as tools of linguistic variation that, in principle, modify the tabatinguense vernacular. It is in this perspective that the variant sociolinguistics LABOV (2008), and the dialectic CARDOSO (2010), CHAMBERS and TRUDGILL (1998) open the field of observation and analysis, since the variation occurs by a set of phenomena not strictly linguistic, but also extralinguistic. The research was carried out according to the theoretical-methodological principles of multidimensional geolinguistics (THUN, 1998), which through the synchronic clipping contemplates the spatial differences concomitant with social variables. As an initial hypothesis of our study, we consider that Portuguese is predominant in the city, but the contact with the Spanish language is offering variation to the speech of the inhabitants of the city through the diffusion of the Hispanic lexias. In this sense, our objective was to describe the sociodialetal particularities of the Tabatinga speaker. To do so, we use linguistic charts to demonstrate how the lexicon of the city of Tabatinga has been influenced by the Spanish language. We carried out the research in 4 points of the city, that comprise the districts Santa Rosa, San Francisco, Vila Para??so and Comara. Our research contemplated the study of the variation in the diatopic, diastetic, diageneric and diagenerational levels. Eight informants were interviewed per point of inquiry in a total of 32 interviews. Our questionnaire consisted of 106 questions from the most varied semantic fields. As a result, we could verify that the city presents with markedly heterogeneous linguistic characteristics, since, the Hispanic lexias were observed in several degrees of frequency, even with arealizations. We could also verify how the different social strata shape their verbal repertoire in front of the Portuguese-Spanish contact. Como hip??tese inicial do nosso estudo, consideramos que o portugu??s ?? predominante na cidade, mas o contato com a l??ngua espanhola est?? ofertando varia????o ?? fala dos moradores da cidade por meio da difus??o das lexias hisp??nicas. Nesse sentido, nosso objetivo foi descrever as particularidades sociodialetal do falante de Tabatinga. Para tanto, utilizamos as cartas lingu??sticos a fim de demonstrar como o l??xico da cidade de Tabatinga tem sido influenciado pela l??ngua espanhola. Realizamos a pesquisa em 4 pontos da cidade, que compreendem os bairros de Santa Rosa, S??o Francisco, Vila Para??so e Comara. Nossa investiga????o contemplou os n??veis diat??picos, diastr??ticos, diagen??ricos e diageracional. Foram entrevistados 8 informantes por ponto de inqu??rito no total de 32 entrevistas e o question??rio foi constitu??do por 106 quest??es dos mais variados campos sem??nticos. Como resultado, pudemos constatar que a Tabatinga apresenta carater??sticas lingu??sticas marcadamente heterog??nea, uma vez que, as lexias hisp??nicas foram observadas em diversos graus de frequ??ncia, inclusive com arealiza????es. Tamb??m pudemos verificar como os diferentes estratos sociais moldam seu repert??rio verbal diante do contato portugu??s-espanhol.

23. Vascular involvement in Behçet's disease: the immunopathological process.

- Abstract
Behçet's disease is a rare form of systemic vasculitis that affects small to large vessels. It is characterized by mucocutaneous, pulmonary, cardiovascular, gastrointestinal, and neurological manifestations. Its clinical presentation is quite wide, ranging from milder cases to severe cases, with multisystemic involvement, characteristically with exacerbations and remissions. Its etiopathogenesis is still unclear, although there is evidence of genetic, environmental, and immunological factors, such as the association with the HLA-B51 allele. In conjunction, all of these point to an abnormal immunopathological process, with activation of cells of innate and adaptive immunity, such as NK cells, neutrophils, and T cells, which generate specific response patterns and cytokines capable of generating mediators that can damage and inflame blood vessels, resulting in venous and arterial occlusions and/or aneurysm formation., Competing Interests: Conflicts of interest: No conflicts of interest declared concerning the publication of this article., (Copyright© 2021 The authors.)

24. The efficacy of 2 different doses of dexamethasone to control postoperative swelling, trismus, and pain after third molar extractions.

- Abstract
This article addresses the effect of 2 different concentrations (4 and 12 mg) of dexamethasone to control pain, swelling, and trismus after third molar surgery. A clinical study was conducted with 27 male and female patients, all presenting with bilaterally displaced mandibular third molars. The treatment protocol required a surgical removal of each tooth in 2 separate operations. The patients were given a preoperative dose of dexamethasone-4 mg for one surgery, 12 mg for the other. The choice of which side would be operated on first and which dose of dexamethasone would be taken was performed randomly, under double-blind conditions. The trismus was assessed by measuring the interincisal distance. Pain intensity was measured both by the amount of painkillers (acetaminophen 750 mg) taken postsurgery and by the Visual Analogue Pain Scale. Data were collected 1 hour preoperative, then at 24 and 48 hours postoperative. A statistical analysis (student's t, Wilcoxon and Friedman tests) of the results showed no significant differences (α = 0.05) between the analyzed variables for the 2 doses of dexamethasone (4 and 12 mg).

- Abstract
In order to study certain epidemiological features of multidrug-resistant tuberculosis (MDR-TB) carriers and their influence on the control and treatment, a group of patients was evaluated over a four-year period, selected by: Mycobacterium tuberculosis isolation from sputum; resistance to Rifampin, Isoniazid and one more drug, or, failure of reserve regimen, all cases were from a tuberculosis reference unit in the City of S o Paulo. A total of 182 patients were reviewed, with a mean age of 35.7 +/- 6.8 years and 112 (61.5%) were male. These patients was classified according to therapeutic history, as: primary MDR-TB (with initial sensitivity test) 11 (6%); post primary MDR-TB (after irregular use previous treatment) 134 (74%), and indeterminate MDR-TB (failure after regular use of initial and reserve regimens) 37 (20%). Contagion was identified in 41/170 patients, acquired through domiciliary rather than institutional transmission. There were four familial outbreaks and none were institutional. The most frequent condition associated with these cases was abandonment of therapy (45%) followed by alcoholism (27%), sequential failure in the treatment regimens (23%), MDR contagion (15%), drug reaction (6%), HIV positive (4%) and diabetes (3%). There was resistance to Rifampin+Isoniazid in 100%, Streptomycin in 83% and Ethambutol in 47%. Conventional X-ray revealed cavities in all, though only 35 (19%) were unilateral. These cases are discussed and some suggestions presented.
