Abstract

Ongoing research and the use of modern technology have shed new light on the Uncoverings 1982 paper 'The Quilts of Grant Wood's Family and Paintings.' This work has yielded a more complete genealogy; updated quilt descriptions and current repositories; and leads to recent scholarship on the artist, his works, and the quilts. The author's interest in Grant Wood and the family quilts began with a 1981 visit to the Iowa City home of Ed Green to view the historic quilts fr om Grant's family. Green was a friend and assistant to Grant and his sister, Nan Wood Graham. Green was seeking advice on a future repository for the quilts. He provided Nan's contact information. Although the family quilts were not depicted in his paintings, Grant would have known about them. Grant, Nan, and their mother, Hattie, lived and worked together, living their Quaker heritage through their mutual support and respect for Grant's creative problem-solving ability., Editor's Note: All research involves continual growth and study. Abstract

Purpose Constipation can be a significant problem in critically unwell patients, associated with detrimental outcomes. Opioids are thought to contribute to the mechanism of bowel dysfunction. We tested if methylnaltrexone, a pure peripheral mu-opioid receptor antagonist, could reverse opioid-induced constipation. Methods The MOTION trial is a multi-centre, double blind, randomised placebo-controlled trial to investigate whether methylnaltrexone alleviates opioid-induced constipation (OIC) in critical care patients. Eligibility criteria included adult ICU patients who were mechanically ventilated, receiving opioids and were constipated (had not opened bowels for a minimum 48 h) despite prior administration of regular laxatives as per local bowel management protocol. The primary outcome was time to significant rescue-free laxation. Secondary outcomes included gastric residual volume, tolerance of enteral feeds, requirement for rescue laxatives, requirement for prokinetics, average number of bowel movements per day, escalation of opioid dose due to antagonism/reversal of analgesia, incidence of ventilator-associated pneumonia, incidence of diarrhoea and Clostridium difficile infection and finally 28 day, ICU and hospital mortality. Results A total of 84 patients were enrolled and randomized (41 to methylnaltrexone and 43 to placebo). The baseline demographic characteristics of the two groups were generally well balanced. There was no significant difference in time to rescue-free laxation between the groups (Hazard ratio 1.42, 95% CI 0.82-2.46, p = 0.22). There were no significant differences in the majority of secondary outcomes, particularly days 1-3. However, during days 4-28, there were fewer median number of bowel movements per day in the methylnaltrexone group, (p = 0.01) and a greater incidence of diarrhoea in the placebo group (p = 0.02). There was a marked difference in mortality between the groups, with ten deaths in the methylnaltrexone group and two in the placebo group during days 4-28 (p = 0.007). Conclusion We found no evidence to support the addition of methylnaltrexone to regular laxatives for the treatment of opioid-induced constipation in critically ill patients; however, the confidence interval was wide and a clinically important difference cannot be excluded., Author(s): Parind B. Abstract

The Solar System Quilt made in the 1870s-1880s by Ellen Harding Baker has drawn attention in recent years from quilt artists, historians, academic scholars, and the general public. Reasons for this include its unique subject matter; connection to changing roles and educational opportunities for women; availability on the Internet; and presence as part of the quilt collection in our nations attic, the Smithsonian National Museum of American History. Much of the available information, however, is incomplete, and generally based on insufficient examination of the most appropriate evidence. Research into period resources available in Iowa archives yields a more complete account of the Solar System Quilt and its artist, Ellen Harding Baker (1847-1886). This study looks for evidence of significant events in Ellen's life (including lectures, public acknowledgements, and circumstances of her lifestyle) and possible impacts on her by the changing role of women in nineteenth-century United States. 16. Self Efficacy and Cultural Awareness: A Study of Returned Peace Corps Teachers.

Abstract

This study investigated how returned Peace Corps teachers viewed the Peace Corps experience and its impact on their self-efficacy and cultural awareness. Participants were 154 returned Peace Corps teachers who completed a questionnaire that contained demographic questions, a self-efficacy scale, and a teacher-efficacy scale. A subset of 15 teachers were interviewed to examine cultural awareness, self-efficacy, teacher-efficacy, and the importance of the Peace Corps experience. A group of 6 teachers out of the 15 were observed in their classrooms. The observation focused on practices that were associated with high self-efficacy teachers and effective multicultural teachers. Data analysis indicated teachers ranked the Peace Corps experience highly in its effect on their self-efficacy and cultural awareness, and less highly in its effect on their teaching. The results indicate that the Peace Corps functioned as a mastery experience. The effect was robust and persistent. Returned volunteers attributed many changes to the Peace Corps, even if the experience had been more than 30 years ago. Teachers ranked the Peace Corps experience as very important in affecting their personality and somewhat important in affecting their teaching. The questionnaire is appended. (SM)

33. Mindfulness and Hand Hygiene to Improve Burnout, Compassion Satisfaction, and Secondary Traumatic Stress in Interdisciplinary Teams: A Quality Improvement Project.

Abstract

Background: Burnout (BO) and healthcare-associated infections (HAIs) are both linked to high-cost burdens in the United States. The literature supports that BO reduction programs addressing chronic stress can reduce the retraining costs of nurses and healthcare workers. Mindful-ness has been shown in the literature to improve compassion satisfaction, focus, creativity, and work environment and decrease work-related stress. Improving compassion satisfaction in nurses and healthcare teams can prevent compassion fatigue, which can cause symptoms of irritability, and can lead to burnout. Proper hand hygiene can reduce the high costs associated with healthcare-associated infections (HAIs). Local problem: Baseline data at a metropolitan university-based clinic showed a moderate level of secondary traumatic stress (STS), BO and compassion satisfaction (CS), and hand hygiene compliance of 55%. Methods and Interventions: An interprofessional care team N = 31 in a quality improvement project practiced box breathing with positive self-messaging during routine hand hygiene for 20 seconds. Following an education intervention on mindfulness and hand hygiene for staff, measurement of mindfulness and hand hygiene participation was collected weekly and reported bi-monthly for six months. Direct observation and self-report checklists were utilized with attention to quality and rates of hand hygiene and a pause for a deep breath for mindfulness. Self-reported levels of BO, STS, and CS were evaluated pre-and post-intervention with Professional Quality of Life (ProQOL 5) surveys. Jean Watson's Theory of Human Caring was chosen as the theoretical framework. This framework was chosen because of the focus on self-care and care for others. Self-care can be effective in increasing CS and preventing BO Results: Improvement was identified in CS from 4 to 5 (high) on the ProQOL 5 (1-5 Likert scale) p = .017, n = 8. There was no change in BO or STS from 3 (moderate) on the Likert scale. A process measure demonstrated a strong willingness of the providers and staff to implement box breathing during hand hygiene throughout their day. A goal of mindfulness participation of 95% was achieved and hand hygiene compliance improved by 30-45% without negatively impacting clinic patient volumes. Implications: Mindfulness during hand hygiene is a cost-effective, feasible way to improve BO, STS, and CS and hand hygiene for interdisciplinary care teams. This intervention can be replicated in similar clinical settings but may realize different results. Box breathing with positive self-messaging during hand hygiene can be incorporated along with other self-care techniques into pre-li-censure nursing education to prepare nursing students to better manage job-related stress, enhance job satisfaction, and prevent BO. Abstract

Introduction \ud \ud Gastrointestinal dysmotility and constipation are common problems in intensive care patients. The majority of critical care patients are sedated with opioids to facilitate tolerance of endotracheal tubes and mechanical ventilation, which inhibit gastrointestinal motility and lead to adverse outcomes. Methylnaltrexone is a peripheral opioid antagonist that does not cross the blood–brain barrier and can reverse the peripheral side effects of opioids without affecting the desired central properties. This trial will investigate whether methylnaltrexone can reverse opioid-induced constipation and gastrointestinal dysmotility.\ud \ud Methods \ud \ud This is a single-centre, multisite, double-blind, randomised, placebo-controlled trial. 84 patients will be recruited from 4 intensive care units (ICUs) within Imperial College Healthcare NHS Trust. Patients will receive intravenous methylnaltrexone or placebo on a daily basis if they are receiving opioid infusion to facilitate mechanical ventilation and have not opened their bowels for 48 hours. All patients will receive standard laxatives as per the clinical ICU bowel protocol prior to randomisation. The primary outcome of the trial will be time to significant rescue-free laxation following randomisation. Secondary outcomes will include tolerance of enteral feed, gastric residual volumes, incidence of pneumonia, blood stream and Clostridium difficile infection, and any reversal of central opioid effects.\ud \ud Ethics and dissemination \ud \ud The trial protocol, the patient/legal representative information sheets and consent forms have been reviewed and approved by the Harrow Research Ethics Committee (REC Reference 14/LO/2004). An independent Trial Steering Committee and Data Monitoring Committee are in place, with patient representation.
