O presente artigo relata a experiência sobre formação de estagiários de licenciaturas para atuarem como Profissionais de Apoio Escolar/mediadores de aprendizagem em escola municipal de Jacobina/ Bahia. A formação objetivou promover espaços de formação a fim de contribuir com as práticas pedagógicas a partir de rodas dialogais acerca da caracterização e desmistificação das principais particularidades de aprendizagem dos alunos da instituição escolar e, desenvolver a oficina de atividades lúdicas para criação de materiais que facilitem a mediação pedagógica. Utilizou-se como estratégia metodológica a revisão de literatura, o desenvolvimento de rodas dialogais com os mediadores, a discussão de casos e exposições acerca das características peculiares a transtornos globais do desenvolvimento e a promoção de oficinas de produção de recursos pedagógicos com materiais recicláveis e/ou de baixo custo. Conclui-se ser imprescindível a formação continuada para os mediadores escolares, bem como a disponibilização de recursos para a efetivação do que se propõe a mediação.

Nabil Araújo é doutor em Estudos Literários pela Universidade Federal de Minas Gerais (UFMG) e professor de Teoria da Literatura na graduação e na pós-graduação em Letras da Universidade do Estado do Rio de Janeiro (UERJ). Editor Associado de Matraga – Revista do Programa de Pós-graduação em Letras da UERJ e coordernador do conselho editorial da Coleção Letras UERJ da EdUERJ. Líder do grupo de pesquisa interinstitucional “Retorno à Poética: imagologia, referenciação, genericidade” (CNPq). Orienta pesquisas de mestrado e de doutorado em Estudos Literários. Publicou diversas obras, dentre elas O evento comparatista: da morte da literatura comparada ao nascimento da crítica (EdUEL, 2019) e Teoria da Literatura e História da Crítica: momentos decisivos (EdUERJ, 2020). Por sua tese de doutorado, recebeu o Prêmio UFMG de Teses, em 2014, e o Prêmio ANPOLL de Teses, em 2016. Seu projeto “Ensino de literatura e desenvolvimento da competência crítica: uma ‘terceira via’ didático-pedagógica” foi premiado pela Fundação Carlos Chagas como a melhor experiência educativa inovadora realizada por docente de Licenciatura em 2014. Por duas vezes, em 2015 e em 2019, foi contemplado com o Prêmio Docência Dedicada ao Ensino Anísio Teixeira, conferido pela Pró-reitoria de Graduação da UERJ. Sua produção acadêmica com enfoque em teoria literária nos levou a convidá-lo a explorar a desleitura como operador conceitual e suas possibilidades para pensar a leitura e o ensino de literatura.

Este trabalho apresenta as traduções/reescritas de narrativas distópicas da escritora canadense Margaret Atwood, publicados no Brasil pela editora Rocco entre os anos de 2000 e 2019. Analisa-se os paratextos (capas, contracapas e orelhas) para compreender como esses elementos reescrevem a escritora e suas obras para o público brasileiro, criando um cânone doméstico de sua produção. Como fundamentação teórico-metodológica toma-se Even-Zohar (1990) e outros, sobre teoria de polissistemas; Venuti (2019) sobre cânone doméstico; Lefevere (2007), sobre reescrita e tradução literária; Kathryn Batchelor (2020) e Câmara (2014) sobre paratexto e paratradução. Para o gênero distopia, fundamenta-se em Margaret Atwood (2005), entre outros. Das 6 narrativas distópicas de Atwood, 5 foram publicadas no Brasil pela editora Rocco, e apresentam reedições, em consonância com a maior popularização da escritora, a partir de 2017. As narrativas distópicas de Atwood são publicadas pelo selo principal, e não pelo selo jovem da editora, atestando que a escritora entra no sistema literário brasileiro como escritora de prestígio, e não pelas margens, lócus da literatura traduzida. Com suporte do programa AntConc, observa-se nos paratextos que a escritora e as obras são associadas inicialmente ao gênero de ficção científica e posteriormente à ficção especulativa ou distopia, e destaca-se: a notoriedade da escritora, prêmios recebidos e circulação das obras. O cânone doméstico da autora se concentra nos seus romances, deixando em segundo plano suas outras produções ficcionais e crítico-teóricas, tomadas como base teórica em diálogo com sua reescrita no Brasil.

Introdução: a cirurgia permanece como uma das bases do tratamento oncológico. As complicações cardiovasculares após cirurgias não cardíacas representam uma causa importante de morbidade e mortalidade pósoperatória. Exames adicionais como angiotomografia computadorizada de coronárias (CCTA) na estratificação de risco antes de cirurgias não cardíacas tem tido seu papel estudado Objetivos: avaliar a habilidade da angiotomografia de coronárias em predizer a ocorrência de injúria miocárdica (MINS) no pós-operatório de cirurgias oncológicas de grande porte. Métodos: no período de agosto de 2017 a julho de 2021, foram incluídos 184 pacientes portadores de neoplasia, em programação de tratamento com cirurgia de grande porte, assintomáticos do ponto de vista cardiovascular e com pelo menos dois fatores de risco cardiovascular. Os pacientes foram submetidos a CCTA no pré-operatório e a dosagem de troponina no pós-operatório imediato e até o terceiro dia. O desfecho primário foi a ocorrência de injuria miocárdica nas primeiras 72 horas de pósoperatório. A injuria miocárdica foi definida como valores de troponina T 0,014 ng/mL. Os desfechos secundários foram: taxa de eventos combinados em 30 dias ou até a alta hospitalar definidos por insuficiência renal, choque séptico, arritmia cardíaca, infarto agudo do miocárdio, acidente vascular cerebral, eventos tromboembólicos, morte cardiovascular e morte por todas as causas; taxa de reinternação hospitalar em 30 dias; tempo de internação na UTI e tempo de internação hospitalar; fatores de risco para injúria miocárdica e para eventos combinados. Resultados: a mediana e o intervalo interquartílico de idade foi de 66 (60-73) anos, com 44,6% de pacientes com neoplasias do retossigmoide. Injúria miocárdica ocorreu em 87 pacientes (48%), e eventos combinados ocorreram em 70 pacientes (37,6%). A taxa de reinternação foi de 14,7%, e a mediana do tempo de internação hospitalar e de terapia intensiva foi de 8 dias (6-12 dias) e 2 dias (1-4 dias), respectivamente. A análise logística multivariada para injúria miocárdica identificou como preditores os seguintes: tumor de bexiga (OR 10,40 [IC 95% 2,51-43,20]; P = 0,001), tumor de esôfago (OR 7,39 [IC 95% 2,27- 24,08], P = 0,001), maior tempo de anestesia (OR 1,24 [IC 95% 1,09-1,43]; P = 0,002), escore de cálcio de 401 -1000 (OR 5,92 [IC 95% 1,29-27,08; P = 0,022]) e escore de cálcio > 1000 (4,62 [IC 95%1,18-18,04; P = 0,028]). A análise logística para eventos combinados identificou como preditores os seguintes: lesão coronariana na angiotomografia, sendo que os pacientes com lesões em três ou mais artérias apresentaram HR de 2,10 [IC 95%1,11- 3,96]; P = 0,022), maior porcentagem do tempo de hipotensão na cirurgia (HR 1,02 [IC 95% 1,00-1,04]; P = 0,019) e creatinina elevada pré-operatória (HR 4,71 [IC95%1,60-13,88]; P = 0,005). Conclusões: o escore de cálcio e o número de vasos acometidos detectados pela CCTA foram preditores de injúria miocárdica e eventos combinados em pacientes com câncer submetidos a cirurgia de alto risco. A CCTA deve ser considerada na estratificação de risco cardiovascular nessa população, permitindo a melhor alocação de recursos e aplicação de estratégias adequadas para prevenir complicações Introduction: surgery remains one of the bases of cancer treatment. Cardiovascular complications after non-cardiac surgeries represent an important cause of postoperative morbidity and mortality. Additional tests such as coronary computed tomography angiography (CTA) in risk stratification before non-cardiac surgeries have had their role studied. Objectives: To evaluate the ability of coronary computed tomography angiography to predict the occurrence of myocardial injury (MINS) in the postoperative period of major oncological surgeries. Methods: from August 2017 to July 2021, 184 patients with cancer were included, undergoing treatment with major surgery, asymptomatic from a cardiovascular point of view and with at least two cardiovascular risk factors. Patients underwent CCTA preoperatively and troponin dosage in the immediate postoperative period and until the third day. The primary outcome was the occurrence of myocardial injury in the first 72 hours after surgery. Myocardial injury was defined as troponin T values 0.014 ng/mL. Secondary endpoints were: rate of combined events at 30 days or until hospital discharge defined by renal failure, septic shock, cardiac arrhythmia, acute myocardial infarction, stroke, thromboembolic events, cardiovascular death, and all-cause death; 30-day hospital readmission rate; length of stay in the ICU and length of hospital stay; risk factors for myocardial injury and for combined events. Results: the median and interquartile age range was 66 (60-73) years, with 44.6% of patients with rectosigmoid neoplasms. Myocardial injury occurred in 87 patients (48%), and combined events occurred in 70 patients (37.6%). The readmission rate was 14.7%, and the median length of hospital stay and intensive care was 8 days (6-12 days) and 2 days (1-4 days), respectively. Multivariate logistic analysis for myocardial injury identified the following predictors: bladder tumor (OR 10.40 [CI 95% 2.51-43.20]; P = 0.001), esophageal tumor (OR 7.39 [CI 95 % 2.27-24.08], P = 0.001), longer anesthesia time (OR 1.24 [95% CI 1.09 - 1.43]; P = 0.002), calcium score of 401 -1000 (OR 5.92 [95% CI 1.29-27.08; P = 0.022]) and calcium score >1000 (4.62 [95% CI 1.18-18.04; P = 0.028]). Logistic analysis for combined events identified the following as predictors: coronary lesion on CT angiography, with patients with lesions in three or more arteries having a HR of 2.10 [95% CI 1.11-3.96]; P = 0.022), higher percentage of hypotension time in surgery (HR 1.02 [95% CI 1.00-1.04]; P = 0.019) and preoperative elevated creatinine (HR 4.71 [CI95%1] .60-13.88]; P = 0.005). Conclusions: the calcium score and the number of affected vessels detected by CCTA were predictors of myocardial injury and combined events in cancer patients undergoing high-risk surgery. CCTA should be considered in the stratification of cardiovascular risk in this population, allowing for a better allocation of resources and application of adequate strategies to prevent complications

Para tanto, o texto apresenta reflexoes a partir dos marcos legais inclusivos na educacao e das discussoes teoricas e conceituais sobre cotidiano escolar com Ferraco (2006) e Alvez; surdez com Skliar (1999 ) e Strobel (2009); inclusao com Miranda ( 2006); e Experiencias com Larrosa (2002), entre outros, e de dados a partir de levantamento realizado em revisao sistematica utilizando banco de dados de dissertacoes e teses das Universidades Publicas na Bahia e artigos oriundos do Congresso Baiano de Educacao Inclusiva Como metodologia partimos da abordagem qualitativa Gil (2010) se compromete a apresentar resultados embasados na teoria e nos dados obtidos em revisao sistematica e no mapeamento da escolarizacao de surdos na rede estadual em Conceicao do Coite. Background Patients with Coronavirus Disease 2019 (COVID-19) may present high risk features during hospitalization, including cardiovascular manifestations. However, less is known about the factors that may further increase the risk of death in these patients. Methods We included patients with COVID-19 and high risk features according to clinical and/or laboratory criteria at 21 sites in Brazil from June 10th to October 23rd of 2020. All variables were collected until hospital discharge or in-hospital death. Results A total of 2546 participants were included (mean age 65 years; 60.3% male). Overall, 70.8% were admitted to intensive care units and 54.2% had elevated troponin levels. In-hospital mortality was 41.7%. An interaction among sex, age and mortality was found (p = 0.007). Younger women presented higher rates of death than men (30.0% vs 22.9%), while older men presented higher rates of death than women (57.6% vs 49.2%). The strongest factors associated with in-hospital mortality were need for mechanical ventilation (odds ratio [OR] 8.2, 95% confidence interval [CI] 5.4–12.7), elevated C-reactive protein (OR 2.3, 95% CI 1.7–2.9), cancer (OR 1.8, 95 %CI 1.2–2.9), and elevated troponin levels (OR 1.8, 95% CI 1.4–2.3). A risk score was developed for risk assessment of in-hospital mortality. Conclusions This cohort showed that patients with COVID-19 and high risk features have an elevated rate of in-hospital mortality with differences according to age and sex. These results highlight unique aspects of this population and might help identifying patients who may benefit from more careful initial surveillance and potential subsequent interventional therapies.

Takotsubo syndrome is a disease of great clinical importance that remains underdiagnosed. It is a form of acute heart failure characterized by a transient wall motion abnormality of the left ventricular apex typically triggered by emotional or physical stress. Takotsubo syndrome is commonly associated with cancer and results in poor outcomes. Therefore, early recognition and prompt therapy are essential to improve prognosis. The aim of this manuscript is to review the consequences of the association between cancer and Takotsubo to summarize the available evidence to guide physicians to improve the management of these patients.

Background: Cancer associated venous thrombosis (CAT) can affect 20% of oncologic patients. Its management is challenging due to coexistent risk of bleeding and thrombosis (VTE) recurrence, that are 6 and approximately 4 fold higher compared to non-cancer patients, respectively. Anticoagulant choice needs to be careful and can be guided by a recommendation to avoid those complications. Aim: To evaluate effectiveness and safety of anticoagulation according to a public cancer institution recommendation on CAT management. Methods: This is a retrospective cohort study carried out from October 2018 to January 2020. The recommendation on CAT management included defined criteria to select the proper anticoagulant for each patient, to make adjustments and to handle anticoagulation in challenging situations, like thrombocytopenia, in the perioperative period and facing bleeding and thrombosis recurrence. A 3-month ambulatory was scheduled to evaluate eligibility of outpatients on enoxaparin to switch to rivaroxaban (new at the institution), based on criteria defined by the recommendation. All consecutive adult oncologic outpatients on therapeutic enoxaparin in the institution were considered eligible for the ambulatory. Retrospective data on cancer, comorbidities, bleeding, VTE recurrence and death during 1 year since the first visit were captured from an electronic recording. Cumulative incidence of bleeding and VTE recurrence were calculated by Kaplan-Meier method and log-rank test. Results: 326 patients were referred to the ambulatory and 268 were included in the analyses, with a median age of 63 years (IQR 53-71). Sixty-one percent were female and 86% had solid tumours (53% metastatic). The main sites were the gastrointestinal tract (27%), breast (12%) and lungs (10%). Forty-six percent of the patients switched to rivaroxaban and 6% were able to stop anticoagulation. From the 85 patients who remained on enoxaparin, 51.7% had some dosing adjustment (to 1.5 mg/kg/day or to prophylactic dose). The main reason for not switching to rivaroxaban was drug-drug interaction (27%). One-year cumulative incidence of VTE recurrence was 11.2% (95% CI 7.7-16.2) and of overall bleeding was 13.4% (95% CI 9.4-18.7). Major bleeding occurred in 8 out of 268 patients (2.9%). Death rate was 32%, with no one due to bleeding. No significant difference in outcomes was observed between patients who had switched or not. Discussion: Severity of cancer status was considered relevant for the high rate of deaths. Cumulative incidence of bleeding as well as thrombosis recurrence were similar to the described in the literature, although many clinical trials on direct oral anticoagulants in cancer patients had a shorter period of follow-up. Conclusions: Recommendation-based decision on anticoagulant choice and management showed similar effectiveness and safety independently of the chosen anticoagulant. No relevant hazards due to anticoagulation were observed during the follow-up period.

SARS-CoV-2, the causative agent of coronavirus disease 2019 (COVID-19), is responsible for the largest pandemic facing humanity since the Spanish flu pandemic in the early twentieth century. Since there is no specific antiviral treatment, optimized support is the most relevant factor in the patient's prognosis. In the hospital setting, the identification of high-risk patients for clinical deterioration is essential to ensure access to intensive treatment of severe conditions in a timely manner. The initial management of hypoxemia includes conventional oxygen therapy, high-flow nasal canula oxygen, and non-invasive ventilation. For patients requiring invasive mechanical ventilation, lung-protective ventilation with low tidal volumes and plateau pressure is recommended. Cardiovascular complications are frequent and include myocardial injury, thrombotic events, myocarditis, and cardiogenic shock. Acute renal failure is a common complication and is a marker of poor prognosis, with significant impact in costs and resources allocation. Regarding promising therapies for COVID-19, the most promising drugs until now are remdesivir and corticosteroids although further studies may be needed to confirm their effectiveness. Other therapies such as, tocilizumab, anakinra, other anti-cytokine drugs, and heparin are being tested in clinical trials. Thousands of physicians are living a scenario that none of us have ever seen: demand for hospital exceed capacity in most countries. Until now, the certainty we have is that we should try to decrease the number of infected patients and that an optimized critical care support is the best strategy to improve patient’s survival.

Background Cardiotoxicity (CT) remains an important cause of morbidity and mortality in patients with lymphomas treated with anthracyclines. Current strategies for early detection of CT during chemotherapy are not yet fully established. Purpose To evaluate the performance of cardiac magnetic resonance with T1 mapping and myocardium strain by feature tracking during chemotherapy in detecting anthracycline CT in patients with lymphoma. Methods From June 2017 to March 2019, patients with lymphoma planned to start chemotherapy with anthracyclines were evaluated by a cardiologist to check the eligibility criteria. At baseline (Time 1), at the end of 3° cycle (Time 2) and 30 days after the final cycle (Time 3), patients were evaluated through cardiac biomarkers, electrocardiogram and cardiac magnetic resonance (CMR). Strain, MapT1 and extracellular volume (ECV) were evaluated in all patients. CT was defined as drop of the left ventricular fraction (LVEF) >10% or LVEF decrease below 50%. A p value Result We included 48 patients, mean age was 45.32 (± 17.84) years-old and 25 (52.1%) were female. The prevalence of hypertension, diabetes and dyslipidemia was 18.8%, 10.4% and 10.4%, respectively. CT was diagnosed in 13 patients (27%). At baseline, there was no difference between cardiotoxicity group (CTG) and no cardiotoxicity group (nCTG) in CMR diastolic volume, systolic volume, nativeT1 map and global longitudinal strain (GLS), respectively (116 [103.6–138.1]ml vs 136.3 [115.7–173.8], p=0.069), (46 [38.0–58.5] ml vs 63.0 [44.5–74.2, p=0.069), (1540.6 [1478.3–1591.1]ms vs 1514.8 [1487.5–1786.3]ms, p=0.568) and (−15.94±2.91% vs −14.84±2.65%, p=0.243). Regarding the others CMR parameters, we showed that comparing CTG patients with nCTG patients at Time 3, systolic volumes were higher (54.8 [45.0–67.0] ml vs 78.51 [53.9–96.3] ml, p=0.007), right ejection fraction was lower (53.41±9.73% vs 46.29±3.93%, p=0.002), LVEF was lower (58.7±5.69% vs 46.67±8.12%, p Conclusions Cardiotoxicity is a frequent complication in anthracycline treated patients. CMR evaluation, through analysis of volumes, ejection fraction and strain might early identify these patients, allowing prevention strategies to be initiated to improve cardiovascular outcomes. Thymomas are usually tumors with a good prognosis but with potential to invade nearby structures. We report a case of a 56-year-old woman previously diagnosed with an invasive thymoma that came to the emergence room with symptoms of acute coronary syndrome. After a coronary computed tomography angiography, it was seen that the tumor was invading the myocardial and it was irrigated by the left circumflex coronary and its branches. Considering her poor prognosis, it was decided not to make further interventions. This case highlights a unique and rare case of an unresectable thymoma that was invading the myocardium and it was irrigated by the left circumflex coronary and its branches, causing typical angina due to compromised coronary blood flow by the mass causing ischemia.

Resumo O desafio imposto ao sistema de saúde pela pandemia da COVID-19 faz com que haja uma necessidade de readequações de rotinas e serviços de saúde, com os objetivos de controlar a disseminação do vírus e preservar a saúde. Torna-se ainda mais importante o manejo seguro e correto dos pacientes dos grupos de risco, como os pacientes idosos, os portadores de doenças cardiovasculares e os pacientes com câncer. Dessa forma, a cardio-oncologia ganha novo dimensionamento, no intuito de se adequar às necessidades dos pacientes diante de uma pandemia, reestruturando o sistema de atendimento de forma a oferecer qualidade e segurança na assistência à saúde. As complicações cardiovasculares evidenciadas nos pacientes com COVID-19 resultam de vários mecanismos, que vão desde lesão direta pelo vírus até complicações secundárias à resposta inflamatória e trombótica desencadeada pela infecção. O cuidado adequado do paciente com COVID-19 exige atenção ao sistema cardiovascular em busca de melhores desfechos. Background In recent years, the field of cardio-oncology has grown worldwide, bringing benefits to cancer patients in terms of survival and quality of life. This study reports the experience of a pioneer cardio-oncology programme at University Cancer Hospital in Brazil over a period of 10 years, describing the clinical profile of patients and the clinical outcomes. Methods A retrospective study was conducted on a cohort of patients treated at the cardio-oncology programme from April 2009 to February 2019. We analysed the characteristics of patients and outcomes, including mortality, according to the type of clinical indication for outpatient care (general cardiology, perioperative evaluation and follow-up and treatment cardiotoxicity). Results From a total of 26,435 medical consultations, we obtained the data of 4535 individuals among the medical care outpatients. When we analysed the clinical characteristics of patients considering the clinical indication - general cardiology, perioperative evaluation and cardiotoxicity outpatient clinics, differences were observed with respect to age (59 [48–66], 66 [58–74] and 69 [62–76], p p p p p = 0.650]). Conclusion The number of oncologic patients in the Cardio-Oncology Programme has grown in the last decade. A well-structured cardio-oncology programme is the key to achieving the true essence of this area, namely, ongoing care for cancer patients throughout the disease treatment process, optimizing their cardiovascular status to ensure they can receive the best therapy against cancer.
