Background: Reliable Human Papillomavirus (HPV) testing and genotyping are essential for quality assurance in HPV-based primary screening, disease management and for monitoring the impact of HPV vaccination. The clinical validation of HPV molecular diagnostic assays has significantly contributed to these objectives; however, little emphasis has been placed on assuring sample quality. This study aimed to evaluate the accuracy of sample cellularity assessment using the C-C Motif Chemokine Receptor 5 (CCR5) gene target as a marker of sample adequacy in molecular diagnostics. Methods: Jurkat cell line samples were counted using both a Thoma cell-counting chamber and Fluorescence-Activated Cell Sorting (FACS). Jurkat cell line samples at three different concentrations were subsequently evaluated using the OncoPredict HPV Quality Control (QC) real-time PCR assay, employing CCR5 for molecular cellularity quantification. Results: The cellularity values obtained were comparable across the three different methods for all dilutions of the cell line tested. Conclusions: The results obtained from this study show that CCR5 represents a promising molecular marker for the accurate quantification of sample cellularity, confirming its use as a reliable sample adequacy control, thus reducing the risk of “false-negative” results.

In this new study the Authors demonstrated that the IGFBP3/TMEM219 pathway is a physiological regulator of pancreatic beta cell homeostasis and it is dysregulated in diabetes. IGFBP3/TMEM219 targeting may therefore serve as a therapeutic option in diabetes.

Tubulointerstitial fibrosis is observed in diabetic nephropathy. It is still debated whether tubular cells, undergoing epithelial-mesenchymal transition (EMT) in high glucose (HG) conditions, may contribute to interstitial fibrosis development. In this study, we investigated the phenotypic and molecular EMT-like changes and the alteration of inflammatory and fibrogenic secretome induced by HG in human primary tubular cell cultures. Taking advantage of this in vitro cell model composed of proximal and distal tubular cells, we showed that HG-treated tubular cells acquired a fibroblast-like morphology with increased cytoplasmic stress fibers, maintaining the expression of the epithelial markers specific of proximal and distal tubular cells. HG increased Snail1, miRNA210 and Vimentin mesenchymal markers, decreased N-cadherin expression and migration ability of primary tubular cells, while E-cadherin expression and focal adhesion distribution were not affected. Furthermore, HG treatment of tubular cells altered the inflammatory cytokine secretion creating a secretome able to enhance the proliferation and migration of fibroblasts. Our findings show that HG promotes an activated state of partial EMT in human tubular primary cells and induces a pro-inflammatory and pro-fibrogenic microenvironment, supporting the active role of tubular cells in diabetic nephropathy onset.

5. Evaluation of RAS Mutational Status in Liquid Biopsy to Monitor Disease Progression in Metastatic Colorectal Cancer Patients

In this study we evaluated both~ K- and N-RAS mutations in plasma samples from patients with metastatic colorectal cancer by means of the BEAMing technology, and we assessed their diagnostic performance compared to RAS analyses performed on tissue. The sensitivity of BEAMing in identifying KRAS mutations was of 89.5%, with a fair specificity. The agreement with tissue analysis was moderate. The sensitivity for NRAS was high with a good specificity, and the agreement between tissue analysis and BEAMing was fair. Interestingly, significantly higher mutant allele fraction (MAF) levels were detected in patients with G2 tumors, liver metastases, and in those who did not receive surgery. NRAS MAF level was significantly higher in patients with mucinous adenocarcinoma and for those with lung metastases. A sharp increase in the MAF values was observed in patients who moved towards disease progression. More strikingly, molecular progression always anticipated the radiological one in these patients. These observations pave the way to the possibility of using liquid biopsy to monitor patients during treatment, and to enable oncologists to anticipate interventions compared to radiological analyses. This will allow time to be saved and ensure a better management of metastatic patients in the near future.

6. Cultural Heritage and Religious Phenomenon between Urbicide and Cancel Culture: The Other Side of the Russian–Ukrainian Conflict

The Russian–Ukrainian conflict, in addition to causing an unacceptable loss of human life, is straining the integrity of Ukraine’s cultural heritage, despite the fact that both countries involved are parties to the 1954 Hague Convention for the Protection of Cultural Property in the Event of Armed Conflict and its First Protocol. Churches are one of Ukraine’s most important historical assets, as well as symbolic places of Orthodox religious identity common to both the invaders and the invaded. The destruction of these places and their deliberate damage on the part of both sides appear to be part of a more general conflict concerning internal disagreements between Russian and Ukrainian Orthodoxy, which, in turn, reflect two different historical views of the Russian–Ukrainian relationship. A brief reconstruction of relations between the Orthodox Churches operating on the territory of Ukraine demonstrates how religious affiliation has affected the conflict, causing it to become decisive and deeply divisive, so much so that the Patriarchate of Moscow has become an active part of the conflict. This circumstance favours the hypothesis that it is precisely the religious cultural heritage that is most at risk of deliberate destruction. The Russians, by destroying the symbolic places of Ukrainian religious identity (urbicide), affirm the spiritual unity of the Russian and Ukrainian peoples. For their part, the Ukrainians attempt to erase the Russian presence and the common religious cultural roots by destroying buildings of worship dear to the tradition of the Moscow Patriarchate (cancel culture). They reject the imperial traditions of Russia and, at the same time, claim an independent Church. The question arises as to whether the reconstruction process following the war will take into account the original cultural–religious identities, or whether it will take the opportunity to adopt a new (also) religious identity instead, and whether the old and new instruments offered by law are adequate.

To date treatment protocols in Respiratory and or Internal departments across Italy for treatment of chronic obstructive pulmonary disease (COPD) patients at hospital admission with relapse due to exacerbation do not find adequate support in current guidelines. Here we describe the results of a recent clinical audit, including a systematic review of practices reported in literature and an open discussion comparing these to current real-life procedures. The process was dived into two 8-hour-audits 3 months apart in order to allow work on the field in between meeting and involved 13 participants (3 nurses, 1 physiotherapist, 2 internists and 7 pulmonologists). This document reports the opinions of the experts and their consensus, leading to a bundle of multidisciplinary statements on the use of inhaled drugs for hospitalized COPD patients. Recommendations and topics addressed include: i) monitoring and diagnosis during the first 24 h after admission; ii) treatment algorithm and options (i.e., short and long acting bronchodilators); iii) bronchodilator dosages when switching device or using spacer; iv) flow measurement systems for shifting to LABA+LAMA within 48 h; v) when nebulizers are recommended; vi) use of SMI to deliver LABA+LAMA when patient needs SABA 30 litres/min; viii) contraindication to use DPI; ix) continuation of LABA-LAMA when patient is already on therapy; x) possible LABA-LAMA dosage increase; xi) use of SABA and/or SAMA in addition to LABA+LABA; xii) use of SABA+SAMA restricted to real need; xiii) reconciliation of drugs in presence of comorbidities; xiv) check of knowledge and skills on inhalation therapy; xv) discharge bundle; xvi) use of MDI and SMI in tracheostomized patients in spontaneous and ventilated breathing.

The fibrotic tissue and the stroma adjacent to cancer cells are characterised by the presence of activated fibroblasts (myofibroblasts) which play a role in creating a supportive tissue characterised by abundant extracellular matrix (ECM) secretion. The myofibroblasts remodel this tissue through secreted molecules and modulation of their cytoskeleton and specialized contractile structures. The non-receptor protein tyrosine kinase Arg (also called Abl2) has the unique ability to bind directly to the actin cytoskeleton, transducing diverse extracellular signals into cytoskeletal rearrangements. In this study we analysed the 1ALCTL and 1BLCTL Arg isoforms in Arg−/− murine embryonal fibroblasts (MEF) cell line, focusing on their capacity to activate fibroblasts and to remodel ECM. The results obtained showed that Arg isoform 1BLCTL has a major role in proliferation, migration/invasion of MEF and in inducing a milieu able to modulate tumour cell morphology, while 1ALCTL isoform has a role in MEF adhesion maintaining active focal adhesions. On the whole, the presence of Arg in MEF supports the proliferation, activation, adhesion, ECM contraction and stiffness, while the absence of Arg affected these myofibroblast features. Clear cell renal cell carcinoma (ccRCC) is fundamentally a metabolic disease. Given the importance of lipids in many cellular processes, in this study we delineated a lipidomic profile of human ccRCC and integrated it with transcriptomic data to connect the variations in cancer lipid metabolism with gene expression changes. Untargeted lipidomic analysis was performed on 20 ccRCC and 20 paired normal tissues, using LC-MS and GC-MS. Different lipid classes were altered in cancer compared to normal tissue. Among the long chain fatty acids (LCFAs), significant accumulations of polyunsaturated fatty acids (PUFAs) were found. Integrated lipidomic and transcriptomic analysis showed that fatty acid desaturation and elongation pathways were enriched in neoplastic tissue. Consistent with these findings, we observed increased expression of stearoyl-CoA desaturase (SCD1) and FA elongase 2 and 5 in ccRCC. Primary renal cancer cells treated with a small molecule SCD1 inhibitor (A939572) proliferated at a slower rate than untreated cancer cells. In addition, after cisplatin treatment, the death rate of tumor cells treated with A939572 was significantly greater than that of untreated cancer cells. In conclusion, our findings delineate a ccRCC lipidomic signature and showed that SCD1 inhibition significantly reduced cancer cell proliferation and increased cisplatin sensitivity, suggesting that this pathway can be involved in ccRCC chemotherapy resistance.

The mechanism upon which human kidneys undergo regeneration is debated, though different lineage-tracing mouse models have tried to explain the cellular types and the mechanisms involved. Different sources of human renal progenitors have been proposed, but it is difficult to argue whether these populations have the same capacities that have been described in mice. Using the nephrosphere (NS) model, we isolated the quiescent population of adult human renal stem-like PKHhigh/CD133+/CD24− cells (RSC). The aim of this study was to deepen the RSC in vitro multipotency capacity. RSC, not expressing endothelial markers, generated secondary nephrospheres containing CD31+/vWf+ cells and cytokeratin positive cells, indicating the coexistence of endothelial and epithelial commitment. RSC cultured on decellularized human renal scaffolds generated endothelial structures together with the proximal and distal tubular structures. CD31+ endothelial committed progenitors sorted from nephrospheres generated spheroids with endothelial-like sprouts in Matrigel. We also demonstrated the double commitment toward endothelial and epithelial lineages of single RSC. The ability of the plastic RSC population to recapitulate the development of tubular epithelial and endothelial renal lineages makes these cells a good tool for the creation of organoids with translational relevance for studying the parenchymal and endothelial cell interactions and developing new therapeutic strategies.

An intra-islet incretin system has been recently suggested to operate through modulation of the expression and activity of proconvertase 1/3 and 2 (PC1/3, PC2) in pancreatic alpha-cell accounting for local release of GLP-1. Little is known, whether this alpha-cell activity can be affected by the metabolic alterations occurring in type 2 diabetes, such as hyperglycemia, hyperlipidemia or hyperglucagonemia.AlphaTC1/6 cells from a mice pancreatic cell line were incubated in the presence of two glucose (G) concentration (5.5 and 16.7 mM) for 16 h with or without free fatty acid, IL6 or glucagon. GLP-1 secretion was measured by ELISA and expression of PC1/3 and PC2 by RT-PCR and western blot; cell viability was determined by MTT method, Reactive Oxygen Species generation (ROS) by H2DCFDA fluorescence and apoptosis by Annexin staining and Propidium Iodine (PI) fluorescence.Upon 16.7G incubation, GLP-1 secretion (total and active) was significantly increased in parallel with a significant increment in PC1/3 expression, a slight increase in cell viability and ROS generation and by a decrement in PC2 expression with no change in cell apoptosis. When cells were incubated at 5.5mM glucose with FFA, also an increment in GLP-1 secretion and PC1/3 expression was observed together an increment in ROS generation, a decrement in cell viability, and a modest increment in apoptosis. When incubated with 16.7mM glucose with FFA, the increment in GLP-1 secretion was reduced to basal, accompanied by an increment in apoptosis and ROS generation. This was also observed with IL-6, but in this case, no modification in ROS generation or apoptosis was observed when compared to 16.7mM glucose. The presence of glucagon did not modify any of the parameters studied.These data suggest that under hyperglycemic, hyperlipidemia or inflammatory conditions, alpha cells can increase expression PC1/3 and activate GLP-1 secretion, which may contribute protecting both alpha and beta-cells from glucose and lipotoxicity, while this effect seems to be lost in the presence of both pathophysiological conditions.

PURPOSE: Description of clinical and epidemiological characteristics of patients who underwent surgery for oral cancer in a Medical School Teaching Hospital, and determination of differences with respect to other institutions and/or periods of time. METHOD: The charts of patients undergoing surgery for oral cancer from 1994 to 2002 were reviewed. Data were collected in a spreadsheet in order to analyze clinical and epidemiological features.. RESULTS: A total of 374 patients having undergone 406 operations was identified. Their ages varied from 14 to 94 years (mean = 57.4 years), with 255 men (68.2%), and 295 out 366 Caucasian (80.6%). A majority had tumors of the tongue and/or floor of mouth (55.6%), while 20.3% had lip cancer. Squamous cell carcinoma was found in 90.3%, and glandular carcinoma in 4%. T4 tumors in 39.6%, Tis or T1 lesions in 15.2% of all patients. Nearly 62% had no regional metastases, and the relative incidence in young patients (40 years or younger) reached 8.6%. CONCLUSION: In spite of the predominance of locally advanced tumors, a majority of patients had no neck metastases. The 31.8% incidence in females indicates an increasing incidence of oral cavity cancer among women when compared to previous periods at the same institution.OBJETIVO: Caracterizar epidemiológica e clinicamente a população atendida por câncer de boca em hospital-escola de atenção terciária e quaternária e verificar variações em relação a outras casuísticas e períodos. MÉTODO: Procedeu-se à revisão dos prontuários dos operados por câncer oral de 1994 a 2002. As características clínicas e epidemiológicas foram colocadas em planilhas e analisadas. RESULTADOS: Foram identificados 374 portadores de câncer da boca que foram submetidos a 406 operações. A idade dos doentes variou de 14 a 94 anos (média = 57,4 anos). Duzentos e cinqüenta e cinco eram do gênero masculino (68,2%). Duzentos e noventa e cinco entre 366 doentes eram caucasianos (80,6%). A maioria era portadora de tumores da língua e/ou do soalho da boca (194 entre 349 doentes; 55,6%), e 71 (20,3%) tinham tumores dos lábios. O carcinoma epidermóide foi encontrado em 90,3% e carcinomas glandulares em 4%. Os tumores T4 estavam presentes em 39,6% dos doentes; 15,2% tinham lesões Tis ou T1. Quase 62% dos pacientes não tinham metástases cervicais. A incidência relativa em jovens (com até 40 anos de idade) atingiu 8,6%. CONCLUSÃO: Apesar dos achados freqüentemente referidos na literatura terem se confirmado (como o predomínio de lesões avançadas localmente), a maioria dos doentes não apresentava metástases cervicais. Acima do observado em outras casuísticas, os 31,8% de mulheres na presente casuística indica a preocupante e crescente incidência no gênero feminino.

RESUMO: Ensinar cores a cegos de nascimento é um desafio para professores de física, porém insistimos em fazê-lo. Propomos levar em consideração que todos os sentidos são mobilizados no processo de aprendizagem, sendo assim, multissensorial. O sentido da visão, em nosso caso de estudo, será substituído por outros. Considerando que a formação de conceitos é um processo sociolinguístico, avaliamos e averiguamos o conteúdo imagético do conceito de cores em um teste de associação livre com estudantes cegos e videntes. Concluímos que tais conceitos não dependem exclusivamente do visual; outras associações estão envolvidas na sua construção, embora saibamos que nenhum outro sentido fará um cego perceber visualmente as cores. Assim, é possível uma concepção segundo a qual o ensino de cores a alunos cegos deixe de ser um desafio intransponível.

Resumo A Educação Interprofissional em Saúde (EIP) constitui-se como uma estratégia para formação de profissionais capazes de trabalhar em equipe por meio de competências colaborativas e da aprendizagem baseada em interações. O Projeto Pedagógico da Universidade Federal de São Paulo (Unifesp) baseia-se nos princípios da EIP para a formação de profissionais, dentre eles, terapeutas ocupacionais. Objetivou-se compreender a influência da formação interprofissional na trajetória profissional de terapeutas ocupacionais egressas da Unifesp. Pela abordagem qualitativa, produziram-se informações por aplicação de formulário on-line semiestruturado, analisado posteriormente com base na Análise Temática. Os resultados revelaram que a formação interprofissional possibilita segurança às profissionais nas práticas colaborativas, no fomento ao trabalho em equipe e no desenvolvimento de ações que visam à integralidade do cuidado. Notou-se, porém, dificuldades no reconhecimento da identidade profissional da Terapia Ocupacional e na ação conjunta com profissionais formados em perspectiva disciplinar. Abstract Interprofessional education in healthcare (IEH) is a strategy to prepare professionals who are capable of working in a team by developing interactive-based collaboration and learning competencies. The Federal University of Sao Paulo’s (UNIFESP) education plan is based on the principles of IEH for professional training. The aim of this study was to understand the influence of interprofessional education on the career trajectory of occupational therapy students graduating from UNIFESP. Adopting a qualitative approach, data was collected using an online semi-structured questionnaire and analyzed using thematic analysis. The results show that interprofessional education helped professionals feel secure when engaging in collaborative practice and fostered teamwork and the development of actions aimed at promoting comprehensive care. However, the findings also highlight challenges related to the recognition of the professional identity of occupational therapists and in working together with professionals trained from a uniquely disciplinary perspective. Resumen La Educación Interprofesional en Salud (EIP) se constituye como una estrategia para la formación de profesionales capaces de trabajar en equipo, por medio de competencias colaborativas y del aprendizaje basado en interacciones. The hydrological regime leads to fluctuations in the availability of food resources for fish, which may reflect in their diet variation. Our main goal was to analyze the relationship between water-level fluctuations and the diet and the body condition of Trachelyopterus galeatus in the Upper Paraná River floodplain. The fish used in the study were sampled quarterly along nine years, at nine sampling stations. Our results showed that diet of T. galeatus varied in response to oscillations in the hydrometric level, with the consumption of terrestrial invertebrates being positively related to hydrometric level, while the consumption of aquatic invertebrates was negatively related. The trophic niche breadth and feeding activity were not affected by hydrometric level, but the fish body condition was positively affected. The high food plasticity allows T. galeatus to consume the most available resources (e.g., terrestrial invertebrates in high water levels), which probably contributed to the increase in its body condition in higher hydrometric levels. In summary, the hydrological dynamic is fundamental for the diet variation of omnivorous fish, which can take advantage of the resource availability according to the hydrometric level, increasing its body condition when allochthonous resources as terrestrial invertebrates are supposed to be more available. Resumo O regime hidrológico leva a flutuações na disponibilidade de recursos alimentares para os peixes, o que pode refletir na variação de sua dieta. O principal objetivo foi analisar a relação entre as oscilações do nível hidrométrico e a dieta e condição corporal de Trachelyopterus galeatus na planície de inundação do alto rio Paraná. Os peixes foram amostrados trimestralmente durante nove anos, em nove locais. Nossos resultados mostraram que a dieta de T. galeatus variou em resposta às oscilações do nível hidrométrico, com o consumo de invertebrados terrestres sendo positivamente relacionado com o nível hidrométrico, enquanto o consumo de invertebrados aquáticos foi negativamente relacionado. O nicho trófico e a atividade alimentar não foram afetados pelo nível hidrométrico, mas a condição corporal dos peixes foi positivamente afetada. A alta plasticidade alimentar permite que T. galeatus aproveite recursos mais disponíveis (por exemplo, invertebrados terrestres em níveis elevados), o que provavelmente contribui para o aumento da condição corporal em níveis hidrométricos elevados. Em resumo, a dinâmica hidrológica é fundamental para a variação da dieta de peixes onívoros, que podem aproveitar a disponibilidade de recursos de acordo com o nível hidrométrico, aumentando sua condição corporal quando recursos alóctones, como invertebrados terrestres, são supostamente mais disponíveis.

This paper presents the challenges of production and dissemination of knowledge in Occupational Therapy in Latin America, its place in the global scenario and the weaving of a political-epistemic network of communication, discussion and exchange. A sense of groupwork, processes and intervention as a collective network is proposed to analyse and understand the social relational practices that make up the political-epistemic network between key-subjects, different types of knowledge, as well as different ways of producing and disseminating them for the institutionality of the field of Occupational Therapy itself and for its outreach in society in general. To this end, we describe and analyse the actions of the Amaru Network as a collective strategy to face the challenges of the production and dissemination of hegemonic and excluding knowledge in Occupational Therapy in the Latin American context. We take as the axis of analysis the idea of collective as a conceptual category that serves to designate and guide the counter-hegemonic processes of knowledge production in the scientific and cosmological reality of the Latin American region. The Amaru Network, in proposing and sustaining a social practice engendered in a collaborative network between Latin American countries, turns its gaze to the broadening of epistemological bases for other possible modes of knowledge production. Este ensaio apresenta os desafios da produção e difusão de conhecimento em Terapia Ocupacional na América Latina, seu lugar no cenário global e a tessitura de uma rede político-epistêmica de comunicação, de discussão e troca. Propõe-se um sentido de trabalho em grupo, de processos e intervenção, enquanto coletivo em rede para analisar e compreender as práticas sociais relacionais que conformam a rede político-epistêmica entre sujeitos-chaves, diferentes tipos de conhecimento, bem como distintos modos de produzi-los e difundi-los para a institucionalidade da própria área de Terapia Ocupacional e para seu alcance na sociedade em geral. Para tanto, são descritas e analisadas ações da Rede Amaru como estratégia coletiva no enfrentamento aos desafios da produção e difusão do conhecimento hegemônico e excludente em Terapia Ocupacional que se colocam ao contexto latino-americano. Tomamos como eixo de análise a ideia de coletivo enquanto uma categoria conceitual que serve para designar e orientar os processos contra-hegemônicos da produção de conhecimento na realidade científica e cosmológica da região latino-americana. A Rede Amaru, ao se propor e sustentar uma prática social engendrada em uma rede colaborativa entre países latino-americanos, volta seu olhar para o alargamento das bases epistemológicas para outros modos possíveis de produção de conhecimento

Clear cell Renal Cell Carcinoma (ccRCC) is the most common and metastatic urological cancer. Molecular players of ccRCC progression and metastasis are not completely known. Here, using primary cell cultures from patients' specimens, we found that TGFβ1/Smad signalling is more activated in high versus low grade ccRCC and inversely correlates with Abl2 tyrosine kinase protein expression. TGFβ1 treatment increased ubiquitination and degradation of Abl2 protein in ccRCC cell lines by TGFβ1/Smad pathway activation and reactive oxygen species production. 3D invasion and matrix degradation assays showed that Abl2 promoted TGFβ1-induced ccRCC cell invasion and maturation of invadopodia, a hallmark of tumour invasion and metastasis. Our findings define Abl2 as a new downstream molecule of TGFβ1 signalling and putative target to counteract advanced ccRCC.

O presente capítulo de livro tem como objetivo apresentar os aspectos do abandono afetivo nas relações familiares frente ao ordenamento jurídico. A afetividade ganhou relevância na família contemporânea, destacando-se na legislação brasileira de forma principiológica. Em virtude disso, a legislação impõe, por exemplo, aos pais deveres na relação com seus filhos, como assistir, cuidar, educar e participar de seu desenvolvimento, e em caso de descumprimento, será imputada aos genitores a devida responsabilização pelo dano cometido. A discussão do tema implica na valoração do afeto no cenário jurídico, visto a crescente demanda pela reparação dos prejuízos morais. Como resultado, aponta-se que o dano causado ao descendente pode ser imensurável, todavia, a imputação da responsabilidade civil cumprirá sua função social e trará segurança jurídica na consagração do direito positivo. Nesse sentido, conclui-se que a reparação moral não será em relação ao amor que o genitor demonstra, mas sobre o descumprimento do dever legal de cuidado com a prole. Como metodologia, utilizou-se o método dedutivo, pesquisa bibliográfica, entendimento dos Tribunais, sítios da web, bem como em legislações pátrias.

32. De-intensification of basal-bolus insulin regimen after initiation of a GLP-1 RA improves glycaemic control and promotes weight loss in subjects with type 2 diabetes

To evaluate the impact of adding a glucagon-like peptide-1 receptor agonist (GLP-1 RA) in people with type 2 diabetes (T2D) in basal-bolus (BB) insulin regimen, on insulin requirement, HbA1c, weight loss up to 24 months.Data on subjects with T2D on BB who initiated a GLP-1 RA have been retrospectively collected. HbA1c, body weight, and insulin dose were recorded at baseline, 6, 12, and 24 months after initiation of GLP-1 RA therapy. A linear mixed model for repeated measures was used to evaluate the changes in HbA1c, body weight, and insulin requirement over time.We included 156 subjects (63.5% males; age 62 ± 11 years, HbA1c 70 ± 22.0 mmol/mol; 8.6 ± 4.2%). Compared to baseline, HbA1c and body weight were significantly lower at 6 months after introducing a GLP-1RA and remained stable up to 24 months (all p 0.0001 vs. baseline). At 24 months, 81% of subjects discontinued prandial insulin, while 38.6% discontinued basal insulin as well. Insulin requirement at baseline (aOR 0.144; 95% CI, 0.046-0.456; P = 0.001) was the only significant predictor of prandial insulin discontinuation.Replacing prandial insulin with GLP-1 RA is a valuable strategy to simplify the BB insulin regimen while improving glycaemic control and promoting weight loss in subjects with T2D.

Since it is unknown whether glucose variability (GV) is increased and whether this is related to worsening of insulin secretion and action in prediabetes, we have assessed insulin secretion and sensitivity, and daily GV in early stages of dysglycemia.Twenty subjects with normal glucose tolerance (NGT; age 45.0 ± 9.5 years; BMI 31.1 ± 6.4 kg/m2), 25 with NGT and 1hrOGTT8.6 mmol/L (1hrOGTT; 45.7 ± 8.5 years; 32.4 ± 7.0 kg/m2), and 59 with isolated impaired glucose tolerance (iIGT; 47.7 ± 11.2 years; 31.3 ± 6.1 kg/m2) underwent OGTT and MMTT. CGM was performed with blinded FreeStyle Libre Pro for 24 h under standard conditions. Parameters of beta-cell function, insulin sensitivity and GV were calculated.Overall insulin secretion and action as well as GV progressively worsened across glucose tolerance categories. On a matrix analysis, GV parameters were inversely related to ISSI-2; r = -0.37 to -0.52; p 0.0001; and IGI; r = -0.28 to -0.48; p 0.0001 for CV, SD, J-index, LI, HBGI and MAGE. Insulin secretion (IGI) and b-cell function (ISSI-2) emerged as independent contributors to GV in early stage of dysglycemia accounting for about 16%-38% of its variability.Our results show that daily GV worsens already with mild impairment of glucose tolerance. The increase in GV is inversely related to insulin secretion and action.

Themes related to cyberspace have been gaining more relevance in contemporaneity. Youth appears as the protagonist of this process, promoting and producing new cultural expressions in this universe. Among them, we highlight the fanfictions, narratives virtually built by and for fans. This study aims to identify, from the perspective of Epistemologies of the South, how colonialism, patriarchy and capitalism manifest themselves in fanfictions. A qualitative documentary research was made, with reading and analysis of three narratives published in online platforms. We observed the presence of the power structures: patriarchy, expressed by scenes of objectification and violence against women; colonialism, visualized by the presence of only white characters; and capitalism, through the prominence of the lifestyle of the dominant social classes. The results point out that youth productions have been spaces of reproduction of dominant power relations in the current scenario. [ABSTRACT FROM AUTHOR]

Aims As recommended by the Institute of Medicine (IOM), health practitioners should encourage a healthy nutrition and adequate weight gain during pregnancy in order to ensure favorable pregnancy and fetal outcomes, and to prevent diseases later in life for both mother and child. The purpose of this online survey was to determine the knowledge, attitude, and practice of the 2009 IOM recommendations among healthcare professionals managing nutritional therapy in pregnancies complicated by diabetes in Italy. Methods A cross-sectional survey was conducted by using an online self-administered questionnaire undertaken between October and December 2021. Results Of the 220 participants 89% were diabetologists/endocrinologists/internal medicine specialists and 11% dietitians/nutritionists. The survey found that the 53% of respondents provide a personalized diet to pregnant women with diabetes, while 32% a standard diet plan and only 15% healthy dietary advice. The 69% of the participants investigated for appropriate gestational weight gain, mainly based on pre-pregnancy BMI (96%), gestational weight gain (GWG) at first prenatal visit (80%) and presence of twin pregnancy (58%). Maternal weight gain was evaluated at each regularly scheduled prenatal visit and compared with IOM recommendations for the 87% of healthcare professionals. Diet plan was periodically re-evaluated and/or modified (90% of participants), based on inadequate maternal weight gain and/or fetal growth abnormalities (78%), trimester transition (53%), changes in physical activity and/or a “feel hungry” (50%). Conclusions This survey reported the knowledge and attitude of IOM guidelines and the nutritional knowledge and practice of Italian professionals on the nutritional management of diabetes in pregnancy. The application of these recommendations seemed more feasible in clinics/team dedicated to "Diabetes in Pregnancy".

The discovery of insulin in 1921 – due to the efforts of the Canadian research team based in Toronto – has been a landmark achievement in the history of medicine. Lives of people with diabetes were changed forever, considering that in the pre-insulin era this was a deadly condition. Insulin, right after its discovery, became the first hormone to be purified for human use, the first to be unraveled in its amino acid sequence and to be synthetized by DNA-recombinant technique, the first to be modified in its amino acid sequence to modify its duration of action. As such the discovery of insulin represents a pivotal point in medical history. Since the early days of its production, insulin has been improved in its pharmacokinetic and pharmacodynamic properties in the attempt to faithfully reproduce diurnal physiologic plasma insulin fluctuations. The evolution of insulin molecule has been paralleled by evolution in the way the hormone is administered. Once-weekly insulins will be available soon, and glucose-responsive “smart” insulins start showing their potential in early clinical studies. The first century of insulin as therapy was marked by relentless search for better formulations, a search that has not stopped yet. New technologies may have, indeed, the potential to provide further improvement of safety and efficacy of insulin therapy and, therefore, contribute to improvement of the quality of life of people with diabetes.

Os processos educativos têm exigido e possibilitado delinear e vivenciar experiências de formação e cuidado em resposta aos efeitos da atual pandemia. Em consonância com essa afirmação, o presente trabalho tem por objetivo relatar uma experiência formativa com estudantes do curso de Terapia Ocupacional de uma universidade pública durante a pandemia de Covid-19. A experiência foi analisada em dois momentos, nomeados no relato como “Respiros”. No primeiro momento, em ambiente virtual, foram realizadas rodas de conversa-mosaico sem a presença obrigatória das estudantes; no segundo momento, como um módulo opcional no currículo, foram introduzidas práticas artísticas e corporais como dispositivos para o cuidado, expressão e conexão entre os corpos. As estudantes indicaram a importância de se criar um ambiente confiável e de sustentarmos uma proposta coletiva, dando ensejo à construção de corpos sensíveis, inventivos e críticos, fundamentais no exercício profissional.

Frailty is an age-related syndrome that exposes individuals to increased vulnerability. Although it is potentially reversible, in most cases it leads to negative outcomes, including mortality. The different methods proposed identify frailty after the onset of clinical manifestations. An early diagnosis might make it possible to manage the frailty progression better. The frailty pathophysiology is still unclear although mechanisms, in particular, those linked to inflammation and immunosenescence, have been investigated. A common feature of several clinical aspects involved in senescent organisms is the increase of oxidative stress, described as one of the major causes of deoxyribonucleic acid (DNA) damage accumulation in aged cells including the adult stem cell compartment. Likely, this accumulation is implicated in frailty status. The oxidative status of our frail, pre-frail, and non-frail population was characterized. In addition, the DNA damage in hematopoietic cells was evidenced by analyzing the peripheral blood mononuclear cell and their T lymphocyte, monocyte, circulating hematopoietic progenitor stem cell (cHPSC) subpopulations. The phosphorylation of C-terminal of histone H2AX at amino acid Ser 139 (γ-H2AX), which occurs at the DNA double-strand break focus, was evaluated. In our frail population, increased oxidative stress and a high level of DNA damage in cHPSC were found. This study may have potential implications because the increment of DNA damage in cHPSC could be suggestive of an organism impairment preceding the evident frailty. In addition, it may open the possibility for attenuation of frailty progression throughout specific drugs acting on preventing DNA damage or removing damaged cells

MG53 is a myokine modulating insulin signaling in several tissues; its relationship to glucose tolerance or risk of developing type 2 diabetes mellitus (T2DM) is unknown. This observational, prospective study aimed at evaluating the relationship between MG53 and glucose tolerance, testing whether its circulating levels may be associated with disease progression in a cohort at high risk of T2DM.Five hundred and fifteen subjects who underwent a deep characterization of their glucose tolerance in the years 2003-2005 participated in this study. MG53 levels were measured at baseline. Glucose tolerance status was available over a follow-up of 15 ± 2 years for 283 of them; their vital status as of December 2020 was also retrieved.MG53 levels were significantly lower in subjects with normal glucose tolerance than in subjects with impaired glucose regulation (IGR) or T2DM. Individuals in the highest MG53 levels quartile had more frequently 1h-post load glucose ≥ 155 mg/dL (54% vs 39%; p = 0.015), worse proportional control of β-cell function (p 0.05-0.01), as determined by mathematical modeling, and worse Disposition Index (DI) (0.0155 ± 0.0081 vs 0.0277 ± 0.0030; p 0.0001). At follow-up, baseline MG53 levels were higher in progressors than in non-progressors (120.1 ± 76.7 vs 72.7 ± 63.2 pg/ml; p = 0.001; ROC curve area for incident diabetes of 0.704). In a multivariable regression with classic risk factors for T2DM and DI, MG53 remained independently associated with progression with T2DM.MG53 may be a novel biomarker of glucose dysregulation associated with β-cell dysfunction, likely improving our ability to identify, among high-risk subjects, those more likely to develop T2DM.

In spite of being at target for glucose (1) or traditional cardiovascular (CV) risk factors (2), individuals with type 1 diabetes (T1D) still have an excess of CV mortality and morbidity implying a role for other mechanisms including insulin resistance (IR). Impaired insulin action in T1D was established by clamp technique long ago (3). Estimated glucose disposal rate (eGDR) correlates well with the clamp technique (4) and is a risk marker for microangiopathy (5,6), diabetic kidney disease (DKD) (6), CV risk, and mortality (5,7). In this observational single-center study, we investigated to what extent eGDR is a predictor of CV events, coronary artery disease (CAD), and all-cause mortality irrespective of CV risk factors and DKD in 774 subjects with T1D over a 10-year follow-up, as previously described (8). eGDR (mg/kg/min) was calculated at baseline as follows (4): eGDR = 21.158 − (0.09 × WC) − (3.407 × HTN) − (0.551 × HbA1c), where WC is waist circumference (cm), HTN is hypertension (yes = 1, no = 0), and HbA1c is in %. Follow-up data were retrieved from the national and regional health care registers (ICD-9, Clinical Modification, codes) by searching for CV outcomes (primary outcome) up to 31 December 2017 and for all-cause death up to 31 October 2018. Background and aims Screening for Gestational Diabetes (GDM) is usually recommended between 24 and 28 weeks of pregnancy; however available evidence suggests that GDM may be already present before recommended time for screening, in particular among high-risk women as those with prior GDM or obesity. The purpose of this retrospective study was to evaluate whether early screening (16–18 weeks) and treatment of GDM may improve maternal and fetal outcomes. Methods and results In 290 women at high-risk for GDM, we analyzed maternal and fetal outcomes, according to early or standard screening and GDM diagnosis time. Early screening was performed by 50% of high-risk women. The prevalence of GDM was 62%. Among those who underwent early screened, GDM was diagnosed at the first evaluation in 42.7%. Women with early diagnosis were more frequently treated with insulin and had a slightly lower HbA1c than women with who were diagnosed late. No differences were observed in the prevalence of Cesarean section, operative delivery, gestational age at the delivery, macrosomia, neonatal weight, Ponderal Index and Large-for-Gestational-Age among women with early or late GDM diagnosis or NGT. However, compared to NGT women, GDM women, irrespective of the time of diagnosis, had a lower gestational weight gain, lower prevalence of macrosomia (3.9% vs. 11.4%), small (1.7% vs. 8.3%) as well as large for gestational age (3.3% vs. 16.7%), but higher prevalence of pre-term delivery (8.9% vs. 2.7%). Conclusion Early vs. standard screening and treatment of GDM in high-risk women is associated with similar short-term maternal-fetal outcomes, although women with an early diagnosis were treated to a greater extent with insulin therapy.

El artículo aborda el desarrollo teórico-práctico de los conceptos territorio y comunidad en la Terapia Ocupacional colombiana. El objetivo fue comprender el uso y la elaboración de tales conceptos e identificar cómo se expresan en la práctica. La recolección de datos se realizó en dos etapas: primera, revisión conceptual de la literatura publicada entre 1990 y 2020 en revistas y libros de amplia circulación en América Latina, así como entrevistas a dos autoras recurrentes en las publicaciones seleccionadas; segunda, observación participante de una pasantía profesional orientada a estudiantes de pregrado en Terapia Ocupacional de una universidad colombiana y entrevistas a actores sociales involucrados en esa práctica. Se identifica que los términos territorio y comunidad conforman el vocabulario técnico de las y los terapeutas ocupacionales en Colombia; se refieren históricamente a prácticas realizadas en contextos de pobreza, y adquirieron mayor relevancia con el desarrollo de políticas públicas. Se evidencia, además, una actuación territorial-comunitaria relacionada con el modo de vida comunitario y con la apropiación de los recursos materiales, sociales, culturales y relacionales del territorio, desde una perspectiva colectiva de la vida. Se reitera la necesidad de superar el uso de estos conceptos solo como espacios físicos de actuación profesional.

In this observational study, we assessed the association between use of non-nutritive-sweetened soft drink (NNSSD) and risk of gestational diabetes (GDM) in 376 pregnant women consecutively screened for GDM, observing that NNSSD consumption is common among pregnant women and is associated with an increased risk of GDM, independently from traditional risk factors.

Resumo A produção de conhecimentos nacionais da terapia ocupacional se deu de maneira contextualizada em congruência com o desenvolvimento da profissão, em seus diversos campos de atuação. Nas últimas décadas, viu-se a proposição de perspectivas e conceitos ampliados que apontam possíveis interlocuções nessa pluralidade. Nesse sentido, este estudo buscou compreender como a expressão “produção de vida” tem sido utilizada pela literatura nacional da terapia ocupacional, com base em uma revisão bibliográfica de estratégia multidimensional. Consultou-se a base de dados LILACS e periódicos nacionais da área, identificando 29 produções com a expressão “produção de vida” ou semelhante para análise do perfil das publicações, autores, referências e sentidos envolvidos na compreensão da expressão. Os achados se tornaram numericamente mais significativos nos últimos dez anos; estes se relacionavam com temáticas da saúde mental, campo social, artes e cultura, saúde coletiva, fundamentos da terapia ocupacional e campo do trabalho; e os referenciais citaram autores da terapia ocupacional, psiquiatria, filosofia, psicanálise, psicologia, saúde coletiva e antropologia. A análise de sua compreensão indicou quatro categorias: trabalho como emancipação social; ampliação de ações em saúde; experiência criativa e transformação cultural; e manutenção da própria existência. Ressaltando a produção consistente sobre “produção de vida”, concluiu-se que a expressão envolve sentidos na construção de saberes-fazeres mais conectados com o mundo, com as diferentes formas de existir e de protagonizar a vida. Abstract The production of national knowledge of occupational therapy took place in a contextualized way in line with the development of the profession, in its various fields of activity. In the last decades, there were propositions of expanded perspectives and concepts that point to possible interlocutions in this plurality. In this sense, this study sought to understand how the expression “life production” has been used by the national literature of occupational therapy, based on a bibliographic review of multidimensional strategy. The LILACS database and national journals in the area were consulted, identifying 29 productions with the expression “production of life” or similar, to analyze the profile of publications, authors, and references and the meanings involved in understanding the expression. The findings have become numerically more significant over the past ten years; they were related to themes of mental health, social field, arts and culture, collective health, occupational therapy foundations, and work field; and the references cited occupational therapy, psychiatry, philosophy, psychoanalysis, psychology, collective health, and anthropology authors. The analysis of their understanding indicated four categories: work as social emancipation; expanding health actions; creative experience and cultural transformation; and maintenance of existence. Emphasizing the consistent production on the expression 'life production', we concluded that the expression involves meanings in the construction of know-how more connected to the world, the different ways of existing and being the protagonist of life.
