Introduction Children with septo-optic-pituitary dysplasia (SOD) may experience a range of visual impairments and hormonal dysfunctions beyond developmental delay/intellectual disability. The literature describes sleep fragmentation, circadian rhythm disruptions and reduced sleep efficiency. These manifestations are believed to be closely linked to both structural and functional abnormalities associated with SOD, potentially disrupting the natural circadian rhythm. Both anomalies in midline brain structures and decreased visual input could potentially impact melatonin secretion, although a distinct melatonin profile for SOD patients has yet to be identified. Furthermore, the specific contribution of these factors to sleep disturbances in SOD remains unexplored. The aim of this study is to evaluate the quality of sleep and its characteristics, along with the melatonin profile, among paediatric patients diagnosed with SOD. A comparison will be made between these findings and those of children with isolated bilateral visual impairment, as well as patients with agenesis of the corpus callosum.Methods and analysis Participants aged between 3 and 18 years previously diagnosed with SOD will be recruited prospectively. Each participant will be assessed at baseline and at each follow-up visit scheduled to evaluate the clinical course. Sleep quality and daytime sleepiness changes will be tracked using actigraphic assessment, standardised sleep questionnaires and a sleep EEG. Additionally, plasma and salivary melatonin profiles will be assessed for each participant.Ethics and dissemination This study has been approved by local Ethics Committee (N°0049187/23). The study findings will be shared through publication in an international peer-reviewed journal and presented at both national and international conferences.Trial registration number NCT06262152.

The digitization of healthcare records has revolutionized medical research and patient care, with electronic health records (EHRs) containing a wealth of structured and unstructured data. Extracting valuable information from unstructured clinical text presents a significant challenge, necessitating automated tools for efficient data mining. Natural language processing (NLP) methods have been pivotal in this endeavor, aiming to extract crucial clinical concepts embedded within free-form text. Our research addresses the imperative for robust biomedical entity extraction, focusing specifically on inflammatory bowel disease (IBD). Leveraging novel domain-specific pre-training and entity-aware masking strategies with contrastive learning, we fine-tune and adapt a general language model to be better adapted to IBD-related information extraction scenarios. Our named entity recognition (NER) tool streamlines the retrieval process, supporting annotation, correction, and visualization functionalities. In summary, we developed a comprehensive pipeline for clinical Dutch NER encompassing an efficient domain adaptation strategy with domain-aware masking and model fine-tuning enhancements, and an end-to-end entity extraction tool, significantly advancing medical record curation and clinical workflows.

Objetivo: Avaliar a quantificação de armadilhas extracelulares de neutrófilos, TLR2 e TLR4 em crianças com doença falciforme, após estímulo com PMA. Metodologia: Este projeto foi aprovado pelo Comitê de Ética em Pesquisa do Hospital de Clínicas da Universidade Federal do Triângulo Mineiro (CEP – HC UFTM). O grupo de estudo foi composto por 25 crianças com idades de 1 a 13 anos, diagnosticadas com doença falciforme, atendidas no Ambulatório de Hematologia e Hemoterapia do Hemocentro Regional de Uberaba/MG. Foi coletado sangue periférico, por venopunção, em tubos contendo heparina. O isolamento de neutrófilos foi realizado por meio de centrifugação em gradiente de concentração por Ficoll-Paque™. Os neutrófilos isolados foram mantidos por 4 horas em incubadora úmida a 37°C em meio RPMI-1640 contendo 10% de soro fetal bovino, com e sem estímulo por PMA (phorbol myristate acetate). Posteriormente, as amostras foram marcadas com Sytox Green e CD66a para quantificação da netose; CD66a e marcador TLR2 para quantificar o receptor TLR2 e CD66a e marcador TLR4 para quantificar o receptor TLR4. As quantificações da netose e dos receptores foram realizadas por citometria de fluxo em citômetro FACSCanto II. A análise estatística foi realizada utilizando o programa GraphPad Prism e a significância estatística foi definida como p < 0,05. Resultados: O estudo foi composto por 13 (52%) crianças do sexo masculino e 12 (48%) feminino, com mediana de idade de 7 anos (1 – 13). Em relação aos genótipos ,: 11 (44%) crianças apresentavam genótipo HbSS, 10 (40%) genótipo HbSC e 4 (16%) outros genótipos. Houve diferença estatística (p = 0,0001) na avaliação da netose, sendo observada maior expressão no grupo de amostras tratadas com PMA em relação ao grupo de amostras não tratadas. Houve diferença estatística (p = 0,0015) na quantificação do receptor TLR2, sendo observada maior quantidade desse receptor no grupo de amostras tratadas com PMA em relação ao grupo de amostras não tratadas. Houve diferença estatística (p = 0,002) na quantificação do receptor TLR4, sendo observada maior quantidade de receptor TLR4 no grupo de amostras tratadas em relação ao grupo de amostras não tratadas. Discussão: A doença falciforme é um distúrbio hematológico caracterizado por um processo inflamatório crônico. Tem sido demonstrado que a adesão de células falciformes e neutrófilos ao endotélio colaboram com a vaso-oclusão observada em indivíduos com a doença. Estudos apontam que a alta atividade dos neutrófilos em processos infecciosos nestes pacientes, induz a formação de armadilhas extracelulares de neutrófilos (NETS), influenciando na iniciação e evolução de trombos, bem como a ativação de TLR2 e TLR4. A ativação desses dois receptores é capaz de produzir citocinas e moléculas de adesão importantes na inflamação, mas que de maneira exacerbada podem levar a vaso-oclusão. No presente estudo, foi observada maior expressão de netose, TLR2 e TLR4 nas amostras estimuladas pelo PMA. A literatura relata que o PMA induz notavelmente vias semelhantes a outros estímulos, sendo considerado um bom estímulo artificial para estudos com neutrófilos, conforme demonstrado no presente estudo. Conclusão: Os resultados desse estudo sugerem que o PMA é eficaz no papel de estimulador de netose, TLR2 e TLR4.

Objetivos: Atualmente, o emprego de inibidores de tirosina quinase (TKIs) constitui uma importante opção terapêutica para neoplasias hematológicas. O TKI-258 é um inibidor multi-alvos em fase de estudos que apresenta potencial como quimioterápico para diversos tipos de neoplasias. O índice de seletividade (IS) é um parâmetro bem estabelecido para avaliar a janela entre a citotoxicidade e o efeito antineoplásico de diferentes drogas. O objetivo deste estudo é determinar os índices de citotoxicidade do TKI-258 em diferentes linhagens celulares oncohematológicas e avaliar a seletividade da droga às células tumorais em relação a linfócitos de indivíduos saudáveis. Material e métodos: Foram utilizadas células das linhagens K-562 de leucemia mieloide crônica (LMC), Jurkat de leucemia linfoide aguda (LLA) e Raji de linfoma de Burkitt, bem como células mononucleares de sangue periférico (PBMCs) separadas a centrifugação em gradiente de concentração por Ficoll-Paque™. As células foram mantidas em incubadora úmida a 37°C em meio RPMI-1640 contendo 10% de soro fetal bovino, penicilina e estreptomicina 100 UI/mL e, então, 1x105 células controle e tratadas com TKI-258, em concentrações de 0,25 μM, 0,5 μM, 1 μM, 2 μM e 4 μM, foram semeadas por poço e mantidas por 36 horas. Os índices de citoxicidade (IC50) foram determinados por citometria de fluxo por meio da marcação com iodeto de propídio, marcador de morte celular, e calculados a partir de testes de regressão não linear. O programa GraphPad Prism 9.0.0 foi utilizado para as análises estatísticas. Os IS's foram obtidos a partir da razão entre a citotoxicidade para PBMCs (CC50) e para células neoplásicas (IC50). Resultados: O CC50 do TKI-258 em PBMCs foi de 1,702 (r = 0,9696). Em células K562, o IC50 foi de 2,229 (r = 0,8286); em células Jurkat, de 1,850 (r = 0,8867); e em células Raji, de 0,7623 (r = 0,8246). O índice de seletividade da droga para as diferentes linhagens foi de 0,76, 0,92 e 2,23, respectivamente. Discussão: Visto que as drogas disponíveis para tratamento de neoplasias hematológicas são passíveis de falha, resistência medicamentosa e efeitos adversos importantes, o desenvolvimento de novas terapias é fundamental. Os TKIs alvo-específicos constituem terapia padrão ouro para a LMC, porém poucos estudos visam avaliar a viabilidade da inibição dos receptores de tirosinaquinase como estratégia terapêutica para outros cânceres hematológicos, como LLA e Linfoma de Burkitt. O TKI-258 é um inibidor multi-alvos que apresentou, neste estudo, baixa seletividade para as linhagens de LMC e LLA, porém foi significativamente seletivo para as células de Linfoma de Burkitt, apresentando efeito antioncogênico maior que o efeito citotóxico. Conclusão: O estudo demonstrou que o TKI-258 é eficaz para indução de morte celular de forma seletiva para células Raji de Linfoma de Burkitt, embora a droga não tenha tido bom desempenho para as outras neoplasias testadas. O bom índice de seletividade da droga na linhagem de Burkitt nos testes in vitro pode indicar uma menor incidência de efeitos colaterais relacionados à imunossupressão com o uso in vivo. Desse modo, novos estudos devem ser considerados para avaliar o potencial terapêutico do TKI-258 para o Linfoma de Burkitt.

Objetivos: O linfoma de Burkitt é caracterizado por ser uma forma agressiva de linfoma de células B e seu tratamento é realizado por meio de quimioterapia, mas não utiliza inibidores de receptores tirosinaquinase. O TKI-258 é um inibidor de múltiplos receptores tirosinaquinase, ainda em fase de estudos, já testado em diferentes tumores sólidos. O objetivo deste estudo foi avaliar os índices de morte celular em co-culturas de células mononucleares do sangue periférico (PBMCs) e células de linhagem tumoral de Linfoma de Burkitt (RAJI) após tratamento com interleucina-2 (IL-2), TKI-258 e IL-2 em conjunto com TKI-258. Materiais e métodos: Foram utilizadas PBMCs, extraídas por meio da centrifugação com Ficoll-Paque™, e células imortalizadas da linhagem RAJI de Linfoma de Burkitt. As células foram preservadas em incubadora úmida em meio RPMI-1640 a 37 °C, contendo penicilina/estreptomicina e 10% de soro fetal bovino. O índice de citotoxicidade (IC50) do TKI-258 foi determinado previamente por meio citometria de fluxo e calculado pelo teste de regressão não linear, sendo este de 1,5 μM. Em placa de 96 poços foi realizada a co-cultura das PBMCs com as células RAJI, na proporção de 1×106 para 1×105 células por poço, respectivamente. Em seguida, as co-culturas foram divididas em: não tratadas, tratadas com IL-2 1000UI; TKI-258 1,5 μM e TKI-258 1,5 μM mais IL-2 1000UI, e mantidas em incubadora por 4 horas. O percentual de morte celular das co-culturas foi avaliado por citometria de fluxo com marcação com iodeto de propídeo. A análise estatística foi realizada pelo programa GraphPad Prism 9.0.0 sendo considerado estatisticamente significativo quando p < 0,05. Resultados: Foram encontradas diferenças significativas entre o grupo não tratado e os demais (RM one-way ANOVA, F(3,39) = 35.04; p < 0.0001). Não foram observadas diferenças entre os grupos não tratado e tratado com IL-2 (p = 0.9293). O grupo tratado com TKI-258 apresentou um aumento na taxa de mortalidade das células neoplásicas comparado aos grupos anteriores (NTxTKI p < 0.0001; IL-2xTKI p < 0.0001). A maior taxa de mortalidade de células neoplásicas foi encontrada nas amostras tratadas com TKI-258 em associação com IL-2 (NTxIL-2+TKI p < 0.0001; IL-2xIL-2+TKI p < 0.0001; TKIxIL-2+TKI p < 0.0017). Discussão: A associação do TKI-258 com IL-2 apresentou resultados superiores na morte de células neoplásicas e na sobrevida das PBMCs quando comparado aos demais grupos analisados. Diante disso, a IL-2 parece favorecer a atuação desse fármaco, e permitindo a sua atuação em diversas vias simultaneamente, uma vez que, este é um inibidor multi-alvos. Conclusão: O estudo evidenciou que o TKI-258 em associação com a IL-2 foi capaz de produzir uma maior taxa de mortalidade de células da linhagem RAJI quando comparadas com os demais grupos. Novos ensaios com este inibidor em associação ou não à IL-2, para o tratamento para Linfoma de Burkitt são necessários, uma vez que na atualidade, os inibidores de receptores tirosina quinase não são utilizados e os nossos resultados evidenciam o seu potencial terapêutico para o tratamento desta neoplasia.

The Oscillating Water Column (OWC) represents a highly promising approach for wave energy conversion. This study presents laboratory experiments conducted on a fixed, bottom-detached OWC device to evaluate the impact of various design parameters (specifically, turbine damping, front wall draft, and chamber length in the direction of wave propagation) on the device’s capture width ratio. Despite the extensive research over the past few decades on OWC devices, most studies and field-tested prototypes have been designed for long-fetch sea conditions. Consequently, these devices tend to be larger in size and have higher rated power outputs. In contrast, short-fetch sea conditions necessitate tuning the OWC to the shorter dominant wave frequencies, which calls for the development of smaller devices and specialized turbines, highlighting the need for focused research. This work specifically addresses short-fetch sea conditions, which are representative of moderate wave climates, such as those found in the central Mediterranean region. The study identifies a maximum capture width ratio of approximately 73%. The experimental dataset generated can serve as a benchmark for numerical models under these specific conditions and assist in the development of air turbines optimized for effective performance in short-fetch wave climates.

ObjectiveEligibility for clinical trials in osteoarthritis (OA) is usually limited to Kellgren-Lawrence (KL) grades 2 and 3 knees. Our aim was to describe the prevalence and severity of cartilage damage in KL 2 and 3 knees by compartment and articular subregion.DesignThe Multicenter Osteoarthritis (MOST) study is a cohort study of individuals with or at risk for knee OA. All baseline MRIs with radiographic disease severity KL2 and 3 were included. Knee MRIs were read for cartilage damage in 14 subregions. We determined the frequencies of no, any and widespread full-thickness cartilage damage by knee compartment, and the prevalence of any cartilage damage in 14 articular subregions.Results665 knees from 665 participants were included (mean age 63.8 ± 7.9 years, 66.5% women). 372 knees were KL2 and 293 knees were KL3. There was no cartilage damage in 78 (21.0%) medial tibio-femoral joint (TFJ), 157 (42.2%) lateral TFJ and 62 (16.7%) patello-femoral joint (PFJ) compartments of KL2 knees, and 17 (5.8%), 115 (39.3%) and 35 (12.0%) compartments, respectively, of KL3 knees. There was widespread full-thickness damage in 94 (25.3%) medial TFJ, 36 (9.7%) lateral TFJ and 176 (47.3%) PFJ compartments of KL2 knees, and 217 (74.1%), 70 (23.9%) and 104 (35.5%) compartments, respectively, of KL3 knees. The subregions most likely to have any damage were central medial femur (80.5%), medial patella (69.8%) and central medial tibia (69.9).ConclusionsKL2 and KL3 knees vary greatly in cartilage morphology. Heterogeneity in the prevalence, severity and location of cartilage damage in in KL2 and 3 knees should be considered when planning disease modifying trials for knee OA.

ObjectiveTo determine reliability among four experienced and calibrated readers in cross-sectional and longitudinal semi-quantitative MRI assessments of knee osteoarthritis (OA) in the Multicenter Osteoarthritis (MOST) study.DesignFrom all MOST participants with at least one knee with readable 60-month and 84-month paired knee MRIs (1.0 ​T extremity systems), we selected 10 subjects having a spectrum of baseline disease severity of cartilage, bone marrow lesions, and meniscal damage and a spectrum of longitudinal changes in severity at 24 months follow-up. MRIs were independently assessed using the WORMS grading system by four musculoskeletal radiologists with the chronological sequence known to the readers. Kappa statistics were used to determine agreement between each pair of readers and Kendall's coefficient of concordance to determine average agreement across readers.ResultsFor most features, cross-sectional reliability was substantial to almost perfect. Regarding longitudinal reliability (detection of longitudinal change), inter-reader reliability as weighted kappa values ranged from 0.62 to 0.78 for cartilage damage, 0.75-0.88 for bone marrow lesions, 0.75-0.92 for meniscal tears, 0.67-0.95 for meniscal extrusion, 0.51-0.77 for bone attrition, 0.43-0.76 for osteophytes, 0.31-0.70 for Hoffa-synovitis, and 0.47-0.85 for effusion-synovitis. Kendall's coefficient ranged from 0.65 to 0.98.ConclusionHigh levels of cross-sectional reliability and moderate to high longitudinal reliability was achieved using four experienced readers in semiquantitative MRI-assessment of most knee OA features.

Abstract Biomarker-based differential diagnosis of the most common forms of dementia is becoming increasingly important. Machine learning (ML) may be able to address this challenge. The aim of this study was to develop and interpret a ML algorithm capable of differentiating Alzheimer’s dementia, frontotemporal dementia, dementia with Lewy bodies and cognitively normal control subjects based on sociodemographic, clinical, and magnetic resonance imaging (MRI) variables. 506 subjects from 5 databases were included. MRI images were processed with FreeSurfer, LPA, and TRACULA to obtain brain volumes and thicknesses, white matter lesions and diffusion metrics. MRI metrics were used in conjunction with clinical and demographic data to perform differential diagnosis based on a Support Vector Machine model called MUQUBIA (Multimodal Quantification of Brain whIte matter biomArkers). Age, gender, Clinical Dementia Rating (CDR) Dementia Staging Instrument, and 19 imaging features formed the best set of discriminative features. The predictive model performed with an overall Area Under the Curve of 98%, high overall precision (88%), recall (88%), and F1 scores (88%) in the test group, and good Label Ranking Average Precision score (0.95) in a subset of neuropathologically assessed patients. The results of MUQUBIA were explained by the SHapley Additive exPlanations (SHAP) method. The MUQUBIA algorithm successfully classified various dementias with good performance using cost-effective clinical and MRI information, and with independent validation, has the potential to assist physicians in their clinical diagnosis.

ABSTRACT Large hiatal hernias, besides being more prevalent in the elderly, have a different clinical presentation: less reflux, more mechanical symptoms, and a greater possibility of acute, life-threatening complications such as gastric volvulus, ischemia, and visceral mediastinal perforation. Thus, surgical indications are distinct from gastroesophageal reflux disease-related sliding hiatal hernias. Heartburn tends to be less intense, while symptoms of chest pain, cough, discomfort, and tiredness are reported more frequently. Complaints of vomiting and dysphagia may suggest the presence of associated gastric volvulus. Signs of iron deficiency and anemia are found. Surgical indication is still controversial and was previously based on high mortality reported in emergency surgeries for gastric volvulus. Postoperative mortality is especially related to three factors: body mass index above 35, age over 70 years, and the presence of comorbidities. Minimally invasive elective surgery should be offered to symptomatic individuals with good or reasonable performance status, regardless of age group. In asymptomatic and oligosymptomatic patients, besides obviously identifying the patient’s desire, a case-by-case analysis of surgical risk factors such as age, obesity, and comorbidities should be taken into consideration. Attention should also be paid to situations with greater technical difficulty and risks of acute migration due to increased abdominal pressure (abdominoplasty, manual labor, spastic diseases). Technical alternatives such as partial fundoplication and anterior gastropexy can be considered. We emphasize the importance of performing surgical procedures in cases of large hiatal hernias in high-volume centers, with experienced surgeons.

Previous research in the evolutionary and psychological sciences has suggested that markers or tags of ethnic or group membership may help to solve cooperation and coordination problems. Cheating remains, however, a problem for these views, insofar as it is possible to fake the tag. While evolutionary psychologists have suggested that humans evolved the propensity to overcome this free rider problem, it is unclear how this module might manifest at the group level. In this study, we investigate the degree to which native and non-native speakers of accents – which are candidates for tags of group membership – spoken in the UK and Ireland can detect mimicry. We find that people are, overall, better than chance at detecting mimicry, and secondly we find substantial inter-group heterogeneity, suggesting that cultural evolutionary processes drive the manifestations of cheater detection. We discuss alternative explanations and suggest avenues of further inquiry.

ABSTRACTGeomorphological field surveys and mapping have been carried out in a catchment of the Dolomites (eastern Italian Alps) as part of a research project aiming at the assessment of sediment availability for mass wasting in mountain environments. The study area is the Liera catchment which was severely affected by a high-intensity, regional-scale storm named ‘Vaia’ (27–30 October 2018), which induced relevant morphological changes, caused a major flood in the main stream and triggered debris flows in many tributaries. This contribution gives a snapshot of the present-day state of the study area to improve the understanding of such a landscape by characterizing the processes that control its geomorphological spatio-temporal dynamics and evolution.

RESUMO A dupla carreira é o desenvolvimento simultâneo de duas carreiras em áreas distintas, por exemplo, a esportiva e a acadêmica. O objetivo deste estudo foi analisar o grau acadêmico de atletas mulheres de futsal do estado de São Paulo, tanto em nível de elite, quanto de categorias de base. Aplicamos questionários sobre as carreiras acadêmicas e esportivas a atletas adultas e das categorias sub-15 e sub-17 da Liga Paulista de Futsal entre os anos de 2019 e 2021, além de informações sobre os graus acadêmicos de seus pais e mães. A carreira no futsal não se mostrou uma barreira à trajetória escolar das atletas, considerando seus graus acadêmicos acima da média populacional e em convergência com suas idades, além da oferta de bolsas de estudos relacionadas ao desempenho esportivo.
