The Protecting Access to Medicare Act mandates clinical decision support (CDS) at imaging order entry, necessitating the use of structured indications to map CDS scores. We evaluated the performance of a commercially available artificial intelligence (AI) tool leveraging free-text order entry to facilitate provider selection of the necessary structured indications.Our institution implemented an AI tool offering predicted structured indications based upon the ordering provider's entry of a free-text reason for examination. Providers remained able to order via the traditional direct search for structured indications. Alternatively, they could take the new free-text-AI approach allowing them to select from AI-predicted indications, perform additional direct searches, indicate no matching indication, or exit CDS workflow. We hypothesized the free-text-AI approach would be elected more often and the AI tool would be successful in facilitating selection of structured indications. We reviewed advanced imaging orders (n = 40,053) for the first 3 months (February to May 2020) since implementation.Providers were more likely (P.001) to choose the free-text-AI approach (23,580; 58.9%) to order entry over direct search for structured indications (16,473; 41.1%). The AI tool yielded alerts with predicted indications in 91.7% (n = 21,631) of orders with free text. Ultimately, providers chose AI-predicted indications in 57.7% (n = 12,490) of cases in which they were offered by the tool.Providers significantly more often elected the new free-text-AI approach to order entry for CDS, suggesting provider preference over the traditional approach. The AI tool commonly predicted indications acceptable to ordering providers.

Purpose The aim of this study was to determine if a clinical decision support (CDS) tool could be used in partnership with a private payer to successfully expedite the prior authorization process for advanced (ie, MRI, CT, PET, nuclear medicine) imaging requests. Methods A single academic institution integrated a commercially available CDS tool utilizing the ACR Appropriateness Criteria into the ordering process for outpatient advanced imaging tests within the electronic health record. Ordering providers could elect to use the CDS tool or ignore the available technology. In partnership with a health care insurance company and a contracted radiology benefits management company, orders deemed as “usually indicated” by the CDS tool underwent expedited prior authorization in a pilot program from June 2018 to October 2019. Results Providers used the CDS tool for 15% (1,453 of 9,640) of outpatient advanced imaging orders. Of these orders with elective CDS scores, 69% (n = 997) qualified for an expedited prior authorization process. Conclusions Under specific circumstances, a commercially available CDS tool was used in partnership with a private payer and a radiology benefits management company to expedite prior authorization of outpatient advanced imaging examination orders deemed likely to be appropriate by multispecialty professional guidelines.

Despite technological advances in the analysis of digital images for medical consultations, many health information systems lack the ability to correlate textual descriptions of image findings linked to the actual images. Images and reports often reside in separate silos in the medical record throughout the process of image viewing, report authoring, and report consumption. Forward-thinking centers and early adopters have created interactive reports with multimedia elements and embedded hyperlinks in reports that connect the narrative text with the related source images and measurements. Most of these solutions rely on proprietary single-vendor systems for viewing and reporting in the absence of any encompassing industry standards to facilitate interoperability with the electronic health record (EHR) and other systems. International standards have enabled the digitization of image acquisition, storage, viewing, and structured reporting. These provide the foundation to discuss enhanced reporting. Lessons learned in the digital transformation of radiology and pathology can serve as a basis for interactive multimedia reporting (IMR) across image-centric medical specialties. This paper describes the standard-based infrastructure and communications to fulfill recently defined clinical requirements through a consensus from an international workgroup of multidisciplinary medical specialists, informaticists, and industry participants. These efforts have led toward the development of an Integrating the Healthcare Enterprise (IHE) profile that will serve as a foundation for interoperable interactive multimedia reporting.

Diagnostic and evidential static image, video clip, and sound multimedia are captured during routine clinical care in cardiology, dermatology, ophthalmology, pathology, physiatry, radiation oncology, radiology, endoscopic procedural specialties, and other medical disciplines. Providers typically describe the multimedia findings in contemporaneous electronic health record clinical notes or associate a textual interpretative report. Visual communication aids commonly used to connect, synthesize, and supplement multimedia and descriptive text outside medicine remain technically challenging to integrate into patient care. Such beneficial interactive elements may include hyperlinks between text, multimedia elements, alphanumeric and geometric annotations, tables, graphs, timelines, diagrams, anatomic maps, and hyperlinks to external educational references that patients or provider consumers may find valuable. This HIMSS-SIIM Enterprise Imaging Community workgroup white paper outlines the current and desired clinical future state of interactive multimedia reporting (IMR). The workgroup adopted a consensus definition of IMR as “interactive medical documentation that combines clinical images, videos, sound, imaging metadata, and/or image annotations with text, typographic emphases, tables, graphs, event timelines, anatomic maps, hyperlinks, and/or educational resources to optimize communication between medical professionals, and between medical professionals and their patients.” This white paper also serves as a precursor for future efforts toward solving technical issues impeding routine interactive multimedia report creation and ingestion into electronic health records.

Purpose To determine if radiologists find enough value in available interactive multimedia reporting technology to routinely adopt it into clinical practice. Materials and Methods Our institution’s reporting application (Vue Reporting, Carestream Health) allows the incorporation of multimedia elements, including active hyperlinks, into clinical reports, but would radiologists find enough value in this technique to change their practice? We retrospectively reviewed 559,841 diagnostic reports issued July 2016 to February 2018 for the presence of text hyperlinks that interactively connect to imaging findings in the PACS. Results were subdivided by modality, reporting radiologist role (ie, resident, fellow, attending physician), and subspecialty. Average percentages over the final 6 months were chosen to represent established adoption rates. Results For each modality, the 6-month average percentages of reports containing hyperlinks to imaging findings subdivided by the role of the radiologist who created the report were found to be as follows: CT: residents = 27.6%, fellows = 19.5%, attending physicians = 26.0%; MRI: residents = 26.6%, fellows = 8.7%, attending physicians = 5.1%; and PET/CT: residents = 53.3%, fellows = 46.7%, attending physicians = 19.4%. Rates were 0% to 4% among ultrasound, radiography, and nuclear medicine reports, regardless of radiologist role. The 6-month average percentages of CT and MRI reports with hyperlinks to imaging findings varied by subspecialty from 5.4% to 57.1%. Conclusion Our radiologists found enough value in available interactive multimedia reporting technology to adopt it into their clinical practice, commonly inserting hyperlinks into their CT, PET/CT, and MRI reports to create interactive connections to key imaging findings in the PACS.

Purpose Clinical decision support (CDS) software designed around the ACR Appropriateness Criteria assists health care providers in choosing appropriate imaging studies at the time of order entry. The goal of this study was to determine the impact of commercially available CDS on the ordering habits of inpatient and emergency providers. Methods In 2014, ACR Select was integrated into our electronic health record, though without displaying appropriateness scores in a “silent” mode for 6 months. Then, feedback regarding examination appropriateness was “turned on” at order entry for adult patients in the emergency and inpatient settings for 24 months. We retrospectively compared the appropriateness scores of imaging tests before and after displaying feedback at order entry and evaluated these data by modality and attending versus trainee status. Results The commercially available CDS-generated scores for 34% and 20.4% of pre- and postintervention studies, respectively. After feedback, the relative frequency of low utility studies decreased to 5.4% from 11%, and the relative frequency of indicated studies increased to 82% from 64.5%. This was most pronounced in trainees for whom the percentage of low utility studies decreased from 10.8% (95% confidence interval [CI]: 10.0%, 11.7%) to 4.8% (95% CI: 4.4%, 5.2%) and the percentage of indicated studies increased from 65.6% (95% CI: 64.3%, 66.9%) to 83.7% (83.0%, 84.3%). Conclusions After implementation of a commercially available decision support tool integrated into the electronic health record, there was a significant improvement in imaging study appropriateness scores, more pronounced in studies ordered by trainees.

The purpose of this study was to compare the performance of direct CT arthrography performed after a moderate injection delay with that of MR arthrography performed soon after injection in the diagnosis of meniscal tears.Twenty-five patients underwent direct MR arthrography followed by same-day CT arthrography of the knee. Two blinded musculoskeletal radiologists independently reviewed the MR and CT arthrographic images for the presence of medial or lateral meniscal tears in the anterior horn, body, and posterior horn. Their readings were compared with a reference standard reading, which was based on post-MRI arthroscopic findings (n = 11) or the consensus opinion of two other musculoskeletal radiologists who simultaneously reviewed the paired CT and MR arthrographic examinations using all available clinical, surgical, and imaging information. The individual and combined radiologist agreements with the reference standard were calculated for each modality.Interreader agreement was 91% for MR arthrography and 85% for CT arthrography. The overall combined radiologist agreement with the reference standard was 91% for MR arthrography and 86% for CT arthrography (p = 0.03). For the two readers, the overall accuracy rates for diagnosing a meniscal tear were 82% and 88% with MR arthrography and 74% and 76% with CT arthrography. Only 50% (3/6) of surgically proven tears imbibed gadolinium.Although MR arthrography performed soon after the contrast injection had higher interreader agreement and greater accuracy, CT arthrography performed after a mean postinjection delay of 100 minutes was moderately accurate in the diagnosis of meniscal tears and can be used as an alternative procedure when MR arthrography cannot be completed.

Our institution implemented a read priority scoring system to combat the known limitations of traditional methods for the prioritization of examination interpretations by radiologists. We aimed to determine the impact on report turnaround time (RTAT) and RTAT variability.On examination completion, technologists entered a read priority score (1-9) using provided definitions. We retrospectively reviewed the median RTAT and RTAT variability (i.e., interquartile range length) for radiology examinations (n = 615,541; 2011-2014). We used Spearman correlation coefficients to determine the relationships between read priority scores and the median RTAT and the RTAT variability by year. We compared median RTAT and RTAT variability between early (2011) versus late (2012-2014) adoption phases using distribution-free random permutation tests.Ranked correlations showed yearly improvement, leading to a near-perfect ranking of median RTAT (r = 0.98, p0.001) and a perfect ranking of RTAT variability (r = 1.00, p0.001) by nine levels of priority. Eight of the nine priority levels showed a reduction in median RTAT between the early and late phases, and the three most urgent levels--that is, 1, 2, and 3--improved by 23%, 5%, and 70% (all, p0.001), respectively. Only one priority level (4, defined as outpatient urgent [8% of studies]) showed significant worsening by 15% (p0.001). The three most urgent levels of priority also showed improvements in RTAT variability (61%, 17%, 71%, respectively; all, p0.01). Only the lowest level of priority (9) exhibited a significant worsening in RTAT variability by 9% (p0.01).A reading priority scoring system with defined clinical scenarios yielded desirable prioritization of examination interpretations by radiologists as evidenced by appropriate and improved stratification of RTATs and RTAT variability.

In most settings, radiologists maintain a high-throughput practice in which efficiency is crucial. The conversion from film-based to digital study interpretation and data storage launched the era of PACS-driven workflow, leading to significant gains in speed. The advent of electronic health records improved radiologists' access to patient data; however, many still find this aspect of workflow to be relatively cumbersome. Nevertheless, the ability to guide a diagnostic interpretation with clinical information, beyond that provided in the examination indication, can add significantly to the specificity of a radiologist's interpretation. Responsibilities of the radiologist include, but are not limited to, protocoling examinations, interpreting studies, chart review, peer review, writing notes, placing orders, and communicating with referring providers. Most of the aforementioned activities are not PACS-centric and require a login to one or more additional applications. Consolidation of these tasks for completion through a single interface can simplify workflow, save time, and potentially reduce the incidence of errors. Here, the authors describe diagnostic radiology workflow that leverages the electronic health record to significantly add to a radiologist's ability to be part of the health care team, provide relevant interpretations, and improve efficiency and quality.

Due to mandates from recent legislation, clinical decision support (CDS) software is being adopted by radiology practices across the country. This software provides imaging study decision support for referring providers at the point of order entry. CDS systems produce a large volume of data, providing opportunities for research and quality improvement. In order to better visualize and analyze trends in this data, an interactive data visualization dashboard was created using a commercially available data visualization platform. Following the integration of a commercially available clinical decision support product into the electronic health record, a dashboard was created using a commercially available data visualization platform (Tableau, Seattle, WA). Data generated by the CDS were exported from the data warehouse, where they were stored, into the platform. This allowed for real-time visualization of the data generated by the decision support software. The creation of the dashboard allowed the output from the CDS platform to be more easily analyzed and facilitated hypothesis generation. Integrating data visualization tools into clinical decision support tools allows for easier data analysis and can streamline research and quality improvement efforts.

Extraosseous Gaucher cell deposits are a rare complication of Gaucher disease that can mimic malignancy. We describe a case of Gaucher cell deposition in the subcutaneous soft tissues overlying the lower thoracic spine in an 18-year-old woman with known type III Gaucher disease. This case is unique in the literature because this subcutaneous Gaucher mass was not associated with extension from underlying bone involvement or clear lymph node origin. It demonstrated no discernible continuity with the adjacent thoracic spinous processes, the cortices of which appeared intact. Although patients with Gaucher disease are at increased risk of malignancy, Gaucher cell deposition should remain a differential consideration for soft tissue masses with or without adjacent bone involvement in patients with known Gaucher disease.

Intraneural perineurioma is a benign peripheral nerve neoplasm that typically affects teenagers and young adults and tends to result in a motor-predominant neuropathy. The lesion is rare, but has likely been underdiagnosed due to a lack of familiarity among both clinicians and radiologists. There have been few reports in the radiology literature despite the lesion having a fairly characteristic imaging appearance. We report a case of a 26-year-old woman with an intraneural perineurioma of the left sciatic nerve confirmed with excisional biopsy and pathologic analysis.

The purpose of this study was to present results of single-stage revision anterior cruciate ligament (ACL) reconstruction using an allograft bone dowel for isolated femoral bony deficiency.Sixteen patients underwent single-stage revision ACL reconstruction using an allograft bone dowel for isolated femoral bony deficiency between 2007 and 2012. Twelve patients (75%) completed study visits, which included CT scans as well as completion of validated outcomes measures.The average KT-1000 side-to-side difference was 1.0 mm ± 2.9 mm. The average International Knee Documentation Committee score was 70.2 ± 17.8, the Tegner score was 4.8 ± 2.8, and the visual analog scale pain score was 2.8 ± 2.4. An analysis of CT scans showed that all 12 dowels had excellent (75%) incorporation.A single-stage approach for revision ACL reconstruction using allograft dowels for isolated femoral bony deficiency yields objective and subjective outcomes comparable to those reported in the literature for two-stage and other single-stage techniques, with good incorporation of the dowels.Retrospective case series, level IV.

To determine trends in incidentally detected age- and gender-associated chondrocalcinosis on pelvic CT. Twenty patients of each gender at the center of each decade of life who underwent a CT scan of the pelvis performed 2009–2013 were identified and selected for a total of 400 pelvic CTs. Images were reviewed independently by two radiologists for the presence or absence of chondrocalcinosis within the pelvis. Patients with hip or low back pain, known CPPD arthropathy or any known predisposing condition, prior hip arthroplasty, or articular fracture were excluded. Logistic regression was used to predict the presence/absence of chondrocalcinosis as a function of patient age and gender. The presence/absence of chondrocalcinosis was found to be associated with patient age (p = 0.016) but not patient gender (p = 0.929). In the pelvis, chondrocalcinosis was most frequently identified at the pubic symphysis. Incidental chondrocalcinosis was not identified in any patients under 50 years of age. Chondrocalcinosis increased in frequency from 12.5 at 55 years of age to 27.5 % of patients 95 years of age. Chondrocalcinosis is common and more prevalent in late adulthood, occurs without a gender predilection, and is infrequently identified in patients younger than 50 years of age.

Background: During anatomic anterior cruciate ligament (ACL) reconstruction, we have found that the femoral footprint can best be visualized from the anteromedial portal. Independent femoral tunnel drilling can then be performed through an accessory medial portal, medial and inferior to the standard anteromedial portal. Purpose: To compare the accuracy of independent femoral tunnel placement relative to the ACL footprint using an accessory medial portal versus tunnel placement with a traditional transtibial technique. Study Design: Controlled laboratory study. Methods: Ten matched pairs of cadaveric knees were randomized such that within each pair, one knee underwent arthroscopic transtibial (TT) drilling, and the other underwent drilling through an accessory medial portal (AM). All knees underwent computed tomography (CT) both preoperatively and postoperatively with a technique optimized for ligament evaluation (80 keV with maximum mAs). Computed tomography was performed with a dual-energy scanner. Commercially available third-party software was used to fuse the preoperative and postoperative CT scans, allowing anatomic comparison of the ACL footprint to the drilled tunnel. The ACL footprint was marked in consensus by an orthopaedic surgeon and a musculoskeletal radiologist and then compared with the tunnel aperture after drilling. The percentage of tunnel aperture contained within the native footprint as well as the distance from the center of the tunnel aperture to the center of the footprint was measured. Results: The AM technique placed 97.7% ± 5% of the tunnel within the native femoral footprint, significantly more than 61.2% ± 24% for the TT technique ( P = .001). The AM technique placed the center of the femoral tunnel 3.6 ± 1.2 mm from the center of the native footprint, significantly closer than 6.0 ± 1.9 mm for the TT technique ( P = .003). Conclusion: This study demonstrates that use of an accessory medial portal will facilitate more accurate placement of the femoral tunnel in the native ACL femoral footprint. Clinical Relevance: More accurate placement of the femoral tunnel in the native ACL femoral footprint should improve the ability to achieve more anatomic positioning of the ACL graft.

Conventional MRI is limited for characterizing the posterolateral corner of the knee due to the region's anatomic variability and complexity; further, MRI is a static study and cannot demonstrate pathologic laxity. Stress radiography may provide additional information about instability.We therefore (1) correlated varus stress radiography with MRI findings, (2) compared opening in patients who underwent surgical posterolateral corner stabilization versus those who did not, and (3) determined whether stress radiography findings could supplement MRI for making treatment decisions.We retrospectively studied 26 patients (27 knee injuries) and correlated lateral compartment opening on varus stress radiography with severity of posterolateral corner injury on MRI. We compared radiographic findings in 18 patients with complete injuries who underwent posterolateral corner stabilization with five who did not.A complete posterolateral corner injury on MRI was associated with an average of 18.6 mm (10.0-36.5 mm) of varus opening versus 12.8 mm (7.5-17.0 mm) in partial injuries. Opening in operative cases that underwent stabilization was 16.5 mm (11.0-36.5 mm) versus 11.0 mm (7.5-13.5 mm) for those that did not. Ten of 15 partial injuries underwent stabilization, for which the varus opening was 13.6 mm (11.0-17.0 mm). Average varus opening in partial injuries that did not undergo stabilization was 11.0 mm (7.5-13.5 mm).Varus stress radiography correlated to MRI findings for posterolateral corner injury. The injuries we treated with reconstruction were associated with increased varus opening. In patients with partial posterolateral corner injury on MRI, we used degree of opening on varus stress radiography to aid the decision for stabilization.Level IV, diagnostic study. Scurvy is a lethal but treatable disease that is rare in industrialized countries. Caused by vitamin C deficiency, it is most prevalent in persons of low socioeconomic status and smokers. Low levels of circulating vitamin C result in poor collagen fiber formation that, in turn, leads to demineralized bones, microfractures, and poor healing. Here we report a case of scurvy in a 5-year-old boy with normal radiographs in whom initial concern for leukemia based upon magnetic resonance imaging and clinical presentation led to a bone marrow biopsy revealing gelatinous transformation.

Calcific myonecrosis is an uncommon late sequela of limb trauma that results in calcification and necrosis of damaged muscle. It almost exclusively occurs in the lower extremity. We present a second case of calcific myonecrosis in the upper extremity.

The purpose of this article is to evaluate the accuracy of unenhanced T1-weighted MR images in predicting the vascular status of the proximal pole of the scaphoid in patients with chronic scaphoid fracture nonunions.A database search identified 29 patients with chronic scaphoid nonunions who underwent a preoperative MRI examination and intraoperative assessment of scaphoid viability from 2004 to 2009. T1-weighted MR images were evaluated by two musculoskeletal radiologists. If the proximal pole demonstrated diffusely decreased T1-weighted signal (less than or equal to that of skeletal muscle), the patient was placed in a moderate-to-high risk for avascular necrosis (AVN) category. Otherwise, the patient was placed in a viable-to-low risk for AVN category. Scaphoid viability or necrosis was diagnosed intraoperatively depending on whether punctate bleeding was present. After the patients were classified according to the T1-weighted appearance, the appearance on STIR images was recorded.There were 29 patients (25 male) with a mean age of 21 years. When we compared the MRI results, using only the T1-weighted images, with the surgical findings, unenhanced MRI had a sensitivity, specificity, and accuracy of 55%, 94%, and 79%, respectively, for diagnosing AVN. Increased proximal pole STIR signal was noted with similar frequencies in patients with and without AVN.T1-weighted unenhanced MRI is an acceptable alternative to delayed contrast-enhanced MRI in the preoperative assessment of the vascular status of the proximal pole of the scaphoid in patients with chronic fracture nonunions. STIR images were not beneficial in determining proximal pole viability.

We present the first report of a patient with angiomatoid fibrous histiocytoma of bone in the radiology literature. This tumor initially eluded diagnosis due to its similarities with chronic hematoma and aneurysmal bone cyst. Only two cases of angiomatoid fibrous histiocytoma have been reported in the radiology literature and both of these lesions were in the soft tissues. The fairly distinctive findings in our patient of multiple large cystic chambers with fluid-fluid levels are similar to the findings in the two soft tissue case reports, suggesting that imaging may be used to suggest this specific diagnosis regardless of location, especially in the clinical setting of unexplained hematoma or anemia. Mention of this diagnosis in the radiology report may aid in the final diagnosis at pathology, because special techniques, including fluorescent in situ hybridization, must be applied in order to fully evaluate for the diagnosis.

BackgroundThe recent emphasis on more horizontal femoral tunnel placement for single-bundle anterior cruciate ligament (ACL) reconstructions requires placing a femoral tunnel lower on the lateral wall of the notch. Some surgeons have advocated moving the starting point of the tibial tunnel farther medial to achieve this more horizontal tunnel.PurposeTo compare tibial tunnel aperture changes with transtibial femoral tunnel drilling.Study DesignControlled laboratory study.MethodsTwenty match-paired cadaveric knees (10 specimens) were randomized into 2 groups with equal right and left knee distribution. Ten of the knees underwent tibial tunnel drilling from a medial starting point (group 1), and the corresponding opposite knee of each cadaveric specimen had the tibial tunnel drilled from a central starting point (group 2). Computerized tomography (CT) with thin slices and 3-dimensional reconstruction was used to obtain the dimensions of the apertures, area of the apertures, angles of the tunnels, and location of the starting point and ending point of the tunnels. We also determined the location of the femoral tunnels in the notch for each of the groups. The 10 knees with medial starting points underwent transtibial femoral tunnel drilling and were restudied with CT to evaluate changes in tibial tunnel characteristics. The 10 knees with central starting points underwent femoral drilling from an anteromedial arthroscopic portal.ResultsCentral tibial tunnels were slightly larger than medial tibial tunnels before femoral drilling (106.3 mm3 vs 92.4 mm3). After femoral drilling through the medial tunnels, the apertures were larger than the central tibial apertures (118.6 mm3 vs 106.3 mm3). Medial tibial tunnels resulted in an intra-articular aperture that was farther from the tibial tubercle (43.1 mm vs 16.3 mm), farther from the medial tibial plateau (38.3 mm vs 32.2 mm), and more acute in the coronal plane (50.4° vs 79.3°). Medial tibial tunnels resulted in an intra-articular aperture that was closer to the anterior edge of the tibia (22.6 mm vs 29.6 mm) but with a less acute sagittal plane angle (82.5° vs 54.5°). The average clock-face measurement on the femur was 10:40 (±14 minutes) for the medial starting point and 10:14 (±14 minutes) for the central starting point (drilled from an anteromedial arthroscopic portal) (P = .0016).ConclusionWe observed significantly increased tibial aperture size and shape after transtibial femoral drilling with a medial tibial starting point. Medial tibial tunnels, compared with more central tunnels, resulted in a more acute tibial tunnel in the coronal plane and less acute tibial tunnels relative to the sagittal plane. Medial tibial tunnels started farther from the tibial tubercle but ended farther from the medial joint line and closer to the anterior edge of the tibia in comparison to central tunnelsClinical RelevanceFemoral tunnel placements may be best accomplished using a technique other than transtibial drilling through a medial tibial tunnel. Tibial tunnel angle, intra-articular position, and femoral tunnel placement are affected by the choice of extra-articular starting position.

Clinical decision support platforms for imaging order entry have recently been mandated by the federal government. Little data exists outside of the convener sites on how to go about the implementation process. As an early adopter of a commercially available clinical decision support program for imaging order entry, we present our initial experience.

Background Many hospitals use a traditional categoric system (eg, STAT, ASAP [as soon as possible], routine) to prioritize orders for imaging examination performance. If left undefined, these categories contain ambiguity, which contributes to errant or misused categorizations, and ultimately, lost opportunity to optimally direct resources toward timely patient care. Our hospital implemented ordinal order-priority categories with specific definitions. We sought to determine the impact of this prioritization method on examination performance time and consistency. Methods A four-level numeric priority system with clinical definitions for each category was implemented in 2011 to replace a traditional model for hospital imaging orders. Retrospective analysis was performed on imaging orders for three years (2011-2013) after implementation, to assess the order-to-performance time (OTPT), defined as the time between order placement by the provider and examination completion by the technologist. Consistency was measured by the length of the interquartile range for the OTPT distribution. Comparison was made to orders from the preimplementation year (2010), as a control. Results The OTPT and OTPT consistency for performed examinations were both predictably stratified by order-priority level. Relative to control, we observed a reduction in the percentage of prioritized examinations, as well as modest general improvements in OTPT and OTPT consistency. Conclusions A revised order-priority system with ordinal categorizations and clinical definitions accompanying each priority level at order entry yielded desirable prioritization of imaging examination performance by technologists, as evidenced by appropriate stratification of turnaround times and consistency by level of priority.

To review retrospectively the magnetic resonance (MR) imaging findings and clinical information of patients with Parsonage-Turner syndrome (PTS).The institutional review board did not require its formal approval or informed patient consent at the time of the study. However, the study was HIPAA compliant. The information in a computerized database of 2875 consecutive shoulder MR examinations was retrospectively reviewed. With use of key terms, the database software identified 81 examinations potentially associated with PTS. Both authors together reviewed the 81 imaging reports and the corresponding patients' medical records. In consensus, they made the diagnosis of PTS in 21 patients (two with bilateral involvement) on the basis of MR findings, electromyographic results, and clinical data. They also examined the data of an additional six patients (one with bilateral involvement) obtained from outside facilities. Ultimately, 30 shoulders of 27 patients (18 male, nine female; age range, 12-81 years; mean age, 41 years) were evaluated. The MR findings and clinical information (ie, regarding atrophy, pain, weakness, electromyographic results, neck and spine history, trauma, excessive overhead activity, recent surgery, vaccination, and illness) of all patients with PTS were reviewed. MR findings of diffuse high T2 signal intensity abnormality and fatty atrophy of muscles were evaluated to assess the pattern of nerve involvement. Structural causes (eg, ganglion cyst or other mass) of neurogenic high T2 signal intensity abnormality were excluded at MR imaging.Twenty-nine (97%) of 30 shoulders had suprascapular nerve involvement; in 15 (50%) shoulders, the involvement was limited to this nerve. Fifteen (50%) shoulders had axillary nerve involvement; in only one (3%) shoulder, the involvement was limited to this nerve. One shoulder (3%) had subscapular nerve involvement. Nine (30%) shoulders demonstrated focal muscular atrophy. Eleven (41%) of 27 patients also underwent electromyography; all of these patients demonstrated neuropathies that matched the patterns of neurogenic high T2 signal intensity abnormality seen at MR imaging.The suprascapular nerve was almost invariably involved (in 97% of shoulders) in patients with PTS. Axillary nerve involvement also was commonly observed (in 50% of shoulders). Subscapular nerve involvement was uncommon (in 3% of shoulders).

Our objectives were to evaluate the reliability of MRI in distinguishing simple lipomas, lipoma variants, and well-differentiated liposarcomas (atypical lipomas) and to identify various imaging mimics of well-differentiated liposarcoma.One hundred twenty-six consecutively imaged grossly fatty masses were retrospectively reviewed. MRI examinations, their prospective interpretations, and their corresponding pathology reports were compared to determine the reliability of MRI in distinguishing simple lipomas, lipoma variants, and well-differentiated liposarcomas.The success of MRI in identifying well-differentiated liposarcomas among other fatty masses was as follows: sensitivity, 100%; specificity, 83%; accuracy, 84%; positive predictive value, 38%; and negative predictive value, 100%. MRI was 100% specific in the diagnosis of simple lipoma. Sixty-three percent of lesions considered suspicious for well-differentiated liposarcoma were actually simple lipomas (13%) and benign lipoma variants (50%), including chondroid lipoma (13%), osteolipoma (6%), hibernoma (6%), lipoleiomyoma (6%), angiolipoma (6%), and infarcted lipoma (13%).Because of differences in treatment, prognosis, and long-term follow-up, it is important to preoperatively distinguish simple lipomas from well-differentiated liposarcomas. MRI is highly sensitive in the detection of well-differentiated liposarcomas and highly specific in the diagnosis of simple lipomas. However, when an extremity or body wall lesion is considered suspicious for well-differentiated liposarcoma, it is more likely (64%) to represent one of many benign lipoma variants.

Knee pain and injury are common complaints of athletes presenting to the orthopedic clinic. Wrestlers are no exception, and may present more commonly with anterior knee pain because of the nature of their sport. Morel–Lavallee lesions and prepatellar bursitis have been described in the prepatellar region of wrestlers. We report a distinctly different prepatellar finding on MRI, focally prominent and discrete fibrosis in three collegiate wrestlers. This characteristic appearance and clinical setting have already been repeated in our clinical practice; thus, other MRI readers are also likely to encounter this finding, which in our experience can be considered a "don’t touch" lesion of no clinical significance.

To determine if gadolinium signal intensity at direct magnetic resonance (MR) arthrography is affected by the addition of steroids, anesthetics, or iodinated contrast material.This study did not require approval by the institutional review board because no patients or patient data were involved. An in vitro study was performed to evaluate various concentrations of three gadolinium contrast agents (gadopentetate dimeglumine, gadobenate dimeglumine, and gadofosveset trisodium) diluted in either saline or iodinated contrast material (50% and full-strength iohexol 300). Three steroids (betamethasone, triamcinolone, and methylprednisolone) and three local anesthetics (lidocaine, ropivacaine, and bupivacaine) were added to solutions in clinical doses. T1-weighted fat-suppressed MR imaging sequences were performed in phantoms at 1.5 and 3.0 T. Signal intensities were measured. All experiments were repeated in full for a total of three replications each. The data were analyzed by using two-way factorial analysis of variance.Dilution of gadolinium into iohexol reduced the signal intensity in all samples compared with dilution in saline alone. Peak signal intensities were at 0.625 and 1.25 mmol/L of gadolinium in iohexol at both magnet strengths. At 1.5 T, the addition of steroids and anesthetics to the saline solutions had no impact on the signal intensity curves, with the peak signal intensity at gadolinium concentrations of 2.5 and 1.25 mmol/L. At 3.0 T, the addition of steroids and anesthetics had minimal effect on signal intensity curves, with the peak signal intensity at 1.25 mmol/L of gadolinium.The addition of steroids and/or anesthetics to gadolinium solutions for MR arthrography does not substantially impact signal intensity. When gadolinium is diluted into a 50% or greater strength of iohexol, the signal intensity curve shifts so that the maximum signal intensity is obtained with lower gadolinium concentrations (0.625-1.25 mmol/L).

When associated with a knee dislocation, management of the medial ligamentous injury is challenging, with little literature available to guide treatment.We (1) compared MRI findings of medial ligament injuries between Schenck KDIIIM and KDIV injuries, (2) compared clinical outcomes and health-related quality of life as determined by Lysholm and Veterans Rand 36-Item Health Survey (VR-36) scores, respectively, of reconstructed KDIIIM and KDIV injured knees, and (3) determined reoperation rates of reconstructed KDIIIM and KDIV injured knees.Over a 12-year period, we treated 65 patients with knee dislocations involving bicruciate ligament injury and concomitant medial ligament injuries, without or with posterolateral corner injuries (Schenck KDIIIM and KDIV, respectively); 57% were available for followup at a mean of 6.2 years (range, 1.1-11.6 years). These patients were contacted, and prospectively measured clinical outcomes scores (Lysholm and VR-36) were obtained and compared between subsets of patients. Preoperative MRIs (available for review on 49% of the patients) were rereviewed to characterize the medial ligament injuries.KDIIIM injuries more frequently had complete deep medial collateral ligament tears and posterior oblique ligament tears compared to KDIV injuries. KDIIIM knees had better Lysholm scores (88 versus 67, p = 0.027) and VR-36 scores (88 versus 70, p = 0.022) than KDIV knees. Female sex (Lysholm: 55 versus 85, p = 0.005; VR-36: 59 versus 85, p = 0.003) and an ultra-low-velocity mechanism (injury that occurs during activity of daily living in obese patients) (Lysholm: 55 versus 80-89, p = 0.002-0.013; VR-36: 60 versus 79-88, p = 0.001-0.017) were associated with worse outcomes. The overall reoperation rate was 28%, and the most common indication for reoperation was stiffness.Medial ligament injury is common in knee dislocations. Females who sustain these injuries and patients who have an ultra-low-velocity mechanism should be counseled at the time of injury about the likelihood of inferior outcomes. As ROM deficits are the most commonly encountered complication, postoperative rehabilitation should focus on early ROM exercises as stability and wound healing allow. Future prospective studies are needed to definitively determine whether operative or nonoperative management is appropriate for particular medial ligamentous injury patterns.

Purpose This study aimed to compare the accuracy of tibial tunnel placement using independent femoral (IF) versus transtibial (TT) techniques. Methods Ten matched pairs of cadaveric knees were randomized so that one knee in the pair underwent arthroscopic TT drilling of the femoral tunnel and the other underwent IF drilling through an accessory medial portal. For both techniques, an attempt was made to place the femoral and tibial tunnels as close to the center of the respective anterior cruciate ligament (ACL) footprints as possible. Preoperative and postoperative computed tomography using a technique optimized for ligament evaluation allowed comparison of the anatomic ACL tibial footprint to the tibial tunnel aperture. The percentage of tunnel aperture contained within the native footprint, as well as the distance from the center of the tunnel aperture to the center of the footprint, was measured. Additionally, graft obliquity relative to the tibial plateau was evaluated in the sagittal plane. Results The percentage of tibial tunnel aperture contained within the native footprint averaged 71.6% ± 17.2% versus 52.1% ± 23.4% ( P = .04) in the IF and TT groups, respectively. The distance from the center of the footprint to the center of the tibial tunnel aperture was 3.50 ± 1.6 mm and 4.40 ± 1.7 mm ( P = .27) in the IF and TT groups, respectively. TT drilling placed 6 of 10 tunnels posterior to the center of the footprint versus 3 of 10 tunnels in IF drilling. The graft obliquity angles were 54.8° in TT specimens and 47.5° in IF specimens ( P = .09). Conclusions This study adds to the literature suggesting that TT drilling with an 8-mm reamer has deleterious effects on tibial tunnel aperture and position. IF drilling, which does not involve repeated reaming of the tibial tunnel, is associated with the placement of a higher percentage of the tunnel aperture within the native tibial footprint. There was not a significant difference between the IF and TT techniques in their ability to place the center of the tibial aperture near the center of the footprint or in graft obliquity. Clinical Relevance ACL reconstruction has continued to evolve in an attempt to restore the functional anatomy and biomechanical behavior of the knee. Tibial tunnel characteristics—such as location, aperture topography, and tunnel obliquity—are important factors to consider in ACL reconstruction. This study compares tibial tunnels after IF and TT techniques.
