Abstract

OBJECTIVES: : To prospectively compare clinical breast examination, mammography, ultrasonography, and contrast-enhanced magnetic resonance imaging (MRI) in a multicenter surveillance of high-risk women. MATERIALS AND METHODS: : We enrolled asymptomatic women aged ≥ 25: BRCA mutation carriers; first-degree relatives of BRCA mutation carriers, and women with strong family history of breast/ovarian cancer, including those with previous personal breast cancer. RESULTS: : A total of 18 centers enrolled 501 women and performed 1592 rounds (3.2 rounds/woman). Forty-nine screen-detected and 3 interval cancers were diagnosed: 44 invasive, 8 ductal carcinoma in situ; only 4 pT2 stage; 32 G3 grade. Abstract

Objective: Different perfusion metrics were investigated to determine the accuracy of Perfusion CT (PCT) in differentiating recurrence of brain metastases from radiation necrosis in patients who previously underwent stereotactic radiation therapy (SRT). Patients and Methods: Twenty patients previously treated with SRT underwent PCT examination of the brain. Normalized Cerebral Blood Volume (nCBV) values within the region of interest (ROI) were calculated. Fractional volumes at nCBV values between 1.0 and 2.25, defined as V 1.0-V 2.25, were derived as alternative metrics and compared to the conventional method based on the mean CBV value within the lesion (nCBV mean). The Mann-Whitney test was used to compare the two patient's groups with recurrence and radiation necrosis with respect to the different nCBV metrics. Predictive powers and optimal thresholds for both the nCBV mean and the V 1.0-V 2.25 were evaluated using the Receiver Operating Characteristic Curves. The gold standard was represented either by the histopathological examination or the Magnetic Resonance (MR) imaging follow-up longer than six months. Results and Conclusion: The differences between the patient's group with recurrence and that with radiation necrosis resulted statistically significant for all the metrics, showing the lowest p-value for V 1.75 and V 2. The metrics based on the fractional volumes were found to show higher predictive powers, with the highest value of 0.96 for V 2.0. Quantitative analysis of the CBV map deriving different metrics may potentially improve the diagnostic accuracy of PCT in differentiating brain metastasis recurrence from radiation necrosis. © 2011 Elsevier Ireland Ltd. Abstract

PURPOSE: This study compared the sensitivity of two commercial computer-aided detection (CAD) systems in identifying noncalcified pulmonary nodules on low-dose multidetector computed tomography (MDCT) scans by using a double reference standard. MATERIALS AND METHODS: Three chest low-dose MDCT scans of patients who had undergone lung cancer screening were retrospectively analysed using two distinct commercial CAD systems: LungCAD VC10A, Siemens Medical Solutions (CAD1) and LungVCAR, GE Healthcare (CAD2). The exact location of each finding suggested by each system was recorded by an independent reader according to spatial coordinates (x, y, z). Two panels of experienced thoracic radiologists from two different institutions independently established two reference standards (RS1, RS2) by identifying the true positive findings with spatial coordinates without using CAD. Sensitivity of the two CAD systems, defined by lesionlevel analysis, was tested and sensitivities compared. RESULTS: RS1 identified 34 noncalcified pulmonary nodules, whereas RS2 identified 54. The total number of findings detected by the two CAD systems was 684. CAD1 correctly identified 13/34 nodules (sensitivity 38%) for RS1 and 17/54 (sensitivity 30%) for RS2, whereas CAD2 correctly identified 11/34 nodules (sensitivity 35%) for RS1 and 13/54 (sensitivity 23%) for RS2. Comparison between the two CAD systems did not show a statistically significant difference in terms of sensitivity (p<0.05) for both RS1 (p=0.42) and RS2 (p=0.33). CONCLUSIONS: The two commercial CAD systems had similar sensitivity in detecting noncalcified pulmonary nodules on low-dose MDCT of the chest.

33. Peripheral nervous system involvement in a patient with large T-cell lymphoma arising from a pre-existing mycosis fungoides

Abstract

A 29-year-old man was examined for disseminated erythematous scaling patches and plaques and reddish, partially ulcerated nodules. Histological examination showed a dense, diffuse, epidermotropic infiltrate located in the entire dermis to the subcutaneous tissue, composed mainly of large pleomorphic T lymphocytes. Immunohistochemistry revealed positivity of neoplastic cells for T-cell-associated markers, negativity for CD30 antigen and for B-cell markers. Polymerase chain reaction analysis detected a clonal amplification of T-cell receptor gamma. Based on clinicopathological and molecular findings, the diagnosis of large T-cell lymphoma (LCL) arising from a pre-existing mycosis fungoides was made. Seven months after primary diagnosis, meningeal and peripheral nervous system involvement developed with no other evidence of systemic disease. Despite chemotherapy and radiation therapy, the patient died 3 months after the diagnosis of nervous system involvement. In patients with cutaneous LCL, mild neurological symptoms may precede the complete diagnostic picture by some weeks. A rapid and fatal progression characterizes the clinical course of the disease.

Abstract

Gli autori prendono in considerazione il ruolo diagnostico della risonanza magnetica nella diagnostica delle neurofibromatosi, su una casistica di 13 osservazioni, di cui 9 casi di NF1 e 4 NF2.Lo studio è stato eseguito mediante sistemi RM superconduttivi da 0,5 e 1,5 Tesla, osservando un pattern di aspetti polimorfo comprendente nelle NF1 lesioni multiple insieme in T1 (6 casi), iperintense in T1 (2 casi), sempre iperintense in T2; inoltre si sono riscontrati gliomi delle vie ottiche (3 casi), xantogranulomatosi dei plessi corioidei e neurinomi multipli cervicodorsali.Nelle NF2 si sono invece reperiti schwannomi bilaterali dei nervi acustici (3 casi) e monolaterali (1 caso), meningiomi (4 casi), uno schwannoma del nervo facciale e neurofibromi spinali multipli (2 casi).Secondo la esperienza degli autori, la RM si è rivelata il metodo di scelta nella diagnostica e nel controllo dei pazienti con NF, grazie alla elevata accuratezza e sensibilità della metodica.I risultati ottenuti dimostrano l'elevata accuratezza della metodica e la sua preminenza nella diagnosi e nel controllo di tali forme neoplastiche.

47. Angiomi venosi o «anomalie di sviluppo venoso»: Nostra esperienza con RM ed Angio-RM, studio dell'associazione con gli angiomi cavernosi e revisione della letteratura

Abstract

Sono stati studiati con RM 29 pazienti con anomalie di drenaggio venoso in condizio-ni basali e dopo Gadolinio-DTPA; 24 sono stati sottoposti anche ad angio-RM. Nella valutazione dei risultati è stato assegnato un punteggio: 0 quando l'anomalia non era evidenziata; 1 per la rappresentazione del solo collettore; 2 per la dimostrazione del collettore e delle vene midollari; 3 per la dimostrazione del collettore, delle vene midollari e del tipo di drenaggio. È stata inoltre considerata la presenza di eventuali angiomi cavernosi associati e l'eventuale correlazione con la sintomatologia.Le sequenze SE Tl pesate dopo Gadolinio-DTPA in doppia dose sono risultate quelle con maggiore accuratezza diagnostica (score 77) sia rispetto alle sequenze basali (T1 score 12, T2 score 31) sia nei 24 pazienti sottoposti anche ad angio-RM (SE T1 dopo Gadolinio score 66, angio-RM score 61). L'angio-RM è risultata utile nell' evidenziare il tipo di drenaggio.In 5 casi l'anomalia di sviluppo venoso era associata ad un angioma cavernoso, in 4 casi le due malformazioni erano in stretto rapporto; solo in due casi però l'angioma cavernoso presentava il tipico aspetto a «pop-corn». In un solo paziente l'anomalia venosa si è presentata come evento emorragico, mentre in un altro caso era in stretto rapporto con un'area di basso segnale in T1 e T2, esito di una pregressa emorragia.Nel 34% i pazienti hanno presentato epilessia senza però una costante correlazione tra sede dell'ano-malia venosa e sede dell'anomalia elettroencefalografica.

Abstract

Magnetic Resonance and Magnetic Resonance Angiography (MRA) data, obtained from 44 patients with brain gliomas (13 low-grade astrocytoma [LGA], 6 anaplastic astrocytoma [AA], 25 glioblastoma [GBM]), were analysed to evaluate further relevant information linked to tumour malignancy grading and the potential role of MRA in the diagnostic work-up of neuroepithelial brain tumours.MR scanning was performed with a 1.5 superconductive system utilising SE, T1 and SE T2 sequences; signal intensity, perilesional oedema, degenerative changes, tumour borders, enhancement after Gadolinium-DTPA (Gd-DTPA) infusion and the presence of haemorrhagic areas were considered.Feeding vessels, venous drainage, tumour vascularity, displacement or encasement of the main vessels were considered in the analysis of MRA data, performed with the time-of-flight technique (3D and 2D sequences).Oedema, degenerative changes and enhancement after Gd-DTPA were demonstrated only in AA and GBM; the margins of these tumours were irregular and signal intensity was dishomogeneous with haemorrhagic areas in two cases of GBM. In LGA oedema and enhancement were present in four out of 13 cases and degenerative changes only in one. Tumour margins were regular in nine cases and irregular in four; the signal intensity was always homogeneous.Displacement and encasement of the main vessels were better demonstrated with MRA, above all for AA and GBM. Signs of neovascularity were demonstrated only in GBM, in particular the feeding vessels in 14 out of 25 cases, tumour vascularity in six and venous drainage in ten cases.
