Abstract

The process of identifying suitable genome-wide association (GWA) studies and formatting the data to calculate multiple polygenic risk scores on a single genome can be laborious. Here, we present a centralized polygenic risk score calculator currently containing over 250,000 genetic variant associations from the NHGRI-EBI GWAS Catalog for users to easily calculate sample-specific polygenic risk scores with comparable results to other available tools. Polygenic risk scores are calculated either online through the Polygenic Risk Score Knowledge Base (PRSKB; https://prs.byu.edu ) or via a command-line interface. We report study-specific polygenic risk scores across the UK Biobank, 1000 Genomes, and the Alzheimer's Disease Neuroimaging Initiative (ADNI), contextualize computed scores, and identify potentially confounding genetic risk factors in ADNI. We introduce a streamlined analysis tool and web interface to calculate and contextualize polygenic risk scores across various studies, which we anticipate will facilitate a wider adaptation of polygenic risk scores in future disease research.

Abstract

We report the results of spectroscopic and photometric observations of the emission-line object AS 386. For the first time, we found that it exhibits the B[e] phenomenon and fits the definition of an FS CMa type object. The optical spectrum shows the presence of a B-type star with the following properties: T_ eff = 11000+/-500 K, log L/L_sun = 3.7+/-0.3, a mass of 7+/-1 M_sun, and a distance D = 2.4+/-0.3 kpc from the Sun. We detected regular radial velocity variations of both absorption and emission lines with the following orbital parameters: P_orb = 131.27+/-0.09 days, semi-amplitude K_1 = 51.7+/-3.0 km/s, systemic radial velocity gamma = -31.8+/-2.6 km/s, and a mass function of f(m) = 1.9+/-0.3 M_sun. AS 386 exhibits irregular variations of the optical brightness (V=10.92+/-0.05 mag), while the near-IR brightness varies up to ~0.3 mag following the spectroscopic period. We explain this behavior by a variable illumination of the dusty disk inner rim by the B-type component. Doppler tomography based on the orbital variations of emission-line profiles shows that the material is distributed near the B-type component and in a circumbinary disk. We conclude that the system has undergone a strong mass transfer that created the circumstellar material and increased the B-type component mass. Abstract

Context: We introduce the Dwarf Galaxy Survey with Amateur Telescopes (DGSAT) project and report the discovery of eleven low surface brightness (LSB) galaxies in the fields of the nearby galaxies NGC 2683, NGC 3628, NGC 4594 (M104), NGC 4631, NGC 5457 (M101), and NGC 7814. Aims: The DGSAT project aims to use the potential of small-sized telescopes to probe LSB features around large galaxies and to increase the sample size of the dwarf satellite galaxies in the Local Volume. Methods: Using long exposure images, fields of the target spiral galaxies are explored for extended low surface brightness objects. After identifying dwarf galaxy candidates, their observed properties are extracted by fitting models to their light profiles. Results: We find three, one, three, one, one, and two new LSB galaxies in the fields of NGC 2683, 3628, 4594, 4631, 5457, and 7814, respectively. In addition to the newly found galaxies, we analyse the structural properties of nine already known galaxies. All of these 20 dwarf galaxy candidates have effective surface brightnesses in the range $25.3\lesssim\mu_{e}\lesssim28.8$ mag.arcsec$^{-2}$ and are fit with Sersic profiles with indices $n\lesssim 1$. Assuming that they are in the vicinity of the above mentioned massive galaxies, their $r$-band absolute magnitudes, their effective radii, and their luminosities are in the ranges $-15.6 \lesssim M_r \lesssim -7.8$, 160 pc $\lesssim R_e \lesssim$ 4.1 kpc, and $0.1\times 10^6 \lesssim\left(\frac{L}{L_{\odot}}\right)_r\lesssim127 \times 10^6$, respectively. To determine whether these LSB galaxies are indeed satellites of the above mentioned massive galaxies, their distances need to be determined via further observations. Conclusions: Using small telescopes, we are readily able to detect LSB galaxies with similar properties to the known dwarf galaxies of the Local Group., Comment: Matches the version published in Astronomy & Astrophysics. Abstract

Objective: Fetal movements are often used as a surrogate for fetal wellbeing. Previous research suggests a link between maternal perception of decreased fetal movements (DFM) and small‐for‐gestational‐age (SGA) infants. The aim of this study was to investigate the association between maternal presentation with DFM and birth‐weight centile categories at a large Australian perinatal center. Methods: This was a retrospective study of non‐anomalous singleton infants born at ≥ 28 + 0 weeks' gestation between January 2016 and October 2020 at the Mater Mothers' Hospital in Brisbane, Australia. The primary outcome was the rate of DFM according to birth‐weight centile category. Maternal demographic characteristics included age, body mass index, ethnicity, parity, medical conditions and previous stillbirth. The association between DFM and birth‐weight centile was evaluated using adjusted multinomial regression models. Robust standard errors were used to account for clustering at the patient level. Wald tests and Akaike's and Bayesian information criteria were used to evaluate models. Results: Over the 5‐year study period, 45 042 women met the inclusion criteria. Of these, 6690 (14.9%) women presented with DFM. Of the DFM cohort, 80.9% (5411/6690) had only one presentation with DFM, and 19.1% (1279/6690) had two or more presentations. The overall stillbirth rate was similar in women with DFM (0.1% (8/6690)) and those without DFM (0.1% (50/38 352)). There was no association between DFM (either single or multiple) and infant birth‐weight centile. Conclusions: This study suggests that presentation with DFM is not associated with infant size. Clinicians should consider additional risk factors and the overall clinical context when deciding appropriate management. DFM is not necessarily an indication for an immediate or urgent ultrasound scan to assess fetal size. © 2024 The Authors. Abstract

We present spectroscopic and photometric observations of the emission-line star MWC 930 (V446 Sct) during its long-term optical brightening in 2006--2013. Based on our earlier data we suggested that the object has features found in Luminous Blue Variables (LBV), such as a high luminosity (~3 10^5 Lsun, a low wind terminal velocity (~ 140 km/s), and a tendency to show strong brightness variations (~1 mag over 20 years). For the last ~7 years it has been exhibiting a continuous optical and near-IR brightening along with a change of the emission-line spectrum appearance and cooling of the star's photosphere. We present the object's $V$--band light curve, analyze the spectral variations, and compare the observed properties with those of other recognized Galactic LBVs, such as AG Car and HR Car. Overall we conclude the MWC 930 is a bona fide Galactic LBV that is currently in the middle of an S Dor cycle., Comment: 12 pages, 7 figures

7. Evolution from protoplanetary to debris discs: The transition disc around HD 166191

Abstract

HD 166191 has been identified by several studies as hosting a rare and extremely bright warm debris disc with an additional outer cool disc component. However, an alternative interpretation is that the star hosts a disc that is currently in transition between a full gas disc and a largely gas-free debris disc. With the help of new optical to mid-IR spectra and Herschel imaging, we argue that the latter interpretation is supported in several ways: i) we show that HD 166191 is co-moving with the ~4 Myr-old Herbig Ae star HD 163296, suggesting that the two have the same age, ii) the disc spectrum of HD 166191 is well matched by a standard radiative transfer model of a gaseous protoplanetary disc with an inner hole, and iii) the HD 166191 mid-IR silicate feature is more consistent with similarly primordial objects. We note some potential issues with the debris disc interpretation that should be considered for such extreme objects, whose lifetime at the current brightness is mush shorter than the stellar age, or in the case of the outer component requires a mass comparable to the solid component of the Solar nebula. These aspects individually and collectively argue that HD 166191 is a 4-5 Myr old star that hosts a gaseous transition disc. Though it does not argue in favour of either scenario, we find strong evidence for 3-5 um disc variability. We place HD 166191 in context with discs at different evolutionary stages, showing that it is a potentially important object for understanding the protoplanetary to debris disc transition., Comment: accepted to MNRAS, fixed typos in abstract and axis label

Abstract

Objective: To assess the association between placental biomarkers (placental growth factor (PlGF) and soluble fms‐like tyrosine kinase‐1 (sFlt‐1)/PlGF ratio) and fetoplacental Doppler indices (umbilical artery (UA) pulsatility index (PI) and uterine artery (UtA) PI) in various combinations for predicting preterm birth (PTB) in pregnancies complicated by fetal growth restriction (FGR). Methods: This was a prospective observational cohort study, performed at Mater Mother's Hospital in Brisbane, Queensland, Australia, from May 2022 to June 2023, of pregnancies complicated by FGR and appropriate‐for‐gestational‐age (AGA) pregnancies. Maternal serum PlGF levels, sFlt‐1/PlGF ratio, UA‐PI and UtA‐PI were measured at 2–4‐weekly intervals from recruitment until delivery. Harrell's concordance statistic (Harrell's C) was used to evaluate multivariable Cox proportional hazards regression models featuring various combinations of placental biomarkers and fetoplacental Doppler indices to ascertain the best combination to predict PTB (< 37 weeks). Multivariable Cox regression models were used with biomarkers as time‐varying covariates. Results: The study cohort included 320 singleton pregnancies, comprising 179 (55.9%) affected by FGR, defined according to a Delphi consensus, and 141 (44.1%) with an AGA fetus. In the FGR cohort, both low PlGF levels and elevated sFlt‐1/PlGF ratio were associated with significantly shorter time to PTB. Low PlGF was a better predictor of PTB than was either sFlt‐1/PlGF ratio or a combination of PlGF and sFlt‐1/PlGF ratio (Harrell's C, 0.81, 0.78 and 0.79, respectively). Although both Doppler indices were significantly associated with time to PTB, in combination they were better predictors of PTB than was either UA‐PI > 95th centile or UtA‐PI > 95th centile alone (Harrell's C, 0.82, 0.75 and 0.76, respectively). Predictive utility for PTB was best when PlGF < 100 ng/L, UA‐PI > 95th centile and UtA‐PI > 95th centile were combined (Harrell's C, 0.88) (hazard ratio, 32.99; 95% CI, 10.74–101.32). Conclusions: Low maternal serum PlGF level (< 100 ng/L) and abnormal fetoplacental Doppler indices (UA‐PI > 95th centile and UtA‐PI > 95th centile) in combination have the greatest predictive utility for PTB in pregnancies complicated by FGR. Their assessment may help guide clinical management of these complex pregnancies. © 2023 The Authors. Abstract

Background: Frailty is known to be associated with poorer outcomes in individuals admitted to hospital for medical conditions requiring intensive care. However, little evidence is available for the effect of frailty on patients’ outcomes after traumatic brain injury. Many frailty indices have been validated for clinical practice and show good performance to predict clinical outcomes. However, each is specific to a particular clinical context. We aimed to develop a frailty index to predict 6-month outcomes in patients after a traumatic brain injury. Methods: A cumulative deficit approach was used to create a novel frailty index based on 30 items dealing with disease states, current medications, and laboratory values derived from data available from CENTER-TBI, a prospective, longitudinal observational study of patients with traumatic brain injury presenting within 24 h of injury and admitted to a ward or an intensive care unit at 65 centres in Europe between Dec 19, 2014, and Dec 17, 2017. From the individual cumulative CENTER-TBI frailty index (range 0–30), we obtained a standardised value (range 0–1), with high scores indicating higher levels of frailty. The effect of frailty on 6-month outcome evaluated with the extended Glasgow Outcome Scale (GOSE) was assessed through a proportional odds logistic model adjusted for known outcome predictors. An unfavourable outcome was defined as death or severe disability (GOSE score ≤4). External validation was performed on data from TRACK-TBI, a prospective observational study co-designed with CENTER-TBI, which enrolled patients with traumatic brain injury at 18 level I trauma centres in the USA from Feb 26, 2014, to July 27, 2018. CENTER-TBI is registered with ClinicalTrials.gov, NCT02210221; TRACK-TBI is registered at ClinicalTrials.gov, NCT02119182. Findings: 2993 participants (median age was 51 years [IQR 30–67], 2058 [69%] were men) were included in this analysis. The overall median CENTER-TBI frailty index score was 0·07

22. Threshold tests as a way to encourage long-term, self-regulated learners in engineering

Abstract

Traditionally, educators evaluate the effectiveness of a new assessment in a pre-requisite early subject via pass rates, grades and student satisfaction. It may be more appropriate to measure the impact on student results in later stage subjects. In this work we report on the impact of changing assessment, over 10 years, at a metropolitan Australian university for an initial calculus subject (Mathematics 1) on two follow-on subjects: Mathematics 2 and Fundamentals of Mechanical Engineering. Earlier research found that Online Mastery Tests can harm later learning outcomes even though failure rates drop within the pre-requisite subject. Here we show that Paper Threshold Tests, requiring greater engagement, metacognitive strategies result in fewer passes in the pre-requisite subject however, they were also resulting in a major inversion of the grade distribution toward higher grades in the follow on subjects where 60% of students now obtain marks > 75%.

Abstract

We examined 2-year longitudinal change in clinical features and biomarkers in LRRK2 non-manifesting carriers (NMCs) versus healthy controls (HCs) enrolled in the Parkinson’s Progression Markers Initiative (PPMI). We analyzed 2-year longitudinal data from 176 LRRK2 G2019S NMCs and 185 HCs. All participants were assessed annually with comprehensive motor and non-motor scales, dopamine transporter (DAT) imaging, and biofluid biomarkers. The latter included cerebrospinal fluid (CSF) Abeta, total tau and phospho-tau; serum urate and neurofilament light chain (NfL); and urine bis(monoacylglycerol) phosphate (BMP). Abstract

Importance: A head computed tomography (CT) with positive results for acute intracranial hemorrhage is the gold-standard diagnostic biomarker for acute traumatic brain injury (TBI). In moderate to severe TBI (Glasgow Coma Scale [GCS] scores 3-12), some CT features have been shown to be associated with outcomes. In mild TBI (mTBI; GCS scores 13-15), distribution and co-occurrence of pathological CT features and their prognostic importance are not well understood. Objective: To identify pathological CT features associated with adverse outcomes after mTBI. Design, Setting, and Participants: The longitudinal, observational Transforming Research and Clinical Knowledge in Traumatic Brain Injury (TRACK-TBI) study enrolled patients with TBI, including those 17 years and older with GCS scores of 13 to 15 who presented to emergency departments at 18 US level 1 trauma centers between February 26, 2014, and August 8, 2018, and underwent head CT imaging within 24 hours of TBI. Evaluations of CT imaging used TBI Common Data Elements. Glasgow Outcome Scale-Extended (GOSE) scores were assessed at 2 weeks and 3, 6, and 12 months postinjury. External validation of results was performed via the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. Data analyses were completed from February 2020 to February 2021. Exposures: Acute nonpenetrating head trauma. Main Outcomes and Measures: Frequency, co-occurrence, and clustering of CT features; incomplete recovery (GOSE scores <8 vs 8); and an unfavorable outcome (GOSE scores <5 vs ≥5) at 2 weeks and 3, 6, and 12 months. Abstract

Funding: This work, including collection and genotyping of the UK Young Lacunar Stroke DNA Study 2 (DNA Lacunar 2), was supported by a British Heart Foundation Programme Grant (RG/16/4/32218). Background: The genetic basis of lacunar stroke is poorly understood, with a single locus on 16q24 identified to date. We sought to identify novel associations and provide mechanistic insights into the disease. Methods: We did a pooled analysis of data from newly recruited patients with an MRI-confirmed diagnosis of lacunar stroke and existing genome-wide association studies (GWAS). Patients were recruited from hospitals in the UK as part of the UK DNA Lacunar Stroke studies 1 and 2 and from collaborators within the International Stroke Genetics Consortium. Cases and controls were stratified by ancestry and two meta-analyses were done: a European ancestry analysis, and a transethnic analysis that included all ancestry groups. We also did a multi-trait analysis of GWAS, in a joint analysis with a study of cerebral white matter hyperintensities (an aetiologically related radiological trait), to find additional genetic associations. We did a transcriptome-wide association study (TWAS) to detect genes for which expression is associated with lacunar stroke; identified significantly enriched pathways using multi-marker analysis of genomic annotation; and evaluated cardiovascular risk factors causally associated with the disease using mendelian randomisation. Findings: Our meta-analysis comprised studies from Europe, the USA, and Australia, including 7338 cases and 254 798 controls, of which 2987 cases (matched with 29 540 controls) were confirmed using MRI. Five loci (ICA1L-WDR12-CARF-NBEAL1, ULK4, SPI1-SLC39A13-PSMC3-RAPSN, ZCCHC14, ZBTB14-EPB41L3) were found to be associated with lacunar stroke in the European or transethnic meta-analyses. A further seven loci (SLC25A44-PMF1-BGLAP, LOX-ZNF474-LOC100505841, FOXF2-FOXQ1, VTA1-GPR126, SH3PXD2A, HTRA1-ARMS2, COL4A2) were found to be associated in the multi-trait analysis with cerebral white matter hyperintensities (n=42 310). Two of the identified loci contain genes (COL4A2 and HTRA1) that are involved in monogenic lacunar stroke. The TWAS identified associations between the expression of six genes (SCL25A44, ULK4, CARF, FAM117B, ICA1L, NBEAL1) and lacunar stroke. Abstract

The management of chronic kidney disease is complex. With disease management being the responsibility of parents in the paediatric renal clinic, the responsibility is gradually shifted to adolescents and young adults during the transition to adult care. This multi-perspective qualitative study aimed to explore the experiences of adolescents and young adults, their parents and health professionals to gain an insight into transitional care. Focussing on the transition process and transfer to adult care, 18 adolescents and young adults and eight mothers participated in individual semi-structured interviews. Additionally, three focus groups were conducted with 20 multidisciplinary health professionals. Data were transcribed verbatim and analysed thematically. Similar responses from adolescents and young adults and mothers included the reluctance to leave the paediatric health service. Mothers found the transition to adult care more challenging than the adolescents and young adults. While health professionals acknowledged that engaging adolescents and young adults in their own care was challenging, they believed parents had an important role in facilitating their child's independence. This study highlights that health professionals in both paediatric and adult health services need to work collaboratively. However, importantly, health professionals need to be mindful that parents require an equal amount of engagement as adolescents and young adults, if not more, to mitigate parental barriers in achieving a successful transfer.Copyright © The Author(s) 2021.

44. Hampering teaching excellence? Academics making decisions in the face of contradictions

Abstract

© 2020 Society for Research into Higher Education. Universities might aspire to teaching excellence, but do they enable academic teachers to make good teaching decisions? Using a critical realist perspective, a qualitative interview study in England and Australia explored academics’ experiences of teaching decisions and their responses to strategic, institutional and departmental teaching policy and planning. Complex and contradictory conditions that challenged academics’ experiences of teaching and prevented effective decision-making were found. The paper identifies aspects of university functioning that act to prevent the achievement of teaching excellence. It argues that excellence in teaching requires coherent and integrated approaches and commitment right across the institution. For this to happen, universities need to consider how stated strategic learning and teaching ambitions are communicated, implemented, supported and, importantly, how they are understood and enacted throughout all levels and areas of the organisation, including many that hitherto do not consider they have a role in learning and teaching.

Abstract

Background: Sexually transmissible infection (STI) and blood-borne virus (BBV) diagnoses data are a core component of the Australian National Notifiable Diseases Surveillance System (NNDSS). However, the NNDSS data alone is not enough to understand STI and BBV burden among priority population groups, like Aboriginal and Torres Strait Islander people, because it lacks testing, treatment and management data. Here, we describe the processes involved in establishing a STI and BBV sentinel surveillance network representative of Aboriginal Community-Controlled Health Services (ACCHS) - known as the ATLAS network - to augment the NNDSS and to help us understand the burden of disease due to STI and BBV among Aboriginal and Torres Strait Islander peoples. Methods: Researchers invited participation from ACCHS in urban, regional and remote areas clustered in five clinical hubs across four Australian jurisdictions. Participation agreements were developed for each clinical hub and individual ACCHS. Deidentified electronic medical record (EMR) data relating to STI and BBV testing, treatment and management are collected passively from each ACCHS via the GRHANITEtm data extraction tool. These data are analysed centrally to inform 12 performance measures which are included in regular surveillance reports generated for each ACCHS and clinical hub. Results: The ATLAS network currently includes 29 ACCHS. Regular reports are provided to ACCHS to assess clinical practice and drive continuous quality improvement initiatives internally. Data is also aggregated at the hub, jurisdictional and national level and will be used to inform clinical guidelines and to guide future research questions. The ATLAS infrastructure can be expanded to include other health services and potentially linked to other data sources using GRHANITE. 50. Improving medication adherence in adult kidney transplantation (IMAKT): A pilot randomised controlled trial.

Abstract

Resources to support long-term medication adherence in kidney transplantation are limited. This study aimed to determine the efficacy of an intervention designed for kidney transplant recipients to enhance medication adherence. A single-blind, multi-site, 12-month pilot randomised controlled trial was conducted at all five public hospitals providing adult kidney transplantation in Victoria, Australia. Participants were recruited at 4 to 6 weeks post-transplantation. Thirty-five participants were randomly assigned to a 3-month intervention, involving a face-to-face meeting (a medication review and a consumer-centred video) and health coaching every two weeks. Thirty-six were randomised to receive usual care. All participants were followed for nine months post-intervention. There were no differences in adherence between groups measured by Medication Event Monitoring System (MEMS), however, it was underutilised by 42% of participants. Based on the self-reported Basel Assessment of Adherence to Immunosuppressive Medications Scale (BAASIS©) score, the percentage of adherent participants decreased significantly between baseline and 3 to 12 months in the control group (p-values<0.001) whilst the percentage of adherent participants in the intervention group remained constant over time. No group differences were detected in other outcomes. Due to the complex medication regimen, developing and testing a medication adherence intervention is difficult in kidney transplantation.
