Abstract

'The Domestication of Wolves' is a collection of poems written, revised, and assembled during the four-year period of my PhD. My writing has been informed and guided by the work of the Confessional poets Anne Sexton, Sylvia Plath, Robert Lowell, and John Berryman, particularly by their construction and deployment of richly figurative language to characterise their inner psyches and intense emotional extremes. By drawing on the knowledge and vocabulary gained from my early career as a neuroscientist specialising in psychiatry within the pharmaceutical industry, I attempt to construct new metaphors and reshape other, more established, metaphors to explore the subjective and intangible qualities of human relationships and psychology. I also employ the Confessional lyric 'I' in my exploration of distance, displacement, rootedness, masculinity, blue-collar work, and class - themes to which I have found myself repeatedly drawn. Additionally, I have included a long poem, 'Three Journeys to Harris', which I wrote while writer-in-residence at the Isle of Harris Distillery. The poem is built by layering metaphors depicting the maturation process of whisky and setting them against images of Harris's unique landscape and culture. While individual poems wrestle with a wide variety of subjects - from a fictional disease that turns limbs to spiders to the contemplation of what it means to be a good son - I hope that the collection as a whole offers the reader a satisfying balance between inner and outer worlds, joy and solemnity, and absence and embodied physicality. In the accompanying critical essay, '"Intimate with Madness": Metaphors for the Mind and Mental Illness in Confessional Poetry', I perform close readings, predominantly of the poetry of Anne Sexton but also of the work of her fellow Confessional poets Sylvia Plath, Robert Lowell, and John Berryman. In reading the work of these poets, I argue that their mental illness is often framed in terms of the conceptual metaphors of 'madness as destination' and 'madness as possession'. I explore how each poet negotiates these two conceptual metaphors in different ways, sometimes adhering to the established connotations and sometimes dissenting from them to create distinct literary effects that convey novel insight into evasive, individual pathologies.

Abstract

IntroductionMuch of the higher risk for end-stage kidney disease (ESKD) in African American individuals relates to ancestry-specific variation in the apolipoprotein L1 gene (APOL1). Relative to kidneys from European American deceased-donors, kidneys from African American deceased-donors have shorter allograft survival and African American living-kidney donors more often develop ESKD. The National Institutes of Health (NIH)-sponsored APOL1 Long-term Kidney Transplantation Outcomes Network (APOLLO) is prospectively assessing kidney allograft survival from donors with recent African ancestry based on donor and recipient APOL1 genotypes.MethodsAPOLLO will evaluate outcomes from 2614 deceased kidney donor-recipient pairs, as well as additional living-kidney donor-recipient pairs and unpaired deceased-donor kidneys.ResultsThe United Network for Organ Sharing (UNOS), Association of Organ Procurement Organizations, American Society of Transplantation, American Society for Histocompatibility and Immunogenetics, and nearly all U.S. kidney transplant programs, organ procurement organizations (OPOs), and histocompatibility laboratories are participating in this observational study. APOLLO employs a central institutional review board (cIRB) and maintains voluntary partnerships with OPOs and histocompatibility laboratories. A Community Advisory Council composed of African American individuals with a personal or family history of kidney disease has advised the NIH Project Office and Steering Committee since inception. UNOS is providing data for outcome analyses.ConclusionThis article describes unique aspects of the protocol, design, and performance of APOLLO. Results will guide use of APOL1 genotypic data to improve the assessment of quality in deceased-donor kidneys and could increase numbers of transplanted kidneys, reduce rates of discard, and improve the safety of living-kidney donation.

Abstract

AimsBiomarkers of kidney tubule injury, inflammation and fibrosis have been studied extensively and established as risk markers of adverse kidney and cardiovascular disease (CVD) outcomes. However, associations of markers of kidney tubular function with adverse clinical events have not been well studied, especially in persons with chronic kidney disease (CKD).Methods and resultsUsing a sample of 2377 persons with CKD at the baseline Systolic Blood Pressure Intervention Trial (SPRINT) visit, we evaluated the association of three urine tubular function markers, alpha-1 microglobulin (α1m), beta-2 microglobulin (β2m), and uromodulin, with a composite CVD endpoint (myocardial infarction, acute coronary syndrome, stroke, acute decompensated heart failure, or death from cardiovascular causes) and mortality using Cox proportional hazards regression, adjusted for baseline estimated glomerular filtration rate (eGFR), albuminuria, and CVD risk factors. In unadjusted analysis, over a median follow-up of 3.8 years, α1m and β2m had positive associations with composite CVD events and mortality, whereas uromodulin had an inverse association with risk for both outcomes. In multivariable analysis including eGFR and albuminuria, a two-fold higher baseline concentration of α1m was associated with higher risk of CVD [hazard ratio (HR) 1.25; 95% confidence interval (CI): 1.10-1.45] and mortality (HR 1.25; 95% CI: 1.10-1.46), whereas β2m had no association with either outcome. A two-fold higher uromodulin concentration was associated with lower CVD risk (HR 0.79; 95% CI: 0.68-0.90) but not mortality (HR 0.86; 95% CI: 0.73-1.01) after adjusting for similar confounders.ConclusionAmong non-diabetic persons with CKD, biomarkers of tubular function are associated with CVD events and mortality independent of glomerular function and albuminuria.

21. Some Features of 'Alt' Texts Associated with Images in Web Pages

Abstract

Introduction: This paper extends a series on summaries of Web objects, in this case, the alt attribute of image files. Method: Data were logged from 1894 pages from Yahoo!'s random page service and 4703 pages from the Google directory; an img tag was extracted randomly from each where present; its alt attribute, if any, was recorded; and the header for the corresponding image file was retrieved if possible. Analysis: Associations were measured between image type and use of null alt values, image type and image file size, image file size and alt text length, and alt text length and number of images on the page. Results: 16.6% and 17.3% of pages respectively showed no img elements. Of 1579 and 3888 img tags randomly selected from the remainder, 47.7% and 49.4% had alt texts, of which 26.3% and 27.5% were null. Of the 1316 and 3384 images for which headers could be retrieved, 71.2% and 74.2% were GIF, 28.1% and 20.5%, JPEG; and 0.8% and 0.8% PNG. GIF images were more commonly assigned null alt texts than JPEG images, and GIF files tended to be shorter than JPEG files. Weak positive correlations were observed between image file size and alt text length, except for JPEG files in the Yahoo! set. Alt texts for images from pages containing more images tended to be slightly shorter. Conclusion: Possible explanations for the results include GIF files' being more suited to decorative images and the likelihood that many images on image-rich pages are content-poor.
